Profile of breast augmentation surgery in Brazil

ABSTRACT

Introduction: The latest research by the International Society of Aesthetic Plastic Surgery (ISAPS) showed that 2,524,115 plastic surgeries were performed in Brazil in 2017. Breast augmentation has been one of the most commonly-performed plastic procedures in Brazil, totaling more than 200,000 surgeries in 2016. The objective is to evaluate the current practices and trends in breast augmentation in Brazil.
Methods: A 31- item questionnaire was constructed and sent to 6,200 active members of the Brazilian Society of Plastic Surgery (SBCP). The research addressed some current trends and controversial practices in five areas: current controversies, new technologies, breast implants, technical considerations in secondary procedures, and demographic aspects. The questionnaire was designed to profile practices, procedures, and beliefs among plastic surgeons regarding the use of breast implants in Brazil.
Results: A total of 505 responses were collected, representing a response rate around 10%, which is well above the average response rate for Brazilian Society of Plastic Surgery (SBCP) questionnaires. The results were distributed in five tables by area of interest.
Conclusion: In this study, the most common practices included the use of microtexture round implants and polyurethane-coated silicone as the primary procedure, a subglandular pocket, inframammary incisions, preoperative sizing with round implant samples, intravenous and oral antibiotics, double irrigation antibiotics, an implant size that was generally less than 325 cc, and without drainage. There is no consensus on breast lip balancing and the management of capsular contracture and double bubble deformity.

RESUMO

Introdução: A última pesquisa da Sociedade Internacional de Cirurgia Estética (ISAPS) mostrou que 2.524.115 cirurgias plásticas foram realizadas no Brasil em 2017. O aumento de mama tem sido um dos procedimentos plásticos mais comumente realizados no Brasil, totalizando mais de 200.000 cirurgias no ano de 2016. O objetivo é avaliar as práticas e tendências atuais em aumento de mama no Brasil.
Métodos: Um questionário de 31 itens foi construído e enviado para 6.200 membros ativos da Sociedade Brasileira de Cirurgia Plástica (SBCP). A pesquisa abordou algumas tendências atuais e práticas controversas considerando cinco áreas: controvérsias atuais, novas tecnologias, implantes mamários, considerações técnicas em procedimentos secundários e aspectos demográficos. O questionário foi desenhado para delinear o perfil das práticas, procedimentos e crenças entre os cirurgiões plásticos sobre o uso dos implantes mamários no Brasil.
Resultados: Foram coletadas 505 respostas, representando uma taxa de resposta em torno de 10%, bem acima da taxa média de resposta de um questionário da SBCP. Os resultados foram distribuídos em 5 tabelas de acordo com as áreas de interesse.
Conclusão: Nesta pesquisa, as práticas mais comuns incluíram o uso de implantes redondos de microtextura e silicone revestido com poliuretano no procedimento primário, poket subglandular, incisões inframamárias, dimensionamento pré-operatório com amostras de implante redondas, antibióticos intravenosos e orais, irrigação com duplo antibiótico, implante faixa de tamanho geralmente menos 325cc e sem drenagem. Não há consenso sobre a lipotransferência mamária e o manejo da contratura capsular e da deformidade da bolha dupla.

Submuscular implant in double pocket for augmentation mastopexy

ABSTRACT

Introduction: The use of implants is steadily increasing. Reports have been published of implants in the subglandular, subfascial, total, and partial submuscular positions each with its indications, limitations, and complications.
Methods: This study presents a technique to cover the implant and support it using two muscle flaps. Between November 2009 and April 2012, 80 patients who submitted to inclusion of implants in the submuscular position in double pocket were analyzed, regardless of the access route, the degree of flaccidity, and ptosis.
Results: The versatility of this maneuver allows it to be used in a wide variety of breasts, from breast augmentation without flaccidity to mastopexy with implant, and may be carried out via the periareolar, inframammary, or open access routes. It was used in primary and secondary surgeries, with or without the use of breast tissue flap and removal of skin.
Conclusions: The coverage of the implant with the pectoral muscle by the double pocket technique displayed excellent results, with a low rate of complications and reinterventions. This technique is being studied in a larger sample with a longer follow-up to confirm the results obtained so far.

RESUMO

Introdução: A utilização de implantes mamários vem aumentando. O posicionamento do implante na literatura é relatado como subglandular, subfascial, submuscular total e parcial, cada qual com suas indicações, limitações e complicações.
Métodos: Este trabalho mostra uma manobra para cobertura do implante e sua sustentação utilizando dois retalhos musculares. Entre novembro de 2009 e abril de 2012, foram analisados 80 casos de pacientes submetidas à inclusão de implantes em posição submuscular em duplo bolso, independentemente da via de acesso, do grau de flacidez e ptose.
Resultados: A versatilidade desta manobra permite que seja utilizada em uma ampla variedade de mamas, desde aumento sem flacidez a mamopexia com implante, podendo ser realizada por via periareolar, inframamária ou aberta. Foi utilizada em cirurgias primárias e secundárias, com ou sem uso de retalho de tecido mamário e retirada de pele.
Conclusões: A cobertura do implante com o músculo peitoral pela técnica de duplo bolso demonstrou ótimos resultados, com baixo índice de complicações e reintervenções. Esta técnica está sendo utilizada em uma maior casuística e um seguimento mais longo tem sido feito para confirmar os resultados obtidos até o momento.

Effectiveness of Pessoa's universal technique for single marking in mastoplasty performed while training plastic surgeons

ABSTRACT

Introduction: Reduction mastoplasty is still a challenging procedure for plastic surgeons. The Objective is to show the results of the treatment of different degrees of breast hypertrophy using Pessoa's single marking technique performed while training plastic surgeons.
Methods: A retrospective cohort of 95 patients underwent reduction mastoplasty using Pessoa's technique. The cohort included patients with breast ptosis, hypertrophy, gigantomastia, and/ or breast asymmetry. The procedures were performed by first-, second-, and third-year residents. The exclusion criteria were psychological disorders, morbid obesity, and/or indication for prosthesis implantation without skin and/or areola reduction. The analyzed factors were age, sex, final size, patient's degree of satisfaction, complications, and difficulties experienced by the residents in performing the surgery.
Results: All patients were women aged between 21 and 61 years. Of the patients, 90.53% reported being very satisfied; 5.26%, satisfied; 3.16%, poorly satisfied; and 1.05%, dissatisfied. The complications observed were necrosis of the nipple-areola complex (1.05%), epidermolysis (11.58%), and suture dehiscence (9.47%). The rate of difficulty experienced by the residents varied between 11.1% and 55.6%, depending on the step of the surgical procedure.
Conclusions: A high degree of satisfaction was reported by both the patients and residents. The results confirm the effectiveness of the technique while training residents.

RESUMO

Introdução: A mamoplastia redutora é ainda um procedimento que desafia o cirurgião plástico. O objetivo é demonstrar os resultados no tratamento de variados graus de hipertrofia e ptose mamária utilizando-se a técnica de Marcação Única, proposta por Pessoa, durante o treinamento de novos cirurgiões plásticos.
Métodos: Coorte retrospectivo de 95 pacientes submetidas à mamoplastia redutora pela técnica de Pessoa. Incluídas pacientes com ptose, hipertrofia, gigantomastia e/ou assimetria das mamas. Os procedimentos foram realizados por residentes do primeiro, segundo e terceiro ano. Como fatores de exclusão foram usados: distúrbios psicológicos, obesidade mórbida e/ou pacientes com indicação de implante de prótese sem redução de pele e/ou aréola. Foram avaliados a idade, sexo, volume resultante, grau de satisfação, complicações e dificuldades dos residentes para realização do procedimento cirúrgico.
Resultados: Todas as pacientes do sexo feminino, com idade entre 21 e 61 anos. 90,53% das pacientes declararam-se muito satisfeitas, 5,26% satisfeitas, 3,16% pouco satisfeitas e 1,05% insatisfeita. Como complicações, foram observadas necrose de complexo areolopapilar (1,05%), epidermólise (11,58%) e deiscência de sutura (9,47%). As dificuldades encontradas pelos residentes, dependendo da etapa do procedimento cirúrgico, variaram entre 11,1% e 55,6%.
Conclusões: Verificou-se alto grau de satisfação tanto da parte das pacientes quanto dos residentes envolvidos. Os resultados encontrados confirmam a efetividade da técnica no treinamento de residentes.

Assessment of the quality of life in patients with breast hypertrophy before and after reduction mammoplasty

ABSTRACT

Introduction: To evaluate the results of breast reduction using the BREAST-Q® instrument.
Methods: This is a prospective cohort study of 83 patients with breast hypertrophy who were treated at the Plastic Surgery Service of the University Hospital of the Federal University of Juiz de Fora. The subjects were older than 18, were literate, and underwent surgery between July 2013 and August 2015. We administered the BREAST-Q® reduction mammoplasty/mastopexy questionnaire, both during the preoperative period and at 6 months after surgery. The Student's t-test was used to evaluate the significance of changes in breast satisfaction and the psychosocial, physical, and sexual well-being of the patients.
Results: Seventy-nine patients (95.1%) completed the pre- and postoperative questionnaires. Significant postoperative improvements were observed in breast satisfaction and the psychosocial, physical, and sexual wellbeing. Age, amount of breast tissue removed, and nutritional status did not influence the results. Surgical complications were observed in 10 patients (12.04%).
Conclusion: The BREAST-Q® showed that patients who underwent breast reduction surgery achieved a significant improvement in quality of life and were satisfied with the surgical outcome, hospital care, and nipple appearance in the late postoperative period.

RESUMO

Introdução: Avaliar os resultados da mamoplastia redutora por meio do instrumento BREAST-Q®.
Métodos: Foram selecionadas 83 pacientes portadoras de hipertrofia mamária atendidas no Serviço de Cirurgia Plástica do Hospital Universitário da Universidade Federal de Juiz de Fora (HU-UFJF), conduzidas em um estudo de coorte prospectivo. Com idade superior a 18 anos, alfabetizadas, operadas no período entre julho de 2013 a agosto de 2015. Utilizou-se o questionário BREAST-Q®, módulo para mamoplastia redutora/mastopexia no pré-operatório e com seis meses de pós-operatório. Teste t pareado foi utilizado para avaliar a significância das mudanças na satisfação com as mamas, bem-estar psicossocial, bem-estar físico e bem-estar sexual.
Resultados: Setenta e nove pacientes (95,1%) responderam os questionários de pré e pós-operatório. Melhoras com significância estatística foram observadas nas médias das categorias presentes quando comparados pré e pós-operatório, como satisfação com as mamas, bem-estar psicossocial, bem-estar físico e bem-estar sexual. Idade, peso retirado e estado nutricional não influenciaram os resultados. As complicações foram observadas em dez (12,04%) pacientes.
Conclusão: O BREAST-Q® mostrou que as pacientes com hipertrofia mamária submetidas à cirurgia para redução do volume mamário tiveram uma melhora significativa na qualidade de vida em seus diversos aspectos, além de avaliar como positivo o resultado cirúrgico, a atuação da equipe médico-hospitalar e a satisfação com os mamilos no pós-operatório tardio.

Breast reconstructions: a 16-year retrospective study

ABSTRACT

Introduction: Breast cancer is the second most common type of cancer among women in Brazil. An estimated 59,700 new cases of breast cancer were reported in the 2018-2019 biennium. Breast reconstruction is a safe procedure, and various surgical procedures have been described, including conservative techniques and use of neighborhood flaps, alloplastic materials, and pedicled and microsurgical myocutaneous flaps. The objective of this study was to analyze cases of breast reconstruction after mastectomy for breast cancer performed over a period of 16 years.
Methods: We reviewed the medical records of patients who underwent breast reconstruction after mastectomy for breast cancer between January 2002 and December 2017.
Results: Within the study period, 586 patients underwent breast reconstruction. Breast reconstruction was performed with a transverse rectus abdominis myocutaneous muscle (TRAM) flap in 160 patients, with conservative techniques in 107 patients, with a latissimus dorsi myocutaneous flap (LDMF) in 156 patients, with prostheses in 113 patients, and secondarily in 50 patients. Before October 2007, the proportion of patients who underwent bilateral surgeries with a TRAM flap and LDMF was 30%, and since then, the percentage has increased to 83.3%. One or more types of complications occurred in 203 patients (34.64%) who underwent surgery, with a total of 335 complications . Most outpatient complications did not require surgical reassessment.
Conclusion: The incidence of bilateral surgery increased, which is attributed to the increase in the number of prophylactic mastectomies in the contralateral breast and in the number of reconstructions using a LDMF compared with that using a TRAM flap. A significant increase in the number of reconstructions with silicone implants was also observed.

RESUMO

Introdução: Câncer de mama é o segundo tipo mais comum de câncer entre mulheres no Brasil. Estimam-se 59.700 casos novos de câncer de mama para o biênio 2018-2019. Reconstrução mamária é um procedimento seguro e vários procedimentos cirúrgicos são descritos para sua realização: técnicas conservadoras, retalhos de vizinhança, materiais aloplásticos, retalhos miocutâneos pediculados e microcirúrgicos. O objetivo deste estudo foi analisar os casos de reconstrução mamária pós-mastectomia por câncer de mama, realizados em um período de 16 anos.
Métodos: Foi realizada revisão de prontuários de pacientes submetidas à reconstrução mamária pós-mastectomia por câncer de mama, no período de janeiro de 2002 a dezembro de 2017.
Resultados: No período analisado, 586 pacientes foram submetidas à reconstrução mamária. Em 160 pacientes a reconstrução mamária foi realizada com retalho miocutâneo do músculo retoabdominal (TRAM), 107 com técnicas conservadoras, 156 com retalho miocutâneo do músculo grande dorsal (RGD), 113 com próteses e 50 secundárias. Previamente a outubro de 2007, a porcentagem de pacientes submetidas a cirurgias bilaterais, somando-se TRAM e RGD, era de 30%, e a partir desse período a porcentagem passou para 83,3%. Houve algum tipo de complicação ou intercorrência em 203 (34,64%) pacientes operadas, totalizando de 335 complicações. Grande maioria apresentou intercorrências tratadas ambulatorialmente sem necessidade de reabordagem cirúrgica.
Conclusão: Houve aumento da incidência de cirurgias bilaterais, fato atribuído ao aumento das mastectomias profiláticas na mama contralateral e aumento do número de reconstruções utilizando RGD em comparação com o TRAM, bem como o aumento significativo das reconstruções com implante de silicone.

Post-Bariatric surgery: six surgeries in two steps

ABSTRACT

Introduction: Weight reduction achieved after bariatric surgery can result in deformities of various parts of the body. The need for reduction of leftover skin and body remodeling makes patients seek body contouring surgery to improve their quality of life. The aim of the study was to present a protocol for post-bariatric plastic surgery at Daher Hospital.
Methods: This was a retrospective longitudinal observational study. As routine protocol, we divided the procedures into two steps, performed in different surgical stages, which we termed the two-level principle. Herein we describe the routine of pre, intra, and postoperative care, as well as details of the surgical techniques. Photographic images were analyzed by an external evaluator.
Results: The study included 13 ex-obese patients, with a mean age of 44 years. Of them, 6 patients underwent lower- and upper-level surgeries and another 6 underwent lower-level surgeries. The mean surgical time for the lower level was 4 hours and 54 minutes and that for the upper level was 4 hours and 5 minutes. A total of 92.30% of the patients were satisfied with the results. An external evaluator classified 92.30% of the cases as good or great.
Conclusion: Circumferential abdominoplasty has immense benefits and complementary surgeries are vital for a satisfactory result. Reduction of the surgical time requires great synchronization of the surgical team, which allows procedures to be performed simultaneously by multiple surgical teams working in different body parts. Satisfactory results and low complication rates allow the applicability of our routine in post-bariatric patients.

RESUMO

Introdução: Redução ponderal conseguida após cirurgia bariátrica pode resultar em deformidades de diversos segmentos corporais. Necessidade da redução das sobras de pele e da remodelação do corpo faz com que a cirurgia do contorno corporal seja procurada por pacientes para melhorar a qualidade de vida. Objetivo do estudo é apresentar a rotina de cirurgia plástica pós-bariátrica do Hospital Daher.
Métodos: Estudo observacional longitudinal retrospectivo. Como rotina, dividimos os procedimentos em duas etapas realizadas em atos operatórios distintos: princípio dos dois andares. Descrevemos a rotina dos cuidados pré, intra e pós-operatório, bem como detalhes das técnicas cirúrgicas. Imagens fotográficas foram analisadas por um avaliador externo.
Resultados: Estudo composto 13 pacientes ex-obesos, média de idade de 44 anos. Seis pacientes foram submetidos a cirurgia do andar inferior e superior e outros 6 apenas do andar inferior. Tempo médio da cirurgia do andar inferior foi de 4 horas e 54 minutos e do andar superior foi de 4 horas e 5 minutos. 92,30% dos pacientes mostraram-se satisfeitos com o resultado. Avaliação externa revelou uma taxa de 92,30% dos casos classificados como bons ou ótimos.
Conclusão: Abdominoplastia circunferencial traz grandes benefícios e as cirurgias complementares são primordiais para um resultado satisfatório. Redução do tempo cirúrgico exige grande sincronização da equipe cirúrgica, permitindo realizar procedimentos combinados por times cirúrgicos trabalhando em compartimentos corporais diferentes, em tempo simultâneo. Resultados satisfatórios obtidos e baixos índices de complicação possibilitam aplicabilidade da nossa rotina em pacientes pós-bariátricos.

"Orthoglossopelveplasty" and the algorithm for its use in the Pierre Robin sequence

ABSTRACT

Introduction: Several patients with the Pierre Robin sequence (micrognathia, glossoptosis, and airway obstruction) have an altered, shortened, and retractable genioglossus muscle that prevents protraction of the tongue and keeps the anterior part of the tongue vertical and its volume posteriorly displaced. This can contribute to supraglottic obstruction, feeding difficulty, and inversion of the growth stimulation forces of the mandibular body.
Methods: A retrospective study of patients with the Pierre Robin sequence treated between 2012 and 2017 with "orthoglossopelveplasty," which includes modification of glossopexy, releasing the anomalous genioglossus of its insertion and releasing the tongue to raise its anterior third and advance the volume of its base using a traction suture of the tongue base to the mandible symphysis. We present a treatment algorithm that prioritizes the need for surgery associated, or not, with mandibular distraction in accordance with respiratory and/or feeding difficulty severity.
Results: Twelve cases of oropharyngeal obstruction treated from 2012 to 2017 are presented, their priorities and orthoglossopleoplasty are discussed, and the proposed algorithm is applied.
Conclusion: Anatomical reorganization of the musculature in a correct anterior position provides protraction and functionality to the tongue, clears the airway in the oropharynx, and improves the feeding function and mandibular development, with low surgical morbidity rates and few complications.

RESUMO

Introdução: Muitos pacientes portadores de sequência de Pierre Robin (micrognatia, glossoptose e obstrução de via aérea) apresentam o músculo genioglosso alterado, encurtado e retrátil, que impede a protração lingual, mantendo a parte anterior da língua verticalizada e seu volume deslocado em direção posterior. Isso pode corroborar para obstrução supraglótica, dificuldade alimentar e inversão das forças de estímulo do crescimento do corpo mandibular.
Métodos: Estudo retrospectivo de pacientes com Pierre Robin tratados entre 2012 e 2017 pela equipe, com descrição da "ortoglossopelveplastia", que propõe uma modificação na glossopexia, soltando o genioglosso anômalo da sua inserção, liberando a língua para elevar seu terço anterior e avançar o volume de sua base, sendo auxiliada por ponto de tração da base lingual à sínfise mandibular. Apresentamos um algoritmo de tratamento proposto que prioriza a necessidade desta cirurgia, associada ou não à distração mandibular, de acordo com a gravidade da dificuldade respiratória e/ou alimentar.
Resultados: São apresentados 12 casos de obstrução da orofaringe atendidos de 2012 a 2017, discutem-se suas prioridades, a ortoglossopelveplastia e se aplica o algoritmo proposto.
Conclusão: A reorganização anatômica da musculatura em uma posição anteriorizada correta proporciona protração e funcionalidade à língua, com desobstrução da via aérea na orofaringe, melhora da função alimentar e do desenvolvimento mandibular, com baixa morbidade cirúrgica e poucas complicações.

Validation of the Portuguese version of the Gothenburg Trismus Questionnaire

ABSTRACT

Currently, the Gothenburg Trismus Questionnaire (GTQ) is the only quality-of-life questionnaire specific for the assessment of trismus. The disease, characterized by limited mouth opening, impairs usual activities such as eating, swallowing, talking, and performing oral hygiene, causing great discomfort to patients. Translation of quality-of-life questionnaires plays an important role in promoting health awareness in the populations of different countries. This study aimed to validate the Portuguese version of the GTQ to allow its effective application in Portuguesespeaking populations. The Portuguese version of the GTQ was successfully validated through the following steps: translation, back translation, cultural adaptation, and revalidation.

RESUMO

Atualmente, o Gothenburg Trismus Questionnaire (GTQ) é o único questionário de qualidade de vida específico sobre trismo. A afecção, definida como restrição à abertura da boca, gera prejuízo a atividades habituais como comer, engolir, falar e fazer a higiene oral, trazendo grande desconforto aos pacientes. A tradução de questionários de qualidade de vida desempenha um importante papel no conhecimento da saúde das populações nos diferentes países. O objetivo do presente estudo é apresentar a validação do GTQ para a língua portuguesa, a fim de permitir sua aplicação efetiva nas populações de idioma português. O GTQ foi validado com sucesso para a língua portuguesa conforme os seguintes passos: Tradução, Retradução (back translation), Adaptação cultural e Revalidação.

Coverage of the lower third of the limb and foot injuries using reverse sural fasciocutaneous flap described by Carriquiry

ABSTRACT

Introduction: Skin grafts are not effective to cover lesions in the distal third of the lower limbs that expose the bones, joints, tendons, and blood vessels due to a limited vascular bed and poor granulation of the wounds. These lesions can only be corrected with microsurgical transfer or muscle, myocutaneous, or fasciocutaneous flaps.
Methods: The lower border of the flap was marked 5 cm above the malleolus. The upper border was marked after providing sufficient length for complete coverage of the lesion. The incision was performed at the marked upper border, and the skin and subcutaneous tissue were elevated together with muscle fascia. The sural nerve was preserved in its original bed. The flap was lifted to the marked lower border (the pedicle). At this point, the flap was transposed at a sufficient angle to cover the lesion.
Results: Eight cases of surgery were conducted using the flap described above. All cases had exposed bones and tendons in the distal region of the limb, back of the foot, or both, in which the reverse sural fasciocutaneous flap with the technique proposed by Carriquiry was used. The cases showed satisfactory esthetic and functional results.
Conclusion: The used flap can correct lesions of the lower third of the limbs and foot. It is relatively easy to make, with good vascular supply, and there is no functional loss of the donor area.

RESUMO

Introdução: Lesões no terço distal dos membros inferiores, com exposição de ossos, articulações, tendões e vasos sanguíneos, não são passíveis do uso de enxertos de pele. Isto ocorre porque o leito vascular é exíguo e pela pobre granulação das feridas, podendo apenas ser corrigidas com retalhos musculares, miocutâneos, fasciocutâneos ou transferência microcirúrgica.
Métodos: O retalho em seu limite inferior é demarcado a partir de 5cm acima dos maléolos. Superiormente, é marcado num comprimento suficiente para cobertura total da lesão. Realizada incisão em demarcação prévia, e elevados pele e tecido subcutâneo juntamente com a fáscia muscular. O nervo sural é preservado em seu leito original. A elevação do retalho se dá até o ponto inferior marcado (o pedículo). Neste ponto, o retalho é transposto numa angulação suficiente para alcançar a lesão.
Resultados: Oito casos foram operados utilizando o retalho descrito. Todos apresentavam exposição de ossos e tendões em região distal da perna, dorso do pé ou ambos, nos quais foram utilizados o retalho fasciocutâneo reverso da perna com a técnica proposta por Carriquiry. Os casos apresentaram resultados estético e funcional satisfatórios.
Conclusão: O retalho utilizado se presta à correção de lesões do terço inferior da perna e do pé. É relativamente fácil de ser confeccionado, com bom suprimento vascular, e não há perda funcional do leito doador.

Analysis of Training in Plastic Surgery by the Brazilian Society of Plastic Surgery as Reported by Final Year Trainees

ABSTRACT

Introduction: Much has been discussed about the training of a plastic surgeon nationally and internationally. There is a need to improve and standardize training to ensure the future of this specialty.
Methods: Questionnaires were filled by third year trainees at the Brazilian Congress of Plastic Surgery (Belo Horizonte).
Results: A total of 230 questionnaires were distributed and 113 were included in the study. The respondents included 71 men and 41 women; 34 were from institutions recognized by the Brazilian Society of Plastic Surgery (SBCP) and 71 were from institutions recognized by the Ministry of Education and the SBCP. Ninety-six respondents revealed that purely aesthetic procedures were conducted in their institutions, with an average of 54.3% of aesthetic procedures. The respondents had the least confidence in performing hair transplants and the most confidence in performing an abdominoplasty. The topic most requested for training was rhinoplasty and the least requested was abdominoplasty. The trainees were fairly satisfied with their programs, with an average satisfaction level of 3.89, on a scale of 1 to 5. The procedure that needed to be performed more frequently was rhinoplasty (more than 10 procedures). Most trainees felt that the program prepared them to practice surgeries, with an average of 3.8 on a scale of 1 to 5. Further, 65% found it necessary to have a fellowship, with mastology being the most requested. The most common procedure was reduction mammoplasty. Most of the trainees wanted to work in a private clinic.
Conclusion: In order to improve the level of education, the accredited institutions should meet the requirements necessary for good preparation of the trainees.

RESUMO

Introdução: Muito se discute sobre a formação do cirurgião plástico na especialização médica nacional e internacionalmente. Há necessidade da busca por melhoras e padronização na formação visando o futuro da especialidade.
Métodos: Foi avaliado protocolo preenchido no Congresso Brasileiro de Cirurgia Plástica (Belo Horizonte) por especializandos do terceiro ano.
Resultados: Foram distribuídos 230 protocolos. 113 protocolos foram incluídos. A amostra incluiu 71 homens e 41 mulheres. 34 eram de serviços cadastrados pela Sociedade Brasileira de Cirurgia Plástica (SBCP) e 71 eram de serviços cadastrados pelo Ministério da Educação e SBCP. 96 afirmaram que em seus serviços são realizados procedimentos puramente estéticos, com média de 54,3% de procedimentos estéticos. O procedimento com menos confiança em realizar foi transplante capilar, e mais confiança foi abdominoplastia. Área de interesse mais requisitada foi rinoplastia e a menos foi abdominoplastia. Os especializandos estão regularmente satisfeitos com seus programas, com média de 3,89, em uma escala de 1 a 5. O procedimento que deve ser mais realizado foi rinoplastia, sendo necessário, do ponto de vista deles, realizar mais de 10 procedimentos. Os especializandos sentem-se bem preparados pela programa, com média de 3,8 em uma escala de 1 a 5. 65% deles acham necessário fazer fellow, sendo o mais requisitado de mastologia. O procedimento mais realizado foi mamoplastia redutora. A maioria dos especializandos quer trabalhar em clínica privada.
Conclusão: Visando aprimorar a formação acadêmica, é necessário que os serviços credenciados se adequem aos requisitos necessários para a boa formação dos especializandos.

Reconstruction of the lower eyelid using a full-thickness upper eyelid transposition flap

ABSTRACT

Introduction: The best method to reconstruct an anatomical unit is to replace the injured area with the same tissues.
Methods: Three patients with basal cell carcinoma in the lower eyelid underwent full-thickness lower eyelid excision, followed by immediate reconstruction with a single-stage fullthickness transposition of the upper eyelid tissue.
Results: The patients had almost no complications, and the functions of total closure and protection of the eyeballs of the eyelids were maintained.
Conclusion: The flap proved to be feasible and is an alternative choice for reconstruction of the lower eyelid.

RESUMO

Introdução: O melhor método para reconstruir uma unidade anatômica é substituir a área lesada pelos mesmos tecidos.
Métodos: Três pacientes que apresentavam carcinomas basocelulares na pálpebra inferior foram submetidos à excisão, seguida de reconstrução imediata com a transposição em espessura total de tecido da pálpebra superior em um só tempo.
Resultados: Os pacientes evoluíram quase sem complicações, as pálpebras mantiveram suas funções de fechamento total e proteção do globo ocular.
Conclusão: O retalho se mostrou factível e é mais uma alternativa para reconstruir a pálpebra inferior.

ABSTRACT

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease characterized by painful deep subcutaneous nodules with phlogistic signs, which are initially hard and progress to have a soft consistency. It occurs in 1-4% of the world population. Etiology of HS is still poorly understood and is suggested to occur due to occlusion of the apocrine duct of the hair follicles by triggering factors such as friction of the adipose tissue and poor hygiene, among others. Diagnosis is eminently clinical, through the identification of typical recurrent lesions that include nodules, abscesses, sinus tracts, or scars. There are no pathognomonic tests used to confirm its presence. Progression is variable and difficult to manage, which can be done with topical or systemic therapy or surgical excision. This work reviews the imaging assessment of HS and shows images of a case assessed by magnetic resonance imaging. Imaging assessment, although not specific enough for a diagnosis, is useful to determine the extent of the disease and to exclude differential diagnoses. Moreover, magnetic resonance imaging has an important role in the assessment of anogenital lesions and a potential to reduce recurrences.

RESUMO

A hidradenite supurativa (HS) é uma doença inflamatória crônica da pele caracterizada por apresentar nodulações em níveis profundos da pele, dolorosas e com sinais flogísticos, inicialmente enrijecidas e que evoluem para consistência amolecida. Ocorre em 1 a 4% da população mundial. A sua etiologia ainda é pouco conhecida, sugere-se que aconteça devido à oclusão do ducto apócrino dos folículos pilosos por fatores precipitantes como fricção de tecido adiposo, higiene precária, entre outras. Seu diagnóstico é eminentemente clínico, pela identificação de lesões típicas recorrentes em forma de nodularidades, abcessos, tratos fistulosos ou cicatrizes. Não há testes patognomônicos. Sua evolução é variável e de difícil manejo, o qual pode ser feito com terapia tópica, sistêmica ou por exérese cirúrgica. Este trabalho revisa a avaliação por imagem da hidroadenite supurativa e demonstra imagens de um caso avaliado por ressonância magnética. A avaliação por exames de imagem, apesar de pouco específica para firmar diagnóstico, é muito útil na determinação da extensão da doença, assim como na exclusão de diagnósticos diferenciais, destacando-se o papel da ressonância magnética na avaliação das lesões anogenitais, com potencial de reduzir recorrências.

Rivaroxaban for venous thromboembolism prophylaxis in abdominoplasty after massive weight loss: 396 cases

ABSTRACT

Introduction: Abdominoplasty is one of the most popular aesthetic procedures performed in Brazil. Postbariatric patients present a challenge to the plastic surgeon as not only do they have complex reconstructive challenges but also they have residual medical comorbidities and nutritional deficiencies. A serious and potentially fatal complication of abdominoplasty is venous thromboembolism (VTE). Despite the frequency of this serious complication, the accepted standard methods to prevent VTE in abdominoplasty patients, including chemoprophylaxis, remain controversy.
Objective: To evaluate the author experience with rivaroxaban, for VTE prophylaxis in abdominoplasty patients after massive weight loss.
Methods: A retrospective 396 cases series were conducted. All patients who underwent abdominoplasty after bariatric surgery and received rivaroxaban were included. The prophylactic dose was 10 mg daily for 30 days, beginning 24 hours postoperatively. Patient demographics, comorbidities, type of surgery and complications were recorded.
Results: From July 2015 until July 2018, 396 post bariatric patients (356 women and 40 men) underwent abdominoplasty and received rivaroxaban postoperatively. The mean body mass index prior to their weight loss procedure was 43.8kg/m2 (range, 37.3- 61.9kg/m2) and mean BMI was 27.2kg/m² at the time of the abdominoplasty. Mean patient age was 39.1 years. Only one patient had a symptomatic PTE event. Thirteen patients had a hematoma requiring operative evacuation, and all went on to heal without sequel.
Conclusions: Routine chemoprophylaxis with rivaroxaban for abdominoplasty patients after massive weight loss has a low rate of VTE events. This oral medication is well tolerated and has an acceptable complication profile.

Introdução: Abdominoplastia consiste em um dos procedimentos estéticos mais populares realizados no Brasil. Pacientes pósbariátricos representam um desafio peculiar ao cirurgião plástico, uma vez que não só requerem reconstruções complexas, mas também apresentam comorbidades residuais e deficiências nutricionais. O tromboembolismo venoso (TEV) constitui uma complicação grave e potencialmente fatal da abdominoplastia. Apesar da pequena frequência desta complicação, os métodos aceitos como padrões para prevenção de TEV em pacientes após abdominoplastia, incluindo quimioprofilaxia, permanecem controversos.
Objetivo: Avaliar a experiência do autor com rivaroxabana para profilaxia de TEV em pacientes submetidos a abdominoplastia após grande perda ponderal.
Métodos: Uma série de 396 casos foi conduzida retrospectivamente. Todos os pacientes submetidos à abdominoplastia após cirurgia bariátrica que receberam rivaroxabana foram incluídos. A dose profilática foi de 10mg por dia. Dados demográficos, comorbidades, tipo de cirurgia e complicações foram registrados.
Resultados: 396 casos de pacientes pós-bariátricos (356 mulheres e 40 homens) foram submetidos à abdominoplastia e receberam rivaroxabana no pós-operatório, de julho de 2015 a julho de 2018. A média de idade dos pacientes foi de 39,1 anos. O índice de massa corporal médio no momento da abdominoplastia foi de 27,2kg/m². Houve apenas um caso de tromboembolismo venoso (0,25%). Treze pacientes apresentaram hematoma com necessidade de drenagem.
Conclusões: A quimioprofilaxia de rotina com rivaroxabana para pacientes submetidos à abdominoplastia após grande perda ponderal revela uma baixa incidência de TEV. Esta medicação oral é bem tolerada e apresenta um perfil de complicação aceitável.

Injection of platelet aggregates in facial rejuvenation: a systematic review

ABSTRACT

Introduction: This systematic review was conducted to assess whether the use of a platelet aggregate injection with or without associated facial rejuvenation techniques favors facial rejuvenation in adult patients.
Methods: Randomized clinical trials that compared the use of techniques for facial rejuvenation alone with the same techniques coupled with the injection of platelet aggregates were searched. The search was performed in indexed databases and in the gray literature. The Cochrane Collaboration bias risk tool was applied to assess the quality of the studies.
Results: In total, 7137 articles were identified. Only four studies remained in the qualitative synthesis, and the others were considered as having undefined bias risk in the key domains.
Conclusion: There are few studies in the literature that compare the use of platelet aggregates in facial rejuvenation and those that are available have a risk of "undefined" or "high" bias. There is a need for more well-designed clinical studies comparing the use of platelet aggregate injection with or without associated facial rejuvenation techniques.

RESUMO

Introdução: Essa revisão sistemática foi conduzida para avaliar se a associação da aplicação da injeção de agregados plaquetários quando comparada a outras terapias faciais favorece no rejuvenescimento facial em pacientes adultos.
Métodos: A pesquisa buscou ensaios clínicos randomizados que compararam uso de técnicas de rejuvenescimento facial isoladas com as mesmas técnicas aliadas à injeção de agregados plaquetários. A busca foi realizada em bases de dados indexadas e literatura cinzenta. A ferramenta de risco de viés da "Cochrane Collaboration" foi aplicada para a avaliação da qualidade dos estudos.
Resultados: Foram identificados 7137 artigos. Apenas quatro estudos permaneceram na síntese qualitativa, e os demais foram considerados com risco indefinido de viés nos domínios chaves.
Conclusão: Existem poucos estudos na literatura que comparam o uso de agregados plaquetários em rejuvenescimento facial e os que estão disponíveis têm risco de viés "indefinido" ou "alto". Há necessidade de realizar mais estudos clínicos bem delineados que comparem o uso de injeção de agregados plaquetários associados ou não às técnicas de rejuvenescimento facial.

ABSTRACT

Partial ear defects can be treated in several ways, including primary closure, healing by secondary intention, or flaps. Several surgical options have been described for reconstruction in order to maintain the natural contour of the ear, without sacrificing healthy tissues or changing the aesthetics and function. In this article, we present two cases of reconstruction of chondrocutaneous defects of the ear after resection of basal cell carcinoma in the central region of the ear, with the production of a retroauricular island flap transposed through a cartilaginous window with the de-epidermized pedicle. The donor area healed following a primary closure. This procedure can be performed in a single stage, yields satisfactory aesthetic and functional results, and is safe because the retroauricular region is richly vascularized.

RESUMO

Defeitos parciais de orelha podem ser tratados de diversas formas, dentre elas o fechamento primário, cicatrização por segunda intenção ou retalhos. Diversas opções técnicas foram descritas para a sua reconstrução de modo a manter o contorno natural da orelha, sem sacrificar tecido sadio ou alterar sua estética e função. Apresentamos neste artigo dois casos atendidos no Instituto do Câncer do Hospital de Base de São José do Rio Preto de reconstrução de defeitos condrocutâneos de orelha após ressecção de carcinoma basocelular em região central da orelha, com a confecção de retalho retroauricular ilhado transposto através de uma janela cartilaginosa e com o pedículo desepidermizado. Área doadora com fechamento primário. Tal procedimento constitui técnica segura, pois a região retroauricular é ricamente vascularizada, é de fácil execução, em único estágio e com resultado estético e funcional satisfatório.

ABSTRACT

Eagle syndrome is a rare condition, and its etiology has not yet been well established and its correct treatment is uncertain. Its treatment must be defined together with the patient, be it conservative or surgical, always taking into consideration the patient's expectations, in addition to a solid professional expertise in the modality chosen for the treatment. In this article, we present the case of a 35-year-old patient who was admitted to the Felício Rocho Hospital and discuss the various aspects of the disease, including the treatment modality chosen for the case.

RESUMO

A síndrome de Eagle é uma condição rara e com etiologia ainda não bem estabelecida, a qual se deve ter bastante suspeição para seu correto tratamento. Seu tratamento deve ser definido em conjunto com o paciente, seja ele conservador ou cirúrgico, sempre levando em consideração as expectativas do paciente, além da maior expertise do profissional na modalidade escolhida para o tratamento. Neste artigo, apresentamos uma paciente de 35 anos atendida no Hospital Felício Rocho, discutindo os diversos aspectos da doença, inclusive a modalidade de tratamento escolhida para o caso.

Introduction: Burns are one of the most severe traumatic injuries and their treatment requires a multidisciplinary approach, where the role of the plastic surgeon is vital. The plastic surgeon is entrusted with the challenging goal of restoring the skin's protective function and simultaneously recovering the aesthetic aspect of the burnt area.
Case report: A 27-year-old woman underwent a mastopexy with inclusion of implants, where the skin removed from the breast was used as a full-thickness graft in the mandibular and submental area for the treatment of a scar. The patient showed complete integration of the graft, and no areas of epidermolysis were observed. The aesthetic results were excellent, and the patient was completely satisfied; moreover, an improvement in the dyschromic and hypertrophic cicatricial areas was observed.
Conclusion: An autologous graft using breast skin is a good alternative for the treatment of sequelae of burns on the face and provides excellent aesthetic results.

RESUMO

Introdução: As queimaduras constituem uma das lesões traumáticas mais graves e seu tratamento requer uma abordagem multidisciplinar, em que o papel do cirurgião plástico é fundamental. Restabelecer a função de proteção da pele, mas também recuperar a estética da área, queimada são objetivos desafiadores que o cirurgião plástico procura atingir.
Relato de Caso: Paciente feminino de 27 anos submetida a mastopexia com inclusão de implantes, em que se aproveitou a pele retirada da mama para realizar um enxerto de espessura total em região mandibular e submentoniana para tratamento de cicatriz. A paciente teve uma integração completa do enxerto, sem evidenciar-se áreas de epidermólise. Os resultados estéticos foram excelentes, conseguindo a satisfação da paciente e melhoria das áreas discrômicas e hipertróficas cicatriciais.
Conclusão: O enxerto autólogo a partir da pele da mama constitui uma boa alternativa para o tratamento de sequelas de queimaduras em face, possibilitando ótimos resultados estéticos.

Idiopathic scrotal calcinosis: an unusual skin disorder

ABSTRACT

Idiopathic scrotal calcinosis is a rare benign entity. Patients affected by scrotal calcinosis usually do not have other associated symptoms. We report the case of a 30-yearold man with several calcified nodules in the scrotal sac with onset at age 15 years. A complete surgical excision of the nodules was performed, and the patient recovered well with satisfactory aesthetic and functional results.

RESUMO

A calcinose escrotal idiopática é uma entidade rara, benigna. Não costuma ter outros sintomas associados. O caso reportado se trata de um paciente de 30 anos com diversos nódulos calcificados em bolsa escrotal, com 15 anos de evolução. Como medida terapêutica, foi realizada excisão cirúrgica completa dos nódulos, apresentando boa evolução cirúrgica e resultados estético e funcional satisfatórios.

Facelift and Facial nerve injury: how to deal with?

ABSTRACT

Is largely known that the facial nerve virtually can be injured in every facial lift. Even though its importance, it is difficult to find in the literature articles related to this theme. Complete or incomplete facial paralysis after a facial procedures, may become a very uncomfortable situation between the patient and the surgeon, that is why we propose in this article a guide which intends to help avoid, identify and manage a facial nerve injury in the event of a face lift. Identifying Facial palsy before the surgery: Intending to make the pre-op facial assessment as simple as possible, we suggest a systematic approach examination. Avoiding facial nerve injury during face lifting: Several technical details are discussed along the text aiming to minimize the risk of nerve damage during the surgical procedure. What to in the event of a post-operative facial palsy: The authors propose a logical approach to the facial palsy, suggesting a planning for diagnosis, classification of the lesion and a rational treatment for the injury. Conclusion: This article presents a guide showing some safe technical options to avoid nerve damage during rythidoplasty, how to identify the lesion and treat it if necessary.

RESUMO

Não é novidade para os cirurgiões plásticos, especialmente aos que se dedicam à cirurgia de face, que o nervo facial pode ser lesado virtualmente em qualquer cirurgia de ritidoplastia. Porém, apesar da importância contida nesse tema, não é tarefa simples encontrar na literatura artigos que abordem de forma objetiva a correlação entre lesão do nervo facial e cirurgia facial. Paralisias faciais, sejam elas completas ou não, podem se tornar situações potencialmente conflituosas na relação médicopaciente. Este artigo, portanto, visa propor de forma clara e sucinta, baseada na experiência dos autores, um guia de como evitar, identificar e tratar uma potencial lesão do nervo facial no contexto de uma cirurgia de face, em especial a ritidoplastia.
