Forehead reduction in patients with precapillary incision: 10 years of experience

ABSTRACT

INTRODUCTION: The long forehead gives a less harmonious appearance, seems disproportionate and characterizes the aging face. Surgical procedures for frontal reduction with precapillary incision present insightful information and provide harmonious and pleasant restoration of the frontal region. This study evaluated results obtained in patients who underwent frontal reduction using precapillary incision at Daher Hospital plastic surgery service within the last ten years. METHODS: This was a retrospective longitudinal study. The surgical technique entailed a careful dissection of the bilateral supraorbital ridge. The scalp was dissected in the parietoccipital region in order to easily move forward the flap. The vigorous hemostasis and resection of excessive flap were performed. In the end, synthesis of plans were performed. RESULTS: The age of patients ranged from 48 to 76 years, the mean age was 58 years. Mean surgical time was 3 hours and 38 minutes. No complications were seen such as hematoma, flap necrosis, supratrochlear nerve injury, alopecia, infections, deep vein thrombosis, pulmonary embolism and/or dehiscence. Four patients (7%) had seroma that was drained. The symmetric of eyebrows and scars positioning was considered satisfactory in both assessment by patients and the technical evaluator. CONCLUSION: The aesthetic results in patients who underwent frontal reduction by precapillary incision were satisfactory. The choice of the ideal patient was crucial for success of the surgery.

RESUMO

INTRODUÇÃO: A testa longa confere uma aparência menos harmônica, desproporcional e caracteriza o envelhecimento. Procedimentos cirúrgicos para redução frontal com incisão pré-capilar apresentam indicações criteriosas e proporcionam a restauração harmônica da região frontal. O estudo avalia os resultados obtidos em pacientes submetidos à redução frontal por incisão pré-capilar no serviço de cirurgia plástica do Hospital Daher nos últimos dez anos. MÉTODOS: Trata-se de um estudo observacional longitudinal retrospectivo. A técnica cirúrgica consiste em uma dissecção cuidadosa até o rebordo supraorbital bilateral. O couro cabeludo é dissecado até a região parietoccipital de forma a avançar com facilidade o retalho. Procede-se à hemostasia vigorosa e ressecção do retalho excedente. Por fim, realiza-se a síntese por planos. RESULTADOS: A faixa etária das pacientes variou de 48 a 76 anos, com média de 58 anos. O tempo médio operatório foi de 3 horas e 38 minutos. Não foram observadas complicações como hematoma, necrose do retalho, lesão do nervo supratroclear, alopécia, infecções, trombose venosa profunda, embolia pulmonar e/ou deiscências. Quatro pacientes (7%) apresentaram seroma, os quais foram todos drenados. A simetrização das sobrancelhas e a posição das cicatrizes foram consideradas satisfatórias tanto pela avaliação feita pelos pacientes quanto pela avaliação técnica. CONCLUSÃO: Os resultados estéticos obtidos em pacientes submetidos à redução frontal pela incisão pré-capilar foram satisfatórios. A escolha do paciente ideal foi fundamental para o bom sucesso operatório.

Analysis of the anatomical lines of the medial nasal dorsum after use of spreader grafts in open rhinoplasty

ABSTRACT

INTRODUCTION: First described by Sheen in 1984, the use of spreader grafts has become increasingly routine. Spreader grafts are indicated for recovery or maintenance of the internal nasal valve and correction of septum deviation with improvement in the anatomical lines of the nasal dorsum. This study avaluated the anatomical lines of the middle nasal third (MNT) of the nasal dorsum after the use of spreader grafts in open rhinoplasty. METHODS: A series of 17 patients who underwent rhinoplasty with the inclusion of spreader grafts between March 2013 and November 2014 were analyzed. Spreader grafts were removed from the septum or costal cartilage based on an anthropometric comparison between the medial intercanthal distance and the caudal width of the MNT. RESULTS: Spreader grafts were effective for septum correction without causing excessive enlargement in 12 patients (70.6%). Three patients (17.6%) had undesirable results with local bulging and 2 patients (11.8%) retained their pre-surgery non-standard measures. In the majority of cases (41.2%), there was a local increase of 3 mm, with a mean of increase of 2.67 mm. CONCLUSION: In the author's point of view, even in cases where there was bulging of the anatomical lines of the nasal dorsum, spreader grafts promoted long-lasting satisfactory results, and prevented open-roof deformities and stigmas inherent to the surgery itself.

RESUMO

INTRODUÇÃO: Descritos por Sheen em 1984, os spreader grafts vêm sendo utilizados em rinoplastias de maneira cada vez mais rotineira. São indicados para recuperação ou manutenção da válvula nasal interna e correção do desvio de septo com aprimoramento das linhas anatômicas do dorso nasal. O estudo analisa o comportamento das linhas do terço médio do dorso nasal após o uso de spreader grafts em rinoplastias abertas. MÉTODOS: Foi avaliada uma série de 17 pacientes, operados entre março de 2013 e novembro de 2014, submetidos à exorrinoplastia com inclusão de spreader grafts, retirados do septo ou cartilagem costal, baseando-se em uma comparação antropométrica entre distância intercantal medial e largura caudal do terço médio nasal. RESULTADOS: Os enxertos expansores se mostraram eficazes na retificação do septo, sem causar alargamento excessivo em 12 pacientes (70,6%), sendo que três pacientes (17,6%) obtiveram resultados indesejáveis com abaulamento do local e dois pacientes (11,8%) mantiveram suas medidas fora do padrão. Na maior parte dos casos (41,2%) houve aumento de 3 milímetros locais, sendo a média padrão de 2,67 mm por procedimento. CONCLUSÃO: Constatou-se, no ponto de vista do autor, que os spreader grafts, mesmo nos casos em que houve abaulamento das linhas anatômicas do dorso nasal, promoveram resultados satisfatórios e duradouros, prevenindo deformidades do teto aberto e estigmas inerentes à própria cirurgia.

What is the outcome 5 years after cervicoplasty?

ABSTRACT

INTRODUCTION: An approach based on multiple anatomical structures and the use of different surgical strategies seems to achieve more lasting results in cervical rejuvenation surgery. Thus, extended cervicoplasty is an option with favorable results and low complication rates. However, little has been published regarding the durability of the results. This study aimed to objectively evaluate the long-term results obtained with extended cervicoplasty in difficult cases. METHODS: Twenty patients, classified as having a "difficult neck," underwent extended cervicoplasty and were followed for 5 years. The results at 1- and 5-year post-operative follow-up were evaluated by eight plastic surgeons, using six objective criteria based on a theoretically ideal neck. The comparison of the results obtained at these time points was performed using a paired Student's t-test, with a level of significance of 5%. RESULTS: There was no significant difference in five of the six items evaluated (p-value ranging from 0.137 to 1.000), in the comparison between the first and fifth postoperative years. Subhyoid depression displayed a significantly better mean score in the fifth evaluation year than that observed in the first year after surgery (p = 0.039): from 5.80 ± 0.25 points (mean ± standard error of the mean) in the first year, to 6.45 ± 0.30 points in the fifth postoperative year. CONCLUSION: Extended cervicoplasty is an important alternative in the treatment of the neck region, and even provides lasting results in difficult cases.

RESUMO

INTRODUÇÃO: A abordagem de múltiplas estruturas anatômicas e a utilização de diferentes estratégias cirúrgicas parece contribuir para a obtenção de resultados mais duradouros na cirurgia do rejuvenescimento cervical. Para isso, a cervicoplastia ampliada é uma opção com resultados agradáveis e baixos índices de complicações. A durabilidade dos resultados obtidos é, no entanto, pouco discutida na literatura. O objetivo desse estudo é avaliar objetivamente a manutenção dos resultados a longo prazo obtidos com a cervicoplastia ampliada aplicada em casos difíceis. MÉTODOS: Vinte pacientes, classificadas como "pescoço difícil", foram submetidas à cervicoplastia ampliada e acompanhadas por 5 anos. Os resultados do pós-operatório de 1 e de 5 anos foram avaliados por oito cirurgiões plásticos, por meio de seis critérios objetivos vinculados a um teórico pescoço ideal. A comparação entre os resultados obtidos nestes momentos foi realizada por meio do teste t-student pareado, considerando um nível de significância de 5%. RESULTADOS: Não houve diferença significativa em cinco dos seis itens avaliados (valor de p variando entre 0,137 a 1,000), na comparação entre o primeiro e o quinto ano de pós-operatório. A depressão subhioideia apresentou pontuação média na avaliação do quinto ano significativamente melhor do que aquela observada no primeiro ano após a cirurgia (p = 0,039), passando de 5,80 ± 0,25 pontos (média ± erro padrão da média) no primeiro ano, para 6,45 ± 0,30 pontos no quinto ano de pós-operatório. CONCLUSÃO: A cervicoplastia ampliada se valida como alternativa importante no tratamento da região cervical mesmo em casos difíceis, proporcionando a obtenção de resultados duradouros.

Lower eyelid and malar/palpebral groove reconstructions with a V-Y skin flap after skin tumor removal

ABSTRACT

INTRODUCTION: The most commonly used flaps for the reconstruction of the lower eyelid and malar/palpebral groove are cervicofacial flaps owing to their horizontal traction, which results in a lower risk of ectropion and canthal dystopias. V-Y skin flap is not often used for the fear of the vertical traction being transferred to the lower eyelid, which would lead to the occurrence of ectropion and dystopias. Our aim is to demonstrate that, if well executed, this flap may be a good option for reconstructions of this region. METHODS: Sixty-eight eyelid reconstructions have been performed at the ACCamargo Cancer Center between December 2012 and May 2015. Of these, 29 patients underwent reconstruction of the lower eyelid or malar/palpebral groove using a V-Y skin flap after skin tumor removal. RESULTS: Among the palpebral reconstructions, the most commonly used was the V-Y flap (29 cases, 42.6%). Two cases (6.9%) developed ectropion. No case of seroma, hematoma, infection, or partial loss of the flap has been observed. CONCLUSION: The V-Y flap is a good option for eyelid reconstruction when well indicated, designed, and executed, since it presents low complication rates and the possibility to be performed under local anesthesia. Therefore, this flap is a good choice for patients without the clinical conditions necessary to undergo surgery under general anesthesia.

RESUMO

INTRODUÇÃO: Os retalhos mais comumente utilizados para reconstruções de pálpebra inferior e junção malar/palpebral são os cervicofaciais, pois a sua tração horizontal levaria a menor risco de ectrópio e distopias cantais. O retalho cutâneo em V-Y frequentemente não é utilizado pelo receio de que a tração vertical seja transferida à pálpebra inferior, gerando ectrópio e distopias. Nosso objetivo é demonstrar que, se bem executado, este retalho pode ser uma boa opção para reconstruções nesta região. MÉTODOS: Foram realizadas 68 reconstruções palpebrais no ACCamargo Cancer Center, entre dezembro de 2012 e maio de 2015. Dentre estes pacientes, 29 foram submetidos à reconstrução de pálpebra inferior ou junção malar/palpebral, após ressecção de tumores cutâneos, com retalhos cutâneos em V-Y. RESULTADOS: Dentre as reconstruções palpebrais, o retalho V-Y foi o mais utilizado (29 casos, 42,6%). Dois casos (6,9%) evoluíram com ectrópio. Não houve nenhum caso de seroma, hematoma, infecção ou perda parcial do retalho. CONCLUSÃO: O retalho V-Y é uma boa opção para reconstruções palpebrais quando bem indicado, desenhado e executado, pois apresenta baixos índices de complicações e também por poder ser realizado sob anestesia local, sendo uma boa escolha para pacientes sem condições clínicas de serem submetidos à anestesia geral.

Versatility of the mid-forehead flap in facial reconstruction

ABSTRACT

INTRODUCTION: Reconstruction of complex facial defects is a challenge to the plastic surgeon. Different missing anatomical units must be accessed in different ways and with individualized goals, always tailoring the options to the patient's needs. The objective is to examine the role of the mid-forehead flap in the reconstruction of different anatomical facial units. METHODS: Retrospective analysis of patients who were operated on by the author from February 2010 to June 2015. Patients were evaluated according to age, sex, lesion etiology, defect location, number of operations performed per patient, and postoperative complications. RESULTS: Fifteen patients (mean age, 69 years) underwent facial reconstruction with a mid-forehead flap. Thirteen patients required more than one operation for pedicle refinement and transection. There was one case of partial necrosis of the flap in the columella region, with satisfactory healing by second intention. There was no infection or hematoma. All secondary cartilage grafts showed integration into the recipient bed. CONCLUSIONS: The mid-forehead flap remains an important tool for the reconstruction of major facial defects. It allows the transfer of frontal tissue in an efficient and reliable way with minimal deformity in the donor area, resulting in an esthetically acceptable reconstruction.

RESUMO

INTRODUÇÃO: A reconstrução de defeitos complexos faciais é um desafio ao cirurgião plástico. Diferentes unidades anatômicas ausentes devem ser acessadas de maneiras distintas e com objetivos próprios, sempre adequando as possibilidades ao paciente em questão. O objetivo é mostrar o papel do retalho médio-frontal na reconstrução de diferentes unidades anatômicas faciais. MÉTODOS: Análise retrospectiva de pacientes operadas pela autora, no período de fevereiro de 2010 a junho de 2015. Os pacientes foram avaliados em relação à idade, sexo, etiologia da lesão, localização do defeito, número de tempos cirúrgicos realizados por paciente e complicações pós-operatórias. RESULTADOS: Quinze pacientes foram submetidos à reconstrução facial com retalho médiofrontal, com média de idade de 69 anos. Treze pacientes necessitaram mais de um tempo cirúrgico para refinamento e transecção do pedículo. Houve um caso de necrose parcial do retalho na região da columela, com cicatrização satisfatória por segunda intenção. Não houve infecção ou hematoma. Todos os enxertos cartilaginosos secundários se integraram ao leito receptor. CONCLUSÕES: O retalho médio-frontal permanece como importante ferramenta na reconstrução de grandes defeitos faciais. Permite a transferência de tecido frontal de forma eficiente e confiável com mínima deformidade na área doadora, possibilitando uma reconstrução esteticamente aceitável.

Microsurgical head and neck reconstruction: retrospective analysis of 30 free flaps

ABSTRACT

INTRODUCTION: Reconstructive head and neck surgery is a challenge for the plastic surgeon and requires a large technical arsenal, in which microsurgery plays an essential role. The objective to retrospectively analyze microsurgical head and neck reconstruction performed at the Pontifical Catholic University of Campinas over a period of 2 years. METHODS: The medical charts of patients who underwent microsurgical reconstruction after head and neck surgery between June 2014 and October 2016 were reviewed to determine which flap was used, the length of the vascular pedicle, recipient vessels, microvascular anastomoses, surgical time, length of hospital stay, complications, and success rate. RESULTS: Thirty microsurgical reconstructions were performed using 3 types of flaps: anterolateral thigh (n = 15), antebrachial radial (n = 8), and fibula (n = 7). Recipient vessels: facial artery (70%) and facial vein (50%), as 92.4% of microvascular anastomoses were end-to-end. The average operative time was 10.1 hours. The average length of hospital stay was 10.7 days. Two flaps were lost due to arterial thrombosis, resulting in a success rate of 93.3%. CONCLUSIONS: The microsurgical reconstructions were effective for the repair of complex defects as well as to partially restore the shape and function of the affected tissues. Complications were observed in less than 50% of the cases, although these presented with significant morbidity. The success rate was similar to that reported by major centers of reconstructive microsurgery. The learning curve is long, although it tends to improve with staff training and the acquisition of experience over time.

RESUMO

INTRODUÇÃO: Cirurgia reconstrutiva em cabeça e pescoço representa desafio ao cirurgião plástico e requer amplo arsenal técnico, no qual a microcirurgia figura papel essencial. O objetivo é analisar retrospectivamente as reconstruções microcirúrgicas em cabeça e pescoço realizadas na Pontifícia Universidade Católica de Campinas no período de dois anos. MÉTODOS: Pacientes submetidos à reconstrução microcirúrgica após cirurgia de cabeça e pescoço, entre junho de 2014 e outubro de 2016, tiveram seus prontuários revisados para avaliação do retalho utilizado, comprimento do pedículo vascular, vasos receptores, anastomoses microvasculares, duração da cirurgia, tempo de internação, complicações e índice de sucesso. RESULTADOS: Foram realizadas 30 reconstruções microcirúrgicas com três tipos de retalhos: anterolateral da coxa (n = 15), antebraquial radial (n = 8) e fíbula (n = 7). Vasos receptores: artéria facial (70%) e veia facial (50%), sendo que 92,4% das anastomoses microvasculares foram término-terminais. As cirurgias duraram, em média, 10,1 horas. O tempo médio de internação hospitalar foi de 10,7 dias. Houve perda de dois retalhos por trombose arterial, levando à taxa de sucesso de 93,3%. CONCLUSÕES: As reconstruções microcirúrgicas analisadas foram eficazes na reparação dos defeitos complexos, restaurando parcialmente forma e função dos tecidos acometidos. Foram observadas complicações em menos da metade dos casos, porém com morbidade elevada. O índice de sucesso foi semelhante ao de grandes centros de microcirurgia reconstrutiva. A curva de aprendizado é longa, mas tende a melhorar com o treinamento da equipe e aquisição de experiência ao longo do tempo.

Prevalence of nasal reconstruction techniques used over the past four years at the Plastic Surgery Service of Ceará

ABSTRACT

INTRODUCTION: Nasal skin lesions are significance in plastic surgery because of the difficult of aesthetic and functional reconstruction. Central position of the nose on the face, and its structure, rigid architecture, and low mobility are among the possible causes. Several techniques used to correct nasal defects have been documented since the origins of plastic surgery. We evaluated retrospectively and analyzed the main surgical techniques and their indications in cases of nasal reconstruction performed at the Plastic Surgery Service of Ceará over a period of 4 years. METHODS: This was a retrospective clinical study including 151 patients who underwent resection of nasal lesions from January 2010 to December 2014 at a public hospital in Fortaleza. Variables included were sex, age, and surgical technique. RESULTS: Most of patients were men with a mean age of 63.2 years, they accounted for 52.3% of the sample. The main reconstruction forms were primary suture (28.5%), rotational flap (22.5%), bilobate (14.6%), advancement flap (8.6%), nasogenian (6%), frontal (4.7%) and skin graft (15.2%). CONCLUSION: Because of the different nasal reconstruction possibilities, the surgical technique choice, after resections, should consider defect location and size, contour and nasal anatomy for a better aesthetic and functional result. At our service, reconstruction with primary lesion synthesis was predominant with a slight predominance among men and older patients. Other techniques were used given the preference for better aesthetic and functional result for each patient.

RESUMO

INTRODUÇÃO: As lesões cutâneas nasais são de grande importância para o cirurgião plástico em virtude da dificuldade em se promover reconstruções estéticas e funcionais quando da ressecção das mesmas. Várias técnicas tem sido descritas para se corrigir um defeito nasal, sendo documentadas desde os primórdios da cirurgia plástica. MÉTODOS: Foi realizado um estudo clínico retrospectivo de uma série de casos consecutivos de cento e cinquenta e um pacientes submetidos a ressecção de lesões nasais no período de janeiro de 2010 a dezembro de 2014 em hospital publico de Fortaleza. Foram analisadas as variáveis sexo, idade, técnica cirúrgica. RESULTADO: Do total de prontuários analisados, a maioria dos pacientes era do sexo masculino correspondendo a 52,3%, com idade média de 63,2 anos. As principais formas de reconstrução foram a sutura primária (28,5%), retalho de rotação (22,5%), bilobado (14,6%), retalho de avanço (8,6%), nasogeniano(6%), médio frontal (4,7%) e enxerto (15,2%). CONCLUSÃO: Diante das inúmeras possibilidades de reconstrução nasal conclui-se que a escolha da técnica cirúrgica utilizada após as ressecções deve levar em consideração a localização e tamanho do defeito, contorno e anatomia nasal para um melhor resultado estético e funcional de cada paciente. Em nosso serviço predominou a reconstrução com síntese primária da lesão ocorrendo leve predomínio em pacientes do sexo masculino e idosos. Outras técnicas foram utilizadas priorizando-se o melhor resultado estético e funcional para cada paciente.

Evaluation of cases of mammoplasty with the areolar inferior pedicle technique

ABSTRACT

INTRODUCTION: Mammoplasty is one of the most common plastic surgeries in Brazil. Among the surgical techniques described in the literature, the areolar inferior pedicle technique is the most significant because of its versatility, safety, and satisfactory aesthetic results. This study avaluated cases of mammoplasty with the areolar inferior pedicle technique performed by the author, and to propose a small change in the attachment of the pedicle to the pectoralis major muscle. METHODS: A total of 74 patients underwent the surgery from June 2007 to December 2014. RESULTS: The average patient age was 35.63 years; the mean body mass index was 25.8; and the mean follow-up duration was 28 months. Complications occurred in 33.76% of the cases, and the average amount of resected tissue was 585 g. The degree of satisfaction was excellent or good in 93% of the cases. The sensitivity of the areolar-papillary complex (APC) was maintained in 72% of left-breast reconstructions and 77% of right-breast reconstructions. Moreover, there was a complete loss of sensitivity in 3% of left-breast cases. CONCLUSION: The use of mammoplasty with the areolar inferior pedicle was versatile and safe, and produced favorable outcomes. In addition, the sensitivity of the APC was adequately maintained. Further studies might confirm the importance of the change in fixation of the pedicle suggested in this study.

RESUMO

INTRODUÇÃO: A mamoplastia é uma das cirurgias plásticas mais realizadas no Brasil. Dentro do universo de técnicas descritas, destaca-se o pedículo inferior areolado por sua versatilidade, segurança e bons resultados estéticos. O estudo avalia os casos de mamoplastia pela técnica do pedículo inferior areolado realizados pelo autor e apresentar uma pequena modificação na forma de fixação do pedículo ao músculo peitoral maior. MÉTODOS: Foram avaliadas 74 pacientes operadas no período de junho de 2007 a dezembro de 2014. RESULTADOS: A idade média das pacientes foi de 35,63 anos, o índice de massa corporal médio foi 25,8 e o tempo médio de seguimento, de 28 meses. Complicações ocorreram em 33,76% dos casos, a quantidade média de tecido ressecado foi 585 gramas. O grau de satisfação foi ótimo ou bom em 93% dos casos. Houve manutenção da sensibilidade do complexo areolopapilar (CAP) em 72% das mamas à esquerda e 77% à direita. Em 3% das mamas à esquerda houve perda completa da sensibilidade. CONCLUSÃO: A mamoplastia pelo pedículo areolado inferior mostrou-se neste estudo versátil, segura e com bons resultados. Além disso, houve boa manutenção da sensibilidade do CAP. Estudos posteriores poderão ratificar a importância da modificação na fixação do pedículo sugerida neste trabalho.

ABSTRACT

INTRODUCTION: Abdominoplasty is one of the most common plastic surgery procedures used worldwide. It covers the esthetic appearance and reconstruction of the abdominal wall. Many variations of the incisions, musculofascial repair, and remodeling of the navel have been described. In 1965, Callia performed an incision on the pubis with lateral extensions. In 1967, Pitanguy performed a low horizontal incision with the extremities out and down, and promoted the plication of the rectus abdominis muscles without opening the aponeurosis. Besides demonstrating the ease of implementation and safety of the procedure, the objective of this study was to perform classical abdominoplasty in combination with median and external pararectal plications, also referred to as a corset technique, to recover abdominal wall tension and then liposuction at the end of the procedure to improve abdominal body contour. METHODS: Sixty-two women with a mean age of 37 years underwent operation between 2011 and 2013. RESULTS: None of the women in the present study had hematoma, infection, necrosis, or epidermolysis. Late encapsulated seroma was observed in one patient. Five patients had a "dog ear," and eight had unsightly scars but no keloid. All the patients were satisfied with the surgical outcome. CONCLUSION: The technique described in this study adds two more plications to the classic abdominoplasty, which lead to higher tension in the abdominal wall and more satisfactory esthetic results. Besides being easy to implement, the technique enables lower positioning of the suprapubic transverse incision, which can be easily hidden even with short clothing.

INTRODUÇÃO: Abdominoplastia é um dos procedimentos mais realizados na Cirurgia Plástica mundial. Abrange o aspecto estético e reconstrutor da parede abdominal. Muitas variações das incisões, do reparo musculofascial e da remodelagem do umbigo têm sido descritas. Em 1965, Callia realizou incisão sobre o púbis com prolongamentos laterais. Em 1967, Pitanguy fez incisão horizontal baixa com as extremidades para fora e para baixo e promoveu a plicatura dos músculos retos abdominais sem a abertura da aponeurose. O objetivo deste estudo foi realizar abdominoplastia clássica associada às plicaturas mediana e pararretais externas para recuperar a tensão da parede abdominal - técnica em espartilho - e também à lipoaspiração ao final do procedimento, no intuito de melhorar o contorno corporal abdominal, além de mostrar a facilidade de execução e a segurança do procedimento. MÉTODOS: Foram operadas 62 mulheres no período de 2011 a 2013, com idade média de 37 anos. RESULTADOS: Não houve hematoma, infecção, necrose ou epidermólise no presente estudo. Observou-se a presença de seroma encapsulado tardio em uma paciente. Houve cinco casos de "dog ear", oito de cicatrizes inestéticas, mas nenhuma queloideana. Todas as pacientes sentiramse satisfeitas. CONCLUSÃO: Esta técnica agregou mais duas plicaturas musculares à abdominoplastia clássica, levando a uma maior tensão na parede abdominal, com resultado estético mais satisfatório. Conseguiu-se um posicionamento mais baixo da incisão transversal suprapúbica, que pode ser facilmente oculta por roupas menores, além da facilidade de execução.

Abdominoplasty after bariatric surgery: experience in 315 cases

ABSTRACT

INTRODUCTION: The abdomen is a critical region and is subjected to body contour deformity after significant weight loss. Knowing the profile of patients who undergo abdominoplasty after bariatric surgery facilitates the choice of the optimal time and surgical technique to restore abdominal contour, which is associated with a minimal rate of complications. METHODS: A cross-sectional, descriptive, analytical, and retrospective clinical study was performed on medical record data of 315 patients who underwent abdominal dermolipectomy following bariatric surgery from January 2013 to December 2014. RESULTS: A total of 265 female and 50 male patients were analyzed, with an average age of 39.9 years and an average body mass index of 27.5 kg/m2 after weight loss. The average time interval between bariatric surgery and dermolipectomy was 3 years and 11 months. Anchor abdominoplasty was used in 252 patients, while suprapubic transverse abdominoplasty was performed in 63 patients. The complication rate was 17.3%, including minor complications such as hematomas, seromas, and pathological scars, in addition to one case of pulmonary embolism. CONCLUSION: The epidemiological profile of patients with deformity of the abdominal contour after bariatric surgery was in agreement with that reported in the current literature, except for earlier hospital discharge in the group studied.

INTRODUÇÃO: O abdome é considerado área crítica de deformidade do contorno corporal após perda ponderal significativa. Conhecer o perfil dos pacientes submetidos à abdominoplastia pós-cirurgia bariátrica facilita a escolha do tempo ideal e da técnica cirúrgica que visa restaurar este contorno com uma taxa mínima de complicações. MÉTODOS: Estudo clínico, transversal, descritivo, analítico e retrospectivo com dados de 315 prontuários de pacientes submetidos à dermolipectomia abdominal pós-cirurgia bariátrica de janeiro de 2013 a dezembro de 2014. RESULTADOS: Foram analisados 265 pacientes do gênero feminino e 50 do gênero masculino, com idade média de 39,9 anos e índice de massa corporal médio de 27,5 Kg/m2 após emagrecimento. O tempo médio de intervalo entre a cirurgia bariátrica e a dermolipectomia foi de 3 anos e 11 meses. A abdominoplastia em âncora foi utilizada em 252 pacientes e em 63 realizou-se abdominoplastia com cicatriz transversal suprapúbica. A taxa de complicações foi de 17,3%, incluindo complicações menores como hematomas, seromas e cicatrizes patológicas e um caso de embolia pulmonar. CONCLUSÃO: O perfil epidemiológico dos pacientes com deformidade de contorno abdominal pós-cirurgia bariátrica estudados assemelha-se ao apresentado na literatura atual, exceto pela alta hospitalar precoce.

Continuous absorbable suture versus interrupted permanent suture in the treatment of diastasis of the rectus abdominis muscle

ABSTRACT

INTRODUCTION: Plication of the rectus abdominis muscle is an important step in remodeling the abdomen in abdominoplasties. It can be performed using several types of threads with interrupted or continuous sutures, according to the surgeon's preferences. Few studies in the literature compared the different plication techniques in abdominoplasties. The present study aimed to compare continuous anchored suture with polydioxanone (PDS) with interrupted nylon suture regarding surgical time, relapse, and costs and discuss the advantages and disadvantages of each method. METHODS: A randomized clinical trial in which 30 patients were enrolled for each of the two groups, 15 with continuous suture and 15 with interrupted suture, was conducted. The plication time was compared between the groups using the Student's t-test. Between 10 and 12 months postoperatively, all patients underwent ultrasonography for investigating diastasis recurrence. RESULTS: Continuous suture decreased the plication time by almost 40% when compared to the interrupted suture (p < 0.001), without affecting the esthetic outcome. The PDS used in the continuous suture implied higher costs than those with the nylon threads used in the interrupted suture. No recurrence was found on the postoperative control ultrasonography results. CONCLUSION: Continuous suture reduced the plication time by almost 40% (p < 0.001) when compared to interrupted suture. Despite being associated with higher costs, PDS seems to be a good alternative to non-absorbable sutures, such as nylon threads, in plications of the rectus abdominis, since it is a more biocompatible material.

INTRODUÇÃO: A plicatura do músculo reto abdominal é um passo importante no remodelamento do abdômen nas abdominoplastias. Ela pode ser realizada com vários tipos de fios e com suturas interrompidas ou contínuas, de acordo com as preferências do cirurgião. Poucos estudos na literatura compararam diferentes técnicas de plicatura nas abdominoplastias. O objetivo do estudo é comparar a sutura contínua ancorada com polidioxanona (PDS) com a sutura interrompida com náilon quanto ao tempo cirúrgico, recidiva, custos e discutir vantagens de desvantagens de cada método. MÉTODOS: Ensaio clínico randomizado no qual 30 pacientes foram arroladas para cada um dos grupos, 15 sutura contínua e 15 sutura interrompida. O tempo de plicatura foi comparado entre os grupos utilizando-se o teste t de Student. Entre 10 a 12 meses de pós-operatório, todas as pacientes foram submetidas à ecografia em busca de recidivas da diástase. RESULTADOS: A sutura contínua diminuiu em quase 40% o tempo de plicatura quando comparada à sutura interrompida (p < 0,001), sem prejuízo no resultado estético. O PDS utilizado na sutura contínua implicou maiores custos que os fios de náilon utilizados na sutura interrompida. Nenhuma recidiva foi encontrada na ecografia de controle no pós-operatório. CONCLUSÃO: A sutura contínua quando comparada à sutura interrompida foi capaz de reduzir o tempo de plicatura em quase 40% (p < 0,001). A utilização do PDS, apesar dos maiores custos, parece ser uma boa alternativa às suturas com fios não absorvíveis como o náilon nas plicaturas do músculo reto abdominal, uma vez que se trata de um fio de maior biocompatibilidade.

Surgical treatment of hidradenitis suppurativa with anterior and posterior lateral thoracic fasciocutaneous flaps

ABSTRACT

INTRODUCTION: Hidradenitis suppurativa (HS) is a chronic, multifactorial, and often recurrent bacterial infection, affecting the skin and subcutaneous tissues. However, complete HS resolution can be achieved through surgical treatment. A series of patients with HS complications is described herein, along with their evolution and complications after complete axillary surgical resection and lateral thoracic fasciocutaneous flap rotation. The evaluation of associated affected areas is also reported. METHODS: Between 2009 and 2014, 6 patients with an average age of 25.5 years (range: 15 to 35 years) underwent surgery for the treatment of HS. All patients had long-standing, chronic axillary lesions that were refractory to non-surgical treatment. RESULTS: Six patients with HS (2 males and 4 females) underwent surgery. The average follow-up period was 16 months (range: 4 months to 5 years). Complete resolution was achieved and no HS recurrence was observed. Bilateral resection was performed in 4 cases, and unilateral resection in 2. Five patients also had inguinal involvement, and 3 had surgery concurrent with the axilla. Five patients had partial dehiscence and serosanguinous discharge, followed by complete scar formation by second intention healing. CONCLUSION: Surgery is often the most appropriate and definitive treatment for HS. The lateral thoracic fasciocutaneous flap technique is associated with high success rates in this patient population.

INTRODUÇÃO: A hidradenite supurativa (HS) é uma infecção bacteriana crônica, multifatorial e frequentemente recorrente na pele e nos tecidos subcutâneos. No entanto, a resolução completa pode ser conseguida por meio de cirurgia. É descrita uma série de pacientes com complicações de HS, sua evolução, as complicações após a ressecção cirúrgica completa axilar e a rotação de retalho fasciocutâneo torácico lateral, além da análise das áreas acometidas associadas. MÉTODOS: Entre 2009 e 2014, seis pacientes com idade entre 25,5 anos em média (intervalo: 15-35) foram submetidos à cirurgia para HS. Todos os pacientes apresentaram lesões de longa data axilares crônicas refratárias ao tratamento clínico. RESULTADOS: Seis pacientes com HS (dois homens e quatro mulheres) foram submetidos à cirurgia. O período de acompanhamento foi de 16 meses, em média (intervalo: 4 meses - 5 anos). A resolução completa foi alcançada e nenhuma recorrência foi observada. A ressecção foi bilateral em quatro casos e unilateral em dois. Cinco pacientes apresentaram lesão inguinal, das quais três foram operadas no mesmo tempo cirúrgico. Cinco pacientes apresentaram deiscência parcial e descarga serossanguinolenta, seguida de cura completa após cicatrização por segunda intenção. CONCLUSÃO: A cirurgia é frequentemente o tratamento definitivo mais adequado para a HS. A técnica de retalho fasciocutâneo torácico lateral está associada a altas taxas de sucesso nesta população de doentes.

Versatility of the medial gastrocnemius muscle flap in reconstructing lower limb soft tissue lesions

ABSTRACT

INTRODUCTION: Covering complex lower limb wounds is a major challenge. The medial gastrocnemius muscle flap is usually indicated for reconstruction of loss of substance of the knee and proximal third of the leg. The objective of this study was to evaluate the results using gastrocnemius muscle flaps in lower limb wounds. METHODS: A total of 28 patients with loss of substance in the lower limbs were treated with medial gastrocnemius muscle flaps. Data were obtained from clinical records of patients who underwent surgery from 2003 to 2012. RESULTS: In 28 patients who underwent surgery, the age range was 4 to 57 years, with a mean of 30.6 years. The etiology of the lesions was a motorcycle accident in 16 cases (57.1%), being run over in 8 cases (28.6%), car crash in 2 cases (7.2%), meningococcemia in 1 case (3.6%), and snake bite in 1 case (3.6%). After an average follow-up of 15 months (range: 6 to 26 months), all patients had stable coverage. The mean number of preoperative debridements was 2.4 (range: 1 to 8). In 22 patients, the flaps were made without the release of the muscle origin and in 6 patients the muscle flaps were separated from their origin in the femur. CONCLUSIONS: Transfer of a gastrocnemius muscle flap is a simple and safe procedure in the treatment of lower limb injuries. The application of the medial gastrocnemius muscle flap was demonstrated in different situations, with satisfactory results and easy reproducibility.

RESUMO

INTRODUÇÃO: A cobertura de feridas complexas de membros inferiores é um grande desafio. O retalho muscular do gastrocnêmio medial é comumente indicado para reconstrução de perdas de substância do joelho e terço proximal da perna. O objetivo desse trabalho é avaliar os resultados da utilização do retalho muscular do gastrocnêmio nos casos de ferida em membros inferiores. MÉTODOS: Um total de 28 pacientes com perda de substância de membros inferiores foi tratado com retalho muscular do gastrocnêmio medial. Os dados foram obtidos em registros clínicos dos pacientes operados no período de 2003 a 2012. RESULTADOS: Vinte e oito pacientes foram operados, com idades variando de 4 a 57 anos, e média de 30,6 anos. A etiologia das lesões foi acidente por motocicleta em 16 casos (57,1%), atropelamentos em oito casos (28,6%), acidente automobilístico em dois casos (7,2%), meningococcemia em um caso (3,6%) e acidente ofídico em um caso (3,6%). Depois de um seguimento médio de 15 meses (variação de 6 a 26 meses), todos pacientes mostraram cobertura estável. O número médio de desbridamentos pré-operatórios foi 2,4 (variação de 1 a 8). Em 22 pacientes, os retalhos foram confeccionados sem a liberação da origem muscular e em seis pacientes os retalhos musculares foram separados de sua origem no fêmur. CONCLUSÕES: A transferência do retalho muscular do gastrocnêmio é um procedimento simples e seguro no tratamento de lesões de membros inferiores. Foi demonstrada a aplicação do retalho muscular do gastrocnêmio medial em diferentes situações, com resultados satisfatórios e fácil reprodução.

Labiaplasty and its current status

ABSTRACT

INTRODUCTION: Nymphoplasty or labiaplasty consists of a surgical approach to hypertrophy of the labia minora, and is aimed at correcting asymmetry and removing redundant tissue of the vulva. Hypertrophy of the labia minora is characterized by tissue that exceeds the limits of the labia majora under normal conditions. Surgery is an option in patients who complain of unpleasant odor, urinary infection, interference with sexual activity, and/or personal dissatisfaction; in the latter case, the surgical indication has an aesthetic purpose. The aim of this article was to report our experience with wedge resection of the labia minora in patients with hypertrophy. METHODS: We retrospectively analyzed 53 cases of labiaplasty performed with the wedge resection technique from 2010 to 2015. The results were evaluated for postoperative anatomical appearance, the degree of patient satisfaction, and the effect on sexual activity. RESULTS: Patients were satisfied with the postoperative aesthetic appearance. There was improvement in sexual satisfaction, ability to perform physical activities, and hygiene. No complications were observed, such as suture dehiscence, infection, or tissue necrosis. One patient required reoperation in the postoperative period due to asymmetry of the labia minora. Another patient complained of excessive exposure of the clitoris, but she had preexisting clitoral hypertrophy. CONCLUSION: Labiaplasty using the wedge resection technique relieved personal discomfort and provided functional and aesthetic improvement. Due to the lack of validation and standardization of labiaplasty techniques, further clinical research should be performed.

RESUMO

INTRODUÇÃO: A ninfoplastia consiste na abordagem cirúrgica da hipertrofia dos pequenos lábios, que visa ao aperfeiçoamento da assimetria destes e do tecido redundante da vulva. A hipertrofia de pequenos lábios caracteriza-se por aqueles que, em posição e condições normais, sobressaem-se aos grandes lábios. A cirurgia tem sido proposta na presença de transtornos, como odor desagradável, infecção urinária, comprometimento da atividade sexual e/ou insatisfação pessoal, cuja indicação cirúrgica possui fins estéticos. O objetivo deste artigo é relatar nossa experiência com a ressecção em cunha dos pequenos lábios nas pacientes que apresentam tal hipertrofia. MÉTODOS: Foram analisados retrospectivamente 53 casos de ninfoplastia pela técnica de ressecção em cunha, no período de 2010 a 2015. Os resultados foram analisados quanto ao aspecto anatômico no pós-operatório, o grau de satisfação das pacientes e a interferência na atividade sexual. RESULTADOS: As pacientes se mostraram satisfeitas com o aspecto estético pós-cirúrgico. Houve melhora no desempenho sexual, na prática de atividades físicas e condições de higiene. Não foram verificadas complicações, como deiscência de sutura, infecção ou necrose tecidual. Uma paciente precisou ser reoperada devido à assimetria de pequenos lábios no pós-operatório. Outra paciente queixou-se da excessiva exposição do clitóris, porém o mesmo já apresentava discreta hipertrofia prévia. CONCLUSÃO: A ninfoplastia pela técnica de ressecção em cunha apresentou benefícios em relação ao desconforto pessoal, melhora funcional e estética. Devido à ausência de validação e padronização nas técnicas de ninfoplastia atuais, novos estudos clínicos devem ser realizados.

Z-plasty: a comparative study between theoretical and practical elongation

ABSTRACT

INTRODUCTION: The study of the final elongation values produced by 45º, 60º, 75º, and 90º-angle z-plasties performed on the skin of the lower abdomen of fresh cadavers were compared with the theoretical mathematical data published in the literature. METHODS: Four z-plasties with 2-cm main and secondary branches each, and with 45º, 60º, 75º, and 90º angles were performed on the skin of the lower abdomen in 11 fresh cadavers to evaluate the final elongation produced. RESULTS: The mean elongation at their respective angles was lower than that found in the literature, with a p value of <0.01. CONCLUSION: The elongation values obtained from the present study showed a significant difference from the published values.

INTRODUÇÃO: O estudo do alongamento final produzido por zetaplastias com ângulos de 45, 60, 75 e 90 graus na pele do abdome inferior de cadáveres frescos foi comparado com os dados matemáticos teóricos apresentados pela literatura. MÉTODOS: Foram realizadas quatro zetaplastias com ramos principais e secundários de 2 cm cada um deles e ângulos de 45, 60, 75 e 90 graus na pele do abdome inferior de 11 cadáveres frescos para avaliar o alongamento final produzido. RESULTADOS: A média dos alongamentos para os respectivos ângulos foi inferior ao encontrado na literatura, com valor de p < 0,01. Risk factors for infection in burn in children and adolescents: a cohort study

ABSTRACT

INTRODUCTION: Prevention of infection in burned patients poses great challenges such as infection, which constitutes the most common cause of mortality after burn injury. An analysis of burned patients younger than 20 years-old was performed prospectively to identify the incidence of nosocomial infection (NI) and risk factors for N. We included in the study those admitted and treated from January 2012 to December 2012 at a public burn unit in Brazil. METHODS: A total of 136 patients admitted at Burn Care Unit during the study period fulfilled inclusion criteria. We collected information related to NI and epidemiological data. RESULTS: Most patients were male (63.2%) and non-white (57.4%). Patients' mean age was 7.64 years. Scalding was the most frequent causal agent (45.6%). The mean body surface area (BSA) was 15.7%. Most of patients (80.9%) had two or more damaged areas. Surgical treatment with grafts was needed in 69 cases (50.7%). Infection was seen in 59 cases (43.4%), and sepsis in 27 (19.8%). Cultures were positive in 22.8%, and Staphylococcus aureus was the most frequent isolated agent (31.2%) found. Antimicrobial drugs were used in 45.6% of patients. The mean length of hospital stay was 22.36 days and mortality rate was 2.5%. According to the statistical analysis there was an association between infection and admission to the Intensive Care Unit (ICU), invasive procedures, grafts, length of hospital stay and BSA. CONCLUSION: Risk factors for infection in burn patients are length of hospital stay, BSA, invasive procedures, admission at ICU and grafts.

RESUMO

INTRODUÇÃO: A prevenção de infecções em pacientes queimados é grande desafio, pois trata-se da causa mais comum de mortalidade após queimaduras. Um estudo prospectivo em 136 pacientes queimados menores de 20 anos, internados e tratados entre janeiro e dezembro de 2012, foi realizado para identificar a incidência e fatores de risco para infecção hospitalar (IH). MÉTODOS: Pacientes internados na Unidade de Tratamento de Queimados que aceitaram participar e preencheram os critérios de inclusão, participaram do estudo. Foram coletadas informações referentes à IH e dados epidemiológicos durante o período de internação. RESULTADOS: Do total dos pacientes, a maioria era do gênero masculino (63,2%) e não branco (57,4%). A média de idade foi 7,64 anos. Escaldadura foi o agente causal mais frequente (45,6%). A média de superfície corporal queimada (SCQ) foi 15,7%. A maioria dos pacientes (80,9%) apresentou duas ou mais áreas acometidas por queimadura. O tratamento cirúrgico com enxertos foi necessário em 69 casos (50,7%). Ocorreu infecção em 59 pacientes (43,4%) e sepse em 27 (19,8%). As culturas foram positivas em 22,8% e Staphylococcus aureus foi o agente isolado mais frequente (31,2%). Antimicrobianos foram usados em 45,6% dos pacientes. O tempo médio de permanência hospitalar foi 22,36 dias e a taxa de mortalidade 2,5%. Houve associação entre infecção e admissão em Unidade de Terapia Intensiva (UTI), monitorização invasiva, enxertos, tempo de internação hospitalar e SCQ. CONCLUSÃO: Os fatores de risco para infecção em queimaduras são: tempo de internação hospitalar, superfície corporal queimada, monitorização invasiva, internação em UTI e enxerto.

Effects of radiofrequency on the healing of skin wounds in rats: analysis using digital planigraphy and histological evaluation

ABSTRACT

INTRODUCTION: Given the extensive use of plastic surgery and the search for better aesthetic and functional results, it is necessary to research ways to improve healing and scarring. The objective is to evaluate the effects of three radiofrequency (RF) sessions in healing the skin of mice. METHODS: Forty-eight rats were divided into four groups by day of sacrifice and treatment (RF group: RG; control group: CG). Dissection of the excisional wound of 2 cm x 2 cm (4 cm²) was performed and a 6-mm punch was used to hold two excisional wounds 0.6 cm in diameter. After 24 h, radiofrequency was performed using Spectra® directly on the wound in the dorsal region for 7 minutes at 38ºC. This was repeated three times on alternate days. For the control group, the radiofrequency protocol was performed with the device switched off. RESULTS: A larger area of the square wound was measured on postoperative day three in RG (RG7: 3.3 cm² ± 0.7 cm² vs. CG7: 2.4 cm² ± 0.4 cm²; p = 0.009). On day 14, the square wound in RG was greater than in CG (RG14: 1.9 cm² ± 0.5 cm² vs. CG14: 1.0 cm² ± 0.3 cm²; p = 0.001). There was a 90% closure of wounds in CG14. In RG14, 60% of the wounds were re-epithelized while 40% remained ulcerated. In CG7, 70% of the remaining wounds were ulcerated and 30% were re-epithelized. In RG7, 8% were re-epithelized and 92% remained ulcerated. CONCLUSION: Radiofrequency has a negative influence on the healing process, as indicated by mice that received radiofrequency having a persistent ulcerated wound.

RESUMO

INTRODUÇÃO: Tendo em vista o número de cirurgias plásticas e a busca por melhores resultados estético-funcionais fazem-se necessárias pesquisas para encontrar meios para melhorar a cicatrização e as cicatrizes. O objetivo é avaliar os efeitos de três sessões de radiofrequência na cicatrização da pele de ratos. MÉTODOS: Quarenta e oito ratos machos foram divididos em quatro grupos conforme ao grupo que pertenciam e o dia do sacrifício (grupo radiofrequência - GR - e grupo controle - GC). Realizada a dissecação da ferida excisional de 2 cm x 2 cm (4 cm²) e utilizou-se um punch de 6 mm para a realização de duas feridas excisionais de 0,6 cm de diâmetro. Após 24 h, foi realizada a radiofrequência com o equipamento Spectra® na região dorsal, diretamente sobre as feridas por 7 minutos, com temperatura de 38ºC. Repetida por três vezes, em dias alternados. No grupo controle foi realizada com o aparelho desligado. RESULTADOS: Foi encontrada área maior na ferida quadrada, no 3º dia pós-operatório do GR (GR7 3,3 cm² ± 0,7 cm² vs. GC7 2,4 cm² ± 0,4 cm², p = 0,009). No 14º dia a ferida quadrada do GR foi maior do que no GC (GR14 1,9 cm² ± 0,5 cm² vs. GC14 1,0 cm² ± 0,3 cm², p = 0,001). Houve fechamento de 90% das feridas no GC14. No GR14, 60% das feridas foram reepitelizadas enquanto 40% permaneceram ulceradas. No GC7, 70% das feridas de permaneceram ulceradas e 30% foram reepitelizadas. Já no GR7, 8%, foram reepitelizadas e 92% permaneceram ulceradas. CONCLUSÃO: A radiofrequência tem influência negativa no processo cicatricial, mostrando que, nos ratos que receberam a radiofrequência, o quadrado permaneceu ulcerado.

ABSTRACT

Seroma formation remains a common complication of abdominoplasty. The use of drain, flap fixation points, and fibrin glue has been described to reduce the incidence of seroma formation. The authors present herein an easy-to-reproduce technique to decrease the risk of bleeding and eliminate the devitalized tissue caused by the detachment. The proposed strategy consists of washing and debridement, which was conducted with mechanical friction using moist flap and abdominal wall dressings.

RESUMO

A formação de seroma após uma abdominoplastia continua sendo uma complicação frequente. O uso de dreno, os pontos de fixação do retalho e o uso da cola de fibrina foram descritos com a finalidade de diminuir a sua incidência. Os autores apresentam uma técnica de fácil reprodução para reduzir o risco de sangramento, bem como para eliminar o tecido desvitalizado decorrente do descolamento. A tática proposta constitui-se de lavagem e desbridamento por meio da fricção mecânica com compressas umedecidas do retalho e da parede abdominal.

General treatment and wound management in hereditary epidermolysis bullosa: indication and experience using silver hydrofiber dressing

ABSTRACT

INTRODUCTION: Hereditary epidermolysis bullosa (EB) is a rare disorder characterized by cutaneomucous fragility, with formation of blisters during minimal trauma. Treatment consists of clinical and nutritional support and management of pain and skin lesions. Silver hydrofiber (Aquacel Ag®) is a type of carboxymethylcellulose fiber dressing with silver that can be used in selected cases of EB. OBJECTIVE: To review the literature on the general treatment and management of cutaneous lesions in congenital EB and evaluate the indication and experience of using silver hydrofiber dressing. METHODS: The review included original articles and systematic reviews published between 2009 and 2014. We also selected two patients with congenital EB treated at the Plastic Surgery Division of Hospital das Clínicas of the Faculty of Medicine of Ribeirão Preto at the University of São Paulo. RESULTS: There is a shortage of scientific evidence related to the treatment of skin lesions in congenital EB, with most recommendations being based on expert opinions. Hydrofiber is indicated in most consensuses for wounds with some exudation and has been shown to be more absorbent than alginate. In our experience, there was apparent improved control of pain, bleeding, and hypothermia with the use of hydrofiber, which has the advantage of not requiring daily changes and can remain on the wound for up to two weeks. CONCLUSIONS: The general and lesion treatments in EB are challenging. Hydrofiber with silver is a treatment option for wounds in hereditary EB, without the need for daily dressing changes.

INTRODUÇÃO: Epidermólise bolhosa (EB) hereditária é uma desordem rara caracterizada pela fragilidade cutaneomucosa, com formação de bolhas ao mínimo trauma. O tratamento consiste em suporte clínico, nutricional, manejo da dor e das lesões cutâneas. A hidrofibra com prata (Aquacel Ag®) é um tipo de curativo de fibra de carboximetilcelulose e prata que pode ser utilizada em casos selecionados de EB. OBJETIVO: Revisão da literatura sobre o tratamento geral e o manejo das lesões cutâneas na EB congênita, além de avaliar a indicação e experiência usando curativo de hidrofibra com prata. MÉTODOS: A revisão incluiu artigos originais e revisões sistemáticas, publicados entre 2009 e 2014. Selecionamos ainda dois pacientes com EB congênita tratados na Divisão de Cirurgia Plástica do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo. RESULTADOS: Há escassez de evidências científicas relacionadas ao tratamento das lesões cutâneas na EB congênita, sendo a maioria das recomendações baseadas em opiniões de especialistas. A hidrofibra está indicada na maioria dos consensos para feridas com alguma exsudação e mostrou-se mais absorvente que o alginato. Em nossa experiência, houve aparente melhor controle da dor, do sangramento e da hipotermia com o uso da hidrofibra, que apresenta a vantagem de não necessitar de trocas diárias, podendo permanecer na ferida por até duas semanas. CONCLUSÕES: O tratamento geral e das lesões na EB é um desafio. A hidrofibra com prata é uma opção de tratamento para as feridas na EB hereditária, sem necessidade de trocas diárias de curativo.

ABSTRACT

Facial aging is a consequence of multiple intrinsic and extrinsic interactive factors, leading to loss of volume and repositioning of facial fat and bone remodeling. Breakthroughs have been seen in recent decades in the ability to reduce the signs of aging. Filling techniques and numerous products available on the market have generated new interest in the study of facial anatomy. Fillers are able to restore volume in limited areas and are suitable for treating facial wrinkles and loss of subcutaneous volume. The gold standard remains hyaluronic acid because it is more biocompatible and has greater permanence in the skin, without being definitive. The objective of this report is to review the anatomy of the middle third of the face and the main indications for filling and the techniques used for rejuvenation.

RESUMO

O envelhecimento facial é consequência de múltiplos fatores intrínsecos e extrínsecos que interagem entre si, levando à perda de volume e reposicionamento da gordura facial, assim como o remodelamento ósseo. Nas últimas décadas, houve um grande avanço na Medicina em reduzir os sinais do envelhecimento. As técnicas de preenchimento e os inúmeros produtos disponíveis no mercado trouxeram novo interesse no estudo da anatomia da face. Os preenchedores têm como função restaurar o volume de áreas restritas, portanto, são adequados para tratar rugas faciais e perda de volume subcutâneo. O padrão ouro atualmente é o ácido hialurônico, por ser mais biocompatível e ter uma permanência maior na pele, sem ser definitivo. O objetivo desse artigo é revisar a anatomia do terço médio da face, assim como as principais indicações de preenchimento e técnicas utilizadas para o rejuvenescimento.

ABSTRACT

The Gorlin-Goltz syndrome (GGS) is a hereditary, autosomal dominant condition, with high penetrance and variable expressivity, resulting from mutations in the genes PTCH1, PTCH2, or SUFU. The diagnosis is based on the presence of 2 major criteria or a major criterion associated with 2 minor criteria, including multiple basal cell carcinomas, keratocystic odontogenic tumor (KOT), and bifid rib. Other endocrine, neurological, ophthalmologic, genital, respiratory, and cardiovascular alterations are found in the literature, but with variable manifestations. This study reports the case of a patient diagnosed with GGS associated with diastolic congestive heart failure and type 2 diabetes mellitus, who underwent multiple treatments for components of the syndrome. More recently, the patient underwent decompression followed by cystic enucleation of two KOTs in the jaw, oral rehabilitation with removable prosthodontics, cardiological evaluation, and attempted clinical control of endocrine and cardiac problems.

RESUMO

A síndrome de Gorlin-Goltz (SGG) é uma condição hereditária, autossômica dominante, com alta penetrância e expressividade variável, decorrente de mutações nos genes PTCH1, PTCH2 ou SUFU. O diagnóstico é baseado na presença de dois critérios maiores ou um critério maior associado a dois critérios menores, dentre eles múltiplos carcinomas basocelulares, tumor odontogênico ceratocístico (TOC) e costela bífida. Outras alterações endócrinas, neurológicas, oftalmológicas, genitais, respiratórias e cardiovasculares são encontradas na literatura, porém com manifestações variáveis. O objetivo deste trabalho é relatar um caso clínico de uma paciente sistematicamente diagnosticada com SGG associada à insuficiência cardíaca congestiva diastólica e diabetes mellitus 2 submetida a tratamentos seriados das respectivas manifestações sindrômicas. Mais recentemente, à descompressão cística seguida da enucleação de dois TOC em mandíbula, reabilitação oral com prótese total removível, avaliação cardiológica e tentativa de controle clínico das alterações endócrinas e cardíacas.

Surgical item left in the abdomen six years after abdominoplasty: case report

ABSTRACT

The retained surgical item in patients after closure of the wound is a situation that although rare is preventable and requires specific care such as institutional protocols for prevention. We report a case of removal of an already encapsulated pads by fibrin tissue (textiloma) from a patient six years after an abdominoplasty, which formed a palpable mass in her abdomen. The retained surgical items lead to variable symptoms such as palpable masses, compressions, non-absorptive loss and, sometimes, severe complications. The diversity of manifestations combined with their few frequency, most of the times, lead to underdiagnosis. Treatment should be individualized for each case, although in case of symptoms removal is indicated in most cases. Surgical removal is associated with complications as longer as objects remain in patient's body.

RESUMO

A permanência de corpos estranhos em pacientes após o fechamento da ferida operatória é uma situação que, embora rara, é evitável e demanda cuidados específicos como protocolos institucionais de prevenção. O caso relata a retirada de uma compressa já encapsulada por tecido de fibrina (textiloma) de uma paciente seis anos após abdominoplastia, formando uma massa palpável em seu abdômen. A permanência desses itens cirúrgicos leva a sintomas variáveis como massas palpáveis, compressões, síndromes disabsortivas e, algumas vezes, graves complicações. A diversidade de manifestações combinada a sua pouca frequência levam, muitas vezes, ao subdiagnóstico. O tratamento deve ser individualizado para cada caso, embora na presença de sintomas a retirada é indicada na grande maioria das vezes. A cirurgia de retirada está mais associada a complicações quanto maior tempo de permanência dos objetos no corpo do paciente.

ABSTRACT

Perineal and gluteal regions may be affected by a wide spectrum of diseases, and the treatment may require extensive surgery and cause functional, aesthetic, and psychosocial damages at various levels. Wounds in the extensive perineal and gluteal regions after surgical treatment of neoplasia may represent a challenge in local reconstruction. Size, location, and availability of tissue around the lesion may hinder wound primary closure, requiring the use of one or more flaps. This article reports a case of reconstruction of the perineal and gluteal regions after oncological resection at the plastic surgery service of the Hospital das Clínicas at the Federal University of Minas Gerais. The muscle and fasciocutaneous flaps of the gluteus maximus and myocutaneous of the semimembranosus were used.

RESUMO

As regiões glútea e perineal podem ser afetadas por um variado espectro de doenças, cujo tratamento pode demandar extensas mutilações e acarretar em prejuízo funcional, estético e psicossocial em variados graus. Feridas extensas da região perineal e glútea após o tratamento cirúrgico de neoplasias podem representar um desafio para a reconstrução local. Tamanho, localização e disponibilidade de tecido em torno da lesão são fatores que podem impedir o seu fechamento primário, tornando necessário o uso de um ou mais retalhos. Este artigo relata um caso de reconstrução de períneo e região glútea após ressecção oncológica, no serviço de Cirurgia Plástica do Hospital das Clínicas da Universidade Federal de Minas Gerais (UFMG). Foram utilizados os retalhos muscular e fasciocutâneo de glúteo máximo e miocutâneo de semimembranoso.

Surgical treatment of complications resulting from adjuvant radiotherapy: a case report

ABSTRACT

INTRODUCTION: Breast cancer is the leading cause of death among malignancies affecting women. Treatment options range from surgical treatment, radiotherapy, chemotherapy and hormone therapy. The immediate breast reconstruction helps to benefit the psychosocial aspects of patients, however, depend on the technique used a number of complications can appear, especially after adjuvant radiotherapy. CASE REPORT: We report a case of a 65-year-old woman, underwent quadrantectomy and radiotherapy in 1988. In 2010, the patient presented an injury on the scar and she was referred to mastectomy with reconstruction of retail large dorsal and adjuvant radiotherapy. Upon examination, we observed flap necrosis with infection on the axillary region, lymphedema, radiodermatitis, fibroses and joint limitation. In 2014, the patient sought our service to perform a new restorative approach. A bipedicled transverse abdominal flap was decided to be adequate to her case. After surgery, the patient reported mild pain in the upper pole that managed with debridement and dressing. Seven months after surgery there was a complete healing of the flap and patient was satisfied with the surgery. CONCLUSION: The knowledge of surgical techniques associated with the correct scaling of steps are essential for surgical success and management of complications that may appear in breast reconstruction of patients undergoing radiotherapy.

RESUMO

INTRODUÇÃO: O câncer de mama é a principal causa de morte entre as mulheres por doenças malignas. As opções de tratamento variam entre a abordagem cirúrgica, radioterápica, quimioterápica e hormonal. A reconstrução mamária imediata atua em aspectos psicossociais das pacientes, contudo, a depender da técnica realizada, diversas complicações podem surgir, principalmente pós-radioterapia adjuvante. RELATO DO CASO: Paciente sexo feminino, 65 anos, submetida à quadrantectomia e radioterapia em 1988. Evoluiu com aparecimento de ferimento em local de cicatriz em 2010, sendo submetida à mastectomia com reconstrução com retalho grande dorsal imediata e radioterapia adjuvante. Apresentou necrose do retalho, com infecção em região axilar, linfedema, radiodermite, retração cicatricial intensa e limitação articular. Em 2014, procurou nosso serviço para nova abordagem reparadora. Indicado retalho transverso do abdome bipediculado. No seguimento pós-operatório houve pequeno sofrimento em polo superior manejado com desbridamentos e curativos. Após sete meses de pós-operatório, houve cicatrização completa do retalho e satisfação da paciente com procedimento cirúrgico. CONCLUSÃO: O conhecimento das técnicas cirúrgicas, associado ao correto escalonamento, é fundamental para um bom sucesso cirúrgico e manejo das complicações que venham a acontecer nas reconstruções mamárias em pacientes submetidas à radioterapia.

Abdominal dermocutaneous flap for reconstruction of a bleeding area in necrotizing fasciitis

ABSTRACT

INTRODUCTION: The sequelae caused by Fournier's syndrome are extensive, systemic, debilitating, and deforming. They require both general and local treatments with antibiotics and subsequent debridement to delimit non-necrotic margins, which result from extensive bleeding areas. METHODS: The area affected and described in this study is the upper third of the right thigh. The goal was to use the dermocutaneous excess, usually removed in abdominoplasties. Abdominoplasty was routinely performed, and the cutaneous excess of the hypogastrium was transferred to cover a bleeding area in the upper third of the thigh throughout its extension. In the bleeding region, adequate occlusion was performed, and a tissue with similar thickness and elasticity was used to minimize the deformities that affected both the donor and receiving areas. RESULTS: The bleeding area was covered in a single surgery. CONCLUSION: The use of the cutaneous excess of the hypogastrium to cover the bleeding region of the upper third of the thigh is an alternative technique, which can be conducted in a single surgery in patients presenting excess abdominal skin.

RESUMO

INTRODUÇÃO: As sequelas decorrentes da síndrome de Fournier são extensas, de repercussão sistêmica, debilitantes e deformantes, que exigem tratamento geral e local com o uso de antibióticos e sucessivos desbridamentos para delimitar os limites sem necroses resultando áreas cruentas e extensas. MÉTODOS: O caso presente envolveu o terço superior da coxa direita. A proposta foi a de utilizar o excedente dermocutâneo, que normalmente é eliminado nas abdominoplastias. A abdominoplastia foi realizada dentro da rotina em que este excedente cutâneo do hipogástrio foi transferido para cobrir a área cruenta em toda a sua extensão no terço superior da coxa. A oclusão foi adequada na área cruenta, com um tecido semelhante em espessura e elasticidade, minimizando a deformidade tanto na área doadora quanto na receptora. RESULTADOS: Foi obtida total cobertura da área cruenta num único tempo operatório. CONCLUSÃO: A utilização dos excessos cutâneos do hipogástrio para recobrir a área cruenta no terço superior da coxa segue como mais uma opção técnica num único tempo operatório em pacientes com excesso cutâneo abdominal.

ABSTRACT

The study of nasal morphology is of fundamental importance for a better understanding and planning of rhinoplasty. The objective of this study is to analyze the basic shape of the nose seen as an isolated structure, and especially in equilibrium with the patient's face. The existing patterns to do so are quite subjective, since the concept of beauty depends on several factors. The classic proportion measurements assist the surgeon with the conception and aesthetic view. When combined with an understanding of the anatomy and surgical technique they determine the final outcome of rhinoplasty. Thus, the concept of "optimal" nose must be applied on an individual basis, respecting classical anthropometry patterns, gender, ethnic-racial, cultural and psychosocial factors. Morphology is not limited only to the static aspect of the nose, but is directly related to personal dynamics, mimicry and personality of the patient.

RESUMO

O estudo da morfologia nasal é de fundamental importância para um melhor entendimento e planificação das rinoplastias. O objetivo deste trabalho é analisar a forma básica do nariz visto como estrutura isolada, e principalmente em equilíbrio com a face do paciente. Os padrões determinados são bastante subjetivos, pois o conceito de beleza depende de vários fatores. As medidas clássicas de proporção auxiliam na concepção e visão estética do cirurgião. E quando associadas ao conhecimento da anatomia e técnica cirúrgica irão determinar o resultado final da rinoplastia. Logo, o conceito do nariz "ideal" deve ser aplicado para cada um, respeitando os padrões clássicos da antropometria, o sexo, grupo étnico-racial, fatores culturais e psicossociais. A morfologia não se restringe apenas ao aspecto estático do nariz, mas relaciona-se diretamente com a dinâmica pessoal, a mímica e a personalidade.
