Intramuscular 30% polymethylmethacrylate (PMMA) implants in a non-protein vehicle: an experimental study in rats

ABSTRACT

Introduction: In the present study, the stability and biocompatibility of a 30% polymethylmethacrylate (PMMA) filling material implanted in the masseter muscle of rats were investigated according to the cytologic characteristics presented in the graft versus host reaction. Methods: The study included 20 rats, which were divided into 4 groups: groups I, II, III, and IV corresponded to animals evaluated 7, 14, 45, and 60 days after surgery, respectively. The implant was placed in the right masseter muscle at the level of the mandibular angle. Results: After 7 days, lymphoplasmacytic inflammatory infiltrates, a fibrous capsule, a large number of neutrophils, macrophages, and exudate were observed. The second group (14 days) showed granulation tissue composed of a lymphoplasmacytic inflammatory infiltrate, newly formed vessels, and a fibrous capsule. However, the second group also exhibited regeneration of the muscle fibers, and a decreased number of neutrophils and exudate. After 45 and 60 days, the inflammatory infiltrate decreased in intensity compared to the first 2 groups. Conclusions: The inflammatory reaction caused by PMMA is transient and does not compromise the function and the shape of the masseter muscle tissue, suggesting that PMMA is biocompatible.

RESUMO

Introdução: Este trabalho busca avaliar, em ratos, a estabilidade e a biocompatibilidade de um material de preenchimento à base de polimetilmetacrilato (PMMA) a 30% implantado no músculo masseter, por meio do padrão, e a organização reacional no tecido receptor. Métodos: Foram utilizados 20 ratos, divididos em quatro grupos: grupo I, que correspondeu ao período de 7 dias de pós-operatório; grupo II, de 14 dias; grupo III, de 45 dias; e grupo IV, de 60 dias. O implante foi realizado no músculo masseter direito, na região do ângulo da mandíbula. Resultados: No período de 7 dias, observou-se presença de infiltrado inflamatório linfoplasmocitário, com formação de cápsula fibrosa e presença de grande número de neutrófilos, macrófagos e formação de exsudatos. Em 14 dias, observou-se a formação de um tecido de granulação composto por infiltrado inflamatório linfoplasmocitário, vasos de neoformação e cápsula fibrosa. Porém, nesse tempo experimental, nota-se a regeneração das fibras musculares e a diminuição do número de neutrófilos e exsudatos. Após 45 dias e 60 dias, observou-se, no tecido muscular, diminuição da intensidade do infiltrado inflamatório, comparativamente aos tempos experimentais anteriores. Conclusões: A reação inflamatória provocada pelo PMMA é transitória e não compromete as funções e o contorno desse tecido muscular, o que sugere que o PMMA é biocompatível.

Fat embolism syndrome secondary to the use of polymethylmethacrylate in bioplasty: a systematic review

ABSTRACT

Fat embolism syndrome (FES) is defined as the presence of fatty emboli, not only in the pulmonary system, but also in the systemic circulation, with inflammatory, neurological and cutaneous manifestations. Currently, in cosmetic surgery, the polymethylmethacrylate (PMMA) material is indicated for skin filling techniques used in injectable form in small non-incisional aesthetic procedures, known as bioplasty. The objective of this study was to carry out a systematic review of the literature to show clinical studies that addressed the appearance of fat embolism syndrome in patients who underwent PMMA (polymethylmethacrylate) infiltration in bioplasty. Even with the exhaustion of sources and the expansion of systematic research that followed the precepts of the PRISMA protocol, the answer was only one article that was added to the present study. Unfortunately, no reviews or experimental studies are looking at FES in PMMA infiltration. Most of the available reports are about liposuction and correlate with bone trauma when PMMA use is mentioned. This review serves as a wake-up call for nonscientific studies and reports on the risks of using PMMA in bioplasty, but no reports or articles were obtained related to life-threatening fat embolism.

RESUMO

A síndrome da embolia gordurosa (SEG) é definida como a presença de êmbolos gordurosos, não apenas no sistema pulmonar, como também na circulação sistêmica, com manifestações inflamatórias, neurológicas e cutâneas. Na cirurgia estética, atualmente, o material polimetilmetacrilato (PMMA) apresenta indicação para técnicas de preenchimento cutâneo, utilizado na forma injetável em pequenos procedimentos estéticos não incisionais, conhecidos como bioplastia. O presente estudo teve como objetivo fazer uma revisão sistemática de literatura para mostrar estudos clínicos que abordassem a ocorrência de síndrome da embolia gordurosa em pacientes que se submeteram a infiltração de PMMA (polimetilmetacrilato) na bioplastia. Mesmo com esgotamento de fontes e ampliação da pesquisa sistemática que seguiu os preceitos do protocolo PRISMA, a resposta foi de apenas um artigo que foi adicionado ao presente estudo. Infelizmente não há revisões ou estudos experimentais que contemplem a SEG na infiltração com PMMA. A maioria dos relatos disponibilizados é proveniente de lipoaspiração e correlacionados ao trauma ósseo quando citado o uso de PMMA. Esta revisão serve de alerta para os estudos e relatos não científicos sobre os riscos da utilização de PMMA na bioplastia, mas não se obteve relatos ou relacionados com a embolia gordurosa, potencialmente fatal.

Facial filling with polymethylmethacrylate in patients with acquired immunodeficiency syndrome

ABSTRACT

INTRODUCTION: Patients with acquired immunodeficiency syndrome (AIDS) who use highly active antiretroviral therapy (HAART) can develop lipodystrophy syndrome, for which facial filling with polymethylmethacrylate is a treatment option. The objective is to analyze the procedure of facial filling and evaluate patients in relation to their perception, discomfort, revelation of the diagnosis to third parties, expectation concerning facial filling, and satisfaction with the treatment outcome and its impact on their lives. METHODS: Sixty-three patients who underwent facial filling were evaluated. Procedures performed between January and July 2009 were assessed, the records of the patients were analyzed, and the outpatient lipodystrophy protocol of the STD/AIDS and Viral Hepatitis Municipal Program of São Bernardo do Campo was used. RESULTS: All the 63 patients who agreed to participate in the research completed the study. Only 6 patients (9.5%) were from other municipalities, while 57 patients (90.5%) were residents of São Bernardo. Of the patients, 68.2% were men and 100% were Caucasian. The mean age of the patients was 49.7 years. Human immunodeficiency virus was diagnosed 11.5 years prior on average, with 10-year average use of HAART and 3.8-year average time of facial lipoatrophy. Most of the patients used stavudine and/or efavirenz. The patients themselves felt more uncomfortable with facial changes. Among the patients, 85.7% did not reveal the diagnosis to third parties. CONCLUSION: All of the patients were satisfied or very satisfied with the result obtained, which had a favorable impact on their lives. The filling surgical procedure had no adverse effects.

RESUMO

INTRODUÇÃO: Pacientes que vivem com síndrome da imunodeficiência adquirida (AIDS) em uso da Terapia Antirretroviral de Alta Potência (TARV) são suscetíveis a desenvolver síndrome lipodistrófica. O preenchimento facial com polimetilmetacrilato é opção de tratamento. O objetivo é analisar o procedimento de preenchimento facial e avaliar os pacientes em relação à percepção, incômodo, revelação do diagnóstico, expectativa quanto ao preenchimento e a satisfação e impacto em suas vidas. MÉTODOS: Análise em 63 pacientes submetidos ao preenchimento facial. Foram realizados procedimentos, analisados prontuários dos pacientes e o Protocolo do Ambulatório de Lipodistrofia do Programa Municipal de doenças sexualmente transmissíveis (DST)/AIDS e Hepatites Virais de São Bernardo do Campo, atendidos no período de janeiro a julho de 2009. RESULTADOS: Todos os 63 pacientes iniciais que concordaram em participar da pesquisa permaneceram até o término deste trabalho. Apenas seis pacientes (9,5%) eram de origem de outros municípios, enquanto 57 pacientes (90,5%) eram moradores de São Bernardo. 68,2% eram homens e 100% brancos. A média das idades foi 49,7 anos. Em média, o Vírus da Imunodeficiência Humana (HIV) foi diagnosticado há 11,5 anos, com tempo médio de uso de TARV por 10 anos e tempo médio de lipoatrofia facial de 3,8 anos. A maioria fez uso de Estavudina e/ou Efavirenz. Quem ficava mais desconfortável com as alterações na face eram os próprios pacientes. 85,7% não revelaram o diagnóstico para terceiros. 100% dos pacientes ficaram satisfeitos ou muito satisfeitos com o resultado obtido. CONCLUSÃO: 100% dos pacientes ficaram satisfeitos ou muito satisfeitos com o resultado obtido. Em 100% dos casos houve um impacto favorável na vida. Não houve efeitos adversos ao procedimento cirúrgico de preenchimento.

Reconstruction of chest wall after resection of large tumors

ABSTRACT

INTRODUCTION: Complex procedures and implants are required in the final stages of chest wall reconstruction after tumor excision. This process requires multidisciplinary care with participation from thoracic and plastic surgeons, a radiologist, and a physical therapist. The goal of this study was to describe the options for chest wall reconstruction after neoplasm resection at Hospital Sarah Brasilia. METHOD: A retrospective study of one-time chest wall reconstruction after tumor excision, respiratory physical therapy with noninvasive ventilation, and exercises was conducted. RESULTS: Between 2007 and 2012, 10 patients underwent surgery (seven men, three women; age range: 10-31 years); eight patients had metastatic thoracic tumors (e.g., osteosarcoma, synovial sarcoma, sclerosing epithelioid fibrosarcoma, and rhabdomyosarcoma) and two had tumors originating from the chest wall (fibromatosis and chondrosarcoma). The outcomes were good after the immediate postoperative period, with extubation occurring at the end of surgery and chest tube removal between the fifth and eighth postoperative day. Three cases (30%) involved complications of atelectasis (10%), tumor recurrence (10%), or death. CONCLUSION: One-time chest wall reconstruction using polypropylene mesh, polymethylmethacrylate, and muscle flaps was possible and was associated with early recovery of pulmonary function and a low rate of immediate complications.

RESUMO

INTRODUÇÃO: Nos estágios finais da reconstrução torácica, consequente a exéreses tumorais, são necessários procedimentos complexos e implantes. O que requer cuidados multidisciplinares, com a participação dos cirurgiões torácicos, plástico, radiologista e fisioterapeuta. O objetivo foi descrever as opções de reconstrução torácica após ressecção de neoplasia, realizado no Hospital Sarah Brasília. MÉTODO: Estudo retrospectivo de reconstrução torácica em tempo único, após excisão de tumor, fisioterapia respiratória com ventilação não invasiva e exercícios. RESULTADOS: Entre 2007 a 2012 foram operados 10 pacientes, sete homens e três mulheres; idade 10 a 31 anos; oito apresentavam tumores torácicos metastáticos (osteosarcoma, sinoviosarcoma, Fibrosarcoma epitelioide esclerosante e Rabdomiosarcoma) e dois originários da parede torácica (fibromatose e condrosarcoma). Observou-se boa evolução no pós-operatório imediato, com extubação ao final da cirurgia, retirada do dreno torácico entre 5° e 8° PO. As complicações foram: atelectasia (10%), recorrência tumoral (10%), e óbito em 3 (30%) casos . CONCLUSÃO: Foi possível a reconstrução torácica em tempo único utilizando tela de polipropileno, polimetilmetacrilato e retalhos musculares, com recuperação precoce da função pulmonar e baixo índice de complicações imediatas.

Use of a carbon dioxide laser in the treatment of delayed polymethylmetacrylate reaction in the face: a case report

ABSTRACT

The use of synthetic implants as dermal facial fillers may cause adverse reactions, which may occur owing to inappropriate techniques or the intrinsic nature of the product. Polymethylmetacrylate (PMMA) microspheres is one of the materials used. This is a report of a case of nodular reaction in the face of a female patient, after 15 years of Artecoll® implant use, a product composed of microspheres of bovine collagen and PMMA. She was treated with a carbon dioxide laser. The delayed effect caused by the application of synthetic implants in the face, such as Artecoll®; the limiting factors of conventional resection with a scalpel and scissors; and the option for the use of carbon dioxide laser are outlined. Highlighted are the ablative and precise functions in the removal of compromised tissue, the preservation of healthy and noble tissues, the high hemostatic potential, and low morbidity, with smaller scars, edema, ecchymosis, and rapid postoperative recovery.

RESUMO

O uso de implantes sintéticos para o preenchimento na face pode acarretar reações adversas. Estas podem ocorrer da má técnica ou decorrentes do produto. Entre os materiais utilizados, está o polimetilmetacrilato (PMMA) na forma de microesferas. Trata-se do relato de um caso de reação nodular na face de paciente do sexo feminino, após 15 anos do uso de implante de Artecoll®, produto composto por microesferas de colágeno bovino e PMMA. Foi tratada com laser de gás carbônico. Evidenciou-se o efeito tardio provocado da aplicação de implantes sintéticos na face como o Artecoll®, os fatores limitantes para ressecção convencional com bisturi e tesoura, e a opção da utilização do laser de CO2. Destacando-se a função ablativa e precisa na remoção do tecido comprometido, a preservação dos tecidos sadios e nobres, o grande potencial hemostático e a baixa morbidade, com menor cicatriz, edema, equimose e a rápida recuperação pós-operatória.
