Umbilical Restoration in Abdominoplasty: A Simple Rectangular Technique

ABSTRACT

The authors present a simple technique for umbilicus restoration in abdominal dermolipectomy. The procedure consists of a rectangular incision in the skin of the abdomen, slightly curved at the superior and inferior poles, after freeing the umbilicus with a circular incision in the abdominal tissue. The technique gives the umbilicus a natural, circular shape, obtaining a satisfactory result. Different drawings and pictures are shown to explain the skin procedure.

RESUMO

Os autores apresentam uma técnica simples para a restauração do umbigo na dermolipectomia abdominal. Este procedimento está representado por uma incisão cutânea retangular, ligeiramente curvilínea nos pólos superior e inferior, feita na pele do abdome, após uma incisão circular no tecido abdominal liberando o umbigo. Esta técnica deixa o umbigo com uma forma natural, circular, apresentando um resultado satisfatório. Diferentes desenhos e figuras propõem-se a explicar o procedimento cutâneo.

Technique for umbilicoplastie avoiding one of the most important stigma of abdominoplasties

ABSTRACT

The author makes a brief historical report of abdominoplasties, pointing its worst stigma: the unnatural umbilical scar with stitches marks and the flat abdomen over the umbilicus. To avoid these problems he presents his technique for umbilicoplasties where the association of liposuction with double lumen canula, with less trauma, and a proper way to cut and fixate the umbilicus with no external suture, gives better and more natural results.

RESUMO

O autor faz um pequeno histórico das abdominoplastias, chamando a atenção para o seu maior estigma que é a cicatriz umbilical pós-cirúrgica, quase sempre pouco natural e com cicatrizes de pontos que a tornam bastante inestética. Outro estigma é a aparência plana do abdome, acima do umbigo. Para evitar estes problemas, apresenta sua técnica para umbilicoplastias em cirurgia plástica abdominal, onde a associação da lipoaspiração com cânulas de duplo lúmen, menos traumáticas, para formar o sulco supra-umbilical e a forma especial de incisar e fixar o umbigo com pontos internos proporcionam excelente resultado estético que reproduz toda a naturalidade destas estruturas anatômicas.

Omphaloplasty based on an isosceles triangle with double fixation in abdominoplasty

ABSTRACT

Introduction: In conventional abdominoplasty, the creation of a new umbilical scar is challenging. Several surgical techniques and approaches have previously been described and applied, but not always with satisfactory results. The objective is to demonstrate the applicability and satisfaction with omphaloplasty based on an isosceles triangle with double fixation in abdominoplasty.
Methods: The study included 97 female patients aged between 25 and 65 years. All underwent classic abdominal dermolipectomy with moderate abdominal liposuction of the entire anterior abdomen and flanks by the same surgeon and were evaluated at 90, 180, and 360 days postoperatively.
Results: Patients were satisfied with the umbilicus in most cases (92.8%). Some umbilical scars had contracted (3.1%) and others appeared unsightly (4.1%). No necrosis was observed.
Conclusion: This technique was effective and easy to perform, with satisfactory umbilical scar aesthetic outcomes in abdominal dermolipectomy.

RESUMO

Introdução: Na abdominoplastia convencional, a cicatriz do novo umbigo representa o ponto de maior desafio. Em sua execução, já foram descritas e utilizadas várias técnicas e táticas cirúrgicas, com resultados nem sempre satisfatórios, sob o ponto de vista do paciente e também do médico. O objetivo é demonstrar a aplicabilidade e satisfação com a onfaloplastia em triângulo isósceles e com dupla fixação na abdominoplastia.
Métodos: Foram selecionadas 97 pacientes do sexo feminino, com idades entre 25 e 65 anos. Todas foram submetidas à dermolipectomia abdominal clássica associada à lipoaspiração moderada de todo abdome anterior e flancos e avaliadas com 90, 180 e 360 dias pós-operatórios, pelo mesmo cirurgião.
Resultados: Observou-se um índice de resultados satisfatórios das cicatrizes umbilicais na maioria dos casos (92,8%). Algumas cicatrizes umbilicais apresentaram estenoses (3,1%) e outras, cicatrizes inestéticas (4,1%). Não se observaram necroses.
Conclusão: A utilização desta técnica demonstrou ser eficaz, de fácil execução e com resultados muito satisfatórios na estética da cicatriz umbilical nas dermolipectomias abdominais.

Comparative study between omphaloplasty techniques

ABSTRACT

Introduction: The formation of pathological scar in omphaloplasty compromises the result of the abdominoplasty. Many techniques have been proposed with the objective to prevent such unfavorable evolution. Method: Seventy five patients submitted to abdominoplasty successively had been placed, randomized, into two groups. The first group (n=38) was composed for three circular techniques and the second group (n=37) by three non-circular techniques. The results had been analyzed with one, two, four and six postoperative months, concerning the presence of cicatricial hypertrophy or stenosis, the subjective satisfaction of the patients and the satisfaction of the surgeon (based in the incidence of the other criteria). The statistic analysis was carried through with the accurate test of Fisher (significant for p < 0.05). Results: By the end of six months, in the group of circular techniques, seven (18.2%) patient kept stenosis, eight (22.2%) said themselves unsatisfied with the aspect of the umbilical scar, and in the evaluation of the surgeon, nine (23.7%) patients presented inadequate result. In the group of the non-circular techniques, one (2.7%) patient kept stenosis, one (2.7%) said herself unsatisfied, and in the evaluation of the surgeon, five (13.6%) patients presented inadequate result. The use of non-circular techniques showed significant difference concerning the satisfaction of the patient (p=0.0286) and almost significant (p=0.0561) concerning the formation of stenosis. The use of circular techniques showed a relative risk seven times bigger in the prevalence of stenosis. All other parameters had not shown significant difference between the groups. Conclusion: The non-circular techniques of omphaloplasty, in this casuistry, have provided greater satisfaction to the patients. The comparison of several parameters between different techniques of omphaloplasty contributes for the elaboration of an ideal drawing of scar.

Introdução: A formação de cicatriz patológica na onfaloplastia compromete o resultado da abdominoplastia. Várias técnicas foram propostas com o objetivo de evitar tal evolução desfavorável. Método: Setenta e cinco pacientes submetidas a abdominoplastia foram sucessivamente alocadas, alternadamente, em dois grupos. O primeiro grupo (n=38) era composto por três técnicas circulares e o segundo grupo (n=37) por três técnicas não-circulares. Os resultados foram analisados com um, dois, quatro e seis meses pós-operatórios, quanto à presença de hipertrofia ou estenose cicatricial, à satisfação subjetiva da pacientes e à satisfação do cirurgião (baseada na incidência dos demais critérios). A análise estatística foi realizada com o teste exato de Fisher (significativo para p < 0,05). Resultados: Ao fim de seis meses, no grupo de técnicas circulares, sete (18,2%) pacientes mantinham estenose, oito (22,2%) diziam-se insatisfeitas quanto ao aspecto da cicatriz umbilical, e, na avaliação do cirurgião, nove (23,7%) pacientes apresentavam resultado inadequado. No grupo das técnicas não-circulares, uma (2,7%) paciente manteve estenose, uma (2,7%) dizia-se insatisfeita e, na avaliação do cirurgião, cinco (13,6%) pacientes apresentavam resultado inadequado. O uso de técnicas não-circulares mostrou diferença significativa quanto à satisfação da paciente (p=0,0286) e quase significativa (p=0,0561) quanto à formação de estenose. O uso de técnicas circulares mostrou risco relativo sete vezes maior na prevalência de estenose. Os demais parâmetros não mostraram diferença significativa entre os grupos. Conclusão: As técnicas não-circulares de onfaloplastia, nesta casuística, proporcionaram maior satisfação às pacientes. A comparação de vários parâmetros entre diferentes técnicas de onfaloplastia contribui para a elaboração de um desenho ideal de cicatriz.

Neoomphaloplasty using an X-shaped incision in 401 consecutive abdominoplasties

ABSTRACT

BACKGROUND: The search for a natural-looking umbilicus that does not compromise the aesthetic results of abdominoplasty began in the 1950s. Since then, the umbilical transposition technique has remained the preferred method among most surgeons. The objective of this study was to describe a new technique for umbilical reconstruction using small X-shaped incisions in the abdominal skin to create 4 small V-shaped flaps. These flaps are then sutured to the aponeurosis using absorbable sutures. METHODS: This technique was used in 401 consecutive abdominoplasties and a survey was conducted to determine the level of patient satisfaction with the aesthetic results of their umbilicus. RESULTS: Remarkably natural-looking umbilici were obtained using the described technique, which was also easily reproducible, did not cause any apparent scars, had a low incidence of complications (9.7%), and resulted in high patient satisfaction with the aesthetic results of the surgery. CONCLUSIONS: The described technique represented a simple, safe, and fast approach to create new, natural-looking umbilici with a low rate of complications.

RESUMO

INTRODUÇÃO: A procura por um umbigo de aspecto natural, que não comprometa o resultado estético das abdominoplastias, começou na década de 1950 e, desde então, a técnica de transposição umbilical é a preferida pela maioria dos cirurgiões. O objetivo deste artigo é descrever nova técnica de reconstrução umbilical, utilizando pequenas incisões em forma de X na pele abdominal, que criam 4 pequenos retalhos em forma de V, os quais são suturados à aponeurose com suturas absorvíveis. MÉTODO: Essa técnica foi utilizada em 401 abdominoplastias consecutivas e foi realizada uma pesquisa para determinar o nível de satisfação dos pacientes com o resultado estético dos umbigos. RESULTADOS: Foram criados umbigos extremamente naturais, de maneira facilmente reprodutível e sem cicatrizes aparentes, com baixa incidência de complicações (9,7%) e alto índice de satisfação dos pacientes. CONCLUSÕES: A técnica utilizada permitiu a criação de um novo umbigo com aparência natural, de maneira simples, segura e rápida, com baixo índice de complicações.

Omphaloplasty: Y/V technique

ABSTRACT

Introduction: Omphaloplasty is a crucial procedure during abdominoplasty surgery. Although it is currently considered an important step, omphaloplasty was not always performed during abdominoplasties, and the umbilicus was sometimes discarded together with the fat flap. Various techniques were used to preserve the umbilicus and underwent modifications with time to allow for an increasingly natural result.
Method: The proposed "Y"/"V" technique consists of modeling the umbilical stump to perfectly fit in the same place where the umbilicus was located. The umbilical island, after being modeled, is sutured, resulting in a "Y"/ "V" image, which gives rise to the name.
Results: A low number of complications (11.34%) were observed when analyzing the postoperative follow-up data. Suture dehiscence, umbilical stenosis, color alterations in the scar, and keloid scars were the complications observed, which were surgically corrected six months postoperatively.
Conclusion: The proposed technique is simple to implement, with low rates of complications and results in a more natural aspect of the neoumbilicus scar. Further studies are required to prove its effectiveness in comparison to the other techniques that are currently in use.

Introdução: A onfaloplastia é um momento crucial durante a cirurgia de abdominoplastia. Apesar de ser considerada uma etapa de grande importância nos dias de hoje, a onfaloplastia não foi sempre utilizada nas abdominoplastias, sendo o umbigo, algumas vezes, descartado junto ao retalho gorduroso. Com a finalidade de preservar a cicatriz umbilical, várias técnicas foram utilizadas e, com o tempo, vêm sofrendo modificações que possibilitam um resultado cada vez mais natural.
Método: A técnica "Y"/"V" proposta consiste em modelar o coto umbilical de modo que este encaixe perfeitamente no mesmo local onde havia a cicatriz umbilical. A ilha umbilical, após ser modelada, é suturada, resultando em uma imagem de "Y"/"V", razão pela qual a técnica recebe este nome.
Resultados: Durante o estudo, foi evidenciado um número baixo de complicações (11,34%) ao analisar o pós-operatório. Deiscência de sutura, estenose umbilical, alterações crômicas na cicatriz e queloide foram as complicações observadas, sendo corrigidas cirurgicamente seis meses após a cirurgia.
Conclusão: A técnica proposta demonstra simples execução, com baixos índices de complicações e aspecto mais natural da cicatriz do neoumbigo. Portanto, tornam-se cada vez mais necessários estudos que a utilizem para comprovar sua eficácia perante às demais técnicas utilizadas atualmente.

ABSTRACT

INTRODUCTION: The minilipoabdominoplastia with umbilicus detachment represents a surgical procedure restricted to cases in which isolated liposuction would cause worsening of sagging skin, while the conventional abdominoplasty would imply excessive skin resection. It allows xyphopubic plication of the rectus abdominis muscle and good resection of the excessive lower abdominal skin. The objective is to provide patients with high umbilucus treated with liposuction, mini-abdominoplasty with umbilucus detachment and reinsertion below, and comparison of two techniques for umbilical reinsertion. METHODS: We included six women aged 32- to 50-year-old and who underwent surgery under spine and general anesthesia from January to June 2013 in the Hospital of the Federal University of Triângulo Mineiro, Uberaba, Minas Gerais, Brazil. RESULTS: After surgery, patients had natural silhouettes of body contours and umbilicus repositioned without external scars. CONCLUSION: The technique used allows treatment of moderate sagging skin of upper and lower abdomen with need of a small incision, the technique provides umbilicus scarring without external scarring and less chance of complications due to the small detachment.

RESUMO

INTRODUÇÃO: A minilipoabdominoplastia com desinserção do umbigo representa uma tática cirúrgica restrita àqueles casos nos quais a lipoaspiração isolada causaria piora da flacidez, enquanto a abdominoplastia convencional implicaria em ressecção exagerada de pele. Permite plicatura xifopúbica dos retos abdominais e boa ressecção do excedente cutâneo abdominal inferior. O objetivo é apresentar pacientes com umbigo alto tratados com lipoaspiração, miniabdominoplastia com desinserção de umbigo e reinserção abaixo, e comparar duas técnicas para reinserção umbilical. MÉTODOS: Foram operadas seis pacientes no período de janeiro a junho de 2013 no Hospital Escola da Universidade Federal do Triângulo Mineiro (UFTM), Uberaba, MG, todas do sexo feminino, com idades de 32 a 50 anos, submetidas à anestesia peridural e geral. RESULTADOS: Pós-operatórios com contornos corporais de silhuetas naturais e cicatrizes umbilicais reposicionadas sem cicatrizes externas. CONCLUSÃO: A técnica empregada permite o tratamento da flacidez moderada do abdome superior e inferior com uma incisão menor, proporciona cicatriz umbilical sem cicatriz externa e menores chances de complicações em virtude do menor descolamento.

Secondary omphaloplasty: description of a new proposal

ABSTRACT

INTRODUCTION: The navel is an essential component of abdominal aesthetics. Omphaloplasty sequelae can compromise the result of an abdominal abdominoplasty. The purpose of this article was to propose an alternative procedure that is simple to perform and well accepted by patients, with favorable cosmetic results. METHODS: We surveyed cases of abdominal dermolipectomy in teaching hospitals and in private practice in the last 5 years (433 cases). RESULTS: We found 30 cases (6.9%) of sequelae distributed as follows: 14 cases (3.2%) in the hypogastric scar and 16 cases in the umbilicus (3.8%). They are characterized as follows: 7 cases (1.6%) of constriction of the navel and 9 cases (2.07%) of scar hypertrophy. CONCLUSION: We analyzed the results of the proposed technique of secondary omphaloplasty in the immediate (up to 30 days) and late (1-year) postoperative periods.

INTRODUÇÃO: O umbigo é um componente essencial à estética do abdome. Sequelas de onfaloplastia podem comprometer o resultado final de uma dermolipectomia abdominal. Objetivo do trabalho é propor um procedimento alternativo, de fácil execução, com resultado estético favorável e bem aceito pelas pacientes. MÉTODOS: Fizemos o levantamento dos casos de dermolipectomia abdominal operados no serviço de ensino e na clínica privada nos últimos cinco anos (433 casos) RESULTADOS: Encontramos 30 casos (6,9%) com sequelas, assim distribuídos: 14 casos (3,2%) na cicatriz hipogástrica, e 16 casos de sequelas de umbigo (3,8%), assim caracterizados: sete casos (1,6%) constrição de umbigo e nove casos (2,07%) hipertrofia cicatricial. CONCLUSÃO: Analisamos os resultados da técnica de onfaloplastia secundária proposta quanto ao pós-operatório imediato (até 30dias) e tardio (um ano).

Dermolipectomy and Correction of Multiple Abdominal Hernias with Abdominal Wall Reinforcement: Case Report

ABSTRACT

We report a case of a patient with a large abdominal alteration in which hernia repair would only bring non-esthetic results, compromising the psychological aspect, and maintaining the skin diseases, as well as the patient's complaints. In the treatment of abdominal hernias, a simple closure of the defect is performed8. In most cases, this simple closure needs to be addressed by a medial below and supra-umbilical incision. This type of surgery is little accepted by patients, since the results are often unsightly scars and dissatisfied patients. The first abdominoplasties were performed in order to correct large ventricular hernias3.

RESUMO

Fazemos relato de caso de uma paciente com uma alteração abdominal grande na qual a correção da hérnia unicamente traria resultados não estéticos, comprometendo o psicológico, e preservaria as doenças de pele, assim como as queixas da paciente. No tratamento das hérnias abdominais é realizado um fechamento simples do defeito8. Esse fechamento simples, na maioria dos casos, precisa ser abordado por uma incisão mediana infra e supraumbilical. Esse tipo de cirurgia é pouco aceitado pelos pacientes, já que muitas vezes o resultado são cicatrizes inestéticas e pacientes insatisfeitos. As primeiras abdominoplastias foram realizadas com o intuito de corrigir grandes hérnias ventrais3.

Vertical relocation of the umbilical scar in abdominoplasty of Bozola and Psillakis Group IV cases - standardization of procedures

ABSTRACT

INTRODUCTION: The navel is the only natural visible scar on the body. It is an essential part of abdominal aesthetics, making umbilicoplasty critical for the success of abdominoplasty. The position and natural contour are the two important factors that are most relevant in evaluating the aesthetic result of the umbilical scar. Classifications and standards are important tools to improve the diagnosis and refine the treatment of abdomen aesthetic disorders. Furthermore, it has facilitated appropriate reproduction of the procedure and served as a basis for comparative studies. OBJECTIVES: This study presents the author's experience with procedure standardization for repositioning and re-implantation of the umbilical scar in abdominoplasty group IV cases. The study demonstrates the navel vertical relocation technique, along with details of the new position, and assesses the quality of results obtained and the degree of patient satisfaction. CASE STUDIES AND METHODS: Twenty female patients underwent the procedure between May 2010 and May 2012. The minimum follow-up period was thirty days and the maximum follow-up period was two years. RESULTS: Most results were considered excellent, with a high level of patient satisfaction and no major complications. There was no need for re-intervention. The technique was shown to be safe, simple and easy to perform. CONCLUSION: The marking and surgical planning, with a proposed maximum end limit for repositioning of the umbilical scar, can be valuable both in aiding the most complex diagnoses of aesthetic abdomen defect types and evaluating the technical aspects of abdominoplasty that are most appropriate for treatment.

INTRODUÇÃO: O umbigo é a única cicatriz natural visível do corpo. É parte essencial da estética abdominal, fato que torna a umbilicoplastia fundamental no sucesso da abdominoplastia. A posição e a naturalidade de contorno são os dois fatores mais relevantes na avaliação do resultado estético da cicatriz umbilical. Classificações e padronizações têm sido ferramentas importantes para aprimoramento do diagnóstico e refinamentos no tratamento dos distúrbios estéticos do abdome. Além disso, têm facilitado a reprodutibilidade dos procedimentos e servido de base para estudos comparativos. OBJETIVOS: Apresentar a experiência do autor com uma padronização tática para o reposicionamento e reimplante da cicatriz umbilical em casos de abdominoplastias do grupo IV. Demonstrar a tática de realocação vertical do umbigo com detalhamento da nova posição, avaliar a qualidade do resultado obtido e o grau de satisfação das pacientes. MÉTODOS: Vinte pacientes, todas do sexo feminino, foram submetidas ao procedimento entre maio de 2010 e maio de 2012. O período mínimo de acompanhamento foi de trinta dias e o máximo, de dois anos. Resultados: A maioria dos resultados foi considerada excelente, atingindo alto nível de satisfação das pacientes, sem apresentar grandes complicações. Não foram necessárias reintervenções. A técnica mostrou-se segura, simples e de fácil execução. CONCLUSÃO: A marcação e o planejamento cirúrgico, com a proposta de um limite caudal máximo para o reposicionamento da cicatriz umbilical, podem ser de grande valia tanto no auxílio aos diagnósticos mais complexos dos tipos de defeitos estéticos do abdome quanto nas indicações das técnicas de abdominoplastias mais adequadas ao tratamento.

Abdominoplasty: neoomphaloplasty without scar or fat excision

ABSTRACT

Introduction: Under the cultural aspect the umbilicus is the only scar accepted in the body, its dysmorphia or absence is noted negatively. Distortions of this scar are still frequent in the abdominoplasties determined by several different aesthetic technical procedures to imitate its natural aspect. The scope of this presentation is another attempt to obtain a neoomphaloplasty based on an easy technical procedure, with a quite natural final aspect and without scars. Methods: During the abdominoplasty, the umbilicus and its stalk are amputated and the dermis of the skin over the future projection is sutured directly to the fascia below, after the local fat excesses be divulsioned to produce the natural navel aspect without scar. Results: Along the last nine years, 319 patients were operated by this technique, with a natural aspect and minimal unaesthetic effects.

RESUMO

Introdução: A cicatriz do umbigo é a única do corpo aceita culturalmente, cuja dismorfia ou ausência chama a atenção negativamente. As distorções dessa cicatriz, que ainda ocorrem com frequência nas abdominoplastias, determinaram a elaboração de inúmeros procedimentos técnicos com a finalidade estética de imitar o seu aspecto natural. Apresentamos mais uma opção sobre neo-onfaloplastia no decurso das plásticas de abdome, mediante uma técnica de fácil execução, com efeito estético próximo ao umbigo natural e sem cicatrizes cutâneas. Método: No decurso da abdominoplastia, o umbigo e o seu pedículo são amputados e a derme da pele na área de projeção do neoumbigo é suturada diretamente no plano da fáscia muscular abaixo, após divulsão tecido adiposo local para produzir a necessária umbilicação sem cicatriz. Resultados: No período de nove anos, foram operados 316 pacientes com a utilização desta técnica, com constante aspecto natural e mínimos efeitos pouco estéticos.

Aesthetic and functional surgery of umbilicus: transumbilicous plication technique

Introduction: The umbilicus is essential for the aesthetic appearance of the abdomen, the vertical, long and deep shape is desired by the patients, however, in traditional aesthetic abdominal surgery, the rectus sheath around umbilicus is not plicated, the ring of umbilicus is not reduced, staying a gap of weakness, and the navel is widened and flat. Objective: The authors present a technique for aesthetic and functional repair of umbilicus, its efficacy and specially the safety, are analyzed. Methods: The technique consists in approach the umbilicus with a "U' shape suture across the umbilicus, plicating the rectus sheath, reducing the ring of the umbilicus, and deepening its base. It's made a retrospective study of 30 female patients, with the age between 26 and 59 years, media of 42.1 years, in the period of one year, all of them were submitted a abdominoplasty, except one which is made treatment of isolated diastasis and hernia. Results: All the patients were satisfied about the size and the shape of the umbilicus, there is no recurrence of diastasis or hernias, no ischemic complications was presented, nor scars contractions. Conclusions: The technique is simple, efficient with good functional and aesthetic results, and overall safe, without ischemic complications.

Introdução: O umbigo é fundamental na estética do abdome, o formato vertical, alongado e fundo é desejado pelas pacientes, entretanto, nas cirurgias estéticas abdominais, a região umbilical e peri-umbilical frequentemente é negligenciada, deixando uma lacuna sem plicatura aponeurótica e sem estreitamento do anel umbilical, resultando em um umbigo alargado e "raso". Objetivo: Os autores apresentam uma técnica de tratamento estético e funcional do umbigo, e avaliam a sua eficácia e, principalmente, a sua segurança. Método: A técnica consiste na abordagem do umbigo com uma sutura em "U", transumbilical, que aproxima a aponeurose, fecha o anel umbilical e aprofunda a sua base. O estudo é retrospectivo do período de um ano, com 30 pacientes de sexo feminino, com idade entre 26 e 59 anos, com média de 42,1 anos, todas submetidas à abdominoplastia, exceto uma na qual foi corrigido isoladamente o umbigo inestético. Resultados: Todas as pacientes ficaram satisfeitas com o tamanho e o formato estético do umbigo, não houve recidiva da diástase da aponeurose nem da hérnia umbilical, não houve complicações isquêmicas, nem de contraturas cicatriciais. Conclusão: A técnica é simples, eficaz no tratamento funcional e estético, e a sutura transumbilical provou ser segura, sem complicações isquêmicas.

Neoonphaloplasty in abdominal dermolipectomy: double

ABSTRACT

Introduction: The goal of this scientific research is demonstrate possibility of umbilicus reconstruction or re-implantation during the abdominal dermolipectomy. Method: Isolating the navel before the dermolipectomy with a triangular incision and re-sitting it with "V" incision in the final abdominal flap and the creation of a "V" double scar. Results: A perfect approximation of the borders of the internal umbilicus scars and obvious improved aesthetic were obtained. Conclusion: The outcomes of this technique demonstrated harmony in the results and prevented the umbilical retraction, very common on the other techniques.

RESUMO

Introdução: Este trabalho tem como finalidade a reconstrução ou o reimplante do umbigo durante a cirurgia de dermolipectomia abdominal. Método: Através do isolamento do umbigo antes da dermolipectomia, realiza-se uma incisão triangular e reimplante deste com incisão em "V" no retalho abdominal final e confecção de uma peça em "V" do umbigo anteriormente isolado. Resultados: Observa-se perfeita coaptação destas bordas confeccionadas, situando-se a cicatriz umbilical na parte interna do neoumbigo, permitindo, assim, melhora estética do umbigo. Conclusão: Observou-se que, em sua totalidade, o resultado demonstrou harmonia e evitou-se a retração umbilical constritiva pós-cicatricial tão comum nas técnicas circulares ou em fenda.

Onphaloplasty: "infinity" technique

ABSTRACT

Introduction: Omphaloplasty is considered one of the most stigmas in abdominoplasty. Given this, many techniques were developed in order to achieve more natural and hidden scars. These techniques were created from the basic principle in a way that is natural and to prevent scar retraction of the circular forms. Methods: The authors present a technique that offers an oblique shape, broken by two small side flaps in order to reduce scar retraction obtained by drawing in the longitudinal direction of the infinity symbol. Results: The navels were evaluated for location, the final form (oblique) and size. Among the complications were stenosis (2.5%), partial dehiscence (2%) and keloids (1%). The aesthetic result was satisfactory for patients in 91% of cases, the patient's quality of healing, keloid or hypertrophic scars were the major causes of dissatisfaction. Conclusion: The use of the "infinity" technique was effective in the treatment of the umbilicus in complete abdominoplasty, offering more choice and an easy implementation for omphaloplasty.

Introdução: A onfaloplastia é considerada um dos procedimentos com mais estigmas na abdominoplastia. Diante disso, diversas técnicas foram desenvolvidas com o objetivo de alcançar resultados mais naturais e com cicatrizes ocultas. Essas técnicas foram criadas a partir do princípio básico de uma forma que seja natural e que impeça a retração cicatricial das formas circulares. Método: Os autores apresentam uma técnica que oferece uma forma oblíqua quebrada por dois pequenos retalhos laterais com o intuito de reduzir a retração cicatricial, obtidos pelo desenho no sentido longitudinal do símbolo do infinito. Resultados: Os umbigos foram avaliados quanto à localização, forma final (oblíqua) e o tamanho. Entre as complicações houve estenoses (2,5%), deiscências parciais (2%) e queloides (1%). O resultado estético foi satisfatório para os pacientes em 91% dos casos, a qualidade da cicatrização do paciente, queloides ou cicatrizes hipercrômicas foram as maiores causas de insatisfação. Conclusão: A utilização da técnica do "infinito" se mostrou efetiva no tratamento do umbigo em abdominoplastias completas, oferecendo mais uma opção e de fácil execução para as onfaloplastias.

Umbilicaplasty: experience with the skin mini-circle technique

ABSTRACT

Introduction: The umbilicaplasty is another important technical detail in the lower pubic transversal abdominoplasty. The described and published techniques have similar scope to preserve the navel pedicle but frequently leaving visible and even hypertrophic scars. From the stenotic umbilicus to the reverse open bally button bigger than the patterns, call the attention negatively base on the narrow safe limits to obtain the desired aesthetic aspects. Methods: Seventy two female patients submitted to abdominoplasty under the same umbilicaplasty procedures were followed in a period of 26 months. After the umbilical pedicle dissection from the skin flap a 5 mm mini skin circle is preserved on its free extremity. After the abdominoplasty steps be partially concluded the umbilicaplasty is performed. A small fat cone from the skin flap is resected below the umbilicus final position, a 7 mm transversal incision is done in the skin flap to extrude and suture with isolated non absorbable stitches the small skin circle of the umbilicus. Results: The obtained results were considered satisfactory by the surgical team and the patients, without post operative problems that had require special care. Conclusion: It has been an easy procedure without secondary aesthetic and technical problems and lack of visible scars.

Introdução: A umbilicoplastia compreende mais um detalhe técnico importante no decurso das abdominoplastias tipo incisão pubiana transversal baixa. As técnicas descritas e publicadas têm o mesmo princípio de preservar o umbigo original, deixando com frequência cicatrizes geralmente visíveis, retráteis e mesmo hipertróficas. Paralelamente, a estenose cicatricial ou o inverso, uma área umbilical maior do que a dos padrões estéticos, chama a atenção negativamente, decorrente de uma estreita margem de segurança para se obter efeitos estéticos desejados. Método: Foram revisados 72 pacientes do sexo feminino, operadas de abdominoplastia no período de 26 meses, com os mesmos detalhes técnicos de umbilicoplastia. Após da dissecação do pedículo umbilical, um minicírculo de pele com cerca de 5 mm de diâmetro é preservado na sua extremidade livre. Após as etapas da abdominoplastia, a reposição do umbigo é realizada mediante a exérese de um cone de gordura no retalho do abdome situado na projeção do mesmo e uma incisão cutânea transversal de 7 mm faz exteriorizar o círculo de pele que é suturado na pele mediantes pontos isolados de fio não absorvível. Resultados: Os resultados obtidos foram considerados satisfatórios pela equipe cirúrgica e pelas pacientes, sem presença de problemas pós-operatórios que exigissem cuidados especiais. Conclusão: A técnica de umbilicoplastia apresentada tem sido um procedimento de fácil execução, sem problemas secundários de caráter técnico ou estético e pela ausência de cicatrizes visíveis.

Anatomical reconstruction of the umbilicus

ABSTRACT

BACKGROUND: The absence of the umbilicus causes significant changes in the appearance of the abdomen, thus making it an essential part of anatomy. The reconstruction procedures should account of the anatomical position to ensure a natural appearance after treatment. The new umbilicus should have a good shape and appropriate position, but should not have an unaesthetic appearance, stenosis, or enlargement. METHODS: This study included 162 patients who previously underwent open gastroplasty for the treatment of morbid obesity and 2 patients who underwent laparoscopic surgery. We used 2 parallel rectangular flaps measuring 2 × 1.5 cm that were sutured together and fixed to the aponeurosis of the rectus abdominis muscles. The flaps exhibited a curvilinear-shaped skin transition. The attachment flaps were created with 2 parallel points that reached the dermis on both sides of the flap and aponeurosis. Thus, the union of 2 flaps was obtained, which were slightly compressed at their distal portion. Thereafter, the fat tissue around the new umbilicus was approximated. RESULTS: The technique described produced all anatomical units including a base, groove, and impeller as well as adequate depth of the umbilical scar in all cases. There were no cases of umbilical stenosis, hypertrophic scars, or keloid scars in the new umbilici. CONCLUSIONS: This technique for the reconstruction of the umbilicus takes into account the anatomical units and provides a very natural appearance, eventually resulting in a slight excess of skin in the upper portion, thus giving a graceful appearance.

RESUMO

INTRODUÇÃO: A ausência da cicatriz umbilical causa alteração importante na forma do abdome, tornando-se, desse modo, imprescindível na anatomia do mesmo. Sua reconstrução deve seguir seus aspectos anatômicos, conferindo aspecto natural ao final do tratamento. A nova cicatriz umbilical deve ter boa forma, bom posicionamento, não apresentar cicatriz estigmatizante, estenose ou alargamentos, e ter aspecto natural. MÉTODO: Foram operados 162 pacientes submetidos previamente a gastroplastia aberta para tratamento de obesidade mórbida e 2 pacientes submetidos a cirurgia videolaparoscópica. Foram utilizados 2 retalhos retangulares paralelos, medindo 2 cm x 1,5 cm, que foram suturados entre si e fixados à aponeurose dos músculos reto abdominais. Os retalhos apresentavam a transição com a pele em forma curvilínea. A fixação dos retalhos foi realizada com 2 pontos paralelos, que atingiam a derme do retalho de um lado, a derme do retalho do lado oposto e a aponeurose. Desse modo, foi obtida a união dos 2 retalhos, comprimindo-se levemente a porção distal dos mesmos. Em seguida, foi realizada aproximação do tecido gorduroso ao redor da nova cicatriz umbilical. RESULTADOS: Com a técnica apresentada, obteve-se a formação de todas as unidades anatômicas (mamelão, sulco e rodete), além de profundidade adequada em todas as cicatrizes umbilicais reconstruídas. Não foi observado nenhum caso de estenose umbilical, cicatriz hipertrófica ou queloide nas novas cicatrizes umbilicais. CONCLUSÕES: A técnica apresentada para reconstrução da cicatriz umbilical, respeitando as unidades anatômicas, proporciona forma bem natural e, com o passar do tempo, ocorre uma sobra de pele em sua porção superior, conferindo graciosidade à mesma.

Triangular umbilicoplasty with skin flap

ABSTRACT

Background: Umbilicoplasty techniques vary greatly, in both the manner in which the incision the umbilical scar is incised, as well as the manner in which the skin of the abdominal flap is opened and repaired at the aponeurosis and/or the umbilical stump. As the postoperative appearance of the umbilical scar is aesthetically unsatisfying, the authors sought to develop a new technique aimed at providing patients with a greater degree of aesthetic and postoperative satisfaction. Methods: The abdominoplasties included in this study were performed in 194 patients at Clínica Valle Pereira (Florianópolis, SC) between February 2009 and January 2011. All patients underwent conventional abdominoplasties and triangular umbilicoplasties with skin flaps. Results: Only 8 (4.13%) patients had mild complications. There were no severe complications. Positive satisfaction was reported by patients in 188 (96.91%) cases and by surgeons in 186 (95.88%) cases. Conclusions: The technique described in this study demonstrates versatility, simplicity in application, and reproducibility, bringing greater harmony in body contouring and improved appearance of the umbilical scar, a major stigma of abdominoplasty.

Introdução: A técnica de umbilicoplastia varia muito, tanto na forma como é incisada a cicatriz umbilical, quanto na abertura cutânea no retalho abdominal e sua fixação na aponeurose e/ou coto umbilical. Descontentes com o aspecto pós-operatório da cicatriz umbilical, os autores viram a necessidade de desenvolver uma nova técnica, com o objetivo de proporcionar aos pacientes maior grau de naturalidade e satisfação pós-operatória. Métodos: Foram incluídas neste estudo as abdominoplastias realizadas na Clínica Valle Pereira (Florianópolis, SC), no período compreendido entre fevereiro de 2009 e janeiro de 2011, totalizando 194 pacientes. Todos os pacientes foram submetidos a abdominoplastia convencional e umbilicoplastia triangular com retalho dérmico. Resultados: Apenas 8 (4,13%) pacientes apresentaram complicações leves. Não houve complicações graves. A satisfação dos pacientes foi positiva em 188 (96,91%) casos; entre os cirurgiões, a satisfação foi positiva em 186 (95,88%) casos. Conclusões: A técnica demonstra versatilidade, facilidade de execução e reprodutibilidade, proporcionando harmonia no contorno corporal e naturalidade ao principal estigma da abdominoplastia, a cicatriz umbilical.

Umbilical base plicature: technique proposition for treatment of the bulge aspect and to avoid stigmas after abdominoplasty

ABSTRACT

Introduction: The umbilicoplasty (onfaloplasty) is a main surgical step during abdominoplasty. Esthetic concerns and important reference to this area in the post surgical time are assessed, by the patient and the surgeon. The bulge aspect of the umbilical base, similar to umbilical hernias, is considered unattractive. This situation could exist in the pre surgical evaluation or it could be an unfavorable outcome. Purpose: To present a complementary technique to be associated during umbilicoplasty, using the plicature of the umbilical base to aponeurosis, with the objective of treat a pre existent bulge of umbilicus or to avoid this undesirable post surgical alteration. Methods: Twenty female patients were submitted to conventional abdominoplasty with umbilical base plicature during a period of 12 months. In 10 cases there were a bulge aspect in pre surgical. None of the patients had umbilical hernia. The follow up has duration of 6 - 24 months. Results: There were good results in all cases, with treatment of the deformity when it was present, with no recidive or sequela. There were no cases of local ischemia or dehiscence. In the cases without previous deformity there weren't alterations after surgery. Conclusions: The plicature of the umbilical base is a technique that could be used with success in the treatment of an existent umbilical bulge and to avoid this undesirable situation after abdominoplasty.

Introdução: A umbilicoplastia (onfaloplastia) corresponde a um tempo fundamental durante a realização das abdominoplastias. Considerações estéticas e importante destaque no período pós-operatório, tanto pela paciente quanto pelo cirurgião, são atribuídas a esta região. A protrusão da base umbilical, similar ao aspecto de hérnias, é considerada aspecto não atrativo. Essa situação pode existir no pré-operatório ou evoluir como resultado pós-operatório desfavorável. Objetivo: Apresentar uma técnica complementar a ser associada durante a realização da umbilicoplastia, com a realização da plicatura da base da cicatriz umbilical à aponeurose, objetivando tratar protrusões existentes ou evitar esta indesejável alteração no pós-operatório. Método: Vinte pacientes do sexo feminino foram submetidas à abdominoplastia convencional com realização da plicatura da base umbilical, durante período de 12 meses. Em 10 casos, havia protrusão no pré-operatório. Nenhuma das pacientes apresentava hérnia nessa topografia. O acompanhamento pós-operatório variou de 6 a 24 meses. Resultados: Foram obtidos bons resultados em todos os casos, com tratamento da deformidade proposta quando presente, sem recidivas ou sequelas locais. Não houve casos de isquemia local ou deiscência. Nos casos em que não se evidenciava nenhuma deformidade prévia, não houve alterações no pós-operatório. Conclusão: A plicatura da base umbilical é uma técnica que pode ser utilizada com sucesso para tratamento da protrusão já estabelecida, bem como para evitar que essa situação indesejada possa ocorrer após abdominoplastias.

Indications of neo-omphaloplasty in patients undergoing abdominoplasty

ABSTRACT

Introduction: The navel is the only natural scar accepted by the human being. Under normal conditions, the navel is located in the midline of the anterior abdominal wall, about 18 to 23 cm from the vulvar commissure, varying from 4 cm up to 2 cm below a horizontal line passing through the two anterior superior iliac spines, it has a variable depression and it is circular or discreetly elongated.
Method: A protocol with six indications for neo-omphaloplasty was performed in patients submitted to surgical abdominoplasty procedure. Presentation of the technique: The surgical technique of neo-omphaloplasty with incision in X was performed, as described by Dr. Ticiano Cesar Teixeira.
Results: In general, good results were obtained, both in the opinion of the plastic surgeons of the service and the patients.
Conclusions: We believe that neo-onfaloplasty should be performed in selected cases.

Introdução: O umbigo é a única cicatriz natural aceita pelo ser humano. Em condições normais, o umbigo localiza-se na linha média da parede abdominal anterior, cerca de 18 a 23 cm da comissura vulvar, com variação de 4 cm acima a 2 cm abaixo de uma linha horizontal que passa pelas duas espinhas ilíacas anterossuperiores, apresenta uma depressão variável e tem forma circular ou discretamente alongada.
Método: Realização de um protocolo com seis indicações para neo-onfaloplastia em pacientes submetidos a procedimento cirúrgico de abdominoplastia. Apresentação da técnica: Foi realizada a técnica cirúrgica de neo-onfaloplastia com incisão em X, descrita pelo Dr. Ticiano Cesar Teixeira.
Resultados: De maneira geral, foram obtidos bons resultados, tanto na opinião dos cirurgiões plásticos do serviço como dos pacientes.
Conclusão: Consideramos que a realização de neo-onfaloplastia deve ser indicada em casos selecionados.

RESUMO

Objetivo: Relatar um caso de onfaloplastia.
Relato de Caso: Paciente do gênero feminino, J.P.O., de 30 anos, atendida no Serviço de Cirurgia Plástica Dr. Ewaldo Bolivar, com queixa de "umbigo grande".
Conclusão: Cirurgia realizada no Santos Day Hospital, utilizando técnica em retalho semicircular, evoluindo com satisfação o total da paciente.
