ABSTRACT

Introduction: The lipodystrophy observed in positive HIV patients, resultant of the use of antiretrovirals leads to the reduction or loss of self-esteem, social exclusion and difficulties in the familiar and social relationship. These psychological riots contribute for the difficulty of adhesion or even the abandonment to the treatment. In 2004, a portray of "Health Department" included, in the table of the "Only Service of Health", eight procedures for the treatment of positive HIV patients who present lipodistrophy, in between them, the face fulfilling with polimetilmetacrilato (PMMA). Method: The present study observed the results of the procedure for 12 months in 46 patients. The average of age of the group was 35 years of old with predominance of the masculine sex in 56% of the cases. The submitted regions to the treatment had been the nasojugal (100%), the temporal (83%) and the parotid (23%). The injected average volume in the face of each patient was of 12.5 ml (varying between 4 and 30 ml). For 69% it did not have necessity of complementary procedures. However, 27% of the patients had been submitted through 2 fulfillings and 4% to 3 fulfillings, being most frequent the nasojugal region (86%). Complementaries fulfillings were done, at least, 3 months after the last procedure. Results: The time of treatment with antiretrovirals did not show an evident relation with the injected volume. The fulfilling procedure had no problem itself in the great majority of the time, wether in intra or postoperative , for the medical team or to the patients. Conclusion: The achieved levels of satisfaction confirm the expectations of the Health Department into the rescue of the self-esteem and the social reinclusion of these patients.

Introdução: A lipodistrofia observada nos pacientes HIV positivos, resultante do uso de anti-retrovirais, leva à diminuição ou perda da auto-estima, exclusão social e dificuldades no relacionamento familiar e social. Estes distúrbios psicológicos contribuem para a dificuldade de adesão ou até o abandono ao tratamento. Em 2004, uma Portaria do Ministério da Saúde incluiu, na tabela do Serviço Único de Saúde, oito procedimentos para o tratamento de pacientes HIV positivos que apresentam lipodistrofias, entre eles o preenchimento facial com polimetilmetacrilato (PMMA). Método: O presente estudo observou os resultados do procedimento por 12 meses em 46 pacientes. A média de idade do grupo foi de 35 anos, com predominância do sexo masculino em 56% dos casos. As regiões submetidas a tratamento foram a nasojugal (100%), a temporal (83%) e a parotídea (23%). O volume médio injetado na face de cada paciente foi de 12,5 ml (variando entre 4 e 30 ml). Para 69% não houve necessidade de procedimentos complementares. Porém, 27% dos pacientes realizaram 2 preenchimentos e 4% foram submetidos a 3 preenchimentos, sendo a região nasojugal a mais freqüente (86%). Os preenchimentos complementares eram feitos, no mínimo, 3 meses após o último procedimento. Resultados: O tempo de tratamento com anti-retrovirais não mostrou uma relação evidente com o volume infundido. O procedimento de preenchimento transcorreu, na grande maioria das vezes, sem nenhuma intercorrência intra e pós-operatória, tanto para a equipe médica quanto para os pacientes. Conclusão: Os níveis de satisfação obtidos confirmam as expectativas do Ministério da Saúde quanto ao resgate da auto-estima e a reinclusão social destes pacientes.
