Endoscopic forehead and midface surgery: eight years experience

ABSTRACT

In the early 90's, plastic surgery videoendoscopic technique was introduced and accepted as a effective method of forehead rejuvenation, mainly for brow and frontal area elevation. This procedure presents multiple indications and is performed to correct brow ptosis and frown lines caused by the depressor muscles. From January 1996 to December 2004, 207 patients were submitted to videoendoscopic forehead surgery in association or not with midface surgery with the some technique. Results were satisfactory with maintenance of brow elevation, decrease of glabelar wrinkles, superiorlateral rotation of oculo-palpebral elements and elevation of midface.

RESUMO

Desde o início dos anos 90, a técnica videoendoscópica em cirurgia plástica foi introduzida e aceita como método eficaz para o rejuvenescimento do terço superior da face, principalmente para elevação dos supercílios e região frontal. Este procedimento apresenta múltiplas indicações, sendo realizado para corrigir a ptose dos supercílios e rugas glabelares causadas pelos músculos depressores. No período de janeiro de 1996 a dezembro de 2004, 207 pacientes foram submetidas à cirurgia endoscópico da região frontal, associando ou não o tratamento do terço médio da face pela mesma técnica. Os resultados obtidos foram satisfatórios com manutenção da elevação dos supercílios, redução das rugas de expressão de glabela, rotação dos elementos óculo-palpebrais no sentido látero-superior e elevação do terço médio da face.

Hemostatic net: an alternative for the prevention of hematoma in rhytidoplasty

ABSTRACT

BACKGROUND: Hematoma is the most common postoperative complication of rhytidoplasty. Its occurrence increases morbidity and impairs recovery. Internal sutures to close detached areas are used in abdominoplasty to prevent seromas. Taking this into account and in order to reduce the number of patients with hematomas post rhytidoplasty, we have developed a similar surgical procedure in which a hemostatic net is made of continuous nylon 4-0 transfixing stitches to include the skin and the superficial musculoaponeurotic system-platysma. METHODS: A total of 366 patients who underwent rhytidoplasty of at least the middle third of the face between July 2009 and September 2011 were included in the study. Group A was identified as the control group; it included the first 120 patients assessed retrospectively. The remaining 246 patients, who underwent the surgical procedure here proposed and who were evaluated prospectively formed group B. We observed the incidence of hematoma, ischemia, and necrosis in the first 72 postoperative hours. RESULTS: In group A, 17 (14.2%) patients developed hematomas, whereas none occurred in group B. The surgical procedure did not significantly increase the occurrence of ischemia: 11 (9.2%) patients in group A and 16 (6.5%) in group B had this complication (P < 0.3964). There was also no significant increase in the incidence of necrosis, which was observed in 3 (2.5%) patients in group A and 4 (1.6%) in group B (P < 0.4298). CONCLUSIONS: The hemostatic net is an efficient method for the prevention of early hematomas following rhytidoplasty. This surgical procedure did not result in a significant increase in the incidence of ischemia and necrosis.

RESUMO

INTRODUÇÃO: Hematoma é a complicação pós-operatória mais frequente em ritidoplastia. Sua ocorrência aumenta a morbidade e prejudica a recuperação. Pontos de adesão internos para fechamento das áreas descoladas em abdominoplastias evitam o surgimento de seroma. Baseados neste princípio, e com o objetivo de reduzir o número de pacientes com hematomas em ritidoplastia, foi desenvolvida tática cirúrgica análoga em que se confecciona uma rede hemostática de pontos contínuos e transfixantes de náilon 4-0, englobando a pele e o SMAS-platisma. MÉTODO: Foram incluídos no estudo 366 pacientes consecutivos, submetidos a ritidoplastia de pelo menos o terço médio da face, entre julho de 2009 e setembro de 2011. O grupo A, incluindo os primeiros 120 pacientes avaliados retrospectivamente, foi considerado controle. O grupo B foi constituído pelos demais 246 pacientes, operados com a tática cirúrgica proposta e avaliados prospectivamente. Observaram-se as incidências de hematoma, isquemia e necrose nas primeiras 72 horas de pós-operatório. RESULTADOS: No grupo A, 17 (14,2%) pacientes apresentaram hematoma, enquanto no grupo B não houve nenhum caso. A tática cirúrgica não aumentou significativamente a ocorrência de isquemia: 11 (9,2%) pacientes no grupo A e 16 (6,5%) no grupo B tiveram essa complicação (P < 0,3964). Também não houve alteração significativa na incidência de necrose, complicação observada em 3 (2,5%) pacientes do grupo A e em 4 (1,6%) do grupo B (P < 0,4298). CONCLUSÕES: A rede hemostática é método eficaz na prevenção de hematomas precoces em ritidoplastias. Essa tática cirúrgica não levou a aumento significativo da incidência de isquemia e necrose.

Canthopexy and tarsal reinforcement using a periosteal flap

ABSTRACT

INTRODUCTION: The low resistance of the palpebral tissues to the retractable forces of postoperative scarring can predispose patients to undesirable surgical outcomes. Among the more effective techniques for solving this issue are the periosteal flaps. This report aims to present a technique for canthopexy and tarsal reconstitution using a periosteal flap to obtain a consistent and definitive reinforcement of the tarsal ligament and its continuity to the inner face of the sidewall of the orbit. METHODS: From July 2009 to December 2012, 49 patients underwent canthopexy with flap rotation of the arcus marginalis of the inferior tarsus. The functional and aesthetic results were evaluated together with the presence of complications. RESULTS: Eighty-four cantopexies and tarsal reinforcements using a periosteal flap were performed in 49 patients. No complications such as hematoma, infection, or necrosis were observed. Complete remission of the preoperative signals was observed in 45 (91.8%) patients, while the other 4 (8.2%) showed partial remission on at least one of the sides related to prior ocularis paralysis. CONCLUSIONS: Canthopexy reinforced by a tarsal periosteal flap provides good tarsal ligament reconstitution with appropriate positioning of the outer corner and good contact of the eyelid with the ocular globe.

RESUMO

INTRODUÇÃO: A pouca resistência dos tecidos palpebrais quando submetidos ao poder retrátil dos planos cicatriciais predispõe a resultados cirúrgicos indesejáveis. Dentre as técnicas mais resolutivas destacam-se as que utilizam retalhos de periósteo. O presente artigo tem por objetivo apresentar uma técnica de cantopexia e reconstituição tarsoligamentar com retalho de periósteo, a fim de obter um reforço consistente e definitivo do tarso e sua continuidade tecidual à face interna da parede lateral da órbita. MÉTODO: No período de julho de 2009 a dezembro de 2012, foram operados pacientes candidatos à realização de cantopexia com rotação de retalho do arcus marginalis sobre o tarso inferior. Foram avaliados os resultados estéticos e funcionais, bem como a presença de complicações. RESULTADOS: Foram realizadas 84 cantopexias e reforço tarsal com retalho de periósteo, em 49 pacientes. Não foram observadas complicações, como hematoma, infecção ou necrose. Remissão total dos sinais pré-operatórios foi verificada em 45 (91,8%) pacientes e 4 (8,2%) apresentaram remissão parcial em pelo menos um dos lados, relacionada à prévia paresia orbicular. CONCLUSÕES: A cantopexia com reforço tarsal com retalho de periósteo proporciona boa reconstituição tarsoligamentar, com posicionamento adequado do canto externo e bom contato da pálpebra com o bulbo ocular.

Temporary hemostatic traction (THT) in rhytidoplasty

ABSTRACT

INTRODUCTION: To slow the aging process, facial contouring surgery was developed as a rejuvenating technique, and is constantly improving, as are the results. However, as with any surgical procedure, complications are possible, the main one being hematoma. Dissatisfied with the prospect of hematoma and the use of drains, and with a goal of lower costs, the authors felt the need to develop a new surgical procedure, temporary hemostatic traction (THT). METHODS: Sixty-four rhytidoplasties, all performed at the Valle Pereira Clinic (Florianópolis, SC, Brazil) from May 2012 to January 2013, were included in this study. All patients underwent the new THT procedure. RESULTS: Only two patients developed epidermolysis with optimal evolution following THT, and one developed generalized facial edema. There were no instances of hematoma at any of the postoperative stages. CONCLUSIONS: This new surgical technique avoids the use of drains and decreases dead space, thereby improving flap adhesion, and decreases hematoma, seroma, and postoperative edema; it also decreases tension on the incision, hence improving the results of rhytidoplasty.

RESUMO

INTRODUÇÃO: Na tentativa de desacelerar o processo de envelhecimento, as cirurgias de contorno facial foram estabelecendo-se como técnicas rejuvenescedoras, sendo constantemente aprimoradas, assim como os resultados. Porém, como qualquer procedimento cirúrgico, há a possibilidade de intercorrências e a principal complicação delas é o hematoma. Descontentes com a presença desta complicação, com o uso de drenos e visando menor custo, os autores viram a necessidade de desenvolver uma nova tática cirúrgica chamada tração hemostática temporária - THT. MÉTODOS: Foram incluídas neste estudo todas as ritidoplastias realizadas na Clínica Valle Pereira (Florianópolis, SC), no período compreendido entre maio de 2012 e janeiro de 2013, totalizando 64 pacientes. Todos foram submetidos à nova tática cirúrgica chamada tração hemostatica temporária. RESULTADOS: Apenas 2 pacientes apresentaram epidermólise com ótima evolução sob a THT, e 1 caso de edema facial generalizado. Não houve caso com hematoma, em qualquer período pós-operatório. CONCLUSÕES: esta nova tática cirúrgica evita o uso de drenos, diminui o espaço morto aumentando a adesão do retalho descolado e, consequentemente, diminui a presença de hematoma, seroma e edema pós-operatório, assim como diminui a tensão cicatricial na incisão da pele, visando aprimorar os resultados das ritidoplastias.

Pubic region's treatment in abdominoplasty of patients with great loss of weight

ABSTRACT

Introduction: Thinking about the affliction of the patients with great weight loss because of their bodies, we developed a new procedure to treat the pubic area in these patients. Objectives: To describe a new way to treat the pubic area in patients with great loss of weight with an important pubic ptosis. Methods: The incision of the abdominoplasty is performed a little bit lower in the pubic region, about 5cm from the vulvar fissure with the skin pulled up. Then, the Scarpa fascia is fixed more cranial in the aponeurosis after its undermining, with symmetric nots. You can see a great improvement in this region as soon as you treat it. Results: We had an 100% satisfaction range among our patients, once we could improve substantially the looking of this region without increasing the size of the scar, and could also make an effective resection of the excess of skin and fat tissue. Conclusion: This is a new solution to treat the ptotic pubic region in patients with great loss of weight, improving their body contour. It showed to be a low morbidity technique, easy to performed, less invasive, giving great results and with no additional scars for the patient.

RESUMO

Introdução: Com base na angústia dos pacientes com grande perda ponderal em relação ao seu corpo, desenvolvemos uma nova tática para tratamento da ptose da região pubiana. Objetivos: Demonstrar tática cirúrgica para tratamento da região pubiana de pacientes com grande perda ponderal e ptose importante dessa região. Método: Faz-se a marcação convencional da abdominoplastia com incisão pubiana mais baixa, a 5 cm da fúrcula vulvar, tracionando-se a pele para cima. Realiza-se a fixação mais cranial da fáscia de Scarpa na aponeurose com pontos simétricos em "U", após seu descolamento, observando-se imediata melhora do aspecto dessa região. Resultados: Obtivemos satisfação de 100% entre os pacientes, por conseguirmos melhorar o aspecto da região pubiana sem aumentar a cicatriz resultante e realizar uma retirada efetiva da redundância de pele e de tecido celular subcutâneo. Conclusão: Trata-se de uma nova solução para melhora da região pubiana e do contorno corporal dos pacientes pós grande perda ponderal, apresentando baixa morbidade, sendo pouco invasiva, de fácil execução e de resultados agradáveis, com nenhuma cicatriz adicional ao paciente.

Anchor-line abdominoplasty in ex-obese patients

ABSTRACT

Thirty-nine ex-obese patients (post-gastroplasty or diets) with surplus skin-fatty and classics abdominal excess, were submitted to abdominoplasty. The technique consists of vertical and horizontal incision (at anchor), without realease or minimum realease corrections of incisional hernia, aponeurosis plication of the straight-abdominal muscles, draining vacuum, shortening the umbilical scar and pubis region pexia.

RESUMO

Foram tratados 39 pacientes ex-obesos (pós-gastroplastia ou dietas) que apresentavam grandes sobras dermogordurosas com excessos abdominais clássicos, submetidos a abdominoplastia. A técnica consiste de incisão vertical e horizontal (em âncora), sem descolamentos ou descolamentos mínimos, correções de hérnias incisionais, plicatura da aponeurose dos músculos reto-abdominais, drenagem a vácuo, encurtamento da cicatriz umbilical e pexia da região púbica.

Temporofrontal galea periosteal lifting with double access: a conservative procedure

ABSTRACT

BACKGROUND: Surgical treatment of the superior third of the face should usually involve an extensive area, including the frontal and temporal regions. However, the open coronal technique has been avoided because it carries a risk of potential complications and a wide scar. The technique has been supplanted by endoscopic and other less-invasive techniques, which are perhaps less efficacious. This article proposes an open technique that maintains the coronal lifting efficiency without its typical complications. METHODS: A retrospective analysis of 20 consecutive female patients who underwent cervicofacial facelift from February 2008 to July 2011 was performed. The patients had a mean age of 53.3 years (range, 46-71 years). The sensitivity of the regions covered (i.e., anesthesia, hypoesthesia, hyperesthesia, paresthesia), motor function of the temporal branch of the facial nerve (i.e., frontal muscle movement), and healing (i.e., widening and alopecia) were evaluated. RESULTS: There were no sensory, motor, or vascular complications. Moreover, neither cicatricial enlargement nor alopecia was observed. One patient required revision of the corrugator muscle myectomy, and another underwent myectomy of the lateral orbicularis muscle of the eye for optimal results. CONCLUSIONS: The proposed technique is simple, safe, does not require special equipment or devices, and has the advantage of wide and harmonious distribution of the flap, including the temporal and frontal regions.

RESUMO

INTRODUÇÃO: O tratamento do terço superior da face deve ser amplo, englobando as regiões frontal e temporal. Entretanto, em decorrência das potenciais complicações e da cicatriz extensa, o lifting coronal tem sido evitado e substituído por técnica endoscópica e procedimentos menos invasivos, porém, talvez, menos eficazes. O objetivo deste estudo é propor uma tática cirúrgica com eficácia similar à do lifting coronal, sem, no entanto, as complicações típicas desse procedimento. MÉTODO: Foi realizada análise retrospectiva de 20 pacientes do sexo feminino, consecutivas, com média de idade de 53,3 anos, submetidas a lifting cervicofacial, no período de fevereiro de 2008 a julho de 2011. Foram avaliadas a sensibilidade da região abordada (anestesia, hipoestesia, hiperestesia, parestesia), a função motora do ramo temporal do nervo facial (movimento do músculo frontal), e a cicatrização (alargamento e alopecias). RESULTADOS: Não houve complicações sensitivas ou motoras, bem como vasculares. Também não foram observados alargamento cicatricial ou alopecias. Uma paciente precisou de revisão da miectomia dos músculos corrugadores e outra foi submetida a miectomia do músculo orbicular lateral do olho, para otimização do resultado. CONCLUSÕES: A tática é segura, simples, não necessita de equipamentos ou materiais especiais, e tem a vantagem da ampla e boa distribuição cutânea, englobando as regiões frontal e temporal.

Evaluation of long-term results of rhytidoplasty

ABSTRACT

Many articles have been published about rhytidoplasty, however, few report on the long-term results. The objective of this article is to try to evaluate the results of face lifts and to define long-term results. The evaluation of rhytidoplasty is not an easy task. Different methods are utilized but there are no parameters that can assess the final product unless the result is bad. The process of aging does not cease after the facial rejuvenating surgery. Several important factors are involved in the result and the duration of the surgical effects including: the different ages at which patients are submitted to face lifts, the different surgical goals and the experience and good sense of the surgeon. Thus, this subject is of special interest to the plastic surgeon. In this article we tried to explore some of these aspects. Is it possible this is a philosophical discussion?

RESUMO

Vários artigos são publicados sobre ritidoplastia, porém, poucos abordam seus resultados tardios. O objetivo deste artigo é tentar avaliar os resultados do face lift e definir qual é o resultado tardio. A avaliação da ritidoplastia não é tarefa fácil. Diferentes métodos são utilizados, porém não há parâmetros que possam avaliar o produto final, a não ser que um resultado ruim ocorra. O processo do envelhecimento não cessa após a cirurgia de rejuvenescimento facial. Vários fatores, de modo importante, fazem parte do resultado e da duração dos efeitos da cirurgia: as diferentes idades em que os pacientes são submetidos ao face lift, as diferentes propostas cirúrgicas, a experiência e o bom senso do cirurgião. Sendo assim, este assunto torna-se extremamente interessante para o cirurgião plástico. Neste artigo, tentamos expor alguns destes aspectos. Seria esta uma discussão filosófica?

Abdominoplasty and pubic lifting

ABSTRACT

Introduction: The present paper relates the conduct, planning, results and conclusions about abdominoplasty associated to pubic lifting method. Methods: A hundred and twenty patients were submitted surgery between May 2005 to June 2009. No significant complications were observed, that they could pledge the final results. Conclusion: Good results were obtained due to adequate planning for each case in private.

Introdução: O presente trabalho relata a conduta, planejamento, resultados e conclusões das abdominoplastias associadas ao lifting do púbis. Método: Foram analisados 120 pacientes, submetidos a cirurgia no período de maio de 2005 a junho de 2009. Resultados: Não foram observadas complicações significativas, que pudessem comprometer os resultados finais. Conclusão: Concluiu-se que os bons resultados foram decorrentes do adequado planejamento para cada caso em particular.

Rejuvenation of the pudenda region, with or without abdominoplasty

ABSTRACT

Introduction: The authors propose a simple, comprehensive and easily reproducible method to suspend to thin and fixate the pubic region on a better position, usually in association with abdominoplasties. Method: The technique consists in approaching the pubic region with open defating associated or not with traditional liposuction and a cross traction with absorbable sutures, anchoring them in the anterior rectus apponeurosis. Results: Pictures will show the goal was achieved, with thinner, younger and more harmonious pudenda region obtained with the method. There are some reports of better performance during intercourse, probably because of the superficialization of the clitoris. Some mates also reported a better quality intercourse after the procedure. Discussion: The main author started to address this region with liposuction ten years ago , which is also a good option. However, in his hands the results became more promising when he began to use this traction at first a parallel traction and now a crossed one. Conclusion: With these results the authors were able to show that this method can be employed usually after an abdominoplasty to reach the main proposed objectives which are: a thinner, narrower and better looking pubic region.

RESUMO

Introdução: Os autores propõem um método de simples compreensão e de fácil reprodução para a abordagem e tratamento da região pudenda, geralmente realizado no final de uma dermolipectomia abdominal. Método: A técnica consiste em lipectomia aberta com tesoura, podendo ser associada a lipoaspiração convencional e pexia com tracionamento cruzado da região pudenda, devolvendo as características de jovialidade à área em questão. Resultados: Pode-se constatar pelas fotografias, que houve estreitamento do triângulo e ainda elevação da rafe mediana, posicionada mais anatomicamente. Relatos foram obtidos quanto à melhora da qualidade e desempenho do ato sexual, tanto pelas pacientes quanto por seus parceiros. Discussão: Há dez anos, o autor principal aborda esta região, inicialmente utilizava lipoaspiração como ferramenta. Os resultados tornaram-se mais promissores quando foi iniciado o uso de pexia, anteriormente paralela e atualmente cruzada. Conclusão: Conclui-se que o método descrito, propõe e atinge os seus principais objetivos: tornar a região pudenda mais magra, estreita e harmoniosa. Por trazer benefícios para a paciente e para seu companheiro, a técnica poderá ser adicionada ao arsenal do cirurgião plástico brasileiro que tem como intuito uma melhora global no aspecto de sua cirurgia do contorno corporal.

ABSTRACT

The author's analyze the use of face-lifting with periauricular scar in 107 cases. The main differential of this technique is the incision that circles the auricle, and accommodate in post auricular region, finishing in a unique scar in temporal region. It's indicated at first for the patients who initiate early the treatment of facial rhytidosis.

RESUMO

Os autores analisam a aplicabilidade da ritidoplastia com cicatriz periauricular em 107 pacientes operados. Técnica esta, que tem como diferencial, a incisão que circunda o pavilhão auricular e se acomoda no sulco retroauricular, terminando em cicatriz única na região temporal. É indicada primariamente em pacientes que iniciam mais cedo o tratamento da ritidose facial.

Abdominoplasty: neoomphaloplasty without scar or fat excision

ABSTRACT

Introduction: Under the cultural aspect the umbilicus is the only scar accepted in the body, its dysmorphia or absence is noted negatively. Distortions of this scar are still frequent in the abdominoplasties determined by several different aesthetic technical procedures to imitate its natural aspect. The scope of this presentation is another attempt to obtain a neoomphaloplasty based on an easy technical procedure, with a quite natural final aspect and without scars. Methods: During the abdominoplasty, the umbilicus and its stalk are amputated and the dermis of the skin over the future projection is sutured directly to the fascia below, after the local fat excesses be divulsioned to produce the natural navel aspect without scar. Results: Along the last nine years, 319 patients were operated by this technique, with a natural aspect and minimal unaesthetic effects.

RESUMO

Introdução: A cicatriz do umbigo é a única do corpo aceita culturalmente, cuja dismorfia ou ausência chama a atenção negativamente. As distorções dessa cicatriz, que ainda ocorrem com frequência nas abdominoplastias, determinaram a elaboração de inúmeros procedimentos técnicos com a finalidade estética de imitar o seu aspecto natural. Apresentamos mais uma opção sobre neo-onfaloplastia no decurso das plásticas de abdome, mediante uma técnica de fácil execução, com efeito estético próximo ao umbigo natural e sem cicatrizes cutâneas. Método: No decurso da abdominoplastia, o umbigo e o seu pedículo são amputados e a derme da pele na área de projeção do neoumbigo é suturada diretamente no plano da fáscia muscular abaixo, após divulsão tecido adiposo local para produzir a necessária umbilicação sem cicatriz. Resultados: No período de nove anos, foram operados 316 pacientes com a utilização desta técnica, com constante aspecto natural e mínimos efeitos pouco estéticos.

Segmented approach to the midface. Triangle on cheek area: dermal fat suture point

ABSTRACT

Introduction: The surgical treatment presented by the authors has tactical innovations and unique features that make the treatment safer. Objective: To show a different approach to cheek area and fix the skin with dermal fat points. Methods: Approach the cheek area with progressive retractors, leaving bands. Skin fixation with dermal fat points using nylon 4.0 favors tension reduction in the scar. Results: Thirty-nine pacients were operated using this tactic. The period of hospitalization was 24 hours and there was only one case of hematoma. Conclusion: It is an efficient and simple method where the approach to the cheek area using retractors show low bleeding. The adhesion left by this method encourage better skin irrigation. Dermal fat points keep the skin in place and reduce the tension in the scar.

Introdução: O tratamento cirúrgico apresentado pelos autores tem inovações táticas e peculiares que tornam o tratamento seguro. Objetivo: Demonstrar uma maneira diferente de abordar a região geniana e de fixar a pele com pontos dermogordurosos. Método: Abordagem da região geniana com os descoladores progressivos, deixando bridas. A fixação da pele com pontos dermogordurosos usando fio de nylon 4.0 favorece a diminuição da tensão na cicatriz pré-auricular. Resultados: Foram operados trinta e nove pacientes usando essa tática. O período de internação foi de 24 horas, sendo observado apenas um caso de hematoma. Conclusão: O método proposto é eficiente e simples, onde a abordagem da região geniana com o uso dos descoladores é de pouco sangramento. As bridas deixadas por esse método favorecem melhor irrigação da pele. Os pontos dermogordurosos mantêm a pele no local e diminuem a tensão na cicatriz pré-auricular.

Umbilical scar autonomization: a safe technique for secondary abdominoplasties

ABSTRACT

Background: In lipominiabdominoplasty and mid-abdominoplasty procedures, the umbilicus is usually undermined from its aponeurotic fixation; this modifies its normal vascular pattern. In patients undergoing these procedures and candidates for a secondary classic abdominoplasty, trophic changes, including necrosis, may occur in the umbilical scar. To avoid trophic complications in the neo-umbilicus, autonomization of the umbilical scar was carried out. Methods: Three candidates for a secondary classic abdominoplasty underwent the umbilicus autonomization process in the private clinic of the corresponding author. One incision, 1 cm from the umbilical scar, was performed from the skin to the aponeurotic plane on each side, with an interval of 15 days between the two surgical incisions. After the second incision, the patient waited for a further 15 days. Thus, the whole process took 30 days before classic abdominoplasty was performed. Results: No trophic alterations or necrosis in the umbilical scar were observed in these cases. Conclusions: The aesthetic results were satisfactory, indicating the effectiveness of this method.

Introdução: Nos procedimentos de lipominiabdominoplastia e midiabdominoplastia, usualmente, o umbigo é destacado de sua fixação aponeurótica, o que modifica o padrão vascular do umbigo. Em pacientes submetidos a esses procedimentos e candidatos a abdominoplastia clássica secundária, podem ocorrer alterações tróficas da cicatriz umbilical e, até mesmo, necroses. Utilizou-se a manobra de autonomização da cicatriz umbilical para evitar complicações tróficas do neoumbigo. Métodos: Foram submetidas ao processo de autonomização da cicatriz umbilical 3 pacientes candidatas a abdominoplastia clássica secundária, na clínica privada do autor principal. A técnica compreende uma incisão a 1 cm da cicatriz umbilical, desde a pele até o plano aponeurótico, de cada lado, com intervalo de 15 dias entre cada etapa, no total de dois tempos cirúrgicos. O processo como um todo leva 30 dias, antes da abdominoplastia clássica. Resultados: Não foram observadas alterações tróficas ou necroses da cicatriz umbilical nos casos submetidos ao processo de autonomização. Conclusões: Os resultados estéticos foram satisfatórios, o que atestou a efetividade do método.

Surgical treatment of clitorian hypertrophy

ABSTRACT

Introduction: Moderate clitoris hypertrophy has been described in 25% of the women. When its shape and volume disturb at the sexual activity, combined or not by psychological problems, initially patients search for help from gynecologists and or by endocrinologists and not rarely are send to plastic surgeons. Our experience is based on 9 patients with ambiguous external genitalia, submitted to these surgeries during the last 15 years. Three cases were selected to demonstrate the obtained results with the technique. Two female patients with Prader type II clitoris aspect were selected; one of them presents the problem at adult age and, in the other, the etiology was possible congenital. The third patients presented hypertrophic clitoris combined with pseudo-hermaphroditism with ovarium, atrophic testicles as described in Prader Type V. Methods: The all three patients were submitted to clitoris reduction by dissection of the mucous-cutaneous covering and its excess buried by suture. The third patient with pseudo-hermaphroditism had also neo-vagina reconstruction. Results: The surgical procedures preserved esthetic and sensibility, with clitoris dimensions reduction. Conclusion: The surgical procedures presented moderate simplicity, but within the surgical criteria to preserve the functions and the external genitalia anatomy.

RESUMO

Introdução: Segundo vários autores, a hipertrofia discreta do clitóris está presente em 25% das mulheres. Quando o volume passa a incomodar no ato sexual, acompanhado ou não de distúrbios psicológicos, as pacientes buscam inicialmente ajuda de ginecologistas ou endocrinologistas e não raramente são encaminhadas à cirurgia plástica para a resolução dos problemas. Método: Nossa experiência é baseada em 9 pacientes, durante 15 anos, portadoras de genitália ambígua, submetidas a tratamento cirúrgico. Foram selecionados 3 casos para ilustrar os resultados obtidos com a técnica. Duas pacientes com hipertrofia clitoriana do tipo II de Prader foram selecionadas; uma de origem adquirida na idade adulta, e outra de origem possivelmente congênita. A terceira paciente apresentava anomalias sexuais múltiplas, de origem congênita: agenesia de vagina, hipertrofia clitoriana, presença de ovários e testículos atrofiados, pseudo-hermafroditismo feminino, catalogada como tipo V de Prader. Todos os três pacientes foram submetidas a redução do clitóris pela dissecção da cobertura músculo-cutânea (desenluvamento) e sepultamento do corpo clitoriano por sutura. Resultados: Os procedimentos cirúrgicos realizados preservaram a estética e a sensibilidade, com redução das dimensões do clitóris. Conclusão: Os procedimentos apresentam relativa simplicidade, porém dentro de critérios cirúrgicos buscando a preservação das funções e da anatomia da genitália externa.

Umbilical base plicature: technique proposition for treatment of the bulge aspect and to avoid stigmas after abdominoplasty

ABSTRACT

Introduction: The umbilicoplasty (onfaloplasty) is a main surgical step during abdominoplasty. Esthetic concerns and important reference to this area in the post surgical time are assessed, by the patient and the surgeon. The bulge aspect of the umbilical base, similar to umbilical hernias, is considered unattractive. This situation could exist in the pre surgical evaluation or it could be an unfavorable outcome. Purpose: To present a complementary technique to be associated during umbilicoplasty, using the plicature of the umbilical base to aponeurosis, with the objective of treat a pre existent bulge of umbilicus or to avoid this undesirable post surgical alteration. Methods: Twenty female patients were submitted to conventional abdominoplasty with umbilical base plicature during a period of 12 months. In 10 cases there were a bulge aspect in pre surgical. None of the patients had umbilical hernia. The follow up has duration of 6 - 24 months. Results: There were good results in all cases, with treatment of the deformity when it was present, with no recidive or sequela. There were no cases of local ischemia or dehiscence. In the cases without previous deformity there weren't alterations after surgery. Conclusions: The plicature of the umbilical base is a technique that could be used with success in the treatment of an existent umbilical bulge and to avoid this undesirable situation after abdominoplasty.

Introdução: A umbilicoplastia (onfaloplastia) corresponde a um tempo fundamental durante a realização das abdominoplastias. Considerações estéticas e importante destaque no período pós-operatório, tanto pela paciente quanto pelo cirurgião, são atribuídas a esta região. A protrusão da base umbilical, similar ao aspecto de hérnias, é considerada aspecto não atrativo. Essa situação pode existir no pré-operatório ou evoluir como resultado pós-operatório desfavorável. Objetivo: Apresentar uma técnica complementar a ser associada durante a realização da umbilicoplastia, com a realização da plicatura da base da cicatriz umbilical à aponeurose, objetivando tratar protrusões existentes ou evitar esta indesejável alteração no pós-operatório. Método: Vinte pacientes do sexo feminino foram submetidas à abdominoplastia convencional com realização da plicatura da base umbilical, durante período de 12 meses. Em 10 casos, havia protrusão no pré-operatório. Nenhuma das pacientes apresentava hérnia nessa topografia. O acompanhamento pós-operatório variou de 6 a 24 meses. Resultados: Foram obtidos bons resultados em todos os casos, com tratamento da deformidade proposta quando presente, sem recidivas ou sequelas locais. Não houve casos de isquemia local ou deiscência. Nos casos em que não se evidenciava nenhuma deformidade prévia, não houve alterações no pós-operatório. Conclusão: A plicatura da base umbilical é uma técnica que pode ser utilizada com sucesso para tratamento da protrusão já estabelecida, bem como para evitar que essa situação indesejada possa ocorrer após abdominoplastias.

Circumferential abdominoplasty after loss weight

ABSTRACT

Background: Obesity is increasing all around the world and for this reason bariatric surgeries are better nowadays and patients are looking for these procedures. After loss weight, patients need plastic surgery to improve the body contouring. Many surgical techniques are available to treat the abdominal area, flanks and buttocks. Purpose: The purpose of this article is to compare our personal technique of circumferential abdominoplasty with the majors surgeons about the place of flap's ressection, zones of adherence's release, scar placement, waist definition and improvement of buttocks. Methods: The circumferential abdominoplasty were performed in 8 patients, between April 2007 and December 2008. Results: The patients showed up with better body contouring, better waist definition and improvement of the buttocks. Conclusion: The better place of flap's ressection is in the lower aspect of the trunk, the partial release of the zones of adherence is important to allow the suspension of trochanteric areas; the most pleased scar's placement is in the superior aspect of buttocks; the waist definition is according with the flap position and it's ressection; and the buttocks improvement is better when autologous flaps are used in these areas.

RESUMO

Introdução: Em decorrência do aumento da obesidade na população, assim como a melhora nas técnicas cirúrgicas bariátricas, o número de pacientes com histórico de obesidade e perda acentuada de peso cresceu significativamente nos últimos anos. Existem várias técnicas cirúrgicas que visam à reconstrução dos tecidos frouxos da região abdominal, região dos flancos e trocantéricas nestes pacientes. Objetivo: O objetivo deste trabalho é a comparação entre o resultado da nossa técnica de abdominoplastia circunferencial realizada em pacientes após grande perda ponderal em relação às técnicas mais usadas atualmente, considerando-se: localização do retalho circunferencial a ser excisado, liberação das zonas de aderência, localização final da cicatriz, definição da cintura e aumento da região glútea. Método: A abdominoplastia circunferencial foi realizada em 8 pacientes, no período de abril de 2007 a dezembro de 2008. Resultados: Os pacientes operados apresentaram melhora do contorno corporal, melhor definição da cintura, e melhora do contorno da região glútea. Conclusão: A localização mais adequada do retalho circunferencial a ser excisado posteriormente é a região mais inferior do tronco, a liberação parcial das zonas de aderência promove a suspensão dos tecidos das regiões trocantéricas sem permitir a descida tão drástica da cicatriz; a localização final mais adequada da cicatriz é na parte superior do contorno glúteo; a definição da cintura está diretamente relacionada com a posição do retalho a ser ressecado e com a retirada do tecido adiposo desta área; o aumento da região glútea pode ser realizado com emprego de retalhos autólogos.

Alternative surgical technique in treatment of essential blepharospasm

ABSTRACT

Background: Essential blepharospasm is bilateral, progressive and can lead to functional blindness. It predominates in female patients and its cause is unknown. Case report: A 52-year patient, with progressive eyelid spasms along two years of evolution, received five applications of botulinum toxin, responding only to the first ones. In the initial evaluation she showed a palpebral fissure of 2 mm with maximum voluntary opening. She was submitted to resection of an ellipse of skin of upper eyelid along with the orbicular muscle and, by the same surgical access, the corrugators muscles were resected, with preservation of the supra-orbital and supra-trochlear nerves. In the postoperative period, she evolved with maximum voluntary opening of the palpebral fissure of 12 mm. With five months postoperatively, there was partial recurrence, with palpebral fissure of 8 mm. Receiving botulinum toxin on lower dose, she presented opening of 10 mm. Conclusion: This alternative surgical technique for treatment of blepharospasm showed a similar result to the techniques described in the literature, however resecting the orbicularis muscle only superiorly and the corrugators by the same surgical approach, preserving the innervations of the frontal region.

RESUMO

Introdução: Blefaroespasmo essencial é bilateral, progressivo e pode levar a cegueira funcional. Predomina em pacientes do sexo feminino e sua causa é desconhecida. Relato do Caso: Paciente de 52 anos, com espasmos palpebrais progressivos em dois anos de evolução, recebeu cinco aplicações de toxina botulínica, com resposta apenas após as primeiras infiltrações. Na avaliação inicial, apresentava fenda palpebral com abertura voluntária máxima de 2 mm. Foi submetida à ressecção de um fuso de pele da pálpebra superior contendo o músculo orbicular e, pela mesma via de acesso, foram ressecados os músculos corrugadores, com preservação dos nervos supra-orbitários e supratrocleares. No pós-operatório, evoluiu com abertura voluntária máxima da fenda palpebral de 12 mm. Com cinco meses de pós-operatório, houve recidiva parcial, com fenda palpebral de 8 mm. Submetida a tratamento com toxina botulínica, em menor dose, apresentou abertura de 10 mm. Conclusão: Esta técnica cirúrgica alternativa para o tratamento do blefaroespasmo demonstrou resultado semelhante às técnicas descritas na literatura, porém ressecando os músculos orbiculares apenas superiormente e os corrugadores pela mesma via de acesso palpebral, preservando a inervação da região frontal.

Rhytidoplasty in an 11-year-old patient as adjuvant treatment for post inflammatory elastolysis and cutis laxa - challenges in the treatment

ABSTRACT

Cutis laxa is a rare morbidity, is caracterized as a connective tissue disorder manifested primarily by sagging cut and may affect other organs. The authors report a case of cutis laxa in a patient of 11 years old with the development of a senile fascia with pronounced ritidose perioral, periocular and nasolabial sulcus. Rhytidectomy was performed as an auxiliary treatment. The patient showed a good evolution of the postoperative scars with good quality and remarkable improvement in sagging of the midface and later recurrence of the same part in the follow-up evaluation two years. Patient and family were very pleased with the outcome. The surgical team, however, realized the limitation of the procedure and the need for additional treatment with auxiliaries and the like as peeling.

RESUMO

Cútis laxa é uma morbidade rara, caracterizada por ser uma desordem do tecido conjuntivo que se manifesta principalmente por flacidez cutânea, podendo acometer variavelmente outros órgãos. Os autores relatam um caso de cútis laxa em paciente de 11 anos que desenvolveu face senil com pronunciada ritidose perioral e periocular, bem como acentuação dos sulcos nasogenianos. Foi realizada ritidoplastia como tratamento cirúrgico. Apresentou boa evolução pós-operatória, com cicatrizes de boa qualidade e melhora notável na flacidez do terço médio da face e, posteriormente, recorrência parcial da mesma no acompanhamento tardio de dois anos. Paciente e familiares ficaram muito satisfeitos com o resultado. A equipe cirúrgica, entretanto, percebeu limitação do resultado cirúrgico e necessidade de complementação com tratamentos auxiliares como peelings e similares.

O porquê da plicatura de smas como primeira opção em serviço de residência médica

RESUMO

INTRODUÇÃO: O lifting facial é uma cirurgia que continua em crescimento e há inúmeras opções de técnicas para sua execução. Devido à complexidade anatômica e cirúrgica destes procedimentos, o médico residente se depara com grande temor no aprendizado inicial de ritidoplastias. A plicatura do sistema musculoaponeurótico superficial da face (SMAS), por mobilizar menos tecido e ser em plano superficial a estruturas nobres, possibilita menos complicações e mesmo assim apresenta resultados satisfatórios aos pacientes. MÉTODOS: Levantamento de casos operados entre março a dezembro de 2016, avaliar o grau de satisfação das pacientes, por meio de questionário com pontuação, e descrever complicações encontradas no pós-operatório de pacientes. RESULTADOS: 17 dos 19 pacientes estavam satisfeitos (90%) com o resultado estético da cirurgia, sendo atribuída uma nota média dentre todos de 8,2 (muito satisfeitos). Como complicações, 3 evoluíram com hematoma (15%), 1 com necrose retroauricular (5%) e 1 com deiscência pós tragal (5%). CONCLUSÃO: A plicatura do SMAS é uma técnica segura, tem menor curva de aprendizado, baixa incidência de complicações e obtém um bom índice de satisfação pelos pacientes, sendo uma ótima opção de técnica para treinamento inicial de liftings faciais em serviço de residência médica.
