Three-dimensional computed tomography (multislice) in the previous diagnosis of the nasal deformities

ABSTRACT

Introduction: Aware to the new technique for diagnosis denominated tomography multislice, whose quality of images and anatomical details surprised us, we developed a research and scientific work comparing the tomographical images to the patients' previous pictures and the trans-operative images, all done inside of patterns pre-established in the positions: front (AP), profile, fronto-naso and mento-naso. Objective: Those standardized positions were confronted in the three levels (static and dynamic) of depth that multislice propitiates: superficial (cutaneous level); medium (alar cartilages, square and lateral superior or triangular cartilages) and deep level (bone). Methods: Were selected six patients: four patients candidates to primary rhinoplasty and two for secondary rhinoplasty. Due to the excellence of the comparative results, now we request this preoperative resource, practically to all the cases in that we will accomplish rhinoplasties. Results: Fidelity was verified among the tomographic multislice images, when compared with the trans-operative images and the preoperative pictures. Conclusions: This study is very usefull for young plastic surgeons, as well as in special cases were is needed pre-operative details.

RESUMO

Introdução: O recurso diagnóstico denominado tomografia multislice, apresenta qualidade de imagens e detalhamento anatômico surpreendentes. Procurou-se fazer um trabalho comparando as imagens tomográficas às fotografias prévias dos pacientes e às imagens transoperatórias, todas feitas dentro de padrões preestabelecidos, nas posições: frente (AP), perfil, fronto-naso e mento-naso. Objetivo: Comparar as posições padronizadas confrontando- se nos três planos (estáticos e dinâmicos) de profundidade que a tomografia multislice nos propicia: plano superficial (relevo cutâneo); plano médio (cartilagens alar, quadrangular e lateral superior ou triangular) e plano profundo (ósseo). Método: Foram selecionados seis pacientes: quatro pacientes candidatos a rinoplastia primária e dois de rinoplastia secundária, que fizeram parte do corpo do trabalho. Resultados: Verificou-se fidelidade entre as imagens tomográficas multislice, quando comparadas às imagens transoperatórias e as fotos préoperatórias. Conclusão: Este estudo é de grande valia para jovens cirurgiões plásticos ou mesmo em casos de minucioso diagnóstico pré-operatório.

Evaluating judgments and decisions related to lawsuits involving aesthetic plastic surgery

ABSTRACT

Introduction: There is a legal consensus that the results of medical activities represent obligations of means, not results. However, there is ample discussion when it comes to aesthetic procedures. Resolution 1621/2001 of the Federal Council of Medicine also defines the objective of a medical act in plastic surgery as an obligation of means. This study evaluated 106 cases between November 2015 and November 2017 to verify whether the decisions of the Judicial Power agree with the Resolution of the Federal Council of Medicine. The number of lawsuits and the percentage of claims granted or denied were quantified, and the opinions of jurists and courts that supported the claims granted were verified. The number of cases in which the judge's decision was related to the opinion of a medical expert was also quantified.
Methods: The authors searched the judgment database located on the website of the Court of Justice of the State of São Paulo (SP) for damage related to aesthetic plastic surgery, using the keyword "Plastic Surgery" for all actions.
Results: A total of 61 claims (58%) were denied, and 45 (42%) were granted. In 96% of cases (102) the judgment was positively related to the expert report.
Conclusion: There were 102 cases in which the judgment agreed with the expert reports and only four cases in which the judgment did not agree with the reports. These data show the crucial importance of experts' reports in defining judicial judgments. The analyses of all judgments showed that there were no cases in which the judge considered the Resolution of the Federal Council of Medicine.

RESUMO

Introdução: É consenso no meio jurídico que os resultados referentes às atividades médicas sejam obrigação de meio, e não de resultado. Contudo, há grande discussão quando se trata de procedimentos estéticos. A Resolução nº 1621/2001, do Conselho Federal de Medicina, define que o objetivo do ato médico na cirurgia plástica também constitui obrigação de meio. O estudo avaliou, entre novembro de 2015 a novembro de 2017, 106 casos, para verificar se o entendimento do Judiciário se alinha à Resolução do CFM. Foram quantificados o número de processos e a porcentagem dos casos julgados como procedentes ou improcedentes, além de verificar as principais posições doutrinárias e jurisprudenciais que embasaram as sentenças admitidas como procedentes. Foi, ainda, quantificado o número de casos cuja decisão do magistrado foi relacionada com o posicionamento do laudo pericial médico.
Métodos: Foi feita busca no banco de sentenças do site do Tribunal de Justiça do Estado de São Paulo (SP), por meio da palavra-chave "Cirurgia Plástica", de todos os processos de indenização relacionados a cirurgias plásticas estéticas.
Resultados: Foram sentenciados como improcedentes 61 casos (58%). Foram sentenciados como procedentes 45 casos (42%). Em 96% dos casos (102) a sentença relacionou-se positivamente com a análise pericial.
Conclusão: Foram 102 sentenças concordantes aos laudos periciais e apenas quatro casos cuja sentença divergiu do entendimento do laudo. Estes dados mostram a importância crucial da análise pericial para a definição das sentenças judiciais. Analisando todas as sentenças, observou-se que em nenhum caso os juízes levaram em conta a Resolução do CFM.

Evaluation of the facial soft tissue thickness in a Brazilian in vivo population "facial soft tissue thickness in Brazilians"

ABSTRACT

Introduction: Nowadays there is a huge demand for individual identification in both civil and criminal justice fields. There are several ways to identify individuals, depending on whether he is alive or dead (cadaver or skeleton). This research aimed to create a database for facial soft tissue thickness in living individuals of a specific Brazilian population, according to age, sex and body mass index.
Methods: Measurements of facial soft tissues thickness were done in 101 patients (62 men and 39 women, aged 18 to 106 years). CT scans were performed taking into account 20 craniometric points previously selected. The soft tissue thickness in these points were initially found and compared with age, sex and nutritional status, and with another populations data worldwide.
Results: According about sex was no significant difierence in points: nasion, rhinion, midphiltrum, supradentale and lateral orbit. Distances with significant difference for both sexes between the diferent populations were: supradentale; infradentale; supramentale; lateral orbit; zygomatic arch and occlusal line. Some anthropometric points showed significant differences between sex, age groups and nutritional status. Between sexes, men had greater means. Among age groups, there was also significant differences in some distances. In relation to nutritional status, the distances were lower among normal weight and higher among the obese.
Conclusion: When considering various populations, soft tissue thickness had significant differences in many craniometric points highlighting how distinct they might be.

RESUMO

Introdução: Hoje em dia há uma enorme demanda para identificação individual tanto no campo da justiça civil e criminal. Existem várias maneiras de identificar indivíduos, dependendo se ele está vivo ou morto (cadáver ou esqueleto). Esta pesquisa teve o objetivo de criar um banco de dados para a espessura do tecido mole facial em indivíduos vivos de uma determinada população brasileira, de acordo com idade, sexo e índice de massa corporal.
Métodos: medições da espessura de tecidos moles faciais foram realizadas em 101 pacientes (62 homens e 39 mulheres, com idade entre 18 e 106 anos). TC foi realizado tendo em conta 20 pontos craniométricos selecionados previamente. A espessura dos tecidos moles nestes pontos foram inicialmente medida e comparada com a idade, sexo e estado nutricional e com dados de outras populações em todo o mundo.
Resultados: De acordo com o sexo não houve diferenças significativas nos pontos: násio, rínion, meio do filtro labial midphiltrum , supradentale, e órbita lateral. Distâncias com diferenças significativa para ambos os sexos entre as diferentes populações foram: supradentale; infradentale; supramentale; órbita lateral; arco zigomático e linha oclusal. Alguns pontos antropométricos mostraram diferenças significativas entre sexo, faixa etária e estado nutricional. Entre os sexos, os homens apresentaram médias maiores. Entre os grupos etários, houve também diferenças significativas em algumas distâncias. Em relação ao estado nutricional, as distâncias foram menores entre peso normal e maior entre os obesos.
Conclusão: Ao considerar várias populações, a espessura do tecido mole mostrou diferenças significativas em muitos pontos craniométricos destacando como eles podem ser distintas.

Domestic three-dimensional scanner and printer in patients undergoing rhinoplasty

ABSTRACT

The principles for a successful rhinoplasty include preoperative consultation and planning, as well as a comprehensive clinical analysis and defining rhinoplasty goals. Three-dimensional domestic scanning and printing have recently become available. We sought to objectively describe this method as an intraoperative aid in patients' anatomy. This method can be used trans-operatively to help surgeons compare the results of his or her technique, check adherence to the surgical plan, and improve his or her surgical decision-making. We found that the application of 3-dimensional printing had a positive effect on the treatment of patients with aesthetic nose disorders.

RESUMO

Os princípios para uma rinoplastia bem-sucedida incluem consulta e planejamento pré-operatório e uma análise clínica abrangente que defina as metas da cirurgia. Mais recentemente, a digitalização e a impressão doméstica em 3 dimensões tornaram-se disponíveis. O objetivo deste estudo é descrever um método de digitalização em 3 dimensões e de impressão doméstica da anatomia real do paciente para ser usada como ajuda intraoperatória. Nós apresentamos uma forma de uso desta tecnologia no transoperatório, auxiliando o cirurgião a comparar os resultados obtidos após suas manobras, verificar a sua adesão ao plano cirérgico previamente estabelecido e melhorar a sua tomada de decisão durante a cirurgia. Em conclusão, a aplicação da impressão doméstica em 3 dimensões demonstra um efeito positivo sobre o tratamento de alterações estéticas do nariz.

The use of virtual planning in orthognathic surgery

ABSTRACT

Introduction: Orthognathic surgery involves the manipulation of facial bone architecture through osteotomies to restore form and function, correcting malocclusion, maxillomandibular disproportions, and facial asymmetries. Virtual planning in orthognathic surgery is carried out with the help of software that uses real measurements of the craniofacial skeleton and records of the patient's occlusion through 3D analysis.
Method: 18 patients with dentofacial deformities were evaluated, according to Angle's classification, who underwent orthognathic surgery using virtual planning between 2018 and 2019. The inclusion criteria were patients between 16 and 60 years old with maxylo-mandibular disproportions in which orthodontic treatment alone was not sufficient. Exclusion criteria were the presence of cystic or tumoral lesions in the jaw and clinical comorbidities that contraindicated surgery. Virtual planning was carried out on all patients, using Dolphin® Imaging 11 software and surgical guides made with a 3D printer.
Results: The intermediate surgical guide presented perfect adaptation on the occlusal surfaces, promoting great stability for the repositioning and fixation of the maxilla in intermediate occlusion. The 18 operated patients responded as "completely satisfied" in relation to the aesthetic-functional result in this series studied. A very great similarity was found between the position of the maxillofacial skeleton in the preoperative virtual planning and that obtained post-operatively through the evaluation of teleradiography.
Conclusion: Virtual planning in craniomaxillofacial surgery has numerous advantages, such as reduced pre-operative laboratory time, greater precision in the creation of surgical guides, and better reproducibility of simulated results.

RESUMO

Introdução: A cirurgia ortognática envolve manipulação da arquitetura óssea facial, através de osteotomias, para restaurar a forma e a função, corrigindo a má oclusão, as desproporções maxilomandibulares e assimetrias faciais. O planejamento virtual em cirurgia ortognática é realizado com ajuda de softwares que utilizam as medidas reais do esqueleto craniofacial e registros da oclusão do paciente, através de uma análise 3D.
Método: Foram avaliados 18 pacientes com deformidades dentofaciais, de acordo com a classificação de Angle submetidos a cirurgia ortognática com o uso do planejamento virtual, entre 2018 e 2019. Os critérios de inclusão foram pacientes entre 16 e 60 anos com desproporções maxilo-mandibulares nas quais o tratamento ortodôntico isolado não era suficiente. Os critérios de exclusão foram a presença de lesões císticas ou tumorais nos maxilares e comorbidades clínicas que contraindicavam a cirurgia. O planejamento virtual foi realizado em todos os pacientes, utilizando o software Dolphin® Imaging 11 e os guias cirúrgicos confeccionados em impressora 3D.
Resultados: O guia cirúrgico intermediário apresentou adaptação perfeita nas faces oclusais promovendo grande estabilidade para o reposicionamento e fixação da maxila na oclusão intermediária. Os 18 pacientes operados responderam como "totalmente satisfeitos" em relação ao resultado estético-funcional nessa série estudada. Foi encontrada uma semelhança muito grande da posição do esqueleto maxilofacial no planejamento virtual pré-operatório e o obtido no pós-operatório, através da avaliação das telerradiografias.
Conclusão: O planejamento virtual em cirurgia craniomaxilofacial possui inúmeras vantagens, como diminuição do tempo laboratorial pré-operatório, maior precisão na confecção dos guias cirúrgicos e melhor reprodutibilidade dos resultados simulados.
