ABSTRACT

The purpose of this study is to present a technical alternative to reconstruct muscle-cutaneous defects on the face, with transfer of innervated muscles sub-segments (sub-units) of the latissimus dorsi flap to replace multiples facial mimetic muscles. It is described three clinical cases of mimetic muscle losses associated to the skin after trauma. These cases were treated with microsurgical transfer of the sub-units of the latissimus dorsi muscle that present size, shape and weight similar than mimetic muscles. The segmental ramus of the toracodorsal nerve was anastomosed on buccal ramus in two cases and mandibular marginal ramus of the facial nerve in one case. Each ones of the sub-segments have an artery and vein that nourish and drain the muscle unit and nerve with motor. Studies were done about muscular reconstruction but these present limitations to replace multiple mimetic muscles on different vectors of actions, remaining asymmetry. This technique showed to be safe and efficient after the evaluation and comparison with digital photo and video registration between static and dynamic aspects on pre and post-operative one year and using electromyography digital video registration.

RESUMO

O objetivo deste estudo é apresentar uma alternativa técnica para reconstrução de perdas músculo-cutâneas na face, com a transferência de subsegmentos (subunidades) inervados do músculo grande dorsal para a substituição dos diversos músculos da mímica facial. São descritos três casos de trauma com perda de pele e músculos da mímica. Foram corrigidos com transferências microcirúrgicas de subunidades de músculo grande dorsal que apresentam, forma, tamanho e peso similares aos dos músculos da mímica. Respeitaram-se as origens e inserções e vetores de ação para a mímica desejada. Cada um dos subsegmentos tem artéria e veia que irrigam a unidade muscular e também nervo com componente motor. Os pacientes foram avaliados, comparando-se o pré e pós-operatório de um ano com foto e vídeo com mímica em repouso e dinâmica e eletromiografia (documentada em vídeo digital). Observou- se atividade mioelétrica dos músculos transplantados. Verificou-se que a técnica foi eficaz do ponto de vista funcional e estético.
