Analysis of complications from fat grafting performed at a public university hospital between 2015 and 2018: a cross-sectional retrospective study

ABSTRACT

Introduction: Fat grafting consists of collecting fat from one fat donor area via liposuction, then reallocating this tissue into a receptor area through syringes and cannulas. According to the literature, it is a safe process used in aesthetic and reconstructive surgeries. The objective is to evaluate complication rates of fat grafting procedures performed in a public university hospital from 2015 to 2018 in comparison with data previously reported in the literature.
Methods: A retrospective observational study developed at a public university hospital in Campinas-SP based on the revision of medical records of patients who underwent the fat grafting procedure in this institution from 2015 to 2018.
Results: Regarding the complications, the vast majority corresponded to fat reabsorption (62%). Bruise corresponded to the second most common (38% of cases), followed by edema (19%). Pain and dyschromia followed next, with 10% and 7% of cases, respectively. Only one case of operative wound infection was described. Other less frequent complications included hyperemia (5%), asymmetries (5%), desquamation (2%), local paraesthesia (3%) and skin lesion (2%). No other larger complication was reported, such as fat embolism or severe surgical complications such as bleeding, sepsis, or anaphylaxis.
Conclusion: The procedures of liposuction for fat collection and fat grafting were proved to be safe, yielding low complication rates in the studied sample, which agrees with data reported in the literature.

RESUMO

Introdução: A lipoenxertia consiste no processo de coleta de gordura de uma área doadora de gordura por meio de lipoaspiração seguida da realocação desse tecido em área receptora por meio de seringas e cânulas. É um processo seguro utilizado em cirurgias estéticas e reconstrutivas, de acordo com a literatura. O objetivo é avaliar as taxas de complicações dos procedimentos de lipoenxertia realizados em um hospital público universitário no período de 2015 a 2018, em comparação com dados previamente relatados na literatura.
Métodos: Estudo observacional retrospectivo desenvolvido em hospital universitário público de Campinas - SP a partir da revisão de prontuários de pacientes submetidos a lipoenxertia nesta instituição de 2015 a 2018.
Resultados: Em relação às complicações, a grande maioria correspondeu à reabsorção de gordura (62%). O hematoma correspondeu ao segundo mais comum (38% dos casos), seguido do edema (19%). Dor e discromia vieram em seguida, com 10% e 7% dos casos, respectivamente. Apenas um caso de infecção de ferida operatória foi descrito. Outras complicações menos frequentes foram hiperemia (5%), assimetrias (5%), descamação (2%), parestesia local (3%) e lesão cutânea (2%). Nenhuma outra complicação maior foi relatada, como embolia gordurosa ou complicações cirúrgicas graves como sangramento, sepse, anafilaxia, entre outras.
Conclusão: Os procedimentos de lipoaspiração para coleta de gordura e lipoenxertia mostraram-se seguros e com baixo índice de complicações na amostra estudada, concordando com dados da literatura.

Anatomic study of pockets areas in the hypogastrium

ABSTRACT

BACKGROUND: In lipodystrophies of the anterior abdominal wall, variables such as genetic factors, weight gain, age, and loss of muscle tone, either alone or in combination, determine generalized aesthetic changes or changes localized in the epigastrium, hypogastrium, or both. The aim of this study was to describe the limits, distribution, and dimensions of fat bags in the hypogastrium, as well as their anatomic relationships with the neighboring structures. METHODS: Fifty-seven anatomical specimens were dissected from patients undergoing abdominoplasty, during which the hypogastrium was resected en bloc. Moreover, the anterior abdominal wall was also dissected in 8 cadavers. In all samples, the lamellar layer was resected, keeping the dermis-epidermis, areolar layer, and superficial fascia intact. RESULTS: In all specimens, 2 symmetrical adipose clusters were observed bilaterally in the areolar layer, which were in the shape of a bag and had peculiar coloration. These bags were analyzed in relation to their neighboring tissues: the skin, superficial fascia, and deep fat layer. CONCLUSIONS: The lamellar layer was considered as a structural base of the skin, owing to its thinness. The areolar layer showed accumulations and varying amounts of fat in the various body regions without precise limits, which were particularly observed in dissections of the lower abdominal wall and identified by their elasticity, volume, and coloration.

RESUMO

INTRODUÇÃO: Nas lipodistrofias da parede abdominal anterior, fatores genéticos, aumento de peso, idade e lasseamento da musculatura, de forma isolada ou associada, determinam alterações estéticas generalizadas ou localizadas no epigástrio, no hipogástrio ou, mesmo, em ambas as regiões. O objetivo deste estudo é descrever os limites, a disposição, as relações anatômicas das bolsas adiposas no hipogástrio com as estruturas vizinhas e suas dimensões. MÉTODO: Foram dissecadas 57 peças anatômicas de pacientes submetidas a abdominoplastia, em que o hipogástrio foi ressecado em monobloco. Além disso, foi realizada dissecção da parede abdominal anterior em 8 cadáveres. Em todas as peças, a camada lamelar foi ressecada, mantendo-se a derme-epiderme, a camada areolar e a fáscia superficial íntegras. RESULTADOS: Em todas as peças, foram observados dois cúmulos adiposos na camada areolar bilateralmente, simétricos, em forma de bolsa e de coloração peculiar. Essas bolsas foram também analisadas em relação aos tecidos cutâneos vizinhos: pele, fáscia superficial, camada profunda de gordura. CONCLUSÕES: A camada lamelar foi considerada estrutural como base da pele, pela sua fina espessura, e a camada areolar apresentou acúmulos e volumes variáveis de tecido adiposo nas diversas regiões do corpo, sem limites precisos, particularmente observados nas dissecções da parede abdominal inferior, pela sua elasticidade, pelo volume e pela coloração.

Autologous Fat Graft in Rabbits

ABSTRACT

The aim of this study was to assess the fate of autologous fat tissue grafts harvested and processed by five different methods: blunt suction, sharp suction, dissection, treatment, and lavage. Fat was harvested from the subcutaneous tissue of the interscapular region in 25 rabits and injected into the subcutaneous area of the convex surface of the ear. Harvesting was by either open surgical excision. Where open surgical excision was used, the frgmants were cut into smaller pieces with scissors. Suction in the blunt and sharp suction groups was performed using cannulas with blunt or sharp edged suction holes, respectively. In both suction groups, the harvested tissue was injected without further processing. In the treatment group, 2 ml of cell 199 culture medium and Earle's basic salt solution were added, and in the lavage group the tissue was washed with Ringer's lactate. Animals were killed at 7 (n=9), 180 (n=8), and 360 (n=8) days. Serial cross sections were taken From each recipient area and the specimens processed for histology. The images from each section were digitalized in a computer and, with the assistence of image analysis softwere, the volume of remaining fat cells was calculated for each recipient area. The percentage volumes of fat cells found in each group at 360 days were: blunt suction 14,0%, sharp suction 35%, dissection 45%, treatment 27%, and lavage 16% (p=0.003).

RESUMO

A pesquisa consistiu em analisar o transplante livre de tecido adiposo autóloqo, em coelhos, obtido por 5 difirentes modalidades de colheita e preparo, a saber: aspirado, corte, dissecado, tratado e lavado. Nas duas primeiras modalidades, o tecido adiposo autólogo foi colhido por sucção. O aspirado (A) foi colhido com cânula de orifícios de bordas rombas e o corte (C) de bordas afiadas. As três demais modalidades de tecido adiposo foram obtidas por dissecção a céu aberto e fragmentação com tesoura delicada. O tecido assim colhido, fragmentado e injetado chamou-se dissecado (D). O tratado (T) recebeu adição de 2ml de meio de cultura de células 199 com sais de Earle. O lavado (L) foi submetido a lavagem com Ringer-lactato. As áreas doadoras do tecido adiposo foram as bolsas subcutâneas interescapulares. As orelhas dos coelhos foram as receptoras. Os animais foram sacrificados com 7 dias (9 animais) 180 (8 animais) e 360 dias (8 animais). Todas as áreas de transplante foram seccionadas transversalmente a cada 1 cm. A superfície de secção foi medida em sistema integrado para análise de imagem e cálculo do volume. Os cortes histopatolágicos foram quantificadas em: adipócitos, estroma, regiões císticas e inflamação. O volume percentual de adipócitos remanescentes nas 5 modalidades revelou aos 360 dias: Aspirado - 13,9%; Corte - 34,6%; Dissecado - 45,2%; Tratado - 27,0%; Lavado - 16,4%, com p = 0,003.

Giant lipomas: a 14-case series

ABSTRACT

INTRODUCTION: Lipomas are the most common benign mesenchymal tumors worldwide. To be considered giant, they must be at least 10 cm in one of its dimensions or weigh at least 1,000 g. The objective of this study is to analyze a series of cases of giant lipomas seen in our practice. METHOD: We conducted a retrospective analysis of the medical and anatomopathological records of patients diagnosed with lipomas who underwent surgery between January 2003 and January 2010. RESULTS: We evaluated 14 patients with a mean age of 52.3 years (range, 33-72 years) and a mean evolution time until treatment of 4.4 years (range, 6 months to 20 years); 64% of cases presented no symptoms. All patients underwent surgical tumor resection. The average weight of the pieces was 3.8 kg (range, 512 g to 22 kg), while the average length of the longest axis was 27.5 cm (range, 15-66 cm). Postoperative complications were observed in 42% of cases. CONCLUSION: We observed prolonged periods of evolution until treatment, even in cases of large-volume lesions. Open surgical excision should be considered the treatment of choice for these tumors.

INTRODUÇÃO: Os lipomas são os tumores mesenquimais benignos mais frequentes na população. Para ser considerado gigante, deve apresentar pelo menos 10 cm em uma de suas dimensões ou pesar ao menos 1.000g. O objetivo do trabalho é analisar uma série de casos de lipomas gigantes atendidos em nosso serviço. MÉTODO: Análise retrospectiva de prontuários e registros anatomopatológicos dos diagnósticos de lipomas submetidos a tratamento cirúrgico no período de janeiro 2003 a janeiro 2010. RESULTADOS: Foram avaliados 14 pacientes, com média de idade de 52,3 anos (33-72) e tempo médio de evolução até o tratamento de 4,4 anos (seis meses - 20 anos); 64% dos casos não apresentavam sintomas. Todos os pacientes foram submetidos à ressecção cirúrgica da neoplasia. O peso médio das peças foi de 3,8 kg (512 g - 22 kg) e o tamanho médio no maior eixo foi de 27,5 cm (15-66 cm). Foram observadas complicações pós-operatórias em 42% dos casos. CONCLUSÃO: Observamos períodos prolongados de evolução até o tratamento, mesmo em casos com lesões de grande volume. A exérese cirúrgica aberta deve ser considerada o tratamento de eleição para estes tumores.

Giant lipomas of the hand, case series

ABSTRACT

Introduction: Lipomas are sporadic in hand and represent less than 5% of soft tissue tumors in this topography. Giant lipomas, even less common, are defined as having a diameter greater than 5 cm.
Methods: We performed a retrospective analysis of the cases of patients with giant lipomas of the hand seen in the period 2017-2020, at Hospital Pasteur and Hospital de Clínicas, in Montevideo, Uruguay.
Results: We obtained four patients during the study period. The age of presentation was between 51 and 62 years (mean 57 years). Three lipomas were deep intramuscular, and one was superficial to the regional muscles. The largest had a longer axle of 70mm. All were symptomatic. Marginal resection was performed in all of them, and the postoperative period was uneventful or recurrent. Histopathology confirmed typical lipoma in three cases and lipoma with bone metaplasia in the other.
Conclusion: Giant lipomas of the hand present as painless, slow-growing tumors that may be symptomatic when compressing adjacent structures. Magnetic resonance imaging is the imaging study of choice for these cases. The applicable treatment is marginal surgical resection, and recurrence is uncommon.

RESUMO

Introdução: Os lipomas são esporádicos na mão e representam menos de 5% dos tumores de partes moles nesta topografia. Os lipomas gigantes, ainda menos comuns, são definidos por terem um diâmetro maior do que 5cm.
Métodos: Executamos uma análise retrospectiva dos casos de pacientes com lipomas gigantes de mão, atendidos no período de 2017-2020, no Hospital Pasteur e Hospital de Clínicas, em Montevidéu, Uruguai.
Resultados: Obtivemos quatro pacientes no período estudado. A idade de apresentação foi entre 51 e 62 anos (média de 57 anos). Três dos lipomas eram intramusculares profundos e um era superficial aos músculos regionais. O maior possuía eixo mais extenso de 70mm. Todos eram sintomáticos. A ressecção marginal foi realizada em todos eles, e o pós-operatório transcorreu sem intercorrências ou recorrências. A histopatologia confirmou lipoma típico em três casos, e lipoma com metaplasia óssea no outro.
Conclusão: Lipomas gigantes de mão se apresentam como tumores indolores e de crescimento lento, que podem ser sintomáticos ao comprimir estruturas adjacentes. A ressonância magnética é o estudo de imagem escolhido para estes casos. O tratamento aplicável é a ressecção cirúrgica marginal e a recorrência é pouco comum.

Fat grafting for the treatment of radiodermatitis after treatment of head and neck cancer

ABSTRACT

Introduction: Although treatment of head and neck cancer has evolved in recent years, it is still a challenge to preserve and restore facial symmetry afterward; this fact significantly compromises the quality of life of patients. In reconstructive surgery, autologous fat grafting has been used to treat volume and contour defects. The objective is to review the effects of fat grafting in patients with radiodermatitis due to head and neck cancer radiotherapy treatment.
Methods: Literature review in PubMed and SciELO databases, looking for studies published in the last 20 years in Portuguese, English, or Spanish. The descriptors "adipose tissue", "transplantation", "neoplasms", "head and neck neoplasms" and "radiotherapy" and the non-descriptive terms "fat grafting", "fat transplantation" and "fat graft" were used.
Results: Of 212 articles found, only seven complied with the eligibility criteria.
Conclusion: The review suggests that fat grafting in head and neck cancer reconstruction is a safe treatment and optimizes aesthetic and functional results. Prospective studies are lacking for a better definition of techniques and results.

RESUMO

Introdução: Embora o tratamento do câncer de cabeça e pescoço tenha evoluído nos últimos anos, ainda é um desafio preservar e restaurar a simetria facial após o mesmo, comprometendo significativamente a qualidade de vida dos pacientes. Em cirurgia reconstrutiva, o enxerto de gordura autóloga vem sendo usado para tratar defeitos de volume e de contorno. O objetivo é revisar os efeitos do enxerto de gordura em pacientes com radiodermite, devido ao tratamento radioterápico de câncer de cabeça e pescoço.
Métodos: Revisão de literatura nas bases PubMed e SciELO, buscando estudos publicados nos últimos 20 anos nas línguas portuguesa, inglesa ou espanhola. Foram utilizados os descritores "adipose tissue", "transplantation", "neoplasms", "head and neck neoplasms" e "radiotherapy"; e os termos não descritores "fat grafting", "fat transplantation" e "fat graft".
Resultados: De 212 artigos encontrados, apenas 7 estavam em conformidade aos critérios de elegibilidade.
Conclusão: A revisão sugere que a lipoenxertia em reconstrução oncológica de cabeça e pescoço é um tratamento seguro e permite otimizar os resultados estéticos e funcionais. Faltam estudos prospectivos para melhor definição de técnicas e de resultados.

Skin coverage of the anterior region of the knee with an anterolateral reverse flow thigh flap based on the perforating artery of the transverse branch of the lateral femoral circumflex artery: a case report

ABSTRACT

The patella is an uncommon place for the appearance of bone tumors, benign or malignant, and pseudotumoral lesions. In the literature, there is a small amount of work on this topic. This case report is about a patient with a metastatic tumor in the patella and reconstruction of the deformity with an anterolateral reverse flow thigh flap. Skin and soft tissue reconstruction in the region around the knee are often challenging for the plastic surgeon and aim to cover the exposed bone, preserving the joint function of the knee and providing a good esthetic result. There is an option for the use of free flaps, where there is a need for adequate material and trained staff. However, the vascular anastomosis region at the back of the knee makes the procedure even more complex. The anterolateral reverse thigh fasciocutaneous flap, first described in 1990 by Zhang et al., appears to be an effective and reliable option for reconstructing defects around the knee. It has a long vascular pedicle length and the availability of soft parts. There are some anatomical variations in which the cutaneous perforator does not originate from the descending branch. This report presents a patient with a skin perforator originating from the transverse branch of the lateral femoral circumflex artery, with an acceptable aesthetic result for skin coverage in the anterior region of the knee.

RESUMO

A patela é um local infrequente para o aparecimento de tumores ósseos, benignos ou malignos, assim como de lesões pseudotumorais. Na literatura existe uma quantidade pequena de trabalhos sobre esse tema. Esse relato de caso trata-se de uma paciente com tumor metastático em patela e reconstrução da deformidade com retalho anterolateral da coxa de fluxo reverso. A reconstrução cutânea e de tecidos moles na região ao redor do joelho é frequentemente desafiadora para o cirurgião plástico e tem como objetivo cobrir a parte óssea exposta, preservando a função articular do joelho e conferir um bom resultado estético. Existe a opção pela utilização de retalhos livres, onde há necessidade de material adequado e equipe treinada. Contudo, a região de anastomose vascular na parte posterior do joelho torna o procedimento ainda mais complexo. O retalho fasciocutâneo anterolateral da coxa reverso, descrito pela primeira vez em 1990, por Zhang et al., parece ser uma opção efetiva e confiável na reconstrução de defeitos ao redor do joelho, haja visto que possui um longo comprimento do pedículo vascular e disponibilidade de partes moles. Existem algumas variações anatômicas em que a perfurante cutânea não se origina do ramo descendente. Esse relato traz uma paciente com perfurante cutânea oriunda do ramo transverso da artéria circunflexa femoral lateral com resultado estético aceitável para cobertura cutânea em região anterior do joelho.

Eagle syndrome: a case report

ABSTRACT

Eagle syndrome is a rare condition, and its etiology has not yet been well established and its correct treatment is uncertain. Its treatment must be defined together with the patient, be it conservative or surgical, always taking into consideration the patient's expectations, in addition to a solid professional expertise in the modality chosen for the treatment. In this article, we present the case of a 35-year-old patient who was admitted to the Felício Rocho Hospital and discuss the various aspects of the disease, including the treatment modality chosen for the case.

RESUMO

A síndrome de Eagle é uma condição rara e com etiologia ainda não bem estabelecida, a qual se deve ter bastante suspeição para seu correto tratamento. Seu tratamento deve ser definido em conjunto com o paciente, seja ele conservador ou cirúrgico, sempre levando em consideração as expectativas do paciente, além da maior expertise do profissional na modalidade escolhida para o tratamento. Neste artigo, apresentamos uma paciente de 35 anos atendida no Hospital Felício Rocho, discutindo os diversos aspectos da doença, inclusive a modalidade de tratamento escolhida para o caso.

Surgical correction of buried penis with a skin graft: case report

ABSTRACT

Introduction: Buried penis is a rare pathology related to the reduction of penile length. It is described with a higher incidence in children, but it also affects adults. This population is mainly associated with obesity and fat accumulation in the suprapubic region, being considered a pathology that is difficult to manage and treat. Surgical intervention is the basis of treatment, and the main objective is the return of urinary and sexual function. Multiple surgical techniques are described for reconstruction in patients with buried penis and must be adapted to the characteristics presented by the patient.
Case Report: This article reports the surgical technique of dermolipectomy associated with the fixation of the base of the penis in pubic symphysis and a partial skin graft to approach a patient with a buried penis.
Conclusion: The presented technique showed good functional postoperative results due to clinical improvement of the patients' initial complaints and aesthetic ones, verbalized by the patient and the legal guardian due to the patient's low cognitive level.

Introdução: Pênis embutido é uma patologia rara relacionada à redução do comprimento peniano. É descrita com maior incidência em crianças, porém acomete também adultos. Nesta população está associado principalmente a obesidade e acúmulo de gordura em região suprapúbica, sendo considerada uma afecção de difícil manejo e tratamento. A intervenção cirúrgica é a base do tratamento e o objetivo principal é o retorno da função urinária e sexual. Múltiplas técnicas cirúrgicas são descritas para reconstrução em pacientes com pênis embutido e devem ser adaptadas para as características apresentadas pelo paciente.
Relato de Caso: Nesse artigo é relatada a abordagem de um paciente com pênis embutido, utilizando-se da técnica cirúrgica de lipoaspiração superficial e dermolipectomia associada à fixação da base do pênis em sínfise púbica com o uso de enxerto de pele parcial.
Conclusão: A técnica apresentada demonstrou bons resultados funcionais pós-operatórios, devido apresentar melhora clínica das queixas iniciais do paciente, e estéticos, verbalizados pelo paciente e pela responsável legal em virtude do baixo nível cognitivo do paciente.

ABSTRACT

Introduction: Neurofibromatosis is an autosomal dominant disorder, and type 1 is associated with an increased risk of tumor formation with neurocutaneous involvement. The variable evolution, often with limiting tumors, in addition to the significant incidence of cases requiring treatment, makes it fundamental to discuss procedures already performed in medical practice for early, careful, and individualized recognition of the diagnosis and treatment of the patient. The report aims to present a surgical case of neurofibromatosis, calling attention to the surgical technique, the characteristics of the disease, and the importance of the procedure in the quality of life of patients limited by the condition.
Case Report: A 23-year-old male patient with a large mass neurofibroma in the gluteus and posterior surface of the right leg, in addition to café au lait stains in the distal third of the legs. He was treated with surgery to remove the tumor and a flap and graft in the affected region. The procedures were performed by a multidisciplinary team, allowing the total removal of the tumor mass, with subsequent skin grafting in the hip and thigh lesion on the right side and the fasciocutaneous flap in VY in the area. There were no significant complications in the immediate postoperative period.
Conclusion: Neurofibromas can become limiting and impair patients' quality of life with neurofibromatosis type 1; therefore, early management and diagnosis are essential. Although the condition does not present a cure, there is a need for research into less invasive and preventive treatments for injuries.

RESUMO

Introdução: A neurofibromatose é um distúrbio autossômico dominante e o tipo 1 está associado a um aumento do risco de formação de tumores com acometimento neurocutâneo. A evolução variável, muitas vezes com tumorações limitantes, além da incidência significativa de casos que necessitam de tratamento, torna fundamental a discussão de condutas já realizadas na prática médica para um reconhecimento precoce, cuidadoso e individualizado do diagnóstico e do tratamento do enfermo. O relato objetiva apresentar um caso cirúrgico de neurofibromatose, chamando atenção para a técnica cirúrgica, as características da doença e a importância do procedimento na qualidade de vida de pacientes limitados pela afecção.
Relato de Caso: Paciente de 23 anos, sexo masculino, com neurofibroma de grande massa em glúteo e face posterior da perna direita, além de manchas café com leite em terço distal de pernas. Foi tratado com uma cirurgia de retirada do tumor, além de retalho e enxerto na região acometida. Os procedimentos foram realizados por equipe multidisciplinar, possibilitando a retirada total da massa tumoral, com posterior realização de enxerto de pele na lesão do quadril e coxa em lado direito, e o retalho fasciocutâneo em V-Y na área. Não houve complicações significativas nos pós-operatórios imediatos.
Conclusão: Os neurofibromas podem se tornar limitantes e prejudicar a qualidade de vida dos pacientes com neurofibromatose tipo 1, por isso, o manejo e o diagnóstico precoces são essenciais. Apesar da afecção não apresentar cura, há a necessidade de pesquisas em tratamentos menos invasivos e preventivos para as lesões.

Reduction cheiloplasty in bikini: case report

ABSTRACT

Introduction: The lips are the central characteristic in the lower third of the face, being essential for mimicry and facial expression, speech, and swallowing, without forgetting the aesthetic component. The presence of excessive large lips (macroqueilia) represents an occasional but significant challenge in plastic surgery. The technique called "bikini reduction cheiloplasty," described in 2007, unlike the techniques previously described, is focused on reducing lips and restoring their anatomy, providing a more harmonious lip contour.
Case Report: Male patient, 18 years old, black race with a complaint of excessively large lips for whom surgical treatment using the bikini technique was proposed, with a good aesthetic result.
Conclusion: Based on the correct indication and execution, we can conclude that the technique of reducing cheiloplasty in bikini is a safe alternative, easy to perform, and that provides a satisfactory aesthetic result.

RESUMO

Introdução: Os lábios são a característica central no terço inferior da face, sendo fundamentais para mímica e expressão facial, fala e deglutição, sem se esquecer do componente estético. A presença de lábios excessivamente grandes (macroqueilia) representa um desafio ocasional, mas significativo na cirurgia plástica. A técnica denominada "queiloplastia redutora em biquíni", descrita em 2007, à diferença das técnicas anteriormente descritas, não está focada apenas na redução de lábios, mas também na restauração da sua anatomia, proporcionando um contorno labial mais harmônico.
Relato de Caso: Paciente do sexo masculino, 18 anos, raça negra com queixa de lábios excessivamente grandes para o qual foi proposto tratamento cirúrgico usando a técnica do biquíni tendo um bom resultado estético.
Conclusão: Baseados na correta indicação e execução, podemos concluir que a técnica de queiloplastia redutora em biquíni é uma alternativa segura, de fácil execução e que proporciona um resultado estético satisfatório.

Giant lipoma: surgical treatment

ABSTRACT

Introduction: Giant lipomas (>1000g), although rare, have relevant medical importance, as they can affect any region of the body and are diagnosed in advanced stages, in which there is functional impairment. They can also be confused with malignant soft tissue neoplasms. Furthermore, the natural history of the emergence and evolution of giant lipomas is still not exactly known, which justifies further studies. The current study aims to present the results of the surgical treatment of a case of giant lipoma.
Case Report: Case report that occurred in the plastic surgery service of the Roberto Santos Hospital in Salvador, Bahia, in the year 2016. A 62-year-old male farmer, 1.80 m tall, 74 kg, with a dorsal tumor measuring 35 cm in diameter, evaluated through clinical history and computed tomography examination and submitted to elliptical excision and plastic closure in W. There were no postoperative complications. Histopathology confirmed the diagnosis of giant lipoma (2881g). Post-surgical results were considered excellent by the patient and surgical team.
Conclusion: The giant lipoma was satisfactorily removed without complications, with excellent functional and aesthetic results.

Introdução: Lipomas gigantes (>1000g), embora raros, têm importância médica relevante, pois podem acometer qualquer região do corpo e ser diagnosticados em fases avançadas, nas quais há comprometimento funcional. Eles também podem ser confundidos com neoplasias malignas de tecidos moles. Ademais, a história natural de surgimento e evolução de lipomas gigantes ainda não é exatamente conhecida, o que justifica a realização de estudos adicionais. O objetivo do corrente estudo é apresentar os resultados do tratamento cirúrgico de um caso de lipoma gigante.
Relato de Caso: Relato de caso ocorrido no serviço de cirurgia plástica do Hospital Roberto Santos em Salvador, Bahia, no ano de 2016. Homem de 62 anos, lavrador, 1,80m de altura, 74kg, com tumoração dorsal de 35cm de diâmetro, avaliado por meio de história clínica e exame de tomografia computadorizada e submetido a excisão elíptica e fechamento em plástica em W. Não houve complicações pós-operatórias. A histopatologia confirmou o diagnóstico de lipoma gigante (2881g). Os resultados pós-cirúrgicos foram considerados excelentes pelo paciente e equipe cirúrgica.
Conclusão: O lipoma gigante foi removido satisfatoriamente sem complicações, com excelentes resultados funcionais e estéticos.

Adipose tissue-derived stem cell autologous grafts: a new approach to application in the treatment of burn victims and reconstructive plastic surgery

ABSTRACT

In Brazil, 1 million burn accidents occur annually, and subsequent wound infections account for 75% cases of deaths among these patients, in addition to inducing deformities in the affected areas. Therefore, the aim of this study was to discuss the current status of mesenchymal stem cells, with an emphasis on adipose-derived stem cells (ADSCs), in combination with plasma gel, glue fibrin, and membranes (scaffold). The use of gels and membranes supports cell growth, and aims at potential application in reconstructive plastic surgery for the treatment of burn patients or individuals requiring skin grafts. This study explores and discusses the role of mesenchymal stem cells, adipose-derived mesenchymal stem cells, glue fibrin, plasma gel, and the scaffold. This research collected information from the Virtual Health Library (VHL) and PubMed. A considerable number of articles have been published on burn treatment. However, there is little research on burn treatment with ADSCs, glue fibrin, plasma gel, and scaffold. An ADSC autograft combined with a biological dressing is promising in reconstructive plastic surgery for the treatment and recovery of burn patients or individuals with other injuries that require skin grafts. These features can reduce pain and aid in drying of the lesion, thus promoting neovascularization and wound reepithelialization.

RESUMO

No Brasil, 1 milhão de acidentes com queimaduras acontecem por ano e as infecções são responsáveis por 75% dos óbitos nestes pacientes, além de deixar lesões que ocasionam deformidades nas áreas atingidas. Sendo assim, o objetivo deste trabalho é fornecer uma visão atual sobre células-tronco mesenquimais (MSCs), com ênfase nas células-tronco derivadas do tecido adiposo (ADSCs), associadas a gel de plasma, gel de fibrina e membranas (scaffold). O uso de géis e membranas tendem a auxiliar o crescimento celular visando sua possível aplicação na Cirurgia Plástica Reparadora para o tratamento pacientes queimados ou que necessitam de enxerto de pele. O presente trabalho abordou de forma exploratória e narrativa o tema células-tronco mesenquimais, células-tronco mesenquimais derivadas do tecido adiposo, gel de fibrina, gel de plasma e scaffold. O tipo de pesquisa empregada foi conduzido com coleta de informações utilizando-se a Biblioteca Virtual em Saúde (BVS) e PubMed. O número absoluto de artigos publicados relacionados ao tratamento de queimaduras é considerável. Até o momento, a quantidade de pesquisas relacionadas à terapia com células-tronco derivadas do tecido adiposo, gel de fibrina, gel de plasma e scaffold para o tratamento de queimaduras apresenta-se escassa. O autoenxerto de ADSCs associado a biocurativos torna-se uma perspectiva promissora na Cirurgia Plástica Reparadora para o tratamento e recuperação de pacientes que sofreram queimaduras ou outros acidentes que necessitam de enxerto de pele. Estes recursos podem reduzir a dor e prover a dessecação da lesão, promovendo neovascularização e a reepitelização da ferida.

Nasal reconstruction with paramedian frontal flap after cancer resection

ABSTRACT

Introduction: Basal cell carcinoma is the most frequent type of tumoral lesion of the skin, often affecting the nasal region. The resulting defects require the use of nasal reconstruction techniques. The first plastic surgery described is the nasal reconstruction, according to the Indian method. Over the centuries, it has been the object of several modifications by the biggest names in the specialty, culminating in the paramedian frontal flap.
Case Report: A 62-year-old male patient went to the Plastic Surgery Service of the Federal Hospital of Ipanema, presenting an ulcerated, scaly lesion, occupying the entire lateral region of the nose with three years of evolution, suggesting at physical examination malignant skin tumor. It was performed cutaneous lesion excision and immediate reconstruction with a paramedian frontal flap and further refinement surgery.
Conclusion: Due to its vascularization by the supratrochlear artery and texture similar to the nasal tissue, the paramedian frontal flap is the gold standard for the correction of significant nasal defects.

RESUMO

Introdução: O carcinoma basocelular é o tipo mais frequente de lesão tumoral de pele acometendo frequentemente a região nasal. Os defeitos resultantes exigem o emprego de técnicas de reconstrução nasal. A primeira cirurgia plástica descrita é a reconstrução nasal segundo o método indiano. Ao longo dos séculos sofreu diversas modificações pelos maiores nomes da especialidade, culminando no retalho frontal paramediano.
Relato do Caso: Paciente do sexo masculino, 62 anos, apresentou-se no serviço de Cirurgia Plástica do Hospital Federal de Ipanema com lesão ulcerada, descamativa, ocupando toda a região lateral do nariz com três anos de evolução, sugestiva ao exame físico de tumor maligno de pele. Foi realizada exérese da lesão cutânea e imediata reconstrução com retalho frontal paramediano e posterior cirurgia de refinamento.
Conclusão: Devido à sua vascularização pela artéria supratroclear e textura semelhante ao tecido nasal, o retalho frontal paramediano constitui-se como o gold standard para a correção de grandes defeitos nasais.

Preoperative clinical management of patients who are candidates for facial transplantation

ABSTRACT

Introduction: Face transplantation has gained recognition, changing the clinicalsurgical scenario for restoring complex facial defects, as it attributes functional and aesthetic recovery to patients who have suffered serious accidents. At the time of writing this article, in official publications, 43 patients had already undergone facial transplantation worldwide. Face transplantation has numerous pieces of evidence that can irrefutably provide improvements to the patient. For this, preoperative care for the patient must be carefully established so that there is good surgical performance.
Case Report: Male patient, 46 years old, reports that, at the age of 6, he had burns due to exposure to gasoline, with 72% of his body surface burned, showing sequelae of burns and surgical reconstructions on the face, with redundant and ptotic skin flap on the left cheek, absence of upper and lower lip and exposure of lower teeth.
Conclusion: It is important to publicize this innovative procedure in different medical specialties and preoperative care through a thorough investigation, which attributes better surgical effectiveness, allowing the rescue of their facial identity, once stigmatized.

RESUMO

Introdução: O transplante de face adquiriu reconhecimento, alterando o panorama clínico-cirúrgico para a restauração de defeitos faciais complexos, visto que atribui recuperação funcional e estética a pacientes que sofreram acidentes graves. Até o momento da redação deste artigo, em publicações oficiais, 43 pacientes já haviam realizado o transplante facial em todo mundo. O transplante de face possui inúmeras evidências que podem fornecer melhorias ao paciente de forma irrefutável. Para isso, cuidados pré-operatórios ao paciente devem ser cuidadosamente estabelecidos para que haja um bom desempenho cirúrgico.
Relato de Caso: Paciente sexo masculino, 46 anos, relata que, aos 6 anos de idade, teve queimadura por exposição à gasolina, com 72% de superfície corporal queimada, apresentando sequelas de queimaduras e reconstruções cirúrgicas na face, com retalho cutâneo redundante e ptótico em bochecha esquerda, ausência de lábio superior e inferior e exposição dos dentes inferiores.
Conclusão: Ressalta-se a importância da divulgação desse procedimento inovador em diferentes especialidades médicas e dos cuidados pré-operatórios através de uma investigação minuciosa, que atribuem uma melhor eficácia cirúrgica, possibilitando o resgate de sua identidade facial, uma vez estigmatizada.

ABSTRACT

Dermatofibrosarcoma protuberans (DFSP) is a soft tissue neoplasm with remarkable local aggressiveness, high levels of recurrence (> 40%) and low metastasizing potential. Occurs predominantly in trunk and proximal extremities, being uncommon in scalp, where represents less than 5% of all cases. Usually presents average size of 5 cm at the time of surgical excision. The objective of this study is to report a patient with giant tumor of scalp (20x16x14 cm), emphasizing its peculiarities and therapeutic options.

O dermatofibrossarcoma protuberans (DFSP) é uma neoplasia de tecidos moles de notável agressividade local, altos índices de recorrência (> 40%) e baixo potencial metastatizante. Ocorre principalmente em tronco e porção proximal de extremidades, sendo incomum em couro cabeludo, onde perfaz menos de 5% do total de casos. Habitualmente apresenta dimensões médias de 5 cm à ocasião da ressecção cirúrgica. O objetivo deste trabalho é relatar caso de paciente com tumoração gigante em couro cabeludo (20x16x14 cm), enfatizando suas peculiaridades e discutindo possibilidades terapêuticas.

Galactorrhea after breast augmentation: case report and literature review

ABSTRACT

Introduction: Augmentation mammoplasty is one of the most frequently performed surgeries in plastic surgery. Among its rarer occurrences are galactorrhea, spontaneous milk output by the papilla, and galactocele, a collection of milky material in the surgical bed. There is little literature on this extremely rare occurrence, and through this article, we conduct a literature review and report on a case operated in our service.
Case Report: We present a case of a 35-year-old patient who underwent breast augmentation surgery via the inframammary fold, with an implant placed in the subglandular plane, which evolved, on the thirtieth postoperative day, with galactorrhea exteriorized through the incision, and we propose a treatment for such intercurrence. Six months after the operation, the breasts were symmetrical, without signs of contracture or additional alterations; the patient was satisfied with the result and without new episodes of galactorrhea or galactocele.
Conclusion: Although rare, with an incidence of less than 1%, galactorrhea can occur as a postoperative complication, and knowledge about it, as well as the forms of treatment, will benefit both patient and surgeon.

RESUMO

Introdução: A mamoplastia de aumento é uma das cirurgias mais frequentemente realizadas na cirurgia plástica. Entre suas intercorrências mais raras, está a galactorreia, saída de leite pela papila de forma espontânea, e o galactocele, uma coleção de material leitoso no leito cirúrgico. Pouca literatura existe sobre esta raríssima intercorrência e por meio deste artigo realizamos uma revisão da literatura e relato de um caso operado no nosso serviço.
Relato de Caso: Apresentamos um caso de uma paciente de 35 anos que foi submetida a cirurgia de mamoplastia de aumento via sulco inframamário, com implante alocado no plano subglandular, que evoluiu, no trigésimo dia pós-operatório, com galactorreia exteriorizada pela incisão, e propomos um tratamento para tal intercorrência. Com seis meses de pós-operatório, as mamas se encontravam simétricas, sem sinais de contratura ou alterações adicionais, paciente satisfeita com o resultado e sem novos episódios de galactorreia ou galactocele.
Conclusão: Embora rara, com uma incidência de menos de 1%, a galactorreia pode ocorrer como intercorrência pós- operatória e o conhecimento desta, bem como as formas de tratamento, irá beneficiar tanto paciente como cirurgião.

Stem cell perspectives in Plastic Surgery

ABSTRACT

Introduction: The recent discovery that adult tissues have pluripotent stem cells has spurred a series of studies on such cells, because the discovery dismisses ethical, religious, and immunologic issues. SCs have recently attracted attention in plastic surgery, because the use of adipose tissue and skin as sources of SC production and differentiation has opened new avenues for the restorative or esthetic treatment of congenital and acquired deformities. Objective: The aim of this review is to discuss the current published research on adult or somatic stem cells, with emphasis on those derived from adipose tissue and skin, and the prospects of stem cell therapy in plastic surgery. Methods: The Medline and LILACS databases through the Virtual Health Library (http://www.bireme.br/php/index.php) and Google (www.google.com.br) were searched for articles published in 1997-2009. Results and Conclusions: Recently published studies on stem cells related to plastic surgery reported on new avenues for the use of adipose tissue and skin as sources of stem cell production and differentiation to correct congenital and acquired deformities, scarring, burns, stretch marks, and hyperchromic and hypochromic spots. The online search also provided information on obtaining and preparing material (fat and skin) for use in other medical specialties. The innumerable studies on stem cells worldwide emphasize the need to focus on this promising area of research and treatment in plastic surgery.

Introdução: A recente descoberta que tecidos adultos têm células-tronco com capacidade pluripotencial deu início a uma série de pesquisas, pois descarta questões ético-religiosas e imunológicas. A Cirurgia Plástica despertou interesse recentemente para o tema, e participa nas pesquisas de células-tronco a partir do momento que o tecido adiposo e a pele tornaram-se fontes de obtenção e diferenciação de células-tronco, abrindo novas perspectivas de tratamento para deformidades congênitas e adquiridas, com finalidade reparadora ou estética. Objetivo: Mostrar o estágio atual das pesquisas publicadas com células-tronco adultas ou somáticas, com ênfase nas derivadas da gordura e da pele, e as perspectivas de terapia celular com células-tronco que mostrem relação com a Cirurgia Plástica. Método: Utilizamos o sistema Medline, via website da Biblioteca Virtual em Saúde (http://www. bireme.br/php/index.php), pesquisando artigos no período de 1997-2009, o sistema LILACS pelo mesmo endereço eletrônico e no mesmo período e, por último, o programa de busca Google, para o mesmo período (www.google.com.br). Resultados e Conclusões: Os estudos recentes publicados sobre células-tronco relacionados à Cirurgia Plástica demonstram novas perspectivas para correção de deformidades adquiridas e congênitas, alterações cicatriciais, queimaduras, estrias, manchas hiper e hipocrômicas, bem como participação na obtenção e no preparo de material (gordura e pele) para uso em outras especialidades médicas. Os estudos se multiplicam em todo o mundo, e é importante que a especialidade Cirurgia Plástica esteja envolvida e atenta a esta promissora área de pesquisa e tratamento.

Supraclavicular flap for head and neck oncological reconstruction: A series of 62 cases

ABSTRACT

Introduction: The oncological reconstruction of extensive defects in the head and neck requires the plastic surgeon to make a difficult decision between the use of free flaps and pedicled flaps. The supraclavicular flap is one of the main examples of a pedicled flap, being versatile, with a thin thickness and similar color to the region to be reconstructed.
Method: A retrospective study was carried out by collecting data from medical records of patients admitted to the Cancer Institute of the State of São Paulo between December 2010 and March 2020.
Results: Among the 62 patients reconstructed with a supraclavicular flap, 37 were male and 25 female. Fifty-eight patients (93.5%) had some associated comorbidity. In total, 27 complications related to the flap (43.5%) were recorded, 5 of which were total necrosis (8%).
Conclusion: The supraclavicular flap plays an important role in head and neck oncological reconstructions and should be considered as an option in patients who are poor candidates for microsurgical flaps.

RESUMO

Introdução: A reconstrução oncológica de defeitos extensos em cabeça e pescoço impõe ao cirurgião plástico a difícil decisão entre o uso de retalhos livres e retalhos pediculados. O retalho supraclavicular é um dos principais exemplos de retalho pediculado, sendo versátil, com espessura delgada e cor semelhante à região a ser reconstruída.
Método: Um estudo retrospectivo foi realizado através da coleta de dados de prontuário de pacientes internados no Instituto do Câncer do Estado de São Paulo, entre dezembro de 2010 e março de 2020.
Resultados: Dentre os 62 pacientes reconstruídos com retalho supraclavicular, 37 eram do sexo masculino e 25 do sexo feminino. Cinquenta e oito pacientes (93,5%) possuíam alguma comorbidade associada. Ao todo, 27 complicações relacionadas ao retalho (43,5%) foram registradas, sendo 5 necroses totais (8%).
Conclusão: O retalho supraclavicular possui importante papel nas reconstruções oncológicas de cabeça e pescoço e deve ser considerado como opção em pacientes maus candidatos a retalhos microcirúrgicos.

ABSTRACT

Cutis laxa is a rare connective tissue disease characterized by dysfunction of elastic fibers. Individuals affected by this disease complain about their aged appearance. Treatments are based on the use of cosmetics or surgical techniques, with plastic surgery being an extremely relevant tool. Blepharoplasty aims to improve the senile appearance and provide rejuvenation in the area around the eyes, making the look appear more rested and alert. This is a retrospective observational study using medical record data.
Case Report: A female patient, 17 years old, was referred to the Plastic Surgery Service of the Walter Cantídio University Hospital, Fortaleza-CE, for treatment due to dissatisfaction with her appearance. She underwent upper and lower blepharoplasty associated with canthopexy without canthotomy. In the postoperative period, a satisfactory result was observed for the proposed surgery and adequate correction of existing changes.
Conclusion: The importance of facial surgical correction in cases of lax skin is observed, highlighting the relevance of applying appropriate surgical techniques and improving them in this patient profile.

RESUMO

Cútis laxa é uma rara doença do tecido conectivo caracterizada pela disfunção das fibras elásticas. Indivíduos acometidos por essa enfermidade queixam-se de sua aparência envelhecida. Os tratamentos se baseiam no uso de cosméticos ou em técnicas cirúrgicas, sendo a cirurgia plástica uma ferramenta de extrema relevância. A blefaroplastia tem o objetivo de melhorar a aparência senil e proporcionar rejuvenescimento na área ao redor dos olhos, fazendo com que o olhar pareça mais descansado e alerta. Trata-se de um estudo observacional retrospectivo utilizando os dados do prontuário.
Relato do Caso: Paciente do sexo feminino, 17 anos, encaminhada ao serviço de Cirurgia Plástica do Hospital Universitário Walter Cantídio, Fortaleza- CE, para tratamento devido à insatisfação com a sua aparência. Submetida a uma blefaroplastia superior e inferior associada a cantopexia sem cantotomia. No período pós-operatório, foi observado resultado satisfatório para a cirurgia proposta e adequada correção das alterações existentes.
Conclusão: Observa-se a importância da correção cirúrgica facial nos casos de cútis laxa, ressaltando a relevância da aplicação de técnicas cirúrgicas adequadas e o aprimoramento das mesmas nesse perfil de paciente.
