Hydroliposuction: 22 Years Experience with Tumescent Lipoplasty

ABSTRACT

The author presents his experience with Hydroliposuction, a procedure developed in 1980 and 1981 as the first enhancement of the Illouz's technique. This study includes considerations on the history of liposuction; the natural evolution of Curettage and Suction Curette and, particularly, it addresses how this form of suction came about, after the use of tumescent hydration of the fat tissue to be suctioned. From the first steps in the suction of fat in 1980, when Ivez Illouz arrived to Brazil, invited by Vinicius Faria, the author found that it would be better to work with thin cannulas in an excessively hydrated region and has defended this method, which he regards as the safest in the technique of liposuction, intensely discussed in the first three years of lipoplasty in Brazil, in contrast with the dry technique, initiated in 1982, which defefended conducting the surgery without any previous hydration. The technique and its statistical results are presented.

RESUMO

O autor apresenta sua experiência com a Hidrolipoaspiraçâo, um procedimento desenvolvido em 1980 e 1981 como primeiro refinamento da técnica de Illouz. Tece considerações sobre a história da lipoaspiraçâo, evolução natural da Curetagem e Curetagem-Sucção e, em particular; de como chegou a essa forma de aspiração, precedida de hidratação intumescente do tecido gorduroso a ser aspirado. Desde seus primeiros passos na sucção de gorduras em 1980) quando Ivez Illouz chegou ao Brasil) convidado por Vinicius Faria) o autor determinou que seria melhor trabalhar com cânulas finas em uma região excessivamente hidratada e tem defindido este método) o qual considera o mais seguro na técnica de lipoaspiração, intensamente discutido ao longo dos três primeiros anos da lipoplastia no Brasil) em comparação com a técnica seca) surgida em 1982) que defindia a realização da cirurgia sem nenhuma hidratação prévia. Descreve a técnica e apresenta resultados estatísticos.

Ascending upper blepharoplasty or upper eyelid lifting

ABSTRACT

The author presents his experience with a personal technique used, in the initial cases, for correction of sub-eyebrow skin ptosis over the upper eyelid, a frequent occurrence after an upper blepharoplasty performed without a concomitant temporal and frontal lifting. Initially, the author makes a brief historical report about the incisions in blepharoplasty. The access way for this procedure is a sub-eyebrow zigzag incision that can be used for frontal and glabelar procedures and also for primary blepharoplasties in some patients. It is a zigzag or W sub-eyebrow incision, below and between the inferior eyebrow hairs, followed by a subcutaneous undermining and orbicularis muscle incisions to treat the fat pads and Roof excesses, to ascend the eyelid's lateral corner and to perform a resection of the skin excess, making a true upper eyelid lifting. In the studied period along the past 6 years, 42 patients were submitted to this technique and the author extended the indications, listed in a selection of patients. High satisfaction levels with the initial results and few complications were observed. All the patients had important aesthetic improvement. The author concludes that this new method is a simple and safe solution for correction of bad results in eyelid surgery and a new alternative to upper blepharoplasty.

RESUMO

O autor relata sua experiência com uma técnica pessoal que, inicialmente, foi utilizada para correção de ptose da pele da reborda orbitária (pele subsuperciliar) sobre a pálpebra, a qual é resultado insatisfatório de blefaroplastia superior, executada isoladamente em pacientes com indicação de ritidoplastia frontotemporal. Inicialmente, o autor faz algumas considerações históricas sobre os acessos utilizados na blefaroplastia. A via de acesso da técnica também facilita o acesso às regiões frontal e glabelar e pode, em certos pacientes, ser utilizada para blefaroplastia primária. É uma incisão em ziguezague ou W, junto aos pêlos inferiores do supercílio, seguida de descolamento supra-orbicular e, por este campo, são realizados os demais tempos para o tratamento de bolsas, excessos de gordura retro-orbicular (Roof), levantamento do canto lateral e ressecção do excesso cutâneo, fazendo um verdadeiro lifting da pálpebra superior. Ao longo dos últimos 6 anos, foram operados 42 pacientes por esta técnica e, durante o período estudado, o autor ampliou as indicações. Estas estão relacionadas na seleção dos pacientes para o método utilizado. Os resultados iniciais foram altamente satisfatórios, com baixo índice de complicações e, no acompanhamento tardio, todos os pacientes obtiveram melhora estética importante. O autor conclui que esta nova abordagem representou uma solução simples e segura para correção de maus resultados em cirurgia palpebral e uma nova alternativa para a blefaroplastia superior.

Local Injection of Hyaluronidase in Increasing Skin Flap Survival: An Experimental Study

ABSTRACT

Previously, clinical observations have suggested that local injection of hyaluronidase (HDL) increase skin flaps survival(23). We have now extended these observations, analyzing the effects of HDL in rabbit skin flaps. Therefore, dorso-lateral, cranially pedicled skin flaps were treated with 1 ml HDL (200 U/ml, treated flaps) and compared, after 7 days of surgery, with flaps injected with 1 ml saline (control 1) or no injected at all (control 2). The efficiency of HDL was confirmed by the percentage of necrosis in the flap area (evaluated by tissue color and capillary filling) which achieved 1.47% ± 2.91) 27.64% ± 25.89 and 30.14% ± 27.96 in the treated groups, control 1 and control 2 flaps respectively. We conclude that HDL is capable of preventing skin flaps necrosis.

RESUMO

Observações clínicas realizadas previamente pelo autor senior(23) sugeriram que a injeção local de hialuronidase (HLD) aumenta a sobrevivência de retalhos cutâneos. Neste trabalho experimental, analisamos os efeitos da HLD em retalhos cutâneos de coelhos. Para tanto comparamos, após 7 dias da cirurgia, retalhos dorsolaterais de pedículo craneal, injetados diariamente com 1 ml de HLD (200 U/ml, retalhos tratados, com retalhos injetados diariamente com 1 ml de soro fisiológico (controle 1) e com outros não injetados (controle 2). A eficiência da hialuronidase foi confirmada pela percentagem de necrose nos retalhos (avaliada pela cor e enchimento capilar) que chegou a 1,47% ± 2,91, 27,64% ± 25,89 e 30,14% ± 27,96 nos grupos tratado, controle 1 e controle 2 respectivamente. Concluímos que a HLD é capaz de prevenir necrose em retalhos cutâneos.

Marking the recipient sites in hair restoration surgery - new methodology to attain symmetric density

ABSTRACT

INTRODUCTION: In hair restoration surgery, recipient sites for insertion of follicular units are made with microblades or needles, with the goals of achieving the closest proximity possible and to maintain vascular nutritional support for the grafts. This study describes the methodology for preparatory marking of the frontal scalp with dots or lines. Stamps developed by the author are used to make incisions at these sites in order to attain symmetric density. METHODS: Several types of stamps are described. Individualized options for use in the frontal region were defined in four groups of patients; three types of inks were used in order to choose the ink with greater permanence in the skin. Groups A and B were respectively tested with common concentrations of inks for medical use and with a permanent ink. Groups C and D were tested with an alcoholic solution of 3% gentian violet, with and without the application of a barrier film spray. RESULTS: Stamps moistened with the ink tested in group D marked lines and dots on the scalp which were resistant to blood and frequent washing, when covered with a barrier film. CONCLUSIONS: The use of stamps to mark bald skin with the methodology described improves incision symmetry and slot identification for the insertion of follicular units, in turn resulting in symmetric density in the frontal region.

RESUMO

INTRODUÇÃO: Em cirurgias de restauração capilar, as incisões receptoras para a inserção das unidades foliculares são realizadas com micro lâminas ou agulhas, procurando conseguir a maior proximidade possível, mas visando a segurança da nutrição sanguínea dos enxertos. O objetivo deste trabalho é relatar uma metodologia de marcação prévia de traços ou pontos na pele calva da região frontal, usando carimbos criados pelo autor, objetivando fazer incisões nessas marcas, a fim de se obter simetria de densidade. MÉTODOS: Alguns tipos de carimbos são descritos, e também suas opções de utilização na região frontal, juntamente com três tipos de tinta, uma em cada indivíduo de quatro grupos de pacientes, visando a escolha da tinta que apresentasse maior tempo de permanência na pele. Nos grupos A e B, foram testados, respectivamente, o uso das concentrações usuais das tintas de uso médico e o de uma tinta permanente. Nos grupos C e D, foi testada uma solução alcoólica de 3% de violeta de genciana, sem e com uma película de barreira em spray. RESULTADOS: Os carimbos descritos, umedecidos com o corante testado no grupo D, deixaram traços e pontos sobre o couro cabeludo que, cobertos com a película de barreira, foram resistentes a sangue e lavagens freqüentes. CONCLUSÕES: O uso de carimbos para marcar a pele calva, usando o método descrito, facilita a simetria das incisões e a identificação das fendas para a inserção das unidades foliculares, resultando em densidade simétrica na região frontal.
