Anatomical Study of the Superior Pedicle of Retroauricular Skin Flaps

ABSTRACT

Since the description of the flap proposed by Washio in 1969, several vascular pedicle models have been designed for the retroauricular flap, aiming chiefly to reduce the thickness of the pedicle so as to migrate the flap as an island. In spite of those advances, there are still problems such as venous stasis and partial loss of the flap. In a study in cadavers, twenty pedicles were dissected in order to find the presence or absence of veins in the region, maximum length of the pedicle and anatomical reference points for a safe dissection. All pedicles were located. The venous distribution found, that was different from that described in other studies, could account for the problems in venous drainage.

RESUMO

Desde a descrição do retalho proposto por Washio em 1969, foram idealizados diversos modelos de pedículo vascular para o retalho retroauricular, visando principalmente à diminuição da espessura do pedículo a fim de migrar o retalho de forma ilhada. Apesar desses avanços, existem ainda problemas tais como estase venosa e perda parcial do retalho. Em estudo cadavérico, foram dissecados vinte pedículos procurando estabelecer a presença ou ausência de veias na região, comprimento máximo do pedículo e pontos de reparo anatômicos para uma dissecção segura.Todos os pedículos foram localizados, e a distribuição venosa encontrada, não descrita em outros estudos, poderia explicar os problemas de drenagem venosa.

Impact of extracurricular activities in Plastic Surgery on the knowledge and practical skills of medical students

ABSTRACT

INTRODUCTION To assess the impact of extracurricular activities in plastic surgery on the knowledge and practical skills of medical students at the Federal University of São Paulo. METHODS: Six students participating in extracurricular activities and eight students who did not participate were randomly selected and evaluated before and after the program by using multiple-choice tests and a Surgical Skills Assessment (SSA) instrument to assess their practical skills. RESULTS: The study group obtained higher scores in the theoretical tests, when the scores before starting the program (average, 15.8; standard deviation, 2.7) were compared with those received after their participation in extracurricular activities (average, 21.8; standard deviation, 1.3) (p = 0.02). Similar results were obtained with the SSA instrument for the period before the start of (average, 9.8; standard deviation, 1.4) and after (average, 13.5; standard deviation, 1.6) (p = 0.02) the program. The control group did not show significant changes in the scores received for theoretical tests when the period from August 2010 (average, 14.0; standard deviation, 4.1) to June 2011 (average, 13.8; standard deviation, 2.6) (p = 0.40) was evaluated. Similar results were obtained with the SSA instrument for the evaluation period between August 2010 (average, 8.5; standard deviation, 2.2) and June 2011 (average, 9.6; standard deviation, 1.7) (p = 0.18). CONCLUSION: The students participating in extracurricular activities in plastic surgery at the Federal University of São Paulo improved their knowledge and acquired better practical skills compared with students who did not participate.

RESUMO

INTRODUÇÃO Avaliar o impacto da monitoria em Cirurgia Plástica da Universidade Federal de São Paulo no conhecimento e nas habilidades práticas em estudantes de Medicina. MÉTODOS: Seis acadêmicos participantes da monitoria e oito acadêmicos não participantes foram selecionados aleatoriamente e avaliados antes e depois do programa, por meio de testes de múltipla escolha e através do instrumento Surgical Skills Assessment (SSA), para avaliação das habilidades práticas. RESULTADOS: O Grupo estudo obteve aumento significativo nas notas de prova teórica, entre o período antes do programa (média 15,8 e desvio padrão de 2,7) e após (média 21,8 e desvio padrão 1,3) (p = 0,02). Resultado semelhante foi encontrado com o instrumento SSA entre o período antes (média 9,8 e desvio padrão 1,4) e após o programa (média de 13,5 e desvio padrão 1,6) (p = 0,02). O grupo controle não obteve mudança significativa nos testes teóricos em agosto de 2010 ( média de 14,0 e desvio padrão 4,1) e junho de 2011 ( média de 13,8 e desvio padrão de 2,6 ) (p = 0,40 ). Resultado semelhante foi encontrado com o instrumento SSA em agosto de 2010 (média de 8,5 e desvio padrão 2,2) e junho de 2011 (média de 9,6 e desvio padrão 1,7) (p = 0,18). CONCLUSÃO: Acadêmicos participantes na monitoria de Cirurgia Plástica da Universidade Federal de São Paulo apresentaram melhor conhecimento e melhores habilidades práticas em relação aos não participantes.

Seroma in lipoabdominoplasty and abdominoplasty: ultrasonographic comparative study

ABSTRACT

Background: Seroma is one of the most common complications in abdominoplasty. Objective: To compare the rate of seroma formation in patients who underwent either abdominoplasty and lipoabdominoplasty. Methods: The sample consisted of 41 female patients, who were divided into three groups and underwent one of the following procedures: group A (n=21), abdominoplasty and group B (n=20), lipoabdominoplasty. In order to investigate seroma formation, abdominal ultrasound was performed in 5 regions of the abdominal wall [epigastrium (EPI), umbilical (UMB), hypogastrium (HYPO), right iliac fossa (RIF) and left iliac fossa (LIF)], at two postoperative periods: (P1), between postoperative days 11 and 14, and (P2), between postoperative days 18 and 21. Results: The rate of seroma formation at both P1 and P2 was significantly higher in group A than in group B. It was observed that, in group A at P1, the regions RIF and LIF developed larger fluid collections. In group B, there were significantly larger fluid collections in the HYPO region at P1, and in the UMB and HYPO regions at P2. Conclusion: Lipoabdominoplasty is effective technique for the prevention of seroma compared with abdominoplasty.

RESUMO

Introdução: O seroma é uma das complicações mais frequentes nas abdominoplastias. Objetivo: Comparar a incidência de seroma em pacientes submetidos à abdominoplastia convencional e à lipoabdominoplastia. Método: Foram estudadas 41 pacientes, divididas em dois grupos: Grupo A (21 pacientes submetidas à abdominoplastia convencional e Grupo B (20 pacientes submetidas à lipoabdominoplastia). Para a investigação da formação de seroma foi realizado ultrassonografia de cinco regiões da parede abdominal (epigástrio (EPI), umbilical (UMB), hipogástrio (HIPO), fossa ilíaca direita (FID) e fossa ilíaca esquerda (FIE)) em dois momentos: entre o 11º e o 14º dia de pós-operatório (DPO) (P1) e entre o 18º e 21º DPO (P2). Resultados: A incidência de seroma tanto em P1 quanto em P2 foi significativamente maior no grupo A que no grupo B. Observou-se que no grupo A, no P1, as regiões FID e FIE apresentaram maior incidência de coleções fluidas. No grupo B, houve uma maior incidência de coleções fluidas na região HIPO em P1 e nas regiões UMB e HIPO em P2. Conclusão: Houve menor incidência de seroma nos pacientes submetidos à lipoabdominoplastia em relação à abdominoplastia convencional nos momentos estudados.
