ABSTRACT

Introduction: The identification of complications of primary palatoplasty may vary among professionals from different areas of health due to the lack of standardization of the fistula classification. This study aimed to verify the consensus among professionals of plastic surgery (PC) and Speech-Language Pathology (SLP), regarding the occurrence of fistula, according to what was reported in the same craniofacial service.
Methods: Analysis of the chart's records of the areas of the PC and SLP of 466 patients with cleft lip and palate was performed about the presence and location of fistula about the presence and location of fistulas, as reported in 466 medical records of patients with a history of unilateral cleft lip and palate. To compare the findings between both areas, a gold standard classification for the occurrence of fistula (GSF) was established by an experienced plastic surgeon.
Results:
The PC area reported that 25% of the 466 patients had a fistula compared to the 37% reported by the SLP, while the GSF indicated fistula in 35% of the cases. The Kappa statistic reveals regular agreement between GSF and PC (r = 0.32) and substantial agreement between GSF and SLP (r = 0.63).
Conclusion: There was a discrepancy between the areas of Speech-Language Pathology and plastic surgery regarding the occurrence and location of the fistula after primary palatoplasty in the same craniofacial center. The data indicates the need to create and implement a standardized fistula classification system. In this way, craniofacial teams can use it effectively, taking advantage of the scientific evidence that emerges from the results of cleft lip and palate treatment.

ABSTRACT

The purpose of this paper is to highlight the importance of multidisciplinary assistance to treat patients with cleft lip and/or palate, to evaluate the treatment protocols used by the main centers in Brazil, and to compare them to the ones used by the main centers in other countries. The medical literature regarding congenital malformation was reviewed. A treatment protocol questionnaire was elaborated and sent to all plastic surgery centers in Brazil that are recognized by the Brazilian Plastic Surgery Society and to other centers known as active in the treatment of cleft lip and/or palate. The data about cleft lip and/or palate surgeries peiformed in the last 5 years were obtained from the Brazilian Health Ministry files. Different treatment protocols have been used in the various centers. There is a consensus about the most applied surgical techniques, the best age group to perform the sugeries)and the necessity of a multidisciplinary team. The number of surgeries increased from 9,696 in 1995 to 21,022 in 1999. Eighty-three percent of the surgeries performcd in 1999 were made in the State of São Paulo. There is no consensus about treatment protocols for cleft patients in Brazil. However, the best results came from centers that adopt a treatment protocol, have well-trained surgeons and perform long-term follow-up.

ABSTRACT

INTRODUCTION: In the present study, we aimed to outline the overall practice profile of plastic surgeons in the state of Paraná. It represents an initial project that aimed not to provide generalizations but to understand how plastic surgeons think and their professional aspirations, goals, and performance. METHODS: A survey was conducted using a questionnaire consisting of 37 multiple-choice questions, mostly focusing on the current job market for plastic surgeons. The link to the online questionnaire was e-mailed to 263 members of the Brazilian Society of Plastic Surgery-Regional Paraná (SBCP-PR), which was also accessible via the SBCP-PR website, through a banner on its home page. The overall time to complete the questionnaire ranged from 3 to 5 minutes. RESULTS: The response rate was 40.5%. Most interviewees operated only in one city, and more than half provided services only through health-care insurance plans and private practice. Half of the participants considered the market as extremely competitive and searched for several alternatives for self-maintenance, including increasing installment payment terms, lowering prices, and working at other locations. A significant proportion of the respondents believed that they lost more than 30 patients owing to pricing in the previous year and performed 10-20 surgeries per month on average. Most of the surgeons did not operate with an auxiliary plastic surgeon because this would increase their operational cost or because technologists could perform this function. Most participants believe that the number of trained residents in the state of Paraná is excessive. CONCLUSIONS: In Paraná, plastic surgeons are concerned about the job market and therefore search for alternatives to remain competitive.

ABSTRACT

Introduction: The growing number of patients with massive weight loss after bariatric surgery is correlated with the demand for body contouring surgery. Such procedures reduce physical and psychological complaints, positively influencing the quality of life of these people. However, there is little data on the response of services that offer surgical treatment for morbid obesity to this need. The study aims to measure the prevalence of body contouring surgery between 2015 and 2018, in patients previously underwent on a bariatric surgery, in 2014 and 2015, at a University Hospital.
Methods: Research in the hospital information system and medical records in order to assess the institutional prevalence of post-bariatric body contouring surgery. Patients who did not undergo both surgeries in our Service, those who underwent these surgical procedures in other years, as well as those with incomplete medical records were excluded.
Results: Bariatric surgeries were performed in 208 patients. Of these, 11% (n=23) underwent 27 body contouring surgeries, with abdominal dermolipectomy (n=16) being the most frequently performed. The performance of more than one procedure to correct body deformity occurred in 13% (n=3) of patients. The mean age of patients undergoing body contouring surgery was 37 years, the majority was female (96%, n=22).
Conclusion: Body contouring surgery is an important step in the treatment of morbid obesity and has a restorative feature. There is a huge lack of this therapy, which irreparably compromises the results obtained by bariatric surgery.

ABSTRACT

Introduction: Head and neck neoplasms often affect fundamental functions, such as swallowing, speech, eating, and socializing. Evaluating their treatment should consider the physician's opinion and the patient's perspective. This difficulty in assessing the success of treatment led to the development of the FACE-Q Head and Neck Cancer Module, a questionnaire of patient-reported outcomes that measure the appearance, facial function, quality of life, and experience of care to head and neck neoplasms. The objective is to translation, cultural adaptation, and linguistic validation of the FACE-Q Head and Neck Cancer questionnaire for Brazilian Portuguese.
Methods: The translation, cultural adaptation, and linguistic validation of the full questionnaire took place in four stages, using official guidelines from the World Health Organization and the International Society of Pharmacoeconomics and Outcomes Research.
Results: A semantic, idiomatic, and conceptually equivalent Brazilian Portuguese version was achieved through a linguistically validated translation of the English FACE-Q Head and Neck Cancer module.
Conclusion: The Brazilian Portuguese version presents a version equivalent to the original English instrument, which can be used as a critical patient-reported outcome assessment.

ABSTRACT

BACKGROUND: The Internet has become one of the most important sources of health information. However, few studies have assessed the characteristics of Internet usage by patients or how the information obtained affects decisions to undergo surgery. This study describes the characteristics of Internet usage by patients who intended to undergo plastic surgery at the Unified Health System (SUS) and private networks and how it affected their confidence. METHODS: A cross-sectional observational study was performed using data collected by 200 surveys filled out by patients before the operation. One hundred surveys were sent to patients attending the outpatient clinic of Plastic Surgery of Professor Edgard Santos University Hospital (SUS), and the remaining 100 to patients treated in private health clinics in Salvador, BA. RESULTS: The frequency of Internet use to seek health information on plastic surgery differed significantly between the SUS (n = 24; 24%) and private health services (n = 64; 64%). Most of the patients (n = 48; 54.5%) visited 1 to 3 sites. The websites of plastic surgery centers were the most sought (n = 74; 84.1%), followed by the Brazilian Society of Plastic Surgery website. Half of the patients reported the existence of contradictory information. Moreover, 71 (80.7%) of the respondents reported feeling that undergoing surgery was safe. CONCLUSIONS: The Internet was widely used to seek information on plastic surgery. Although visited sites contained contradictory information, most patients felt safe before undergoing their desired surgery.

ABSTRACT

Introduction: Keloids are one of the most aggressive spectrums of healing disorders. They have a unique pathophysiology and multiple specific genetic and cellular factors, which have not yet been fully elucidated. So far, literature reviews have found the influence of genetics, injury site, and ethnicity on the incidence and rate of recurrence. Furthermore, the need to associate an adjuvant method with surgical excision has already been verified, but the best therapy has yet to be defined.
Method: A retrospective analysis of medical records was carried out to assess the profile of patients who underwent postoperative radiotherapy with an electron beam to treat keloids at the Hospital das Clínicas da Faculdade de Medicina de Botucatu between 2015 and 2019.
Results: Data from 131 patients were evaluated, with 269 keloid scars treated. The average duration of treatment was 51 days, and the number of sessions was 17. White patients were predominant (78%) and women (70%), with surgical incision being the main cause of formation (49%) and ear the most identified location (33%). Women were more likely to complete the proposed treatment (p=0.04), while non-literates completed less than those who had at least completed primary or secondary education (p<0.0001).
Conclusion: Postoperative radiotherapy for keloid scars is a well-established treatment in the literature and an important tool for plastic surgeons. Knowing the profile of patients who need this therapy is essential to create methods that improve adherence and results.

Keywords: Keloid; Scar; Radiotherapy, adjuvant; Reconstructive surgical procedures; Medical records

ABSTRACT

The attention and enthusiasm of doctors in the pre- and postoperative period is of extreme importance in maintaining good doctor/patient relationships. An adequate organization and documentary record contributes to achieving a good relationship and ensures an important legal tool for physicians. With current technological advances, the electronic medical record is a secure and democratic way to deal with this information. The government has sought to implement this system in public institutions; however, results are still modest, perhaps mainly due to the lack of resources for investment in this area. In light of this, and given the relevance of practical, informative, and dynamic medical records, we aim to present our experience with the use of a complementary digital resource that is commonly associated with medical records and uses a free of cost digital platform for storing data in the "cloud". This system can provide additional information about each patient, including outpatient follow-up, as well as photographic records of the pre-, intra-, and post-operative periods, and also facilitates quick, synchronized, and remote access through the internet. The system generates optimization of resources, surgical planning, and improvement in patient/ doctor relationships. It also leads to greater integration of the medical team, particularly in the discussion of cases and distribution of surgeries by preceptors and residents. Thus, it is an alternative resource to improve medical charts with important data regarding the performance of medical teams, paying special attention to the peculiarities of plastic surgery.

ABSTRACT

Introduction: Telemedicine refers to providing medical services through information technology and communication between patients and service providers. Plastic surgery is a field especially favorable to the application of telemedicine since visual inspection of the patient guides both diagnosis and clinical follow-up.
Methods: An integrative review was performed in the articles published in telemedicine in plastic surgery in the PubMed database, using the terms MeSH: "telemedicine" AND "plastic surgery." After tracking and applying eligibility criteria, the remaining articles' full text were digitized, with their data extracted for detailed analysis.
Results: A total of 134 articles were identified, all in English, in the initial search for PubMed. After screening and application of the exclusion criteria, 12 studies were included for the integrative review. The articles found in this review demonstrated benefits in the use of telemedicine applied to plastic surgery. Especially in the postoperative follow-up of patients, in the reduction or extinction of the need for face-to-face visits, in the satisfaction of patients in the consultations carried out by telemedicine, in the reduction of financial costs and response time for referrals, in the diagnostic accuracy similar to the face-to-face assessment and in improving access to specialized care in remote places, both geographically and socially unfavorable. Four articles discussed barriers and limitations, observed or potential, to the use of telemedicine in plastic surgery, such as data security, legislation, costs and perception of negative influence on doctor-patient communication.
Conclusion: The analyzed studies show an expressive gain in knowledge about telemedicine applied to plastic surgery in recent years.

Keywords: Telemedicine; Plastic surgery; Telesurgery; Patient care; Review.

ABSTRACT

Introduction: patient safety has become an increasingly present topic in health research. Plastic surgery is a specialty in evidence, and it is necessary to adapt patient safety to its particularities.
Methods: Systematic review to investigate actions related to patient safety in plastic surgery. MEDLINE and SCIELO were chosen to locate the studies. The descriptors: "patient safety" and "plastic surgery" were used in the MEDLINE database. In SCIELO, the descriptors: "segurança do paciente" e "cirurgia plástica" were used. In both cases, the publication period was between 2012-2018, totaling 15 articles.
Results: The countries that published the most on the subject were the United States and Brazil. The most frequent concern was safety related to the training of plastic surgery residents. Tools like the checklist have also been used to improve security. Another concern that requires more study will be if the weekends have higher complications with surgeries performed during the week. However, well-formulated medical records, as well as the Informed Consent Form (ICT), appear to have a more solid basis in patient safety. Pre-anesthetic consultation also seems to favor patient safety. Furthermore, finally, the use of WhatsApp seems to be a safe tool, and that improves the care provided by the medical team.
Conclusion: The need for more in-depth studies on this topic is emphasized, considering that a systematized protocol was not found.

ABSTRACT

INTRODUCTION Besides the diverse situations in which plastic surgery is performed, the procedure has an aesthetic character associated with societal issues, related to the ideals of beauty and to the search for body perfection. Currently, specific body measurements are increasingly being given importance, and shown by the media as being associated with success at diverse social levels. The objective of this study was to identify the concept of plastic surgery, as reported by the print media in Brazil and available in the virtual environment, by analyzing and describing the type of information that is reported to the general public. METHOD: For the data collection, a systematic search was performed on all news articles published by the print media in Brazil and available in the virtual environment. For the search, the descriptor used was "cirurgia plástica" ("plastic surgery" in English) in Portal Interjornal®. All news articles containing the term "plastic surgery" published between January 2008 and December 2011 were included in the study. The selected news articles were read and subjected to a previously established protocol. RESULTS: A total of 1983 news articles were analyzed. From this total, 1367 (68.94%) were classified as aesthetic related, 423 (21.33%) as being associated with reparative surgery, 47 (2.37%) as aesthetic and reparative, and 146 (7.36%) as undetermined. CONCLUSION: The concept of plastic surgery reported by the media was generally associated with aesthetic surgery. Both the content and the quality of information provided by the media are not enough to adequately clarify to the public the real role of the specialty and the care that should be taken when undergoing any type of surgical procedure.

ABSTRACT

Introduction: This study aims to understand the factors that determine the choice of a plastic surgeon from the patient's perspective. This is a pilot project, which should be broadened and deepened by other studies.
Method: A survey was devised in the form of a questionnaire with 22 multiple choice questions. The average response time was 8 minutes. The questions addressed various aspects, such as the recommendation, training, and accreditation of the surgeon; perception of the first consultation; and the clinic. There was no possibility of identifying the patient or surgeon; hence, anonymity was guaranteed.
Results: The response rate was 86.66%. The majority (92.22%) of the respondents were female, with a mean age of 35 years. Most had completed secondary and higher education, with an average monthly family income of R$ 2,000 to 10,000. Almost 40% did not know the accreditation of the surgeon chosen and 33.7% did not know the length of training the surgeon had undergone. The majority (81.6%) believed they had paid an average amount for their surgery and half did not research the surgeon's online profile. The majority (67%) had not undergone any previous plastic surgery. A list of 10 items in descending order of importance was presented.
Conclusions: The determining factors for the choice of the plastic surgeon in this sample, in descending order were: 1) Recommendation, 2) Accreditation, and 3) First consultation. Price was not the most important factor and online presence was one of the last items cited. A significant proportion of respondents did not know either the accreditation level or the length of training of their surgeon.
