Rhytidoplasty practices and histological notes for the facial skin of post-bariatric patients

ABSTRACT

Introduction: The number of patients classified as overweight or obese has resulted in an increased number of post-bariatric patients, generating a demand for rhytidoplasty in this public profile. expose the routine and surgical tactics in a teaching hospital, patient profiles, subcutaneous cellular tissue, and the superficial musculoaponeurotic system are compared between post-bariatric and non-post-bariatric patients, as well as histological superficial comparison of the skin.
Methods: A review of the data of 32 cases treated in 2012-2016 was conducted in a public hospital in Goiânia-GO.
Results: The tactics of rhytidoplasty used in post-bariatric patients do not differ significantly from those used in nonpost- bariatric patients. A histopathological analysis of the collected samples showed marked differences between post-bariatric and non-post-bariatric patients.
Conclusion: The surgical technique of rhytidoplasty of post-bariatric patients sustained specific details with satisfactory postoperative surgical results, skin with worse histological quality, perioperative complications within the expected range, and the need for further studies to assess the procedure's durability

RESUMO

Introdução: O número de pacientes classificados com sobrepeso ou obesos e, com isso, dos pacientes pós-bariátricas vem aumentando, gerando uma demanda por ritidoplastias neste perfil de público. O objetivo é expor a rotina e táticas cirúrgicas em um hospital de ensino, o perfil destes pacientes, além de um comparativo superficial histológico da pele, tecido celular subcutâneo e SMAS, entre os pacientes pós e não pós-bariátrica.
Métodos: Foi realizado revisão de dados do centro cirúrgico e de prontuário entre os anos de 2012 e 2016 em um hospital público na cidade de Goiânia-GO, com levantamento de 32 casos.
Resultados: A tática de ritidoplastia utilizada nos pacientes pós-bariátrica não apresenta grandes diferenças entre a utilizada nos pacientes não pós-bariátrica. A análise histopatológica das amostras colhidas evidenciou diferenças marcantes entre os pacientes pós e não pós-bariátrica.
Conclusão: Percebeu-se que a técnica operatória na ritidoplastia do paciente pós-bariátrica sustentou detalhes específicos, com resultado cirúrgico pós-operatório satisfatório, uma pele de pior qualidade histológica, com complicações perioperatórias dentro do esperado e com necessidade de maiores estudos para avaliar durabilidade do procedimento.

Sistematizacion and conduct of plastic surgery service of hospital geral de goiânia in mammoplasty in massive weight loss patients in last ten years

ABSTRACT

BACKGROUND: With the increase of the population obesity indices occurred a growth of the numbers of ex-obese patients and aesthetic/reconstructive plastic surgery in this population. Objectives: To present a systematic approach adopted in patients undergoing breast surgery after massive weight loss at Hospital Geral de Goiânia (HGG) in the last 10 years, and assess the results. METHOD: We conducted a retrospective study of cases of breast surgery after massive weight loss in the Department of Plastic Surgery of HGG, from January 2002 to December 2011. The patients were individually analyzed preoperatively and submitted to the classification of Kaluf to breast dysmorphism after major weight loss. The operative technique varied depending on the classification of the type of breast. RESULTS: The sample totaled 73 female patients, with mean age of 44 years and mean weight loss of 48 kg. In 41 patients (56.16%) was used the technique with inclusion of prostheses and in 32 (43.83 %) chose to not include silicone implants. Complications occurred in eight patients (10.95%), one case of hematoma (1.36%), two cases of suffering partial nipple-areola complex (2.73%), five cases of small suture dehiscence or delayed healing (6.84%) and one case of breast ptosis in the immediate postoperative period (1.36%). CONCLUSION: Due to the wide range of dimorphisms in emaciated patients, in particular changes in their breasts, we note that it is necessary to establish protocols and routines for effective assessment and treatment, with the least possible morbidity, associated with better cosmetic results.

RESUMO

INTRODUÇÃO: Com o aumento dos índices de obesidade populacional ocorreu um crescimento do número de pacientes ex-obesos, e de cirurgias plásticas nesta população. OBJETIVOS: Apresentar a sistematização e conduta adotada nos pacientes submetidos a mamoplastia, após grande perda ponderal, operados no Hospital Geral de Goiânia (HGG) nos últimos 10 anos, bem como avaliar os resultados obtidos. Método: Foi realizado um estudo retrospectivo dos casos de mamoplastia após grande perda ponderal, operados no Serviço de Cirurgia Plástica do HGG, de janeiro de 2002 a dezembro de 2011. As pacientes foram analisadas individualmente no pré-operatório e submetidas à classificação de Kaluf, para dismorfias mamárias após grande emagrecimento. A técnica operatória variou em função da classificação do tipo de mama. RESULTADOS: A casuística totalizou 73 pacientes, do sexo feminino, com faixa etária média de 44 anos, e perda ponderal média de 48 Kg. Em 41 pacientes (56,16%) foi utilizada a técnica com inclusão de próteses, e em 32 (43,83%) optou-se por não incluir os implantes de silicone. As complicações ocorreram em oito pacientes (10,95%), sendo um caso de hematoma (1,36%), dois casos de sofrimento parcial do complexo aréolo-mamilar (2,73%), cinco casos de pequenas deiscências de sutura ou retardo de cicatrização (6,84%) e um caso de ptose de mama no pós-operatório imediato (1,36%). CONCLUSÃO: Devido à grande variação de dismorfismos em pacientes emagrecidos, em especial as alterações mamárias, notamos que se torna necessário o estabelecimento de protocolos e rotinas para avaliações e tratamentos eficazes, com a menor morbimortalidade possível, associados aos melhores resultados estéticos.

ABSTRACT

The obesity represents a chronic illness. The loss of weight after bariatric surgery varies from 40 to 60% of the initial weight, bringing deformities to the post-bariatric patients. The authors present, in this study, an analysis of 165 surgical procedures had been carried through at Service of Plastic Surgery of the General Hospital of Goiânia. They intended to evaluate the results gotten with the adopted surgical techniques, their complications and the psychological profile of the patients after the plastic surgeries. One concludes that the systematization and classification of the diverse types of deformities guide the surgeon in the choice of the surgical technique to be implemented. The complications in the post-bariatric patients are greater than in the habitual ones. The high degree of personal satisfaction and the social reintegration are evident after the plastic surgeries.

RESUMO

A obesidade representa uma doença crônica. A perda de peso pós-cirurgia bariátrica varia de 40 a 60% do peso inicial, trazendo deformidades aos ex-obesos. Os autores apresentam, neste estudo, uma análise de 165 procedimentos cirúrgicos, realizados no Serviço de Cirurgia Plástica do Hospital Geral de Goiânia. Objetivou-se avaliar os resultados obtidos com as técnicas cirúrgicas adotadas, suas complicações e o perfil psicológico dos pacientes após as cirurgias plásticas. Conclui-se que a sistematização e classificação dos diversos tipos de deformidades norteiam o cirurgião na escolha da técnica cirúrgica a ser implementada. As complicações nos pacientes ex-obesos são maiores do que nas habituais. O alto grau de satisfação pessoal e a reintegração social são evidentes após as cirurgias plásticas.

Pubic region's treatment in abdominoplasty of patients with great loss of weight

ABSTRACT

Introduction: Thinking about the affliction of the patients with great weight loss because of their bodies, we developed a new procedure to treat the pubic area in these patients. Objectives: To describe a new way to treat the pubic area in patients with great loss of weight with an important pubic ptosis. Methods: The incision of the abdominoplasty is performed a little bit lower in the pubic region, about 5cm from the vulvar fissure with the skin pulled up. Then, the Scarpa fascia is fixed more cranial in the aponeurosis after its undermining, with symmetric nots. You can see a great improvement in this region as soon as you treat it. Results: We had an 100% satisfaction range among our patients, once we could improve substantially the looking of this region without increasing the size of the scar, and could also make an effective resection of the excess of skin and fat tissue. Conclusion: This is a new solution to treat the ptotic pubic region in patients with great loss of weight, improving their body contour. It showed to be a low morbidity technique, easy to performed, less invasive, giving great results and with no additional scars for the patient.

RESUMO

Introdução: Com base na angústia dos pacientes com grande perda ponderal em relação ao seu corpo, desenvolvemos uma nova tática para tratamento da ptose da região pubiana. Objetivos: Demonstrar tática cirúrgica para tratamento da região pubiana de pacientes com grande perda ponderal e ptose importante dessa região. Método: Faz-se a marcação convencional da abdominoplastia com incisão pubiana mais baixa, a 5 cm da fúrcula vulvar, tracionando-se a pele para cima. Realiza-se a fixação mais cranial da fáscia de Scarpa na aponeurose com pontos simétricos em "U", após seu descolamento, observando-se imediata melhora do aspecto dessa região. Resultados: Obtivemos satisfação de 100% entre os pacientes, por conseguirmos melhorar o aspecto da região pubiana sem aumentar a cicatriz resultante e realizar uma retirada efetiva da redundância de pele e de tecido celular subcutâneo. Conclusão: Trata-se de uma nova solução para melhora da região pubiana e do contorno corporal dos pacientes pós grande perda ponderal, apresentando baixa morbidade, sendo pouco invasiva, de fácil execução e de resultados agradáveis, com nenhuma cicatriz adicional ao paciente.
