Effects of hyperbaric oxygenation and N-acetylcysteine on the survival of random-pattern skin flaps in rats

ABSTRACT

BACKGROUND: Advances in plastic surgery techniques have enabled reconstruction of extensive wound damage, especially through the use of random flaps. However, the limiting factor for the use of these flaps is the unpredictable blood supply, which may produce irreversible damage to the microcirculation and result in partial or complete flap necrosis, making the wound more susceptible to infection. Therefore, improvement of random flaps, especially in the distal extremity, has been an essential goal for the success of this technique. The objective of this study was to investigate the effects of hyperbaric oxygenation (HBO), N-acetylcysteine (NAC), and the combination of both (HBO + NAC) on the degree of necrosis in modified McFarlane random skin flaps on Wistar rats. METHODS: A total of 32 male Wistar rats were randomly divided into a sham treatment group (SG, n = 8), N-acetylcysteine group (NACG, n = 8), hyperbaric oxygenation group (HBOG, n = 8), and hyperbaric oxygenation plus N-acetylcysteine group (HNG, n = 8). Modified McFarlane random flaps were created in the dorsal region of the rats. RESULTS: The average area of the flaps exhibiting necrosis was 18.3%, 24.3%, 12.6%, and 14.9%, in the SG, NACG, HBOG, and HNG, respectively. The necrotic areas in the HBOG and HNG were significantly smaller than that in the NACG. CONCLUSIONS: HBO treatment was associated with a reduction in the area of necrosis in the skin flaps. NAC treatment alone gave poor results. The use of HBO and NAC in combination did not improve the outcome compared with the use of HBO alone. The findings suggest that oxygen diffusion through the interstitial space was the factor responsible for the favorable results of HBO.

RESUMO

INTRODUÇÃO: Os avanços das técnicas em cirurgia plástica permitiram a reconstrução de extensos defeitos causados por ferimentos, entre as quais destaca-se a utilização dos retalhos randômicos. No entanto, o fator limitante para a utilização desses retalhos é a imprevisibilidade de sua vascularização distal, o que poderá ocasionar danos irreversíveis à microcirculação, resultando em necrose parcial ou completa do retalho, tornando a ferida mais suscetível a infecção. Portanto, melhorar a viabilidade do retalho randômico, principalmente em sua extremidade distal, tem sido uma meta importante para o sucesso dessa técnica. O objetivo deste estudo foi investigar o papel da oxigenação hiperbárica (OHB), da N-acetilcisteína (NAC) e da associação de ambas (OHB + NAC) na área de necrose em retalhos randômicos modificados de McFarlane em pele de ratos Wistar. MÉTODO: No total, 32 ratos Wistar machos foram divididos aleatoriamente em grupo Sham (GS, n = 8), grupo N-acetilcisteína (GNAC, n = 8), grupo oxigênio hiperbárico (GOHB, n = 8) e grupo oxigênio hiperbárico + N-acetilcisteína (GHN, n = 8). Sob anestesia geral, foi executado um retalho randômico modificado de McFarlane na região dorsal dos ratos. RESULTADOS: A necrose média foi de 18,3%, 24,3%, 12,6% e 14,9%, respectivamente, nos grupos GS, GNAC, GOHB e GHN. Os grupos GOHB e GHN apresentaram diferença significativa quando comparados ao grupo GNAC. CONCLUSÕES: A OHB está associada a redução da área de necrose do retalho cutâneo. A NAC foi associada a maus resultados quando usada isoladamente. A associação dos dois procedimentos, OHB e NAC, não potencializou os resultados favoráveis observados com o uso da OHB isoladamente. As descobertas sugerem que a difusão de oxigênio através do espaço intersticial foi o fator determinante de resultados mais favoráveis da OHB.

Cadaver dissection activities in medical residency: experience of the Plastic Surgery Service of Hospital Universitário Professor Edgard Santos, Universidade Federal da Bahia

ABSTRACT

BACKGROUND: Understanding human anatomy is crucial to the development of accurate surgical techniques. Training with anatomical specimens in the university environment is often insufficient for learning. Thus, the importance of using cadavers to teach anatomy and surgical skills in undergraduate and post-graduate courses is reinforced. This study aimed to report scientific outcomes related to cadaver dissection in the Plastic Surgery Service of Hospital Universitário Professor Edgard Santos (HUPES), Universidade Federal da Bahia, from 2009 to 2011, and the importance of this activity for plastic surgery resident training. METHODS: We performed a retrospective observational analysis of scientific production resulting from training activities related to cadaver dissection for the period from 2009 to 2011. RESULTS: We developed activities related to education, research, and extension by means of an agreement signed between the HUPES and the Forensic Medicine Institute of the State of Bahia. Among the educational activities undertaken, dissection activities led to the creation of rich educational materials relevant to the daily practice of resident physicians. With regard to research, several papers have been written, resulting in publications and presentations at scientific meetings. Cadaver dissection also was the subject of a conclusion dissertation and a PhD thesis. Through the partnership, extracurricular training was conducted for medical students as well as members of the Plastic Surgery League of Bahia (LBCP), allowing them to develop skills that had previously not been practiced at the university. CONCLUSIONS: The Plastic Surgery Department - HUPES obtained considerable gains by developing activities related to cadaver dissection. The service is contributing to progress in the three pillars of education: education, research, and extension.

RESUMO

INTRODUÇÃO: O adequado conhecimento da anatomia do ser humano é crucial para o desenvolvimento de uma técnica cirúrgica apurada. O treinamento com peças anatômicas no meio universitário, muitas vezes, é insuficiente para o aprendizado. Assim, o uso de cadáveres para o ensino de anatomia e habilidades cirúrgicas na graduação e pós-graduação tem sua importância reforçada. O objetivo deste estudo foi relatar a produtividade do Serviço de Cirurgia Plástica do Hospital Universitário Professor Edgard Santos (HUPES) da Universidade Federal da Bahia, no que se refere às atividades de dissecção de cadáveres realizadas de 2009 a 2011, bem como a importância dessa atividade no treinamento dos residentes do serviço. MÉTODO: Foi realizada análise observacional retrospectiva da produção do serviço em decorrência das atividades de treinamento em dissecção de cadáveres, no período de 2009 a 2011. RESULTADOS: Foram desenvolvidas atividades no âmbito de ensino, pesquisa e extensão por meio de convênio firmado entre o Serviço de Cirurgia Plástica do HUPES e o Instituto Médico Legal do Estado da Bahia. Entre as ações pedagógicas empreendidas, a atividade de dissecção proporcionou a criação de um material rico de ensino que inclui diversos retalhos importantes para a prática diária dos médicos residentes. Em relação à pesquisa, foram desenvolvidos diversos artigos científicos que geraram publicações e apresentações em eventos científicos. Essa atividade deu origem, ainda, a uma monografia de conclusão de residência e a uma tese de doutorado. Por meio do convênio firmado, foi realizado estágio extracurricular para os acadêmicos de medicina, membros da Liga Baiana de Cirurgia Plástica (LBCP), em que puderam desenvolver habilidades não praticadas na universidade. CONCLUSÕES: Pode-se concluir que o Serviço de Cirurgia Plástica do HUPES obteve ganhos consideráveis com o desenvolvimento das atividades de dissecção cadavérica. Isso demonstra que o serviço vem consolidando sua atuação nos três pilares da educação: ensino, pesquisa e extensão.

Comparative study of the use of ethyl cyanoacrylate adhesive and intracutaneous suture for cutaneous excision closure

ABSTRACT

BACKGROUND: The advantages of cyanoacrylates in cutaneous wound closure have been widely demonstrated in the literature. However, these products have not been widely used in Brazil due to the high costs of octyl cyanoacrylates. Moreover, there have been only a few studies on the most affordable product, ethyl cyanoacrylate, as a cutaneous adhesive. The objective of this study was to evaluate the efficacy of ethyl cyanoacrylate in cutaneous excision repair. METHODS: A retrospective study conducted in patients with superficial skin lesions who underwent elliptical excision and repair by primary closure. The excisions were closed using deep relaxing sutures and either ethyl cyanoacrylate (case group; CaG) or intradermal suture (control group; CoG) on the skin surface. RESULTS: Thirty-three patients were identified with 43 cutaneous lesions (CaG, n = 20; CoG, n = 23). Unsightly scars (CaG = 20%; CoG = 48%), eye irritation (CaG = 15%; CoG = 0), dehiscence (CaG = 5%; CoG = 0), infection (CaG = 0; CoG = 4.3%), and contact dermatitis (CaG = 0; CoG = 4.3%) were the problems observed. The cosmetic results were satisfactory in almost all excisions (CaG = 100%; CoG = 95.7%). CONCLUSIONS: Ethyl cyanoacrylate was adequately tolerated in this study group, with no necrosis, allergic reactions, or infections. Ethyl cyanoacrylate presented several advantages over suture use, including low complication rates and high patient satisfaction.

RESUMO

INTRODUÇÃO: As vantagens dos cianoacrilatos em síntese cutânea têm sido sobejamente demonstradas na literatura. Entretanto, esses produtos têm sido subutilizados no Brasil, em decorrência dos elevados custos do octilcianoacrilato. Ademais, a forma mais economicamente acessível, etilcianoacrilato, tem sido pouco estudada como adesivo cutâneo. O objetivo deste estudo foi avaliar a eficiência do etilcianoacrilato na síntese de excisões cutâneas. MÉTODO: Estudo retrospectivo realizado em pacientes portadores de lesões cutâneas superficiais submetidos a excisão elíptica e reparo por fechamento primário. As excisões foram fechadas por meio de suturas profundas relaxadoras e, na superfície da pele, foi utilizado etilcianoacrilato (grupo caso - GCa) ou sutura intradérmica (grupo gontrole - GCo). RESULTADOS: Foram encontrados 33 pacientes portadores de 43 lesões cutâneas (GCa = 20 excisões; GCo = 23 excisões). Cicatrizes inestéticas (GCa = 20%; GCo = 48%), irritação ocular (GCa = 15%; GCo = 0), deiscência (GCa = 5%; GCo = 0), infecção (GCa = 0; GCo = 4,3%) e dermatite de contato (GCa = 0; GCo = 4,3%) foram os problemas encontrados. Os resultados cosméticos foram considerados satisfatórios na maioria das excisões (GCa = 100%, GCO = 95,7%). CONCLUSÕES: O etilcianoacrilato foi adequadamente tolerado neste grupo de estudo, sem induzir necrose, reações alérgicas ou infecções, apresentando diversas vantagens de uso em relação à sutura, incluindo reduzido índice de complicações e grande satisfação dos pacientes.

Epidemiology of burns in Minas Gerais

ABSTRACT

BACKGROUND: Burns are lesions to organic tissues caused by external agents, resulting in destruction of the epithelial covering. This study aims to clarify the epidemiological profile of burns at Hospital João XXIII, Belo Horizonte, MG, Brazil - the largest center for burn care in Latin America. METHODS: A database of 687 patients admitted to Hospital João XXIII from February 2009 to July 2010 was created; it included patient demographics, burn etiology, surface and burned areas, intentionality, time of admission, and death profiles among other data. RESULTS: Most of the admitted patients were male (62.5%), and the mean age was 29 years old; 66% were from Belo Horizonte, and 34% were from the countryside of other states. Alcohol was the most frequent etiologic agent (34.4%), which was responsible for the most extensive burns (average burned body surface: 28%) and responsible for most deaths (52.7%). With regard to intentionality, 79% were accidental burns, followed by suicide attempts (12%) and aggression (9%). The average period of admission was 23.5 days with a mortality rate of 16.3%, which is decreasing progressively. During the monitoring period, 984 debridement and 584 grafting procedures were performed. CONCLUSIONS: The obtained data are similar to those available in the literature. They also evidence the importance of burn prevention and highlight the need to re-examine how flammable products are marketed in order to decrease the morbidity and mortality rates caused by burns, since most burns are accidental and caused by liquid alcohol.

RESUMO

INTRODUÇÃO: Queimaduras são lesões aos tecidos orgânicos causadas por agentes externos, com destruição do revestimento epitelial. O objetivo deste estudo é divulgar o perfil epidemiológico das queimaduras do maior centro de queimados da América Latina, localizado no Hospital João XXIII, em Belo Horizonte, MG, Brazil. MÉTODO: Foi criado um banco de dados com 687 pacientes internados no Hospital João XXIII, no período de fevereiro de 2009 a julho de 2010, incluindo identificação dos pacientes, etiologia da queimadura, superfície e áreas queimadas, intencionalidade, tempo de internação e perfil dos óbitos, entre outros dados. RESULTADOS: A maioria dos pacientes internados era do sexo masculino (62,5%), com média de idade de 29 anos, sendo 66% provenientes de Belo Horizonte e 34%, do interior ou de outros estados. O álcool foi o agente etiológico mais frequente (34,4%), o causador das queimaduras mais extensas (média de 28% de superfície corporal queimada) e o maior responsável pelos óbitos (52,7%). Quanto à intencionalidade, 79% foram queimaduras acidentais, seguidas pelas tentativas de autoextermínio (12%) e agressão (9%). A média do tempo de internação foi de 23,5 dias, com taxa de mortalidade de 16,3%, que vem caindo progressivamente. Foram realizados 984 desbridamentos e 584 enxertias durante o período de acompanhamento. CONCLUSÕES: Os dados obtidos são similares aos disponíveis na literatura e evidenciam a importância da prevenção e da fiscalização na forma de comercialização dos produtos inflamáveis para diminuir a morbidade e a mortalidade causadas pelas queimaduras, uma vez que a maioria é acidental e decorrente do álcool líquido.

Analysis of the incidence of pressure ulcers at Hospital Metropolitano de Urgência e Emergência in Ananindeua, PA

ABSTRACT

BACKGROUND: Pressure ulcers are one of many complications to which hospitalized patients are susceptible. Such ulcers may lead to partial or total destruction of cutaneous, subcutaneous, and muscular tissues, as well as of bones and articulations. METHODS: An observational, longitudinal, and prospective study was carried out at Hospital Metropolitano de Urgência e Emergência em Ananindeua, PA, Brazil. Relevant data related to gender, age (younger or older than 60 years old), impairment degree, and region of pressure ulcer impairment were collected from Intensive Care Therapy 1 (UTI 1), Surgical Clinic 1, and the Neuro Clinic. RESULTS: Among 474 studied patients, 17 developed pressure ulcers, with greater incidence among male patients (88.2%), at stage II (41.2%) and in the sacral region (40%). Patients aged more than 60 years had an incidence rate of 58.8% for pressure ulcers. CONCLUSIONS: Data obtained in this study are in accordance with the results presented in the literature. It is important to highlight the need for creating risk scales and preventive measures for decreasing the incidence of pressure ulcers in hospitals.

RESUMO

INTRODUÇÃO: A úlcera de pressão configura-se como uma das diversas complicações às quais pacientes hospitalizados estão suscetíveis, podendo levar à destruição parcial ou total de estruturas como tecidos cutâneo, subcutâneo e muscular, ossos e articulações. MÉTODO: Estudo do tipo observacional, longitudinal, prospectivo, desenvolvido no Hospital Metropolitano de Urgência e Emergência em Ananindeua, PA, Brasil. Foram coletados dados pertinentes às variáveis sexo, idade (maior ou menor que 60 anos), grau de acometimento e local de acometimento das úlceras de pressão, nos setores Unidade de Terapia Intensiva 1 (UTI 1), Clínica Cirúrgica 1 e Neuroclínica. RESULTADOS: Dentre os 474 pacientes estudados, 17 desenvolveram úlcera de pressão, com maior incidência no sexo masculino (88,2%), em estágio II (41,2%) e na localização sacral (40%). Pacientes com mais de 60 anos de idade apresentaram incidência de 58,8% de úlcera de pressão. CONCLUSÕES: Os dados obtidos neste estudo estão de acordo com os resultados apresentados na literatura. Ressalta-se a necessidade de criação de escalas de risco e medidas preventivas para diminuir a incidência de úlceras de pressão nos hospitais.

Study of patients with hyperhidrosis treated with botulinum toxin: a 10-year retrospective analysis

ABSTRACT

BACKGROUND: Hyperhidrosis is characterized by excessive sweating of the forehead, hands, feet, and armpits, either alone or in combination. It affects about 1% of the population. This study aimed to observe the effects of botulinum toxin in patients with hyperhidrosis and demonstrates the application technique of botulinum toxin, the areas of incidence of the disease, and the duration of the results. METHODS: A retrospective analysis of 39 patients with primary hyperhidrosis treated between July 2000 and July 2010 and followed up for 12 months was carried out. Of these patients, 36% were male and 64% were female. Patient ages ranged from 16 to 41 years. A total of 135 areas were treated. Treatment consisted of intradermal injections of botulinum toxin. The total dose applied ranged from 37.5 U to 150 U, with an average dose of 75 U for each treated area. RESULTS: The therapeutic effect of botulinum toxin was observed from the third day after treatment, with a 50% reduction in symptoms within the first week of treatment and up to 94% reduction in the number of hyperhidrosis events after the second week. The reduction of symptoms lasted, on average, for 7 months. No cases of compensatory hyperhidrosis or mortality were observed. CONCLUSIONS: The treatment of primary hyperhidrosis with type A botulinum toxin, although temporary, is an effective, safe, and minimally invasive treatment option. It has a high degree of satisfaction and allows patients to return to their professional activities on the same day. Side effects and complications are temporary, infrequent, and regress without sequelae.

RESUMO

INTRODUÇÃO: A hiperidrose caracteriza-se por sudorese excessiva, isolada ou associada, da testa, das mãos, dos pés e das axilas. Atinge cerca de 1% da população. O objetivo deste estudo foi observar os efeitos da toxina botulínica nos pacientes com hiperidrose, demonstrando a técnica, as áreas de incidência e a duração dos resultados obtidos. MÉTODO: Foi realizada análise retrospectiva de 39 pacientes com hiperidrose primária tratados no período de julho de 2000 a julho de 2010, acompanhados durante 12 meses. Desses pacientes, 36% eram do sexo masculino e 64%, do sexo feminino. A idade variou de 16 anos a 41 anos. No total, foram tratadas 135 áreas. Realizou-se tratamento com injeções intradérmicas de toxina botulínica. A dose total aplicada variou entre 37,5 U e 150 U, com dose média de 75 U para cada região tratada. RESULTADOS: O efeito terapêutico foi observado a partir do terceiro dia, com redução de 50% dos sintomas na primeira semana do tratamento e de até 94% do quadro de hiperidrose após a segunda semana de tratamento. A redução dos sintomas durou, em média, 7 meses. Nenhum caso de hiperidrose compensatória foi observado. A mortalidade foi nula. CONCLUSÕES: O tratamento da hiperidrose primária com toxina botulínica tipo A, embora temporário, é uma opção de tratamento eficaz, segura, pouco invasiva e com alto grau de satisfação, permitindo aos doentes o retorno às atividades profissionais no mesmo dia. Os efeitos colaterais e as complicações são temporários, pouco frequentes e regridem sem deixar sequelas.

Facial paralysis: epidemiological analysis in a rehabilitation hospital

ABSTRACT

BACKGROUND: Facial paralysis is characterized by permanent or temporary loss of facial expression due to facial nerve injury. Several factors influence the development of facial nerve lesions. The purpose of this study was to evaluate the epidemiological aspects and incidence of sequelae after facial paralysis at a rehabilitation institution. METHODS: We performed a retrospective study of facial paralysis patients admitted to a rehabilitation hospital between January 2001 and January 2005. Sequelae were analyzed according to gender, age, etiology, functional status as measured by the House-Brackmann scale, evaluation time, follow-up, and surgical procedures. Statistical analyses were performed with Epi-info 3.2.2 software. RESULTS: A total of 285 facial paralysis patients, 157 male and 128 female, were admitted for a rehabilitation program. All subjects followed a rehabilitation program, and 29 (10.2%) underwent surgery; 80% were admitted during the 3rd week of the paralysis or later, and 121 (42.5%) showed gradual recovery after 3 months, either spontaneously or after clinical or physical therapies. Nevertheless, 119 (41.8%) sustained irreversible partial or complete facial paralysis. CONCLUSIONS: The prevalence of facial paralysis was greater among patients younger than 20 years. Among these patients, paralysis had different causes, and these patients were admitted with lower House-Brackmann grades. Most cases were associated with neurological deficits leading to facial paralysis of central or congenital origin.

RESUMO

INTRODUÇÃO: A paralisia facial é a perda temporária ou permanente dos movimentos da mímica facial em decorrência do acometimento do nervo facial. São vários os fatores que influenciam a evolução das lesões do nervo facial. Este estudo teve como objetivo avaliar os aspectos epidemiológicos e a frequência de sequelas após paralisia facial em um serviço de reabilitação. MÉTODO: Estudo retrospectivo dos pacientes com paralisia facial atendidos em hospital de reabilitação no período de janeiro de 2001 a janeiro de 2005. As sequelas foram avaliadas quanto a sexo, idade, etiologia, graduação funcional conforme a escala de House-Brackmann, tempo de evolução, seguimento e intervenções cirúrgicas. Para realização da análise estatística utilizou-se o programa Epi-Info versão 3.2.2. RESULTADOS: Foram admitidos para programa de reabilitação 285 pacientes portadores de paralisia facial, sendo 157 do sexo masculino e 128 do feminino. Todos os pacientes se submeteram a programa de reabilitação e 29 (10,2%), a cirurgia. Dentre os pacientes analisados, 80% foram admitidos a partir da terceira semana do surgimento da paralisia, e 121 (42,5%) tiveram recuperação gradual em 3 meses, espontaneamente, com tratamento clínico ou fisioterápico. Por outro lado, 119 (41,8%) pacientes permaneceram com paralisia facial parcial ou completa e irreversível. CONCLUSÕES: Os casos admitidos foram mais frequentes em pacientes com menos de 20 anos de idade, com causas diversas e quando admitidos em graus menores segundo a escala de House-Brackmann, pois muitos deles se associavam a déficits neurológicos consequentes a paralisia facial de origem central ou congênita.

Correlation between electromyographic data and facial disability index in patients with long-term facial paralysis: implications for treatment outcomes

ABSTRACT

BACKGROUND: Several techniques are available for the assessment of facial movement and activity, and facial disability can be evaluated through self-administered questionnaires. However, the relationship between these objective and subjective measurements has not been examined to date. The present study examined the relationship between electromyographic data of the levator anguli oris muscle with the facial disability index in patients with long-term facial paralysis. We hypothesized that individuals with greater facial asymmetry have lower facial disability index scores. METHODS: Patients were assessed using a clinical scale for the evaluation of facial expression, 2 facial disability index subscales, and the results of surface electromyography (sEMG). Seventeen long-term facial paralysis patients were analyzed and compared to 17 age- and gender-matched healthy controls. RESULTS: Significant differences between right and left hemifaces during smiling and at rest were detected in the experimental group, but not in the controls. Statistical analyses also indicated a weak correlation between sEMG (facial asymmetry) and facial disability index. CONCLUSIONS: The use of modern data analysis techniques such as sEMG in combination with self-reported data is of great benefit to clinicians and their patients. The identification of a combination of measurements from randomized trials that can best determine the most effective treatment for patients with facial paralysis should be the objective of future studies.

RESUMO

INTRODUÇÃO: Apesar de o movimento facial e de a atividade muscular poderem ser quantificados por meio de diversas técnicas e de a inabilidade facial poder ser quantificada por meio de questionários de autoavaliação, a relação entre essas medidas objetivas e subjetivas ainda não foi investigada. O objetivo do presente trabalho foi correlacionar dados eletromiográficos dos músculos elevadores do ângulo da boca com o índice de inabilidade facial em pacientes com paralisia facial de longa duração. A hipótese do estudo foi de que indivíduos com maior assimetria facial apresentariam escores menores no índice de inabilidade facial. MÉTODO: A avaliação consistiu na aplicação de uma escala clínica para avaliação da mímica facial, de duas subescalas do Índice de Inabilidade Facial e da realização do exame de eletromiografia de superfície (EMGs). Foram analisados 17 pacientes com paralisia facial de longa duração e os resultados foram comparados ao grupo controle, composto por 17 indivíduos saudáveis pareados por gênero e idade. RESULTADOS: Os participantes do grupo pesquisa apresentaram diferenças significantes entre as hemifaces nas tarefas de repouso e sorriso. O mesmo não foi observado para os participantes do grupo controle. A análise estatística indicou correlação fraca entre os dados da EMGs (assimetria facial) e o Índice de Inabilidade Facial. CONCLUSÕES: O uso de técnicas científicas modernas de análise de dados, como a EMGs, combinadas a medidas de autoavaliação oferece grandes possibilidades para clínicos e seus pacientes. A combinação de diferentes medidas em estudos randomizados que verifiquem o tipo de tratamento que oferece melhor resultado aos pacientes com paralisia facial deverá ser abordada em estudo futuro.

Correction of prominent ears by the cartilaginous incision technique, definition of the antihelix with Mustardé sutures, and fixation of the ear cartilage at the mastoid

ABSTRACT

BACKGROUND: Prominent ear is the most common congenital defect of the ear, with an incidence of 5% in Caucasians. Surgical treatment should correct the auriculocephalic and conchoscaphal angles as well as protrusion of the lobe when present. This paper aims to report the experience of our service in the treatment of prominent ears with a combination of several available techniques. METHODS: Forty-seven patients operated with a combination of previously described techniques were evaluated, and cartilaginous incision, Mustardé sutures for antihelix definition, and concha-mastoid fixation were performed. Patients less than 15 years of age were operated under general and local anesthesia, while the remaining patients underwent only local anesthesia. All patients were reassessed on the first postoperative day. RESULTS: The postoperative results were considered satisfactory by both patients and surgical staff, with no stigma development in the operated ear. CONCLUSIONS: The best treatment of prominent ears is achieved by a combination of techniques. The approach used on the studied patients has produced natural-looking results with low complication rates, satisfying the surgical staff and, most importantly, the patients.

RESUMO

INTRODUÇÃO: A orelha de abano é o mais comum de todos os defeitos congênitos da orelha, com incidência de 5% em caucasianos. O tratamento cirúrgico deve corrigir os ângulos auriculocefálico e escafoconchal, bem como a protrusão do lóbulo, quando presente. O objetivo deste trabalho é demonstrar a experiência de nosso serviço no tratamento da orelha de abano com a combinação de diversas técnicas disponíveis. MÉTODO: Foram avaliados 47 pacientes, operados com a associação de técnicas já descritas anteriormente, sendo utilizados incisão cartilaginosa, pontos de Mustardé para definição de antélice e fixação da concha na mastoide. Os pacientes com menos de 15 anos de idade foram operados sob anestesias geral e local, e os demais foram submetidos somente a anestesia local. Todos os pacientes foram reavaliados no primeiro dia de pós-operatório. RESULTADOS: Os resultados pós-operatórios foram considerados satisfatórios pelos pacientes e pela equipe cirúrgica, sem o aparecimento do estigma de orelha operada. CONCLUSÕES: O melhor tratamento de orelhas proeminentes é obtido com a associação de diversas técnicas. A abordagem empregada nos pacientes avaliados tem apresentado resultados naturais e com baixos índices de complicação, satisfazendo a equipe cirúrgica e, principalmente, os pacientes.

Retrospective analysis of the approach to nasal fractures at Unicamp Clinical Hospital

ABSTRACT

INTRODUCTION: Nasal fractures are common injuries, frequently considered to be minor. Nevertheless, they may cause significant damage, from both a functional and aesthetic perspective. METHODS: Medical records of 144 patients admitted for nasal fracture treatment between February 2002 and October 2008 to the Plastic Surgery Area of the Unicamp Clinical Hospital were analyzed. RESULTS: Patients were predominantly male (75.7%), with a male to female ratio of 3.1:1. The most number of cases was observed in 21 to 30 year olds, and the majority of fractures (31.8%) were caused by physical aggression. Closed reductions were performed on all patients under local and topical anesthesia after an average of 8 days. Among the studied patients, 31 experienced residual deformity. CONCLUSIONS: Patients typically affected by nasal fractures are young male adults. Closed reduction surgical treatment yields acceptable results, with correct injury evaluation and treatment timing.

RESUMO

INTRODUÇÃO: As fraturas nasais são lesões de grande incidência, frequentemente consideradas de menor importância. No entanto, podem trazer prejuízos importantes do ponto de vista tanto funcional como estético. MÉTODO: Foram analisados prontuários médicos de 144 pacientes submetidos a tratamento de fratura nasal na Área de Cirurgia Plástica do Hospital de Clínicas da Unicamp, no período de fevereiro de 2002 a outubro de 2008. RESULTADOS: Observou-se predomínio de pacientes do gênero masculino (75,7%), com proporção entre gêneros masculino e feminino de 3,1:1. A faixa etária mais acometida foi aquela entre 21 anos e 30 anos de idade. A maioria das fraturas (31,8%) foi causada por agressão física. Todos os pacientes foram tratados por meio de redução fechada, sob anestesia local e tópica, após um período médio de 8 dias. Entre os pacientes analisados, 13 evoluíram com deformidade residual. CONCLUSÕES: O paciente tipicamente afetado pelas fraturas nasais é o indivíduo adulto jovem e do gênero masculino. O tratamento cirúrgico por meio de redução fechada promove resultados aceitáveis, se respeitados os princípios de avaliação correta da lesão e do momento de indicação do tratamento.

Surgical treatment of retracted columella in primary noses

ABSTRACT

BACKGROUND: Retracted columella may be present in primary noses as well as in patients who have undergone aesthetic or functional rhinoplasty. This study aims to demonstrate the use of the labial portion of the depressor septi nasi (the depressive muscle of the nasal tip) and upper lip orbicularis muscle for retracted columella correction. METHODS: The subjects included 20 patients between 20 and 35 years old (4 males and 16 females) who underwent primary aesthetic rhinoplasty. RESULTS: Moderate edema was observed postoperatively in all patients and was resolved within 1 to 3 months. V-shaped scars at the base of the columella presented excellent aesthetic aspects. CONCLUSIONS: The surgical technique is easy and the learning curve is fast. Columella correction with the presented technique provides natural and predictable results.

RESUMO

INTRODUÇÃO: A columela retraída pode estar presente em narizes primários e em pacientes já submetidos a rinoplastias estéticas e/ou funcionais. O objetivo deste trabalho é demonstrar a utilização do músculo depressor do septo nasal (músculo depressor da ponta nasal) em sua porção labial e do músculo orbicular do lábio superior na correção da columela retraída. MÉTODO: No total, 20 pacientes com idades variando entre 20 anos e 35 anos, sendo 4 do sexo masculino e 16 do sexo feminino, foram submetidos a rinoplastia estética primária. RESULTADOS: Presença de edema moderado pós-operatório foi observada em todos os casos, com resolução dentro de 1 mês a 3 meses. As cicatrizes em "V" na base da columela apresentaram excelente aspecto estético. CONCLUSÕES: A técnica cirúrgica é de fácil execução e apresenta curva de aprendizado rápida. A correção da columela com a técnica apresentada proporcionou resultados naturais e previsíveis.

Assessment of long-term patient satisfaction after rhinoplasty performed at the Plastic Surgery Department of Santa Casa de Misericórdia de Porto Alegre

ABSTRACT

BACKGROUND: There are currently few studies assessing patient satisfaction after rhinoplasty or other aesthetic procedures. This is a retrospective study of patient satisfaction after rhinoplasty performed at the Plastic Surgery Department of Santa Casa de Misericórdia de Porto Alegre. METHODS: All (n = 119) patients who underwent rhinoplasty at the Plastic Surgery Department of Santa Casa de Misericórdia de Porto Alegre between 2007 and 2009 were requested to fill out a validated questionnaire to assess the degree of postoperative satisfaction. RESULTS: Sixty (50.42%) patients were included in the study. The global satisfaction index with respect to the aesthetic and functional results showed that 85.48% of the patients were satisfied; 88.3% of patients were pleased with the aesthetic results, and 83.3% reported satisfactory functional results and could breathe easily. Regarding the impression of the patients' family and friends, 93.3% reported that they were completely pleased; 93.3% reported that the postoperative nasal form rarely or never limited social or professional activities; 75% of the patients were completely confident that nasal appearance was the best possible, while 76.7% reported that their nasal appearance could not be changed by another surgery and 83.4% did not believe that nasal function could be changed with another surgery. CONCLUSIONS: Rhinoplasty performed at the Plastic Surgery Department of Santa Casa de Misericórdia de Porto Alegre resulted in a high level of patient satisfaction.

INTRODUÇÃO: A rinoplastia, assim como diversos procedimentos estéticos, apresenta parcos estudos objetivos quanto à satisfação dos pacientes. O objetivo do presente estudo é avaliar o grau de satisfação a longo prazo dos pacientes submetidos a rinoplastia na Santa Casa de Misericórdia de Porto Alegre, em um estudo retrospectivo, utilizando questionário validado. MÉTODO: Foram selecionados todos (n = 119) os pacientes submetidos a rinoplastia no Serviço de Cirurgia Plástica da Santa Casa de Misericórdia de Porto Alegre, nos anos de 2007, 2008 e 2009, para aplicação de um questionário validado, com o objetivo de avaliar o grau de satisfação pós-operatória. RESULTADOS: Foram contatados 60 (50,42%) pacientes no estudo. Quanto à satisfação global com o resultado estético-funcional da rinoplastia, 85,48% dos pacientes afirmaram estar satisfeitos. Estratificando esse valor, foi observado que 88,3% dos pacientes estavam muito ou completamente satisfeitos com o aspecto estético pós-operatório do nariz; quanto à função nasal pós-operatória, 83,3% reportaram estar muito ou completamente aptos a respirar pelo nariz; quanto à impressão de amigos e familiares sobre o aspecto final do nariz, 93,3% afirmaram estar muito ou completamente satisfeitos com o resultado; 93,3% afirmaram que raramente ou nunca tiveram limitação das atividades sociais ou profissionais em decorrência do aspecto nasal pós-operatório; 75% afirmaram estar muito ou completamente confiantes de que a aparência de seu nariz era a melhor possível de ser obtida; 76,7% afirmaram que definitivamente não gostariam de alterar a aparência do nariz com outra cirurgia; e 83,4% afirmaram que definitivamente não gostariam de alterar a função nasal com outra cirurgia. CONCLUSÕES: Constatou-se que a rinoplastia realizada no Serviço de Cirurgia Plástica da Santa Casa de Misericórdia de Porto Alegre apresentou alto índice de satisfação dos pacientes.

Evaluation of craniofacial growth in patients with cleft lip and palate undergoing one-stage palate repair

ABSTRACT

BACKGROUND: Primary surgeries such as cheiloplasty and palatoplasty interfere with the morphology and physiology of the maxillofacial complex, causing alterations in its growth and development in unilateral cleft lip and palate (UCLP) patients. The aim of the present study was to perform a preliminary analysis of the effects of surgery on maxillofacial growth through an examination of dental arch relationships. METHODS: Forty-five patients with UCLP who underwent primary surgery for the repair of cleft lip and palate were evaluated. Comparative analysis of plaster models of dental arches was performed by two orthodontists using the Atack index. RESULTS: Some patients (44.4%) analyzed showed scores of 1 and 2, representing the most favorable maxillofacial growth conditions. The intermediate score (score 3) was found in 40% of patients, while 15.6% showed unfavorable maxillary growth tendencies (scores 4 and 5). The mean score at 4 years of age was 2.62 + 0.98. A correlation between scores 1 and 2 in the present study with those of previous studies resulted in a significant difference (P = 0.23) in comparison to Bongaarts' series, which obtained the best results. Our results for scores 3, 4, and 5 were similar to those of 3 related studies in terms of the percentages obtained, which were better than those of the first study that used the Atack index for the evaluation of primary surgeries. CONCLUSIONS: The Atack index enabled the analysis of the effects of primary surgery on maxillofacial growth and a comparison with the results obtained by other centers for the treatment of UCLP.

RESUMO

INTRODUÇÃO: As cirurgias primárias, queiloplastia e palatoplastia, interferem na morfologia e na fisiologia do complexo maxilofacial, provocando alterações em seu crescimento nos portadores de fissura transforame incisivo unilateral (FTIU). Este estudo tem por objetivo avaliar precocemente os efeitos das cirurgias sobre o crescimento maxilofacial, por meio da relação dos arcos dentários desses pacientes. MÉTODO: Foram avaliados 45 pacientes portadores de FTIU submetidos a cirurgias primárias de lábio e palato. Foi realizado estudo comparativo de modelos dos arcos dentários decíduos em que a relação maxilomandibular foi avaliada por 2 ortodontistas, que aplicaram o índice Atack. RESULTADOS: Dentre os pacientes analisados, 44,4% se encontravam nos escores 1 e 2, apresentando condições mais favoráveis de crescimento maxilomandibular. O escore intermediário (escore 3) correspondeu a 40% da amostra e os escores 4 e 5, a 15,6%, apresentando tendência a crescimento desfavorável. A média obtida aos 4 anos de idade foi de 2,62 + 0,98. Ao relacionar os escores 1 e 2 com outros estudos, houve diferença significativa (P = 0,023) comparativamente à série de Bongaarts, que apresentou os melhores resultados. Nos resultados para os escores 3, 4 e 5, observaram-se proporções semelhantes às de três dos estudos relacionados e melhores em relação ao primeiro a utilizar o índice Atack na avaliação de suas cirurgias primárias. CONCLUSÕES: A aplicação do índice Atack possibilitou a análise dos resultados de cirurgias primárias sobre o crescimento maxilofacial e a comparação destes com os dados obtidos por centros de referência para tratamento da FTIU.

Comparative study among cleft patients with velopharyngeal insufficiency treated with speech therapy and pharyngoplasty

ABSTRACT

BACKGROUND: Inadequacy of speech and voice due to velopharyngeal insufficiency is a major stigma for cleft patients. Therefore, the study of this clinical condition is essential to improve the development and social relations of children with this ailment. This study aimed to assess alterations in the speech, velopharyngeal sphincter, and larynx of patients with cleft palate and cleft lip and palate who underwent to lip closure, palatoplasty, and speech therapy and developed transitory velopharyngeal insufficiency. Furthermore, these patients were compared with cleft palate and cleft lip and palate patients who developed persistent velopharyngeal insufficiency treated with lip closure, palatoplasty, speech therapy, pharyngoplasty, and complementary speech therapy. METHODS: From June 1997 to May 2002, 132 cleft palate and cleft lip and palate patients aged between 4 years 11 months and 19 years 3 months with transitory velopharyngeal insufficiency and persistent velopharyngeal insufficiency were assessed. After applying inclusion and exclusion criteria, 44 patients, 18 females and 26 males, were divided into 2 groups: group I, 20 patients who underwent lip closure, palatoplasty and speech therapy; and group II, 24 patients who underwent lip closure, palatoplasty, speech therapy, pharyngoplasty, and complementary speech therapy. Speech therapy consisted of articulatory therapy of oral airflow, myofunctional therapy, rapid phonemic acquisition technique, and voice therapy. Surgical treatment consisted of producing a flap from the pharynx posterior wall of the upper pedicle. RESULTS: Alterations in speech, the velopharyngeal sphincter, and the larynx were more frequent in group I than in group II. CONCLUSIONS: Cleft patients with persistent velopharyngeal insufficiency should be treated with pharyngoplasty and complementary speech therapy in order to correct alterations in speech, the velopharyngeal sphincter, and the larynx.

INTRODUÇÃO: A inadequação da fala e da voz decorrente da insuficiência velofaríngea é o principal estigma do paciente fissurado; assim, o estudo dessa condição clínica é fundamental, proporcionando melhor desenvolvimento das crianças e de suas relações sociais. O objetivo do presente estudo é avaliar alterações fonoaudiológicas, do esfíncter velofaríngeo, da laringe de pacientes fissurados palatais e labiopalatais tratados com queiloplastia, palatoplastia e fonoterapia, que desenvolveram insuficiência velofaríngea transitória, e fissurados palatais e labiopalatais, que desenvolveram insuficiência velofaríngea persistente tratados com queiloplastia, palatoplastia, fonoterapia, faringoplastia e fonoterapia complementar. MÉTODO: No período de junho de 1997 a maio de 2002, foram avaliados 132 fissurados palatais e labiopalatais que desenvolveram insuficiência velofaríngea transitória e insuficiência velofaríngea persistente, com idade entre 4 anos e 11 meses e 19 anos e 3 meses. Observando-se os critérios de inclusão e exclusão, 44 pacientes, sendo 18 do gênero feminino e 26 do gênero masculino, foram divididos em 2 grupos: grupo I, 20 pacientes submetidos a queiloplastia, palatoplastia e fonoterapia; e grupo II, 24 pacientes submetidos a queiloplastia, palatoplastia, fonoterapia, faringoplastia e fonoterapia complementar. O tratamento fonoaudiológico consistiu de terapia articulatória de fluxo aéreo bucal, terapia miofuncional, técnica de aquisição fonêmica rápida e terapia de voz. O tratamento cirúrgico consistiu de confecção de retalho da parede posterior da faringe, de pedículo superior. RESULTADOS: As alterações fonoaudiológicas, do esfíncter velofaríngeo e da laringe foram mais frequentes nos pacientes do grupo I, quando comparados aos do grupo II. CONCLUSÕES: Pacientes fissurados que evoluíram para insuficiência velofaríngea persistente devem ser tratados com faringoplastia e fonoterapia complementar para correção das alterações fonoaudiológicas, do esfíncter velofaríngeo e da laringe.

Quality of life among children with cleft lips and palates: a critical review of measurement instruments

BACKGROUND: Multidisciplinary teams that treat patients with cleft lips and palates seek to promote quality of life (QoL) in this population. In this study, we aim to identify instruments in the literature that can be used to assess comprehensive aspects of QoL (related to functionality, aesthetics, and outcomes) for this population. METHODS: We searched literature on PubMed published between 2001 and 2011 for specific instruments used to evaluate QoL among cleft lip and palate patients using the following keywords: quality of life & oral cleft, quality of life & craniofacial deformities, quality of life & oral cleft speech, voice related quality of life. RESULTS: We identified 457 papers related to the subject and evaluated the following specific pediatric instruments for assessing QoL among cleft lip and palate patients: Quality of Life Instrument - Craniofacial Surgery (YQOL-CS), Youth Quality of Life Instrument-Facial Differences (YQOL-FD), Child Oral Health Quality of Life Questionnaire (COHQOL), Child Oral Health Impact Profile (COHIP), Pediatric Voice Outcome Surgery (PVOS) and Pediatric Voice-Related Quality of Life Survey (PVRQOL). CONCLUSIONS: No pediatric QoL instrument exists in the current literature that comprehensively evaluates children with cleft lips and palates, including aspects related to aesthetics, conceptual and perceptual consequences, and functionality (chewing and respiratory systems, and vocal resonance) associated with this important congenital craniofacial anomaly.

RESUMO

INTRODUÇÃO: A equipe multidisciplinar que trata crianças portadoras de fissura labiopalatina busca promover qualidade de vida (QV) a essa população. O objetivo do presente trabalho foi identificar, na literatura, instrumentos que possibilitem a avaliação global da QV dessa população, envolvendo aspectos estéticos, funcionais e resposta do indivíduo ao tratamento. MÉTODO: Foi realizada revisão da literatura disponível no PubMed, no período de 2001 a 2011, em busca de instrumentos específicos para avaliação de QV em crianças portadoras de fissura labiopalatina, com as seguintes palavras-chave (em língua inglesa): quality of life & oral cleft, quality of life & craniofacial deformities, quality of life & oral cleft speech, voice related quality of life. RESULTADOS: Foram identificados 457 artigos relacionados ao tema. Foram avaliados os seguintes instrumentos específicos para a faixa etária pediátrica utilizados na mensuração da QV de pacientes com fissura labiopalatina: Quality of Life Instrument - Craniofacial Surgery (YQOL-CS), Youth Quality of Life Instrument-Facial Differences (YQOL-FD), Child Oral Health Quality of Life Questionnaire (COHQOL), Child Oral Health Impact Profile (COHIP), Pediatric Voice Outcome Surgery (PVOS) e Pediatric Voice-Related Quality of Life Survey (PVRQOL). CONCLUSÕES: Não se identificou, na literatura, um instrumento que avaliasse integralmente as crianças com fissura labiopalatina, no que se refere aos aspectos estéticos, às consequências conceituais e perceptuais, bem como a todos os aspectos funcionais (aparelhos mastigatório e respiratório e ressonância vocal) que envolvem essa importante anomalia craniofacial congênita.

Mandible fracture: epidemiological study of 70 cases

ABSTRACT

BACKGROUND: Mandible fractures are most often caused by direct trauma, but occasionally, pathologic fractures may occur due to tumor lesions. The surgical approach is determined by the type and the location of the mandible fracture. This study aimed to evaluate the epidemiological profile of 70 patients who suffered mandible fracture and underwent surgery at Instituto Dr. José Frota (Fortaleza, CE) between January 2005 and December 2009. METHODS: We analyzed medical records of 70 patients who underwent surgical treatment for mandibular fractures and assessed gender, age, skin color, anatomic site of fracture, etiology, associated fractures, treatment, hospitalization and complications. RESULTS: There was male predominance (90%) among patients, with a mean patient age of 28.25 + 11.04 years. Most patients (80%) were Caucasian and from small towns (68%). The most affected jaw sites, in descending order, were the parasymphysis region, body, and angle. The major cause was traffic accidents (70%), with motorcycle accidents being the most prevalent (55.7%). The main associated facial fractures were maxilla fracture type Le Fort and zygomatic-orbital. Surgical treatment was carried out with rigid internal fixation in most patients (75%). The only complication was infection in one patient. CONCLUSIONS: Reduction and fixation of mandibular fractures should occur as accurately and quickly as possible, since malocclusion is an important long-term complication. In our study, we did not observe such a complication; there was only one case of infection. Concurrent with the findings of other studies, there was a prevalence of young adult males among patients, and traffic accidents were the main cause. The fractures were frequently located in the parasymphysis region and were corrected through rigid internal fixation in 75% of cases.

INTRODUÇÃO: As fraturas de mandíbula são frequentemente causadas por traumatismo direto, mas, eventualmente, podem surgir fraturas patológicas, em função de lesões tumorais. A abordagem cirúrgica é determinada pelo tipo e pela localização da fratura na mandíbula. O objetivo deste estudo foi avaliar o perfil epidemiológico de 70 pacientes que sofreram fratura de mandíbula e foram operados no Instituto Dr. José Frota (Fortaleza, CE) no período de janeiro de 2005 a dezembro de 2009. MÉTODO: Foram analisados prontuários de 70 pacientes submetidos a cirurgia para correção de fratura de mandíbula, avaliando-se sexo, idade, cor, procedência, sítio anatômico da fratura, etiologia, fraturas associadas, tratamento, tempo de internação e complicações. RESULTADOS: Houve predomínio do sexo masculino (90%), com média de idade de 28,25 + 11,04 anos. A maioria dos pacientes (80%) era de cor branca e procedente do interior (68%). Os locais da mandíbula mais acometidos foram a região da parassínfise, o corpo e o ângulo, tendo como principal causa os acidentes de trânsito (70%), sendo os acidentes motociclísticos (55,7%) mais prevalentes. As principais fraturas de face associadas foram de maxila do tipo Le Fort e zigomático-orbitário. O tratamento cirúrgico foi feito com fixação interna rígida na maioria dos pacientes (75%). A única complicação foi infecção, observada em um paciente. CONCLUSÕES: A redução e a fixação das fraturas mandibulares devem ocorrer tão precisa e rapidamente quanto possível, visto que a maloclusão é uma complicação grave a longo prazo. Neste estudo, essa complicação não foi observada, havendo apenas um caso de infecção. Coincidindo com os achados de outros estudos, houve prevalência de adultos jovens do sexo masculino e de acidentes de trânsito como etiologia. As fraturas foram localizadas preferencialmente na região da parassínfise e corrigidas por meio de fixação interna rígida em 75% dos casos.

ABSTRACT

BACKGROUND: Nowadays, more and more immediate breast reconstructions are carried out after cancer treatment. For performing radical mastectomy, there are several recognized techniques. In quadrantectomy, the options for defect repair range from primary closure to the use of oncoplastic surgery techniques. In the case of external quadrant tumors, there is no option of rotation of the entire latissimus dorsi muscle, which usually exceeds the actions necessary for repairing lost volume. A new option for post-quadrantectomy reconstruction in external quadrants is proposed in this study. METHODS: The surgical technique consists of dissection of the lateral segment of the latissimus dorsi muscle by an extension of the incision for lesion removal in the same supine position, followed by rotation and modeling of this segment to repair the defect caused by cancer removal. Twenty-one women aged 33 to 69 years underwent quadrantectomies of external quadrants followed by reconstruction with the technique described in this paper. To evaluate the method, each result had a score attributed to volume, shape, symmetry, and changes in the nipple areolar complex, as well as scar and operated quadrant surface. RESULTS: The obtained results of volume and shape were superior in relation to those of symmetry. The average grade was 7.048, equivalent to B concept. CONCLUSIONS: The new technique decreased surgical time. The acquisition of the B concept demonstrates the method's feasibility.

RESUMO

INTRODUÇÃO: Cada vez mais são realizadas reconstruções imediatas da mama pós-tratamento do câncer. Para as mastectomias radicais, há várias técnicas consagradas. Na quadrantectomia, as opções de reparação do defeito vão do fechamento primário à utilização de técnicas de cirurgia oncoplástica. Para os tumores dos quadrantes externos, a opção de rotação de todo o músculo latíssimo do dorso é desnecessária, uma vez que usualmente excede ao necessário à reparação do volume perdido. Neste trabalho é proposta uma nova opção para a reconstrução pós-quadrantectomia em quadrantes externos. MÉTODO: A técnica cirúrgica consiste na dissecção do segmento lateral do músculo latíssimo do dorso pelo prolongamento da incisão para a retirada da lesão, no mesmo decúbito dorsal, seguida de sua rotação e modelagem para reparar o defeito decorrente da extirpação do câncer. No total, 21 mulheres com idade entre 33 anos e 69 anos foram submetidas a quadrantectomias de quadrantes externos, seguidas da reconstrução com a técnica descrita neste trabalho. Para a avaliação do método, cada resultado teve atribuído um escore a volume, forma, simetria e alterações do complexo areolopapilar, da cicatriz e da superfície do quadrante operado. RESULTADOS: Os resultados obtidos na avaliação do volume e da forma foram superiores em relação aos da simetria. A nota média foi 7,048, equivalente ao conceito B. CONCLUSÕES: A nova técnica propiciou diminuição do tempo cirúrgico. A aquisição do conceito B demonstra a viabilidade do método.

Breast reconstruction using latissimus dorsi muscle flap: a novel surgical technique with reduced scarring

ABSTRACT

BACKGROUND: The latissimus dorsi muscle flap is one of the most commonly used surgical options for breast reconstruction. The donor area scar on the back, although sometimes hidden beneath the bra strap, is often extensive and enlarged or hypertrophic. The aim of this study is to describe the dissection and rotation of the latissimus dorsi muscle using 2 reduced vertical incisions. METHODS: Twelve female patients underwent unilateral breast reconstruction with latissimus dorsi muscle flap. Ten patients had Poland's syndrome, and their breast reconstruction was performed with a tissue expander at the first instance, with subsequent latissimus dorsi muscle flap rotation to cover the silicone prosthesis. Two patients experienced volume decrease, irregularities, and breast retractions after adenomastectomy; their prosthesis were also covered by a latissimus dorsi muscle flap. The surgical procedure consisted of a 5-cm longitudinal incision on the lateral border of the latissimus dorsi muscle and another on the inframammary fold. Dissection, section, and rotation of the muscle were performed through skin retraction. RESULTS: The final extension of the scar at follow-up averaged 6.2 cm. The mean surgical time for flap rotation and synthesis of the donor area was 80 minutes. No postoperative complications were observed in any of the cases. CONCLUSIONS: The reduced incision for dissection of the latissimus dorsi flap for breast reconstruction proved to be feasible and safe; this method provides aesthetic advantages without increasing cost or surgical time.

RESUMO

INTRODUÇÃO: O retalho de músculo grande dorsal (MGD) é uma das opções cirúrgicas mais usadas nas reconstruções mamárias. A cicatriz da área doadora no dorso, embora por vezes situada sob a alça do sutiã, frequentemente é extensa e pode ser de qualidade ruim (alargada ou hipertrófica). O objetivo deste trabalho é descrever a dissecção e rotação do MGD através de duas incisões reduzidas. MÉTODO: No total, 12 pacientes do sexo feminino foram submetidas a reconstrução mamária unilateral com retalho de MGD isolado (sem pele). Dessas pacientes, 10 eram portadoras de síndrome de Poland e tiveram suas mamas reconstruídas com expansor tecidual num primeiro tempo, com posterior rotação do retalho de MGD para cobrir a prótese de silicone definitiva; e outras 2 pacientes tinham evoluído com diminuição do volume, irregularidades e retrações da mama após adenomastectomia, e tiveram sua prótese coberta pelo MGD. O procedimento cirúrgico consistiu de incisão longitudinal de 5 cm sobre a borda lateral do MGD e no sulco inframamário. Realizou-se dissecção, secção e rotação do músculo somente com a retração da pele. RESULTADOS: Em seguimento ambulatorial, a extensão da cicatriz apresentou média de 6,2 cm. O tempo cirúrgico médio para rotação do retalho e síntese da área doadora foi de 80 minutos. Não foram observadas complicações pós-operatórias nesta série. CONCLUSÕES: A incisão reduzida para dissecção do MGD nas reconstruções mamárias mostrou-se bastante factível e segura, com vantagem estética e sem aumento dos custos e do tempo cirúrgico.

ABSTRACT

BACKGROUND: The correction of breast ptosis associated with skin sagging and low projection is still a subject of controversy in the literature. This study aims to describe the experience of the Plastic Surgery Department of Universidade Estadual de Campinas (Unicamp) with the technique of mastopexy with breast implants in a double plane and the pectoralis major muscle flap. METHODS: A retrospective study of 20 patients with grade II or III mammary ptosis, who underwent surgery between June 2008 and September 2010, was performed. RESULTS: A 9- and 12-month follow-up of patients showed neither breast or chest deformities nor recurrence of ptosis. All patients presented with good breast projection and adequate upper pole fill, with long-lasting and satisfactory results. CONCLUSIONS: Mastopexy with breast implants and the pectoralis major muscle flap technique is easy to perform, with a relatively short learning curve, good reproducibility, and satisfactory long-lasting results.

RESUMO

INTRODUÇÃO: A correção da ptose mamária associada à flacidez de pele e baixa projeção é ainda tema de discussões e controvérsias na literatura. O objetivo deste estudo é descrever a experiência da Disciplina de Cirurgia Plástica da Universidade Estadual de Campinas (Unicamp) com a técnica de mastopexia com implante mamário associado a retalho de sustentação do músculo peitoral maior. MÉTODO: Foi realizado estudo retrospectivo de 20 pacientes com ptose mamária graus II ou III, operadas no período de junho de 2008 a setembro de 2010. RESULTADOS: Após seguimento pós-operatório entre 9 meses e 12 meses, não foram observados casos de deformidades mamárias ou torácicas e nenhuma paciente evoluiu com recidiva da ptose. Foram observados, em todas as pacientes, manutenção de boa projeção da mama e adequado preenchimento do polo superior mamário, gerando resultados duradouros e satisfatórios. CONCLUSÕES: A técnica de mastopexia com uso de implantes associados a retalho de músculo peitoral maior é de fácil realização, com curva de aprendizado relativamente curta, boa reprodutibilidade, e resultados duradouros e satisfatórios.

Comparative analysis of mammaplasty techniques based on the long-term effect on the nipple-areolar-complex to inframammary crease distance

ABSTRACT

BACKGROUND: Breast cosmetic surgery has focused on the correction of breast volume, suspension, and shape. In most of the breast reduction and mastopexy techniques currently in use, the vascular pedicles responsible for the blood supply to the nipple areolar complex (NAC) are of particular importance. The objective of the present study was to evaluate the results obtained with different techniques and pedicle choices with regard to the maintenance of lower breast pole shape during a postoperative 6-month follow-up period. METHODS: A total of 20 female patients (40 breasts) with a body mass index (BMI) ranging from 22 to 25 and ages between 21 and 54 years who underwent breast lift and reduction surgery at the Plastic Surgery and Burns Department of Hospital do Servidor Público Estadual de São Paulo were retrospectively analyzed. The superior medial pedicle and Liacyr Ribeiro type I pedicle techniques for mammary reduction were used. The distance between the vertical scar of the NAC and the inframammary crease was measured in the immediate postoperative period and after 6 months to compare the effect of each pedicle on vertical distance maintenance. Statistical analyses were performed by Student's t test, and P values < 0.005 were considered statistically significant. RESULTS: The pedicles used were superior (n = 16), medial (n = 20), and Liacyr Ribeiro type I (n = 4). The vertical scar (NAC-inframammary crease) measured between 5 cm and 6 cm (mean = 5.6 cm) during the immediate postoperative period, while later postoperative measurements ranged between 5.5 cm and 8 cm (mean = 6.6 cm). The differences in the NAC-inframammary crease distance between the Liacyr Ribeiro type I pedicle (P = 0.2048), superior pedicle (P = 0.0012), and medial pedicle (P = 0.0057) based surgeries only reached statistical significance in the superior pedicle surgery with respect to an increase in this measurement. CONCLUSIONS: Measurement of the vertical distance between the most caudal portion of the NAC and the inframammary crease scar is simple and might contribute to our understanding of the effect of different pedicle techniques on the long-term results of mammaplasty.

INTRODUÇÃO: A cirurgia plástica mamária tem como foco adequação do volume, suspensão e forma da mama. Diversas técnicas são utilizadas para redução e mastopexia, com grande atenção ao pedículo responsável pelo suprimento vascular do complexo areolopapilar (CAP). O objetivo deste trabalho é avaliar a técnica e a escolha do pedículo na manutenção da forma do polo inferior da mama no seguimento pós-operatório de 6 meses. MÉTODO: Foi realizada análise prospectiva de 20 pacientes do sexo feminino, totalizando 40 mamas, com índice de massa corporal (IMC) entre 22 e 25 e idade variando de 21 anos a 54 anos, submetidas a mamoplastia (redutora e mastopexia) no Serviço de Cirurgia Plástica e Queimaduras do Hospital do Servidor Público Estadual de São Paulo. Foram empregadas técnicas baseadas em pedículos superior e medial e pedículo tipo I de Liacyr Ribeiro. A distância entre a cicatriz vertical do CAP e o sulco inframamário foi mensurada no pós-operatório imediato e depois de 6 meses, a fim de comparar o efeito de cada pedículo na manutenção da distância vertical. A análise estatística foi feita pelo teste t de Student, considerando significativos valores de P < 0,005. RESULTADOS: Os pedículos utilizados foram superior (n = 16), medial (n = 20) e tipo I de Liacyr Ribeiro (n = 4). A distância da cicatriz vertical (CAP-sulco inframamário), no pós-operatório imediato, variou de 5 cm a 6 cm, com média de 5,6 cm. As medidas pós-operatórias tardias variaram entre 5,5 cm e 8 cm, com média de 6,6 cm. No que se refere às variações na medida da distância CAP-sulco inframamário quando relacionadas às técnicas de pedículo tipo I de Liacyr Ribeiro (P = 0,2048), de pedículo superior (P = 0,0012) e de pedículo medial (P = 0,0057), apenas o pedículo superior apresentou significância quanto ao alargamento dessa distância. CONCLUSÕES: A medida da distância vertical entre a porção mais caudal do CAP e a cicatriz horizontal posicionada no sulco inframamário (CAP-sulco inframamário) pode ser obtida de forma simples, a fim de entender a influência de cada pedículo no resultado tardio das mamoplastias.

Transareolopapilar incision for breast augmentation: 10-year experience at the Ivo Pitanguy Institute

ABSTRACT

BACKGROUND: There are numerous access routes for inserting implants during breast augmentation surgery. In 1966, Pitanguy described the transareolopapilar route. The aim of this study was to assess the use of transareolopapilar incision during breast augmentation surgery at the Ivo Pitanguy Institute (Rio de Janeiro, RJ, Brazil), over the past 10 years. METHODS: Retrospective analyses of the size of the implants used, indications for transareolopapilar incision, postoperative complications, and postoperative scarring were performed. RESULTS: Fifty-three patients with a mean age of 33.54 years were included, and the mean follow-up period was 11.6 months. Most (60.4%) of the implants were <200 ml. Twelve patients required a second operation due to a breast lump (1 case); infection (1 case); capsular contracture (1 case); and dissatisfaction with breast shape (4 cases), volume (4 cases), and unilateral scarring (1 case). Sixteen (30.2%) patients developed some form of minor postoperative complication; 13 (24.5%) had one or more scarring issues, including hypochromia (18.9%), hypertrophy (1.9%), scar retraction (1.9%), and areola bifida (1.9%). Twenty (37.7%) patients underwent postoperative follow-up for more than one year and were satisfied with the postoperative scar. CONCLUSIONS: The transareolopapilar incision facilitates the insertion of small-to-moderate size implants with a low rate of postoperative complications and a low incidence of scarring, provided the correct surgical technique is used.

RESUMO

INTRODUÇÃO: Várias vias de acesso foram criadas para a inclusão de implantes na cirurgia de aumento das mamas. Em 1966, Pitanguy descreveu a via de acesso transareolopapilar. O objetivo do presente estudo é avaliar as mamoplastias de aumento realizadas no Instituto Ivo Pitanguy (Rio de Janeiro, RJ, Brasil), nas quais se utilizou a incisão transareolopapilar, nos últimos 10 anos. MÉTODO: Realizado estudo retrospectivo, analisando-se os seguintes parâmetros: tamanho dos implantes, indicação da incisão transareolopapilar e complicações pós-operatórias, como alterações cicatriciais. RESULTADOS: Foram incluídas no estudo 53 pacientes, com média de idade de 33,54 anos e tempo médio de seguimento de 11,6 meses. A maioria (60,4%) dos implantes possuía menos de 200 ml. Doze pacientes foram submetidas a reintervenções pelas seguintes razões: nódulo mamário (1 caso), infecção (1 caso), contratura capsular (1 caso), e insatisfação com a forma das mamas (4 casos), com o volume (4 casos) e com a cicatriz unilateral (1 caso). Dezesseis (30,2%) pacientes desenvolveram alguma complicação menor no pós-operatório e 13 (24,5%) apresentaram alguma alteração cicatricial no pós-operatório: hipocromia (18,9%), hipertrofia unilateral (1,9%), retração cicatricial unilateral (1,9%), e aréola bífida (1,9%). Vinte (37,7%) pacientes realizaram seguimento pós-operatório superior a um ano e relataram satisfação com a cicatriz. CONCLUSÕES: A incisão transareolopapilar permite a inclusão de implantes de tamanho pequeno a moderado, com baixo índice de complicações pós-operatórias e cicatriciais, desde que seguida a correta técnica cirúrgica.

Abdominoplasty after gastroplasty: evaluation of 100 cases operated on by the "fish" technique

ABSTRACT

BACKGROUND: Morbid obesity is a worldwide endemic disease. Its growing incidence makes it a public health problem. Bariatric surgery is an alternative weight loss method with the objective of controlling obesity-associated diseases and improving quality of life. The surgical treatment of the deformities resulting from significant weight loss is a new challenge for plastic surgeons. This paper presents a new technique for abdominoplasty in formerly obese patients with a midline abdominal scar resulting from gastroplasty. METHODS: A total of 28 men and 72 women who underwent abdominoplasty after gastroplasty by the "fish" technique described in this study were analyzed. RESULTS: The results obtained with the technique presented were satisfactory, resulting in a new body contour and a low complication rate. CONCLUSIONS: Abdominoplasty with markings forming a fish shape is successful for the removal of excess abdominal skin in patients who underwent bariatric surgery.

RESUMO

INTRODUÇÃO: A obesidade classe III é uma doença endêmica em todo o mundo. Trata-se de problema de saúde pública em decorrência de sua incidência progressiva. A cirurgia bariátrica oferece alternativa de redução ponderal, visando ao melhor controle das doenças que têm a obesidade como fator de risco e à melhoria da qualidade de vida. O tratamento cirúrgico das deformidades secundárias a grandes emagrecimentos é um novo desafio ao cirurgião plástico. O objetivo deste trabalho é apresentar uma nova técnica para abdominoplastia do paciente ex-obeso, portador de cicatriz abdominal mediana decorrente de gastroplastia. MÉTODO: Foram analisados 28 homens e 72 mulheres, submetidos a dermolipectomia abdominal pós-gastroplastia, segundo a técnica do "peixinho", apresentada neste trabalho. RESULTADOS: A técnica apresentada demonstrou-se satisfatória, resultando em novo contorno corporal, com baixo índice de complicações. CONCLUSÕES: A abdominoplastia com marcação em peixe é mais uma opção no arsenal do cirurgião plástico para tratamento do excedente cutâneo do abdome de pacientes submetidos a cirurgia bariátrica.

Reconstruction of pelviperineal injuries with perforator flaps: clinical experience with 22 cases

ABSTRACT

INTRODUCTION: Plastic surgery consultation is commonly sought for the treatment of pelviperineal injuries in general hospitals. The objective of this study was to present the experience acquired in the treatment of perineal, sacral, and hip injuries with the use of perforator flaps. METHODS: Patients referred to the Plastic Surgery Division of the Clinical Hospital of Medicine College of Universidade de São Paulo for evaluation of pelviperineal and hip wounds from February to May 2009 were retrospectively evaluated. A total of 22 patients underwent reconstruction with skin and fasciocutaneous flaps based on the perforator vessels, according to the inclusion criteria. The average follow-up period was 6 months. RESULTS: Pelviperineal injuries consisted of pressure ulcers in 20 cases (91%), deep infection in 1 case (45%), and perineal hidradenitis in 1 case (4.5%). The choice of flap for reconstruction was dependent on the local wound: 15 cases (68.2%) of sacral ulcers were repaired with a superior gluteal artery perforator flap; 3 cases (13.6%) of ischial ulcers were repaired with an inferior gluteal artery perforator flap; and 2 cases (9.1%) of trochanteric ulcers were repaired using a tensor fascia lata perforator flap. A fasciocutaneous gluteofemoral flap was selected for reconstruction of post Fournier' syndrome in 1 patient and was used after resection of perineal hidradenitis in 1 patient. A new suture for late primary closure was necessary in 3 (13.6%) cases in which the suture line dehiscence was < 10% of the injury perimeter during the first 15 post-operative days. There were no cases of > 3% necrosis of the flap surface. These results were maintained during the follow-up evaluation period. CONCLUSIONS: The results of the study were satisfactory, and the utility of surgical flaps without the incorporation of muscle for pelviperineal reconstruction was demonstrated. This treatment alternative decreases donor site morbidity and preserves the muscular tissue for future interventions.

RESUMO

INTRODUÇÃO: As lesões pelveperineais representam grande parcela das interconsultas para o cirurgião plástico em hospitais gerais. O objetivo do presente trabalho é apresentar a experiência obtida no tratamento de pacientes com lesões perineais, sacrais e de quadril com o uso de retalhos com vasos perfurantes. MÉTODO: Foram estudados, retrospectivamente, os pacientes submetidos a avaliação pela equipe da Divisão de Cirurgia Plástica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, nos meses de fevereiro a maio de 2009, que apresentavam feridas em região pelveperineal e no quadril. No total, 22 pacientes foram submetidos a reconstrução com retalhos cutâneos e fasciocutâneos baseados em vasos perfurantes, de acordo com os critérios de inclusão. O período de seguimento médio foi de 6 meses. RESULTADOS: A lesão pelveperineal foi úlcera por pressão em 20 (91%) casos, infecção profunda em 1 (4,5%), e hidradenite perineal em 1 (4,5%). A opção dos retalhos foi previamente estabelecida, dependendo do local da ferida: úlceras sacrais, retalho baseado nas perfurantes da artéria glútea superior em 15 (68,2%) casos; úlceras isquiáticas, retalho baseado nos vasos perfurantes da artéria glútea inferior em 3 (13,6%) casos; e úlceras trocantéricas, retalho tensor da fascia lata perfurante em 2 (9,1%) casos. Retalho fasciocutâneo inervado gluteofemoral foi a opção para a reconstrução pós-síndrome de Fournier em um paciente e após ressecção de hidradenite perineal em outro. Houve necessidade de nova sutura para fechamento primário tardio em deiscência < 10% do perímetro da lesão em 3 (13,6%) casos, durante os primeiros 15 dias de pós-operatório. Não houve casos de necrose > 3% da superfície do retalho. Os resultados foram mantidos no período de seguimento avaliado. CONCLUSÕES: Os resultados obtidos no presente estudo foram satisfatórios e ficou demonstrada a utilidade de retalhos cirúrgicos sem incorporação de músculo para reconstruções pelveperineais. Essa alternativa para tratamento é menos mórbida para as áreas doadoras e preserva o tecido muscular para possível intervenção futura.

Psychological approach for post-bariatric plastic surgery

ABSTRACT

Nowadays, plastic surgery plays an important role in the surgical treatment of morbid obesity, and it is used to restore optimal body contour in a large number of patients with massive weight loss. The outcome of surgery may affect the patient's psychological behavior, and a multispecialty approach should be adopted, before and after the gastric bypass, to better educate and prepare patients for the dramatic changes they will experience in their body image. The increase in bariatric surgery has also resulted in a greater demand for plastic surgery. In addition to the multispecialty services, patients are seeking out private practice surgeons who will need to recognize and manage the psychological aspects of candidates who undergo body reshaping. This paper reviews the literature on the complex psychological environment of obese patients, emphasizing the identification and management of psychological disorders, and providing plastic surgeons with tools for safer planning and superior outcomes in body contouring after massive weight loss.

RESUMO

Atualmente, a cirurgia plástica vem se firmando cada vez mais como parte integrante do tratamento cirúrgico da obesidade mórbida, na medida em que visa a devolver as melhores condições de contorno corporal ao enorme contingente de pacientes submetidos a grandes perdas ponderais. Os aspectos peculiares que acompanham essa nova trajetória do paciente obeso exigem abordagem interdisciplinar, com cuidadoso acompanhamento psicológico, antes e depois da cirurgia bariátrica, que deverá prepará-lo continuamente para as grandes transformações impostas a sua imagem corporal. Com a popularização das gastroplastias e a crescente demanda por procedimentos de contorno corporal após grandes emagrecimentos, é cada vez mais comum a presença desses pacientes nos consultórios de cirurgiões que não estão ligados aos serviços multidisciplinares, e que, portanto, precisam conhecer, avaliar e lidar também com os aspectos psicológicos envolvendo candidatos a cirurgia plástica pós-bariátrica. Este trabalho estabelece uma revisão da literatura acerca do complexo ambiente psicológico na obesidade, voltada para o cirurgião plástico, com ênfase na identificação e no controle das condições psíquicas desfavoráveis, possibilitando o melhor planejamento operatório em pacientes com perda significativa de peso após cirurgia bariátrica.

Palatoplasty training model

ABSTRACT

Cleft lip and palate is the most common facial defect and causes several aesthetic and functional problems. The use of training models is recommended for training new surgeons, especially for surgeries in the oral cavity where surgical access and instrument handling are difficult. This paper describes a replicable training model of cleft palate closure that helps new surgeons acquire the technical skills required to carry out safe palatoplasty surgeries. The training model is composed of 5 steps, including theoretical classes, monitoring of real surgery procedures, and the preparation of the training model. The training model proposed here allows the surgeon to become familiar with surgery in the small and deep oral cavity; to learn how to use magnifying glasses and/or microscopes; and to perform incisions, dissection, and suturing at a depth. The model can be made manually and inexpensively, and is easy to replicate. The proposed simulator provides realistic training in palatoplasty and can be used to teach surgical techniques.

RESUMO

A fissura labiopalatina é o defeito facial mais facilmente encontrado e implica diversos problemas estéticos e funcionais. O uso de modelos de treinamento é uma opção recomendável no treinamento de jovens cirurgiões, principalmente em cirurgias da cavidade oral, em que o acesso cirúrgico e o manejo dos instrumentos são dificultados. O objetivo deste artigo é descrever um modelo reprodutível de treinamento em fechamento de fendas palatinas, desenvolvendo segurança e habilidade técnica nos jovens cirurgiões para realização da palatoplastia. O modelo de treinamento é constituído de cinco etapas, incluindo aulas teóricas, vivência do ato operatório e confecção do modelo de treinamento. O modelo de treinamento proposto neste artigo permite que o cirurgião se familiarize com o trabalho em cavidade pequena e profunda, aprenda a utilizar lentes de aumento e/ou microscópio, e treine incisões, dissecção e sutura em profundidade, podendo ser confeccionado manualmente e com baixo custo. O simulador proposto permite treinamento realístico em palatoplastia, podendo ser usado para o ensino da técnica cirúrgica, sendo facilmente reprodutível.

ABSTRACT

Complex wounds are characterized by complete loss of cutaneous cover. The most common plastic surgery technique is the autogenous skin graft; however, the amount of material available from donor areas is often limited. The development of synthetic or biological products as skin substitutes is therefore an area of interest. The present study aimed to classify the different types of skin substitutes available based on three criteria: the skin layer to be replaced, which can be categorized into epidermal (E), dermal (D), and dermal-epidermal composites (C); the durability in the wound bed, which can be temporary (T) or permanent (P); and the origin of the material, subdivided into biological (b), biosynthetic (bs), and synthetic (s).

RESUMO

Feridas complexas podem resultar na perda completa do revestimento cutâneo. A solução consagrada pela cirurgia plástica é a enxertia de pele autógena, porém há casos em que ocorre escassez de área doadora cutânea, um problema ainda não totalmente solucionado. Assim, atualmente há muito interesse por materiais sintéticos ou biológicos que possam ser utilizados como substitutos cutâneos. O objetivo deste estudo foi introduzir uma forma mais didática de agrupar diferentes tipos de substitutos cutâneos. Acreditamos que esses produtos podem ser classificados de forma mais abrangente se forem divididos segundo três critérios: camada substituída da pele, subdivididos em epidérmicos (E), dérmicos (D) e compostos dermoepidérmicos (C); duração no leito da ferida, subdivididos em temporários (T) e permanentes (P); e origem do material constituinte, subdivididos em biológicos (b), biossintéticos (bs) e sintéticos (s).

Complications after breast implant use in Poland's syndrome: a case report

ABSTRACT

Poland's syndrome is described as the absence of the pectoralis major muscle associated with deformities of the hands such as variable syndactylia, absence of the middle phalanx, fusion of the carpal bones, or shortening of the forearm. There may be variations in the extent of the thoracic impairment from the absence of anterior portions of ribs or costal cartilages to "sunken chest," which is generally used to describe thoracic depressions associated with skeletal dysplasia. In general, the areolar-papillary complex is in an upper region in men, and women have hypoplastic breasts. A 32-year-old female patient with Poland's syndrome sought our outpatient service wishing to improve the appearance of her thorax and breasts. She had undergone surgical intervention 7 years earlier with implantation of a silicone breast prosthesis, aiming to minimize the characteristic thoracic deformity. After an assessment, we opted to replace the prosthesis. During the intraoperative period, an area of osteocartilaginous resorption about 4 cm in diameter was observed in a region of costochondral articulation. After the surgery, the patient progressed as expected. Extensive research was carried out on the deformity found, and the patient was referred to a thoracic surgeon. Anterior regional bone resorption in the left hemithorax can be an important surgical finding. It can also represent a decrease in cardiac protection, necessitating greater care in cases of breast prosthesis replacement and the requirement of specific imaging examinations such as CT scans in order to minimize surgical risks.

RESUMO

A síndrome de Poland é descrita como ausência do músculo peitoral maior, associada a deformidade das mãos, como sindactilias variáveis, ausência de falanges médias, fusão dos ossos do carpo ou encurtamento do antebraço. Pode haver variação na extensão do comprometimento torácico, desde o sunken chest, termo utilizado para as depressões torácicas associadas a displasia esquelética em geral, até a ausência das porções anteriores das costelas ou cartilagens costais. Em geral, o complexo areolopapilar é posicionado mais superiormente nos homens e nas mulheres a mama é hipoplásica. É relatado o caso de paciente portadora de síndrome de Poland, com 32 anos de idade, que procurou nosso ambulatório de residentes com o desejo de melhorar a aparência do tórax e da mama. Foi realizada intervenção cirúrgica sete anos antes, com implante de prótese mamária de silicone, no intuito de amenizar a deformidade torácica característica. Após avaliação, optou-se pela troca da prótese. No intraoperatório, observou-se área de reabsorção osteocartilaginosa com cerca de 4 cm de diâmetro, em região de articulação costocondral. Após a cirurgia, a paciente evoluiu conforme o esperado. Foi realizada investigação ampla da deformidade encontrada e a paciente avaliada foi encaminhada ao cirurgião torácico. A reabsorção óssea regional anterior no hemitórax esquerdo pode ser um achado cirúrgico e representar diminuição da proteção cardíaca, tornando necessária maior atenção para os casos de substituição de prótese mamária, com realização de exame de imagem específico, como tomografia computadorizada, com o objetivo de minimizar o risco cirúrgico.

Fasciocutaneous flap of the inner thigh for scrotal reconstruction in Fournier syndrome

ABSTRACT

Fournier syndrome is a severe disease that should be treated aggressively with debridement and broad-spectrum antibiotics. It usually causes loss of a large area of tissue and may spread to other areas beyond the perineum such as the abdomen, lower limbs, dorsum, chest, and retroperitoneum. The loss of the scrotum is a common condition in this pathology. Two cases of scrotum reconstruction with the fasciocutaneous flap of the inner thighs with excellent recovery, good aesthetic results, and few complications are reported.

RESUMO

A síndrome de Fournier é uma doença grave e que deve ser agressivamente tratada com desbridamentos e antibioticoterapia de amplo espectro. Geralmente causa perda de grande área de tecido, podendo espalhar-se para outras áreas além do períneo, como abdome, membros inferiores, dorso, tórax e retroperitônio. A perda da bolsa escrotal é uma situação frequente nessa doença. São relatados dois casos de reconstrução de bolsa escrotal com retalho fasciocutâneo de região interna de coxas, com excelente recuperação, bom resultado estético e poucas complicações.

Reversed hemisoleus flap for wound coverage in the distal third of the leg

ABSTRACT

Reconstruction of the distal third of the leg represents a challenge for many plastic surgeons. Although microsurgical flaps have been the method of choice for this reconstruction, many hospitals do not have equipment or microsurgical staff trained for this type of procedure. Moreover, the patient's clinical condition does not allow a more complex surgery in some cases. This study reports two cases of reconstruction of the distal third of the leg with reverse flap of the soleus muscle, based on the perforators of the posterior tibial artery, as an alternative to microsurgical flaps. The reverse soleus flap had an excellent outcome with short surgical duration, easy implementation, excellent resolution, and low morbidity of the donor area.

RESUMO

A reconstrução do terço distal da perna representa um desafio para muitos cirurgiões plásticos. Usualmente, os retalhos microcirúrgicos têm sido considerados de escolha para essas reconstruções; no entanto, muitos hospitais ainda não dispõem de equipamentos ou equipe microcirúrgica treinada nesse tipo de procedimento, além dos casos em que as condições clínicas do paciente não permitem uma cirurgia de maior porte. O presente trabalho propõe o relato de dois casos de reconstrução do terço distal da perna com retalho reverso do músculo solear, baseado nas perfurantes da artéria tibial posterior, como alternativa aos retalhos microcirúrgicos. O retalho solear reverso apresentou excelente resultado, com pequeno tempo cirúrgico, fácil realização, excelente resolubilidade e pouca morbidade da área doadora.

Fingertip replantation and revascularization: literature review and a case report of fingertip amputation of the fifth finger of a one-year-old child

ABSTRACT

The distal phalanx has important functions and must be replanted in case of accidental amputation. Microsurgery is the preferred technique; however, the necessary vein anastomosis can be very difficult or even impossible, especially in small children. This paper reports a case of microsurgical replantation of the distal phalanx of the fifth finger of a one-year-old male child who suffered a crush-type amputation of the fifth fingertip in a house door. The fingertip was replanted eight hours after the amputation, with anastomosis of a very thin artery and vein. The region exhibited congestion, which persisted for more than one week without necrosis; no other procedure was required after the replantation. This paper presents a literature review of the difficulty of restoring venous return in children. Several techniques for restoring venous return in difficult cases, as well as determinants of survival of the amputated part, are reviewed and presented; however, there are no specific studies on procedures for the age group in question. Children probably have better functional recovery or adaptation than do adults, but the amputated part has the same chance of survival, and some of the techniques used in adults are not applicable in children. The literature on this topic is scant, and a study on the most practical and efficient method for fingertip replantation in small children is warranted.

RESUMO

A falange distal é funcionalmente importante e deve ser reimplantada em casos de acidente. A técnica de eleição é microcirúrgica, porém a anastomose da veia pode ser extremamente difícil ou impossível, principalmente em crianças pequenas. Este artigo reporta um caso de reimplante microcirúrgico da falange distal do dedo mínimo em uma criança de sexo masculino, de um ano de idade, que sofreu amputação da falange distal do dedo mínimo tipo "guilhotina", na porta residencial. A falange foi reimplantada 8 horas após a amputação, com anastomose de uma artéria e uma veia extremamente finas. A região apresentou congestão, que durou mais de uma semana, porém sem necrose, não necessitando condutas coadjuvantes. Neste artigo, que também faz revisão bibliográfica da conduta no retorno venoso difícil em crianças dessa faixa etária, várias técnicas foram revistas e apresentadas na solução de retorno venoso difícil, assim como os fatores que influenciam a sobrevivência da parte amputada, porém não há um estudo específico de conduta para essa faixa etária. A criança, provavelmente, tem melhor recuperação ou readaptação funcional, porém a parte amputada tem as mesmas chances de sobrevivência no reimplante, e algumas técnicas possivelmente não sejam aplicáveis. A literatura é escassa, sendo necessária a realização de um estudo para determinar uma conduta prática e eficiente especificamente para essa faixa etária.
