Diced cartilage graft to increase the nasal radix

ABSTRACT

INTRODUCTION: The main complaint of patients visiting plastic surgery clinics for rhinoplasty is the nasal dorsum. Cartilage grafts are routinely used in the nasal radix to reduce inappropriate nasofacial angles or redefine the transition point of the radix and then lengthen or shorten the nose. The aim of this study is to present cartilage nasal radix grafts prepared with diced cartilage and introduced to the radix with the aid of a syringe. METHOD: We evaluated the surgical procedure and the results obtained by analyzing the medical records of 37 patients who underwent this surgery between May 1, 2011 and May 1, 2013. RESULTS: In all cases, we achieved the expected result of successfully increasing the nasal radix. No graft absorption was clinically detected. CONCLUSION: The use of a diced cartilage graft as described in this study effectively increased the nasal radix.

RESUMO

INTRODUÇÃO: A principal queixa dos pacientes que procuram as clínicas de cirurgia plástica para serem submetidos a rinoplastia é o dorso nasal. A utilização de enxerto cartilaginoso em radix nasal é rotineira e suas indicações habituais são para diminuir o ângulo nasofacial inadequado ou para redefinir o ponto de transição do radix, para alongamento ou encurtamento do nariz. Este trabalho tem o intuito de apresentar a enxertia de cartilagem do radix nasal, com cartilagem fragmentada e introduzida até o radix com seringa. MÉTODO: Foi analisado o ato cirúrgico e seu resultado através da revisão de prontuários dos pacientes submetidos ao ato cirúrgico no período de 1/maio/2011 a 1/maio/2013, totalizando 37 casos. RESULTADOS: Em todos os casos, conseguiu-se o resultado planejado de aumento do radix nasal com sucesso. Não foi observada absorção clinicamente detectável do enxerto. CONCLUSÃO: A utilização do enxerto de cartilagem fragmentada realizada da forma descrita neste trabalho mostrou-se eficaz para a elevação do radix nasal.

Use of Bernard-Webster flap for lower lip reconstruction after excision of squamous cell carcinoma: analysis of functional results

ABSTRACT

INTRODUCTION Defects in the lower lips after the excision of squamous cell carcinoma (SCC) are usually repaired using several surgical techniques. However, the functional reconstruction of full-thickness defects in the lower lip remains a challenge. Therefore, the objectives of this study were to describe surgical strategies for the reconstruction of full-thickness defects of the lower lip after surgical excision of the SCC and evaluate the functional results. METHODS: This was a retrospective study of all patients with full-thickness defects in the lower lip after excisions of SCC that were greater than one third of the lip that were repaired using the Bernard-Webster flap between 2011 and 2014. The functional results (sphincteric, motor and sensory function) were evaluated according to previously used criteria. RESULTS: Six lip defects were reconstructed without complications using Bernard-Webster flaps. During the postoperative period, the patients reported liquid incontinence (16.67%), intermittent salivary incontinence (16.67%), and difficulty fully opening the mouth (33.33%). The tests revealed sensory deficits in the labiomental areas (100%) and lower lip (33.33%). There was full recovery of all functional changes an average of 3 months after surgery. The observed late functional results were considered satisfactory. CONCLUSIONS: The Bernard-Webster flap was an excellent alternative to repairing the defects that affect more than one third of the lower lip since it allowed the use of similar local tissues, a single surgery could be performed, and the result was functionally efficient.

RESUMO

INTRODUÇÃO Defeitos dos lábios inferiores após exéreses de carcinoma espinocelular (CEC) vêm sendo reparados com inúmeras técnicas cirúrgicas. No entanto, a reconstrução funcional de defeitos de espessura total do lábio inferior continua a ser um desafio. O objetivo deste estudo, portanto, foi descrever as estratégias cirúrgicas para a reconstrução de defeitos de espessura total do lábio inferior, subsequentes a exéreses cirúrgicas de CEC, bem como avaliar resultados funcionais. MÉTODO: Trata-se de um estudo retrospectivo de todos os pacientes com defeitos de espessura total de lábio inferior após exérese de CEC maiores que um terço do lábio, reconstruídos com retalhos de Bernard-Webster, no período de 2011 a 2014. Os resultados funcionais (funções esfincteriana, motora e sensorial) foram avaliados de acordo com critérios previamente utilizados. RESULTADOS: Seis defeitos labiais foram reconstruídos com retalhos de Bernard-Webster, sem complicações. No período pós-operatório, os pacientes relataram incontinência para líquidos (16,67%), incontinência salivar intermitente (16,67%) e dificuldade para abrir a boca completamente (33,33%). Os testes revelaram déficits sensoriais nas regiões mentoniana (100%) e de lábio inferior (33,33%). Houve recuperação completa de todas as alterações funcionais, em média, três meses após as intervenções cirúrgicas. Os resultados funcionais tardios foram considerados satisfatórios. CCONCLUSÕES: O retalho de Bernard-Webster mostrou-se uma excelente alternativa para reparar defeitos que comprometem mais de um terço do lábio inferior, pois permitiu a utilização de tecidos vizinhos semelhantes, pôde ser realizado em tempo único e foi funcionalmente eficaz.

Unique surgery for the upper body segment in patients with massive weight loss: thoracobrachio-mammoplasty

ABSTRACT

INTRODUCTION Ex-obese patients require a complex surgical approach because of the large amount of excess skin due to their massive weight loss. In some cases, several plastic surgeries are needed, and there is no existing standard in the coordination of these surgeries. In the upper segment of the body, the arms, side of the thorax, and breasts are usually affected, mainly in women. Several techniques have been developed with the aim of achieving better results with better hidden scars. Some techniques may be associated, being carried out in a single surgical procedure. A well-structured surgical team leads to a reduced surgical time, which means higher safety for the patient. METHOD: We present a technique for standardization in the treatment of ex-obese patients that is performed in a single step, comprising mammoplasty (according to Pitanguy's technique or with placement of breast prosthesis), thoracoplasty (with the removal of excess skin on the side of the chest), and brachioplasty (performed with a rectilinear drawing at the lowest part of the arms). RESULTS: Seven cases were evaluated in terms of surgical time, location of the scars, and final shape and symmetry. The complications included partial dehiscence (14%) and hypertrophic scars (14%). The aesthetic result was satisfactory for 84% of the patients; on the other hand, the quality of cicatrization, keloid, and hypertrophic scars were the major causes of dissatisfaction. CONCLUSION: The use of the thoracobrachio-mammoplasty technique in a single surgical time was effective in the treatment of ex-obese patients, offering yet another option among other surgeries that these patients usually need.

RESUMO

INTRODUÇÃO Os pacientes ex-obesos necessitam de uma abordagem complexa, diante do grande excesso de pele decorrente da perda ponderal. Em alguns casos, muitas cirurgias plásticas são necessárias, não havendo uma padronização na associação destas cirurgias. No segmento superior do corpo, a região dos braços, a lateral do tórax e as mamas normalmente são acometidas, principalmente nas mulheres. Diversas técnicas foram desenvolvidas com o objetivo de alcançar resultados melhores e com cicatrizes mais escondidas. Algumas técnicas podem ser associadas, sendo realizadas em um único tempo cirúrgico. Quando a equipe é bem estruturada, o tempo cirúrgico é reduzido, significando mais segurança para o paciente. MÉTODO: Os autores apresentam uma técnica que oferece padronização no tratamento do ex-obeso, que é realizada em tempo único: a Mamoplastia (pela técnica de Pitanguy ou com aposição de prótese mamária), a Toracoplastia (com a retirada do excesso de pele na lateral do tórax) e a Braquioplastia (realizada com um desenho retilíneo na parte mais inferior dos braços). RESULTADOS: Os sete casos foram avaliados quanto ao tempo cirúrgico, à localização das cicatrizes, à forma final e à simetria. Entre as complicações, houve deiscências parciais (14%) e cicatrizes hipertróficas (14%). O resultado estético foi satisfatório para os pacientes em 84% dos casos, sendo que a qualidade da cicatrização do paciente, queloide ou cicatrizes hipercrômicas, foi a maior causa de insatisfação. CONCLUSÃO: A utilização da técnica de Toracobraquio-mamoplastia em um único tempo se mostrou efetiva no tratamento do ex-obeso, oferecendo mais uma opção, diante das outras cirurgias que estes pacientes normalmente necessitam.

Surgical treatment of 510 patients with highly active antiretroviral therapy-associated lipodystrophy

ABSTRACT

INTRODUCTION The treatment of acquired immunodeficiency syndrome has undergone important advances since the introduction of highly active antiretroviral therapy (HAART). This treatment led to the elimination of the virus in the bloodstream and increased survival; however, metabolic and structural changes became evident. One of these changes is lipodystrophy, the redistribution of body fat. With one of the largest samples worldwide, the aim of this work was to present some of the various surgical alternatives as well as the results obtained for minimizing the impact of lipodystrophy. METHOD: From July 2005 to July 2013, 510 patients with HAART-associated lipodystrophy underwent surgery in the Clinic of Plastic Surgery, Heliópolis Hospital. All patients submitted to prior clinical and immunological assessments made with the aid of the infectious diseases team. The present study was approved by the Research Ethics Committee of the ABC Foundation. RESULTS: The 510 patients included 335 women and 175 men with an age range of 16-74 years. Liposuction of the cervicodorsal fat pad (buffalo hump) was predominant (199 cases). With regard to the response stimulated through a subjective questionnaire, a high degree of satisfaction was observed with a significant increase in self-esteem and greater adherence to antiretroviral treatment. CONCLUSION: The surgical correction of body lipodystrophy demonstrably improves the aesthetics of patients using HAART; however, its psychological and social effects are even more important since self-esteem increases and stigma decreases, which leads to better adherence to antiretroviral treatment.

RESUMO

INTRODUÇÃO O tratamento da Síndrome da Imunodeficiência Adquirida vem sofrendo importantes avanços desde a introdução da terapia antirretroviral altamente ativa, conhecida como HAART (high active antirretroviral therapy). Este tratamento levou à eliminação do vírus na corrente sanguínea e ao aumento na sobrevida, entretanto alterações metabólicas e estruturais tornaram-se evidentes. Uma dessas alterações é a redistribuição de gordura corpórea, também denominada lipodistrofia. Com uma das maiores casuísticas mundiais, o objetivo deste trabalho é demonstrar algumas das alternativas cirúrgicas, bem como os resultados obtidos na tentativa de minimizar o impacto da lipodistrofia. MÉTODO: No período de julho de 2005 a julho de 2013, 510 pacientes portadores de lipodistrofia secundária ao uso de HAART foram operados pela Clínica de Cirurgia Plástica do Hospital Heliópolis. Todos esses pacientes foram submetidos à prévia avaliação clínica e imunológica, sob auxílio da equipe de Infectologia. O presente trabalho foi aprovado pelo Comitê de Ética em Pesquisa da Fundação do ABC. RESULTADO: Dentre os 510 pacientes, 335 eram do sexo feminino e 175 do sexo masculino, com idades variando entre 16 e 74 anos. Quanto aos procedimentos, destacou-se lipoaspiração da giba e dorso, com 199 casos. Quanto à resposta estimulada através de questionário subjetivo, observou-se elevado grau de satisfação, aumento significativo da autoestima e maior adesão ao tratamento antirretroviral. CONCLUSÃO: A correção cirúrgica da lipodistrofia corporal comprovadamente melhora o aspecto estético do paciente que faz uso da HAART; porém, o efeito psicológico e social é ainda mais importante, elevando a autoestima, com diminuição dos estigmas, e proporcionando uma maior adesão ao tratamento antirretroviral.

Giant lipomas: a 14-case series

ABSTRACT

INTRODUCTION: Lipomas are the most common benign mesenchymal tumors worldwide. To be considered giant, they must be at least 10 cm in one of its dimensions or weigh at least 1,000 g. The objective of this study is to analyze a series of cases of giant lipomas seen in our practice. METHOD: We conducted a retrospective analysis of the medical and anatomopathological records of patients diagnosed with lipomas who underwent surgery between January 2003 and January 2010. RESULTS: We evaluated 14 patients with a mean age of 52.3 years (range, 33-72 years) and a mean evolution time until treatment of 4.4 years (range, 6 months to 20 years); 64% of cases presented no symptoms. All patients underwent surgical tumor resection. The average weight of the pieces was 3.8 kg (range, 512 g to 22 kg), while the average length of the longest axis was 27.5 cm (range, 15-66 cm). Postoperative complications were observed in 42% of cases. CONCLUSION: We observed prolonged periods of evolution until treatment, even in cases of large-volume lesions. Open surgical excision should be considered the treatment of choice for these tumors.

INTRODUÇÃO: Os lipomas são os tumores mesenquimais benignos mais frequentes na população. Para ser considerado gigante, deve apresentar pelo menos 10 cm em uma de suas dimensões ou pesar ao menos 1.000g. O objetivo do trabalho é analisar uma série de casos de lipomas gigantes atendidos em nosso serviço. MÉTODO: Análise retrospectiva de prontuários e registros anatomopatológicos dos diagnósticos de lipomas submetidos a tratamento cirúrgico no período de janeiro 2003 a janeiro 2010. RESULTADOS: Foram avaliados 14 pacientes, com média de idade de 52,3 anos (33-72) e tempo médio de evolução até o tratamento de 4,4 anos (seis meses - 20 anos); 64% dos casos não apresentavam sintomas. Todos os pacientes foram submetidos à ressecção cirúrgica da neoplasia. O peso médio das peças foi de 3,8 kg (512 g - 22 kg) e o tamanho médio no maior eixo foi de 27,5 cm (15-66 cm). Foram observadas complicações pós-operatórias em 42% dos casos. CONCLUSÃO: Observamos períodos prolongados de evolução até o tratamento, mesmo em casos com lesões de grande volume. A exérese cirúrgica aberta deve ser considerada o tratamento de eleição para estes tumores.

Reconstruction of a burned nipple-areola complex using an autonomized star flap

ABSTRACT

INTRODUCTION Reconstruction of the nipple-areola complex is the final stage of breast reconstruction. The most common nipple reconstruction techniques use local flaps or grafts. However, these techniques in cases of burns produce undesirable outcomes due to the decreased vascularization of damaged skin. The objective of this work was to evaluate the use of the autonomized star flap in the nipple reconstruction of burned breasts. METHODS: Nipples were reconstructed in eight female patients in two surgeries each. RESULTS: There were no complications such as necrosis, dehiscence, complete loss of projection of the new nipple, or infection. The mean projection at the end of surgery was 15.25 mm; 6 months after reconstruction, it was 3 mm, showing stability. All patients were satisfied with their results. CONCLUSION: We conclude that autonomization leads to safe reconstruction of the nipple on burned breasts and maintains a satisfactory minimal projection of the reconstructed nipple.

RESUMO

INTRODUÇÃO A reconstrução do complexo areolomamilar é a fase final da reconstrução mamária. As técnicas de reconstrução do mamilo mais utilizadas são com retalhos locais ou enxertos. A utilização destas técnicas em mamas que sofreram queimaduras apresenta resultados indesejados, em decorrência da menor vascularização da pele lesada. O objetivo deste trabalho foi avaliar a utilização do retalho trilobado autonomizado na reconstrução do mamilo em mamas queimadas. MÉTODOS: Oito pacientes do sexo feminino tiveram seus mamilos reconstruídas em dois tempos cirúrgicos. RESULTADOS: Não ocorreram complicações, como necrose, deiscência, perda completa da projeção do novo mamilo ou infecção. A projeção média ao término da cirurgia foi de 15,25 mm e, após seis meses de reconstrução, foi de 3 mm, permanecendo estável. Todas as pacientes ficaram satisfeitas com os resultados. CONCLUSÃO: Concluímos que a autonomização deu segurança à reconstrução do mamilo em mamas queimadas e manteve projeção mínima satisfatória do mamilo reconstruído.

Star nymphoplasty: a surgical technique for labia minora hypertrophy

ABSTRACT

INTRODUCTION: In recent years, women have been frequently consulting the plastic surgeon concerning the shape and dimensions of the labia minora and other surrounding structures, not only for aesthetic but also functional reasons, e.g., dyspareunia. The authors present a technique for the aesthetic and functional treatment of the external female genitalia, the vulva, with the purpose of correcting hypertrophy of the labia minora (nymphs), without changing their form. METHOD: This technique consists in the incision in the shape of a star leading to a shortening of nymphs both anteroposteriorly and craniocaudally. This is a 15-year retrospective study, with clinical and surgical follow-up of 64 female patients with an age range between 14 and 58 years, all submitted to star nymphoplasty, the technique proposed in this study. RESULTS: The results were obtained from the patients operated from January 1996 to December 2011, at the Hospital da Lagoa and at Interclínica-Centroplástica, Jardim Botânico, Rio de Janeiro, RJ. A high rate of patient satisfaction with the size and aesthetic shape of the genitalia was achieved. There was a low rate of complications and remaining complaints. CONCLUSION: This surgical procedure is done with the objective of reshaping the tissue structure of the hypertrophic labia minora. From a technical point of view, it can be considered as a simple and effective functional and aesthetic treatment of the female genitalia.

RESUMO

INTRODUÇÃO: O cirurgião plástico tem sido consultado com frequência, nos últimos anos, sobre a forma e as dimensões dos pequenos lábios vulvares e outras estruturas circunvizinhas, não só pela questão estética, mas também pela funcional, da dispareunia. Os autores apresentam uma técnica de tratamento estético e funcional da genitália externa feminina, a vulva, com o intuito de corrigir a hipertrofia dos pequenos lábios (ninfas), sem alterar-lhes a forma. MÉTODO: A técnica consiste na abordagem do pequeno lábio vulvar, com uma incisão em forma de estrela, produzindo um encurtamento das ninfas tanto ântero-posteriormente como crânio-caudalmente. O estudo é de tipo retrospectivo de um período de 15 anos, com seguimento clínico e cirúrgico de 64 pacientes de sexo feminino, com uma faixa etária entre 14 e 58 anos, todas submetidas à ninfoplastia em estrela, técnica proposta neste estudo. RESULTADOS: Os resultados foram obtidos da ficha dos pacientes operados no período compreendido entre janeiro de 1996 e dezembro de 2011, no Hospital da Lagoa e na Interclínica-Centroplástica, Jardim Botânico, Rio de Janeiro-RJ. Alto índice de satisfação das pacientes com o tamanho e o formato estético da genitália. Houve uma baixa taxa de complicações e queixas residuais. CONCLUSÃO: Este ato cirúrgico é feito com o objetivo de remanejar a estrutura tecidual hipertrófica dos pequenos lábios vulvares. De um ponto de vista técnico, pode-se considerar como um procedimento simples e eficaz no tratamento tanto funcional como estético da genitália feminina.

Morphometric study of the latissimus dorsi muscle in human fetuses

ABSTRACT

INTRODUCTION: The latissimus dorsi muscle (LDM) is broad and triangular and performs extension, adduction, and internal rotation motions of the arm. The LDM is vascularized by the thoracodorsal vessels and perforating branches of the posterior intercostal and lumbar arteries and is considered a type V Mathes and Nahai flap that is of great applicability in plastic surgery. The goal of the present study was to analyze the morphometry and vascularization of LDM in human fetuses. METHOD: The axillary region and the LDM of eight human fetuses (three female, five male) were dissected at 20-32 weeks' gestation in the supine position with full arm abduction. We measured the subscapular and thoracodorsal vessel lengths and conducted a muscle morphometric analysis. RESULTS: In all fetuses, the thoracodorsal vessels conferred primary vascularization of the LDM. In 25%, the subscapular vein was directly confluent with the axillary vein; 25% of the cases presented with a double scapular circumflex vein. The branch to the serratus anterior muscle was unique in all cases. In 50% of cases, the angular branch of the thoracodorsal artery was visualized, while 25% of them came from the branch to the serratus anterior muscle. The distances between muscle insertion and input of the neurovascular pedicle were 1.1 and 1.9 cm in fetuses 21 and 26 weeks, respectively. The anterior muscle margin was on the mid-axillary line in all fetuses. CONCLUSION: The observed constant morphometry and reduced anatomical variation of the vascular pedicle enabled the possibility of research involving the use of LDM in intrauterine surgical reconstructions.

INTRODUÇÃO O músculo latíssimo do dorso (MLD) é largo, triangular e realiza extensão, adução e rotação medial do braço. É vascularizado pelos vasos toracodorsais e ramos perfurantes das artérias intercostais posteriores e lombares, configurando retalho tipo V de Mathes e Nahai, de grande aplicabilidade em cirurgia plástica. O objetivo é analisar a morfometria e a vascularização do MLD em fetos humanos. MÉTODO: Dissecou-se a região axilar e o MLD de oito fetos humanos formolizados (três do sexo feminino e cinco do sexo masculino), entre 20 e 32 semanas gestacionais, em decúbito dorsal e abdução completa do braço. Mensuraram-se os comprimentos dos vasos subescapulares e toracodorsais, e foi realizada a morfometria do músculo. RESULTADOS: Em todos os fetos, os vasos toracodorsais conferiram a vascularização primária do MLD. Em 25%, a veia subescapular era tributária direta da veia axilar; 25% dos casos apresentaram veia circunflexa da escápula dupla. O ramo para o músculo serrátil anterior foi único em todos os casos. Em 50% dos casos, o ramo angular da artéria toracodorsal foi visualizado e, em 25% deles, era proveniente do ramo para o músculo serrátil anterior. A distância entre a inserção do músculo e a entrada do pedículo neurovascular variou entre 1,1 e 1,9 cm em fetos de 21 e 26 semanas, respectivamente. Todos os fetos apresentaram a margem anterior do músculo na linha axilar média. CONCLUSÃO: A morfometria constante e a reduzida variação anatômica do pedículo vascular encontradas possibilitam a realização de pesquisas envolvendo o uso do MLD em reconstruções cirúrgicas intraútero.

Liposuction-related complications and death: survey conducted by the BSPS commission on liposuction

ABSTRACT

INTRODUCTION: Liposuction is a surgical procedure that was developed approximately 30 years ago by Fournier, Illouz, and others. In 2011, a total of 211,108 liposuction procedures were performed in Brazil versus 325,332 in the United States. Over the years, liposuction performed with the tumescent technique has had a good safety profile. It is also performed by other specialists, including dermatologists, and has been described as an outpatient procedure in public institutions in Brazil. The scarcity of data on the subject has led the Brazilian Society of Plastic Surgery (BSPS) to create a commission composed of 10 full members with the aim of investigating the factors involved in severe liposuction-related complications and death in Brazil. METHODS: An online questionnaire was created using an anonymous link on the BSPS website. The questionnaire was sent to the registered email addresses of 4,441 associate and full members of the BSPS. The first question was about the occurrence of liposuction-related complications. The respondents could only continue to answer the questionnaire if the answer to this question was "yes." In addition to the questionnaire, the BSPS sent letters to the official state and federal institutions in which the case or complication/death-related records and/or deaths as a consequence of liposuction were expected to exist. The significance of the association between death and the various factors was determined using the chi-square test. The significant variables were included in Poisson Regression Models. The significance level was set at 5%. The data were analyzed using STATA software version 10.0. RESULTS: Thromboembolic events, combined surgical procedures, and the facilities where the procedure was performed were risk factors involved in liposuction-related mortality. CONCLUSION: The implementation of programs that aim to standardize liposuction procedures may effectively reduce liposuction-related mortality rates. This study aimed to make a positive educational impact on the development of future safe liposuction protocols.

RESUMO

INTRODUÇÃO: A lipoaspiração, procedimento cirúrgico com cerca de 30 anos desde sua sistematização por Fournier, Illouz e outros, atingiu, em 2011, o total de 211.108 cirurgias realizadas no Brasil e 325.332, nos Estados Unidos. Ao longo dos anos, foi reputado como procedimento seguro, baseado em conceitos da técnica tumescente. Realizado também por outras especialidades, já foi descrito como método ambulatorial, inclusive em instituições públicas no Brasil, por dermatologistas. Motivada por essa lacuna de dados, a Sociedade Brasileira de Cirurgia Plástica criou uma comissão composta por dez membros titulares, com o objetivo de realizar uma pesquisa para reconhecer a presença de possíveis fatores envolvidos nas intercorrências graves e nos óbitos, após uma cirurgia de lipoaspiração no Brasil. MÉTODOS: Foi elaborado um questionário on-line através de um link anônimo e disponível para consulta, no site da SBCP. O questionário foi enviado, através dos e-mails de cadastro, para 4.441 membros associados e titulares da SBCP. A primeira questão indagava sobre a ocorrência de intercorrências em lipoaspiração. Somente a resposta positiva era então direcionada ao restante do questionário. Além do questionário, foram enviadas cartas pela SBCP às instituições oficiais estaduais e federais, nas quais prováveis registros de processos ou cadastro de fatos relacionados a intercorrências e/ou óbitos após lipoaspiração poderiam existir. A significância da associação entre óbito e os diversos fatores foi medida com aplicação do teste qui-quadrado. As variáveis significantes foram incluídas em Modelos de Regressão de Poisson. Foi considerado o nível de significância de 5%. Os dados foram analisados com auxílio do programa STATA versão 10.0. RESULTADOS: Fenômenos tromboembólicos, associação de cirurgias e o local de realização da cirurgia demostraram ser fatores de risco envolvidos na mortalidade de lipoaspiração. CONCLUSÃO: Programas visando à normatização na realização das lipoaspirações podem ser eficazes na diminuição da mortalidade em lipoaspiração. Acreditamos que este artigo pode promover um impacto positivo educacional e, futuramente, gerar protocolos de segurança em lipoaspiração.

ABSTRACT

INTRODUCTION: Several reduction mammoplasty and mastopexy techniques are described in the literature, with the aim of reconstituting the upper pole, offering better projection, and providing adequate treatment for breast ptosis. However, particular attention should be devoted to the safety of the technique, with maintenance of vascularization, tissue innervation, and the capability of breastfeeding. Female patients operated on with the inferior pedicle flap technique were compared with those operated on with purse-string circular suturing. METHODS: Eighty-five patients who had undergone reduction mammoplasty or mastopexy without implants, between January 2011 and December 2012 at Unicamp's Clinical Hospital, were evaluated. Thirty-one patients who only underwent reduction mammoplasty by Pitanguy's technique (without the use of flaps or circular sutures) were excluded. Of the remaining 54 patients, five were subsequently excluded for not attending medical appointments or failure to have postoperative ultrasonography. A group of 16 patients who had undergone circular suturing and a group of 33 operated on by the inferior pedicle technique were considered. RESULTS: Demographic data were similar for both groups. A higher number of minor complications and unsatisfactory results were observed in the group that underwent the inferior pedicle technique, who also had a higher rate of relevant post-operative ultrasonography events. CONCLUSION: The circular suturing technique resulted in a high satisfaction rate, lower number of complications, and longer lasting results than the inferior pedicle technique, during the period analyzed in this study.

RESUMO

INTRODUÇÃO: Diferentes técnicas para mamoplastia redutora e mastopexia são descritas na literatura, visando a resultados que reconstituam o polo superior, ofereçam melhor projeção e proporcionem tratamento adequado para a ptose mamária. No entanto, devemos nos atentar para a segurança da técnica, com manutenção da vascularização, inervação dos tecidos e possibilidade de amamentação. Análise comparativa com pacientes operadas pela técnica com retalho de pedículo inferior e pacientes operadas pela técnica com sutura circular em bolsa. MÉTODOS: Análise de 85 pacientes submetidas à mamoplastia redutora ou mastopexia sem implantes, entre janeiro de 2011 e dezembro de 2012, no Hospital de Clínicas da Unicamp. Foram excluídos 31 pacientes, as quais foram submetidas apenas à mamoplastia redutora pela técnica de Pitanguy (sem utilização de retalhos ou sutura circular). Dentre as 54 pacientes restantes, cinco foram posteriormente excluídas devido ao não comparecimento à consulta ou à não realização da ultrassonografia pós-operatória. Foram agrupadas 16 pacientes submetidas à sutura circular contínua e 33 pacientes operadas pela técnica de pedículo inferior. RESULTADOS: Dados demográficos foram semelhantes nos dois grupos. Maior número de pequenas complicações e resultados insatisfatórios foi observado no grupo submetido à técnica de pedículo inferior, bem como maior índice de achados ultrassonográficos relevantes no pós-operatório. CONCLUSÃO: A técnica de sutura circular contínua apresentou elevado índice de satisfação, menor número de complicações e resultados mais duradouros quando comparados com a técnica de pedículo inferior, durante o período analisado.

Results of the correction of syndactyly of the hand by using Skoog's technique

ABSTRACT

INTRODUCTION: Syndactyly is a congenital anomaly of the hand, occurring in approximately 1 in every 2000 births. It is more common in male patients and is present in both hands in 50% of the cases. The objective of this study was to describe a series of cases of patients with hand syndactyly, operated at the Sarah Hospital in Brasília using modified Skoog's technique and orthosis. METHODS: The modified Skoog's technique was performed to correct syndactyly. Wrist-hand and finger orthosis was applied postoperatively, and the appearance of the scar was assessed according to the Vancouver Scar Scale. RESULTS: Forty-five patients were assessed during five years, of whom 30 (66%) were male and 15 were female, all aged between 1 and 4 years (mean age: 2 years [60%]); 37% of cases were associated with syndromes and 50% were simple bilateral cases. Complications observed in simple syndactyly occurred in 8% of patients and included scar retraction, visible scars, and ungual changes; three patients (6%) underwent revision surgery with z-plasty and confection of a new web space commissure to correct scar retraction, and four patients required finger realignment, observed in complex syndactyly. CONCLUSION: This technique resulted in 92% good functional outcomes, 8% complications associated with the surgical technique, and three revision procedures with z-plasty and confection of the web space commissure with a new flap and skin graft to correct scar retraction. The Vancouver scale was used to assess scar quality, and pigmentation between the fingers (due to skin graft) had the highest score.

RESUMO

INTRODUÇÃO: A sindactilia é uma anomalia congênita comum da mão, tem incidência de aproximadamente um em 2.000 nascimentos, predomínio no sexo masculino e é bilateral em 50% dos casos. Objetivo é descrever uma série de casos de pacientes com sindactilia nas mãos, operados no Hospital Sarah Brasília, com a técnica de Skoog modificada e o uso de órtese. MÉTODOS: Realizou-se a técnica de Skoog modificada para correção de sindactilia. No pós-operatório, foi confeccionada órtese punho-mão e dedos, e procedeu-se à avaliação do resultado da cicatriz conforme a Escala de Vancouver. RESULTADO: Durante cinco anos, avaliaram-se 45 pacientes, sendo 30 (66%) homens e 15 mulheres, com idade de um a quatro anos (média de dois anos - 60%), 37% dos casos mostraram-se associados a síndromes, 50% eram simples e bilateral. As complicações observadas nas sindactilias simples foram encontradas em 8% dos pacientes e caracterizaram-se por retração cicatricial, cicatriz visível e alteração ungueal; três casos (6%) foram reoperados para liberação de retração cicatricial com zetaplastia e confecção de neocomissura e quatro casos demandaram realinhamento digital, observado nas sindactilias complexas. CONCLUSÃO: Com esta técnica, obtiveram-se 92% de resultados funcionais, 8% de complicações relacionadas à técnica cirúrgica e três reoperações para liberação de retração cicatricial, por meio de zetaplastia e aprofundamento da comissura com novo retalho e enxerto de pele Em relação à avaliação quanto à Escala de Vancouver, o item predominante foi a pigmentação existente entre os dedos devido ao enxerto de pele.

Impact of extracurricular activities in Plastic Surgery on the knowledge and practical skills of medical students

ABSTRACT

INTRODUCTION To assess the impact of extracurricular activities in plastic surgery on the knowledge and practical skills of medical students at the Federal University of São Paulo. METHODS: Six students participating in extracurricular activities and eight students who did not participate were randomly selected and evaluated before and after the program by using multiple-choice tests and a Surgical Skills Assessment (SSA) instrument to assess their practical skills. RESULTS: The study group obtained higher scores in the theoretical tests, when the scores before starting the program (average, 15.8; standard deviation, 2.7) were compared with those received after their participation in extracurricular activities (average, 21.8; standard deviation, 1.3) (p = 0.02). Similar results were obtained with the SSA instrument for the period before the start of (average, 9.8; standard deviation, 1.4) and after (average, 13.5; standard deviation, 1.6) (p = 0.02) the program. The control group did not show significant changes in the scores received for theoretical tests when the period from August 2010 (average, 14.0; standard deviation, 4.1) to June 2011 (average, 13.8; standard deviation, 2.6) (p = 0.40) was evaluated. Similar results were obtained with the SSA instrument for the evaluation period between August 2010 (average, 8.5; standard deviation, 2.2) and June 2011 (average, 9.6; standard deviation, 1.7) (p = 0.18). CONCLUSION: The students participating in extracurricular activities in plastic surgery at the Federal University of São Paulo improved their knowledge and acquired better practical skills compared with students who did not participate.

INTRODUÇÃO Avaliar o impacto da monitoria em Cirurgia Plástica da Universidade Federal de São Paulo no conhecimento e nas habilidades práticas em estudantes de Medicina. MÉTODOS: Seis acadêmicos participantes da monitoria e oito acadêmicos não participantes foram selecionados aleatoriamente e avaliados antes e depois do programa, por meio de testes de múltipla escolha e através do instrumento Surgical Skills Assessment (SSA), para avaliação das habilidades práticas. RESULTADOS: O Grupo estudo obteve aumento significativo nas notas de prova teórica, entre o período antes do programa (média 15,8 e desvio padrão de 2,7) e após (média 21,8 e desvio padrão 1,3) (p = 0,02). Resultado semelhante foi encontrado com o instrumento SSA entre o período antes (média 9,8 e desvio padrão 1,4) e após o programa (média de 13,5 e desvio padrão 1,6) (p = 0,02). O grupo controle não obteve mudança significativa nos testes teóricos em agosto de 2010 ( média de 14,0 e desvio padrão 4,1) e junho de 2011 ( média de 13,8 e desvio padrão de 2,6 ) (p = 0,40 ). Resultado semelhante foi encontrado com o instrumento SSA em agosto de 2010 (média de 8,5 e desvio padrão 2,2) e junho de 2011 (média de 9,6 e desvio padrão 1,7) (p = 0,18). CONCLUSÃO: Acadêmicos participantes na monitoria de Cirurgia Plástica da Universidade Federal de São Paulo apresentaram melhor conhecimento e melhores habilidades práticas em relação aos não participantes.

Burn Care Unit of São Paulo Federal University: an epidemiological profile

ABSTRACT

INTRODUCTION: Burns constitute severe trauma and account for 100,000 hospital visits per year in Brazil. Burns units are essential for the treatment of these patients and the generation of knowledge on this subject. METHOD: This study established the annual profile of burn unit attendance between 2009 and 2012. During these years of activity, we compared the number of hospitalized patients, patient origin, burn degree, age, burn body surface area, days of hospitalization, types of surgical procedures, agents, and mortality. Possible combinations were evaluated with the chi-square test. RESULTS: A total of 321 patients were hospitalized. In 2009, 70% of the patients presented secon-degree burns; in 2012, 66% of the patients presented third-degree burns. In 2009, 37% of the patients were referred to other services; in 2012, 72% were referred. The average patient age was 29 years old in 2009 and 44 years old in 2012. Burn body surface was 13% in 2009 and 8% in 2012. Hospitalization time increased from 11 to 21 days. A total of 50 surgical procedures were performed in 2009, while 103 were performed in 2012. Flammable liquids were the most commonly observed causal agents. The overall mortality rate decreased from 7% to 4% over the study period. CONCLUSION: There was an increase in the number of referred and complex patients with higher degree burns who required longer hospital stays and a greater number of procedures over the years. The decreased mortality rate reflected the technical improvement of the unit that was established as a regional reference for the treatment of burn victims.

INTRODUÇÃO: As queimaduras são traumas graves, que geram 100 mil atendimentos hospitalares por ano, no Brasil. As Unidades de Queimados são fundamentais para o tratamento desses pacientes e a produção de conhecimento sobre o tema. MÉTODO: O estudo estabeleceu o perfil de atendimento em cada ano de funcionamento da unidade no período de 2009 a 2012, e comparou, entre esses anos, a quantidade de pacientes internados, a procedência, os graus de queimadura, a idade, a superfície corporal queimada, os dias de internação, os tipos de procedimentos cirúrgicos, os agentes e a mortalidade. Para avaliação das possíveis associações, foi utilizado o teste do Qui-quadrado. RESULTADOS: Foram internados 321 pacientes. Em 2009, 70% dos pacientes apresentavam queimadura de II grau e, em 2012, 66% dos pacientes tinham queimadura de III grau. Em 2009, 37% vieram referenciados de outros serviços, e em 2012, 72%. A idade média dos pacientes foi de 29 anos em 2009 e 44 anos, em 2012. A superfície corporal queimada no início era de 13%, e em 2012, foi 8%. O tempo de internação subiu de 11 para 21 dias. Em 2009, realizaram-se 50 procedimentos cirúrgicos. Em 2012, foram 103. O agente causal mais encontrado foram os líquidos inflamáveis e a mortalidade global caiu de 7% para 4%, ao longo dos anos. CONCLUSÃO: Houve um aumento na quantidade de pacientes referenciados e complexos, com queimaduras de maior grau, exigindo mais procedimentos e maior hospitalização, ao longo dos anos. A taxa de mortalidade diminuiu, refletindo melhora técnica da Unidade, que se estabeleceu como referência regional no tratamento de queimados.

Plastic surgery in the media: the concept of the specialty, as reported by the print media in Brazil

ABSTRACT

INTRODUCTION Besides the diverse situations in which plastic surgery is performed, the procedure has an aesthetic character associated with societal issues, related to the ideals of beauty and to the search for body perfection. Currently, specific body measurements are increasingly being given importance, and shown by the media as being associated with success at diverse social levels. The objective of this study was to identify the concept of plastic surgery, as reported by the print media in Brazil and available in the virtual environment, by analyzing and describing the type of information that is reported to the general public. METHOD: For the data collection, a systematic search was performed on all news articles published by the print media in Brazil and available in the virtual environment. For the search, the descriptor used was "cirurgia plástica" ("plastic surgery" in English) in Portal Interjornal®. All news articles containing the term "plastic surgery" published between January 2008 and December 2011 were included in the study. The selected news articles were read and subjected to a previously established protocol. RESULTS: A total of 1983 news articles were analyzed. From this total, 1367 (68.94%) were classified as aesthetic related, 423 (21.33%) as being associated with reparative surgery, 47 (2.37%) as aesthetic and reparative, and 146 (7.36%) as undetermined. CONCLUSION: The concept of plastic surgery reported by the media was generally associated with aesthetic surgery. Both the content and the quality of information provided by the media are not enough to adequately clarify to the public the real role of the specialty and the care that should be taken when undergoing any type of surgical procedure.

INTRODUÇÃO A cirurgia plástica, apesar das diversas situações em que atua, tem o caráter estético difundido na sociedade, associado aos ideais de beleza e à busca das pessoas pela perfeição corporal. Atualmente, as medidas corporais são cada vez mais detalhadas, exibidas pela mídia como sucesso nos mais diversos âmbitos sociais. O objetivo do presente estudo será identificar o conceito de cirurgia plástica vinculado aos meios de comunicação impressos do Brasil e disponíveis virtualmente, analisando e descrevendo o tipo de informação veiculada ao público leigo. MÉTODO: Para a coleta de dados, foi realizada busca sistemática de todas as notícias publicadas em meios de comunicação impressos do Brasil e disponíveis virtualmente. Para a busca, utilizamos o descritor "cirurgia plástica" no Portal Interjornal®. Foram incluídas no estudo todas as notícias publicadas entre janeiro de 2008 e dezembro de 2011, que contêm o termo "cirurgia plástica". As notícias selecionadas foram lidas e submetidas a um protocolo previamente definido. RESULTADO: Foram analisadas 1983 notícias. Deste total, 1367 (68,94%) foram classificadas como estéticas; 423 (21,33%) como reparadoras; 47 (2,37%) como estéticas e reparadoras, e 146 (7,36%) como não classificáveis. CONCLUSÃO: O conceito da cirurgia plástica veiculado pela mídia está geralmente associado à realização de procedimentos estéticos. Tanto o conteúdo quanto a qualidade das informações disponibilizadas pela mídia são insuficientes para esclarecer o público de forma adequada sobre o verdadeiro papel da especialidade e os cuidados que devem ser tomados ao se submeter a qualquer tipo de procedimento cirúrgico.

Use of mobile applications in plastic surgery

ABSTRACT

INTRODUCTION The modernization of medicine allowed a greater interaction between medical teams and patients. Technological development, especially in the field of communication, has led to the creation of new devices such as smartphones and tablets. The widespread popularity of these devices and the development of applications have allowed their use in medicine, being quick means of accessing information, diagnosis, patient follow-up, surgical simulations, guidelines, electronic books and information on pathological conditions, and therapeutic and surgical procedures. This study is a review of the applications of smartphones and tablets in plastic surgery. METHODS: The application stores Google Play® and Apple Store® in English were assessed until June 2014. Initially, 588 applications related to plastic surgery were found. Based on their descriptions, the applications were classified according to cost, area of operation, store in which the application is made available, and use. RESULTS: After applying the exclusion criteria, 19 applications were selected, of which 11 were related to surgical simulations; five, to clinical evaluations; and three, to microsurgery and flaps. With regard to access, 12 were free and seven were paid. Of these applications, 11 were exclusive to the Apple Store®, two were exclusive to Android®, and six were available in both. CONCLUSION: Approximately 600 applications related to plastic surgery have been developed, but only about 20 of these have clinical applicability. The development of these applications in other languages is needed, facilitating their use in other countries.

INTRODUÇÃO A modernização da Medicina permitiu uma maior interação entre a equipe médica e o paciente. O desenvolvimento tecnológico, principalmente na comunicação, permitiu a criação de novos aparelhos, como smartphones e tablets. A disseminação destes aparelhos e o desenvolvimento de aplicativos permitiram o uso destes na Medicina, sendo um meio rápido de acesso a informação, diagnóstico, acompanhamento de pacientes, simulações cirúrgicas, orientações, livros eletrônicos e informações sobre a patologia, e na conduta terapêutica e cirúrgica. Este estudo é uma revisão para identificação dos aplicativos sobre cirurgia plástica nestes aparelhos: smartphones e tablets. MÉTODOS: Foram pesquisadas, na língua inglesa, as bases de aplicativos google play® e apple store®, encontradas disponíveis até junho de 2014. Foram encontrados, inicialmente, 588 aplicativos relacionados à cirurgia plástica. Com base na descrição dos aplicativos, estes foram classificados quanto a gratuidade, área de atuação, base em que o aplicativo foi encontrado e utilização. RESULTADOS: Após utilização de critérios, foram encontrados 19 aplicativos, dos quais 11 relacionados à simulação cirúrgica, cinco à avaliação clínica e três sobre microcirurgia e retalhos. Quanto ao acesso, 12 eram gratuitos e sete pagos. Quanto à base de aplicativos, 11 eram exclusivos da apple store®, dois exclusivos da android® e seis encontrados em ambas. CONCLUSÃO: Existem atualmente cerca de 600 aplicativos relacionados à cirurgia plástica, porém apenas cerca de 20 destes apresentam aplicabilidade clínica. É necessário o desenvolvimento da acessibilidade através desses aplicativos em outras línguas, facilitando o uso destes em outros países.

ABSTRACT

INTRODUCTION: Shoulder cutaneous fibrosarcoma is an unusual pathology that requires extensive resections. The scapula is a source of flaps widely used in this region. METHOD: A longitudinal and prospective study was carried out in a patient with protuberans cutaneous fibrosarcoma on the right shoulder who underwent resection followed by local reconstruction with a Duoscapular Flap (a combination of scapular and parascapular flaps). RESULTS: The patient had no postoperative complications, as systemic and local complications as well as functional limitations were not observed. CONCLUSION: Duoscapular Flap placement is a novel procedure and a relevant choice for the reconstruction of extensive wounds exposing noble structures in the shoulder. It allows the primary closure of the donor area without increasing the morbidity of the procedure.

RESUMO

INTRODUÇÃO: O Dermatofibrossarcoma do Ombro é patologia incomum e seu tratamento demanda extensas ressecções. O sistema escapular é fonte de retalhos bastante utilizados nesta região. MÉTODO: Realizado estudo longitudinal, prospectivo, através da condução de um caso de Dermatofibrossarcoma Protuberans em ombro direito, submetido a ressecção e reconstrução local com Retalho Duoescapular, obtido através da associação dos retalhos escapular e paraescapular. RESULTADOS: Paciente evoluiu sem intercorrências no pós-operatório, não sendo observadas complicações sistêmicas e locais, e limitações funcionais. CONCLUSÃO: O Retalho Duoescapular é nova e relevante opção para reconstrução de feridas extensas, com exposição de estruturas nobres no ombro. Permite fechamento primário da área doadora, sem acrescentar morbidade ao procedimento.

Excision of eyelid eccrine hidrocystomas via lower blepharoplasty incision

ABSTRACT

INTRODUCTION: Eccrine eyelid hidrocystoma is a rare cystic lesion, usually small and not exceeding 10 mm in diameter. Its treatment is usually by simple excision and biopsy. The rate of local recurrence is high. METHODS: A patient presented with multiple bilateral hidrocystomas, several larger than 10 mm. Under local anesthesia, an infraciliary incision traditionally used for lower blepharoplasty was performed, and hidrocystomas with intact capsules were withdrawn. RESULTS: The patient had excellent esthetic results, with no recurrence for more than six months after the procedure. CONCLUSION: The infraciliary lower blepharoplasty incision is a promising technique for excising hidrocystomas larger than 10 mm. More studies should be conducted to confirm the good esthetic and functional outcomes of this technique.

INTRODUÇÃO: O hidrocistoma palpebral écrino corresponde a lesão cística rara, usualmente de pequenas dimensões, não ultrapassando o diâmetro de 10 mm. Sua abordagem usualmente é por excisão simples e biópsia. É alto o índice de recidiva local. MÉTODOS: Paciente com múltiplos hidrocistomas bilaterais, vários maiores que 10 mm. Sob anestesia local, foi realizada incisão infraciliar tradicionalmente usada para blefaroplastia inferior e retirada dos hidrocistomas com cápsula íntegra. RESULTADOS: Paciente com excelentes resultados estéticos e sem recidivas até o presente momento, mais de seis meses após o procedimento. CONCLUSÃO: A incisão infraciliar de blefaroplastia inferior é uma técnica promissora para exérese de hidrocistomas maiores que 10 mm. Mais estudos devem ser realizados para confirmação do bom resultado estético-funcional desta técnica.

ABSTRACT

INTRODUCTION This qualitative literature review analyzed international scientific publications on possible orofacial myofunctional alterations in patients with Parry-Romberg syndrome by using PubMed. METHODS: The survey was conducted in English, between 2002 and 2012, and was limited to human beings of any age. Publications without full access, duplicated by overlapping keywords, literature reviews, letters to the editor, and those not directly related to the research topic were excluded. RESULTS: We identified 719 studies, of which 21 were within the established criteria. Based on the selected studies, patients with Parry-Romberg syndrome may show changes in soft and hard tissues such as atrophy of the sternocleidomastoid, masseter, and pterygoid muscles; atrophy in the cheek region and depression of the nasolabial fold; deviation of the lips and nose; unilateral tongue atrophy; atrophy of the mouth angle; progressive resorption of the maxilla and mandible bone; atrophy of the zygomatic arch and frontal bone, and facial asymmetry; atrophic root development or pathological resorption of permanent tooth numbers; and jaw reduction and delayed eruption of the upper and lower teeth. CONCLUSION: Despite the growing interest in the diagnosis and symptomatic description of individuals with Parry-Romberg syndrome, publications that address functional and interdisciplinary treatments are scarce. Therefore, specific studies aimed at improving the quality of life of these patients are needed.

INTRODUÇÃO Esta revisão qualitativa da literatura analisou publicações científicas internacionais sobre possíveis alterações miofuncionais orofaciais em pacientes acometidos pela Síndrome de Parry-Romberg, por meio da base de dados PubMed. MÉTODOS: O levantamento realizado limitou-se a seres humanos, de qualquer faixa etária, no idioma inglês, entre os anos 2002 e 2012. As publicações sem acesso completo, repetidas por sobreposição das palavras-chave, revisões de literatura, cartas ao editor e as não relacionadas diretamente ao tema foram excluídas. RESULTADOS: Foram identificados 719 estudos, sendo 21 dentro dos critérios estabelecidos. Com base nos estudos selecionados, pacientes acometidos pela Síndrome de Parry-Romberg podem apresentar alterações dos tecidos mole e duro, tais como atrofia dos músculos esternocleidomastoideo, masseter e pterigoideos; atrofia na região da bochecha e depressão da prega nasolabial; desvio dos lábios e nariz; atrofia unilateral da língua; atrofia do ângulo da boca; reabsorção progressiva do osso da maxila e da mandíbula; atrofia do arco zigomático, do osso frontal e assimetria facial; desenvolvimento atrófico das raízes ou reabsorção patológica dos números de dentes permanentes; redução da mandíbula e erupção atrasada dos dentes superiores e inferiores. CONCLUSÃO: Apesar do crescente interesse pelo diagnóstico e pela descrição sintomatológica de indivíduos com Síndrome de Parry-Romberg, a escassez de publicações que abordem tratamentos funcionais e interdisciplinares é evidente. Verifica-se a necessidade da realização de estudos mais específicos que visem à melhoria da qualidade de vida desses pacientes.

Options for capsulotomy and capsulectomy in the treatment of capsular contracture: are there clinical treatment alternatives to surgery? A literature review

ABSTRACT

INTRODUCTION: Capsular contracture is among the main complications of surgeries involving breast implants. The most commonly used classification to assess the degree of contracture is the Baker grading system, which divides contractures into grades I, II, III, and IV. Of these, grade III and IV contractures are considered significant. Although several causes have been postulated, the etiology of capsular contracture remains uncertain. Conventional treatment for contracture is based on a surgical approach, specifically capsulotomy or capsulectomy. These procedures, however, are not exempt from morbidity, and patients may develop complications such as dehiscence, hematoma, seroma, pneumothorax, asymmetry, and contracture recurrence. This study provides a review of alternatives to conventional surgery described in the literature. METHODS: We researched the PubMed and Cochrane Library databases using the following keywords: "capsular contracture", "capsular contracture treatment", and "capsular contracture breast treatment". We identified 991 articles from which we selected those discussing medication options for contracture treatment other than capsulectomy and capsulotomy. RESULTS: We identified several studies in which drugs, most commonly zafirlukast, were used to reduce capsular contracture. CONCLUSION: Among the various reported drugs, zafirlukast exhibited good efficacy and a low rate of complication. Triamcinolone also appears to be a good option, although professional assistance would be needed for drug administration via infiltration. The other drugs described would require further investigation.

INTRODUÇÃO: A contratura capsular é uma das principais complicações em cirurgias envolvendo implantes mamários. A classificação mais usada para avaliar o grau de contratura é a de Baker, que a divide em graus I, II, III e IV, sendo as de grau III e IV consideradas significativas. Apesar de existirem diversas teorias, a etiologia da contratura capsular permanece incerta. O tratamento convencional para os casos de contratura é a abordagem cirúrgica com realização de capsulotomia ou capsulectomia. Estes procedimentos, no entanto, não estão isentos de morbidades, com complicações como deiscências, hematomas, seromas, pneumotórax, assimetrias e recidiva da contratura. Este estudo faz uma revisão sobre as alternativas ao tratamento cirúrgico convencional, descritas na literatura. MÉTODOS: Foi realizada pesquisa nas bibliotecas da Pubmed e da Cochrane, utilizando-se os termos: 'capsular contracture', 'capsular contracture treatment' e 'capsular contracture breast treatment'. Foram identificados 991 artigos e selecionados os que discutiam opções medicamentosas para o tratamento de contratura, diferentes de capsulectomia e capsulotomia. RESULTADOS: Foram encontrados vårios estudos utilizando drogas com o fim de reduzir a contratura capsular, das quais o Zafirlucaste é apresentado em maior número de trabalhos. CONCLUSÃO: Dentre as várias drogas utilizadas, o Zafirlucaste apresentou boa eficácia, com baixos índices de complicação; a Triancinolona parece ser também uma boa opção, no entanto precisa de profissional habilitado para realizar as infiltrações. As demais drogas necessitam de maiores estudos.

ABSTRACT

Facial fat grafting is not devoid of risks and complications, despite being a widely practiced procedure. In this case report, we describe a novel form of local ischemic complication that occurred secondary to fat grafting. Simultaneously, we propose Hyperbaric Oxygen therapy for the treatment of this situation. We propose that Hyperbaric Oxygen therapy should not be limited solely to fat graft-related ischemia, but should be effective in the management of the more frequently encountered complications associated with polymethylmethacrylate fillers.

RESUMO

A lipoenxertia de face, apesar de consagrada, não é isenta de riscos e complicações. Neste relato de caso, descrevemos de forma inédita uma complicação isquêmica local, causada por enxerto de gordura. Simultaneamente, propomos a terapêutica com Oxigenioterapia Hiperbárica para o tratamento desta situação, não se limitando exclusivamente à gordura, mas principalmente ao PMMA, em que esta complicação é muito mais frequente.

Gestational gigantomastia: how to address this clinical situation

ABSTRACT

Gestational gigantomastia is a rare disorder characterized by an excessive and rapid enlargement of the breasts, resulting in edema and venous congestion of breast tissue. It is a painful condition that causes skin ulceration and local infection. In some cases, these complications lead to an emergency mastectomy or induced abortion. The most probable etiology is an abnormal stimulation of breast tissue that is probably triggered by an abnormally elevated level of hormones or by the hypersensitivity of breast tissue to normal hormone levels. Although the breast volume decreases after pregnancy, it rarely returns to its original state; therefore, breast reduction through mastectomy or mammaplasty is usually necessary. Moreover, there is a high likelihood of recurrence in subsequent pregnancies. The authors report a case of gigantomastia in a primigravida that required pregnancy termination, because of the risk of maternal death, and a subsequent breast reduction surgery.

A gigantomastia gestacional é uma desordem rara, na qual ocorre o crescimento excessivo e rápido das mamas, culminando com edema e congestão venosa das mesmas, além de ocasionar dor, ulceração da pele e infecção local. Estas complicações, em alguns casos, levam à necessidade de mastectomia de emergência ou abortamento induzido. A hipótese etiológica mais aceita é a de que exista uma estimulação anormal do tecido mamário, que pode ser desencadeada por níveis excessivos de hormônios ou por uma hipersensibilidade deste tecido a níveis hormonais normais. Apesar de a mama regredir após a gestação, raramente volta ao estado original; portanto, a redução da mama através de mastectomia ou mamoplastia geralmente é necessária. Além disso, é grande o risco de recorrências em gestações futuras. Os autores relatam o caso de gigantomastia em uma primigesta, com necessidade de interrupção da gravidez, devido ao risco de morte materna, e posterior intervenção cirúrgica com mamoplastia redutora.

Pyoderma gangrenosum after quadrantectomy with intraoperative radiotherapy: a case report

ABSTRACT

Pyoderma gangrenosum is an immune-mediated inflammatory and rare skin disease with an extremely challenging diagnosis. The clinical evolution of the disease is the basis for the diagnosis that involves pustular superficial lesions, painful erythematous halo, rapid progression to painful and sterile ulcerations, unresponsiveness to antibiotics or new surgical interventions, and finally, ready improvement with the use of immunosuppressive drugs. Delayed diagnosis may cause numerous hospitalizations and prolonged therapy, whereas early recognition can prevent the progression of the ulcerations and their morbidities. We report a case of pyoderma gangrenosum that evolved after surgery and was associated with intraoperative radiotherapy for the conservative treatment of breast cancer. In addition, we reviewed reported cases in the literature and therapeutic options. It is conjectured that intraoperative radiotherapy might be related to some immune-mediated stimuli that could trigger the clinical condition.

RESUMO

Pioderma gangrenoso é uma doença inflamatória imunomediada e rara da pele, de diagnóstico extremamente desafiador. A evolução clínica é a base para o diagnóstico, cursando com lesões pustulosas superficiais, halo eritematoso doloroso, rápida progressão para ulcerações dolorosas e estéreis, sem resposta a antibióticos ou a novas intervenções cirúrgicas e, finalmente, com pronta melhora com uso de imunossupressores. O atraso no diagnóstico pode acarretar numerosas internações e terapias prolongadas, sendo que seu reconhecimento precoce, por outro lado, evita a progressão dessas ulcerações e sua morbidade. Relatou-se um caso de pioderma gangrenoso que evoluiu após cirurgia associada à radioterapia intraoperatória no tratamento conservador do câncer de mama, fazendo-se uma revisão de casos relatados na literatura e suas possibilidades terapêuticas. Questiona-se, também, se a radioterapia intraoperatória estaria relacionada com algum estímulo imunomediado, o que poderia ter facilitado o desencadeamento do quadro.

Aplasia cutis congenita on the lower limbs: a case report

ABSTRACT

INTRODUCTION Aplasia cutis congenita (ACC) is a rare disease characterized by the absence of a part of the skin at birth that may be either localized or widespread. The incidence of this disease is 0.1 per 100,000 births. It mainly involves the scalp alone in 60% of the cases. The causes are unclear; however, genetic factors, teratogens (alcohol, cocaine, marijuana, heroin, misoprostol, methimazole, carbimazole, congenital herpes simplex, congenital varicella, and others), impaired blood supply to the skin, trauma, and amniotic band and chromosomal disorders (trisomy 13) are associated with the wounds. CASE REPORT: A female patient was delivered vaginally at 36 weeks 3 days of gestation, with appropriate weight for the gestational age. She was referred to our service after 7 days of birth, presenting total absence of skin integument on both legs, with impairment of approximately 17% of the body surface according to the Lund and Browder chart. The malformation consisted of the total absence of skin and subcutaneous tissue. Given the extent and localization of the wound, treatment with a partial skin graft was elected, thereby filling the totality of the defect. CONCLUSION: Owing to the rarity of ACC and the small number of patients in the series published in the literature, standardization of treatment is still incipient. Currently, only recommendations are available. Further studies are needed in order to investigate the etiology of the disease and to evaluate treatment methods.

RESUMO

INTRODUÇÃO A Aplasia Congênita da Cútis (ACC) é uma doença rara caracterizada pela ausência de uma parte da pele ao nascimento, seja em área localizada ou generalizada. A incidência é de 0,1 a cada 100.000 nascimentos, tendo como acometimento principal o couro cabeludo, ocorrendo de forma isolada nesta localização em 60% dos casos. As causas não são claras, mas fatores genéticos, teratógenos (álcool, cocaína, maconha, heroína, misoprostol, metimazol, carbimazol, herpes simples congênito, varicela congênita, etc.), comprometimento da irrigação sanguínea para a pele, trauma, bandas amnióticas e desordens cromossômicas (trissomia do 13) estão associados com as lesões. RELATO DO CASO: Paciente do sexo feminino, nascida de parto vaginal com 36 semanas e três dias, e peso adequado para a idade gestacional. Referenciada ao nosso serviço após sete dias de nascimento, constando ausência total do tegumento cutâneo em ambas as pernas, com acometimento de aproximadamente 17% da superfície corpórea, segundo a tabela de queimados de Lund e Browder. A má formação consistia na ausência total de pele e tecido celular subcutâneo. Face a extensão e localização da lesão, optou-se pelo tratamento por enxerto de pele parcial, permitindo, dessa forma, preencher a totalidade do defeito. CONCLUSÃO: Devido à raridade da ACC e ao pequeno número de pacientes nas séries publicadas na literatura, a padronização do tratamento ainda é incipiente e o que existe são recomendações. Ainda são necessários estudos que abordem desde a etiologia da doença até a avaliação dos métodos de tratamento.

22 years of an Informational Overview Course on Plastic Surgery

ABSTRACT

For 22 years, a reproducible introductory overview course on plastic surgery for medical students, cosponsored by four medical schools, three universities and the regional chapter of the Brazilian Society of Plastic Surgery (SBCP), has contributed to medical education in Curitiba, a city of 2 million, and serving a metropolitan population of nearly 3 million people. This course helps medical students determine if plastic surgery is the right choice for them and provides valuable information about the specialty itself that will serve all participants as they go forward into their chosen fields, as plastic surgeons or not. This broad-ranging 47 hour course, limited to 120 students, is fully enrolled every year. Its content is weighted approximately 70% toward reconstructive plastic surgery and 30% toward aesthetic plastic surgery. Most lectures are presented by plastic surgeons who are full members of the SBCP. During its first 19 years of operation, 4.6% of the students completing the course have gone on to a general surgery residency followed by a plastic surgery residency, and are current SBCP members. The course has become a valuable educational and marketing tool for the regional chapter of the SBCP. It promotes the unique scope of the specialty and increases awareness among participants, as well as among their many personal and professional contacts (currently and in the future) about the benefits of becoming fully qualified, board-certified plastic surgeons.

Há 22 anos, um curso informativo de cirurgia plástica para acadêmicos de medicina, promovido pela integração de quatro escolas médicas, três universidades e pela regional da Sociedade Brasileira de Cirurgia Plástica vem sendo ministrado, contribuindo para a educação médica em Curitiba, uma cidade de dois milhões de habitantes, em uma área metropolitana de aproximadamente três milhões de pessoas. Este curso informativo proporciona uma visão abrangente da especialidade, para que acadêmicos de medicina possam avaliar sua vocação pessoal para o exercício da cirurgia plástica, além de fornecer conhecimentos básicos indispensáveis à formação de qualquer médico. Anualmente o curso é ministrado em 47 horas e ofertado para 120 alunos. O programa inclui 70% de "cirurgia reconstrutiva" e 30% de "cirurgia estética", sendo a maioria dos professores membros titulares da Sociedade Brasileira de Cirurgia Plástica. Nos primeiros 19 anos de atividade (1992 a 2010), 4,6% dos alunos que concluíram o curso cursaram residência em cirurgia geral, foram admitidos na residência em cirurgia plástica e hoje fazem parte da Sociedade Brasileira de Cirurgia Plástica. O curso tornou-se um importante instrumento de educação médica, complementando o currículo de quatro escolas médicas e promovendo a cirurgia plástica como especialidade única e indivisível, enfatizando a importância da prova de especialista em cirurgia plástica e a conveniência da escolha e da indicação destes especialistas para a realização de qualquer procedimento no âmbito da cirurgia plástica.
