ABSTRACT

BACKGROUND: The advantages of cyanoacrylates in cutaneous wound closure have been widely demonstrated in the literature. However, these products have not been widely used in Brazil due to the high costs of octyl cyanoacrylates. Moreover, there have been only a few studies on the most affordable product, ethyl cyanoacrylate, as a cutaneous adhesive. The objective of this study was to evaluate the efficacy of ethyl cyanoacrylate in cutaneous excision repair. METHODS: A retrospective study conducted in patients with superficial skin lesions who underwent elliptical excision and repair by primary closure. The excisions were closed using deep relaxing sutures and either ethyl cyanoacrylate (case group; CaG) or intradermal suture (control group; CoG) on the skin surface. RESULTS: Thirty-three patients were identified with 43 cutaneous lesions (CaG, n = 20; CoG, n = 23). Unsightly scars (CaG = 20%; CoG = 48%), eye irritation (CaG = 15%; CoG = 0), dehiscence (CaG = 5%; CoG = 0), infection (CaG = 0; CoG = 4.3%), and contact dermatitis (CaG = 0; CoG = 4.3%) were the problems observed. The cosmetic results were satisfactory in almost all excisions (CaG = 100%; CoG = 95.7%). CONCLUSIONS: Ethyl cyanoacrylate was adequately tolerated in this study group, with no necrosis, allergic reactions, or infections. Ethyl cyanoacrylate presented several advantages over suture use, including low complication rates and high patient satisfaction.

RESUMO

INTRODUÇÃO: As vantagens dos cianoacrilatos em síntese cutânea têm sido sobejamente demonstradas na literatura. Entretanto, esses produtos têm sido subutilizados no Brasil, em decorrência dos elevados custos do octilcianoacrilato. Ademais, a forma mais economicamente acessível, etilcianoacrilato, tem sido pouco estudada como adesivo cutâneo. O objetivo deste estudo foi avaliar a eficiência do etilcianoacrilato na síntese de excisões cutâneas. MÉTODO: Estudo retrospectivo realizado em pacientes portadores de lesões cutâneas superficiais submetidos a excisão elíptica e reparo por fechamento primário. As excisões foram fechadas por meio de suturas profundas relaxadoras e, na superfície da pele, foi utilizado etilcianoacrilato (grupo caso - GCa) ou sutura intradérmica (grupo gontrole - GCo). RESULTADOS: Foram encontrados 33 pacientes portadores de 43 lesões cutâneas (GCa = 20 excisões; GCo = 23 excisões). Cicatrizes inestéticas (GCa = 20%; GCo = 48%), irritação ocular (GCa = 15%; GCo = 0), deiscência (GCa = 5%; GCo = 0), infecção (GCa = 0; GCo = 4,3%) e dermatite de contato (GCa = 0; GCo = 4,3%) foram os problemas encontrados. Os resultados cosméticos foram considerados satisfatórios na maioria das excisões (GCa = 100%, GCO = 95,7%). CONCLUSÕES: O etilcianoacrilato foi adequadamente tolerado neste grupo de estudo, sem induzir necrose, reações alérgicas ou infecções, apresentando diversas vantagens de uso em relação à sutura, incluindo reduzido índice de complicações e grande satisfação dos pacientes.

ABSTRACT

This works reports on the experience of the authors with the use of N-Butyl-Cyanoacrylate for the cutaneous closure of primary cheiloplasties in unilateral fissures, comparing the esthetic results with those to 6.0 mononylon sutures. Between April 1999 and December 2003, 62 primary cheiloplasties were performed in the Hospital Municipal Infantil Menino de Jesus utilizing the Millard's rotation-advancement lip repair technique. In Group A, comprised of 35 patients, the skin was sutured using 6.0 mononylon thread. For Group B, comprised of 27 patients, the skin was repaired using N-Butyl- Cyanoacrylate adhesive. The groups were evaluated on the 1st, 7th, 30th and 90th days after surgery. There were no cases of surgical infection. One (3.4%) patient of Group B presented with dehiscence less than one day after surgery requiring conventional suturing. One (2.8%) child of Group A required general anesthesia to remove the suture. The esthetic results of scaring were similar in both the groups studied. The authors stress the advantages of the optimization of this adhesive in the pediatric population. The advantages include the facility and simplicity of its use, the low cost, the time of the surgeon, reduction in the time of surgery and, most importantly, there is no necessity to remove the sutures, thereby reducing the stress of the child and family members in the postoperative return appointment.

RESUMO

Este trabalho mostra a experiência dos autores com o uso do N-Butil- Cianoacrilato no fechamento cutâneo de queiloplastias primárias em fissuras unilaterais, observando os resultados estéticos em relação à sutura de mononylon 6.0. Entre abril de 1999 e dezembro de 2003, 62 queiloplastias primárias foram realizadas no Hospital Municipal Infantil Menino de Jesus, utilizando-se a técnica de rotação e avanço de Millard. No grupo A, composto por 35 pacientes, a pele foi suturada com fio de mononylon 6.0. No grupo B, com 27 pacientes, a pele foi aproximada com o adesivo N-Butil-Cianoacrilato. Os grupos foram avaliados no pós-operatório 1, 7, 30 e 90 dias após a cirurgia. Não houve casos de infecção cirúrgica. Um (3,4%) paciente do grupo B apresentou deiscência no mesmo dia da cirurgia, necessitando de sutura convencional. Uma (2,8%) criança do grupo A necessitou de anestesia geral para a retirada da sutura. Os resultados estéticos das cicatrizes foram semelhantes nos dois grupos estudados. Os autores destacam as vantagens da otimização deste adesivo na população pediátrica, que incluem facilidade e simplicidade no uso, baixo custo, utilização do tempo do profissional, diminuição do tempo cirúrgico e, principalmente, a desnecessidade de remoção das suturas, diminuindo assim o estresse da criança e dos familiares no retorno pós-operatório.
