ABSTRACT
Introduction: The bottoming out deformity is a matter of concern to patients and surgeons in the primary and secondary augmentation mammoplasty.
Method: The author comments the possible deformity physiopathology and his experience employing muscular flaps in augmentation mammoplasty. A new mio-fascial-capsular flap in "delta" shape is also described as an alternative to be used in secondary mammoplasties.
Results: The muscular flaps are a great contribution to the augmentation mammoplasty and the "delta flap" may be effective in the secondary mammoplasties.
Conclusions: The muscular flaps are highly valuable to preserve the breast shape after mammoplasties, in spite an unavoidable minor skin sagging, observed in particular patients.
