Clinical and surgical applications of the forearm flap: analysis of 89 cases

ABSTRACT

Introduction: In the search of skin segments that allow the transport among areas, the fascio-cutaneous forearm flap or Chinese flap arised. It was originated in anatomical studies accomplished by Fan, Qui and Zi, at the Shenyang Military Hospital, in 1978. The objective of this study is to demonstrate the multiple applications and the results obtained with the antebraquial flap. Method: The sample counted with data regarding 89 patients; 84.26% were male and 15.74% female. The forearm flap was used for covering the hand and fist in 61 patients, for thumb reconstruction in 10 patients, as free flap in 15 and in 3 patients it was used with direct flow. Results: Among the 89 patients, 2 developed total flap necrosis. One case developed slightly necrosis of the skin. None of the patients presented blood supply alterations in the hand after the radial or ulnar artery ligature. There was satisfactory covering of the areas, with good results. Conclusions: The antebraquial or "Chinese" flap is indicated for tegument covering in any area of the human body, with good aesthetic and functional results, so much for the donor area as for the rebuilt area.

RESUMO

Introdução: Na procura de segmentos de pele que permitam o transporte entre regiões próximas ou distantes, surgiu o retalho fáscio-cutâneo de antebraço, conhecido por retalho chinês, que teve sua origem em estudos anatômicos realizados por Fan, Qui e Zi, no Shenyang Military Hospital, em 1978. O objetivo deste trabalho é demonstrar as múltiplas aplicações e os resultados obtidos com o retalho antebraquial. Método: A amostra contou com dados referentes a 89 pacientes. Destes, 84,26% eram do sexo masculino e 15,74% do sexo feminino. O retalho foi utilizado para cobertura na mão e punho em 61 pacientes, para reconstrução de polegar em 10, como retalho livre em 15 e, em 3 pacientes, foi usado com fluxo anterógrado (direto), para cobertura de exposição óssea no cotovelo e em terço proximal de antebraço. Resultados: Entre os 89 pacientes operados, 2 evoluíram para necrose total do retalho. Em um caso houve necrose da pele, permanecendo a fáscia íntegra. Nenhum dos pacientes apresentou alteração na vascularização da mão após a ligadura da artéria radial ou da artéria ulnar. Houve cobertura satisfatória das áreas, com bons resultados. Conclusão: O retalho antebraquial ou "chinês" é indicado para coberturas tegumentares em qualquer região do corpo humano, com bons resultados estéticos e funcionais, tanto para a área doadora quanto para a área reconstruída.
