Keloid and hypertrophic scar distribution according to Fitzpatrick skin phototypes

ABSTRACT

BACKGROUND: Keloid and hypertrophic scars have a common physiopathogenic origin and are defined as fibroproliferative scars. Fibroproliferative scars are frequent in individuals with darker skin. However, mixing of "races" renders it difficult to group patients with different skin tones according to morphological and static classifications (white for Caucasians; brown for individuals of Spanish descent (Hispanic/Latino); yellow for individuals of East Asian descent; and black for individuals of African descent) according to their response to sun exposure. It is known that when individuals whose ethnic origin is in colder countries move to tropical countries, they show a higher incidence of these types of scars, which mainly affect parts of the body that are more exposed to the sun. A correlation between fibroproliferative scars and Fitzpatrick phototype, a dynamic classification based on the skin's response to sun exposure, would contribute to an understanding of the pathophysiology of these scars. The aim of this study is to investigate the distribution of fibroproliferative scars according to Fitzpatrick phototypes. METHODS: We classified patients' fibroproliferative scars according to the Muir classification as Long-Term Evolution (keloid scars), Short-Term Evolution (hypertrophic scars), and Intermediate Group (mixed scars), while their skin types were grouped according to the Fitzpatrick classification. RESULTS: Fitzpatrick phototype III and mixed scars were predominant among the patients analyzed (p = 0.001). A correlation (p = 0.025) was observed between fibroproliferative scars and Fitzpatrick phototypes; the higher the phototype, the higher the tendency to develop keloid and mixed scar tissue. CONCLUSIONS: Fitzpatrick skin phototypes proved to be an efficient method to study keloid and hypertrophic scars.

RESUMO

INTRODUÇÃO: Queloide e cicatriz hipertrófica são cicatrizes patológicas com natureza fisiopatogênica comum, denominadas, em conjunto, cicatrizes fibroproliferativas. São mais frequentes em indivíduos de pele mais escura. Contudo, a atual miscigenação dificulta o enquadramento dos pacientes com variadas tonalidades de pele em classificações morfológicas e estáticas (branco ou caucasoide, mulato, pardo, hispânico ou latino, amarelo ou oriental ou mongoloide e negro ou negroide), e diferentes quanto à exposição solar. Sabe-se que pessoas oriundas de países de clima temperado ou frio quando residem em países tropicais aumentam a incidência dessas cicatrizes, principalmente nas áreas de maior exposição solar. Uma relação entre as cicatrizes fibroproliferativas e os fototipos de Fitzpatrick, classificação dinâmica baseada no relato do paciente quanto a sua resposta cutânea após a exposição solar, poderia contribuir para a compreensão da fisiopatologia dessas cicatrizes. Este estudo teve como objetivo investigar a distribuição das cicatrizes fibroproliferativas segundo os fototipos de Fitzpatrick. MÉTODO: Foram avaliados 146 pacientes provenientes do Ambulatório da Disciplina de Cirurgia Plástica da Universidade Federal de São Paulo (Unifesp, São Paulo, SP, Brasil), portadores de qualquer tipo de cicatriz fibroproliferativa, em um ou mais locais do corpo. As cicatrizes fibroproliferativas dos pacientes foram classificadas de acordo com os critérios de Muir em cicatriz tipo queloide (Long-term Evolution, LTE), cicatriz tipo hipertrófica (Short-term Evolution, STE) e cicatriz tipo mista (Intermediate Group, IG), e os tipos de pele foram classificados segundo os fototipos de Fitzpatrick. RESULTADOS: O fototipo Fitzpatrick III e a cicatriz mista foram mais frequentes entre os pacientes avaliados (P = 0,001). Houve associação (P = 0,025) entre as cicatrizes fibroproliferativas e os fototipos de Fitzpatrick, ou seja, quanto maior o fototipo maior a tendência de desenvolvimento de cicatrizes dos tipos queloide e mista. CONCLUSÕES: Os fototipos de pele segundo Fitzpatrick mostraram-se válidos como critério a ser utilizado em estudos de queloide e cicatriz hipertrófica.

Facelift light, rhytidoplasty for the treatment of the face and neck lower third with reduced scarring and adhesion points

ABSTRACT

Introduction: Facelift light represents a technique standardization to treat face and neck lower third, with lesser scars, SMAS plication in all patients, and adhesion points to reduce bruises. Moderate detachment allows the patient to return to their activities faster.
Methods: Surgery is performed after prior periauricular marking of all patients of the area to be detached and the skin's design to be removed. Anesthesia is local with sedation, and without the need for haircuts. Flap detachment, SMAS plication, adhesion points in the anterior and posterior flaps are performed, in addition to the internal suture by layers and liposuction of the subment when necessary. The dressing is compressive without using drains.
Results: One hundred and eighty-five patients underwent rhytidoplasty using this technique since 2014. Their age varied between 31 and 84 years, with an average of 55 years. Lymphatic drainage is performed after seven days, and after two weeks, a considerable reduction in edema has been observed.
Conclusion: Facelift light is a useful technique, simple to perform with reduced scarring and few complications, favoring the early return of patients to their activities.

RESUMO

Introdução: O facelift light apresenta uma padronização da técnica com o objetivo de tratar o terço inferior da face e o pescoço, com menores cicatrizes, plicatura do SMAS em todos os pacientes e utilização de pontos de adesão para redução de hematomas. O descolamento moderado permite um retorno mais rápido do paciente às suas atividades.
Métodos: A cirurgia é feita após uma marcação prévia periauricular de todos pacientes, com a área a ser descolada e o desenho da pele a ser retirada. A anestesia é local e sedação, sem a necessidade de cortes no cabelo. É feito o descolamento dos retalhos, plicatura do SMAS, pontos de adesão nos retalhos anterior e posterior, além da sutura interna por planos e lipoaspiração do submento quando necessária. O curativo é compressivo sem a utilização de drenos.
Resultados: Foram submetidos à ritidoplastia por essa técnica 185 pacientes a partir de 2014. Com a idade variando entre 31 e 84 anos, sendo a média de 55 anos. É feita drenagem linfática a partir de sete dias e com duas semanas observou-se uma redução considerável do edema.
Conclusão: O facelift light é uma técnica eficaz, de simples execução, cicatrizes reduzidas e com poucas complicações, favorecendo o retorno precoce dos pacientes às suas atividades.

RESUMO

O Banco de Pele Dr. Roberto Corrêa Chem, fundado em 2005, é responsável pela captação, preservação, armazenamento e distribuição de pele humana alógena. OBJETIVO: Relatar a produtividade do Banco de Pele da Santa Casa de Misericórdia de Porto Alegre, durante o período de 2008 a 2016. MÉTODOS: Para análise de produtividade do Banco de Pele, realizou-se consulta aos relatórios mensais, cujo preenchimento periódico faz parte da rotina do Banco de Pele. RESULTADOS: Durante o período de 2008 a 2016, foram realizadas 298 captações de pele, 229 envios, com a extensão total de pele enviada de 256.043,89 cm2, média de 28.449,32 cm2 enviados por ano. Em 2016 se encontra o período com a maior quantidade de envios, total de 49 doações. A maior extensão de pele enviada foi em 2013, 41.618,50 cm2. CONCLUSÃO:O Banco de Pele da Santa Casa de Misericórdia de Porto Alegre tem contribuído anualmente com a demanda de pele de todo Brasil.

Areolar reconstruction using eyelid skin

ABSTRACT

Introduction: Breast cancer is currently the second most common neoplasm among women who undergo clinical treatments and breast-mutilating surgeries. Having beautiful and harmonious breasts is a legitimate right, so the concept of reconstructing the breast and nipple-areolar complex becomes important in femalew social reinsertion. This study aimed to present an effective technique of areolar reconstruction using free bilateral eyelid skin grafts and demonstrate its effectiveness through photographic results, comparison with previously described techniques, and patient satisfaction questionnaires. Results: The authors retrospectively evaluated 50 patients who underwent areolar reconstruction with eyelid skin grafts and other techniques performed by a single senior surgeon. Patients who underwent eyelid skin graft surgery did not achieve significantly different results from those who underwent other reconstruction techniques. Patients who underwent areola surgeries reported good satisfaction. Conclusion: The study revealed low complication rates and demonstrated that the use of eyelid skin is a good therapeutic option for areolar reconstruction.

RESUMO

Introdução: Sabe-se que o câncer de mama é hoje a segunda neoplasia mais frequente entre as mulheres, as quais são submetidas desde a tratamentos clínicos até cirúrgicos mutilantes da mama. Ter mamas bonitas e harmoniosas é um direito legítimo, sendo assim o conceito de reconstrução mamária e do complexo areolopapilar, se torna importante na reinserção social feminina. Este estudo visa apresentar uma técnica eficaz de reconstrução areolar, utilizando enxerto cutâneo livre palpebral bilateral, demonstrando sua eficácia através de resultados fotográficos, comparação com outras técnicas já descritas e questionários de satisfação dos pacientes.
Resultados: Foram avaliados, retrospectivamente, 50 pacientes operados de reconstrução areolar com enxerto de pele palpebral e outras técnicas já descritas, operados por um único cirurgião sênior. O grupo de pacientes operados com pele palpebral não obtiveram diferenças significativas dentro da análise de resultados, comparados com outras técnicas de reconstrução. Houve boa satisfação por parte dos pacientes, quanto ao resultado das cirurgias das aréolas.
Conclusão: O estudo obteve baixos índices de complicações, apresentando-se como boa opção terapêutica, dentro do arsenal cirúrgico de reconstrução areolar.

Using tamoxifen in keloid treatment

ABSTRACT

Background: We all know that the survival of the burn victims has increased due to the advances of the treatment in the critical phase. Because of that, the number of patients who shows sequels also increased. The objective of the present study is to measure the improvement in the quality of the lifetime of the patients with hypertrophic scars as result of burn or graft skin reducing, that way, the stigma caused by these scars. Many approaches have been done in order to prevent and treat pathological scars, frequently without success to some patients because the method or because of the difficulty of keeping the patient attached to the treatment. Method: Forty-six patients, 20 men and 26 women, have used 0.1% topic tamoxifen for a period of 4 to 12 months. Weekly evaluations were done during the first month and afterwards monthly, until a year. Photographs were taken after 6 months and a year. Results: Thirty-eight (82.6%) patients told that the improved in the first two weeks of treatment; 93.48% of the patients told that the scar height was reduced. In this study, we used tamoxifen citrate, an antineoplasic drug, to reduce the height of hypertrophic scars of burn marked patients. Conclusion: The results encouraged us to continue the treatment of scars with topic house use of tamoxifen.

Introdução: É sabido que a sobrevivência de pacientes vítimas de queimaduras tem aumentado significativamente devido aos avanços do tratamento na fase aguda. Como conseqüência, a quantidade de pacientes apresentando sequelas também aumentou. O propósito deste estudo foi avaliar a melhora na qualidade de vida de pacientes com cicatrizes hipertróficas resultantes de queimaduras ou após enxertos de pele, através da redução do estigma causado por estas cicatrizes. Várias abordagens têm sido descritas para prevenir e tratar cicatrizes patológicas, freqüentemente sem sucesso em alguns pacientes, por causa do método ou pela dificuldade em se conseguir a aderência do paciente ao tratamento. Método: Quarenta e seis pacientes, 20 homens e 26 mulheres, usaram tamoxifen tópico 0,1%, em um período de 4 a 12 meses. Avaliações semanais foram feitas no primeiro mês e, mensalmente, até o limite de um ano. Fotografias foram tiradas após seis meses e um ano. Resultados: Trinta e oito (82,6%) pacientes relataram melhora no prurido nas primeiras duas semanas de tratamento. Melhora na altura da cicatriz foi relatada por 93,48% dos pacientes. Neste estudo, usou-se citrato de tamoxifeno, uma droga antineoplásica, para redução da altura de cicatrizes hipertróficas de pacientes seqüelados de queimaduras. Conclusão: Os resultados nos encorajaram a continuar o tratamento de cicatrizes com o uso tópico domiciliar de tamoxifeno.

Versatility of the bilobed flap

ABSTRACT

Background: The bilobed flap is a double transposition flap. Its geometric drawing allows a better distribution of tensile forces along its axis of rotation, avoiding distortions and dog ears generated by other flaps and primary closure. It can be harvested in an axial or random pattern depending on where the defect is located on the body. Methods: Retrospectively, this study reviewed 45 consecutive bilobed flaps searching for pathology, anatomical area, clinical evolution and complications. The technique utilized in drawing and harvesting the flap is described in detail. Sizes of the defects were classified as small, moderate and large, taking into account the anatomical sites. A variety of clinical cases is presented exploring the versatility of the flap and etiology of the defects. Results: The bilobed flap was employed in oncological surgery in 93% of the cases while the skin of the head was involved in 71%.The post operative period was free of complications in 85%. Infection rate was 4.4%, trapdoor scaring rate was 4.4% and partial flap loss rate was 11.1%. The overall complication rate was 15%. In spite of all complications there were no compromise in the functional and aesthetic results and the bilobed flaps proved to be resolute and reliable in different clinical presentations and body regions. Conclusions: The bilobed flap is extremely versatile, easily reproducible with wide application in plastic surgery.

RESUMO

Introdução: O retalho bilobado é um retalho de dupla transposição. Sua estruturação geométrica permite melhor distribuição das forças de tensão ao longo de seu eixo de rotação, evitando distorções e redundâncias cutâneas geradas por outros retalhos ou sutura primária. Pode ser confeccionado com padrão vascular axial e aleatório, dependendo da região anatômica. Métodos: Este trabalho faz uma revisão de 45 casos operados, descrevendo em detalhes a técnica utilizada para confeccionar o retalho bilobado. Os defeitos são classificados em pequenos, médios e grandes e diversos casos clínicos são apresentados, explorando a versatilidade desse retalho em diferentes diagnósticos etiológicos e sítios anatômicos. Resultados: Em 93% dos casos, o retalho bilobado foi empregado em reconstruções oncológicas, sendo o segmento cefálico acometido em 71%. Não houve qualquer intercorrência pós-operatória em 85% dos casos. A taxa de infecção foi de 4,4%; de cicatriz em alçapão, de 4,4%; e de epiteliólise e necrose, de 11,1%. A taxa geral de complicações foi de 15%. Entretanto, tais complicações não comprometeram a evolução clínica dos casos e o retalho bilobado mostrou-se resolutivo em diferentes situações, propiciando bom resultado tanto funcional como estético. Conclusões: O retalho bilobado é versátil, de fácil execução e com ampla aplicabilidade em cirurgia plástica.

Evaluation of the effectiveness of a modified liposuction protective device: an experimental study in swine

ABSTRACT

Liposuction is one of the most common procedures in the plastic surgery specialty. In the 2018 ISAPS survey, it was the second most performed surgery worldwide. Due to the repetitive movements typical of the surgery, significant friction is generated at the site, and the consequences are skin burns that can leave unsightly scars and dyschromias. This study aims to create a skin protective device prototype from an old model, which serves this purpose, and to observe its functionality and its effects on the pigskin. The tests were carried out on dead animals without suffering provided by the Veterinary Medicine sector at Universidade Positivo. Three incisions were made in the animal's abdomen to pass the liposuction cannula and another three for the insertion and use of the prototype to compare it with the model device. The established time for liposuction movements was twenty minutes, performed with the 5mm cannula directly in contact with the skin and inside the prototype. The prototype's ergonomics, ease of insertion, and good locking on the skin with different tractions were evaluated. Observation and evaluation of the skin were performed after procedures and incisions' measurements (cm). The cutaneous liposuction protective device prototype presented easy handling and a more efficient skin locking mechanism than the model used. The skin incision for using the prototype was slightly larger, and the skin showed no burning signs.

RESUMO

A lipoaspiração é um dos procedimentos mais comuns na especialidade de cirurgia plástica. No levantamento da ISAPS de 2018 foi a segunda cirurgia mais realizada em todo o mundo. Devido aos movimentos repetitivos próprios da cirurgia, fricção importante é gerada no local e as consequências são queimaduras cutâneas que podem deixar cicatrizes inestéticas e discromias. O objetivo deste estudo é criar um protótipo de um dispositivo protetor da pele, a partir de um modelo antigo, que sirva a esse propósito, e observar sua funcionalidade e os efeitos da sua utilização na pele de suínos. Os testes foram realizados em animais mortos sem sofrimento disponibilizados pelo setor de Medicina Veterinária da Universidade Positivo. Foram feitas três incisões no abdome do animal para passagem da cânula de lipoaspiração e outras três para a inserção e utilização do protótipo, bem como para comparação com o dispositivo modelo. O tempo estabelecido de movimentos de lipoaspiração foi de vinte minutos, realizados com a cânula de 5mm diretamente em contato com a pele e dentro do protótipo. Foi avaliada a ergonomia do protótipo, facilidade de inserção e travamento adequado na pele com diferentes trações. Observação e avaliação da pele após os procedimentos e medição (cm) das incisões foram realizadas. O protótipo do dispositivo protetor cutâneo de lipoaspiração criado apresentou fácil manuseio e mecanismo de travamento na pele mais eficiente quando comparado ao modelo utilizado. A incisão cutânea para uso do protótipo foi ligeiramente maior e a pele não apresentou sinais de queimadura.

Algorithms for the management of scars: the importance of systematizing behaviors

ABSTRACT

Introduction: Pathological scars occur from the hyperproliferation of fibroblasts and can be classified into hypertrophic scars and keloids. Basically, hypertrophic scars do not grow beyond the limits of the original wound, while keloids grow horizontally in a nodular form. Despite the diversity of instruments used to guide the prevention, treatment and follow-up of pathological scars, there is a need for instruments that address local realities. The objective is to carry out a narrative review of the literature on scar management algorithms and create an updated algorithm.
Methods: Descriptive study of narrative literature review, with a search in PubMed, SciELO, LILACS, MEDLINE and Cochrane databases, from November 2010 to November 2020, published in English, Portuguese and Spanish. The descriptors used were: “cicatrix,” “keloid,” “algorithms,” and “wound healing.” The sample selection consisted of identifying the articles, reading the titles and abstracts, and selecting studies related to the topic. Subsequently, the full reading of the selected studies and classification according to the eligibility criteria were carried out. Results: 209 articles were found, and 116 were eliminated due to duplicity, resulting in 45 articles. A total of 8 articles that met the inclusion criteria were identified. Four articles were excluded after analysis and consensus meeting due to the absence of algorithms with scientific rigor; this study is composed of four articles.
Conclusion: Four algorithms were found in the literature review that resulted in the development of an updated algorithm for scars.

RESUMO

Introdução: As cicatrizes patológicas ocorrem a partir de hiperproliferaçãoo de fibroblastos, podendo ser classificadas em cicatrizes hipertróficas e queloides, basicamente as cicatrizes hipertróficas não crescem além dos limites da ferida original, enquanto os queloides crescem horizontalmente de forma nodular. Apesar da diversidade de instrumentos utilizados para orientar a prevenção, tratamento e seguimento de cicatrizes patológicas, existe a necessidade de instrumentos que contemplem realidades locais. O objetivo é realizar uma revisão narrativa de literatura sobre algoritmos para manejo de cicatrizes e criar um algoritmo atualizado.
Métodos: Estudo descritivo de revisão narrativa de literatura, sendo realizado uma pesquisa nas bases de dados PubMed, SciELO, LILACS, MEDLINE e Cochrane, no período de novembro de 2010 até novembro de 2020, publicados nos idiomas inglês, português e espanhol. Os descritores utilizados foram: “cicatrix”, “keloid”, “algorithms” e “wound healing”. A seleção da amostra consistiu da identificação dos artigos, leitura dos títulos e resumos e seleção de estudos relacionados ao tema e, posteriormente, foi realizada a leitura na íntegra dos estudos selecionados e classificação segundo os critérios de elegibilidade.
Resultados: Foram encontrados 209 artigos sendo eliminados 116 devido duplicidade resultando em 45 artigos. Foram identificados um total de 8 artigos que preencheram os critérios de inclusão e após análise e reunião de consenso foram excluídos quatro artigos devido à ausência de algoritmos com rigor científico sendo este estudo composto de quatro artigos.
Conclusão: Foram encontrados quatro algoritmos na revisão de literatura que resultaram na elaboração de um algoritmo atualizado para cicatrizes.

Evaluation of long-term results of rhytidoplasty

Many articles have been published about rhytidoplasty, however, few report on the long-term results. The objective of this article is to try to evaluate the results of face lifts and to define long-term results. The evaluation of rhytidoplasty is not an easy task. Different methods are utilized but there are no parameters that can assess the final product unless the result is bad. The process of aging does not cease after the facial rejuvenating surgery. Several important factors are involved in the result and the duration of the surgical effects including: the different ages at which patients are submitted to face lifts, the different surgical goals and the experience and good sense of the surgeon. Thus, this subject is of special interest to the plastic surgeon. In this article we tried to explore some of these aspects. Is it possible this is a philosophical discussion?

RESUMO

Vários artigos são publicados sobre ritidoplastia, porém, poucos abordam seus resultados tardios. O objetivo deste artigo é tentar avaliar os resultados do face lift e definir qual é o resultado tardio. A avaliação da ritidoplastia não é tarefa fácil. Diferentes métodos são utilizados, porém não há parâmetros que possam avaliar o produto final, a não ser que um resultado ruim ocorra. O processo do envelhecimento não cessa após a cirurgia de rejuvenescimento facial. Vários fatores, de modo importante, fazem parte do resultado e da duração dos efeitos da cirurgia: as diferentes idades em que os pacientes são submetidos ao face lift, as diferentes propostas cirúrgicas, a experiência e o bom senso do cirurgião. Sendo assim, este assunto torna-se extremamente interessante para o cirurgião plástico. Neste artigo, tentamos expor alguns destes aspectos. Seria esta uma discussão filosófica?

ABSTRACT

A case report of Marjolin's ulcer, observed at the "Fundação do ABC" teaching hospital, in São Paulo city. Sixty years-old male patient. Outcome period of more than 50 years, from the scar to the squamous cell carcinoma rise, aggravation due to arsenic contact, to suppurations, and to second-intention healing. Despite the surgical treatment with extensive resection and total axillary lymphadenectomy, the patient presented metastases within a short period of time, dying due to acute respiratory failure resulting from the pulmonary and mediastinal metastases. The authors present a revision on the development of malignant tumors on cutaneous scars, especially on burning scars.

RESUMO

Relato de caso de úlcera de Marjolin observado no Hospital de Ensino da Fundação do ABC, São Paulo. Paciente do sexo masculino, 60 anos de idade, apresenta tempo de evolução do caso superior a 50 anos, da cicatriz até surgimento do carcinoma espinocelular. A lesão agravou-se devido ao contato com o arsênico, devido às supurações, e cicatrizações por segunda intenção. Apesar do tratamento cirúrgico com ressecção ampla e linfadenectomia axilar total associada, apresentou metástases numerosas em curto espaço de tempo, levando o paciente a óbito por insuficiência respiratória decorrente de metástases pulmonares e mediastinais. Os autores apresentam uma revisão sobre o desenvolvimento de tumores malignos sobre cicatrizes cutâneas, especialmente sobre cicatrizes de queimadura.

Z-plasty in axillary retraction: techniques and strategies to facilitate learning by medical residents

ABSTRACT

INTRODUCTION: Herein, we present two cases of post-burn retraction of the axillary region, which were subsequently surgically corrected using multiple Z-plasties. We present a marking strategy to facilitate the learning of this procedure by plastic surgery residents. METHODS: Two patients with axillary and upper limb retractions due to burns with hot liquids were surgically treated using multiple Z-plasties. The strategy proposed in this study recommends the marking of the triangular flap located at the midpoint of the axillary retraction, which then advances and crosses the axis of the scar. Then, subsequent flaps are made at an angle of 60º to each other. We believe that this systematization of marking facilitates the planning and learning of Z-plasty. RESULTS: The study patients achieved an adequate range of motion and functional recovery of the affected limb postoperatively. We observed no cases of necrosis of the flap ends, which should be carefully worked and slightly rounded. The execution of the procedure was simple using the suggested marking strategy. CONCLUSION: The presented marking strategy was reproducible and easy to learn. Multiple Z-plasties were effective in restoring the range of motion of the areas affected by burns that were treated using this method.

RESUMO

INTRODUÇÃO: São apresentados dois casos de retração em região axilar após queimadura em que a correção cirúrgica foi realizada empregando-se zetaplastias múltiplas. Apresentamos uma tática de marcação que visa facilitar o aprendizado desse procedimento pelos residentes de Cirurgia Plástica. MÉTODOS: Dois pacientes com retrações axilares e de membros superiores após queimadura com líquido aquecido foram submetidos à correção cirúrgica empregando-se a zetaplastia múltipla. A tática de planejamento proposta nesse estudo preconiza primeiro a marcação do retalho triangular localizado no ponto médio da retração axilar, o qual irá avançar e cruzar o eixo da cicatriz, e a partir daí o desenho dos retalhos subsequentes com ângulo de 60º entre si. Acreditamos que essa sistematização da marcação facilita o planejamento e aprendizado da plástica em Z. RESULTADOS: Os pacientes apresentaram adequadas amplitude de movimento e recuperação funcional do membro acometido no pós-operatório. Não houve necrose das extremidades dos retalhos, que preconizamos serem cuidadosamente trabalhadas e um pouco arredondadas; e a execução do procedimento foi simples devido à tática de marcação que aplicamos. CONCLUSÃO: A tática de marcação que apresentamos é reprodutível e de fácil aprendizado. A zetaplastia múltipla foi eficaz em restabelecer a amplitude de movimento das áreas com sequela de queimadura tratadas com esse método.

Nanofat injector: a low-cost disposable device for standardization and optimization of grafting time

ABSTRACT

Introduction: Nanofat graft improves skin quality in damage secondary to aging and scar sequelae. We present the initial results of the nanofat graft using a low-cost disposable device, proposing a standardization of its use according to the area to be treated.
Methods: A prospective cohort was conducted from July 2019 to March 2020. The inclusion criterion was patients who underwent nanofat grafting for skin treatment. The exclusion criterion was the previous performance of some invasive treatment of the skin. Twenty consecutive patients who met the prerequisites were analyzed. The results were evaluated in the 6th postoperative month. The patients answered a questionnaire, classifying from 1 - very bad to 10 - excellent, changes in skin quality.
Results: The twenty patients followed did not present any postoperative complications. Edema after the application was reduced between three and seven days. There was no hematoma or infection. Patients who underwent only nanofat grafting without another associated surgery could return to their activities after 24 hours. The scores reported by patients at 6 months were between 7 and 10, with a mean of 8.
Conclusion: The use of the SmartneedleTM system for nanofat grafting presents patient satisfaction similar to other application methods and allows a uniform and standardized distribution of the graft according to the anatomical region and optimizing surgical time

RESUMO

Introdução: O enxerto de gordura nano melhora a qualidade da pele nos danos secundários ao envelhecimento e nas sequelas cicatriciais. Apresentamos resultados iniciais do enxerto de gordura nano com o uso de um dispositivo descartável de baixo custo propondo uma padronização da sua utilização conforme a área a ser tratada.
Métodos: Foi realizada uma coorte prospectiva de julho de 2019 a março de 2020. O critério de inclusão foi pacientes que realizaram enxerto de gordura nano para tratamento da pele. Já o critério de exclusão foi a realização prévia de algum tratamento invasivo da pele. Foram analisadas 20 pacientes consecutivas que preencheram os pré-requisitos. Os resultados foram avaliados no 6º mês de pós-operatório. As pacientes responderam um questionário, classificando de 1 - muito ruim a 10 - excelente, as alterações na qualidade da pele.
Resultados: As vinte pacientes acompanhadas não apresentaram nenhuma complicação pós-operatória. O edema após aplicação reduziu entre três e sete dias. Não houve hematoma nem infecção. As pacientes que realizaram somente enxerto de gordura nano, sem outra cirurgia associada, conseguiram voltar às suas atividades após 24 horas. Os escores relatados pelas pacientes com 6 meses foram entre 7 e 10, com média de 8.
Conclusão: A utilização do sistema SmartneedleT para a enxertia de gordura nano apresenta resultados na satisfação das pacientes semelhante aos outros métodos de aplicação e permite uma distribuição uniforme e padronizada do enxerto conforme a região anatômica, além de otimizar o tempo cirúrgico.

Subcicatricial fat grafting associated with rigotomy: a clinical and microstructural analysis

ABSTRACT

INTRODUCTION: The increase in survival of large burn patients during the acute phase has also increased the prevalence of esthetic-functional scarring sequelae. With regard to the clinical relevance of subcicatricial fat grafting, the actual changes after the procedure need to be understood and microscopically evaluated. METHODS: Eight patients with alcohol burns, with an average time after burn of 12 months (10-14 months), who underwent subcicatricial fat grafting associated with rigotomy were selected. Skin biopsies were performed in before and 14 weeks after operation. The following issues were assessed: 1) esthetic-functional improvement of the scar, by using the Vancouver scar scale; 2) quantitative and qualitative analyses of cicatricial collagen; 3) immunohistochemical analysis of scar vascularization with anti-vascular endothelial growth factor (anti-VEGF) antibody. RESULTS: When comparing the pre- and post-fat grafting period, a significant esthetic-functional improvement and, microscopically, a redefinition of the boundaries of the papillary and reticular dermis were observed, as well as quantitative reduction and reorganization of collagen, in addition to the decrease of the vascularization of the tissue through immunohistochemical analysis. CONCLUSION: The basic principle of the whole physiological healing process is to reestablish local homeostasis, that is, excessively intensified steps that lead to severe clinical changes. When subcicatricial fat grafting associated with rigotomy was performed, qualitative and quantitative improvements of the tissue were verified in this study. Thus, it becomes evident that this procedure can complement the treatment of cicatricial pathologies.

RESUMO

INTRODUÇÃO: O aumento da sobrevida na fase aguda do paciente grande queimado faz também aumentar a prevalência das sequelas cicatriciais estético-funcionais. Ao pensarmos na relevância clínica da lipoenxertia subcicatricial, necessita-se da compreensão e avaliação microscópica das reais alterações após o procedimento. MÉTODOS: Foram selecionados oito pacientes, vítimas de queimadura por álcool, com tempo médio após queimadura de 12 meses (10-14 meses), sendo submetidos à lipoenxertia subcicatricial associada à Rigottomy. Foram realizadas biópsias cutâneas no pré e pós-operatório tardio com 14 semanas. Avaliados os quesitos: 1 - Melhora estético funcional da cicatriz pela escala de Vancouver; 2 - Análise quantitativa e qualitativa do colágeno cicatricial; 3 -Análise imunohistoquímica da vascularização cicatricial com anti-fator de crescimento derivado do endotélio vascular (antiVEGE). RESULTADOS: Ao compararmos o período pré e pós-lipoenxertia, pôde-se avaliar que houve melhora estético-funcional significativa e, microscopicamente, redefinição entre os limites da derme papilar e reticular; redução quantitativa e reorganização do colágeno, além do decréscimo da vascularização tecidual pela análise imunohistoquímica. CONCLUSÃO: O princípio básico de todo processo cicatricial fisiológico é reestabelecer a homeostasia local, ou seja, as etapas exageradamente intensificadas levam a alterações clínicas catastróficas. Ao realizar a lipoenxertia subcicatricial associada à Rigotomia, foi verificada, neste estudo, a melhora qualitativa e quantitativa do tecido. Sendo assim, torna-se evidente o futuro promissor deste procedimento para a complementação terapêutica das patologias cicatriciais.

Fibroproliferative disorders: report and discussion on keloid management

ABSTRACT

Fibroproliferative disorders are expressed in hypertrophic scars and keloids, the latter being more aggressive and derived from an abnormal healing process. They are multifactorial and relate to physical, chemical, biological, and endogenous agents. They have a genetic predisposition, with a higher incidence in Asian and black people. The therapeutic modalities comprise most of the times: compression of the keloid, cryosurgery, application of silicone plates, surgical excision isolated or followed by radiotherapy, laser application, and intralesional injection of corticosteroids. The study aims to report a considerable keloidtype fibroproliferative disorder with a high therapeutic response and discuss its etiologies and various therapeutic modalities.

RESUMO

Os distúrbios fibroproliferativos expressam-se pelas cicatrizes hipertróficas e pelos queloides, sendo estes últimos mais agressivos e derivados de um processo anormal da cicatrização. São multifatoriais relacionando-se com agentes físicos, químicos, biológicos e endógenos. Apresentam predisposição genética, com incidência maior em orientais e negros. As modalidades terapêuticas compreendem, na maioria das vezes: compressão do queloide, criocirurgia, aplicação de placas de silicone, exérese operatória isolada ou seguida de radioterapia, aplicação de laser e injeção intralesional de corticoesteroides. O objetivo do estudo é relatar um caso de distúrbio fibroproliferativo do tipo queloide de grandes dimensões com alta resposta terapêutica e discutir suas etiologias e diversas modalidades terapêuticas.

ABSTRACT

Introduction: Burns are one of the most severe traumatic injuries and their treatment requires a multidisciplinary approach, where the role of the plastic surgeon is vital. The plastic surgeon is entrusted with the challenging goal of restoring the skin's protective function and simultaneously recovering the aesthetic aspect of the burnt area.
Case report: A 27-year-old woman underwent a mastopexy with inclusion of implants, where the skin removed from the breast was used as a full-thickness graft in the mandibular and submental area for the treatment of a scar. The patient showed complete integration of the graft, and no areas of epidermolysis were observed. The aesthetic results were excellent, and the patient was completely satisfied; moreover, an improvement in the dyschromic and hypertrophic cicatricial areas was observed.
Conclusion: An autologous graft using breast skin is a good alternative for the treatment of sequelae of burns on the face and provides excellent aesthetic results.

RESUMO

Introdução: As queimaduras constituem uma das lesões traumáticas mais graves e seu tratamento requer uma abordagem multidisciplinar, em que o papel do cirurgião plástico é fundamental. Restabelecer a função de proteção da pele, mas também recuperar a estética da área, queimada são objetivos desafiadores que o cirurgião plástico procura atingir.
Relato de Caso: Paciente feminino de 27 anos submetida a mastopexia com inclusão de implantes, em que se aproveitou a pele retirada da mama para realizar um enxerto de espessura total em região mandibular e submentoniana para tratamento de cicatriz. A paciente teve uma integração completa do enxerto, sem evidenciar-se áreas de epidermólise. Os resultados estéticos foram excelentes, conseguindo a satisfação da paciente e melhoria das áreas discrômicas e hipertróficas cicatriciais.
Conclusão: O enxerto autólogo a partir da pele da mama constitui uma boa alternativa para o tratamento de sequelas de queimaduras em face, possibilitando ótimos resultados estéticos.

The study reviews the use of Z-plasty in the cheiloplasty techniques used to treat unilateral cleft lip. It highlights the contribution of Brazilian authors, especially that of Perseu Lemos. It also reports the conduct adopted by the senior author in this type of cheiloplasty.

ABSTRACT

Facial aging is a consequence of multiple intrinsic and extrinsic interactive factors, leading to loss of volume and repositioning of facial fat and bone remodeling. Breakthroughs have been seen in recent decades in the ability to reduce the signs of aging. Filling techniques and numerous products available on the market have generated new interest in the study of facial anatomy. Fillers are able to restore volume in limited areas and are suitable for treating facial wrinkles and loss of subcutaneous volume. The gold standard remains hyaluronic acid because it is more biocompatible and has greater permanence in the skin, without being definitive. The objective of this report is to review the anatomy of the middle third of the face and the main indications for filling and the techniques used for rejuvenation.

RESUMO

O envelhecimento facial é consequência de múltiplos fatores intrínsecos e extrínsecos que interagem entre si, levando à perda de volume e reposicionamento da gordura facial, assim como o remodelamento ósseo. Nas últimas décadas, houve um grande avanço na Medicina em reduzir os sinais do envelhecimento. As técnicas de preenchimento e os inúmeros produtos disponíveis no mercado trouxeram novo interesse no estudo da anatomia da face. Os preenchedores têm como função restaurar o volume de áreas restritas, portanto, são adequados para tratar rugas faciais e perda de volume subcutâneo. O padrão ouro atualmente é o ácido hialurônico, por ser mais biocompatível e ter uma permanência maior na pele, sem ser definitivo. O objetivo desse artigo é revisar a anatomia do terço médio da face, assim como as principais indicações de preenchimento e técnicas utilizadas para o rejuvenescimento.

ABSTRACT

Children are the most affected when it comes to burn injuries. Scar retraction of burned skin causes great difficulty for normal development, including the human skeleton. This work demonstrates how the surgical correction of cicatricial retraction in the cervical region allows the increase of neck and head mobility, potentially reducing the impact of the development of the region. The technique used was zetaplasty, with resection of the scar band and without skin grafting. Improvement in head movement was achieved after one month.

RESUMO

Crianças são as mais atingidas quando se trata de acidentes por queimaduras. A retração cicatricial da pele queimada acarreta grande dificuldade para o desenvolvimento normal, inclusive do esqueleto humano. Este trabalho demonstra de que forma a correção cirúrgica da retração cicatricial na região cervical permite o aumento da mobilidade do pescoço e da cabeça, potencialmente diminuindo o impacto do desenvolvimento da região. A zetaplastia foi a técnica utilizada, com ressecção da banda cicatricial e sem enxerto de pele. A melhora da movimentação da cabeça foi alcançada após um mês.

Intraoperative skin traction for wound closure following hygienic bilateral mastectomy: a case report

ABSTRACT

Introduction: Post-mastectomy chest wall defects require reconstruction using a safe and efficient procedure. In cases of local relapse, especially bilateral relapse, the surgical intervention indicated should be based on assessment of the patient's general condition and life expectancy. One such intervention, intraoperative skin traction, has been found to offer the benefits of rapid execution and low morbidity while reducing the scar formation associated with the use of skin flaps. Case Report: Hygienic bilateral mastectomy with dissection of the left axillary lymph nodes was performed on a 45-year-old woman after diagnosis of a relapsed tumor in the right breast and a new tumor in the left one. Based on the assessment of the general state of the patient, intraoperative skin traction of the wound edges was chosen for primary closure of the chest wall. Traction was performed according to the procedure described by Góes et al., which involved using Prolene 2-0 sutures, stretching the skin for 10 minutes, relaxation of the skin for two minutes, until the wound edges are sufficiently proximate to each other to perform primary suturing. This was achieved after three cycles of tension and relaxation. The procedure lasted two hours, and aspiratory drainage was performed with a Portovac 3.2 drain, which was removed after 7 days. The patient did not report pain or experience suture dehiscence or any other complications. Conclusion: In traoperative skin traction is a safe, functionally efficient, low-cost, and low-morbidity technique for wound closure following hygienic bilateral mastectomy.

Introdução: As sequelas da parede torácica após mastectomia requerem reconstrução por meio de conduta segura e eficiente. Nos casos de recidiva local, sobretudo bilateral, a intervenção cirúrgica deverá ser indicada considerando o estado geral do paciente e a expectativa de vida. A tração intraoperatória é uma técnica de baixa morbidade e rápida execução, reduzindo as cicatrizes decorrentes da utilização de retalhos. Relato do Caso: Paciente do sexo feminino, 45 anos que, após diagnóstico de recidiva tumoral em mama direita e novo tumor em mama esquerda, foi submetida à mastectomia bilateral higiênica com esvaziamento axilar à esquerda. Optou-se pela realização de tração cutânea intraoperatória dos bordos da ferida para fechamento primário da parede torácica devido ao estado geral da paciente. A tração foi realizada conforme a técnica de Góes et al., utilizando-se fios de prolene 2, tracionando a pele por dez minutos, com descanso de dois minutos, até que se obtivesse proximidade suficiente dos bordos das lesões para realização de sutura primária, o que foi alcançado após três ciclos de tração e relaxamento. O procedimento teve duração de duas horas, e foi colocado dreno portovac 3.2, que foi retirado após 7 dias. A paciente evoluiu sem queixas álgicas, sem deiscência ou qualquer outra complicação. Conclusão: A tração intraoperatória demonstrou ser uma técnica segura, funcionalmente eficaz, com menores custo e morbidade, para fechamento de ferida resultante de mastectomia bilateral higiênica.

ABSTRACT

Endometriosis is defined as the presence of endometrial glands and stroma outside the uterine cavity. This disease is commonly observed in women, particularly those of reproductive age. The pelvis is the most common location for endometriosis. On the other hand, extrapelvic endometriosis, which is less common, is more difficult to diagnose because of the extreme differences in its presentation. In this article, we describe our experience of a case of endometriosis in an abdominal wall scar.

RESUMO

A endometriose é definida como a presença de glândulas endometriais e estroma fora da cavidade uterina. Essa doença, comum nas mulheres, é geralmente observada durante os anos reprodutivos. Embora a pelve seja o sítio mais comum da endometriose em mulheres, a localização extrapélvica é menos frequente e ainda mais difícil de diagnosticar, em decorrência das apresentações distintas. Neste artigo é descrito um caso de endometriose de cicatriz da parede abdominal.
