Primary palatoplasty using the von Langenbeck technique: surgical experience and aesthetic results of 278 cases

ABSTRACT

Introduction: Palatoplasty with elevated bilateral mucoperiosteal flaps using the von Langenbeck technique associated with intravelar veloplasty is a common procedure with low rates of oronasal fistula (ONF) and velopharyngeal insufficiency. The objective is to present the author's surgical experience and the incidence of ONF among 278 patients who underwent primary palatoplasty using the von Langenbeck technique associated with intravelar veloplasty.
Methods: This retrospective study analyzed the medical records of 278 patients who underwent primary palatoplasty at the Mário Covas Treatment Center for Craniofacial Malformations of the Guilherme Álvaro Hospital located in the municipality of Santos, São Paulo, Brazil, between May 2010 and May 2018.
Results: A total of 278 primary palatoplasty procedures were performed; of them, 225 (80.9%) were performed in two surgical stages and 53 (19.1%) in one surgical stage. The study population included 182 men (65.5%) and 96 women (34.5%). The prevalence of left and bilateral cleft lip and palate was 26.3% and 27%, respectively, and the prevalence of bilateral cleft palate, and right cleft lip and palate was 37.4% and 7.6%, respectively. Sixty-one patients had ONF (21.94%), the incidence of which decreased progressively throughout the study period.
Conclusion: Primary palatoplasty, using the von Langenbeck technique associated with intravelar veloplasty, is reproducible when performed in one or two surgical stages, and considered safe when the learning curve is reached with a complication rate similar to those in the literature.

RESUMO

Introdução: A palatoplastia com elevação de retalhos mucoperiostais bipediculados pela técnica de Von Langenbeck associada a veloplastia intravelar é técnica mais utilizadas na atualidade apresentando na literatura baixa taxa de fístula oronasal e de insuficiência velofaríngea. O objetivo é apresentar a experiência acumulada do autor e avaliar a incidência de fístula oronasal após 278 casos de palatoplastia primária, pela técnica de Von Langenbeck associada a veloplastia intravelar.
Métodos: Estudo retrospectivo de 278 prontuários de pacientes submetidos à palatoplastia primária no Centro de Tratamento de Malformações Craniofaciais Mário Covas - Hospital Guilherme Álvaro - Santos/SP, entre de maio de 2010 a maio de 2018.
Resultados: 278 procedimentos de palatoplastia primária pela técnica relatada, 225 (80,9%) em duas etapas cirúrgicas e 53 (19,1%) em única etapa. Masculino 182 (65,5%) e feminino 96 (34,5%). Fissuras labiopalatais esquerda e bilaterais (26,3% e 27%, respetivamente). As fissuras palatais completas corresponderam a 37,4% e a fissura labiopalatal direita com 7,6%. 61 pacientes apresentaram fístula oronasal (21,94%) observando-se uma diminuição progressiva da incidência em cada período.
Conclusão: A palatoplastia primária pela técnica de Von Langenbeck associada à veloplastia intravelar é uma técnica reprodutível em uma ou duas etapas cirúrgicas e pode ser considerada segura quando alcançada uma adequada curva de aprendizado apresentando um índice de complicações acorde com a literatura mundial.

Breast reconstruction with reverse mini abdominoplasty

ABSTRACT

INTRODUCTION: Mastectomy is very traumatic for women, and breast reconstruction is an essential procedure to improve their psychosocial well-being and quality of life. Most plastic surgeons do not perform breast reconstruction. This practice requires advanced training with a long learning curve. Breast reconstruction using excess flaps in reverse abdominoplasty can be a practical option and of great applicability in carefully selected patients. METHODS: From August 2014 to June 2016, 12 patients underwent breast reconstruction using the reverse mini abdominoplasty technique. Symmetrization was performed in six patients who underwent unilateral reconstruction, with one case reconstructed bilaterally. The implants were placed in the supra-muscular plane and in a submuscular position in seven and five patients, respectively. RESULTS: There was one case each of seroma, epitheliolysis, and partial superficial necrosis of the superficial flap, all with favorable evolution. Only one patient underwent implant extrusion. CONCLUSION: Breast reconstruction using excess flaps in reverse mini abdominoplasty associated with implant placement is a good option for breast reconstruction in selected patients with a low complication rate. Patients with flaccidity and lipodystrophy in the upper abdomen are the ideal candidates for the use of this technique.

RESUMO

INTRODUÇÃO: A mastectomia é muito traumática para a mulher, sendo a reconstrução mamária um procedimento essencial para melhorar o bem-estar psicossocial e a qualidade de vida destas pacientes. A maioria dos cirurgiões plásticos não realiza reconstrução mamária. Sua prática requer um treinamento avançado com longa curva de aprendizado. A reconstrução mamária, utilizando o retalho excedente da abdominoplastia reversa, pode ser uma opção prática e de muita aplicabilidade em pacientes cuidadosamente selecionadas. MÉTODOS: No período de agosto de 2014 a junho de 2016, doze pacientes foram submetidas à reconstrução mamária, com a técnica de miniabdominoplastia reversa. A simetrização foi realizada em seis pacientes que fizeram reconstrução unilateral, sendo um caso reconstruído bilateralmente. Em sete pacientes, os implantes foram colocados em plano supra muscular, e em cinco, em posição sub submuscular. RESULTADOS: Uma paciente apresentou seroma, uma outra teve epiteliólise, um caso de necrose parcial da pele do retalho superficial, todos com evolução favorável. Somente uma paciente teve extrusão do implante. CONCLUSÃO: A reconstrução mamária com retalho excedente de miniabdominoplastia reversa associada à colocação de implante demonstrou ser uma boa opção de reconstrução mamária para casos selecionados, com baixa taxa de complicações. Pacientes com flacidez e lipodistrofia no abdome superior são as candidatas ideais para utilização da técnica.

Evaluation of the Millard technique associated with zetaplasty in the treatment of patients with unilateral cleft lip

ABSTRACT

INTRODUCTION: Cleft lips are very common congenital deformities that affect, in varying degrees, the soft tissues and bone structures of the middle third of the face. Surgical treatment should be performed early and a protocol must be followed, which varies according to the reference center. There are numerous surgical techniques for lip repair, and among them, the Millard technique is the most used. The objective is to evaluate the association of the Millard type I with zetaplasty mucosal technique (used by the author) as the surgery of choice for primary lip repair in patients with unilateral cleft lip, and to understand whether the techniques are appropriate when the number of second surgeries (reoperation) is low. METHODS: Sixty-five patients underwent operations by this technique from January 2007 to December 2012 in Santos, all of whom were followed for at least four years. RESULTS: Of the 65 patients, 10 (15%) were considered to present "unsatisfactory results" and underwent reoperation. CONCLUSION: Primary cheiloplasty with zetaplasty-associated Millard type I is appropriate when the number of reoperations is low, and our results agree with the current literature.

RESUMO

INTRODUÇÃO: As fendas labiais são deformidades congênitas muito comuns e acometem em grau variável as partes moles e estruturas ósseas do terço médio da face. O tratamento cirúrgico deve ser precoce e segue um protocolo que varia de acordo com o centro de referência. As técnicas cirúrgicas de queiloplastia são inúmeras e, dentre elas, uma das mais utilizadas é a de Millard. O objetivo é avaliar a técnica de Millard tipo I associada a uma zetaplastia da mucosa (técnica empregada pelo autor) como cirurgia de escolha na queiloplastia primária dos pacientes portadores de fenda labial unilateral, entendendo que a técnica é adequada se o número de cirurgias secundárias (reoperações) for baixo. MÉTODOS: Foram operados 65 pacientes por essa técnica no período de janeiro de 2007 a dezembro de 2012 em Santos. Todos acompanhados por no mínimo quatro anos. RESULTADOS: Dos 65 pacientes, 10 (15%) foram considerados "resultados insatisfatórios" e reoperados. CONCLUSÃO: Queiloplastia primária à Millard tipo I associada a zetaplastia é adequada, com um número de reoperações baixo e semelhantes aos da literatura atual.

Association of the Tripier myocutaneous flap to scapha cartilage graft: a surgical resource for reconstruction of full thickness defects of lower eyelids

ABSTRACT

The reconstruction of the secondary lower eyelid to a resection of skin cancer is a challenging surgical procedure that must be carefully planned with regards to issues related to lamella and extension of the resection. We present the case of a 69-year-old male patient who, after oncologic resection, presented a total thickness defect greater than 60% in the lower eyelid. For reconstruction, it was indicated that the use of Tripier's unipedic myocutaneous flap with scapha cartilage graft produced favorable results, which confirms that it is worthwhile using these techniques when planning lower eyelid reconstruction.

RESUMO

A reconstrução da pálpebra inferior secundária à ressecção de câncer de pele é um desafio cirúrgico e sua abordagem deve ser planejada por lamelas e extensão da ressecção. Apresentamos o caso de paciente do sexo masculino, 69 anos, que após a ressecção oncológica apresentou defeito de espessura total maior de 60% na pálpebra inferior. Para reconstrução foi indicada a associação do retalho miocutâneo unipediculado de Tripier com enxerto de cartilagem da escafa obtendo resultados favoráveis, mostrando que a associação destas técnicas é uma boa prática na hora de planejar a reconstrução da pálpebra inferior.

Extended and pedicled pectoralis major flap for right orbitofrontal-parietal reconstruction following invasive squamous cell carcinoma resection

ABSTRACT

Introduction: The myocutaneous flap is often used in reconstruction of head and neck defects. However, it is restricted to the middle third of the face. Perforating artery dissection techniques allow further lengthening of the pedicle, thus achieving coverage of the orbitofrontal-parietal region.
Case report: A 63-year-old male with a poorly-differentiated invasive squamous cell carcinoma presented with a final defect of 12.0 × 18.0 cm in the right orbitofrontal-parietal region, with dura mater, frontal sinus, and right upper orbit exposure after resection. We designed a pectoralis major flap, with a cutaneous island equaling the defect in dimensions, in the right parasternal region, from the fourth intercostal space to the subcostal region (extended). The pedicle was sectioned after 4 weeks. The coverage was effective, with no major complications, and a satisfactory aesthetic result.
Conclusion: This flap can be an excellent option for reconstruction of the upper third of the head when there are limitations to microsurgery.

RESUMO

Introdução: O retalho miocutâneo de peitoral maior é um dos mais usados na reconstrução de defeitos da cabeça e pescoço, porém com restrição ao terço médio da face. Com técnicas de dissecção de perfurantes, consegue-se alongar mais o pedículo, obtendo coberturas da região orbito-fronto-parietal.
Relato de Caso: Paciente masculino de 63 anos apresentando carcinoma espinocelular invasivo pouco diferenciado, que após sua ressecção cirúrgica apresentou defeito final de 12,0 x 18,0cm na região órbito-fronto-parietal direita com exposição de dura-máter, seio frontal e órbita superior direita. Foi desenhado retalho de peitoral maior com ilha cutânea de dimensões iguais ao defeito na região paraesternal direita, desde o quarto espaço intercostal até a região subcostal (estendido). O pedículo foi seccionado após 4 semanas. A cobertura foi efetiva, sem complicações maiores e resultado estético satisfatório.
Conclusão: Este retalho mostrou ser uma excelente opção para reconstrução do terço superior da cabeça quando existam limitações para a realização de microcirurgia.
