Free tissue transfers in the management of burns: experience with 58 flaps in 46 patients

ABSTRACT

Tangential excision is the debridement of necrotic tissue until viable tissue is reached. The second phase of surgical treatment is wound closure. The gold standard of wound coverage is autologus skin grafting. However, results can be disappointing in severe burns that involve the upper extremity and the neck. Fifty-eight free flaps for 46 patients with deep burns were reviewed retrospectively to determine indication, methods and outcomes.

RESUMO

A grande maioria das queimaduras de terceiro grau é tratada com a excisão tangencial, seguida da enxertia de pele. No entanto, as deformidades secundárias são frequentes, principalmente nos membros superiores e na cabeça e pescoço. As contraturas causadas pelas queimaduras graves podem ser tratadas das formas mais diversas. Aquelas limitadas às pequenas áreas podem ser tratadas com zetaplastias, enxertos ou retalhos locais; mas quando a contratura é extensa é difícil atingir bons resultados com os métodos convencionais. Os autores descrevem a sistematização e a estratégia na reconstrução primária ou secundária em pacientes queimados com o emprego de 58 retalhos livres, em 44 pacientes.

Using tamoxifen in keloid treatment

ABSTRACT

Background: We all know that the survival of the burn victims has increased due to the advances of the treatment in the critical phase. Because of that, the number of patients who shows sequels also increased. The objective of the present study is to measure the improvement in the quality of the lifetime of the patients with hypertrophic scars as result of burn or graft skin reducing, that way, the stigma caused by these scars. Many approaches have been done in order to prevent and treat pathological scars, frequently without success to some patients because the method or because of the difficulty of keeping the patient attached to the treatment. Method: Forty-six patients, 20 men and 26 women, have used 0.1% topic tamoxifen for a period of 4 to 12 months. Weekly evaluations were done during the first month and afterwards monthly, until a year. Photographs were taken after 6 months and a year. Results: Thirty-eight (82.6%) patients told that the improved in the first two weeks of treatment; 93.48% of the patients told that the scar height was reduced. In this study, we used tamoxifen citrate, an antineoplasic drug, to reduce the height of hypertrophic scars of burn marked patients. Conclusion: The results encouraged us to continue the treatment of scars with topic house use of tamoxifen.

RESUMO

Introdução: É sabido que a sobrevivência de pacientes vítimas de queimaduras tem aumentado significativamente devido aos avanços do tratamento na fase aguda. Como conseqüência, a quantidade de pacientes apresentando sequelas também aumentou. O propósito deste estudo foi avaliar a melhora na qualidade de vida de pacientes com cicatrizes hipertróficas resultantes de queimaduras ou após enxertos de pele, através da redução do estigma causado por estas cicatrizes. Várias abordagens têm sido descritas para prevenir e tratar cicatrizes patológicas, freqüentemente sem sucesso em alguns pacientes, por causa do método ou pela dificuldade em se conseguir a aderência do paciente ao tratamento. Método: Quarenta e seis pacientes, 20 homens e 26 mulheres, usaram tamoxifen tópico 0,1%, em um período de 4 a 12 meses. Avaliações semanais foram feitas no primeiro mês e, mensalmente, até o limite de um ano. Fotografias foram tiradas após seis meses e um ano. Resultados: Trinta e oito (82,6%) pacientes relataram melhora no prurido nas primeiras duas semanas de tratamento. Melhora na altura da cicatriz foi relatada por 93,48% dos pacientes. Neste estudo, usou-se citrato de tamoxifeno, uma droga antineoplásica, para redução da altura de cicatrizes hipertróficas de pacientes seqüelados de queimaduras. Conclusão: Os resultados nos encorajaram a continuar o tratamento de cicatrizes com o uso tópico domiciliar de tamoxifeno.

Lateral thoracic fasciocutaneous flap for reconstruction of axillary defects after resection of hidradenitis suppurativa: a series of 10 cases

INTRODUCTION: Hidradenitis suppurativa (HS) is a chronic follicular inflammation that presents varied clinical features, from isolated small nodules to several abscessed lesions, with formation of fistulas and scars. The axillary region is one of the most frequently affected areas. The objective is to evaluate a series of patients with HS in the axillary region who underwent extensive surgical excision and reconstruction with a lateral thoracic fasciocutaneous flap. METHODS: A retrospective analysis of the medical records of patients who underwent treatment between 2010 and 2012 was conducted. RESULTS: Ten patients were operated, of whom two had bilateral involvement, totaling 12 procedures. The mean progression time before the surgical indication was 38 months. The mean defect size after the resection was 10 x 9 cm. The mean size of the flaps was 15 x 10 cm. With regard to early complications, two cases of dehiscence (axillary region) and one case of partial epitheliolysis (distal segment of the flap) were observed. CONCLUSION: We observed that dissection of lateral thoracic fasciocutaneous flaps is not technically difficult and does not present significant functional or aesthetic sequelae in the donor area, making it a reliable and versatile option for reconstruction of larger axillary defects.

RESUMO

INTRODUÇÃO: A hidradenite supurativa (HS) é uma inflamação folicular crônica que apresenta quadro clínico variado, desde pequenos nódulos isolados até diversas lesões abscedadas, com formação de fístulas e cicatrizes. A região axilar é uma das áreas mais frequentemente acometidas. O objetivo é analisar uma série de casos acometidos por HS na região axilar, submetidos à exérese cirúrgica ampliada e reconstrução com retalho fasciocutâneo torácico lateral. MÉTODOS: Análise retrospectiva dos prontuários dos pacientes submetidos a tratamento no período entre 2010 e 2012. RESULTADOS: Dez pacientes foram operados, sendo que em dois havia acometimento bilateral, totalizando 12 procedimentos. Foi observado um tempo médio de evolução antes da indicação cirúrgica de 38 meses. O defeito médio observado após a ressecção foi de 10 x 9 cm. O tamanho médio dos retalhos foi de 15 x 10 cm. Em relação às complicações precoces, foram observados dois casos de deiscência (região axilar) e um caso de epiteliólise parcial (segmento distal do retalho). CONCLUSÃO: Observamos que a dissecção deste retalho não é tecnicamente difícil e que não apresenta sequelas funcionais ou estéticas significativas na área doadora, sendo uma opção confiável e versátil para reconstrução de defeitos axilares de maior porte.

Surgical treatment of axillary hidradenitis suppurativa using a parascapular flap

ABSTRACT

INTRODUCTION: Hidradenitis suppurativa (HS) is a disease with a prevalence of approximately 1% in the population and is difficult to clinically treat. Through chronification, it may lead to functional limitations, especially in the axillary region. METHODS: This study reports eight cases of axilla with chronic HS, Hurley stage III, treated with wide excision of the diseased part of the axillary region, followed by immediate reconstruction with a parascapular flap. The patients underwent surgery at the Plastic Surgery and Burns Service of the State Government Workers' Hospital of São Paulo. RESULTS: All the treated patients had no recurrence. All the parascapular flaps remained viable, and infection of one of the flaps (12.5%) was the immediate complication. Closure of the entire donor area was performed using primary synthesis. CONCLUSION: The treatment of chronic axillary HS, stage III, based on radical resection and reconstruction with a parascapular flap proved to be an effective alternative to locally control the disease and is a safe procedure to cover the axillary defect.

RESUMO

INTRODUÇÃO: A hidradenite supurativa (HS) é uma doença com prevalência em torno de 1% na população, de difícil tratamento clínico, que em sua cronificação pode levar a limitações funcionais, principalmente na região axilar. MÉTODOS: Este trabalho relata oito axilas com HS crônica estágio III de Hurley tratadas com exérese ampla da doença na região axilar e sua reconstrução imediata com o retalho paraescapular, operados no pelo Serviço de Cirurgia Plástica e Queimaduras do Hospital do Servidor Público Estadual de São Paulo. RESULTADOS: Todas as axilas tratadas não tiveram recorrência da doença (100%). Todos os retalhos paraescapulares mantiveram-se viáveis (100%), sendo a infecção de um dos retalhos (12,5%) a complicação imediata. Fechamento de 100% da área doadora em síntese primária. CONCLUSÃO: O tratamento da HS crônica axilar estágio III baseado na ressecção radical e reconstrução com retalho paraescapular mostrou-se uma opção efetiva no controle local da doença e um retalho seguro para cobertura do defeito axilar.

Evaluation of the effectiveness of a modified liposuction protective device: an experimental study in swine

ABSTRACT

Liposuction is one of the most common procedures in the plastic surgery specialty. In the 2018 ISAPS survey, it was the second most performed surgery worldwide. Due to the repetitive movements typical of the surgery, significant friction is generated at the site, and the consequences are skin burns that can leave unsightly scars and dyschromias. This study aims to create a skin protective device prototype from an old model, which serves this purpose, and to observe its functionality and its effects on the pigskin. The tests were carried out on dead animals without suffering provided by the Veterinary Medicine sector at Universidade Positivo. Three incisions were made in the animal's abdomen to pass the liposuction cannula and another three for the insertion and use of the prototype to compare it with the model device. The established time for liposuction movements was twenty minutes, performed with the 5mm cannula directly in contact with the skin and inside the prototype. The prototype's ergonomics, ease of insertion, and good locking on the skin with different tractions were evaluated. Observation and evaluation of the skin were performed after procedures and incisions' measurements (cm). The cutaneous liposuction protective device prototype presented easy handling and a more efficient skin locking mechanism than the model used. The skin incision for using the prototype was slightly larger, and the skin showed no burning signs.

RESUMO

A lipoaspiração é um dos procedimentos mais comuns na especialidade de cirurgia plástica. No levantamento da ISAPS de 2018 foi a segunda cirurgia mais realizada em todo o mundo. Devido aos movimentos repetitivos próprios da cirurgia, fricção importante é gerada no local e as consequências são queimaduras cutâneas que podem deixar cicatrizes inestéticas e discromias. O objetivo deste estudo é criar um protótipo de um dispositivo protetor da pele, a partir de um modelo antigo, que sirva a esse propósito, e observar sua funcionalidade e os efeitos da sua utilização na pele de suínos. Os testes foram realizados em animais mortos sem sofrimento disponibilizados pelo setor de Medicina Veterinária da Universidade Positivo. Foram feitas três incisões no abdome do animal para passagem da cânula de lipoaspiração e outras três para a inserção e utilização do protótipo, bem como para comparação com o dispositivo modelo. O tempo estabelecido de movimentos de lipoaspiração foi de vinte minutos, realizados com a cânula de 5mm diretamente em contato com a pele e dentro do protótipo. Foi avaliada a ergonomia do protótipo, facilidade de inserção e travamento adequado na pele com diferentes trações. Observação e avaliação da pele após os procedimentos e medição (cm) das incisões foram realizadas. O protótipo do dispositivo protetor cutâneo de lipoaspiração criado apresentou fácil manuseio e mecanismo de travamento na pele mais eficiente quando comparado ao modelo utilizado. A incisão cutânea para uso do protótipo foi ligeiramente maior e a pele não apresentou sinais de queimadura.

ABSTRACT

The authors report the treatment of a large burn scar concerning the neck, and middle and inferior thirds of the face with a paraescapular fasciocutaneous expanded free flap. The use of a previous cutaneous tissue expander associated to microsurgical technique have allowed carrying out a flap whose thickness, texture, color and size were very suitable option. The surgical purpose has proved secure and feasible with a satisfactory aesthetic and functional result.

RESUMO

Os autores relatam o tratamento de um paciente com seqüela de queimadura extensa em região cervical e terço médio e inferior de face com uso de retalho livre expandido, fasciocutâneo, paraescapular. A associação das vantagens do uso de expansor cutâneo com a técnica microcirúrgica permitiu a realização de retalho com características de espessura, textura, coloração e dimensões adequadas para o tratamento. A proposta cirúrgica mostrou-se segura, exeqüível, com bom resultado estético e funcional.

Marjolin's ulcer in chest: a case report

ABSTRACT

A case report of Marjolin's ulcer, observed at the "Fundação do ABC" teaching hospital, in São Paulo city. Sixty years-old male patient. Outcome period of more than 50 years, from the scar to the squamous cell carcinoma rise, aggravation due to arsenic contact, to suppurations, and to second-intention healing. Despite the surgical treatment with extensive resection and total axillary lymphadenectomy, the patient presented metastases within a short period of time, dying due to acute respiratory failure resulting from the pulmonary and mediastinal metastases. The authors present a revision on the development of malignant tumors on cutaneous scars, especially on burning scars.

RESUMO

Relato de caso de úlcera de Marjolin observado no Hospital de Ensino da Fundação do ABC, São Paulo. Paciente do sexo masculino, 60 anos de idade, apresenta tempo de evolução do caso superior a 50 anos, da cicatriz até surgimento do carcinoma espinocelular. A lesão agravou-se devido ao contato com o arsênico, devido às supurações, e cicatrizações por segunda intenção. Apesar do tratamento cirúrgico com ressecção ampla e linfadenectomia axilar total associada, apresentou metástases numerosas em curto espaço de tempo, levando o paciente a óbito por insuficiência respiratória decorrente de metástases pulmonares e mediastinais. Os autores apresentam uma revisão sobre o desenvolvimento de tumores malignos sobre cicatrizes cutâneas, especialmente sobre cicatrizes de queimadura.

Z-plasty in axillary retraction: techniques and strategies to facilitate learning by medical residents

ABSTRACT

INTRODUCTION: Herein, we present two cases of post-burn retraction of the axillary region, which were subsequently surgically corrected using multiple Z-plasties. We present a marking strategy to facilitate the learning of this procedure by plastic surgery residents. METHODS: Two patients with axillary and upper limb retractions due to burns with hot liquids were surgically treated using multiple Z-plasties. The strategy proposed in this study recommends the marking of the triangular flap located at the midpoint of the axillary retraction, which then advances and crosses the axis of the scar. Then, subsequent flaps are made at an angle of 60º to each other. We believe that this systematization of marking facilitates the planning and learning of Z-plasty. RESULTS: The study patients achieved an adequate range of motion and functional recovery of the affected limb postoperatively. We observed no cases of necrosis of the flap ends, which should be carefully worked and slightly rounded. The execution of the procedure was simple using the suggested marking strategy. CONCLUSION: The presented marking strategy was reproducible and easy to learn. Multiple Z-plasties were effective in restoring the range of motion of the areas affected by burns that were treated using this method.

RESUMO

INTRODUÇÃO: São apresentados dois casos de retração em região axilar após queimadura em que a correção cirúrgica foi realizada empregando-se zetaplastias múltiplas. Apresentamos uma tática de marcação que visa facilitar o aprendizado desse procedimento pelos residentes de Cirurgia Plástica. MÉTODOS: Dois pacientes com retrações axilares e de membros superiores após queimadura com líquido aquecido foram submetidos à correção cirúrgica empregando-se a zetaplastia múltipla. A tática de planejamento proposta nesse estudo preconiza primeiro a marcação do retalho triangular localizado no ponto médio da retração axilar, o qual irá avançar e cruzar o eixo da cicatriz, e a partir daí o desenho dos retalhos subsequentes com ângulo de 60º entre si. Acreditamos que essa sistematização da marcação facilita o planejamento e aprendizado da plástica em Z. RESULTADOS: Os pacientes apresentaram adequadas amplitude de movimento e recuperação funcional do membro acometido no pós-operatório. Não houve necrose das extremidades dos retalhos, que preconizamos serem cuidadosamente trabalhadas e um pouco arredondadas; e a execução do procedimento foi simples devido à tática de marcação que aplicamos. CONCLUSÃO: A tática de marcação que apresentamos é reprodutível e de fácil aprendizado. A zetaplastia múltipla foi eficaz em restabelecer a amplitude de movimento das áreas com sequela de queimadura tratadas com esse método.

Subcicatricial fat grafting associated with rigotomy: a clinical and microstructural analysis

ABSTRACT

INTRODUCTION: The increase in survival of large burn patients during the acute phase has also increased the prevalence of esthetic-functional scarring sequelae. With regard to the clinical relevance of subcicatricial fat grafting, the actual changes after the procedure need to be understood and microscopically evaluated. METHODS: Eight patients with alcohol burns, with an average time after burn of 12 months (10-14 months), who underwent subcicatricial fat grafting associated with rigotomy were selected. Skin biopsies were performed in before and 14 weeks after operation. The following issues were assessed: 1) esthetic-functional improvement of the scar, by using the Vancouver scar scale; 2) quantitative and qualitative analyses of cicatricial collagen; 3) immunohistochemical analysis of scar vascularization with anti-vascular endothelial growth factor (anti-VEGF) antibody. RESULTS: When comparing the pre- and post-fat grafting period, a significant esthetic-functional improvement and, microscopically, a redefinition of the boundaries of the papillary and reticular dermis were observed, as well as quantitative reduction and reorganization of collagen, in addition to the decrease of the vascularization of the tissue through immunohistochemical analysis. CONCLUSION: The basic principle of the whole physiological healing process is to reestablish local homeostasis, that is, excessively intensified steps that lead to severe clinical changes. When subcicatricial fat grafting associated with rigotomy was performed, qualitative and quantitative improvements of the tissue were verified in this study. Thus, it becomes evident that this procedure can complement the treatment of cicatricial pathologies.

RESUMO

INTRODUÇÃO: O aumento da sobrevida na fase aguda do paciente grande queimado faz também aumentar a prevalência das sequelas cicatriciais estético-funcionais. Ao pensarmos na relevância clínica da lipoenxertia subcicatricial, necessita-se da compreensão e avaliação microscópica das reais alterações após o procedimento. MÉTODOS: Foram selecionados oito pacientes, vítimas de queimadura por álcool, com tempo médio após queimadura de 12 meses (10-14 meses), sendo submetidos à lipoenxertia subcicatricial associada à Rigottomy. Foram realizadas biópsias cutâneas no pré e pós-operatório tardio com 14 semanas. Avaliados os quesitos: 1 - Melhora estético funcional da cicatriz pela escala de Vancouver; 2 - Análise quantitativa e qualitativa do colágeno cicatricial; 3 -Análise imunohistoquímica da vascularização cicatricial com anti-fator de crescimento derivado do endotélio vascular (antiVEGE). RESULTADOS: Ao compararmos o período pré e pós-lipoenxertia, pôde-se avaliar que houve melhora estético-funcional significativa e, microscopicamente, redefinição entre os limites da derme papilar e reticular; redução quantitativa e reorganização do colágeno, além do decréscimo da vascularização tecidual pela análise imunohistoquímica. CONCLUSÃO: O princípio básico de todo processo cicatricial fisiológico é reestabelecer a homeostasia local, ou seja, as etapas exageradamente intensificadas levam a alterações clínicas catastróficas. Ao realizar a lipoenxertia subcicatricial associada à Rigotomia, foi verificada, neste estudo, a melhora qualitativa e quantitativa do tecido. Sendo assim, torna-se evidente o futuro promissor deste procedimento para a complementação terapêutica das patologias cicatriciais.

ABSTRACT

Introduction: Burns are a type of injury that is among the leading causes of mortality. The use of epidemiology helps in providing subsidies for evaluation and organization of treatment and prevention, aiming at the improvement of public health. The objective is to describe the epidemiological profile of the patients treated at the Queimados Nelson Picollo unit of the Governador Otávio Lages de Siqueira Emergency Hospital (HUGOL).
Method: This study analyzed the epidemiological profile of patients, aged more than 12 years, treated by plastic surgery at the burn treatment unit, Nelson Picollo, from July 2015 to June 2017. The patients who were not admitted to the burn unit were excluded from the study. Data were collected from the MVSoul R. program.
Results: A total of 375 patients visited the hospital; of these, 50% of the patients needed intensive care unit treatments. The mean duration of hospital stay was 23.08 days. The majority of the patients were males, with a mean age of 39.17 years, and the main etiological agents were inflammable liquids, mainly alcohol and gasoline. The mean burned body surface area was 24.67%. A total of 1490 surgical interventions were performed, with an average of 3.97 surgeries per patient. Forty patients died; of these, 29 had airway burns. The total number of patients with airway burns was 73.
Conclusion: This study provides a profile of burns and hospitalizations in the HUGOL burns unit, which corresponds with those from other centers specialized for this type of treatment.

Introdução: Queimadura é um tipo de lesão que está entre as principais causas de mortalidade. A utilização da epidemiologia ajuda no fornecimento de subsídios para avaliação e organização de tratamento e prevenção, visando à melhora da saúde pública. O objetivo é descrever o perfil epidemiológico dos pacientes atendidos no Hospital de Urgências Governador Otávio Lages de Siqueira (HUGOL) na unidade de Queimados Nelson Picollo.
Método: Este estudo analisou o perfil epidemiológico dos pacientes atendidos pela equipe de cirurgia plástica da unidade de tratamento de queimados Nelson Picollo, no período de julho 2015 até junho 2017, com idade superior a 12 anos. Critérios de exclusão: pacientes que não foram internados na unidade de queimados. Os dados foram colhidos do programa MVSoul R.
Resultados: O total de atendimentos foi de 375 pacientes, dos quais 50% necessitaram de tratamentos unidade de terapia intensiva. O tempo médio de internação foi de 23,08 dias. A maioria do sexo masculino, com média de idade de 39,17 anos, com agente etiológico principal as chamas por álcool + gasolina. A média de superfície corporal queimada foi de 24,67%. Foram realizados 1490 intervenções cirúrgicas, média de 3,97 cirurgias por paciente. O total de óbitos foi de 40 pacientes, dos quais 29 apresentaram queimadura de via aérea. O total de pacientes com queimadura de via aérea foi de 73 casos.
Conclusão: Este estudo demonstrou um perfil de queimaduras e internações na unidade de Queimados do HUGOL compatível com outros centros especializados neste tipo de tratamento.

ABSTRACT

Introduction: Burns are one of the most severe traumatic injuries and their treatment requires a multidisciplinary approach, where the role of the plastic surgeon is vital. The plastic surgeon is entrusted with the challenging goal of restoring the skin's protective function and simultaneously recovering the aesthetic aspect of the burnt area.
Case report: A 27-year-old woman underwent a mastopexy with inclusion of implants, where the skin removed from the breast was used as a full-thickness graft in the mandibular and submental area for the treatment of a scar. The patient showed complete integration of the graft, and no areas of epidermolysis were observed. The aesthetic results were excellent, and the patient was completely satisfied; moreover, an improvement in the dyschromic and hypertrophic cicatricial areas was observed.
Conclusion: An autologous graft using breast skin is a good alternative for the treatment of sequelae of burns on the face and provides excellent aesthetic results.

RESUMO

Introdução: As queimaduras constituem uma das lesões traumáticas mais graves e seu tratamento requer uma abordagem multidisciplinar, em que o papel do cirurgião plástico é fundamental. Restabelecer a função de proteção da pele, mas também recuperar a estética da área, queimada são objetivos desafiadores que o cirurgião plástico procura atingir.
Relato de Caso: Paciente feminino de 27 anos submetida a mastopexia com inclusão de implantes, em que se aproveitou a pele retirada da mama para realizar um enxerto de espessura total em região mandibular e submentoniana para tratamento de cicatriz. A paciente teve uma integração completa do enxerto, sem evidenciar-se áreas de epidermólise. Os resultados estéticos foram excelentes, conseguindo a satisfação da paciente e melhoria das áreas discrômicas e hipertróficas cicatriciais.
Conclusão: O enxerto autólogo a partir da pele da mama constitui uma boa alternativa para o tratamento de sequelas de queimaduras em face, possibilitando ótimos resultados estéticos.

ABSTRACT

Chemical burn is a challenge owing to its severity compared to thermal burns. The depth and extent of lesions in different areas of the body lead us to look for different possibilities for the best treatment of the patient. A reverse abdominoplasty surgical technique was proposed; this involves a skin flap that could reduce the exposed area of a chemical burn. The result was satisfactory, reducing the patient's treatment, surgical and hospitalization time, highlighting the proposed technique as an important tool for thoracoabdominal reconstruction.

RESUMO

A queimadura química é um desafio devido à sua agressividade em comparação com as queimaduras térmicas. A profundidade e extensão das lesões em diferentes áreas do corpo nos levam a buscar diferentes possibilidades para auxiliar no melhor tratamento do paciente. Foi proposto, a partir da técnica cirúrgica abdominoplastia reversa, um retalho cutâneo que pudesse reduzir a área exposta de uma paciente vítima de queimadura química. O resultado foi satisfatório, reduziu tempo de tratamento, cirúrgico e de internação da paciente, evidenciando uma ferramenta importante de reconstrução toracoabdominal.

The study reviews the use of Z-plasty in the cheiloplasty techniques used to treat unilateral cleft lip. It highlights the contribution of Brazilian authors, especially that of Perseu Lemos. It also reports the conduct adopted by the senior author in this type of cheiloplasty.

Abdominal wall reconstruction with island tensor fascia lata miocutaneous flap after electrical burn: a case report

ABSTRACT

The authors present a case of a 19 year-old male patient, burned by high tension electricity in 30% of the total body surface, who presented abdominal wall necrosis, upper right limb and penile amputation. After treatment on a specialized burn unit, the patient was referred to the Microsurgery Department of Hospital dos Servidores do Estado do Rio de Janeiro for late reconstruction. The patient presented at that time a volumous bulge on the lower abdomen previously covered with partial thickness skin graft. He was then submitted to a first surgical step, by means of a tensor fascia lata miocutaneous flap coverage, preparing the patient for a penile reconstruction on a second surgical step. There are presented and discussed the surgical technique and the option for the TFL flap.

RESUMO

Os autores apresentam o caso de um paciente de 19 anos vítima de queimadura por eletricidade de alta tensão, acometendo 30% da superfície corporal, com necrose da parede abdominal, amputação do membro superior direito e da genitália. Após tratamento no Centro de Terapia a Queimados, foi encaminhado para o Serviço de Microcirurgia Reconstrutiva do Hospital dos Servidores do Estado do Rio de Janeiro para reconstrução tardia. O paciente apresentava volumoso abaulamento no abdome inferior, com cobertura de enxerto cutâneo de espessura parcial. Foi submetido a reconstrução da parede com retalho ilhado miocutâneo do tensor da fascia lata no primeiro tempo, preparando o paciente para a reconstrução peniana com retalho microcirúrgico, num segundo tempo. A opção por retalho do tensor da fascia lata e a técnica cirúrgica empregada são apresentadas e discutidas.

Reverse posterior interosseous flap of the forearm for the surgical treatment of electric hand trauma: Case report

ABSTRACT

Introduction: Lesions affecting the hands with significant skin loss often require flaps for early coverage, as these permit faster healing. Among the various options, the reverse posterior interosseous flap of the forearm is most commonly used for defects involving the back of the hand and wrist due to low complication rates. Normally, this flap is not used for the reconstruction of defects in the palmar region since its distal reach is insufficient. Case report: We present the case of a male patient with third-degree electrical burns on his right palm, whose reconstruction was performed on the 14th day postinjury using the reverse posterior interosseous flap of the forearm after conservative debridement. The patient presented good postoperative evolution, without long-term complications or functional sequelae. Conclusion: The reverse posterior interosseous flap of the forearm permits adequate coverage of palm injuries, preserving its functionality.

RESUMO

Introdução: Lesões que acometem as mãos com importante perda cutânea frequentemente requerem retalhos para cobertura precoce, visto que permitem melhor reabilitação. Dentre as opções, o retalho interósseo posterior reverso do antebraço é o mais utilizado para defeitos no dorso da mão e punho, com baixas taxas de complicações. Normalmente, esse retalho não é utilizado para a reconstrução de defeitos em região palmar, já que geralmente não alcança esse local.
Relato de caso: Apresentamos o caso de um paciente com queimadura elétrica de terceiro grau, em palma da mão direita, cuja reconstrução foi realizada com o uso do retalho interósseo posterior reverso do antebraço, após debridamentos conservadores, no 14o dia após a queimadura. O paciente apresentou boa evolução pós-operatória, sem complicações ou sequelas funcionais a longo prazo.
Conclusão: O retalho interósseo posterior reverso do antebraço permite cobertura adequada de lesões em palma da mão, preservando sua funcionalidade.

Post-burn cervical retraction in children

ABSTRACT

Children are the most affected when it comes to burn injuries. Scar retraction of burned skin causes great difficulty for normal development, including the human skeleton. This work demonstrates how the surgical correction of cicatricial retraction in the cervical region allows the increase of neck and head mobility, potentially reducing the impact of the development of the region. The technique used was zetaplasty, with resection of the scar band and without skin grafting. Improvement in head movement was achieved after one month.

RESUMO

Crianças são as mais atingidas quando se trata de acidentes por queimaduras. A retração cicatricial da pele queimada acarreta grande dificuldade para o desenvolvimento normal, inclusive do esqueleto humano. Este trabalho demonstra de que forma a correção cirúrgica da retração cicatricial na região cervical permite o aumento da mobilidade do pescoço e da cabeça, potencialmente diminuindo o impacto do desenvolvimento da região. A zetaplastia foi a técnica utilizada, com ressecção da banda cicatricial e sem enxerto de pele. A melhora da movimentação da cabeça foi alcançada após um mês.

Assessment of vertical scar migration after classical abdominoplasty followed by lower abdominal flap fixation

ABSTRACT

Background: In plastic surgery, the postoperative scar is a frequent concern. It is very important for the surgeon to understand the natural evolution of the scar. The abdominoplasty scar has a natural tendency to move upwards over time. In this study, the extent of natural vertical scar migration after abdominoplasty was calculated and the effect of fixing the lower abdominal flap to prevent this rise was assessed. Methods: This prospective and randomized study was conducted at the 38th Nursing service of Santa Casa de Misericórdia do Rio de Janeiro, during 2010, and included 20 female patients with an indication for abdominoplasty. The patients were randomly divided into two groups, A and B. Classical abdominoplasty was performed in group A patients, according to the technique recommended by Prof. Ivo Pitanguy. Patients in group B also underwent lower abdominal flap fixation by suturing of Scarpa's fascia and the rectus abdominis muscle aponeurosis. At both two weeks and six months after surgery, the distance of vertical scar movement was measured in 16 patients after application of exclusion criteria. The average vertical migration and the mean difference between the groups were calculated. Results: The average difference between the groups in vertical migration throughout the scar was 0.4 cm, with a mean overall migration of 1.06 cm and 0.68 cm in groups A (control group) and B (cases with fixation), respectively. Conclusions: The scar formed after abdominoplasty undergoes vertical migration over time; this migration is decreased when the lower abdominal flap is fixed. The surgeon should be aware of scar migration for better planning of the incision location.

Introdução: Na cirurgia plástica, uma constante preocupação é a cicatriz pós-operatória. É muito importante para o cirurgião conhecer a evolução natural da cicatriz. A cicatriz de abdominoplastia tem a tendência natural de subir com o passar do tempo. Neste estudo, calculou-se a migração vertical natural da cicatriz pós-abdominoplastia, avaliando o efeito da fixação do retalho abdominal inferior na prevenção dessa ascensão. Métodos: Estudo prospectivo e randomizado, realizado na 38ª Enfermaria da Santa Casa de Misericórdia do Rio de Janeiro durante o ano de 2010, incluindo 20 pacientes do sexo feminino com indicação de abdominoplastia, divididas em dois grupos, A e B, aleatoriamente. Nas pacientes do grupo A, foi realizada abdominoplastia clássica, segundo técnica preconizada pelo Prof. Ivo Pitanguy, e no grupo B foi incluída a fixação do retalho abdominal inferior por meio de sutura interessando a fáscia de Scarpa e a aponeurose do músculo reto abdominal. Duas semanas e seis meses após a cirurgia, foi medida a distância vertical em 16 pacientes, após aplicação dos critérios de exclusão, calculando-se a média de migração vertical e a diferença média entre os dois grupos. Resultados: A diferença média de migração vertical ao longo de toda a cicatriz foi de 0,4 cm, sendo a média geral de migração nos grupo A (controle) e B (casos com fixação) de 1,06 cm e 0,68 cm, respectivamente. Conclusões: A cicatriz pós-abdominoplastia sofre migração vertical ao longo do tempo, sendo menor quando o retalho inferior é fixado. O cirurgião deve estar ciente da migração sofrida pela cicatriz para melhor planejamento da posição de sua incisão.

One stage total penile reconstruction with radial forearm free flap and penile implant: case report

ABSTRACT

The authors describes a case of total penile reconstruction, in a 24 years old male patient, electric burn accident victim, with 30% of body surface compromised (2nd and 3rd degree), right arm, penis and testicles amputation, and inferior abdominal wall necrosis. After clinical and surgical treatment in a Burn Care Unit, he was refered for late reconstruction in Reconstructive Microsurgery Service of Servidores do Estado Hospital in Rio de Janeiro. The first surgical time was the infra umbilical abdominal wall reconstruction with fascia lata tensor mio-fascial-cutaneous flap. The second surgical time was the total penile reconstruction with radial antebraquial free flap and concomitant inflate penile implant by urological team. The penile implant was fixed in the cavernous body reminiscent and covered by the radial antebraquial flap (18 cm length, 13 cm width) with termino-lateral arterial anastomosis in the femoral artery, and termino-lateral venous anastomosis in the safena vein. Due to local surgical conditions, the pudendum nerve was not localized and we opted to not reconstruct the penile uretra. The patient got a good result, in just one surgical time of total microsurgical faloplasty with penile implant, despite high complication rates described in the literature.

RESUMO

Os autores relatam um caso de reconstrução total de pênis em paciente do sexo masculino, de 24 anos, vítima de queimadura elétrica de alta tensão, comprometendo 30% da área corporal, de 2º e 3º grau, com amputação do membro superior direito, do pênis e testículos, e necrose da parede abdominal baixa. Após tratamento clínico e cirúrgico no Centro de Terapia a Queimados, foi encaminhado para o Serviço de Microcirurgia Reconstrutiva do Hospital dos Servidores do Estado do Rio de Janeiro, para reconstrução tardia. O primeiro tempo cirúrgico foi reconstrução da parede abdominal infra-umbilical com retalho miofasciocutâneo do tensor da fáscia lata. O segundo tempo foi reconstrução peniana total com retalho microcirúrgico antebraquial radial, concomitante com implante peniano inflável, realizado em conjunto com a equipe da urologia. O implante peniano foi fixado no resquício do corpo cavernoso e envolto pelo retalho radial antebraquial, de 18 cm de comprimento e 13 cm de largura, com anastomose arterial término-lateral na artéria femoral e a anastomose venosa término-terminal na veia safena. Em decorrência da gravidade da queimadura e intensa fibrose local, o nervo pudendo não foi encontrado. Optou-se por não reconstruir a uretra peniana concomitante para diminuir a morbidade cirúrgica. Obteve-se um bom resultado, em um único tempo operatório, ou seja, a faloplastia total microcirúrgica com implante peniano, apesar dos altos índices de complicações relatados na literatura.

ABSTRACT

Hidradenitis suppurativa also known as Verneuil's disease is a chronic inflammatory and recurrent disease that occurs in the apocrine glands. It results in recurrent abscesses with purulent secretion, fistulae formation, unaesthetic and retractable scars. The most common sites involved include the axilla, inguinoperineal regions, nipple-areolar complex, and umbilicus. Here, we report a case of severe Verneuil's disease of the axillary region managed with wide excision surgery, and thoracodorsal fasciocutaneous flap and bilateral coverage. This method provides good results for reconstruction of the axillary region.

RESUMO

A hidradenite supurativa, também conhecida como doença de Verneuil, é uma inflamação crônica e recorrente das glândulas apócrinas, sem etiologia conhecida, que evolui frequentemente com saída de secreção purulenta, formação de fístulas, cicatrizes inestéticas e retráteis, podendo acometer região inguinal, axilar, aréolas mamárias e umbigo. Neste trabalho, é apresentado um caso grave de doença de Verneuil em região axilar, em que foi realizada exérese da lesão e reconstrução com retalho fasciocutâneo toracodorsal bilateral como uma opção de reconstrução da região axilar, com boa resolubilidade e resultado estético.

RESUMO

Objetivo: Relatar um caso de onfaloplastia.
Relato de Caso: Paciente do gênero feminino, J.P.O., de 30 anos, atendida no Serviço de Cirurgia Plástica Dr. Ewaldo Bolivar, com queixa de "umbigo grande".
Conclusão: Cirurgia realizada no Santos Day Hospital, utilizando técnica em retalho semicircular, evoluindo com satisfação o total da paciente.
