Repositioning the malar fat pad in facelifting: a simple technique of suspention and fixation

ABSTRACT

Background: The author proposes a simple technique of suspension and fixation of malar fat pad, based on the modern concepts of process of aging and its treatment. Method: The malar fat pad is directly supported and elevated by 4 slings of nylon threads fixed in a hole made on the frontal process of maxillary bone above the alar groove, and laterally, fixed on the temporal fascia and the periosteo of lateral rim. Results: The repositioning and volumetric distribution of midface, jawline and lower eyelid are quite satisfactory. The main complain is moderate and temporary pain, with no other major complications. Discussion: The vector of tissue displacement is upward and concentric, opposing the aging process, and the fixation is consistent. Conclusions: The procedure is simple, safe and efficient, in accordance with the current conceptions, however, longer follow-up analyses is needed.

RESUMO

Introdução: O autor propõe uma técnica simples de suspensão e de fixação do terço médio na cirurgia de rejuvenescimento facial, baseada nos conceitos atuais de etiopatogenia do envelhecimento e de tratamento cirúrgico. Método: A técnica consiste em fixar quatro fios de nylon medialmente no osso da abertura piriforme por meio da confecção de um orifício, e passar os fios em forma de alças lateralmente ao sulco nasogeniano, laçando as estruturas ptosadas, e tracionando e fixando na fáscia temporal e no rebordo lateral da órbita, elevando assim o tecido malar. Resultados: No malar, na linha da mandíbula e na região peri-orbitária, o reposicionamento e a distribuição volumétrica foram bastante satisfatórios. A principal queixa foi de dor moderada e passageira, sem outras complicações relevantes. Discussão: A técnica aborda diretamente as estruturas específicas e a tração é vertical e concêntrica, e fixação consistente. Conclusão: A técnica é simples, segura e eficiente, em concordância com os conceitos modernos, porém, necessita de análises por períodos mais longos.

Endoscopic forehead and midface surgery: eight years experience

ABSTRACT

In the early 90's, plastic surgery videoendoscopic technique was introduced and accepted as a effective method of forehead rejuvenation, mainly for brow and frontal area elevation. This procedure presents multiple indications and is performed to correct brow ptosis and frown lines caused by the depressor muscles. From January 1996 to December 2004, 207 patients were submitted to videoendoscopic forehead surgery in association or not with midface surgery with the some technique. Results were satisfactory with maintenance of brow elevation, decrease of glabelar wrinkles, superiorlateral rotation of oculo-palpebral elements and elevation of midface.

RESUMO

Desde o início dos anos 90, a técnica videoendoscópica em cirurgia plástica foi introduzida e aceita como método eficaz para o rejuvenescimento do terço superior da face, principalmente para elevação dos supercílios e região frontal. Este procedimento apresenta múltiplas indicações, sendo realizado para corrigir a ptose dos supercílios e rugas glabelares causadas pelos músculos depressores. No período de janeiro de 1996 a dezembro de 2004, 207 pacientes foram submetidas à cirurgia endoscópico da região frontal, associando ou não o tratamento do terço médio da face pela mesma técnica. Os resultados obtidos foram satisfatórios com manutenção da elevação dos supercílios, redução das rugas de expressão de glabela, rotação dos elementos óculo-palpebrais no sentido látero-superior e elevação do terço médio da face.

Mini-invasive facelift, an effective procedure through small incisions

ABSTRACT

INTRODUCTION: A restoring procedure of the ageing process of the face should consider the conditions of the soft tissues and the relationship between them and the existing skeletal volume. This could be done through different approaches: open, endoscopic or mini-invasive. To present our concept of mini-invasive facelift, a full facelift through minimal incisions, product of eighteen years of experience. METHODS: The mini-invasive facelift is done through minimal incisions located on the frontal hairline, temporal area, triangular fossae of the auricle, submentalis sulcus and eyelids. A wide dissection is performed to liberate the parieto-temporal fascia from the zigomatic arch, to pull it up in continuity with the SMAS and the platysma as one continuous layer or composite flap. These structures are selectively fixed to a higher position restoring the location of the soft tissues and the contour of the face. RESULTS: We have done this procedure to 672 patients, along 19 years, with minimal complications (described also), and avoiding the pre-auricular incisions in 89% of our patients. In this paper details and tips to perform this procedure in a safer way and to get the most natural results are presented. CONCLUSIONS: Different approaches are described to perform a face lift with specific mini access views according to the regions to be treated , without periauricular scar round 89% of the operated cases.

RESUMO

INTRODUÇÃO: Os procedimentos aplicados nos problemas de envelhecimento facial devem ficar restritos aos tecidos brandos e às relações com o volume prévio do esqueleto. Eles podem ser realizados mediante diversas técnicas: dissecção ampla e aberta, endoscópica e mini-invasiva. O objetivo é apresentar nossa concepção de ritidoplastias mini-invasivas e ritidoplastias completas através de mini-incisões, após 18 anos de experiência. MÉTODOS: A ritidoplastia mini-invasiva é realizada através de mini-incisões localizadas na linha do cabelo da região frontal, região temporal, na fossa triangular da orelha, no sulco submentoniano e nas pálpebras. Ampla dissecção é realizada para liberar a fáscia parietal do arco zigomático para elevar em continuidade com o SMAS e o platisma num único plano. Essas estruturas são seletivamente fixadas em posições mais elevadas, restaurando a posição dos tecidos brandos e o contorno da face. RESULTADOS: Nossa experiência se apoia em 672 pacientes, operados durante 19 anos, com mínimo de complicações a serem registradas, tendo sido evitadas as incisões pré-auriculares em 89% dos pacientes. Estão registrados os detalhes e táticas de como realizar cirurgias com segurança e a qualidade dos resultados. CONCLUSÕES: Diferentes procedimentos são descritos para mostrar como realizamos as ritidoplastias com minivias de acesso, de acordo com as regiões tratadas e sem cicatrizes pré-auriculares em 89% dos casos operados.

Hair transplantation as an alternative for correction of sideburn defects after rhytidoplasty

ABSTRACT

Background: The elevation of the temporal hairline - the sideburn - is one of the most frequent stigmas of a rhytidoplasty. This displacement is intrinsic to the surgery, and is due either to the upper and lateral traction of the flap, or secondary to the superficial undermining of the flap leading to permanent damage of the hair follicle. The intention of this study is to analyze the reconstruction of the temporal hairline after a face lifting through transplantation of hair grafts. Method: The authors follow up the patients who were submitted to a hair graft surgery to correct the elevation of the sideburn after the face lifting that was done in the Plastic Surgery Service of Professor Ivo Pitanguy, between January 2003 and October 2004. The aesthetic result was evaluated, as well as the rate of complications and the satisfaction of the patients. Results: Twenty five patients were evaluated. In the follow up of six and twelve months all patients had an important improvement in aesthetic appearance. The complications were the widening of the donor area scar (4%) and telogen efluvium (28%). Only 8% of the patients were submitted to a second stage of hair transplantation to increase capillary density. Conclusions: The authors conclude that transplantation of follicular grafts is an efficient and safe alternative to correct the elevation of the sideburn after a face lifting, bringing good aesthetic results.

RESUMO

Introdução: A elevação da linha de implantação pilosa temporal - a costeleta - é um dos principais estigmas de pacientes submetidos a ritidoplastias. Este deslocamento pode ser inerente ao procedimento, devido à tração súpero-lateral do retalho, ou pode resultar de dano permanente dos folículos pilosos, por um descolamento superficial do retalho facial. O objetivo deste estudo é analisar a correção da elevação da costeleta pós-ritidoplastias através da técnica de transplante folicular. Método: Os autores acompanharam prospectivamente os pacientes submetidos a transplante folicular, para a correção do apagamento da costeleta pós-ritidoplastias, realizados no Serviço de Cirurgia Plástica do Professor Ivo Pitanguy, no período de janeiro de 2003 a outubro de 2004. Foram avaliados os resultados estéticos, as complicações, assim como o grau de satisfação dos pacientes. Resultados: Vinte e cinco pacientes do sexo feminino foram analisadas. No acompanhamento tardio, realizado seis e doze meses após a cirurgia, todas as pacientes obtiveram melhora estética importante. Como complicações, identificamos 4% de alargamento da cicatriz da área doadora e 28% eflúvio telógeno. Apenas 8% das pacientes foram submetidas a um novo procedimento para aumentar a densidade capilar. Conclusões: Os autores acreditam que o transplante folicular é uma alternativa válida, segura e que apresenta resultados satisfatórios na correção da elevação da costeleta pósritidoplastias.

Segmented scalp peninsula to prevent a raised hairline and misalignment in rhytidoplasty

ABSTRACT

INTRODUCTION: Rhytidoplasty has shown excellent results in the treatment of facial laxity over the years, but in a few cases in which the patient has increased facial laxity associated with high preauricular scalp implantation, we observed elevation of implantation above the root of the helix after flap traction, resulting in an unesthetic facial appearance and marring the surgical procedure. The objective of this study was to demonstrate a surgical technique designed to prevent hairline recession and maintain correct alignment for the facelift. METHODS: Of 25 patients who underwent rhytidoplasty, 12 received a segmented scalp peninsula. Descriptive analysis in this group included age, level of scalp implantation, surgical time, elevation of the hairline, necrosis, postoperative hematoma, unesthetic scars, and neuropraxia. RESULTS: We analyzed the results of a segmented scalp peninsula in 12 patients, ranging in age between 41 and 74 years, with a mean of 60 years. The return to usual activities ranged from 10 to 21 days. No unesthetic changes resulted from placement of the hairline on the sideburn in these 12 patients; there was one (4%) case of hematoma, but no case of necrosis or neuropraxia. CONCLUSION: The segmented scalp peninsula is an easily performed technique, with a low risk of complications and with satisfactory esthetic results.

RESUMO

INTRODUÇÃO: A ritidoplastia tem demonstrado excelentes resultados no tratamento da flacidez facial ao longo dos tempos, porém em alguns casos selecionados, nos quais o paciente apresenta flacidez facial acentuada associada com implantação capilar pré-auricular alta, observamos, após a tração dos retalhos, uma ascensão da implantação capilar acima da raiz da helix, causando um aspecto inestético facial e configurando um estigma cirúrgico para o procedimento. O objetivo desse estudo é demonstrar uma técnica cirúrgica com intuito de prevenir a ascensão capilar e manter o seu correto alinhamento, no lifting facial. MÉTODOS: Foram operados 25 pacientes, submetidos à ritidoplastia, dos quais 12 realizamos a península capilar segmentada. Foi realizada uma análise descritiva desse grupo, quanto à idade, altura da implantação capilar, tempo cirúrgico, elevação do "pé do cabelo", necrose, hematoma pós-operatório, cicatrizes inestéticas e neuropraxia. RESULTADOS: Em relação à população analisada, realizamos a península capilar segmentada em 12 pacientes, ou seja, 48%, a idade variou entre 41 e 74 anos, sendo a média de 60 anos. O retorno às atividades habituais variou de 10 a 21 dias. Não ocorreram alterações inestéticas da implantação da linha capilar na "costeleta" nos 12 pacientes submetidos à península capilar segmentada, houve um (4%) caso de hematoma, nenhum caso de necrose ou neuropraxia. CONCLUSÃO: A península capilar segmentada demonstrou ser uma técnica de fácil execução, baixo índice de complicações e com resultados estéticos satisfatórios.

Temporary hemostatic traction (THT) in rhytidoplasty

ABSTRACT

INTRODUCTION: To slow the aging process, facial contouring surgery was developed as a rejuvenating technique, and is constantly improving, as are the results. However, as with any surgical procedure, complications are possible, the main one being hematoma. Dissatisfied with the prospect of hematoma and the use of drains, and with a goal of lower costs, the authors felt the need to develop a new surgical procedure, temporary hemostatic traction (THT). METHODS: Sixty-four rhytidoplasties, all performed at the Valle Pereira Clinic (Florianópolis, SC, Brazil) from May 2012 to January 2013, were included in this study. All patients underwent the new THT procedure. RESULTS: Only two patients developed epidermolysis with optimal evolution following THT, and one developed generalized facial edema. There were no instances of hematoma at any of the postoperative stages. CONCLUSIONS: This new surgical technique avoids the use of drains and decreases dead space, thereby improving flap adhesion, and decreases hematoma, seroma, and postoperative edema; it also decreases tension on the incision, hence improving the results of rhytidoplasty.

RESUMO

INTRODUÇÃO: Na tentativa de desacelerar o processo de envelhecimento, as cirurgias de contorno facial foram estabelecendo-se como técnicas rejuvenescedoras, sendo constantemente aprimoradas, assim como os resultados. Porém, como qualquer procedimento cirúrgico, há a possibilidade de intercorrências e a principal complicação delas é o hematoma. Descontentes com a presença desta complicação, com o uso de drenos e visando menor custo, os autores viram a necessidade de desenvolver uma nova tática cirúrgica chamada tração hemostática temporária - THT. MÉTODOS: Foram incluídas neste estudo todas as ritidoplastias realizadas na Clínica Valle Pereira (Florianópolis, SC), no período compreendido entre maio de 2012 e janeiro de 2013, totalizando 64 pacientes. Todos foram submetidos à nova tática cirúrgica chamada tração hemostatica temporária. RESULTADOS: Apenas 2 pacientes apresentaram epidermólise com ótima evolução sob a THT, e 1 caso de edema facial generalizado. Não houve caso com hematoma, em qualquer período pós-operatório. CONCLUSÕES: esta nova tática cirúrgica evita o uso de drenos, diminui o espaço morto aumentando a adesão do retalho descolado e, consequentemente, diminui a presença de hematoma, seroma e edema pós-operatório, assim como diminui a tensão cicatricial na incisão da pele, visando aprimorar os resultados das ritidoplastias.

Forehead reduction using precapillary incision: experience report and indications

ABSTRACT

BACKGROUND: A long forehead can give a less attractive and disproportionate appearance. It may also indicate aging. The objective of this study is to show the experience of the authors in forehead reduction using precapillary incision and to support the indications for this procedure. METHODS: Patients who underwent precapillary incision for forehead reduction between 2005 and 2011 were retrospectively analyzed. The indications for an anterior hairline incision were eyebrow ptosis in patients with thin and sparse frontal hair, congenital long forehead, or extensive forehead wrinkles. RESULTS: Precapillary incision was performed in 31 patients, who were followed up for an average of 1.5 years. No problems related to flap vascularization were detected. All patients reported temporary paresthesia, from which they recovered within 1 year. Three patients presented seromas during the postoperative period and were treated using puncturing. All patients reported that the benefits of altering the hairline were greater than the disadvantages of a possibly more visible scar. CONCLUSIONS: Frontal rhytidectomy using precapillary incision is recommended in patients who wish to correct thin and sparse frontal hair, extensive forehead wrinkles, or congenital/ senile long forehead.

RESUMO

INTRODUÇÃO: A testa longa pode dar uma aparência menos atraente, desproporcional e caracterizar o envelhecimento. O objetivo deste trabalho é demonstrar a experiência dos autores na redução da região frontal com incisão pré-capilar e reforçar as indicações desse procedimento. MÉTODO: Foram estudados, retrospectivamente, pacientes submetidos a redução da região frontal por incisão pré-capilar, no período de 2005 a 2011. As indicações para incisão anterior na linha do cabelo foram ptose da sobrancelha, em pacientes que possuíam cabelos frontais finos e escassos, testa longa congênita ou enrugamento amplo de testa. RESULTADOS: A incisão pré-capilar foi realizada em 31 pacientes, com acompanhamento médio de 1,5 ano. Não houve problemas relacionados à vascularização do retalho. Todos os pacientes relataram parestesia temporária, com recuperação em até 1 ano. Três pacientes apresentaram seromas no pós-operatório, tratados com punção. Todos os pacientes relataram que os benefícios da redução da linha do cabelo ultrapassaram as desvantagens de uma cicatriz possivelmente mais visível. CONCLUSÕES: O procedimento de ritidectomia frontal com incisão pré-capilar é indicado para pacientes com cabelos frontais finos e escassos, com enrugamento amplo da testa ou com testa longa congênita/senil e que desejem reduzi-la.

Temporofrontal galea periosteal lifting with double access: a conservative procedure

ABSTRACT

BACKGROUND: Surgical treatment of the superior third of the face should usually involve an extensive area, including the frontal and temporal regions. However, the open coronal technique has been avoided because it carries a risk of potential complications and a wide scar. The technique has been supplanted by endoscopic and other less-invasive techniques, which are perhaps less efficacious. This article proposes an open technique that maintains the coronal lifting efficiency without its typical complications. METHODS: A retrospective analysis of 20 consecutive female patients who underwent cervicofacial facelift from February 2008 to July 2011 was performed. The patients had a mean age of 53.3 years (range, 46-71 years). The sensitivity of the regions covered (i.e., anesthesia, hypoesthesia, hyperesthesia, paresthesia), motor function of the temporal branch of the facial nerve (i.e., frontal muscle movement), and healing (i.e., widening and alopecia) were evaluated. RESULTS: There were no sensory, motor, or vascular complications. Moreover, neither cicatricial enlargement nor alopecia was observed. One patient required revision of the corrugator muscle myectomy, and another underwent myectomy of the lateral orbicularis muscle of the eye for optimal results. CONCLUSIONS: The proposed technique is simple, safe, does not require special equipment or devices, and has the advantage of wide and harmonious distribution of the flap, including the temporal and frontal regions.

RESUMO

INTRODUÇÃO: O tratamento do terço superior da face deve ser amplo, englobando as regiões frontal e temporal. Entretanto, em decorrência das potenciais complicações e da cicatriz extensa, o lifting coronal tem sido evitado e substituído por técnica endoscópica e procedimentos menos invasivos, porém, talvez, menos eficazes. O objetivo deste estudo é propor uma tática cirúrgica com eficácia similar à do lifting coronal, sem, no entanto, as complicações típicas desse procedimento. MÉTODO: Foi realizada análise retrospectiva de 20 pacientes do sexo feminino, consecutivas, com média de idade de 53,3 anos, submetidas a lifting cervicofacial, no período de fevereiro de 2008 a julho de 2011. Foram avaliadas a sensibilidade da região abordada (anestesia, hipoestesia, hiperestesia, parestesia), a função motora do ramo temporal do nervo facial (movimento do músculo frontal), e a cicatrização (alargamento e alopecias). RESULTADOS: Não houve complicações sensitivas ou motoras, bem como vasculares. Também não foram observados alargamento cicatricial ou alopecias. Uma paciente precisou de revisão da miectomia dos músculos corrugadores e outra foi submetida a miectomia do músculo orbicular lateral do olho, para otimização do resultado. CONCLUSÕES: A tática é segura, simples, não necessita de equipamentos ou materiais especiais, e tem a vantagem da ampla e boa distribuição cutânea, englobando as regiões frontal e temporal.

Needle for internal stitches in tissue to promote its suspension

ABSTRACT

Introduction: The author proposes an alternative for suspension of the layer malar fat pad of the middle third of the face, through least minimal incisions and to hide, using a needle created specifically for this finality. Method: The technique consists in "catching" and elevate a portion of cloth, without leaving sequel in the glabrous skin, through incision in pilous zigomatic region of about 1.5 cm, using from 2 to 3 points of sustenance. Results: Ten patients were selected with age between 35 and 45 years and degree of cutaneous aging II/III of Glogau, pointing of the treated areas ptosed, with improvement of the outline, volume and definition of the treated regions. Conclusion: Because of being simple, not much invasive, reversible method and of good results, it can be used like primary or complementary treatment of surgeries of facial rejuvenation, needing, however, evaluation of the results on middle and long term.

Introdução: O autor propõe uma alternativa para suspensão da camada fibroadiposa do terço médio da face, através de incisões mínimas e dissimuláveis, utilizando uma agulha criada especificamente para esta finalidade. Método: A técnica consiste em "fisgar" e tracionar uma porção de tecido, sem deixar seqüela na pele glabra, através de incisão em região pilosa zigomática de aproximadamente 1,5 cm, usando de 2 a 3 pontos de sustentação. Resultados: Foram selecionadas 10 pacientes com idade entre 35 e 45 anos e grau de envelhecimento cutâneo II/III de Glogau, observando-se elevação das áreas ptosadas, com melhora do contorno, volume e definição das regiões tratadas. Conclusão: Por ser um método simples, pouco invasivo, reversível e de bons resultados, pode ser usado como tratamento primário ou complementar de cirurgias de rejuvenescimento facial, necessitando, porém, de avaliação dos resultados a médio e longo prazo.

ABSTRACT

Introduction: Cervicoplasty is considered an important part of facial rejuvenation. Despite the various techniques already described, the search for the ideal treatment is ongoing. The technical difficulties increase in complex cases, in which excessive skin sagging and pronounced muscular ptosis affect the maintenance of long-term results. Objectives: The aim of this study was to assess the suitability of the authors' protocol for extended cervicoplasty and to discuss the technical options available for this procedure. Methods: Eighteen patients underwent "round lifting" associated with platysma flap elevation at the midcervical line and partial subplatysmal fat removal. The patients were followed for 12 months and the late postoperative outcomes were assessed by 5 surgeons of the Brazilian Society of Plastic Surgery. Results: Four patients were excluded because they were lost to follow-up. Of the 14 assessed patients, 7 (50%), 5 (35.7%), and 2 (14.3%) patients had excellent, moderate, and poor outcomes, respectively. No patient had a perfect or bad outcome. Conclusion: Extended cervicoplasty yielded satisfactory results even in difficult cases. The proposed technique is a valid alternative for the plastic surgeon, because it ensures a pleasant and natural outcome that is maintained over time.

Introdução: A cervicoplastia é considerada parte importante na cirurgia do rejuvenescimento facial. Apesar das diversas técnicas já descritas, a busca pelo tratamento ideal ainda persiste. As dificuldades aumentam nos casos considerados complexos, em que a flacidez excessiva de pele e a ptose muscular pronunciada prejudicam a manutenção dos resultados a longo prazo. Objetivos: No intuito de discutir as opções técnicas disponíveis para a cervicoplastia e apresentar a rotina cirúrgica utilizada pelo autor - com ênfase no tratamento ampliado sobre o músculo platisma - é que este estudo foi delineado. Método: Dezoito pacientes foram submetidos ao round lifting associado à confecção de retalhos musculares de platisma, na linha médio-cervical, e à retirada parcial da gordura subplatismal. Os pacientes foram acompanhados por 12 meses e o resultado pós-operatório tardio foi avaliado por cinco cirurgiões plásticos da SBCP. Resultados: Quatro pacientes foram excluídos do estudo por não retornarem para o acompanhamento pósoperatório tardio. Dos 14 pacientes avaliados, 7 (50%) tiveram o resultado classificado como excelente, 5 (35,7%) como moderado e 2 (14,3%) como fraco. Nenhum paciente teve o resultado classificado como perfeito ou ruim. Conclusão: A cervicoplastia ampliada proporcionou a obtenção de resultados satisfatórios, mesmo em casos difíceis. Acreditamos que a técnica é uma alternativa válida para o cirurgião plástico, podendo contribuir para que obtenhamos um resultado pós-operatório agradável, natural e mantido com o tempo.

Frontal and glabellar myotomy: single short frontal scar approach

ABSTRACT

Background: Frontal and glabellar muscle hyperactivity with consequent wrinkles are frequent complaints in plastic surgery. Treatment consists of impediment of related muscles; this can be achieved by application of botulinum toxin or surgery by means of classic coronal incision, endoscopic approach, transblepharoplasty access, or non-endoscopic limited approaches. This paper proposes an alternative of limited approach in which an incision is made on the central frontal scalp for treatment of the frontal and glabellar muscles. Methods: Between March 2009 and April 2010, 20 female patients were treated using a small incision on the frontal scalp, followed by dissection of frontal and glabellar regions. Muscles and supratrochlear and supraorbital nerve branches were identified; subsequently, frontal and glabellar muscle myotomy was performed. Results: All patients showed good results by frontal and glabellar muscle impediment during the evaluation period, with improvement in wrinkles and hyperactivity. Complications consisted of temporary paresthesia and pruritus in the central frontal region and scalp, in addition to recurrence of partial muscle contraction. Conclusions: The small incision approach on the central frontal scalp for treatment of frontal and glabellar muscles seems to be a good alternative to other surgical options due to its small size, inconspicuous scarring, and direct visualization of frontal and glabellar anatomic structures, allowing selective myotomy or myectomy.

Introdução: A hiperatividade da musculatura frontoglabelar, com suas consequentes rugas, são queixas frequentes em consultórios de cirurgia plástica. O tratamento consiste de impedimento desses músculos, o que pode ser obtido com a aplicação de toxina botulínica ou cirurgicamente, por incisão coronal clássica, acesso endoscópico, acesso transblefaroplastia ou acessos limitados não-endoscópicos. Neste artigo é proposta uma alternativa de acesso limitado, por incisão pós-capilar central frontal (IPCF), para tratamento das musculaturas frontal e glabelar. Métodos: Entre março de 2009 e abril de 2010, 20 pacientes do sexo feminino foram submetidas a IPCF, seguida de dissecção das regiões frontal e glabelar. Os músculos e os nervos supratroclear e supraorbitário foram identificados, sendo, em seguida, realizada miotomia dos músculos frontoglabelares. Resultados: Todos os casos apresentaram bons resultados pelo impedimento da musculatura frontoglabelar no período avaliado, com melhora das rugas e da hiperatividade. As complicações observadas foram temporárias e incluíram parestesia e prurido na região frontal central e no escalpe, além de recorrência parcial da contração muscular. Conclusões: A IPCF parece ser uma boa alternativa para o tratamento das musculaturas frontal e glabelar, pela pequena incisão, pela cicatriz inaparente e por permitir visão direta das estruturas anatômicas das regiões frontal e glabelar, permitindo miotomia ou miectomia seletiva.

A rhytidoplasty technique without resultant periauricular scarring

ABSTRACT

BACKGROUND: Surgical skin incisions in rhytidoplasty are very diverse compared to their original form approximately 100 years ago, when the operation was first performed. Many strategies have been proposed to improve this procedure so as to reduce scarring, anatomic distortion of the external ear, and hair loss. To diminish the inconveniences of periauricular scars and hair loss and in order to apply the technique to patients of different age groups, rhytidoplasty without preauricular and retroauricular incisions were performed using 2 surgical access points: the hairline on the temporal region and the submental region. The aim of this study is to report the results obtained using this rhytidoplasty technique which avoided periauricular scarring. METHODS: From October 2008 to February 2012, 228 patients (80% female) underwent rhytidoplasty performed by means of a short incision instead of a coronal incision. Patients received cervical treatment through a submental access, which facilitated an extensive selective dissection linked to the midface plane for platysma treatment and, when necessary, resection of excess skin. SMAS-plasty, liposuction, fat graft, and laser treatment, in addition to other non-surgical procedures, were selectively combined in the same procedure or an additional procedure. The procedure is contraindicated in patients who present after extensive weight loss and with excessive flaccidity of facial and cervical skin. RESULTS: Overall, the aesthetic results obtained with rhytidoplasty without periauricular scarring were considered satisfactory both by the patients and the surgical team and provided good mid-and long-term results. Few complications were observed after surgery; these included asymmetry in the temporal region and non-aesthetic scars, which were detected in 2.6% and 2.2% of the patients, respectively. CONCLUSIONS: Rhytidoplasties without periauricular scarring that were performed through submental and precapillary incisions in the temporal region provided good mid-to long-term results. This justified their use, particularly if we consider that treatment of the midface plane and cervical region might prevent the development of possible complications described in the literature.

RESUMO

INTRODUÇÃO: As incisões cutâneas de acesso nas ritidoplastias têm apresentado grande diversidade desde os seus primórdios, nas duas primeiras décadas do século passado, até a atualidade. Numerosas também têm sido as táticas cirúrgicas em busca de refinamentos que pudessem reduzir estigmas cicatriciais, distorções anatômicas da orelha externa e perda de cabelo. No sentido de reduzir as inconveniências das cicatrizes periauriculares e das alterações pilosas ainda frequentes, com aplicabilidade numa ampla gama de pacientes de diferentes faixas etárias, foi proposta a realização da ritidoplastia sem incisão pré e retroauricular, mediante duas vias de acesso: uma no limite piloso da região temporal e outra, submentoniana. Este estudo tem por objetivo apresentar os resultados obtidos com o emprego dessa técnica de ritidoplastia sem cicatriz periauricular. MÉTODO: No período de outubro de 2008 a fevereiro 2012, 228 pacientes, sendo 80% do sexo feminino, foram submetidos a ritidoplastia mediante uma incisão curta e sem incisão coronal, no Centro Médico Imbanaco (Cali, Colômbia). Todos os pacientes receberam tratamento cervical mediante via de acesso submentoniana com ampla dissecção seletiva e interligada com o andar médio da face, para tratamento do platisma, quando necessário, e ressecção dos excessos cutâneos. Smasplastia, lipoaspiração, enxerto de gordura e laser, além de outros procedimentos não-cirúrgicos, foram utilizados concomitante e seletivamente, no mesmo ato ou em atos operatórios distintos. A técnica foi contraindicada a pacientes após grandes perdas ponderais e excessiva flacidez de pele tanto facial como cervical. RESULTADOS: Os resultados estéticos obtidos com a técnica de ritidoplastia sem cicatriz periauricular foram considerados satisfatórios pelos pacientes e pela equipe cirúrgica na totalidade dos pacientes, com manutenção a médio e longo prazos. A incidência de complicações pós-operatórias foi pequena, sendo mais comum a presença de assimetria da região temporal e de cicatrizes não-estéticas em 2,6% e 2,2% dos pacientes, respectivamente. CONCLUSÕES: As ritidoplastias com vias de acesso submentoniana e pré-capilar na região temporal, sem cicatrizes periauriculares, têm oferecido resultados a médio e longo prazos que justificam sua indicação, pelo fato de permitirem tratamento do andar médio da face e da região cervical, evitando-se as possíveis complicações descritas na literatura.

Reverse frontal lifting: alternative for the treatment of pachydermoperiostosis

ABSTRACT

Introduction: Patient diagnosed with pachydermoperiostosis, presenting a strong cutaneous manifestation, making it impossible to correct the defect by the usual facial lifting techniques. The reverse frontal facelift was the idealized technique for this case. It is an innovative technique, there being no previous publication in the literature.
Case report: Italian male patient, 56 years old, with cutaneous manifestation syndrome, especially in the frontal region of the face, with leathery inelastic skin, which generated aesthetic discomfort, predisposing for a syndromic stigma.
Methods: The technique developed for this case involves an incision, which starts at the root of the helix (point A), bypasses the eyebrow in a sinuous line distant half a centimeter from the implantation of the eyebrow hair, going to the glabella, curving towards the nasal root and going to meet identical dimensions on the other side. From point A, a curved line also leans towards the medial direction, at a distance of 2 cm. The intention is to reduce the distance between the area to be pulled and the incision area, in order to obtain more traction, thus enabling the correction of the aspect of the face in focus.
Results: Caudal traction of the flap by a supraciliary incision made it possible to correct the defect in the frontal region without altering the capillary implantation line or causing excessive eyebrow rise.
Conclusion: The reverse frontal lifting technique was created for a specific case of pachydermoperiostosis syndrome. When correctly indicated, this technique can be used to achieve good results.

Introdução: Paciente com diagnóstico de paquidermoperiostose, apresentando forte manifestação cutânea, impossibilitando a correção do defeito pelas técnicas usuais de lifting facial. O lifting frontal reverso foi a técnica idealizada para o caso. É uma técnica inovadora, não havendo publicação prévia na literatura.
Relato de caso: Paciente italiano, sexo masculino, 56 anos, apresentando síndrome de manifestação cutânea, em especial na região frontal da face, com pele inelástica de aspecto coriáceo, que gerava desconforto estético, ensejando um estigma sindrômico.
Métodos: A técnica elaborada para este caso envolve incisão, que se inicia na raiz da hélice (ponto A), contorna o supercílio em linha sinuosa distando meio centímetro da implantação dos pelos da sobrancelha, indo até a região da glabela, curvando para a raiz do nariz e indo se encontrar com dimensões idênticas do outro lado. Do ponto A, também ascende uma linha curva inclinando-se para o sentido medial, com uma distância de 2cm. A intenção é diminuir a distância entre a área a ser tracionada e a área de incisão, a fim de obter maior tração, possibilitando assim a correção do aspecto da face em foco.
Resultados: A tração caudal do retalho por incisão supraciliar possibilitou a correção do defeito em região frontal sem que houvesse alteração da linha de implantação capilar ou ascensão excessiva das sobrancelhas. Conclusão: A técnica de lifting frontal reverso foi criada para um caso específico de síndrome de paquidermoperiostose. Quando bem indicada esta técnica pode ser utilizada atingindo bons resultados.

Introduction: The surgical treatment presented by the authors has tactical innovations and unique features that make the treatment safer. Objective: To show a different approach to cheek area and fix the skin with dermal fat points. Methods: Approach the cheek area with progressive retractors, leaving bands. Skin fixation with dermal fat points using nylon 4.0 favors tension reduction in the scar. Results: Thirty-nine pacients were operated using this tactic. The period of hospitalization was 24 hours and there was only one case of hematoma. Conclusion: It is an efficient and simple method where the approach to the cheek area using retractors show low bleeding. The adhesion left by this method encourage better skin irrigation. Dermal fat points keep the skin in place and reduce the tension in the scar.

Introdução: O tratamento cirúrgico apresentado pelos autores tem inovações táticas e peculiares que tornam o tratamento seguro. Objetivo: Demonstrar uma maneira diferente de abordar a região geniana e de fixar a pele com pontos dermogordurosos. Método: Abordagem da região geniana com os descoladores progressivos, deixando bridas. A fixação da pele com pontos dermogordurosos usando fio de nylon 4.0 favorece a diminuição da tensão na cicatriz pré-auricular. Resultados: Foram operados trinta e nove pacientes usando essa tática. O período de internação foi de 24 horas, sendo observado apenas um caso de hematoma. Conclusão: O método proposto é eficiente e simples, onde a abordagem da região geniana com o uso dos descoladores é de pouco sangramento. As bridas deixadas por esse método favorecem melhor irrigação da pele. Os pontos dermogordurosos mantêm a pele no local e diminuem a tensão na cicatriz pré-auricular.

ABSTRACT

The earlobe occupies a unique position among the structures of the face and is a very important region in aesthetic composition of the ear. The age alters the shape, width and length of the lobe due to sagging tissue and thus is at odds ratio with other aesthetic elements of the ear, requiring correction. Very little has been described to guide the surgeon in rejuvenating the ear. The correction aged lobe can be made alone or in combination with surgery of facial rejuvenation, as well as by secondary lobe rhytidoplasties inadequately secured to the face. The surgery is fairly simple, rapid recovery, with interesting results. The basic aim of the work is the description of the technique and its indications. The strategic positioning of the final scar in continuity with a depression anatomical ear that is escafa, and hidden by antitragus ensures a proper outcome, with little apparent scars. The technique described is a tactical option to reshape and rejuvenate the ear.

RESUMO

O lóbulo de orelha ocupa posição única entre as estruturas da face e é uma região importantíssima na composição estética da orelha. A idade altera o formato, a largura e o comprimento do lóbulo, em decorrência da flacidez dos tecidos e, assim, fica em desarmonia de proporção com outros elementos estéticos da orelha, exigindo correção. Muito pouco tem sido descrito para orientar o cirurgião no rejuvenescimento da orelha. A correção do lóbulo envelhecido pode ser feita isoladamente ou em combinação com uma cirurgia de rejuvenescimento facial, bem como em ritidoplastias secundárias por lóbulo inadequadamente preso à face. A cirurgia é bastante simples, de recuperação rápida, com resultados interessantes. O objetivo básico do trabalho consiste na descrição da técnica de rejuvenescimento do lóbulo da orelha e suas indicações. O posicionamento estratégico da cicatriz final em continuidade com uma depressão anatômica da orelha, que é a escafa, e ocultada pelo antitrágus garante resultado final adequado, com cicatrizes pouco aparentes. A técnica descrita representa uma opção tática para reformular e rejuvenescer a orelha.

ABSTRACT

Neck rejuvenation by technique usually used in rhytidectomy is recommended for cases of patients suffering from sagging either or not associated with platisma bands with or without generalized adiposity. In cases which these areas are located in the lower cervical, and particularly near the sternal notch and clavicles, we observed the possibility of reverse cervical rhytedectomy.

Rejuvenescimento da região cervical anterior por técnica usualmente utilizada nas ritidoplastias é indicado para casos de pacientes com problemas de flacidez, associada ou não com bandas de platisma, com ou sem adiposidade localizada ou mesmo generalizada. Nos casos em que estes aspectos estão localizados no terço cervical inferior e, em particular, junto à fúrcula esternal e clavículas, observamos a possibilidade de realizar a ritidoplastia cervical reversa.

Design of a novel surgical instrument for rhytidoplasty: the SMAS suspension needle

ABSTRACT

This article describes an alternative surgical instrument that can be used for suspension of the malar fat pad. The needle was created using Kirschner wire to ensure that both tips were blunt. The double tip allows for passage of the wire through the superficial muscular aponeurotic system, ensuring that the suture is sufficiently deep to avoid dermatography. The utility of the new instrument was tested in rhytidoplasties involving minimal dissection. The needle described herein was proven safe, and could easily migrate through the tissues, without creating vascular and nerve lesions, and facilitate the positioning of the fixation points at the intended sites. The use of this needle for rhytidoplasty was quick, simple, and cost-effective, without any resulting dissection or damage to the anatomical structures or tissues.

RESUMO

Neste artigo é apresentado um instrumento cirúrgico alternativo para suspensão da camada fibroadiposa do terço médio da face. A agulha foi confeccionada com fio de Kirschner, de forma que ambas as pontas se tornaram rombas. A dupla ponta possibilita a passagem do fio pelo sistema musculoaponeurótico superficial da face (SMAS), mantendo a sutura com a profundidade necessária para evitar dermatografia. Os testes com o novo instrumento foram realizados em ritidoplastias com dissecção mínima. A agulha descrita demonstrou-se segura, com fácil migração pelos tecidos e ausência de lesões vasculonervosas, facilitando o posicionamento dos pontos de fixação nos locais planejados. A agulha desenvolvida possibilitou a realização da ritidoplastia sem descolamento resultante de forma rápida, simples e custo-efetiva, com mínimo dano às estruturas anatômicas e aos tecidos.

ABSTRACT

Primary pachydermoperiostosis is a rare disease characterized by excessive skull affixing of the periosteum, coexisting with thickening of the epidermis and dermis (pachydermia), thereby causing gross deformities. Owing to the variety of affected structures, there are several surgical options and complementary methods that are used in the treatment of facial alterations in these patients. This report describes the use of subperiosteal detachment as an aesthetic treatment option for the faces of two patients with primary pachydermoperiostosis, operated at the Hospital das Clínicas of the Federal University of Minas Gerais.

RESUMO

A paquidermoperiostose primária é uma doença rara, caracterizada por aposição excessiva do periósteo do crânio, coexistindo com espessamento da epiderme e derme (paquidermia), provocando deformidades grosseiras. Devido à diversidade de estruturas acometidas, há várias opções cirúrgicas e métodos complementares que são utilizados no tratamento das alterações faciais desses pacientes. Esse trabalho apresenta o lifting subperiosteal como uma opção de tratamento estético para a face de pacientes portadores dessa síndrome, através do relato de dois casos operados no Hospital das Clínicas da Universidade Federal de Minas Gerais.

ABSTRACT

Introduction: The peripheral facial palsy is a disease that can leave significant aesthetical and functional sequelae in patients. According to medical literature, Bell's palsy (BP) is the most common cause, representing from 50 to 80% of the cases. However, it must be considered as an exclusion diagnosis. A slightly higher prevalence is estimated in women and its incidence is bimodal with peaks in the third and eighth life decades. Most patients recover within weeks, though a worsening degree may be common in the first 48 hours.Treatment must be performed to prevent complications and the medical procedures are not universally accepted. The following have been mentioned as BP risk factors: arterial hypertension, mellitus diabetes, pregnancy, puerperium and infection caused by type I herpes virus. Although there are several BP associated risk factors, regarding face-lifting surgery there is only one reported case in medical literature. Case Report: In the present study, the authors describe the second case of Bell's palsy after face-lifting surgery.

RESUMO

Introdução: O espasmo hemifacial (EH) é um distúrbio do movimento caracterizado por contrações involuntárias dos músculos inervados pelo nervo facial, a descompressão microvascular (DMV) é considerada tratamento definitivo, com baixas taxas de recidiva. Os casos refratários tornam-se um desafio para o cirurgião plástico. Relato do Caso: MSP, sexo feminino, 53 anos, portadora de EH, refratária à DMV, e com queixas de flacidez dos dois terços inferiores da face. Foi submetida à ritidoplastia, neurectomias e miectomias seletivas. Obteve-se ausência completa dos espasmos, manutenção da mímica facial e rejuvenescimento facial. A ritidoplastia cérvico-facial associada a neurectomias e miectomias mostrou-se eficaz no tratamento do EH.
