Abdominoplasty: clinical experience, complications and review of literature

ABSTRACT

In this study, a review of 424 abdominoplasties, associated or not with other surgeries, was made focusing on the occurrence of complications, comparing these results with those reported in the literature. The commonest complications included 34 cases of seroma, 7 hematomas, 3 dehiscence of sutures, 2 necrosis of the flap and 3 cases of deep vein thrombosis. There were no occurrences of pulmonary emboli or death. The authors stress that for new techniques such as, for example, lipoabdominoplasty, which represent a great advance in the preservation of the vascular circulation of the dermocutaneous flap, preventing necrosis and seromas. Another fundamentally important approach is the review of the incidence of thromboembolic events after abdominoplasties. The study discusses the physiopathology, diagnosis and prophylactic protocol of these events.

RESUMO

Nesse estudo, é feita uma revisão de 424 abdominoplastias, associadas ou não a outras cirurgias, enfocando a ocorrência de complicações, comparando-as com os índices encontrados na literatura. As complicações mais comuns foram: 34 casos de seroma, 7 hematomas, 3 deiscências de sutura, 2 necroses de retalho e 3 casos de trombose venosa profunda. Não houve ocorrência de embolia pulmonar ou óbito. Os autores chamam atenção, ainda, para a publicação de novas técnicas como, por exemplo, a lipoabdominoplastia, que representa um grande avanço na preservação da rede vascular do retalho dermocutâneo, prevenindo necroses e seromas. Outra abordagem de fundamental importância é a revisão realizada da incidência de fenômenos tromboembólicos após abdominoplastias. O estudo discorre, ainda, sobre a fisiopatologia, diagnóstico e protocolo para a profilaxia desses eventos.

Exo-rhinoplasty in the treatment of bulbous nasal tip

ABSTRACT

Over the last few years, our team has systematically utilized exo-rhinoplasty in the treatment of bulbous nasal tips, which are characterized by thick skin, excessively rounded and with bad definition. Initially, only interdomal sutures were performed and it was noticed that the resulting projection using this technique was not long-lasting. More recently, the modified Rethi approach started to be utilized, using interdomal sutures and Peck and Strut-type cartilage grafts; thus obtaining a better definition and projection of the nasal tip with longlasting results.

RESUMO

Nos últimos anos, nossa equipe tem utilizado sistematicamente a exorrinoplastia para tratamento da ponta nasal bulbosa, caracterizada por pele grossa, ponta globosa e mal definida. Inicialmente, era feita apenas a sutura interdomal e foi observado que o resultado da projeção da ponta nasal não era duradouro. Recentemente, passou-se a utilizar o acesso de Rethi modificado, sutura interdomal e enxertos cartilaginosos tipo Peck e "strut", obtendo-se, desta forma, melhor definição e projeção da ponta nasal com resultados persistentes.

Inclusion of the breast prosthesis in double space

ABSTRACT

The work presents a new location as far as the space for implantation of the silicone mammal prótese. It develops a new positioning of the prosthesis, not just sub-glandular or sub-muscular, which is named Inclusion of the Mammal Prosthesis in Double Space. With that, many of the unfavorable results are eliminated, not only in the subglandular, but also in the sub-muscular positioning. This new technique was applied efficiently by its author in 320 operated patients from March 1997 until March 2004, presenting as an additional benefit to the obtained results a better acceptance by the patients in relation to the prior techniques.

RESUMO

Este trabalho apresenta uma nova localização quanto ao espaço de implantação de próteses mamárias de silicone. Desenvolve um novo posicionamento da prótese, não sendo somente subglandular ou submuscular, ao qual se denomina de Inclusão de Prótese de Mama em Duplo Espaço. Com isso, elimina-se grande parte dos resultados desfavoráveis, tanto no posicionamento subglandular, quanto no submuscular. Esta nova técnica foi aplicada com eficiência pelo autor em 320 pacientes operadas, no período de março de 1997 a março de 2004, apresentando, como benefício adicional aos resultados obtidos, melhor aceitação pelas pacientes em relação às técnicas anteriores.

Reconstruction of the limbs with microvascular free flaps in emergency: a 10 years experience with 154 consecutive cases

ABSTRACT

The authors presents the results of the early microsurgical reconstruction of complex trauma of the limbs. The patients aged ranged from 3 to 72 years, with the average 28.4 years. The wound size ranged from 12 to 840 cm2, with the average of 170 cm2. The treatment of the severely injured extremity upon admission to the trauma center is divided into three separate stages: debridement and stabilization; re-exploration of the wound and debridement within 48 hours; soft tissue coverage with free flaps is provided within one week. The numbers and kind of the free flaps are as follows: 2 plantar flaps, 8 gracilis muscle, 9 temporalis fascia, 15 latissimus dorsi, 16 lateral arm flaps, 24 rectus abdominis, 32 radial forearm flaps and 48 perforator free flaps. This study analyses the results obtained with the different flaps, their advantages, and try to find which factors influenced the survival rate. The statistical analysis showed that the magnitude of traumatic insult was the most significant factor associated with failure. Satisfactory results were achieved in 93% of the patients. Free flaps complications were low. The authors concluded that massive injuries to the extremities, regardless of the extent, should be managed by early aggressive debridement and early definitive reconstruction.

RESUMO

Os autores analisam a casuística de 154 pacientes portadores de traumatismos complexos dos membros. A idade dos pacientes variou de 3 a 72 anos, com média de 28 anos e 4 meses. O tamanho da perda cutânea variou de 12 a 840 cm2, com média de 170 cm2. O tratamento das lesões complexas dos membros iniciou-se pelo desbridamento radical, seguido pela irrigação abundante. A seguir, a estabilização esquelética foi realizada, quando necessária. Após 48 horas, foi realizado novo desbridamento e programada a cobertura precoce e definitiva do trauma (até a primeira semana) com retalhos livres. Os retalhos empregados foram: plantar (2), grácil (8), fáscia temporal (9), latíssimo do dorso (15), lateral do braço (16), reto do abdome (24), radial antebraquial (32) e retalhos perfurantes (48). Os autores discutem as indicações dos retalhos livres na urgência, os fatores que influenciaram o índice do sucesso, assim como o planejamento de cada retalho. Fez parte deste estudo a análise dos fatores que poderiam influenciar os resultados. Dentre eles, foram considerados a perda (criada pelo trauma de alta energia entre um objeto e o membro), a idade, o sexo e o número de anastomoses venosas realizadas para cada retalho. A análise estatística pelo teste não paramétrico de Mann-Whitney demonstrou que as grandes perdas cutâneas interferiram no índice de sucesso dos retalhos. O índice das complicações foi baixo, os resultados definitivos foram bons em 93% dos pacientes operados. Baseados, nestes resultados, os autores concluem que o tamanho da perda cutânea tem influência no índice de sucesso dos retalhos e que o emprego dos retalhos livres na urgência é um método eficiente e seguro no tratamento das lesões complexas dos membros.

Late breast reconstruction with prior expansion of the mastectomy area and filling with a de-epidermized TRAM flap

ABSTRACT

Post-mastectomy late breast reconstruction was proposed by means of skin expansion in the first operative stage, substitution of the expander by a de-epidermized TRAM flap mounted in the shape of a cone with an adequate size and proportions in the second stage. In the third stage, the mammary groove and areola-papilla were reconstructed and symmetrization was achieved. Twenty-seven patients, with ages of between 27 and 66 years old and a mean age of 46.9 years, were operated on. In three cases, reconstruction was bilateral. In the long-term evolution, the symmetry and satisfaction of the patients were also assessed. There was no difference in the color of the skin by the transposition of the flap. The sensitivity of the thoracic skin that covered the breast was similar to the preexisting sensitivity after a period of two years. Patients assessed the results using a score of 0 to 10 giving a mean score of 9.28. In conclusion, the prior expansion of thoracic skin in mastectomized areas is a good option in selected patients, improving the sensitivity of the breast and solving the problem of skin color.

RESUMO

A reconstrução mamária tardia pós-mastectomia foi proposta por meio de expansão da pele em primeiro tempo operatório, substituição do expansor por retalho TRAM desepidermizado e montado em forma de cone com medidas e proporções adequadas em segundo tempo. No terceiro tempo, foram reconstruídos o sulco mamário, a aréola-papila e realizada a simetrização. Vinte e cinco pacientes, entre 27 e 66 anos, com idade média de 46,9 anos, foram operadas. Em 3 casos, a reconstrução foi bilateral. Evolução tardia, simetria e satisfação das pacientes também foram avaliadas. Não houve bicromia de pele pela transposição à distância do retalho. A sensibilidade da pele torácica que cobriu a mama foi semelhante à preexistente após dois anos. As pacientes avaliaram seus resultados com notas de 0 a 10. A média foi de 9,28. Conclui-se que a expansão prévia da pele torácica nas áreas mastectomizadas é boa opção em pacientes selecionadas, melhorando a sensibilidade mamária e fazendo desaparecer a bicromia de pele.

Use of chelants and neoangiogenics substances in skin flaps

ABSTRACT

Cutaneous flaps are largely used by plastic surgeon and have many indications, like in burn repairs, injuries, deformities and face reconstructions. However, they present several complications. Necrosis is the most warningfull complication, so many substances had been used to avoid it, but none of them has proved to be efficient in reduce putting out necrosis. We compared and verified, in this project, the efficiency of catalase, glutathione, retinoic and glycolic acid in the injury and cutaneous flaps, healing by macroscopic and microscopic analysis of rabbit's skins. Fibrosis, scar/crust, congestion, spongiosis, inflammatory process, neovascularization increase and epiderm atrophy were some of results of histopathologic analysis. Thus, the use of pool-antinecrosis was indicated to cutaneous grafts that are going to cover large devitalized areas with large continuity solution.

RESUMO

Retalhos cutâneos são amplamente utilizados por cirurgiões plásticos e têm muitas indicações, como em reparações de queimaduras, injúrias, deformidades e reconstruções faciais. No entanto, eles apresentam muitas complicações. Necrose é a complicação mais alarmante, e muitas substâncias têm sido utilizadas para evitá-la, mas nenhuma delas teve sua eficiência comprovada na redução deste processo. Neste trabalho, foi comparado e verificado o desempenho da catalase, da glutationa, dos ácidos retinóico e glicólico em injúrias e na cicatrização dos retalhos cutâneos, por meio de análises macroscópicas e microscópicas de peles de coelhos. Fibrose, cicatrizes/crostas, congestão, espongiose, processo inflamatório, aumento da neovascularização e redução da atrofia epidérmica foram alguns dos resultados da análise histopatológica. Portanto, o uso dessas substâncias pode ser indicado para retalhos cutâneos que irão cobrir grandes áreas desvitalizadas com grande solução de continuidade.

The use of cyanoacrylate in the cutaneous repair of primary cheiloplasties

ABSTRACT

This works reports on the experience of the authors with the use of N-Butyl-Cyanoacrylate for the cutaneous closure of primary cheiloplasties in unilateral fissures, comparing the esthetic results with those to 6.0 mononylon sutures. Between April 1999 and December 2003, 62 primary cheiloplasties were performed in the Hospital Municipal Infantil Menino de Jesus utilizing the Millard's rotation-advancement lip repair technique. In Group A, comprised of 35 patients, the skin was sutured using 6.0 mononylon thread. For Group B, comprised of 27 patients, the skin was repaired using N-Butyl- Cyanoacrylate adhesive. The groups were evaluated on the 1st, 7th, 30th and 90th days after surgery. There were no cases of surgical infection. One (3.4%) patient of Group B presented with dehiscence less than one day after surgery requiring conventional suturing. One (2.8%) child of Group A required general anesthesia to remove the suture. The esthetic results of scaring were similar in both the groups studied. The authors stress the advantages of the optimization of this adhesive in the pediatric population. The advantages include the facility and simplicity of its use, the low cost, the time of the surgeon, reduction in the time of surgery and, most importantly, there is no necessity to remove the sutures, thereby reducing the stress of the child and family members in the postoperative return appointment.

RESUMO

Este trabalho mostra a experiência dos autores com o uso do N-Butil- Cianoacrilato no fechamento cutâneo de queiloplastias primárias em fissuras unilaterais, observando os resultados estéticos em relação à sutura de mononylon 6.0. Entre abril de 1999 e dezembro de 2003, 62 queiloplastias primárias foram realizadas no Hospital Municipal Infantil Menino de Jesus, utilizando-se a técnica de rotação e avanço de Millard. No grupo A, composto por 35 pacientes, a pele foi suturada com fio de mononylon 6.0. No grupo B, com 27 pacientes, a pele foi aproximada com o adesivo N-Butil-Cianoacrilato. Os grupos foram avaliados no pós-operatório 1, 7, 30 e 90 dias após a cirurgia. Não houve casos de infecção cirúrgica. Um (3,4%) paciente do grupo B apresentou deiscência no mesmo dia da cirurgia, necessitando de sutura convencional. Uma (2,8%) criança do grupo A necessitou de anestesia geral para a retirada da sutura. Os resultados estéticos das cicatrizes foram semelhantes nos dois grupos estudados. Os autores destacam as vantagens da otimização deste adesivo na população pediátrica, que incluem facilidade e simplicidade no uso, baixo custo, utilização do tempo do profissional, diminuição do tempo cirúrgico e, principalmente, a desnecessidade de remoção das suturas, diminuindo assim o estresse da criança e dos familiares no retorno pós-operatório.

Large volume or area vibroliposuction?

ABSTRACT

The authors present a method of evaluation of large scale liposuction, establishing a parallel to burn evaluation. The table of Lund and Browder was utilized as a parameter. The authors determined a maximum area to be treated, discuss the technique, medications and controls utilized to prevent severe complications. They also present results obtained from 487 surgeries, during the period of January 1998 and July 2004, with vibroliposuction performed in body areas which varied from 20 to 30%.

RESUMO

Os autores apresentam metodologia de avaliação para as grandes lipoaspirações, estabelecendo paralelo com queimaduras. Utilizam-se da tabela de Lund e Browder como parâmetro. Determinam área máxima de tratamento, discutem a técnica, as medicações e controles utilizados para prevenir complicações graves. Apresentam os resultados obtidos em 487 cirurgias com vibrolipoaspiração realizadas em áreas corporais que variam de 20 a 30%, no período de janeiro de 1998 a julho de 2004.

Patient with siliconomas and their association with breast tumors

ABSTRACT

The authors report on a case of a patient who, after the injection of industrial silicon for breast enlargement by non-healthcare professionals, was consulted in the Plastic Surgery Outpatients' Clinic with algia and carcinophobia. The patient presented with multiple siliconomas and was submitted to a subcutaneous mastectomy and implantation of silicon prosthesis in the same operation. The anatomopathological examination demonstrated lobular carcinoma of the right breast as well as findings compatible with siliconomas. Four months after surgery, this patient presented with enlarged axillary nodules and the presence of metastatic lymph nodes of another tumor in the counter-lateral axillary region, thus synchronic tumors. Approximately one year after, the patient evolved with probable retro-ocular and hepatic metastasis and today is undergoing chemotherapy. There are works that show the relationship between the injection of silicon and the formation of siliconomas and the evolution to breast cancer, although this is still a controversial subject. However, the seriousness of such cases is not discussed due to the difficult and late diagnosis as in this case.

RESUMO

Relatamos o caso de uma paciente que, após injeção de silicone industrial por leigos, para aumentar o volume das mamas, procurou o ambulatório de Cirurgia Plástica com quadro de algia e carcinofobia. Apresentava múltiplos siliconomas. Foi submetida a mastectomia subcutânea e implante de próteses de silicone no mesmo tempo cirúrgico. O exame anatomopatológico revelou carcinoma lobular na mama direita, além de achado anatomopatológico compatível com siliconomas. Essa paciente, quatro meses após a cirurgia, apresentou nódulos axilares aumentados e presença de linfonodo metastático de outro tumor na axila contralateral, portanto tumores sincrônicos de mama. Aproximadamente um ano após, evoluiu com prováveis metástases retroculares e hepáticas e, atualmente, encontra-se em tratamento oncológico quimioterápico. Existem trabalhos que mostram a relação entre a injeção de silicone e formação de "siliconomas" e evolução com câncer de mama, sendo ainda assunto controverso; no entanto, não se discute a gravidade dos casos, pelo diagnóstico difícil e tardio, como nesse caso.

Foot reconstruction using a free latissimus dorsi muscle flap post-amputation of the contra-lateral extremity

The authors report on a case of foot reconstruction using a free latissimus dorsi muscle flap followed by split-thickness skin grafting, after an injury and contralateral extremity amputation. They describe the indications, surgical procedure and the patient's evolution.

RESUMO

Os autores relatam um caso de reconstrução do pé com retalho muscular livre do grande dorsal seguida de enxertia dermoepidérmica, após trauma extenso e amputação do membro contralateral. Destacam-se as indicações, técnica operatória e evolução clínica da paciente.

Evaluation of long-term results of rhytidoplasty

ABSTRACT

Many articles have been published about rhytidoplasty, however, few report on the long-term results. The objective of this article is to try to evaluate the results of face lifts and to define long-term results. The evaluation of rhytidoplasty is not an easy task. Different methods are utilized but there are no parameters that can assess the final product unless the result is bad. The process of aging does not cease after the facial rejuvenating surgery. Several important factors are involved in the result and the duration of the surgical effects including: the different ages at which patients are submitted to face lifts, the different surgical goals and the experience and good sense of the surgeon. Thus, this subject is of special interest to the plastic surgeon. In this article we tried to explore some of these aspects. Is it possible this is a philosophical discussion?

RESUMO

Vários artigos são publicados sobre ritidoplastia, porém, poucos abordam seus resultados tardios. O objetivo deste artigo é tentar avaliar os resultados do face lift e definir qual é o resultado tardio. A avaliação da ritidoplastia não é tarefa fácil. Diferentes métodos são utilizados, porém não há parâmetros que possam avaliar o produto final, a não ser que um resultado ruim ocorra. O processo do envelhecimento não cessa após a cirurgia de rejuvenescimento facial. Vários fatores, de modo importante, fazem parte do resultado e da duração dos efeitos da cirurgia: as diferentes idades em que os pacientes são submetidos ao face lift, as diferentes propostas cirúrgicas, a experiência e o bom senso do cirurgião. Sendo assim, este assunto torna-se extremamente interessante para o cirurgião plástico. Neste artigo, tentamos expor alguns destes aspectos. Seria esta uma discussão filosófica?
