Perioperative systematization for the prevention of hematomas following face-lift procedures: a personal approach based on 1,138 surgical cases

ABSTRACT

Introduction: Hematoma, the most frequent complication of face-lift procedures, may require a second surgical approach, which delays patient recovery. In the literature, its incidence ranges from 0.2% to 8%, and further studies are essential to standardize preventive measures. The objective is to present a proposal of perioperative systematization for effectively prevention of hematoma formation after rhytidectomies.
Methods: We analyzed the medical records of 594 patients who underwent operation by the author between 2011 and 2018 to compare the incidence of hematomas before and after the systematization implemented in 2015.
Results: From July 2011 to December 2014, before the adoption of the systematization, the incidence of hematomas was 3.43% in 233 cases. After its adoption, the incidence decreased to 1.66% in 361 cases. The last 177 consecutive cases did not have this complication.
Conclusion: We observed a significant reduction in the incidence of hematomas following rhytidectomy after the use of the proposed standardization. None of the measures would be effective alone; thus, their combined adoption is essential in preventing this serious complication.

Introdução: O hematoma, complicação mais frequente do face-lift, pode exigir reabordagem cirúrgica e atrasar a recuperação do paciente. Na literatura, sua incidência varia entre 0,2 e 8%, sendo fundamentais novos estudos para padronização das medidas de prevenção. O objetivo é apresentar uma proposta de sistematização perioperatória que previna eficientemente a formação de hematomas em ritidoplastias.
Métodos: Foram analisados 594 prontuários de pacientes operados pelo autor entre os anos de 2011 a 2018 a fim de se comparar as incidências de hematomas anteriores e posteriores à sistematização implementada no ano de 2015.
Resultados: De julho de 2011 a dezembro de 2014, antes da adoção da sistematização, houve uma incidência de hematomas de 3,43% em 233 casos. Após sua adoção, houve uma queda para 1,66% em 361 casos realizados. Os últimos 177 casos consecutivos não apresentaram a complicação.
Conclusão: Observamos redução expressiva da incidência de hematomas pósritidoplastias após o uso da padronização proposta. Nenhuma das medidas adotadas seria eficiente isoladamente, sendo o conjunto essencial na prevenção desta grave complicação.

Temporal lift with repositioning of the orbicular muscle and eyebrow tail

ABSTRACT

Introduction: The superficial musculoaponeurotic system (SMAS) is the best structure we have at our disposal to elevate and reposition the face and neck in the facelift. However, in the temporoorbital region, this structure is often neglected. In the search for a simple, efficient and safe solution to treat temporoorbital droop, the author describes a temporal lift tactic with eyebrow tail repositioning to treat aging and sagging of the temporoorbital region.
Methods: Treatment was performed in 358 patients between 2017 and 2020 in complete or temporary lifts. Only 30 were included in the article because they were exclusively submitted to temporal lifts, with or without blepharoplasty. Through a broken marginal intracapillary incision in the temporal region and with supraSMAS detachment, musculoaponeurotic treatment of the orbitotemporal region was performed, in addition to excess skin resection.
Results: The tactic presented was efficient in lifting and opening the tail of the eyebrows in all treated cases, in addition to the effect of loss of contractile function of the lateral portion of the orbicularis muscle, with a significant improvement in periorbital wrinkles and orbitotemporal sagging.
Conclusion: The effectiveness and excellent results achieved with the described operative tactic, associated with the scarcity of isolated or specific treatment options for the orbitotemporal region, make the proposed temporoorbital lift an excellent alternative for the rejuvenation of this region.

RESUMO

Introdução: O sistema musculoaponeurótico superficial (SMAS) é a melhor estrutura que temos à nossa disposição para elevar e reposicionar a face e o pescoço no face lift. No entanto, na região têmporo-orbitária, esta estrutura é frequentemente negligenciada. Na busca por uma solução de execução simples, eficiente e segura para tratar a queda têmporo-orbitária, o autor descreve uma tática de lift temporal com reposicionamento da cauda das sobrancelhas para tratamento do envelhecimento e flacidez da região têmporo-orbitária.
Métodos: Foi realizado o tratamento em 358 pacientes entre 2017 e 2020, em lifts completos ou apenas temporais. Destes, apenas 30 foram incluídos no artigo por terem sido submetidos exclusivamente a lifts temporais, acompanhados ou não de blefaroplastias. Através de incisão intracapilar marginal quebrada em região temporal e com descolamento supraSMAS, foi realizado tratamento musculoaponeurótico da região órbito-temporal, além de ressecção da pele em excesso.
Resultados: A tática apresentada foi eficiente na elevação e na abertura da cauda das sobrancelhas em todos os casos tratados, além do efeito de perda da função contrátil da porção lateral do músculo orbicular, com melhora significativa das rugas periorbitais e da flacidez órbito-temporal.
Conclusão: A eficácia e os ótimos resultados alcançados com a tática operatória descrita, associada à escassez de opções de tratamento isolado ou específico da região órbito-temporal, tornam o lift têmporo-orbitário proposto uma excelente alternativa para o rejuvenescimento desta região.

A new concept in neck aging: "cervicofacial waist"

ABSTRACT

Introduction: The aging of the lower third of the face stimulates the definition deletion of the mandibular border and the cervicofacial angle, besides the appearance of platysmal bands in the anterior neck region. Patients have high expectations that the facelift will significantly resolve such effects. To achieve these results, we have developed an approach to defining what we call the cervicofacial waist.
Methods: The author proposes a new surgical facelift approach that has been performed on 444 consecutive patients since 2015, which involves an aggressive plication of the SMAS-platysma plus lateral traction, also by plication, of the platysma medial bands. Results: Despite the difficulty in comparing results with different techniques, we perceive a significant improvement in our results with the new surgical technique, without increasing the complication rates.
Discussion: We are looking for new techniques because the cervical region's traditional treatments seem flawed and based on ill-founded concepts. These involve the medial plication of the platysma bands by a submental approach, bringing them closer together and impairing the cranial elevation of the SMAS-platysma. Thus, we started to directly perform lateral plications on the bands through the lateral access of the face detachment, making a cervicofacial waist.
Conclusion: The sum of the effects of the plication of the SMAS with the lateral plication of the platysma medial band makes the cervicofacial definition clearer with important optimization of the desired aesthetic effects.

RESUMO

Introdução: O envelhecimento do terço inferior da face promove o apagamento da definição da borda mandibular e do ângulo cervicofacial, além do surgimento das bandas platismais na região anterior do pescoço. Os pacientes têm grande expectativa de que o facelift resolva de maneira significativa tais efeitos. Para alcançar estes resultados, desenvolvemos uma abordagem cuja finalidade é a definição marcante do que chamamos de cintura cervicofacial.
Métodos: O autor propõe uma nova abordagem cirúrgica de facelift já realizada em 444 pacientes consecutivos, desde 2015, que envolve uma plicatura agressiva do SMAS-platisma somada a uma tração lateral, também por plicatura, das bandas mediais do platisma.
Resultados: Apesar da dificuldade em comparar resultados com diferentes técnicas, percebemos claramente uma melhora significativa nos nossos resultados com a nova técnica cirúrgica, sem aumento dos índices de complicações.
Discussão: Buscamos novas técnicas pelo fato de que os tratamentos clássicos da região cervical nos parecem falhos e baseados em conceitos mal fundamentados. Estes envolvem a plicatura medial das bandas platismais por abordagem submentoniana, aproximando-as e prejudicando a elevação cranial do SMAS-platisma. Assim, passamos a realizar plicaturas laterais diretamente sobre as bandas através do acesso lateral do descolamento da face, com confecção de uma cintura cervicofacial.
Conclusão: A soma dos efeitos da plicatura do SMAS com a plicatura lateral da banda medial do platisma torna a definição cervicofacial mais nítida com otimização importante dos efeitos estéticos desejados.

Neoomphaloplasty using an X-shaped incision in 401 consecutive abdominoplasties

ABSTRACT

BACKGROUND: The search for a natural-looking umbilicus that does not compromise the aesthetic results of abdominoplasty began in the 1950s. Since then, the umbilical transposition technique has remained the preferred method among most surgeons. The objective of this study was to describe a new technique for umbilical reconstruction using small X-shaped incisions in the abdominal skin to create 4 small V-shaped flaps. These flaps are then sutured to the aponeurosis using absorbable sutures. METHODS: This technique was used in 401 consecutive abdominoplasties and a survey was conducted to determine the level of patient satisfaction with the aesthetic results of their umbilicus. RESULTS: Remarkably natural-looking umbilici were obtained using the described technique, which was also easily reproducible, did not cause any apparent scars, had a low incidence of complications (9.7%), and resulted in high patient satisfaction with the aesthetic results of the surgery. CONCLUSIONS: The described technique represented a simple, safe, and fast approach to create new, natural-looking umbilici with a low rate of complications.

RESUMO

INTRODUÇÃO: A procura por um umbigo de aspecto natural, que não comprometa o resultado estético das abdominoplastias, começou na década de 1950 e, desde então, a técnica de transposição umbilical é a preferida pela maioria dos cirurgiões. O objetivo deste artigo é descrever nova técnica de reconstrução umbilical, utilizando pequenas incisões em forma de X na pele abdominal, que criam 4 pequenos retalhos em forma de V, os quais são suturados à aponeurose com suturas absorvíveis. MÉTODO: Essa técnica foi utilizada em 401 abdominoplastias consecutivas e foi realizada uma pesquisa para determinar o nível de satisfação dos pacientes com o resultado estético dos umbigos. RESULTADOS: Foram criados umbigos extremamente naturais, de maneira facilmente reprodutível e sem cicatrizes aparentes, com baixa incidência de complicações (9,7%) e alto índice de satisfação dos pacientes. CONCLUSÕES: A técnica utilizada permitiu a criação de um novo umbigo com aparência natural, de maneira simples, segura e rápida, com baixo índice de complicações.

Necrose extensa em face pós-ritidoplastia: relato de caso

RESUMO

Introdução: Necrose do retalho cutâneo representa uma importante complicação cirúrgica pós-ritidoplastia, de difícil tratamento e com possibilidade de graves deformidades.
Métodos: Por meio do relato de caso de ritidoplastia que evoluiu com grande necrose bilateral, objetiva-se discutir os diversos aspectos desta complicação pós-operatória.
Resultados: Homem de 62 anos, submetido a ritidoplastia, que apresentou grande hematoma expansivo bilateral tratado com drenagem cirúrgica. Evoluiu rapidamente com necrose bilateral pré-auricular do retalho cutâneo. Após tratamento conservador, houve completa retração e epitelização da ferida, seguido de correção cirúrgica das cicatrizes com ótimo resultado. Quando a necrose se instala rapidamente inviabilizando tentativas de melhora da perfusão local, as opções de tratamento tornam-se limitadas: desbridamentos que devem ser tardios e conservadores, cicatrização por segunda intenção e curativos diversos.
Conclusão: Mesmo em grandes necroses de face, é possível se obter ótimos resultados finais através de tratamento conservador e pouco intervencionista.
