Facial rejuvenation with fat grafting: systematization and study of 151 consecutive cases

ABSTRACT

Introduction: Considering that the loss of facial volume is a primary factor associated with aging, an increased demand for safe, long-lasting, and biocompatible filling materials has been observed. Thus, the use of fat grafting has gained considerable popularity. However, there are open questions about the safety, efficacy, and durability of fat grafting. Moreover, most studies have not presented the volumes injected in each region, making learning challenging for beginners in the area. In this study, the results of facial rejuvenation with fat grafting in 151 consecutive cases were analyzed.
Methods: Fat was collected via manual suction, centrifuged at 448 g (2000 rpm/radius 10 cm) for 4 min, and injected with microcannulas that are 1-1.1 mm in size. The injection sites and corresponding injection volumes were identified.
Results: The mean follow-up time was 289.29 days (minimum: 7 days, maximum: 1254 days, and standard deviation [SD]: 275.1), and the mean injection volume was 32 mL (range: 4-68 mL, SD: 14). Moreover, no complications were observed.
Conclusion: Fat grafting is a safe, predictable, and effective procedure, and it can be used for facial rejuvenation in certain cases.

RESUMO

Introdução: Considerando que a perda de volume facial é fator primário de envelhecimento, tem acontecido um aumento da demanda por materiais de preenchimento que sejam seguros, de longa duração e biocompatíveis. Nesse sentido, a utilização do enxerto de gordura vem ganhando bastante popularidade. Entretanto, existem questionamentos sobre segurança, eficácia e durabilidade da lipoenxertia. Além disso, a maioria dos artigos da literatura não menciona volumes injetados em cada área, dificultando o aprendizado dos iniciantes. Nesse estudo, analisam-se os resultados de uma série de 151 casos consecutivos de rejuvenescimento facial com lipoenxertia.
Métodos: A gordura foi colhida por meio de sucção manual, centrifugada a 448g (2000 rpm/ raio 10cm) por 4 minutos e injetada com microcânulas de 1 a 1,1mm. Descreve-se a sistematização de áreas de injeção, com os respectivos volumes a serem aplicados.
Resultados: Encontrou-se seguimento médio de 289,29 dias (mínimo 7, máximo 1254, DP 275,1), o volume médio injetado foi de 32 ml, variando de 4 a 68 (DP 14). Não houve complicações.
Conclusão: A lipoenxertia é um procedimento seguro, previsível e efetivo, como opção de tratamento, para rejuvenescimento facial, em determinados casos.

Bichectomy and its contribution to facial harmony

ABSTRACT

Introduction: Buccal fat removal was described for the first time by Heister in 1732. In 1802, Bichat identified the adipose nature of this tissue. There is a close association between buccal fat and masticatory muscles, which helps infants suck owing to the sliding motion of the tissues. Its prominence diminishes with age, both through modest reduction in volume and facial growth. Bichectomy or bichatectomy is a surgical procedure that involves extraction of Bichat's fat pad for aesthetic and/or functional purposes. This study aimed to show that bichectomy is a low-risk surgical procedure with good results, provided it is appropriately indicated and is performed following technical standards according to the anatomy of the facial structures.
Methods: This prospective cohort study was conducted between January 2016 and April 2018 and included 59 patients with a mean age of 31 years. The follow-up time was 12-26 months.
Results: Of 59 patients, 46 were female, and 13 were male. All patients underwent same-day surgery. On average, 3.2 mL of fat was removed.
Conclusion: Removal of Bichat's fat according to anatomical principles is safe. This technique can improve facial harmony. Success depends on application of appropriate techniques.

RESUMO

Introdução: O tecido gorduroso bucal foi descrito pela primeira vez em 1732, por Heister. Em 1802, Bichat diagnosticou sua natureza adiposa. Há uma estreita relação entre a gordura bucal e músculos da mastigação, auxiliando os lactentes na sucção do leite por atuar com um tecido de deslizamento. Sua projeção diminui com o aumento da idade, tanto pela redução discreta de seu volume quanto pelo crescimento facial. A bichectomia ou bichatectomia é o procedimento cirúrgico que visa remover a "bola gordurosa de Bichat", com fins estéticos e/ ou funcionais. O objetivo é demonstrar que a bichectomia é um procedimento de baixo risco cirúrgico e com bons resultados, desde que seja bem indicada e siga os padrões técnicos, respeitando a anatomia das estruturas faciais.
Métodos: Estudo prospectivo de coorte, realizado entre janeiro de 2016 e abril de 2018, totalizando 59 pacientes, com média de idade de 31 anos. O tempo de seguimento foi de 12 a 26 meses.
Resultados: Foram avaliados 59 pacientes, com média de idade de 31 anos, sendo 46 do sexo feminino e 13 do sexo masculino. Todos os pacientes receberam alta no mesmo dia. Em média, foram retirados 3,2ml de volume da gordura de Bichat.
Conclusão: A retirada de porções das gorduras de Bichat é segura, desde que sejam observados os detalhes anatômicos. Pode oferecer maior harmonia entre os três terços da face. O sucesso dos resultados depende de uma indicação precisa para o procedimento.

Breast reconstruction with a latissimus dorsi myocutaneous flap and implants: evaluation of strategies adopted to improve the results and evaluation of satisfaction by Breast-Q

ABSTRACT

Introduction: The paper describes a 14-year experience with breast reconstruction using a latissimus dorsi myocutaneous flap (LDMF) and breast implants. The objective was to delineate the experience with LDMF and breast implants, skin island schedules and dissection details, flap transposition and placement of the implant under a double layer, strategies to minimize damage in the donor area, and strategies of breast symmetry and reconstruction of the nipple-areola complex, in association with evaluation using the Breast-Q questionnaire.
Methods: A review of medical records was performed between April 2003 and June 2017.
Results: In the period, 76 patients with a mean age of 50.09 years underwent reconstruction with a LDMF, which was bilateral in 11, for the right breast in 34, for the left breast in 31, immediate in 41, late in 22, and for rescue in 13.
Conclusion: We conclude and verified with the Breast-Q questionnaire that with a precise indication, the proposed reconstruction technique with a double subpectoral plane and coverage with a LDMF is safer with a lower complication rate.

RESUMO

Introdução: O trabalho descreve 14 anos em reconstrução mamária com o retalho miocutâneo do grande dorsal (RGD) e implantes mamários. O objetivo é delinear experiência com o RGD e implantes mamários, programações das ilhas de pele e detalhes da dissecção, transposição de retalho e colocação do implante sob dupla camada muscular, estratégias para minimizar os danos na área doadora, estratégias de simetrização mamária e reconstrução do complexo areolopapilar, associado a avaliação do questionário Breast-Q.
Métodos: Foi realizada revisão de prontuários entre abril de 2003 a junho de 2017.
Resultados: No período 76 pacientes foram reconstruídas com o RGD, com idade média de 50,09 anos, sendo 11 bilaterais, 34 mama direita e 31 mama esquerda. 41 imediatas, 22 tardias e 13 de resgate. A média de satisfação foi 72,36% por meio do Breast-Q.
Conclusão: Concluímos e comprovamos com o questionário do Breast-Q que a indicação precisa aliada a técnica de reconstrução proposta com plano duplo subpeitoral associada a cobertura do RDG, propicia uma loja mais segura, diminuído o índice de rippling.

Complications of immediate breast reconstruction after total mastectomy using conical and non-conical prostheses

ABSTRACT

Introduction: In the biennium 2016-2017, approximately 2,160 new cancer cases were identified by the Brazilian National Cancer Institute (Instituto Nacional do Câncer; INCA) for the state of Ceará and 57,960 new cases for every 100,000 women in Brazil. Breast reconstruction with alloplastic implants presents early and late surgical complications directly or indirectly related to the surgical technique used to perform mastectomy and implantation of the synthetic material. This study aimed to analyze the complications found in a group of patients submitted for immediate breast reconstruction using conical and non-conical prostheses.
Methods: This is a cross-sectional, retrospective, observational cohort study that analyzed the medical records of patients who underwent skin-sparing total mastectomy with immediate breast reconstruction using conical and non-conical prostheses, performed by the Plastic Surgery and Reconstructive Microsurgery Service at the HUWC of the Federal University of Ceará from January 2016 to 2018.
Results: A total of 13 (54.1%) patients presented with complications. The conical prostheses showed the highest number of complications, i.e., 7 (63.6%) patients, whereas only 6 (46.1%) patients with non-conical prostheses had complications.
Conclusion: Our study showed a higher frequency of complications with the use of conical prostheses when compared to non-conical prostheses, mainly due to the formation of a sore area at the tip of the conical prosthesis that resulted in extrusion.

RESUMO

Introdução: No biênio 2016-2017 foram estimados pelo Instituto Nacional do Câncer (INCA) aproximadamente 2.160 casos novos de câncer para o estado do Ceará e 57.960 casos novos para cada 100.000 mulheres no Brasil. A reconstrução mamária com implante de material aloplástico apresenta complicações cirúrgicas precoces e tardias que são direta ou indiretamente relacionadas à técnica cirúrgica utilizada para a realização da mastectomia e a implantação do material sintético. O presente estudo tem por objetivo analisar as complicações encontradas em um grupo de pacientes submetidas à reconstrução mamária imediata com uso de prótese cônica e não cônica.
Métodos: Corresponde a um estudo de coorte transversal, retrospectivo, observacional com análise de prontuário de pacientes que foram submetidos à mastectomia total poupadora de pele com reconstrução imediata da mama com o uso de prótese cônica e não cônica, no período de janeiro de 2016 a janeiro de 2018, realizada pelo Serviço de Cirurgia Plástica e Microcirurgia Reconstrutiva do HUWC da Universidade Federal do Ceará.
Resultados: As complicações ocorreram em 13 (54,1%). A prótese que apresentou o maior número de complicações foi a de formato cônico, com complicações em 7 (63,6%) pacientes, enquanto somente 6 (46,1%) pacientes com prótese de formato não cônica apresentaram complicações.
Conclusão: O nosso estudo mostrou maior frequência de complicações com o uso de próteses cônicas em relação à não cônica devido principalmente à formação de uma área de sofrimento na ponta da prótese cônica que resultou em extrusão das mesmas.

Thoracic wall reconstruction using myocutaneous and fasciocutaneous flaps in patients with locally advanced and metastatic breast cancer

ABSTRACT

Introduction: Breast cancer is the most common cancer among women worldwide. Locally advanced breast cancer is characterized by clinical stage IIIb or IV and accounts for 20- 25% of all cases. Defects are reconstructed using myocutaneous and fasciocutaneous flaps, primarily from the latissimus dorsi and rectus abdominis muscles. The objective is to evaluate the results of thoracic wall reconstructions in cases of locally advanced breast cancer using fasciocutaneous and myocutaneous flaps.
Methods: This was a retrospective, observational, and descriptive single-center study. Variables studied included defect size and flap dimensions, myocutaneous flap type, presence of cutaneous and visceral metastasis, postoperative evolution, and complications.
Results: We selected 11 patients with a mean age of 49 years; the left side was the most commonly affected. The most common tumor type was invasive ductal carcinoma. The flaps were made of latissimus dorsi VY (LDVY) in two patients, latissimus dorsi associated with thoracoabdominal flaps (LDVYTA) in two, vertical rectus abdominus myocutaneous flap (VRAM) in four, and thoracoabdominal flaps (TA) in three. The mean defect area was 421.72 cm2, while the mean flap area was 451 cm2. The most frequent complication was partial dehiscence (seven patients). Six patients achieved lethal exit. VRAM flaps presented more complications. The mean survival for VRAM was 25.5 months, LDVY was 17 months, TA was 17 months, LDVYTA was 20.5 months.
Conclusion: Myocutaneous and fasciocutaneous flaps are effective for chest wall reconstruction after locally advanced breast cancer resection.

RESUMO

Introdução: Câncer de mama localmente avançado é caracterizado pelos estádios clínicos IIIb ou IV e representam de 20 a 25% de todos os casos. A reconstrução dos defeitos é feita com retalhos musculocutâneos e fasciocutâneos, sendo os mais utilizados o latíssimo do dorso e o reto abdominal. O objetivo é avaliar resultados das reconstruções de parede torácica em câncer de mama localmente avançados com retalhos musculocutâneos e fasciocutâneos.
Métodos: Estudo retrospectivo, observacional descritivo, em único centro. Variáveis estudadas: dimensões do defeito e do retalho, tipo de retalho utilizado para a reconstrução, metástases cutâneas e viscerais, evolução pós-operatória e complicações.
Resultados: 11 pacientes, com média de idade de 49 anos, com o lado esquerdo mais acometido. O tipo tumoral mais encontrado foi o carcinoma ductal invasivo. Os retalhos realizados foram: 2 latíssimos do dorso com desenho VY (LDVY), 2 latíssimos do dorso associados a retalho toracoabdominal (LDVYTA), 4 verticais do músculo reto do abdome (VRAM) e 3 toracoabdominais (TA). A área média dos defeitos foi 421,72cm2 e a área média dos retalhos utilizados foi de 451cm2. A complicação mais frequente foi deiscência parcial da ferida operatória, presente em 7 pacientes. Da amostra, 6 pacientes atingiram êxito letal. VRAM foi o retalho que apresentou mais complicações. A sobrevida média para VRAM foi de 25,5 meses, para LDVY de 17 meses, TA de 17 meses e LDVYTA de 20,5 meses.
Conclusão: Os retalhos musculocutâneos e fasciocutâneos são eficazes para a reconstrução da parede torácica após a ressecção de neoplasias mamárias localmente avançadas.

Evaluation of the nipple-areola complex after reduction mammoplasty with dermal release versus resected volume of the breast tissue

ABSTRACT

Introduction: The nipple-areola complex (NAC) plays an important role both in breastfeeding and sexual lives of the patients. Because of this, we must mention possible complications of reduction mammoplasty, such change or even loss of sensitivity of the NAC.
Method: This was a prospective, randomized, controlled, double-blind study to evaluate the relationship between the sensitivity of the NAC after reduction mammoplasty with dermal release and the resected volume of breast tissue. This study was conducted between August 2013 and August 2015 at the Hospital das Clínicas of the Federal University of Pernambuco (HC-UFPE), in Recife City, Brazil.
Results: The study involved 39 patients. The mean age and body mass index (BMI) of the patients were 31.7 years and 25.5 kg/m2, respectively. None of the patients was a smoker. The complication rate was 41%, including 7 cases of non-aesthetic scars, 6 cases of hematoma (bruises), and 4 cases of dehiscence. No patient had pain or NAC necrosis, and all the patients expressed being satisfied with the final surgical result. No statistically significant differences were found between the patients of the experimental and control groups, both in the group with resection of up to 300 g and those with resection >300 g.
Conclusions: The maneuver of dermic release did not result in any differences in the sensitivity of the NAC, regardless of the quantity of resected breast tissue.

RESUMO

Introdução: O complexo areolopapilar (CAP) desempenha grande importância tanto na amamentação quanto na vida sexual das pacientes, consequentemente, devemos ressaltar que uma complicação potencial da mamoplastia redutora é a alteração ou até a perda da sensibilidade do CAP.
Método: Estudo prospectivo, randomizado, controlado e duplo-cego para avaliar a relação entre a sensibilidade do complexo areolopapilar após mamoplastia redutora com liberação dérmica e o volume ressecado de tecido mamário. O estudo ocorreu no período de agosto de 2013 a agosto de 2015, no Hospital das Clínicas da Universidade Federal de Pernambuco (HC-UFPE).
Resultados: O estudo totalizou 39 pacientes. A média de idade da amostra foi de 31,7 anos, índice de massa corporal (IMC) médio de kg/m2 e nenhuma das pacientes era tabagista. A taxa de comorbidade foi de 5,1% da amostra. O índice de complicações foi de 41%, composto de 7 casos de cicatriz inestética, 6 de hematoma e 4 de deiscência. Não houve caso de sofrimento ou necrose do CAP e todas as pacientes se mostraram satisfeitas com o resultado cirúrgico final. Não houve diferenças estatisticamente significantes em nenhuma das ocasiões entre os pacientes do grupo experimento e controle, tanto no grupo de ressecção de até 300 gramas quanto acima de 300gramas.
Conclusões: A manobra de liberação dérmica não provocou diferença na sensibilidade do complexo areolopapilar, independentemente da quantidade de tecido mamário ressecado.

Abdominoplasty Marking Instrument

ABSTRACT

Introduction: Data from the International Society of Aesthetic Plastic Surgery indicate that abdominoplasty is the fourth most common plastic surgery performed worldwide. Several surgical techniques aim to restore the abdominal contour. The indication for the technique used should aim to improve any individual deformity present in the abdomen. Abdominoplasty marking is done using a ruler, protractor, compass, nylon thread, toothpick, pen, and/or methylene blue, according to the surgeon's preference. Here we aimed to produce a surgical instrument that optimizes the abdominoplasty marking procedure.
Methods: The study was approved by the ethics research committee. Graphic designs were developed to identify each part of the instrument, a precedence search was performed, and a patent application was requested.
Results: We developed an abdominoplasty marking instrument consisting of a horizontal ruler that receives a vertical ruler at its midpoint, two pivotal rulers with a basal protractor, all marked in millimeters, and a central area that allows marking in this region with methylene blue or a pen. Moreover, it acts as a compass and features side pivotal rulers derived from a basal grade protractor.
Conclusion: The instrument developed in this study was designed to minimize the amount of material used and optimize abdominoplasty marking time.

RESUMO

Introdução: A abdominoplastia, um dos procedimentos de cirurgia plástica mais comuns no Brasil, ficou em quarto lugar no ranking mundial, segundo dados da International Society of Aesthetic Plastic Surgery. Diversas técnicas cirúrgicas visam restaurar o contorno abdominal. A indicação da técnica utilizada deve visar a melhoria da deformidade individual presente no abdome. A marcação da abdominoplastia se faz por meio de régua, transferidor, compasso, fio de náilon, palito, canetas e/ou azul de metileno, de acordo com a preferência do cirurgião. O objetivo é produzir um instrumento cirúrgico com a finalidade de otimizar o tempo de marcação no procedimento de abdominoplastia.
Métodos: O estudo foi aprovado pelo Comitê de Ética em Pesquisa, foram desenvolvidos desenhos gráficos para identificar cada uma das partes do instrumento, foi realizada a busca de anterioridade e, posteriormente, realizado o pedido de patente.
Resultados: Foi desenvolvido um instrumento para marcação da abdominoplastia, que configura uma régua horizontal que recepciona uma régua vertical no seu ponto médio e duas réguas pivotantes com transferidor basal, todas essas peças milimetradas e com vazado central, que permitem o risco nessa região com o azul de metileno ou caneta afim e, por conseguinte, a marcação do procedimento de abdominoplastia. Além disso, também funciona como compasso e de cujas laterais derivam réguas pivotantes com transferidor de grau basal.
Conclusão: O instrumento desse estudo foi criado para minimizar a quantidade de material utilizado e otimizar o tempo da marcação da abdominoplastia.

Reverse anterolateral thigh flap: a reconstruction option for the lower limbs

ABSTRACT

Introduction: Cutaneous and soft tissue reconstruction in the region around the knee is often challenging for the plastic surgeon and aimed at providing good knee joint function with acceptable aesthetic appearance. Surgical planning requires considering the simplest technique that leads to wound closure, with minimal morbidity in the donor area. The literature has reported only few cases of the use of the reverse anterolateral (ALT) flap for defects in the popliteal fossa, knee, and leg proximal portion. The objective is to describe a series of cases of patients who underwent reconstruction with a reverse ALT flap for wounds in the popliteal fossa, knee, and upper third of the leg.
Methods: Among four patients, three received popliteal fossa reconstructions and one received knee and leg proximal third reconstructions. All the patients underwent wound/tumor extensive resection, dissection of the reverse ALT flap, and transposition to the defect.
Results: During follow-up for 2 to 18 months, minimal morbidity was observed in the donor area, with a total coverage of the defect, optimal functional result of the affected limb, and good aesthetic result. Only one case had a partial necrosis (15%) of the flap due to hematoma and pedicle compression. In the other patients, no evidence of flap failure, dehiscence, seroma, or infection were found.
Conclusion: The reverse ALT flap was a viable option, presenting good functional and aesthetic results for cutaneous and soft tissue reconstructions in the popliteal fossa, knee, and upper third of the leg.

RESUMO

Introdução: A reconstrução cutânea e de tecidos moles na região ao redor do joelho é frequentemente desafiadora para o cirurgião plástico e tem como objetivo fornecer uma boa função articular do joelho com aparência estética aceitável. O planejamento cirúrgico requer considerar a técnica mais simples que leve ao fechamento da ferida, com mínima morbidade na área doadora. Há poucos relatos do uso do retalho anterolateral (ALT) reverso na literatura, em se tratando de defeitos na fossa poplítea, joelho e porção proximal da perna. O objetivo é descrever uma serie de casos de pacientes submetidos à reconstrução de ferida em fossa poplítea, joelho e terço superior da perna com retalho ALT reverso.
Métodos: Quatro pacientes, dos quais 3 eram reconstruções de fossa poplítea e 1 reconstrução de joelho e terço proximal da perna. Todos submetidos à ressecção ampla da ferida/tumor, dissecção do retalho ALT reverso, e transposição para o defeito.
Resultados: Follow-up de 2 a 18 meses, evidenciando-se morbidade mínima para área doadora, com cobertura total do defeito e ótimo resultado funcional do membro afetado, com bom resultado estético. Apenas em um dos casos evidenciou-se sofrimento e necrose parcial (15%) do retalho devido a hematoma e compressão do pedículo, nos demais não foi evidenciado sofrimento dos retalhos, deiscência, seroma ou infecção.
Conclusão: O retalho ALT da coxa reverso mostrou-se uma opção viável, apresentando bom resultado funcional e estético para a reconstrução cutânea e de tecidos moles na região de fossa poplítea, joelho e terço superior da perna.

ABSTRACT

Introduction: The use of many flaps has been described to repair substance loss in the medial and distal thirds of the lower leg. The main advantage of the muscle medial hemisoleus flap is preserving one half of the innervated medial soleus muscle in situ and keeping the foot's plantar flexion power. Moreover, the flap has a longer arc of rotation. As recently described, new vascular concepts and techniques have increased flap viability. This study aimed to examine the use and advantages of the medial hemisoleus flap with emphasis on an indication and complication analysis.
Methods: Over a 10-year period, nine medial hemisoleus flaps were executed to repair traumatic wounds with tibial exposure in eight patients. We describe the indications and surgical techniques in detail.
Results: Complete wound healing was achieved in all patients. There was a low donor-site complications rate. One patient suffered from partial necrosis of the extremity flap. The mean operative time was 2 h.
Conclusions: The medial hemisoleus flap is useful for repairing substance loss and provides coverage with intermediary thickened tissue, high vascularity, minimal donor-site morbidity, foot plantar flexion power preservation, faster rehabilitation, accessible surgical techniques, and a shorter operative time.

RESUMO

Introdução: Muitos retalhos são descritos para reparar as perdas de substância nos terços médio e distal da perna. A principal vantagem do retalho de músculo hemisóleo medial é a preservação da inervação da metade do músculo sóleo no sítio doador, mantendo a força de flexão plantar. Além disso, este retalho tem um arco de rotação maior quando comparado ao retalho de músculo sóleo convencional. Novos conceitos vasculares e técnicos, descritos recentemente, aumentaram a viabilidade deste retalho. O objetivo deste estudo é relatar a utilização do retalho de hemisóleo medial enfatizando as vantagens, reafirmando as indicações e analisando as complicações.
Métodos: Em dez anos, um total de nove retalhos de hemisóleo medial foram confeccionados para a reparação de feridas traumáticas com exposição da tíbia em oito pacientes. Os critérios de indicação e técnica cirúrgica são descritos detalhadamente.
Resultados: A cicatrização completa da perda de substância foi alcançada em todos os pacientes. Observou-se baixo índice de complicações no leito doador e em um paciente houve necrose parcial da extremidade do retalho. O tempo médio de realização do procedimento foi de duas horas.
Conclusões: Retalhos de hemisóleo medial são muito úteis na reparação destas perdas de substância e permitem a cobertura com tecido de espessura intermediária, rica vascularização, baixo índice de morbidade na área doadora, preservação da função motora plantar, reabilitação mais rápida no pós-operatório, técnica cirúrgica acessível e menor tempo operatório.

Patient safety: changes in hemoglobin and serum iron after liposuction and/or abdominoplasty

ABSTRACT

Introduction: Liposuction is the second commonest procedure performed in Brazil. The scope for liposuction has broadened with advances in the technique, with better equipment, and increased safety in the surgical environment. However, there are concerns about patient safety. Safety committees have made recommendations on various aspects of the procedure including infiltration, anesthesia, patient selection, and aspirated volume. The Brazilian Society of Plastic Surgery and the Federal Council of Medicine have also determined the parameters for aspiration safety volumes, and for the aspiration of body surface. However, supporting literature for the recommendations is scarce. The objective is to evaluate changes in hemoglobin, serum iron, and total protein levels in the peri-operative days (between 7 and 10 days), in addition to its impact in patients.
Methods: We performed a prospective study, with patients submitted to either liposuction alone, or in conjunction with abdominal dermolipectomy for esthetic indications. We evaluated 30 patients, collecting their demographic data, weight, body mass index, hemoglobin, serum iron, and total protein levels in both, the pre-operative period, and between 7 and 10 post-operative days.
Results: The fall in hemoglobin levels were between 2 and 6 g /dL, with an average of 3.1 g /dL between 7 and 10 post-operative days . A minimum hemoglobin value of 7.8 g/dL was noted between 7 and 10 days after surgery, while the mean reduction of serum iron was found to be 44.87 g/dL.
Conclusion: The reduction in hemoglobin levels was responsible for the clinical symptoms. Iron supplementation was required in the immediate post-operative period.

Introdução: A lipoaspiração é o sendo segundo procedimento mais realizado no Brasil. Com os avanços da técnica, melhores equipamentos e aumento da segurança no ambiente cirúrgico, passaram-se a ser lipoaspiradas grandes áreas. Contudo, existe uma preocupação com a segurança do paciente. Comitês de segurança têm feito recomendações sobre a infiltração, anestesia, seleção do paciente, volume aspirado, entre outros. A Sociedade Brasileira de Cirurgia Plástica e o Conselho Federal de Medicina também determinaram parâmetros de volumes de segurança do aspirado e superfície corporal aspirada. Ainda assim, a literatura de apoio para as recomendações é escassa. O objetivo é avaliar as alterações dos níveis de hemoglobina, ferro sérico e proteínas totais no pré e pós-operatório mediato (7-10 dias), assim como seus reflexos nos pacientes.
Métodos: Realizamos um estudo prospectivo, com pacientes submetidos à lipoaspiração isolada e associada à dermolipectomia abdominal, por indicações estéticas. Avaliamos 30 pacientes, sendo coletados dados demográficos, peso, Índice de Massa Corporal, níveis de hemoglobina, ferro sérico e proteínas totais, no pré-operatório, e entre 7 a 10 dias de pós-operatório.
Resultados: A queda da hemoglobina foi entre 2-6g/dl, com média de 3,1g/dl aos 7-10 dia de PO. Tivemos como valor mínimo de hemoglobina 7,8g/dl no pós-operatório com 7-10 dias, enquanto o ferro sérico apresentou redução média de 44,87g/dl.
Conclusão: A redução no nível da hemoblogina foi responsável pela sintomatolgia clinica apresentada. A reposição de ferro se mostrou necessária no pós-operatório imediato.

ABSTRACT

Introduction: Nowadays there is a huge demand for individual identification in both civil and criminal justice fields. There are several ways to identify individuals, depending on whether he is alive or dead (cadaver or skeleton). This research aimed to create a database for facial soft tissue thickness in living individuals of a specific Brazilian population, according to age, sex and body mass index.
Methods: Measurements of facial soft tissues thickness were done in 101 patients (62 men and 39 women, aged 18 to 106 years). CT scans were performed taking into account 20 craniometric points previously selected. The soft tissue thickness in these points were initially found and compared with age, sex and nutritional status, and with another populations data worldwide.
Results: According about sex was no significant difierence in points: nasion, rhinion, midphiltrum, supradentale and lateral orbit. Distances with significant difference for both sexes between the diferent populations were: supradentale; infradentale; supramentale; lateral orbit; zygomatic arch and occlusal line. Some anthropometric points showed significant differences between sex, age groups and nutritional status. Between sexes, men had greater means. Among age groups, there was also significant differences in some distances. In relation to nutritional status, the distances were lower among normal weight and higher among the obese.
Conclusion: When considering various populations, soft tissue thickness had significant differences in many craniometric points highlighting how distinct they might be.

RESUMO

Introdução: Hoje em dia há uma enorme demanda para identificação individual tanto no campo da justiça civil e criminal. Existem várias maneiras de identificar indivíduos, dependendo se ele está vivo ou morto (cadáver ou esqueleto). Esta pesquisa teve o objetivo de criar um banco de dados para a espessura do tecido mole facial em indivíduos vivos de uma determinada população brasileira, de acordo com idade, sexo e índice de massa corporal.
Métodos: medições da espessura de tecidos moles faciais foram realizadas em 101 pacientes (62 homens e 39 mulheres, com idade entre 18 e 106 anos). TC foi realizado tendo em conta 20 pontos craniométricos selecionados previamente. A espessura dos tecidos moles nestes pontos foram inicialmente medida e comparada com a idade, sexo e estado nutricional e com dados de outras populações em todo o mundo.
Resultados: De acordo com o sexo não houve diferenças significativas nos pontos: násio, rínion, meio do filtro labial midphiltrum , supradentale, e órbita lateral. Distâncias com diferenças significativa para ambos os sexos entre as diferentes populações foram: supradentale; infradentale; supramentale; órbita lateral; arco zigomático e linha oclusal. Alguns pontos antropométricos mostraram diferenças significativas entre sexo, faixa etária e estado nutricional. Entre os sexos, os homens apresentaram médias maiores. Entre os grupos etários, houve também diferenças significativas em algumas distâncias. Em relação ao estado nutricional, as distâncias foram menores entre peso normal e maior entre os obesos.
Conclusão: Ao considerar várias populações, a espessura do tecido mole mostrou diferenças significativas em muitos pontos craniométricos destacando como eles podem ser distintas.

Quality of life and self-esteem in elderly patients who did or did not have cosmetic surgery

ABSTRACT

Introduction: The reality of aging has caught up with the field of plastic surgery, shown by the growth in the number of elderly patients who undergo cosmetic surgery (CS). To evaluate its importance in elderly women, this study examined differences in quality of life and self-esteem among those who did or did not undergo CS.
Methods: This casecontrol study included 25 elderly women who underwent CS and a control group of 25 elderly women who did not undergo CS; the groups were matched by socioeconomic data. Assessment methods included the Mini-Mental State Examination, a quality of life (QOL) questionnaire (World Health Organization Quality of Life-Bref), the Rosenberg Self-Esteem Scale, and a questionnaire developed for the study of sociodemographic data, motivation, and satisfaction with CS.
Results: The mean age was 67.26 years, with a mean of 9.96 years of education. The most common surgeries were abdominoplasty and blepharoplasty. The most common motivations were physical discomfort, desire to improve QOL, and dissatisfaction with self-image. No subjects were found to have low self-esteem and the level of satisfaction with personal or social life was high. There was no difference in QOL or self-esteem between the 2 groups.
Conclusion: Physical and psychological motivations cause the elderly to undergo CS. There was no difference in QOL or self-esteem among elderly women who did or did not have CS. Elderly women who underwent CS showed high levels of satisfaction with their personal and social life.

RESUMO

Introdução: A realidade do envelhecimento populacional chegou ao campo da Cirurgia Plástica, provada pelo crescimento do número de idosos que se submetem à cirurgia estética (CE). A pesquisa objetiva avaliar a importância da CE para o idoso, e se existe diferença de qualidade de vida e autoestima entre idosas que se submeteram e que não se submeteram à cirurgia estética.
Métodos: Pesquisa caso-controle, sendo o grupo-caso formado por 25 idosas que se submeteram à CE e o grupo-controle por 25 idosas que não fizeram CE, pareados pelos dados socioeconômicos. Os instrumentos aplicados foram: Minimental, questionário de qualidade de vida (WHOQOL-BREF), escala de autoestima de Rosenberg e um questionário elaborado para pesquisa de dados sociodemográficos, motivação e satisfação com a CE.
Resultados: A média de idade foi 67,26 anos e a escolaridade média, de 9,96 anos. As cirurgias mais realizadas foram a abdominoplastia e a blefaroplastia. Os motivos mais escolhidos foram o desconforto físico, o desejo de melhoria da qualidade de vida (QV) e a insatisfação com a autoimagem. Não foram encontradas idosas com baixa autoestima e o nível de satisfação foi alto quando relacionado com a própria vida ou a vida social. Não houve diferença de QV e autoestima entre os dois grupos analisados.
Conclusão: As motivações das idosas para realização de CE são de ordem física e psicológica. Não houve diferença de QV e autoestima entre idosas submetidas e não submetidas à CE. Analisando-se as idosas submetidas à CE, foram comprovados altos níveis de satisfação pessoal e na vida social.

Use of a formula to assess patient satisfaction and expectations through neuro-linguistic programming

ABSTRACT

Introduction: A major concern of patients who are candidates for a plastic surgery is failure to achieve expected results. Neurolinguistic programming (NLP) provides an understanding of the structure of language, clues about how people think, and how these patterns of thought affect behavior. The objective of this study was to validate the use of a formula based on NLP to assess patient satisfaction and expectations.
Methods: NLP techniques were employed to improve communication and assess patient expectations. Using NLP, the average number of surgeries performed in the past 5 years and the reappraisal index were compared with those in the previous 5 years, in which the methods described in this article were not employed.
Results: The average annual growth rate in the previous 5 years was 5% and the average rate verified after introduction of NLP methodology increased to 10%; the reappraisal and additional surgery index decreased from 20% to 10% in the same period.
Conclusions: The use of NLP helped us to understand patient expectations in a more consistent manner for the periods analyzed, significantly increasing the number of operated patients and reducing the rate of reappraisal and additional surgeries.

RESUMO

Introdução: Uma das maiores preocupações dos pacientes candidatos a uma cirurgia plástica é a de não se atingir os resultados esperados. A Programação Neurolinguística (PNL) oferece entendimentos da estrutura da linguagem para dar pistas sobre como as pessoas pensam e como esses padrões de pensamento, por sua vez, afetam o comportamento. O objetivo do presente trabalho é verificar a utilização de uma fórmula, com o uso da PNL, na satisfação e na determinação da expectativa do paciente e os resultados obtidos após a sua utilização.
Métodos: Técnicas de PNL foram empregadas para melhorar a comunicação e equalizar a expectativa dos pacientes. Foram analisados a média de cirurgias realizadas e o índice de refazimento de cirurgias nos últimos cinco anos em comparação com os cinco anos anteriores, em que não se empregavam os métodos descritos neste artigo.
Resultados: A média de crescimento nos cinco anos anteriores era de 5% ao ano e a média verificada após a introdução da metodologia apresentada aumentou para 10% ao ano e o índice de refazimento e complementações cirúrgicos reduziu de 20% para 10%, no mesmo período.
Conclusões: O uso da PNL nos ajudou a entender a expectativa, de maneira mais consistente, nos períodos analisados, aumentando significativamente o número de pacientes operados e reduziu o índice de refazimento e de complementações de cirurgias em nossa experiência.

Determining factors in choosing a plastic surgeon

ABSTRACT

Introduction: This study aims to understand the factors that determine the choice of a plastic surgeon from the patient's perspective. This is a pilot project, which should be broadened and deepened by other studies.
Method: A survey was devised in the form of a questionnaire with 22 multiple choice questions. The average response time was 8 minutes. The questions addressed various aspects, such as the recommendation, training, and accreditation of the surgeon; perception of the first consultation; and the clinic. There was no possibility of identifying the patient or surgeon; hence, anonymity was guaranteed.
Results: The response rate was 86.66%. The majority (92.22%) of the respondents were female, with a mean age of 35 years. Most had completed secondary and higher education, with an average monthly family income of R$ 2,000 to 10,000. Almost 40% did not know the accreditation of the surgeon chosen and 33.7% did not know the length of training the surgeon had undergone. The majority (81.6%) believed they had paid an average amount for their surgery and half did not research the surgeon's online profile. The majority (67%) had not undergone any previous plastic surgery. A list of 10 items in descending order of importance was presented.
Conclusions: The determining factors for the choice of the plastic surgeon in this sample, in descending order were: 1) Recommendation, 2) Accreditation, and 3) First consultation. Price was not the most important factor and online presence was one of the last items cited. A significant proportion of respondents did not know either the accreditation level or the length of training of their surgeon.

Introdução: Esta pesquisa tem como objetivo entender os fatores que determinam a escolha de um cirurgião plástico, na visão dos pacientes. É um projeto piloto que deve ser ampliado e aprofundado em outros estudos.
Método: Foi idealizada uma pesquisa com 22 perguntas de múltipla escolha na forma de questionário. O tempo médio de resposta era de 8 minutos. As perguntas abordavam vários aspectos como a indicação/formação/titulação do cirurgião, percepção da primeira consulta e do consultório. Não havia possibilidade de identificação da paciente ou do cirurgião. O anonimato era garantido.
Resultados: O índice de resposta foi de 86,66%. A maioria (92,22%) era do sexo feminino, com idade média de 35 anos. A maior parte tinha ensino médio e superior completos, com ganho familiar médio mensal de R$ 2 a 10 mil. Quase 40% não sabiam da titulação do cirurgião escolhido e 33,7% não sabia o tempo de formação do mesmo. A maioria (81,6%) acredita ter pago um valor na média pela sua cirurgia e metade não pesquisou a apresentação online do cirurgião. A maioria (67%) não tinha feito nenhuma cirurgia plástica previamente. Foi apresentada uma lista de 10 itens em ordem decrescente de importância.
Conclusões: Os fatores determinantes para a escolha do cirurgião plástico nesta amostra, em ordem decrescente são: 1-Indicação, 2-Titulação e 3-Primeira consulta. Preço não está entre os primeiros atributos e apresentação
online foi um dos últimos itens citados. Uma parte significativa dos pesquisados não conhece a titulação e nem o tempo de formação do seu cirurgião.

Formal training in scientific research increases the participation of plastic surgery residents in peer-reviewed articles

ABSTRACT

Introduction: The objectives of this study were as follows: (1) to outline a scientific research skills training program, (2) to evaluate the profile of participation of plastic surgery residents in articles, and (3) to analyze the impact of the implementation of the training program on quantitative bibliometric indexes.
Methods: This was a bibliometric analysis of the participation of plastic surgery residents of a single institution in articles published in peer-reviewed journals between 2006 and 2014. The data collected were the number of authors, position of residents among authors, article titles, indexing databases and impact factor of the journals, study design, and levels of evidence. Two periods (January 2006 to January 2010 [A] and February 2010 to February 2014 [B]) were created to study the evolutionary profile of the impact of the implementation of the training program outlined in this study.
Results: A significant predominance (p < 0.05) was observed among articles published in national journals in the Portuguese language and in the SciELO and LILACS databases, and articles without residents as corresponding author, without impact factor, without assumptions, and with a level of evidence III (retrospective studies). The inter-period comparative analysis revealed a significant increase (p < 0.05) in the numbers of published articles and residents with publications at the end of their residency, in the involvement of one or more residents, and in the articles published in English (period A < period B).
Conclusion: The implementation of a scientific research skills training program led to an increase in research activity of (peer-reviewed articles) during the residency.

Introdução: Os objetivos deste estudo foram: (1) delinear um programa de treinamento em habilidades de pesquisa científica, (2) avaliar o perfil da participação dos residentes de Cirurgia Plástica em artigos, e (3) analisar o impacto da implementação do programa de treinamento sobre índices bibliométricos quantitativos.
Métodos: Trata-se de uma análise bibliométrica da participação de residentes de Cirurgia Plástica de uma única instituição em artigos publicados em periódicos revisados por pares entre 2006 e 2014. Dados coletados: número de autores, posição dos residentes entre os autores, títulos, bases de indexação e fator de impacto dos periódicos, desenhos dos estudos e níveis de evidência. Dois períodos (janeiro/2006-janeiro/2010 [A] e fevereiro/2010-fevereiro/2014 [B]) foram criados para estudar o perfil evolutivo do impacto da implementação do programa de treinamento delineado neste estudo.
Resultados: Houve predomínio significativo (p < 0,05) de artigos publicados em periódicos nacionais, em língua portuguesa, nas bases de dados SciELO e LILACS, artigos sem residentes como autor correspondente, sem fator de impacto, sem hipóteses e com nível de evidência III (estudos retrospectivos). A análise comparativa interperíodos revelou um aumento significativo (p < 0,05) de artigos publicados, de residentes com publicações ao término da residência, da participação de um ou mais residentes e de artigos publicados em inglês (período A < período B).
Conclusão: A implementação do programa de treinamento em habilidades de pesquisa científica determinou um aumento da atividade de pesquisa (artigos revisados por pares) durante a residência.

ABSTRACT

Introduction: Skin expansion is a physiological process defined as the ability of human skin to increase its superficial area in response to a stress or given deformation. In reconstructive surgery, skin expanders are implanted beneath the skin and periodically infiltrated with a saline solution to provide an extra flap of skin. When the prescribed internal volume of the expander is reached, reconstructive surgery is performed.
Methods: A semiautomatic and portable device was developed and built to facilitate a skin expansion procedure. The device aims to simplify the infiltration process, providing mobility and independence to the patient and assuring the physician of the infiltration quality and precision. The device also enables continuous expansion in hospitalized patients.
Results: Using a code, the doctor accesses the menu of the device and sets the maximum pressure and/or value for each expander of the patient. The patient can control the infiltration velocity and reverse or stop the operation. All data are recorded on a simcard and include date, time, initial and final volumes, and initial and final pressures of each procedure for each expander.
Conclusions: The device motorizes and optimizes the expansion, allowing the doctor to prescribe a maximum infiltration pressure or volume. All data are recorded to provide an important database of skin behavior related to sex, race, age, and expansion site.

RESUMO

Introdução: A expansão da pele é um processo fisiológico definido como a capacidade de aumentar sua área superficial em resposta a uma tensão ou a uma dada deformação. Para realizar a cirurgia reconstrutiva, os expansores de pele são implantados sob a pele e periodicamente infiltrados com uma solução salina para fornecer um retalho extra de pele. Quando o volume interno prescrito do expansor é alcançado, a cirurgia reconstrutiva é realizada.
Métodos: Foi desenvolvido um dispositivo semiautomático e portátil para facilitar um procedimento de expansão da pele. O dispositivo tem como objetivo simplificar o processo de infiltração, proporcionando mobilidade e independência para o paciente, e assegurando ao médico a qualidade e a precisão das infiltrações realizadas. O dispositivo também permite expansão contínua em pacientes hospitalizados.
Resultados: Usando um código, o médico tem acesso ao menu do dispositivo e define a pressão máxima e/ou o valor máximo para cada expansor do paciente. O paciente pode realizar a infiltração e ter acesso ao controle da velocidade de infiltração, reverter ou parar a operação. Todos os dados são gravados em um SIM Card e incluem data, hora, volumes inicial e final, e pressão inicial e final de cada procedimento para cada expansor.
Conclusões: O dispositivo automatiza e otimiza a expansão, de modo que o que o médico possa prescrever um limite para cada expansão, seja uma pressão máxima ou voluma infiltrado. Todos os dados são gravados, fornecendo um importante banco de dados sobre o comportamento de pele relacionado a gênero, raça, idade e local da expansão.

High-definition abdominal liposuction with silicone tubing

ABSTRACT

Introduction: Recently, some authors have described high definition liposuction in the abdominal region, achieved by the vigorous marking of the grooves of the linea alba, linea semilunaris, and transverse tendinous intersections within the rectus abdominis muscle. The aim is to present a pilot study of high-definition abdominal liposuction technique using conventional material and silicone tubing in the dressings .
Methods: Twenty cases were submitted to conventional liposuction, followed by superficial liposuction under the linea alba, linea semilunaris, and tendinous intersections, until a groove was formed at these sites. Flexible silicone tubing was externally fixed with Micropore tape in the linea alba, bilateral linea semilunaris, and tendinous intersections, and removed after 5 days.
Results: A mean increase of 20 minutes was required to perform these steps. The marks of the silicone tubing were evident at the first follow-up, and less prominent at the second follow-up. No color change, ischemia, or significant pain was observed using this technique. After 3 months, there were no cases of dermatitis, cutis marmorata, seroma, infection, contour irregularity or need for additional procedures.
Conclusion: High-definition abdominal liposuction was performed using conventional materials and externally fixed temporary silicone tubing.

RESUMO

Introdução: Recentemente, alguns autores têm descrito a lipoaspiração de alta definição, na qual, na região abdominal, é alcançada pela marcação vigorosa dos sulcos das linhas alba, semilunar e interseções tendíneas transversais do músculo reto abdominal. O objetivo é apresentar estudo piloto de técnica de lipoaspiração de alta definição abdominal utilizando material convencional e tubos de silicone nos curativos.
Métodos: Foram descritos 20 casos, submetidos à lipoaspiração convencional e em seguida lipoaspiração superficial sob as linhas alba, semilunar e interseções tendíneas, até que se obtivesse a formação de um sulco nestes locais. Utilizamos tubos de silicone maleáveis nas linhas alba, semilunar bilateral e interseções tendíneas, fixados externamente com micropore, os quais foram removidos após 5 dias.
Resultados: Houve um acréscimo médio de 20 minutos para realização destas etapas. As marcas dos tubos de silicone apresentaram-se muito evidentes no primeiro retorno, estando mais discretas no segundo retorno. Não foram observadas alterações de coloração, isquemia ou dor maior pela técnica empregada. Após 3 meses, não houve nenhum caso de dermatite, cútis marmorata, seroma, infecção, irregularidade de contorno ou necessidade de procedimentos adicionais.
Conclusão: Foi realizada lipoaspiração de alta definição abdominal, com material convencional e uso de tubos de silicone temporários fixados externamente.

Use of stereophotogrammetry for measuring the volume of external facial anatomy: a systematic review

ABSTRACT

Introduction: Photographic documentation is important in several medical specialties, such as plastic surgery. Two-dimensional photography has limitations in capturing structure depth and volume, requiring other instruments to evaluate these changes. Several technologies have been developed for three-dimensional analysis of objects, of which stereophotogrammetry uses computerized analysis of two or more simultaneous photographs of the object to produce a three-dimensional geometric model. The advantages of stereophotogrammetry include lower cost, portability, absence of radiation, and speed of image capture. The aim of the present study was to perform a bibliographic review evaluating the use and accuracy of stereophotogrammetry for measuring the volume of facial structures.
Methods: Using a combination of MeSH keywords and free terms, a search was performed in the Cochrane Library and MEDLINE databases. The search included all articles published on or before May 2018.
Results: 2,213 studies were initially retrieved using this search strategy. Of these, 27 studies were selected based on the eligibility criteria, of which 21 were non-randomized case studies and 6 were randomized clinical trials. The methodological quality of the studies varied between 50 and 67%, on a grading scale from 0 to 100%.
Conclusions: Stereophotogrammetry is a promising technology that is increasingly being used to check for facial volume variations with high accuracy and reproducibility. More studies with higher methodological quality are needed for evaluating the accuracy and use of stereophotogrammetry for facial volume evaluations.

Introdução: O registro fotográfico tem sido importante para diversas especialidades médicas como a Cirurgia Plástica. A fotografia em duas dimensões apresenta limitações para capturar profundidade e volume de estruturas outros instrumentos para avaliar essa alteração. Diversas tecnologias foram desenvolvidas para analisar objeto em três dimensões, sendo a estereofotogrametria uma tecnologia que utiliza a análise computadorizada de duas ou mais fotografias simultâneas do objeto para produzir um modelo geométrico em três dimensões. As vantagens da estereofotogrametria incluem menor custo, portabilidade, ausência de radiação e rapidez da captura das imagens. O objetivo deste trabalho foi realizar uma revisão bibliográfica avaliando o uso e a acurácia da estereofotogrametria para mensuração de volume de estruturas na face.
Métodos: Foi realizada pesquisa nos bancos de dados Cochrane Library e Medline até maio de 2018 utilizando uma combinação de descritores Mesh e termos livres.
Resultados: Foram obtidos inicialmente 2213 estudos observando a estratégia de busca. Seguindo os critérios de elegibilidade, foram selecionados 27 artigos, sendo 21 relatos de casos não randomizados e 6 ensaios clínicos randomizados. A qualidade metodológica dos estudos variou de 50 a 67%, segundo uma pontuação que vai de 0 a 100%.
Conclusões: A estereofotogrametria é uma tecnologia promissora e tem sido cada vez mais utilizada para verificar variações de volume na face com alta acurácia e reprodutibilidade. Faltam estudos com melhor qualidade metodológica avaliando a acurácia e o uso da estereofotogrametria na avaliação de volume facial.

Laser treatment for stretch marks: a literature review

ABSTRACT

Stretch marks are scars on the dermis that cause patients to be self-conscious and that pose challenges in clinical treatment. They result from rapid stretching of the skin and often observed in adolescents, pregnant women, obese individuals, and people experiencing rapid change in weight. They initially appear as erythematous and edematous plaques (red striae), and as they mature, they become whitish and atrophic (striae alba) due to the degradation and reorganization of elastin and collagen fibers. Currently, laser treatment is an emerging noninvasive therapeutic modality that is successful in reducing the excessive vascularization of red striae and in stimulating the production of collagen and elastin in the alba. In the present literature review, PubMed was searched for articles on the treatment of atrophic striations with laser that were published from January 2000 to December 2016. The authors have found 28 articles that met the inclusion criteria. Only few randomized clinical trials have evaluated the long-term efficacy and safety of energybased treatments. Based on clinical cases and systematic reviews, both ablative and non-ablative fractional lasers caused moderate improvement in the appearance of distending striae. The 1540-nm non-ablative fractional laser is more likely to be considered the first-line treatment for stretch marks. However, future studies should focus on research with longer duration, studies with objective and standardized measures for the evaluation of results, such as biopsy results, and molecular studies showing an increase in elastic and collagen fibers that correlate to the improvement in the clinical appearance of the striae after using lasers with systematized parameters.

RESUMO

Estrias de distensão são cicatrizes na derme que constrangem os pacientes e oferecem desafios ao tratamento clínico. Resultam do rápido estiramento da pele, frequentemente presente em adolescentes, gestantes, obesos e indivíduos com rápida mudança de peso. Apresentam-se inicialmente como placas eritematosas e edemaciadas (estrias rubras) e, conforme amadurecem, tornam-se esbranquiçadas e atróficas (estrias albas), devido à degradação e reorganização de fibras de elastina e de colágeno. Atualmente, lasers representam modalidade terapêutica não invasiva emergente, que já demonstrou sucesso na redução da vascularização excessiva das estrias rubras, e no estímulo à produção de colágeno e elastina nas albas. Foi realizada revisão da literatura no PubMed referente ao tratamento de estrias atróficas com laser de janeiro de 2000 até dezembro de 2016. Os autores encontraram 28 artigos que se enquadraram nos critérios de inclusão. Existem poucos estudos clínicos randomizados avaliando a eficácia ao longo prazo e a segurança de aparelhos à base de energia. Baseado em casos clínicos e revisões sistemáticas, ambos os lasers - ablativo e não ablativo - fracionados demonstraram melhora modesta do aspecto das estrias distensas. Há tendência a sugestão do laser não ablativo fracionado de 1.540 nanômetro como sendo opção de primeira escolha para tratamento das estrias distensas. Porém, estudos futuros devem focar em desenhos de pesquisa com duração mais longa, medidas objetivas e padronizadas da avaliação dos resultados como biópsias e estudos moleculares, que demonstrem aumento nas fibras elásticas e fibras de colágeno, que correlacionem com a melhora do aspecto clínico das estrias após aplicação de lasers com parâmetros sistematizados.

ABSTRACT

The auricular complex is commonly affected by tumors or traumatic events. Several surgical options are available for restoration of ear anatomy. We report the case of a patient who presented with tumor lesions on more than one occasion in the anterior left ear and required surgical intervention to correct the defect generated by excision. As a proposed procedure, we designed and executed a superiorly-based insular flap that was rotated to the scaphoid fossa through a tunnel created in the projection of the triangular fossa; this was performed in a single stage, and the patient was satisfied with the aesthetic and functional results.

RESUMO

O complexo auricular representa sede frequente de lesões, especialmente de origem tumoral ou resultante de eventos traumáticos. Diversas são as opções cirúrgicas para restaurar a integridade anatômica da orelha. Relatamos o caso de um paciente que apresentou lesões tumorais em mais de uma ocasião na face anterior da orelha esquerda e que precisou de intervenção cirúrgica para a correção do defeito gerado pela excisão tumoral. Como procedimento proposto, planejamos e executamos um retalho insular de base superior, que por meio de um túnel criado na projeção da fossa triangular foi rodado para a fossa escafoide, reparando-a em um único tempo cirúrgico, apresentando resultados estético e funcional satisfatórios ao paciente.

Pyoderma gangrenosum following reduction mammoplasty: a case report

ABSTRACT

Pyoderma gangrenosum (PG) is an inflammatory neutrophilic dermatosis of unknown etiology and is idiopathic in 25%- 50% cases. In approximately 50% of PG cases, an association with systemic diseases, such as Crohn's disease, monoclonal gammopathies, seropositive arthritis, collagenosis, Behcet's disease, Wegener's granulomatosis, and myeloproliferative and infectious diseases (mainly hepatitis and AIDS), has been described. Clinically, PG presents four variants: ulcerated, bullous, vegetative, and pustular. The most frequent form is ulcerative, which begins as a papule or nodule and evolves rapidly into ulcerated and painful lesions. In approximately 25% of PG cases, the onset of new lesions can be triggered by traumas such as insect bites, intravenous injections, and biopsy, a phenomenon known as pathergy. Here, we present a case of extensive PG of the breasts following reductive mammoplasty surgery. It was a difficult case to diagnose and was initiated in the postoperative period of another service. Due to the breakdown of the doctor-patient relationship, the patient approached us for assistance. The patient showed an excellent response to corticotherapy (intra and perilesional corticotherapy with triamcinolone) during debridement and oral steroid (prednisone) therapy in the weaning phase.

RESUMO

O pioderma gangrenoso (PG) é uma dermatose neutrofílica inflamatória, de etiologia desconhecida. O PG é idiopático em 25-50% dos casos. Em aproximadamente 50% dos casos tem sido descrita a associação com doenças sistêmicas, tais como: doença de Crohn, gamopatias monoclonais, artrites soropositivas, colagenoses, doença de Behcet, granulomatose de Wegener, doenças mieloproliferativas e infecciosas, principalmente hepatites e Aids. Clinicamente, apresenta quatro variantes: ulcerada, bolhosa, vegetante e pustulosa. A forma mais frequente é a ulcerativa, que se inicia com pápula ou nódulo e evolui rapidamente para lesões ulceradas e dolorosas. Em até 25% dos casos de PG, o surgimento de novas lesões pode ser desencadeado por traumas, tais como picadas de insetos, injeções intravenosas e biópsia - fenômeno conhecido por patergia. Nesse trabalho, é apresentado um caso de PG extenso das mamas em pós-operatório de mamoplastia redutora, de difícil diagnóstico; iniciado na evolução pós-operatória em outro serviço. Devido à quebra da relação médico-paciente, vem procurar nosso serviço. Apresentou ótima resposta ao tratamento com corticoterapia (corticoterapia intra e perilesionais com triancinolona) no ato do desbridamento, e introdução de corticoterapia via oral (prednisona) em esquema escalonado de desmame.

Simultaneous treatment of sacral and ischial pressure ulcers with a single flap: case presentation

ABSTRACT

Pressure ulcers are alterations of the integrity of the skin and underlying tissues, caused by pressure, more commonly on bony prominences, especially in areas devoid of sensitivity, which lead to necrosis and ulceration. Data from the international literature estimate that 3-14% of hospitalized patients develop pressure ulcers. We herein describe the simultaneous correction of extensive sacral and ischial ulcers in a young paraplegic patient, using a gluteus maximus fasciomyocutaneous flap from the posterior aspect of the thigh.

Úlceras de pressão são alterações da integridade da pele e tecidos subjacentes, causadas por pressão, mais usualmente sobre proeminências ósseas, especialmente em áreas desprovidas de sensibilidade, levando à necrose e ulceração. Dados da literatura internacional estimam que 3 a 14% dos pacientes hospitalizados desenvolvem úlceras de pressão. Descrevemos a correção simultânea de úlceras sacral e isquiática extensas em paciente paraplégico jovem utilizando retalho fasciomiocutâneo de glúteo máximo e de face posterior da coxa.

Reinsertion of traumatic avulsion of the superficial and deep flexor tendons of the little finger using the pull-out technique

ABSTRACT

Simultaneous closed traumatic avulsion of both the deep flexor tendon and the superficial flexor tendon of the fingers is rare, and only nine cases have been reported in the literature since 1984. Several surgical procedures for repairing flexor tendon lesions have been described, but there is no consensus on the best approach to reinsert the tendon. We report the case of a patient who suffered a traumatic avulsion of the superficial flexor tendon and the deep flexor tendon of the fifth finger, which was surgically treated using the pull-out technique.

A lesão fechada simultânea por avulsão do tendão flexor profundo e do tendão flexor superficial dos dedos é rara, tendo sido relatados somente nove casos na literatura desde 1984. Foram descritas diversas técnicas para o reparo cirúrgico da lesão dos tendões flexores, todavia, sem um consenso sobre a melhor forma de reinserção do tendão. Relatamos o caso de um paciente que sofreu avulsão traumática do tendão flexor superficial e do tendão flexor profundo do 5º dedo, que foi tratado cirurgicamente pela técnica de Pull-Out.

The use of digital photography in the follow-up of surgical neoplastic wound: a case report

ABSTRACT

Introduction: The characteristics of cancer wounds are unusual and are often difficult to describe in writing. Therefore, digital photography may be used to describe and manage these wounds.
Case Report: A young female patient with a recurring lesion measuring 20 × 18 × 13.7 cm in the left thigh was diagnosed with synovial sarcoma. Digital photography was used to facilitate evaluation and treatment of her complex cancer wound. The patient underwent tumor excision, dressings, and skin grafting; the follow-up was carried out with photographic records using a cell phone camera, which were inputted into the electronic medical record and interpreted by the medical team.
Conclusion: The use of photographs was important because it allowed rapid wound evaluation, could be shared with the team to evaluate the treatment plan, and was an important tool for legal purposes and teaching.

Introdução: As feridas oncológicas apresentam características peculiares, muitas vezes difíceis de expressar no texto escrito. A fotografia digital pode contribuir na descrição e na assistência destas feridas.
Relato do Caso: Relatar o caso de paciente do sexo feminino, jovem, com lesão recidivada medindo 20x 18x13,7cm na coxa esquerda e diagnóstico de sarcoma sinovial. Utilizou-se a fotografia digital como facilitador na avaliação e tratamento de ferida oncológica complexa. A paciente foi submetida à exérese do tumor, curativos e enxertia de pele; o acompanhamento foi realizado com registros fotográficos com câmera do telefone celular, inserido no prontuário eletrônico e proporcionou a fotointerpretação pela equipe de saúde.
Conclusão: O uso da fotografia foi importante, pois permitiu a avaliação rápida da ferida, pôde ser transmitida à equipe para avaliação do plano de tratamento e foram importantes ferramentas legais e para o ensino.

Formal training in two-dimensional standardized photographic documentation during residency in plastic surgery

ABSTRACT

Introduction: Plastic surgery requires diverse skills, knowledge, and competencies, and its scope is broad and challenging. Thus, all plastic surgeons go through extensive education/training, which begins during residency and continues throughout their lives. This study aims to outline a formal training program in 2-dimensional standardized photographic documentation ("photodocumentation") to complement surgical and scientific training during residency in Plastic Surgery.
Methods: The components (strategies, topics covered, skills acquisition, and feedback processes) of training in photodocumentation during residency in Plastic Surgery were detailed.
Results: Topics in the photodocumentation training program included resident activities, interactions among residents, professional photographers, and surgeons/preceptors, studio settings, cameras, lenses, principles (exposure, aperture, speed, ISO, and depth of field), lighting, preparation and positioning of patients and the camera, photoshoot conditions, photo views, photo database, image formats, computerized photogrammetry, and ethical and medical-legal issues.
Conclusion: Training in photodocumentation complements the arsenal of skills needed by residents in Plastic Surgery for continuing education, in order to better prepare them for future careers.

RESUMO

Introdução: A Cirurgia Plástica exige inúmeras habilidades, conhecimentos e competências, e seu escopo de atuação é amplo e desafiador. Dessa forma, todos os cirurgiões plásticos passam por uma extensa educação/formação, que se inicia durante a residência e continua ao longo da vida. O objetivo deste estudo foi delinear um treinamento formal em documentação fotográfica bidimensional padronizada para complementar o treinamento cirúrgico e científico durante a residência em Cirurgia Plástica.
Métodos: Os componentes (as estratégias, os tópicos abordados e os processos de aquisição de habilidades e feedback) do treinamento em documentação fotográfica durante a residência em Cirurgia Plástica foram detalhados.
Resultados: As atividades dos residentes, as interações entre os residentes o fotógrafo profissional e os cirurgiões/preceptores e os tópicos abordados (estúdio fotográfico, câmeras fotográficas, objetivas, princípios fotográficos [exposição, abertura, velocidade, ISO e profundidade de campo], iluminação, preparo e posicionamento dos pacientes e da câmera fotográfica, situações fotográficas, vistas fotográficas, banco fotográfico, formatos de imagens, fotogrametria computadorizada e questões éticas e médico-legais) do programa de treinamento em documentação fotográfica foram delineados.
Conclusão: O treinamento em documentação fotográfica complementa o arsenal de habilidades necessários aos residentes de Cirurgia Plástica em formação, com o intuito de melhor prepará-los para as carreiras futuras.
