Senile entropion of the lower lid: surgery performed by the etiopathogeny

ABSTRACT

Background: The entropion is an inward rotation of the eyelid margins as the epithelium and the eyelashes rub against the eye resulting irritation. The purpose of this study is present a surgical treatment with the association of three techniques that resolves the main causes of senile entropion and the follow up. Methods: Fourteen eyelids with senile entropion underwent surgical repair with wedge tarsal shortening, plication of the lid retractors with "U" sutures and a segmental resection of pre septal orbicular. In follow up was available any problems, recurrence, complications and complain resolution. Results: From 2006 July to 2008 April, ten patients with senile entropion were operated. Complain was ocular irritation (100%) and excessive tearing (80%). In physical examination, all patients had hyperemia. Five take out the lashes, and 3 used tapes to take the lid down. The surgery time was 26 minutes, without problems. All patients discharged with chemosis that resolved in the first 2 weeks postoperatively. Nine were completely treated and one had partial recurrence. Conclusions: The study demonstrated, with details, a model of treatment for senile entropion based in three main causes. All patients didn't have immediate complications, but one patient had partial recurrence, that was treated with the same procedure.

RESUMO

Introdução: O entrópio é uma rotação interna da pálpebra, em que o epitélio e os cílios raspam o olho, ocasionando irritação. Este trabalho tem o objetivo de apresentar, detalhadamente, um tratamento cirúrgico baseado na associação de três técnicas, cada uma direcionada a tratar uma das três principais causas do entrópio senil e sua evolução. Método: Quatorze pálpebras com entrópio senil foram submetidas ao procedimento cirúrgico, que consistiu de encurtamento tarsal horizontal, plicatura dos retratores ao tarso com pontos em "U" e ressecção de uma fita muscular da pálpebra inferior. Em todos os retornos, foram avaliadas intercorrências, recidivas, complicações e resolução dos sintomas. Resultados: De julho de 2006 até abril de 2008, foram operadas 10 pacientes com entrópio senil. As queixas eram irritação ocular (100%) e lacrimejamento (80%). Ao exame físico, todos os pacientes apresentavam hiperemia. Cinco arrancavam os cílios e 3 usavam fitas adesivas para tracionar a pálpebra inferiormente. As cirurgias levaram, em média, 26 minutos e transcorreram bem, sem intercorrências. Todos receberam alta bem, com equimose que regrediu em 2 semanas. Nove pacientes tiveram melhora dos sintomas e um apresentou recidiva parcial do entrópio. Conclusão: Foi demonstrada uma modalidade de tratamento para o entrópio senil baseada nas suas três principais causas, com bons resultados. Tivemos recidiva parcial em um paciente, que foi tratada, repetindo-se o procedimento.

External dacryocystorhinostomy results in the treatment of lacrimal duct stenoses

ABSTRACT

Introduction: The surgical treatment of lacrimal obstruction by external dacryocystorhinostomy technique consists of the formation of a fistula between the lacrimal sac and nasal passage. Has long been considered the standard technique for obstruction of the lacrimal acquired or congenital etiology. Objective: To present the results of a series of cases submitted to Kasper technique external dacryocystorhinostomy. Methods: This is a retrospective study of a series of cases operated at the Hospital Regional da Asa Norte, from December 1999 to January 2008. Results: We describe ten cases with external da-cryocystorhinostomy technique performed, five women and five men. The age ranged from 8 to 56 years. The most common etiology was traumatic and the main clinical symptom was epiphora. 90% of patients had remission of clinical symptoms of lacrimal obstruction presented preoperatively. There was one case of adverse developments that were required reoperation. Conclusion: With this technique we obtained good results, especially in cases of an acquired defect, with the advantages of direct visualization and easier to create the wide ostium and lacrimal flap lacrimal sac.

RESUMO

Introdução: O tratamento cirúrgico da obstrução das vias lacrimais pela técnica de dacriorinocistostomia externa consiste na formação de uma fístula entre o saco lacrimal e a passagem nasal, durante muito tempo foi considerada a técnica padrão para obstruções das vias lacrimais adquiridas ou congênitas. Objetivo: Apresentar os resultados de uma série de casos submetidos a dacriocistorinostomia externa com a técnica de Kasper modificada. Método: Trata-se de estudo retrospectivo de uma série de casos operados no Hospital regional da Asa Norte, entre dezembro de 1999 a janeiro de 2008. Resultados: Foram operados dez casos, cinco mulheres e cinco homens. A idade variou de 8 a 56 anos. A etiologia mais frequente foi traumática. O principal sintoma clínico foi a epífora. Noventa por cento dos pacientes apresentaram remissão dos sintomas clínicos de obstrução da via lacrimal apresentados no pré-operatório. Houve um caso de evolução desfavorável em que foi necessária a reoperação. Conclusão: Com esta técnica obtiveram-se bons resultados, principalmente em obstruções de causas adquiridas, tendo como vantagens a visão direta e a maior facilidade para criação do largo óstio e retalho do saco lacrimal.

Total ear reconstruction on Clinical Hospital of the Federal University of Goiás

ABSTRACT

Introduction: Repair or rebuild partially or completely an ear is a challenge in medicine and requires technical ingenuity and surgical skill. The use of autogenous material (costal cartilage) is still considered the best surgical option. The success of the reconstruction of the ear depends crucially on the development of the cartilaginous framework. Methods: This is a cross-sectional study. We analyzed the records of 39 patients admitted to the clinic of the Plastic Surgery of the Hospital of the Federal University of Goiás, during the period from 1997 to 2007. All patients underwent total ear reconstruction by the same surgeon, using autologous costal cartilage. The reconstruction was performed in two surgical times. About the patients, was observed the predominance of children and adolescents (6 to 14 years), male and from capital cities (Goiânia). There was no predominance of right or left microtia. The average time of the first surgical time was 140 minutes and the second 120 minutes. Resuts: The pain at the site of costal cartilage removal was the main complication. There were no cases of extrusion of the cartilaginous framework, infection and perforation of the pleura. The findings are for a better understanding and characterization of patients. Conclusion: The standardization of the technique, in this case using costal cartilage to build the framework headset, along with experience of the surgeon can bring great benefits to patients, especially regarding to minimize the complications of surgeries performed.

RESUMO

Introdução: Reparar ou reconstruir parcial ou totalmente uma orelha constitui-se um desafio na Medicina e requer engenhosidade técnica e habilidade cirúrgica. A utilização de material autógeno (cartilagem costal) ainda é considerada a melhor opção cirúrgica. O sucesso da reconstrução da orelha depende sobremaneira da elaboração do arcabouço cartilaginoso. Método: O estudo é do tipo transversal, retrospectivo. Foram avaliados prontuários de 39 pacientes admitidos no ambulatório de Cirurgia Plástica do Hospital das Clínicas da Universidade Federal de Goiás, durante o período compreendido entre 1997 a 2007. Todos foram submetidos à reconstrução total de orelha pelo mesmo cirurgião, sendo utilizada a cartilagem costal autóloga e realizada a reconstrução em dois tempos cirúrgicos. Observou-se predomínio de crianças e adolescentes (6 a 14 anos), sexo masculino e procedência da capital (Goiânia). Não houve predominância entre microtia direita e esquerda. O tempo médio do primeiro tempo cirúrgico foi de 140 minutos e, do segundo, 120 minutos. Resultados: A dor no local da retirada da cartilagem costal foi a principal complicação. Não houve nenhum caso de extrusão do arcabouço cartilaginoso, infecção e perfuração da pleura. Os resultados desse trabalho são válidos para um melhor conhecimento e caracterização dos pacientes. Conclusão: A padronização da técnica, nesse caso utilizando cartilagem costal para a construção do arcabouço auricular, aliada à experiência do cirurgião, pode trazer grandes benefícios aos pacientes, especialmente no que diz respeito à minimização das complicações advindas das cirurgias realizadas.

Surgical treatment of the cleft lip by Göteborg technique: 7 years follow up

ABSTRACT

Introduction: After a scientific internship with the Göteborg University, in Sweden, we started to performe at SOBRAPAR Hospital the new technique of cleft lip surgery, that has been improved during these years. Since 2000, this technique became a pattern in our protocol for cleft lip interventions. We began to call it as The Göteborg Technique. Method: There were reevaluated 46 patients operated during the period of 7 years submitted to Göteborg technique to correct the cleft lip or unilateral cleft lip and palate, using personal evaluation patterns. The surgical technique used was evaluated in relation to its applicability and results, since the Hospital is a Trainee Center for residents in Plastic Surgery. It was used following the preexisting protocol for cleft lip surgery. Results: The technique performed demonstrated to be of an easy applicability due to the unique measure pattern to marc the reference points in the lip sides, making it easy during the teaching process. There were predominancy of male (65%). Only 28% of the patients were operated following the protocol related to the age (3 months), the factors that contributed to this delay were anemia and low weight. In the general evaluation of the results, we observed 84.7% of patients with satisfactory results, following protocol of evaluation preestablished by the author. Conclusions: The performed technique showed to be of an easy applicability with good results, mainly when used in the correct age as indicated in the protocol of treatment of patients with cleft lip in the Institution.

RESUMO

Introdução: Após intercâmbio científico com a Universidade de Göteborg, da Suécia, passamos a utilizar no hospital da SOBRAPAR nova técnica de labioplastia, que foi sendo aperfeiçoada ao longo dos anos, sendo que a partir do ano de 2000, tornou-se técnica padrão no protocolo para labioplastia neste hospital. Passamos a denominá-la como técnica de Göteborg. Método: Foram reavaliados 46 pacientes operados ao longo de 7 anos, nos quais o autor utilizou a técnica de Göteborg para correção de fissura labial ou labiopalatal unilateral, avaliando os resultados com emprego de critérios padronizados. A técnica cirúrgica empregada foi avaliada com relação a sua aplicabilidade e resultados, já que o hospital é um centro de treinamento para médicos residentes, sendo aplicada seguindo-se protocolo preexistente para labioplastia. Resultados: A técnica empregada mostrou-se de fácil aplicabilidade devido à padronização de medida única para a marcação dos pontos de referência nas vertentes labiais, facilitando o ensino da mesma. Houve predomínio do sexo masculino (65%); somente 28% dos pacientes foram operados conforme determinava o protocolo (3 meses), sendo identificados como fatores de morbidade que colaboraram para este atraso na cirurgia a anemia e o baixo peso. Na avaliação geral dos resultados, observamos 84,7% dos pacientes com resultados satisfatórios, seguindo-se protocolo de avaliação preestabelecido pelo autor. Conclusões: A técnica utilizada demonstrou-se de fácil aplicabilidade, com bons resultados, principalmente quando aplicada na idade correta, como indica o protocolo de tratamento de pacientes fissurados da instituição.

Aesthetic and functional evaluation of rhinoplasty using autologous cartilage grafts

ABSTRACT

Introduction: Rhinoplasty is considered one of the most challenging procedures in the plastic surgery. Therefore, several techniques were developed to reach more natural and predictable results. These techniques were created from basic principles, which the adequate maintenance of the nose structures is essential for the aesthetic and functional treatment during rhinoplasty. Methods: The authors presents twelve patients that were operated between January of 2008, and March of 2009. They all were submitted to rhinoplasty for aesthetic and functional deformities corrections. During the procedures there were used autologous grafts from septal cartilage, auricular shell and rib, according to the criteria presented by the author. Results: The most used grafts were the strut grafts (100%), spreader grafts (92%), alar contour grafts (58%) and lateral crural strut grafts (33%). There were no suffering or pathological scars on the operated cases. Among the complications in the donated areas, were a case of hematoma in the ear, a case of hypertrophic scar in thorax, and two cases of pustules on septal mucous membrane. The aesthetic result was satisfactory for the patients in 92% of the cases, while from the functional point of view, in 58% of the cases there was an improvement in the breathing quality, and in 42% the function remained unchanged. Conclusion: The use of autologous cartilage grafts demonstrated effectiveness in the aesthetic and functional deformities treatment, providing appropriate support to the osseocartilaginous skeleton and to the internal and external nasal valves, with acceptable complications rate and little morbidity on donated areas.

RESUMO

Introdução: A rinoplastia é considerada um dos procedimentos mais desafiadores da cirurgia plástica. Diante disso, diversas técnicas foram desenvolvidas com o objetivo de alcançar resultados mais naturais e previsíveis. Essas técnicas foram criadas a partir do princípio básico de que a manutenção das estruturas de suporte do nariz é fundamental para o tratamento estético e funcional durante a rinoplastia. Método: Os autores apresentam uma série de doze pacientes operados entre janeiro de 2008 e março de 2009, submetidos à rinoplastia para correções de deformidades estéticas e funcionais. Durante os procedimentos, foram utilizados enxertos autólogos de cartilagens do septo, concha auricular e costela. Resultados: Os enxertos mais utilizados foram os strut grafts (100%), spreader grafts (92%), alar contour grafts (58%) e lateral crural strut grafts (33%). Não houve sofrimento ou cicatrizes patológicas nos casos operados. Entre as complicações nas áreas doadoras, foram observados: um caso de hematoma em orelha, um caso de cicatriz hipertrófica em tórax e dois casos de pústulas em mucosa septal. O resultado estético foi satisfatório para os pacientes em 92% dos casos enquanto, do ponto de vista funcional, em 58% dos casos, houve melhora na qualidade da respiração e, em 42%, a função se manteve inalterada. Conclusão: A utilização de enxertos autógenos de cartilagem se mostrou efetiva no tratamento das deformidades estéticas e funcionais, proporcionando suporte adequado ao esqueleto osteocartilaginoso e às válvulas internas e externas nasais, com índice de complicações aceitáveis e pequena morbidade das áreas doadoras.

Otoplasty using cartilage abrasion and perichondrioplasty and conchamastoide suture

ABSTRACT

Introduction: Prominent ear has been treated with many different methods. The surgical correction of the conchoscaphal angle and of the distance between the antihelix rim and temporal scalp has been performed both by Brazilian and foreign surgeons. Methods: This paper describes our experience with the combination of different surgical techniques in the correction of prominent ears of 80 patients. The method includes the abrasion of the antihelix, so as to weaken it, and then its reshaping with perichondrioplasty. Concha was attached to the mastoid after removal of the posterior auricular muscle. Results: All patients were operated in the morning, with local anesthesia and sedation, and were discharged in the same day. The result shows a very natural look due to the weakening of the cartilage by abrasion, not breakage, and its reshaping with adequate sutures.

RESUMO

Introdução: A deformidade congênita conhecida como orelha proeminente já foi tratada com diferentes técnicas. Diversos métodos utilizados para a correção do ângulo céfalo-auricular e escafo-conchal são utilizados tanto por autores estrangeiros como brasileiros. Método: Este trabalho foi realizado com 80 pacientes, visando demonstrar a experiência do autor sênior na combinação de diversas técnicas. O tratamento da anti-hélix foi feito por meio da raspagem da cartilagem, buscando seu enfraquecimento, e posterior modelação, com a pericondrioplastia, enquanto o tratamento da concha foi feito pela sua fixação na mastóide após esvaziamento. Todos os pacientes foram operados no período da manhã sob anestesia local e sedação, com alta no mesmo dia. Resultados: A técnica resulta em aspecto visual bastante natural, evitando assim o "estigma de orelha operada", já que a cartilagem não é quebrada, sendo somente enfraquecida e posteriormente remodelada com pontos.

Photograph standardization and evaluation in periorbital surgery

ABSTRACT

Introduction: For comparative analysis of photographs the standardization becomes necessary. Unlike other regions of the body, the periorbital region has an extremely dynamic anatomy, that makes standardization a challenge. There are nine parameters if not standardized can influence the objective evaluation of periorbital region photographs. These parameters are: the facial coronal plane, camera's plane, the gaze position, camera height in relation to the patient, the use of flash, facial movements, distance of the patient to the camera, camera lens focal length (zoom) and lighting conditions. Objective: The objective of this study is to demonstrate that small variations in the parameters mentioned above can cause significant changes in the periorbital elements. Methods: To make the standardization an adapted ophthalmologic slit lamp was used. Twelve patients were photographed in front profile using the standardization and observed the change of the palpebral fissure measured at mid pupillary line by changing the coronal plane, gaze's position, use of flash and height variation of the camera. Results: With the parameters changes we obtained statistically changes of vertical palpebral fissure in all patients. Conclusion: The changes in the position used at work are subtle and hardly noticed when we took pictures free no setting plans and distances, falsifying results in objective analysis of the photographs when not adequately standardized.

RESUMO

Introdução: Para análise comparativa de fotografias é necessário haver padronização. Diferentemente de outras regiões do corpo, a região periorbital, por se constituir de uma anatomia extremamente dinâmica, torna a padronização um desafio. Existem nove parâmetros que, se não padronizados, podem influenciar a avaliação objetiva de fotografias da região periorbital. Entre estes parâmetros temos o plano facial coronal, plano da câmera fotográfica, posição do olhar, altura da câmera em relação ao paciente, uso do flash, mímica facial, distância do paciente à câmera fotográfica, distância focal da câmera fotográfica (zoom) e as condições de iluminação. Objetivo: O objetivo deste estudo é demonstrar que pequenas variações nos parâmetros citados anteriormente podem provocar alterações significativas nos elementos periorbitais. Método: Para manutenção dos nove parâmetros necessários à padronização foi utilizada uma lâmpada de fenda oftalmológica adaptada. Doze pacientes foram fotografados em perfil frontal, utilizando a padronização, sendo observada a variação da fenda palpebral medida na linha médio pupilar, alterando-se o plano coronal, posição do olhar, uso do flash e variação da altura da câmera. Resultados: Com a variação dos parâmetros foi obtida alteração significativa da fenda palpebral vertical em todos os pacientes. Conclusão: As mudanças de posição utilizadas no trabalho são sutis e dificilmente percebidas quando tiramos foto livre sem fixar os planos e distâncias, falseando resultados na análise objetiva das fotografias quando não adequadamente padronizadas.

Modified minilifting results: prospective study

ABSTRACT

Introduction: The modified mini-facelift is based on a standardized tecnic consisting of a minimum undermining marked preoperatively. The plication of the muscle-aponeurotic superficial (SMAS) is done pulling all the tissues of the face, promoting facial rejuvenation with the maintenance of a flap with a lower risk of necrosis by maintaining smaller undermining area and minimum area for hematomas. Methods: We operated on 36 patients undergoing rhytidectomy technique of modified mini-lifting as they fulfilled the some criteria. Anthropometric data were collected, a questionnaire about the patient's history was performed. Results: From February 2008 to March 2009, 36 patients were consecutively operated by the modified mini-facelift technique. The age ranged from 44 to 67 years, with a mean 57 anos. The patients return to normal activities ranged from 7 to 14 days, a period that coincides with a reduction in more than 80% of postoperative edema. The vacuum drain was removed the next morning (1st day after surgery) in 34 patients (94.4%), the 2nd postoperative day in 1 (2.94%) case and in 1 (2.94%) case of postoperative hematoma. There were no cases of necrosis of the flap. There was 1 (2.94%) neuropraxia facial which was resolved spontaneously. Conclusion: The technique of modified mini-lifting is satisfactory for presenting good results, low complication rate and early return of patients to their activities.

Introdução: O mini-lifting modificado se baseia em uma técnica padronizada, que consiste em um descolamento mínimo demarcado no pré-operatório. A plicatura do sistema músculo-aponeurótico superficial (SMAS) é realizada tracionando-se todos os tecidos da face, a fim de promover o seu rejuvenescimento com a manutenção de um retalho que apresenta menor risco de necrose por ter menor descolamento e, por isso, uma mínima área para formação de hematomas. Método: Foram operadas 36 pacientes submetidas a ritidoplastia pela técnica do mini-lifting modificado, que preencheram critérios pré-estabelecidos. Foram coletados os dados antropométricos e um interrogatório sobre a história clínica do paciente realizado. Resultados: No período de fevereiro de 2008 a março de 2009, foram operadas consecutivamente 36 pacientes pela técnica de mini-lifting modificado. A idade variou entre 44 e 67 anos, sendo a média de 57 anos. O retorno das pacientes às atividades habituais variou de 7 a 14 dias, período que coincide com a redução em mais de 80% do edema pós-operatório. O dreno a vácuo foi retirado na manhã seguinte em 34 (94,4%) pacientes, no 2º dia pós-operatório em 1 (2,94%) caso e houve 1 (2,94%) caso de hematoma pós-operatório. Não houve caso de necrose do retalho. Houve 1 (2,94%) caso de neuropraxia do facial, que foi resolvida espontaneamente. Conclusão: A técnica de mini-lifting modificado foi considerada satisfatória, por apresentar bons resultados, baixo índice de complicações e retorno precoce das pacientes às suas atividades.

ABSTRACT

Background: The surgical procedures searching the anti aging face come evolving since 1920. The first stories described small resections of skin that if had disclosed inefficacious total. The gradual requirement for better resulted caused the search for more efficient surgical procedures mainly in what it says respect to the treatment of the cervical region of face and neck where the deformities were badly corrected or presented precocious return. Objective: The objective of this study was to introduce a new approach for treatment of the cervical region of face and neck, mainly in the patients who present great flaccidity of skin and the muscles platysma, with or without excess of fat in the cervical region, therefore with the current behaviors still we have resulted unsatisfactory. Method: One hundred and eighty patients had been operated, following the technique of gravitational cervical lifting, and followed during five years (January of 2003 to May of 2008). There were 172 (96.6%) women and 8 (4.4%) men. The average age was 57.7 years old, the youngest was 41 years old and the oldest was 81 years old. Conclusions: In most cases is done extensive lipectomy using scissors, complete resection of the platysma bands and their traction vector lower. Besides, it made the skin traction in the cervical area in oblique sense to the line of hair implantation in the cervical area. The durable effect and the success of this technique can be proved by the increase in the procedure number performed in this period and by the great patient's satisfaction.

RESUMO

Introdução: Os procedimentos cirúrgicos visando ao rejuvenescimento facial vêm evoluindo desde 1920. Os primeiros relatos descreviam pequenas ressecções de pele que se revelaram totalmente ineficazes. A exigência progressiva por melhores resultados ocasionou a busca por abordagens cirúrgicas mais eficazes, principalmente no que diz respeito ao tratamento do terço inferior da face e pescoço, onde as deformidades eram mal corrigidas ou apresentavam recidiva precoce. Objetivo: Apresentar uma nova abordagem para o tratamento do terço inferior da face, principalmente nos pacientes que apresentam grande flacidez de pele e dos músculos platismas, com ou sem excesso de gordura na região cervical, pois com as condutas atuais existe elevada taxa de resultados insatisfatórios. Método: Cento e oitenta pacientes foram operados, seguindo técnica proposta pelo autor sênior, e acompanhados durante cinco anos (janeiro de 2003 a maio de 2008) foram avaliados. Foram analisadas 172 (95,6%) pacientes do sexo feminino e 8 (4,4%) do masculino. A média de idade foi de 57,7 anos, tendo 41 anos e 1 mês o mais jovem e 81 anos e 5 meses o mais idoso. Na maioria dos casos, realiza-se extensa lipectomia com tesoura, completa dissecção das bandas platismais com posterior ressecção total da flacidez das bandas platismais e seu tracionamento vetorial inferior, além da tração oblíqua da pele na região cervical em sentido à linha de implantação do cabelo na região cervical. Conclusão: Os efeitos duradouros e o sucesso da técnica podem ser comprovados pelo crescente número de procedimentos realizados neste período e pela grande satisfação dos pacientes.

Evaluation of the frontal and glabelar wrinkles after multiple botulinum toxin type A injections

ABSTRACT

Methods: Wrinkles in the frontal and glabellar regions of the face were evaluated in patients submitted to a series of cosmetic botulinum toxin applications, with analysis of the intervals between applications and doses used. Twenty-four female patients who received 5 to 7 botulinum toxin applications to the upper third of the face were evaluated. Mean follow-up time was 42.2 ± 4 months and the mean interval between applications was 8 ± 0.51 months. The forehead and glabellar wrinkles were evaluated by a standardized scale comparing photos taken prior to the first application, at rest and during facial expressions. Results: Over time, static wrinkles in the frontal region were reduced in 62.5% of the patients and remained the same in 37.5%, while static wrinkles in the glabellar region were reduced in 47.2% of the patients, remained the same in 50% and were slightly accentuated in 2.8%. Dynamic wrinkles of the frontal region were reduced in 84.7% of the patients and remained the same in 15.3%, while the vertical dynamic wrinkles in the corrugator region were reduced in 63.9% of the patients, remained the same in 26.4% and were slightly accentuated in 9.7%. Horizontal dynamic wrinkles in the procerus region were reduced in 57% of the patients, remained the same 32% and were slightly accentuated in 11%. The mean total dose of toxin used per session was 43.83 ± 1.25 U, and the total cumulated dose was 272.08 ± 20.42 U.

RESUMO

Método: Foram avaliadas as rugas das regiões frontal e glabelar da face em pacientes submetidas a aplicações seriadas da toxina botulínica tipo A com finalidade estética, analisando-se os intervalos entre as aplicações e as doses utilizadas. Vinte e quatro pacientes do sexo feminino receberam de 5 a 7 aplicações da toxina botulínica tipo A no terço superior da face. O tempo médio de acompanhamento foi de 42,2 ± 4 meses e o intervalo médio entre as aplicações de 8 ± 0,51 meses. As rugas frontais e glabelares foram avaliadas segundo escala padronizada, comparando-se fotografias previas à primeira e à ultima aplicação, em repouso e em movimentos da mímica facial. Resultados: No longo prazo, as rugas estáticas da região frontal foram amenizadas em 62,5% das pacientes e inalteradas em 37,5%, enquanto as rugas estáticas da região glabelar foram amenizadas em 47,2% das pacientes, inalteradas em 50% e pouco acentuadas em 2,8%. As dinâmicas da região frontal foram amenizadas em 84,7% das pacientes e inalteradas em 15,3%, enquanto as dinâmicas verticais da região glabelar (corrugadores) foram amenizadas em 63,9% das pacientes, inalteradas em 26,4% e pouco acentuadas em 9,7%. As rugas dinâmicas horizontais da região glabelar (prócero) foram amenizadas em 57% das pacientes, inalteradas em 32% e pouco acentuadas em 11%. A média da dose total de toxina botulínica utilizada por sessão foi de 43,83 ± 1,25 U, e a dose total cumulativa foi de 272,08 ± 20,42 U.

Prevention and treatment of capsular contracture after mammary prosthesis implantation

ABSTRACT

Introduction: Capsular contracture is defined as a spherical scar with contraction of the capsule involving the prosthesis. The proposal suggested by this study is to elaborate a protocol to treat and prevent capsular contracture. Methods: Nine patients were evaluated, who developed capsular contracture. Three patients were graded as Baker II, five as Baker III and one as Baker IV. The Baker II patients were clinically treated associating micro electro neuro stimulation (MENS), ultrasound (US) and manual lymphatic drainage (MLD). The Baker III and IV patients had the breast implant replaced, repositioning the implant in submuscular plane, associating the clinical treatment (MENS/US/MLD) and zafirlukaste 20mg oral intake twice daily for a period of 90 days. Results: There was complete resolution of the contracture, returning to Baker grade I in 8 patients (89%). There was reduction on Baker grade from III to II in one patient, even after breast implant replacement. Clinically, none patient presented side effects. Conclusion: The association of MENS/US/MLD demonstrated an important tool for the management of grade II contractures. The association of breast implant replacement and repositioning in submuscular plane, associated to the clinical treatment (MENS/US/MLD) and zafirlukaste demonstrated good results in capsular contracture resolution. These results encouraged us to use the MENS/US/MLD to prevent capsular contracture in all breast implant patients.

RESUMO

Introdução: A contratura capsular é definida como uma cicatrização esférica com contração da cápsula que envolve a prótese mamária. A proposta desse trabalho foi elaborar um protocolo para prevenção e tratamento da contratura capsular. Método: Foram avaliadas 9 pacientes que desenvolveram contratura capsular, 3 foram classificadas como Baker II, 5 como Baker III e 1 como Baker IV. As pacientes com contratura capsular Baker II foram tratadas clinicamente, associando-se microcorrente (MENS), ultrassom (US) e drenagem linfática manual (DLM). Nos casos de contratura grau III e IV de Baker, substituiu-se a prótese mamária, reposicionando-a em plano submuscular, associando-se tratamento clínico (MENS/US/DLM) e zafirlucaste 20 mg, via oral, de 12/12 horas, por 90 dias. Resultados: Oito (89%) pacientes tiveram resolução completa da contratura, retornando a Baker I e uma (11%) apresentou redução da contratura de Baker III para Baker II, mesmo após substituição da prótese mamária. Clinicamente, nenhuma paciente apresentou efeitos colaterais. Conclusão: A associação MENS/US/DLM mostrou-se como uma importante ferramenta no tratamento das contraturas Baker II. Nos casos de contratura Baker III e IV, a associação MENS/US/DLM, reposicionamento da prótese no plano submuscular e utilização do zafirlucaste demonstrou bons resultados na resolução das contraturas. Os resultados obtidos encorajaram-nos a utilizar a associação MENS/US/DLM de maneira preventiva em todas pacientes submetidas ao implante de prótese de silicone.

Breast reconstruction of the lower quadrants after quadrantectomies

ABSTRACT

Introduction: In the surgical treatment for breast cancer, the quadrantectomies, when done by the simple approximation of the wounded borders, can result in difficult solution sequelae. Methods: The authors present a standardization reconstruction of the lower breast quadrants after quadrantectomies dividing the region into three areas of localization for the resection wound, suggesting specific solutions for each of them, by classic techniques as well as new local flaps procedures. They present the results, the follow-up and the limitations of the utilized techniques in a critical comparative analysis with available reported data.

RESUMO

Introdução: No tratamento cirúrgico do câncer de mama, as quadrantectomias, quando reparadas por meio da simples aproximação das bordas da área cruenta resultante, podem levar a sequelas de difícil solução. Métodos: Os autores apresentam uma padronização para a reconstrução dos quadrantes inferiores da mama após quadrantectomias, subdividindo a região em três áreas de localização do defeito gerado pela ressecção e sugerindo condutas específicas para cada uma destas, por meio da utilização de técnicas clássicas, bem como de inovações em tratamento local com retalhos. Apresentam os resultados, seguimento e limitações das técnicas empregadas, fazendo uma análise comparativa crítica com os dados encontrados na literatura disponível.

Using prosthesis in breast reduction

ABSTRACT

Plastic surgery of the breast is one of the most popular in the world. These patients had breast enlargement, with large excess of loose skin. In such cases, the aesthetic result of breast reduction is unsatisfactory in most patients. It is reflected in the early return of these patients requesting the placement of breast implants. Based on, patient satisfaction, fewer cases of reduction mammoplasty revision, suggests the need to perform breast reduction with breast implant surgery, at the same time, in many selected cases.

RESUMO

A cirurgia plástica da mama é uma das mais realizadas no mundo. Em alguns casos de hipertrofia mamária, observa-se um grande excesso de pele e flacidez. O resultado estético da mamoplastia redutora é insatisfatório na maioria dos pacientes, ocasionando o retorno precoce destas pacientes, solicitando a colocação de implante mamário para a complementação do seu resultado. A observação de bons resultados, a satisfação das pacientes e a diminuição de casos complementares à mamaplastia redutora confirmam a necessidade de se realizar a mamoplastia redutora com colocação de implante mamário, num mesmo tempo cirúrgico, em casos selecionados.

Anatomic study of the arcuate line and its importance on breast reconstruction

ABSTRACT

Objective: The knowledge and the importance of the location of the arcuate line (AL) and its relation to anatomic landmarks in breast reconstruction with a transverse rectus abdominis musculocutaneous (TRAM) flap. Methods: Human cadaveric abdominal walls of 17 formalin-phenol-alcohol solution fixed cadavers were dissected and the AL, inferior deep epigastric vessels and its intersection with the lateral rectus abdominis muscle sheath were identified. With the abdominal cavity opened, the AL was identified by transilumination. The distance between AL to the pubic symphysis, to the umbilicus and to the level of the anterior superior iliac spines (ASIS) were measured. Results: The AL was located at the upper third, between the umbilicus and pubic symphysis in 64% of the dissections. The distance of AL to umbilicus was found to be at a mean of 4.2 ± 1.6 cm and to pubic symphysis was 10.4 ± 1.5 cm. The AL was always located superior to the line draw between the ASIS at a mean of 2.7 ± 1.4 cm. Conclusion: The relationship between AL and surface anatomic landmarks such as ASIS, umbilicus and pubic symphysis is important to a better preoperative knowledge of the AL location and it helps preoperative planning.

RESUMO

Objetivo: Ressaltar a importância da localização da linha semicircular, e suas relações anatômicas, durante as reconstruções mamárias com retalho miocutâneo de reto-abdominal. Método: A parede abdominal de 17 cadáveres formolizados foi dissecada por planos, identificando-se a linha semicircular (LSC), os vasos epigástricos profundos inferiores e sua intersecção com a borda lateral do músculo reto-abdominal. A cavidade abdominal também foi aberta e visualizada a LSC sob transiluminação. Mediu-se a distância da LSC à cicatriz umbilical e ao púbis, além da distância de uma linha imaginária entre as espinhas ilíacas ântero-superiores. Resultados: Em 64% das dissecções, a LSC localizava-se no 1/3 superior da distância compreendida entre a cicatriz umbilical e o púbis. A média da distância da cicatriz umbilical à LSC foi de 4,2 ± 1,6 cm. A da LSC ao púbis foi de 10,4 ± 1,5 cm. A linha biespinha estava localizada sempre abaixo da LSC, estando disposta a cerca de 2,7 ± 1,4 cm. Conclusão: A projeção da LSC a partir da linha biespinha, assim como da cicatriz umbilical ou do púbis, nos oferece pontos externos de referência importantes para a localização da LSC e, portanto, para um planejamento pré-operatório mais seguro.

Neoomphaloplasty in vertical abdominoplasty

ABSTRACT

Introduction: Although beeing a scar, the umbilicus carries important aesthetic and sensual connotation. The layout and appearance of the periumbilical resulting scar is of great concern to our patients. In classical abdominoplasty, the umbilicus is kept in its original position and, through a hole created in the flap, it is reattached. It is created, so a scar that surrounds the entire perimeter of the umbilicus providing all sorts of unsightly results. Methods: We advocate the use of neoomphaloplasty with two lateral flaps routinely in vertical or anchor abdominoplasties. To be more precise in positioning the umbilicus, coinciding the new one with the original site, we developed a security dermoepidermal band. Marking and technical details as well as the results are demonstrated in this article.

Introdução: Apesar de ser uma cicatriz, o umbigo carrega importante conotação estética e sensual. A forma final e o aspecto da cicatriz periumbilical resultante é motivo de grande preocupação aos nossos pacientes. Na abdominoplastia clássica, o umbigo é mantido em sua posição original e, através de um orifício criado no retalho, a este é refixado. Será criada, assim, uma cicatriz que circunda todo o perímetro do umbigo, sujeita, é claro, a toda a sorte de resultados inestéticos. Método: Preconizamos a utilização da neo-onfaloplastia com dois retalhos laterais de forma rotineira na abdominoplastia vertical ou em âncora. Para maior precisão do seu posicionamento, coincidindo com o sítio original, desenvolvemos a faixa dermo-epidérmica de segurança. Os detalhes da marcação e técnica, assim como os resultados são demonstrados neste artigo.

Classic abdominoplasty associated with sectorial vibroliposuction

ABSTRACT

Introduction: The authors propose a technique to treat flaccidity and excess of fat panicle in the upper and central area of the abdomen using the classic abdominoplasty associated with sectorial vibroliposuction non tumescent. The objective is to create a relief of saliencies and depressions in the abdominal area, reduce the projection of the epigastrium and to position the suprapubic transverse scar in a lower place. Method: Between September 2007 and January 2009, 16 female patients, with ages between 24 and 60 years old, with in abdominal group V, according to the classification of Bozola and Psillakis, and with an excess of fat tissue in the upper abdomen underwent classic abdominoplasty associated with sectorial vibroliposuction. Results: All the patients of the process were pleased with the results obtained. In 2 cases, were detected the presence of seromas which had been drained through ambulatory punctures. In another patient, there was hypertrofic scar, which was treated with infiltration of triamcinolone resulting in the complete regression of the hypertrophy. Nodulations in the central area of the abdomen were observed in 2 patients who had undergone superficial vibroliposuction. Conclusion: The procedure is effective in the aesthetic treatment of the abdominal region. When performed at the end of the abdominoplasty in the upper and central part of the abdomen the vibroliposuction improves abdominal contour. It maintains the naturalness of the results and the lower positioning of the transverse suprapubic scar that facilitates its coverage with small clothes. It is secure, fast and easy to implement.

Introdução: Os autores propõem uma técnica para tratar a flacidez e o excesso de panículo adiposo nas regiões superior e central do abdome por meio da abdominoplastia clássica associada à vibrolipoaspiração setorial não tumescente. O objetivo é criar um relevo de saliências e depressões na região abdominal, reduzir a projeção do epigástrio e posicionar a cicatriz transversal suprapúbica em local baixo. Método: Entre setembro de 2007 e janeiro de 2009, 16 pacientes do sexo feminino, com idades entre 24 e 60 anos, com abdomes do grupo V na classificação de Bozola e Psillakis e excesso de tecido adiposo no abdome superior foram submetidos à abdominoplastia clássica associada à vibrolipoaspiração setorial. Resultados: Todas as pacientes da série encontram-se satisfeitas com os resultados obtidos. Em 2 pacientes, detectou-se a presença de seromas, que foram drenados por meio de punções ambulatoriais. Em outra paciente, houve cicatrização hipertrófica, tratada com infiltração de triancinolona e regressão completa da hipertrofia. Nodulações na área central do abdome foram observadas em 2 pacientes, nos quais se realizou vibrolipoaspiração superficial. Conclusão: A técnica descrita é eficaz no tratamento estético da região abdominal. A vibrolipoaspiração não tumescente, realizada no final da abdominoplastia nas regiões superior e central do abdome, melhora o contorno abdominal, mantendo a naturalidade dos resultados e o posicionamento mais baixo da cicatriz transversal suprapúbica, o que facilita sua cobertura com vestes menores. É de execução fácil, segura e rápida.

Gluteal prosthesis: use of liposuction on the improvement of results

ABSTRACT

Introduction: The liposuction brought new possibilities for the improvement of the body contour. The use of prosthesis in order to increase the gluteal region brought interesting options for the treatment of moderate and accentuated hypertrophy, in which the lipograft did not fulfill the expected results. The purpose of the present study was to analyze the results of increase gluteoplasty, its association with liposuction, the surgical technique used, its results and its complications. Method: The study was developed at the Daher Hospital Study Center, retrospectively, where 41 patients had been submitted to increase gluteoplasty with the use of silicone prosthesis, between January 2005 and August 2010. The technique used was of placement of intramuscular prosthesis with incision in the intregluteal sulk. The liposuction was associated or not according to the judgment concerning its necessity. Results: From 41 patients, 40 were females and 1 male, averaging the age of 44.38 years old, ranging between 22 to 65 years of age. The prosthesis volume was ranging between 240 to 360 ml. The association among the surgeries happened in 70% of the patients. The complications were noticed in two patients. Discussion: The liposuction and the gluteal prosthesis appeared as an option for these big corrections, as the lipografting does not reach satisfying results in these cases, due to the lipoabsorption and fatty necrosis. Conclusion: Association of gluteal prosthesis with liposuction is a not so complex procedure, offering good results, with an improvement of the body contour and low rates of complication.

Introdução: A lipoaspiração trouxe novas possibilidades para a melhora do contorno corporal. O uso de próteses para aumentar a região glútea trouxe interessantes opções para tratamento de hipotrofias moderadas e acentuadas, nas quais a lipoenxertia não atingiu resultados satisfatórios. O objetivo deste estudo foi analisar os resultados da gluteoplastia de aumento, sua associação com lipoaspiração, a técnica cirúrgica usada, seus resultados e suas complicações. Método: O estudo foi realizado no Centro de Estudos do Hospital Daher, onde foram analisados 41 pacientes, retrospectivamente, submetidos à gluteoplastia de aumento com prótese de silicone, no período de janeiro de 2005 a janeiro de 2010. A técnica utilizada foi de colocação de prótese intramuscular com incisão no sulco interglúteo. A lipoaspiração foi associada ou não, de acordo com o julgamento quanto a sua necessidade. Resultados: Dos 41 pacientes, 40 eram do sexo feminino e 1 do sexo masculino, com média de idade de 44,38 anos, variando de 22 a 65 anos. O volume da prótese variou de 240 a 360 ml. A associação entre as cirurgias ocorreu em 70% dos pacientes. As complicações ocorreram em duas pacientes. Discussão: A associação entre lipoaspiração e prótese glútea na busca de melhores resultados possibilitou a correção de grandes lipodistrofias, para melhora do contorno corporal, quando a lipoenxertia não traz resultados satisfatórios. Conclusão: A associação de prótese glútea com lipoaspiração é uma conduta segura, de baixa complexidade, que apresenta bons resultados no contorno corporal, com baixas taxas de complicação.

Scrotum reconstruction after necrotizing fasciitis

ABSTRACT

Background: Necrotizing fasciitis of the perineal region is one of the most serious infections. We present analysis of the treatment of patient victims of necrotizing fasciitis submitted to reconstruction of scrotum with remnants of skin and muscle. Methods: Eleven patients were submitted to the covering of the raw areas with local mio-cutaneous flaps and superomedial thigh flap. Results: The patients presented good evolution with resolution of the raw surfaces and long lasting results. Conclusions: We observe that, with an ample and careful dissection of remaining skin of the scrotum, we can cover extensive areas, providing adequate protection of the testicules. In largest areas the superomedial thigh flap comes as an excellent alternative of simple execution and good aesthetic and functional outcomes.

RESUMO

Introdução: Fascite necrosante está entre as mais graves infecções. Apresentamos análise do tratamento de pacientes vítimas de fascite necrosante da região perineal, submetidos à reconstrução do escroto com retalhos de pele e músculo remanescentes e retalho súpero-medial da coxa. Método: Foram analisados, de forma retrospectiva, 11 pacientes submetidos à cobertura das áreas cruentas com retalhos miocutâneos locais ou retalho súpero-medial da coxa. Resultados: Os pacientes apresentaram boa evolução com resolução da área cruenta, resultados duradouros e reproduzíveis. Conclusões: Observamos que, com dissecção ampla e cuidadosa da pele remanescente do escroto, podemos cobrir extensas áreas cruentas, proporcionando proteção adequada aos testículos. Como alternativa para a cobertura de lesões maiores, o retalho súpero-medial da coxa demonstrou boa aplicabilidade e simplicidade na sua execução.

Closed degloving injuries: Morel-Lavallée lesion

ABSTRACT

Introduction: The closed degloving injuries, also called Morel-Lavallée lesion (MLL), are uncommon and often associated with severe trauma. They involve tangential forces able to separate the skin and subcutaneous tissue from the underlying muscle fascia. Objective: This article aims to describe five cases of MLL treated in the Department of Plastic Surgery, Santa Casa de São Paulo. Methods: Forty cases of trunk and limbs degloving injuries treated between 2002 and 2008 were retrospectively analyzed, being 35 cases of conventional or open deglovings and the remaining 5 of MLL. Results: The average degloved body surface was 13.2 ± 1.3%. The lower limbs were affected sites in all patients and in 2 cases were associated with lesions in the lower trunk. In 3 cases the diagnosis was not made early, being evaluated and treated after 2 to 17 days of hospitalization. In 2 patients the primary graft was used (with the skin of injured area), and in other 2 patients a initial debridement followed by skin grafting after granulation was used. Conclusions: The treatment of patients suffering from MLL requires a multidisciplinary approach and adherence to basic surgical principles, with accurately determination of the viable areas and the restoration of skin coverage. It is observed that the lesions are aggravated by mistakes and delays in the initial steps, associated with increased morbidity and longer hospitalization and rehabilitation times.

Introdução: Os desenluvamentos fechados, também chamados de lesão de Morel-Lavallée (LML), são lesões incomuns, frequentemente associadas a traumatismos graves. Envolvem forças tangenciais capazes de separar a pele e o tecido subcutâneo da fáscia muscular subjacente. Objetivo: Este trabalho tem como objetivo a revisão de cinco casos de LML atendidos no Serviço de Cirurgia Plástica da Santa Casa de São Paulo. Método: Foram analisados, retrospectivamente, os prontuários de 40 casos de desenluvamentos de tronco e membros atendidos entre 2002 e 2008, dos quais cinco correspondiam a LML e 35 casos a desenluvamentos convencionais ou abertos. Resultados: A superfície corporal desenluvada média foi de 13,2 ± 1,3 %. Os membros inferiores foram locais acometidos em todos os pacientes e, em 2 casos, houve associação com lesões no tronco inferior. Em 3 dos casos, o diagnóstico não foi realizado de maneira precoce, sendo avaliados e tratados após 2 a 17 dias de internação. Em 2 pacientes, foi realizada enxertia primária e em outros 2 casos foi realizado desbridamento inicial e enxertia de pele após granulação. Conclusões: O tratamento de pacientes vítimas de LML requer uma abordagem multidisciplinar e respeito a princípios cirúrgicos básicos, como a correta determinação das áreas viáveis e o restabelecimento da cobertura cutânea. Observa-se que as lesões são agravadas pelos erros e atrasos nas condutas iniciais, com maior morbidade associada e maiores tempos de internação e reabilitação.

Clinical and surgical applications of the forearm flap: analysis of 89 cases

ABSTRACT

Introduction: In the search of skin segments that allow the transport among areas, the fascio-cutaneous forearm flap or Chinese flap arised. It was originated in anatomical studies accomplished by Fan, Qui and Zi, at the Shenyang Military Hospital, in 1978. The objective of this study is to demonstrate the multiple applications and the results obtained with the antebraquial flap. Method: The sample counted with data regarding 89 patients; 84.26% were male and 15.74% female. The forearm flap was used for covering the hand and fist in 61 patients, for thumb reconstruction in 10 patients, as free flap in 15 and in 3 patients it was used with direct flow. Results: Among the 89 patients, 2 developed total flap necrosis. One case developed slightly necrosis of the skin. None of the patients presented blood supply alterations in the hand after the radial or ulnar artery ligature. There was satisfactory covering of the areas, with good results. Conclusions: The antebraquial or "Chinese" flap is indicated for tegument covering in any area of the human body, with good aesthetic and functional results, so much for the donor area as for the rebuilt area.

Introdução: Na procura de segmentos de pele que permitam o transporte entre regiões próximas ou distantes, surgiu o retalho fáscio-cutâneo de antebraço, conhecido por retalho chinês, que teve sua origem em estudos anatômicos realizados por Fan, Qui e Zi, no Shenyang Military Hospital, em 1978. O objetivo deste trabalho é demonstrar as múltiplas aplicações e os resultados obtidos com o retalho antebraquial. Método: A amostra contou com dados referentes a 89 pacientes. Destes, 84,26% eram do sexo masculino e 15,74% do sexo feminino. O retalho foi utilizado para cobertura na mão e punho em 61 pacientes, para reconstrução de polegar em 10, como retalho livre em 15 e, em 3 pacientes, foi usado com fluxo anterógrado (direto), para cobertura de exposição óssea no cotovelo e em terço proximal de antebraço. Resultados: Entre os 89 pacientes operados, 2 evoluíram para necrose total do retalho. Em um caso houve necrose da pele, permanecendo a fáscia íntegra. Nenhum dos pacientes apresentou alteração na vascularização da mão após a ligadura da artéria radial ou da artéria ulnar. Houve cobertura satisfatória das áreas, com bons resultados. Conclusão: O retalho antebraquial ou "chinês" é indicado para coberturas tegumentares em qualquer região do corpo humano, com bons resultados estéticos e funcionais, tanto para a área doadora quanto para a área reconstruída.

ABSTRACT

Introduction: The thumb is the more important finger of the hand and his lost cause a big incapacity. The purpose of this trial is precognize the pollicization of the four finger technique showing your principles, techniques and advantages. Methods: Six patients were treated in different levels of thumb amputation. Results: In all of the cases were obtained satisfactory results, with return of the functional capacity of the hand.

Introdução: O polegar é o dedo mais importante da mão e sua perda provoca grande incapacidade à mesma. Método: Foram tratados seis pacientes em diferentes níveis de amputação do polegar. Resultados: Em todos os casos, obtivemos resultados satisfatórios, com retorno da capacidade funcional da mão.

Tamoxifen as treatment to keloids and hypertrophic scars

ABSTRACT

Background: Keloids and hypertrophic scars (HScs) are poorly understood cicatricial lesions without a gold-standard treatment. Recent experiments demonstrated that cytokines imbalance has an action in scar pathogenesis, mainly due to transforming growth factor (TGF) β#1 overexpression. Tamoxifen, an antioestrogenic drug, can inhibit TGF-β1 release and consequently fibroblastic proliferation. This indicates a possible role of Tamoxifen as a therapeutic option in keloids and HScs. Method: Eighteen patients were selected for the evaluation of tamoxifen citrate 0.1% for six months. During that evolution, photographic registration, dimensional evaluation and hystopathologic analisis of the lesions were accomplished and questionnaires about aspects of the lesion were applied to patients. Results: There was a significant reduction of the lesion height (p < 0.0001), width (p = 0.009) and length (p = 0.009). Texture criteria, height and itch were the ones that obtained the largest variations among the periods before and after treatment. Histological changes suggested remodelling action of the collagen in those scars. Conclusions: Tamoxifen citrate presented positive results in most of the analyzed criteria, minima side effects and low cost, constituting a safe and comfortable treatment option, mainly in HScs. There will be necessary more research with larger sampling for greater conclusions.

Introdução: Existe pouco entendimento a respeito dos mecanismos que levam à formação dos quelóides e cicatrizes hipertróficas (CHT), assim como não há um tratamento padrão-ouro para essas lesões. Experiências recentes demonstraram que as alterações na formação dessas cicatrizes envolvem concentrações desequilibradas de fatores de crescimento locais, em particular o fator de crescimento transformador (TGF) β1. O tamoxifeno, ao inibir a produção desta citocina, pode ser uma nova opção na conduta terapêutica destas lesões. Método: Dezoito pacientes portadores de quelóides e CHT foram selecionados para a avaliação do uso do citrato de tamoxifeno 0,1% por um período de seis meses. Durante essa evolução, foi realizado registro fotográfico, mensuração e análise histopatológica das lesões e questionários relativos a aspectos da lesão foram aplicados aos pacientes. Resultados: Houve redução significativa da altura (p < 0,0001), da largura (p = 0,009) e do comprimento (p = 0,009) das lesões. Os critérios de textura, altura e prurido foram os que obtiveram as maiores variações entre os períodos pré e pós-tratamento. As alterações histológicas sugerem uma ação de remodelamento do colágeno nessas cicatrizes. Conclusões: O citrato de tamoxifeno apresentou uma resposta favorável na maioria dos critérios analisados, mínimos efeitos colaterais e baixo custo, constituindo uma opção de tratamento segura e cômoda, principalmente nas CHT. São necessários novos estudos com maior amostragem para obtenção de conclusões mais abrangentes.

Surgical treatment cleft craniofacial of middle line

ABSTRACT

The clefts craniofacial constitute congenital rare anomalies. They are located in determined lines through the eyelids, eyebrows, nostrils, lips and jawbones, reaching soft and bone parts. They are of unknown etiology. The classification most used for the rare clefts of face is it of Tessier. It is based on anatomical relations obtained in clinical observations and post-operative finds. The clefts numbers 0 and 14, positions himself in the middle line of the skull and of the face. Can be present the characteristics is excess of tissue or the lack of the same. The correct diagnose is essential for the surgical approach. The treatment is based on suture of the soft cloths, on three plans: cutaneous, muscular and mucous; Zplasty; treatment toothing; bone grafts, when necessary. The authors describe the case of a 2-year-old patient and 3 months, masculine, 0 and 14 showing cracks of Tessier. The first correction was based on the treatment of soft tissue defect of these cracks, planned in two surgical times.

RESUMO

As fissuras craniofaciais constituem anomalias congênitas raras. Localizam-se em determinadas linhas através das pálpebras, sobrancelhas, narinas, lábios e maxilas, atingindo partes moles e ósseas. São de etiologia ainda desconhecida. A classificação mais utilizada para as fissuras raras de face é a de Tessier. É baseada em relações anatômicas obtidas em observações clínicas e achados pós-operatórios. As fissuras números 0 e 14 situam-se na linha média do crânio e da face. Podem-se apresentar como uma fissura com as características de excesso de tecido ou agenesia dos mesmos. O correto diagnóstico é fundamental para abordagem cirúrgica. O tratamento é baseado em sutura dos tecidos moles, em três planos: cutâneo, muscular e mucoso; zetaplastias; tratamento ortodôntico; enxertos ósseos, quando necessário. Os autores descrevem o caso de um paciente de 2 anos e 3 meses, sexo masculino, apresentando fissuras 0 e 14 de Tessier. A primeira abordagem baseou-se no tratamento do defeito das partes moles dessas fissuras, sendo realizada em dois tempos cirúrgicos.

ABSTRACT

Background: Breast reconstruction is complete when the nipple-areola complex (NAC) is reconstructed. Models can be used to train young surgeons, it been a simulator exercise. A model of NAC reconstruction training is the purpose of the this paper. Methods: Ten zone IV TRAM flaps were utilized to the purpose of the study. First, the trainee executes the NAC reconstruction using the flaps techniques supervised by a teacher. Afterwards, the technique skills are used on real situation in the operation room. Results: The model was used by all trainees who acquire technique skills, which was used on real situation. Conclusion: The NAC reconstruction in zone IV TRAM flap is feasible.

Introdução: A reconstrução mamária está completa quando é realizada a reconstrução do complexo aréolo-papilar (CAP). Modelos de treinamento têm agregado benefícios, sendo exercícios de simulação. Apresenta-se um modelo para treinamento em reconstrução do CAP. Método: Selecionou-se dez pacientes, nos quais a zona IV do retalho miocutâneo transverso do músculo reto abdominal (TRAM) foi utilizada para instrução em duas técnicas de retalhos para confecção do CAP. Resultados: O modelo de treinamento foi aplicado em todos os casos, tendo os residentes adquirido habilidades e domínio da técnica, demonstrando segurança na reconstrução dos casos clínicos. Conclusão: A reconstrução do CAP com zona IV do retalho TRAM é um modelo de treinamento útil.

Osteoradionecrosis in face: pathophysiology, diagnosis and treatment

ABSTRACT

Osteoradionecrosis is one of the most severe complications from radiotherapy used for treatment of head and neck cancer. It is a disease in which irradiated bone become necrotic and exposed by losing skin and mucosal barrier, remaining without healing. Osteoradionecrosis in face affect mandible in the most of cases, followed by maxilla. The most important risk factors are radiation dose, tumor staging, and presence of periodontal diseases and teeth extractions. Clinical signs and symptoms for diagnosis include local pain, trismus, halitosis, bone exposition, drainage of secretion and fistulization to skin or mucosa. Conservative management can be used in initial cases, for example antibiotics and antifibrotics drugs. More complex cases need for a surgical approach that involves radical debridement of necrotic bone and soft-tissues, associated with a reconstructive surgery. Among the options to reconstruct mandible, the most used are free flaps composed by bone tissues, like fibula flap, iliac crest flap, scapula flap and radial forearm flap. To maxilla, the most used are ântero-lateral thigh flap and radial forearm flap. These flaps are tissues well-vascularized, which provide appropriate filling and cutaneous coverage of the remaining defect. As osteoradionecrosis has an extreme difficult treatment, efforts must be directed to its prevention.

A osteorradionecrose é uma das mais graves complicações da radioterapia utilizada para o tratamento das neoplasias de cabeça e pescoço. Trata-se de uma doença na qual o osso irradiado torna-se desvitalizado e exposto através da perda da integridade da pele e da mucosa, persistindo sem cicatrização. A osteorradionecrose da face acomete a mandíbula na grande maioria dos casos, seguida da maxila. Os fatores de risco mais importantes são a dose de radiação utilizada, o estadiamento tumoral, a presença de doenças periodontais e extrações dentárias. As características clínicas para o diagnóstico incluem dor local, trismo, halitose, exposição óssea, drenagem de secreção e fistulização para pele ou mucosa. As medidas conservadoras podem ser utilizadas nos casos iniciais, como antibioticoterapia e drogas antifibróticas. Já os casos mais complexos exigem abordagem cirúrgica, com desbridamento radical dos tecidos ósseos e partes moles desvitalizados, associado à reconstrução. Dentre as opções existentes para reconstrução da mandíbula, as mais utilizadas são os retalhos livres de tecido ósseo, como o retalho de fíbula, de crista ilíaca, de escápula e radial do antebraço. Já para a maxila, os mais usados são os retalhos ântero-lateral da coxa e radial do antebraço. Trata-se de tecidos bem vascularizados, que fornecem preenchimento e cobertura cutânea adequada do defeito remanescente. Como a osteorradionecrose apresenta um tratamento de extrema complexidade, os esforços devem ser direcionados para sua prevenção.

Diagnosis and treatment of infantile hemangioma

ABSTRACT

Infantile hemangioma is the most common benign tumor of infancy. Incidence is around 2.5 to 5% in caucasian newborns, with predominance in females. For a long period of time, the lack of a single classification accepted worlwide impaired treatment, management and comparisons between treatment groups. In 1996, The International Society for the Study of Vascular Anomalies proposed a classification for vascular anomalies, dividing vascular anomalies in two main groups: vascular tumors and vascular malformations. Infantile hemangioma is the most frequent vascular tumor. There are still several hypothesis for the explanation hemangioma development. Clinically there are three distinct phases, named proliferative, involutive and involuted. Diagnosis is clinically based and imaging methods like magnetic ressonance, ultrasound and interventional radiology may be useful in some cases. The majority of infantile hemangiomas have favorable resolution due to spontaneous regression. However, in some cases complications are present, like ulceration, bleeding, infection or systemic complications. In these cases, active treatment is indicated, as well as in cases of relative indication, depending on size and tumor location. Surgical resection and medical treatment are used. Medications more frequently used are oral corticosteroids, alfa-interferon - with more side effects and nowadays beta-blockers, with good response, less side effects, but still under research protocols.

O hemangioma infantil é o tumor vascular benigno mais frequente nas crianças. Apresenta incidência de 2,5 a 5% em recém-nascidos vivos caucasianos, com predominância no sexo feminino. Durante um longo período não havia uma classificação diagnóstica aceita internacionalmente, dificultando a criação de condutas, bem como comparações entre os diversos tratamentos. Em 1996, a International Society for the Study of Vascular Anomalies dividiu as anomalias vasculares em duas categorias: tumores vasculares e malformações vasculares. O hemangioma a partir deste momento passou a significar um tumor benigno com características peculiares. A sua origem ainda é incerta e diversas hipóteses são formuladas para explicá-la. O quadro clínico apresenta três fases bem definidas: proliferativa, involutiva e involuída. O diagnóstico é realizado por meio da anamnese e do exame físico; quando indicados, ressonância magnética, ultrassom e métodos angiográficos podem ser auxiliares. A maioria dos hemangiomas infantis tem resolução favorável para regressão completa, sem complicações. No entanto, alguns deles podem levar ao comprometimento funcional ou apresentar complicações como ulcerações, infecções, sangramentos ou complicações sistêmicas durante sua evolução. Estes são os casos em que há indicação terapêutica. Entretanto, outras indicações relativas de tratamento podem ser consideradas, devido à localização e às dimensões do tumor. Diversas opções de tratamentos estão disponíveis além do cirúrgico: os tratamentos clínicos, com corticoesteróides (o mais utilizado), o alfa-interferon, com suas indicações um pouco restritas pelos seus efeitos colaterais e, mais recentemente, os beta-bloqueadores, com boa resposta e poucos efeitos colaterais, mas ainda em fase de pesquisa em protocolos clínicos.

ABSTRACT

Case report of female patient, 15 years, after that be subjected to correction of bilateral cleft lip 8 years ago, developed with sequel called lip tight. The disproportion of the upper lip resulted in functional and cosmetic changes, such as speech, food and smile. These changes have been emphasizing over the years compromising social coexistence. In this case report, the authors demonstrate a surgical option - Abbé flap - to correct this sequel, which is the reconstruction of defects of the upper lip using flap of lower lip, giving appropriate functional outcome with preservation of facial esthetics.

RESUMO

Relato de caso de paciente do sexo feminino, 15 anos, que após ser submetida à correção de lábio leporino bilateral há 8 anos, evoluiu com sequela ocasionada pela diminuição do comprimento horizontal do lábio superior. Essa desproporção do lábio superior acarretou alterações estéticas e funcionais relativas a fonação, alimentação e sorriso. Essas alterações foram acentuando-se com o passar dos anos, comprometendo o convívio social. No presente relato de caso, os autores demonstram uma opção cirúrgica - o retalho de Abbé - para correção de tal sequela, que consiste na reconstrução de defeitos do lábio superior utilizando retalho de lábio inferior, apresentando resultado funcional adequado, com preservação da estética facial.

Removal of injectable silicone in male breasts: surgical approach

ABSTRACT

The silicon liquid, even forbidden, it has still been used in the design of the corporal outline. When precocious complications don't happen, the material is infiltrated in the adjacent tissues and it causes typical reaction of strange body with hard fibrosis and sometimes in long-term, calcification, in such a way that if there is need of his removal presents great difficulties and consequent scars.This work has the purpose of telling a case of a patient that nine years ago submitted himself to the injection of liquid silicon in the breasts, the surgical approach for his removal and the evolution.

RESUMO

O silicone líquido, mesmo proibido, ainda tem sido usado no delineamento do contorno corporal. Quando não ocorrem complicações precoces, o material infiltra-se nos tecidos adjacentes e causa reação típica de corpo estranho, com fibrose dura e, às vezes, a longo prazo, calcificação, de tal forma que, se houver necessidade, há grande dificuldade para sua remoção, com cicatrizes consequentes. Este trabalho tem finalidade de relatar um caso de paciente que há nove anos se submeteu à injeção de silicone líquido nas mamas, a abordagem cirúrgica para sua remoção e a evolução.

Breast reconstruction with latissimus dorsi expanded flap in a patient suffering from necrotizing fasciitis in the chest occurred in the neonatal period

ABSTRACT

Necrotizing fasciitis is an infection of soft tissues rapidly progressive, determining severe functional and aesthetic sequelae. In the chest, this entity is very uncommon, especially in newborns, a situation not found in the literature. This article reports the case of a newborn, female, suffering from necrotizing fasciitis in the breast and chest during the first week of life, being treated with antibiotics and surgical debridement with the wound closure by second intention. At fifteen years of age was observed a large scar area in the left hemithorax and complete absence of breast development on that side. Breast reconstruction was performed using an expanded latissimus dorsi flap and placement of tissue expander with subsequent replacement of the expander by silicone gel implant.

RESUMO

A fasciite necrotizante é uma infecção de tecidos moles, rapidamente progressiva, determinando graves sequelas funcionais e estéticas. No tórax, essa entidade é muito incomum, especialmente em recém-nascidos, situação não encontrada na literatura. Este artigo relata o caso de um recém-nascido, do sexo feminino, vítima de fasciite necrotizante em tórax e região mamária durante a primeira semana de vida, sendo tratada com antibioticoterapia e desbridamento cirúrgico, com fechamento da ferida por segunda intenção. Aos quinze anos de idade, procurou nosso serviço apresentando grande área cicatricial em todo hemitórax esquerdo e ausência completa de desenvolvimento mamário desse mesmo lado. Foi realizada reconstrução mamária com uso de retalho miocutâneo expandido de grande dorsal, com posterior substituição do expansor por implante definitivo de silicone gel.
