Galactorrhea: how to address this unusual complication after augmentation mammoplasty

ABSTRACT

INTRODUCTION: Galactorrhea and galactocele formation after breast augmentation are complications reported in the literature, but the cause remains unknown. METHODS: We present a case of a 28-year-old patient who underwent breast augmentation surgery via the inframammary fold with an implant placed in the subfascial plane, which developed galactorrhea from the incision on the seventh postoperative day, and we propose an algorithm for the diagnosis and treatment of galactorrhea after mammoplasties. RESULTS: The complication was treated with the use of a lactation suppressor, cabergoline, presenting good outcomes. CONCLUSION: Galactorrhea is an uncommon complication after augmentation mammoplasties, which should always be considered in cases of secretions from an incision because it is a differential diagnosis for infections.

RESUMO

INTRODUÇÃO: Galactorreia e formação de galactocele após mamoplastia de aumento é uma complicação descrita na literatura, porém a causa permanece desconhecida. MÉTODOS: Apresentamos um caso de uma paciente de 28 anos que foi submetida à cirurgia de mamoplastia de aumento via sulco inframamário, com implante colocado no plano subfascial, que evoluiu, no 7º dia pós-operatório, com galactorreia exteriorizada pela incisão, e propomos um algoritmo para diagnóstico e tratamento de galactorreia após mamoplastias. RESULTADOS: A complicação foi tratada com o uso de agente supressor da lactação, a cabergolina, apresentando boa evolução. CONCLUSÃO: Galactorreia é uma complicação incomum após mamoplastias de aumento, que deve ser sempre lembrada em casos de drenagem de secreção pela incisão por tratar-se de um diagnóstico diferencial com infecção.

Profile ofplastic surgeons in the state of Paraná, Brazil

ABSTRACT

INTRODUCTION: In the present study, we aimed to outline the overall practice profile of plastic surgeons in the state of Paraná. It represents an initial project that aimed not to provide generalizations but to understand how plastic surgeons think and their professional aspirations, goals, and performance. METHODS: A survey was conducted using a questionnaire consisting of 37 multiple-choice questions, mostly focusing on the current job market for plastic surgeons. The link to the online questionnaire was e-mailed to 263 members of the Brazilian Society of Plastic Surgery-Regional Paraná (SBCP-PR), which was also accessible via the SBCP-PR website, through a banner on its home page. The overall time to complete the questionnaire ranged from 3 to 5 minutes. RESULTS: The response rate was 40.5%. Most interviewees operated only in one city, and more than half provided services only through health-care insurance plans and private practice. Half of the participants considered the market as extremely competitive and searched for several alternatives for self-maintenance, including increasing installment payment terms, lowering prices, and working at other locations. A significant proportion of the respondents believed that they lost more than 30 patients owing to pricing in the previous year and performed 10-20 surgeries per month on average. Most of the surgeons did not operate with an auxiliary plastic surgeon because this would increase their operational cost or because technologists could perform this function. Most participants believe that the number of trained residents in the state of Paraná is excessive. CONCLUSIONS: In Paraná, plastic surgeons are concerned about the job market and therefore search for alternatives to remain competitive.

RESUMO

INTRODUÇÃO: Esta pesquisa tem por objetivo traçar o perfil do cirurgião plástico do estado do Paraná. É um projeto inicial que não tem a pretensão de indicar respostas, mas sim de entender o que pensa, quais os anseios e objetivos na profissão e como atua o profissional dessa especialidade. MÉTODO: Foi idealizada uma pesquisa com 37 perguntas de múltipla escolha, em sua maioria com foco no mercado de trabalho. Ela foi enviada por e-mail aos 263 membros da Sociedade Brasileira de Cirurgia Plástica - Regional Paraná (SBCP-PR), sob forma de um link que dava acesso ao questionário. O link também estava disponível no website da SBCP-PR, com um banner em sua página inicial. O tempo médio para preenchimento do questionário variou de 3 minutos a 5 minutos. RESULTADOS: O índice de resposta foi de 40,5%. A maioria dos entrevistados atua apenas em uma cidade, e mais da metade atende apenas convênios e particulares. Metade dos participantes considera o mercado extremamente competitivo e procura várias alternativas para se manter, entre elas: aumentar parcelamento, baixar preços e atuar em outros locais. Uma parcela significativa acredita ter perdido mais de 30 pacientes por preço no último ano e faz, em média, 10 a 20 cirurgias mensais. A maioria dos participantes não opera com auxiliar cirurgião plástico, pois consideram que aumenta o custo ou que instrumentadoras podem fazer a mesma função. A maioria dos participantes acredita que o número de residentes formados no estado é excessivo. CONCLUSÕES: O cirurgião plástico no Paraná está muito preocupado com o mercado de trabalho e procura alternativas para se manter competitivo.

Tissue expansion at Hospital de Clínicas-UFPR: our experience

ABSTRACT

Background: The shortage of tissue for large defect reconstruction is a challenge for the plastic surgeon. Tissue expansion emerged in this context, and in the last 30 years has become one of the most widely used modalities in reconstructive surgery. Tissue expansion is a very versatile technique that can be performed in patients of all ages for the correction of different pathologies. The most common indications are burn sequelae and giant congenital nevus. The present study describes the indications and use of tissue expanders at the Hospital de Clínicas of Universidade Federal do Paraná. Methods: Patients who underwent tissue expansion for reconstructive surgery between January 2005 and December 2009 were retrospectively reviewed. Results: A total of 24 patients (70.8% female and 29.2% male) were analyzed. Ages ranged from 3 to 46 years old (average, 17.1 years). The most common indication for tissue expansion was the treatment of burn sequelae (62.5%), mainly in the head and neck. Alopecia was the second most prevalent indication (29.2%), followed by scar retraction in the neck (20.8%). Other indications were giant congenital melanocytic nevus (16.7%), Poland's syndrome (8.3%), abdominal scar (8.3%), and amastia (4.2%). Complications developed in 11 patients, and the highest incidence of complications, reported in 8 (72.7%) patients, was among those with burn sequelae as the primary pathology. The complications were infection, rupture, extrusion, wound dehiscence, and displacement of the expander. Conclusions: Tissue expansion is indicated for the treatment of several diseases among which burn sequelae is one of the most common indications.

RESUMO

Introdução: A escassez de tecidos para reconstrução de grandes defeitos é um desafio ao cirurgião plástico. Nesse contexto, surgiu a expansão tecidual, que, nos últimos 30 anos, se tornou uma das modalidades mais utilizadas na cirurgia reparadora. A expansão é uma técnica muito versátil, que pode ser realizada em todas as idades e para correção de diferentes afecções. As principais indicações são sequelas de queimadura e nevo congênito gigante. Este estudo teve como objetivo demonstrar as indicações na utilização dos expansores tissulares e sua evolução em pacientes do Hospital de Clínicas da Universidade Federal do Paraná. Métodos: Estudo retrospectivo, incluindo pacientes submetidos a expansão tecidual para cirurgia reconstrutora, no período de janeiro 2005 a dezembro 2009. Resultados: Foram analisados 24 pacientes, sendo 70,8% do sexo feminino e 29,2% do sexo masculino. A idade variou entre 3 anos e 46 anos (média de 17,1 anos). A principal indicação de expansão tecidual foi o tratamento de sequelas de queimaduras (62,5%), principalmente na região da cabeça e do pescoço. Alopecia foi a indicação mais prevalente (29,2%), seguida por retração cicatricial em região cervical (20,8%). Outras indicações foram nevo melanocítico congênito gigante (16,7%), síndrome de Poland (8,3%), cicatriz abdominal (8,3%) e amastia (4,2%). Entre os pacientes avaliados, 11 evoluíram com alguma complicação, 8 (72,7%) dos quais tinham como doença primária sequela de queimaduras, demonstrando maior incidência de complicações em relação às outras indicações. As complicações encontradas foram: infecção, ruptura, extrusão, deiscência de sutura e deslocamento do expansor. Conclusões: A expansão tissular é indicada no tratamento de múltiplas doenças e uma das principais indicações continua sendo o tratamento de sequelas de queimaduras.

Brachial-dorsal mammoplasty: body contour surgery of the upper region of the trunk after major weight loss

ABSTRACT

INTRODUCTION: In Brazil, 88000 bariatric surgeries were performed in 2014. After weight loss, deflation occurs around the trunk, with sagging and folding of excess skin of the breasts in the posterior dorsal region . We, therefore, propose a surgical procedure to treat upper region of the entire trunk, including breasts, side and rear back, and arms. OBJECTIVE: To present a personal experience in brachial-dorsal mammoplasty surgeries. METHOD: We operated 13 patients from 2007 to 2014, 11 women and two men. RESULTS: The average age was 41.9 years. The brachial-dorsal mammoplasty was performed in all patients. In three cases, L-brachial-dorsal mammoplasty was performed, due to lower latero-posterior deformity. The average surgery duration was 4 hours and 30 minutes. The average Body Mass Index (BMI), before bariatric surgery, was 53.5 kg/m2. Weight loss ranged from 47 to 114 kg. The pre-surgery BMI ranged from 23 to 39 kg/m2. There were no cases of infection, large dehiscence or seroma. Two patients developed hematoma, requiring a new intervention for drainage. CONCLUSION: Deformities of the thoracic region in patients with large weight loss are variable, requiring several surgical treatments that should address the entire chest as a single anatomical area. It is important to understand the deformity that each patient presents and adapt the surgical approach accordingly. The main indication for a brachial-dorsal mammoplasty is lateral drop of the inframammary fold.

RESUMO

INTRODUÇÃO: No Brasil foram realizadas 88 mil cirurgias bariátricas em 2014. Após o emagrecimento, ocorre desinsuflação de todo o tronco, com flacidez e dobra de excesso cutâneo das mamas à região dorsal posterior. Por isso, propomos procedimento cirúrgico que trate a região superior toda do tronco, incluindo mamas, dorso lateral e posterior e braços. OBJETIVO: Mostrar a experiência pessoal em cirurgias de braquio-dorso-mamoplastia. MÉTODO: Foram operados 13 pacientes no período de 2007 a 2014, sendo 11 mulheres e dois homens. RESULTADOS: A média de idade foi de 41,9 anos. A braquio-dorso-mamoplastia foi realizada em todos os pacientes. Em três casos, foi realizada braquio-dorso-mamoplastia em L, devido à menor deformidade latero-posterior. A duração média das cirurgias foi de 4 horas e 30 minutos. O Índice de Massa Corporal (IMC) médio, antes da cirurgia bariátrica, foi de 53,5 kg/m2. As perdas ponderais variaram de 47 a 114 kg. O IMC pré-operatório variou de 23 a 39 kg/m2. Não houve casos de infecção, grandes deiscências ou seroma. Duas pacientes evoluíram com hematoma, necessitando de nova intervenção para drenagem. CONCLUSÃO: As deformidades da região torácica no paciente com perda ponderal maciça são variáveis, sendo necessários diversos tratamentos cirúrgicos que deverão abordar o tórax inteiro como área anatômica única. É importante compreender a deformidade que cada paciente apresenta e adequar a abordagem cirúrgica em conformidade. A indicação principal para uma braquio-dorso-mamoplastia é a queda lateral do sulco inframamário.

ABSTRACT

Introduction: After evaluating the technique chosen for unilateral cleft lip surgery, we notice a negative outcome of the nose aspect, what lead us to introduce and evaluate a new method of nasal molding with a silicone strut. Methods: There were evaluate 20 patients operated by the same technique, 10 patients with immediate silicone strut molding and 10 patients without nasal molding. The results were analyzed with pre-operatory pictures, post operative pictures and a movie of the child in the present date by three expertise cleft lip surgeon, whose did not know which use or not the silicone molding. Results: The age of the patients after the surgery range from 1 to 9 years, which only 60% were operated on the pre set age (3 months), the average time using the Silicone molding strut were 4 months. After statistics analysis, it demonstrated that there was not a better outcome in the nose aspect on patients who's used the nasal molding. Conclusions: After statistics analysis, the group of patients underwent the Göteborg Technique, using immediate silicone strut molding, shown no better results than the group without nasal molding.

RESUMO

Introdução: Após avaliação da técnica empregada em nosso serviço para correção de fenda labial unilateral, observamos uma pontuação negativa maior com relação ao resultado final do nariz, o que nos levou a manter nossa linha de pesquisa, com o objetivo de introduzir e avaliar novo método para moldagem nasal pós-operatória imediata, por meio de molde de silicone. Método: Foram avaliados 20 pacientes operados pela mesma técnica, sendo que 10 deles utilizaram o molde nasal, e os outros 10 não. A avaliação foi feita por três cirurgiões de grande experiência, utilizando fotos pré e pós-operatórias e um filme do paciente com sua idade atual, sem o conhecimento de qual grupo utilizou o molde. Resultados: A idade de avaliação no período pós-operatório variou de 1 a 9 anos, do procedimento cirúrgico; somente 60% dos pacientes foram operados na idade preconizada pelo nosso protocolo (3 meses), o tempo médio de uso do modelador foi de 4 meses. Após a aplicação de análise estatística, observamos que não houve melhora de resultados nos pacientes que usaram o modelador nasal, em comparação ao grupo que não usou. Conclusões: O grupo de pacientes operados pela técnica de Göteborg com a utilização do modelador nasal não apresentou resultados melhores estatisticamente significantes quando comparado ao grupo de pacientes operados pela mesma técnica sem o uso do modelador nasal.

Mini-retromandibular transparotid approach for extracapsular condylar fractures of the mandible

ABSTRACT

BACKGROUND: Most of the current research has highlighted the need for establishing a surgical procedure for the treatment of extracapsular condylar fractures of the mandible, in case with large deviations or when the fractures are associated with other mandibular and/or fractures of the middle third of the face. Here, we aimed to describe the surgical details and postoperative outcomes of the mini-retromandibular transparotid approach for the treatment of extracapsular condylar fractures of the mandible. METHOD: An analysis was conducted among 14 patients with extracapsular condylar fractures of the mandible who presented indications for surgical treatment and underwent the surgery between March 2011 and March 2012. RESULTS: In this case series, the anatomical and functional outcomes achieved were satisfactory and the rate of complications was low. CONCLUSIONS: The mini-retromandibular transparotid approach offers advantages and should be included as an option for the surgical treatment of extracapsular condylar fractures of the mandible.

RESUMO

INTRODUÇÃO: A maioria dos trabalhos atuais enfatiza a necessidade de tratamento cruento das fraturas extracapsulares do côndilo mandibular, quando estas apresentam grandes desvios ou estão associadas a outras fraturas mandibulares e/ou terço médio da face. Este artigo tem por objetivo descrever detalhes cirúrgicos e resultados pós-operatórios do uso do acesso retromandibular transparotídeo reduzido para o tratamento das fraturas extracapsulares do côndilo mandibular. MÉTODO: Foram analisados 14 pacientes portadores de fraturas extracapsulares de côndilo mandibular, com indicação de tratamento cirúrgico, operados no período de março de 2011 a março de 2012. RESULTADOS: Nesta série de pacientes, os resultados anatômicos e funcionais foram considerados satisfatórios e foi observado baixo índice de complicações. CONCLUSÕES: O acesso retromandibular transparotídeo reduzido oferece vantagens e deve ser incluído como opção para o tratamento cruento das fraturas extracapsulares do côndilo mandibular.

Therapeutic options in the management of giant congenital nevus: Experience of the Plastic and Reconstructing Service at Hospital de Clínicas, Federal University of Paraná

ABSTRACT

INTRODUCTION: Several modalities are available for the treatment of giant congenital nevus (GCN). The surgical approach includes partial serial resection or total excision. Objective: To demonstrate the main therapeutic modalities in the treatment of GCN and to assess the incidence of location, age, and size of this lesion at the Plastic and Reconstructive Surgery Service of the Hospital de Clínicas, Federal University of Paraná. METHODS: This retrospective study included patients who had undergone surgical treatment for GCN between January 2004 and January 2010. We collected data such as age, sex, treatment performed, number of surgeries carried out, evolution, and complications. RESULTS: We evaluated 11 patients (8 female and 3 male). The average age was 12.4 years (range, 3-25 years). The GCN subtype most commonly found was intradermal melanocytic nevus, which accounted for 90.9% of cases, with melanocytic nevus accounting for the remaining 9.1%. No cases of melanoma were identified. The most common location was the head and neck. The average diameter of the lesions was 9.1 cm. The techniques used for the reconstruction included primary suture, local flaps, skin graft, and the use of expanders. In the follow-up period, 63.6% of the patients still presented a residual nevus, 27.3% underwent complete resection, and 9.1% were not monitored. CONCLUSION: A higher incidence of GCN was observed in patients aged 3-25 years. The most common location was the face, and the average size was 9.1 cm. The main treatment of patients with GCN was splitting resection, which resulted in satisfactory outcomes.

RESUMO

INTRODUÇÃO: O nevus congênito gigante (NCG) possui diversas modalidades de tratamento. A abordagem cirúrgica inclui a ressecção parcial seriada ou excisão total. Objetivo: Demonstrar as principais modalidades terapêuticas utilizadas para o tratamento do NCG e avaliar a incidência da localização, idade e tamanho destas lesões no Serviço de Cirurgia Plástica e Reparadora do Hospital de Clínicas da Universidade Federal do Paraná. MÉTODOS: Estudo retrospectivo dos pacientes submetidos a tratamento cirúrgico do NCG no período de janeiro de 2004 a janeiro de 2010. Foram coletados dados como: idade, gênero, tratamento realizado, número de cirurgias realizadas, evolução e complicações. RESULTADOS: Foram avaliados 11 pacientes, sendo 8 mulheres e 3 homens. A média de idade foi de 12,4 anos (3 a 25 anos). O subtipo mais encontrado foi o nevus melanocítico intradérmico correspondendo a 90,9% dos casos e 9,1% com nevus melanocítico composto. Não foram identificados casos de melanoma. A localização mais comum foi a região da cabeça e pescoço. O diâmetro médio das lesões foi de 9,1 cm.As técnicas utilizadas para reconstrução foram: sutura primária, retalhos locais, enxerto de pele e uso de expansores. No seguimento, 63,6% dos pacientes apresentavam ainda nevus residual, 27,3% apresentaram ressecção completa e 9,1% perderam seguimento. CONCLUSÃO: Foi observada maior incidência de NCG na faixa etária de 3 a 25 anos, sendo a localização mais comum na face e com tamanho médio de 9,1cm. O principal tratamento instituído para os pacientes com NCG foi a ressecção parcelada, com bons resultados.

Breast reconstruction using latissimus dorsi muscle flap: a novel surgical technique with reduced scarring

ABSTRACT

BACKGROUND: The latissimus dorsi muscle flap is one of the most commonly used surgical options for breast reconstruction. The donor area scar on the back, although sometimes hidden beneath the bra strap, is often extensive and enlarged or hypertrophic. The aim of this study is to describe the dissection and rotation of the latissimus dorsi muscle using 2 reduced vertical incisions. METHODS: Twelve female patients underwent unilateral breast reconstruction with latissimus dorsi muscle flap. Ten patients had Poland's syndrome, and their breast reconstruction was performed with a tissue expander at the first instance, with subsequent latissimus dorsi muscle flap rotation to cover the silicone prosthesis. Two patients experienced volume decrease, irregularities, and breast retractions after adenomastectomy; their prosthesis were also covered by a latissimus dorsi muscle flap. The surgical procedure consisted of a 5-cm longitudinal incision on the lateral border of the latissimus dorsi muscle and another on the inframammary fold. Dissection, section, and rotation of the muscle were performed through skin retraction. RESULTS: The final extension of the scar at follow-up averaged 6.2 cm. The mean surgical time for flap rotation and synthesis of the donor area was 80 minutes. No postoperative complications were observed in any of the cases. CONCLUSIONS: The reduced incision for dissection of the latissimus dorsi flap for breast reconstruction proved to be feasible and safe; this method provides aesthetic advantages without increasing cost or surgical time.

INTRODUÇÃO: O retalho de músculo grande dorsal (MGD) é uma das opções cirúrgicas mais usadas nas reconstruções mamárias. A cicatriz da área doadora no dorso, embora por vezes situada sob a alça do sutiã, frequentemente é extensa e pode ser de qualidade ruim (alargada ou hipertrófica). O objetivo deste trabalho é descrever a dissecção e rotação do MGD através de duas incisões reduzidas. MÉTODO: No total, 12 pacientes do sexo feminino foram submetidas a reconstrução mamária unilateral com retalho de MGD isolado (sem pele). Dessas pacientes, 10 eram portadoras de síndrome de Poland e tiveram suas mamas reconstruídas com expansor tecidual num primeiro tempo, com posterior rotação do retalho de MGD para cobrir a prótese de silicone definitiva; e outras 2 pacientes tinham evoluído com diminuição do volume, irregularidades e retrações da mama após adenomastectomia, e tiveram sua prótese coberta pelo MGD. O procedimento cirúrgico consistiu de incisão longitudinal de 5 cm sobre a borda lateral do MGD e no sulco inframamário. Realizou-se dissecção, secção e rotação do músculo somente com a retração da pele. RESULTADOS: Em seguimento ambulatorial, a extensão da cicatriz apresentou média de 6,2 cm. O tempo cirúrgico médio para rotação do retalho e síntese da área doadora foi de 80 minutos. Não foram observadas complicações pós-operatórias nesta série. CONCLUSÕES: A incisão reduzida para dissecção do MGD nas reconstruções mamárias mostrou-se bastante factível e segura, com vantagem estética e sem aumento dos custos e do tempo cirúrgico.

Influence of immunonutritional supplementation on skin wound healing in rats

ABSTRACT

INTRODUCTION: The wound healing process is immediate and dynamic in order to restore anatomical and functional continuity, and there must be conditions for this process, which include a normal nutritional state. Among the existing supplemental formulas, immuno-enhancing diets have been proposed to improve the wound healing process and patients' clinical conditions. The influence of an immunomodulating diet (Impact®) on different variables of the skin healing process was evaluated. METHOD: Healthy adult rats were randomly divided into four groups of diet supplementation or control. Two groups received their diets only pre-operatively while the other two groups received theirs perioperatively. Rats were subjected to three types of skin lesions. We evaluated the following aspects: changes in weight, development of raw areas, tensiometry of incisional wounds, re-epithelialization rates, and histological parameters. RESULTS: There was no difference in weight changes. There was better closing rates of excisional wounds in groups supplemented with Impact® beginning on the fifth day after surgery (p = 0.02). The groups receiving the dietary supplements obtained the best results in tensiometry (p = 0.03), re-epithelialization rates (p = 0.04), differential cell counts (p < 0.001), and total amount of collagen (p < 0.001). CONCLUSIONS: The study diet (Impact®) promoted better closure rates of raw wounds, faster re-epithelialization, scars with a greater tensile strength, and greater amounts of total collagen in wounds. There was no difference in any of the parameters analyzed compared with the groups supplemented with Impact® pre- and perioperatively.

RESUMO

INTRODUÇÃO: O processo de cicatrização é imediato e dinâmico, com o objetivo de restaurar a continuidade anatômica e funcional, e devem existir condições para esse processo, o que inclui um estado nutricional adequado. Dentre as fórmulas de suplementação existentes, as imunomoduladoras têm sido implicadas na melhora do processo cicatricial e das condições clínicas dos pacientes tratados. Foi avaliada a influência da dieta imunomoduladora (Impact®) sobre diferentes variáveis do processo de cicatrização cutânea. MÉTODO: Ratos adultos e nutridos foram divididos aleatoriamente em quatro grupos, a serem suplementados com a dieta em estudo e com a dieta controle. Dois grupos receberam as respectivas dietas apenas pré-operatoriamente e os outros dois grupos as receberam no perioperatório. Os ratos foram submetidos a três tipos de lesões cutâneas. Foram avaliados os seguintes aspectos: evolução dos pesos, evolução das áreas cruentas, tensiometria das feridas incisionais, taxas de reepitelização e parâmetros histológicos. RESULTADOS: Não houve diferença na evolução dos pesos. Houve melhores índices de fechamento de feridas excisionais nos grupos suplementados com Impact®, a partir do quinto dia de pós-operatório (p=0,02). Os grupos suplementados com a dieta em estudo obtiveram melhores resultados em tensiometria (p = 0,03), taxas de reepitelização (0,04), contagem diferencial de células (p<0,001) e quantidade de colágeno total (p<0,001). CONCLUSÕES: A dieta em estudo (Impact®) promove melhores taxas de fechamento de feridas cruentas, reepitelização mais rápida, cicatrizes com maior resistência tênsil e maiores quantidades de colágeno total nas feridas. Não houve diferença em nenhum dos parâmetros analisados em comparação dos grupos suplementados com Impact® pré e perioperatoriamente.

Correcting historical errors in lower eyelid reconstruction

ABSTRACT

The history of plastic surgery is very rich, with the description of various types of flaps and other surgical techniques over the centuries. Many authors in the nineteenth and early twentieth centuries proposed the development of surgical flaps for the reconstruction of the periorbital region, which allowed the repair of eyelid deformities at the time and still remain of great clinical applicability to date. Owing to the presence of myriads of techniques and eponyms, often describing identical or very similar techniques, a plastic surgeon often feels confused while reading the description of a technique that was learned under another name during training. Sometimes, even in academic applications and evaluations, these techniques may be presented with unusual nomenclatures, alluding to the diffuser rather than to the creator of the technique, leading to misunderstandings and errors. This review aims to correct some historical errors, assisting new surgeons who wish to work in this area.

RESUMO

A história da cirurgia plástica é muito rica, com a descrição de diversos tipos de retalhos e outras técnicas cirúrgicas ao longo dos séculos. Muitos autores, no século XIX e início do século XX, propuseram a confecção de retalhos cirúrgicos para reconstrução da região periorbital, que possibilitaram a reparação de deformidades palpebrais naquele momento e ainda hoje têm uma grande aplicabilidade clínica. Em meio a uma miríade de técnicas e epônimos, que muitas vezes descrevem técnicas idênticas ou muito semelhantes, o cirurgião plástico sente-se confuso ao ler a descrição de uma técnica que durante a sua formação aprendeu com outro nome. Por vezes, mesmo em concursos ou avaliações, essas técnicas podem se apresentar com nomenclatura pouco usual, fazendo alusão ao difusor ao invés do criador da técnica, levando a equívocos e erros. Este artigo de revisão tem o objetivo de corrigir alguns erros históricos, e servir de apoio aos novos cirurgiões que desejam trabalhar nesta área.

ABSTRACT

Gestational gigantomastia is a rare disorder characterized by an excessive and rapid enlargement of the breasts, resulting in edema and venous congestion of breast tissue. It is a painful condition that causes skin ulceration and local infection. In some cases, these complications lead to an emergency mastectomy or induced abortion. The most probable etiology is an abnormal stimulation of breast tissue that is probably triggered by an abnormally elevated level of hormones or by the hypersensitivity of breast tissue to normal hormone levels. Although the breast volume decreases after pregnancy, it rarely returns to its original state; therefore, breast reduction through mastectomy or mammaplasty is usually necessary. Moreover, there is a high likelihood of recurrence in subsequent pregnancies. The authors report a case of gigantomastia in a primigravida that required pregnancy termination, because of the risk of maternal death, and a subsequent breast reduction surgery.

RESUMO

A gigantomastia gestacional é uma desordem rara, na qual ocorre o crescimento excessivo e rápido das mamas, culminando com edema e congestão venosa das mesmas, além de ocasionar dor, ulceração da pele e infecção local. Estas complicações, em alguns casos, levam à necessidade de mastectomia de emergência ou abortamento induzido. A hipótese etiológica mais aceita é a de que exista uma estimulação anormal do tecido mamário, que pode ser desencadeada por níveis excessivos de hormônios ou por uma hipersensibilidade deste tecido a níveis hormonais normais. Apesar de a mama regredir após a gestação, raramente volta ao estado original; portanto, a redução da mama através de mastectomia ou mamoplastia geralmente é necessária. Além disso, é grande o risco de recorrências em gestações futuras. Os autores relatam o caso de gigantomastia em uma primigesta, com necessidade de interrupção da gravidez, devido ao risco de morte materna, e posterior intervenção cirúrgica com mamoplastia redutora.

ABSTRACT

INTRODUCTION: Proteus syndrome is a complex and rare disorder classified as a hamartomatous disease. It was first described in two patients in 1979, by Cohen and Hayden. Proteus syndrome is difficult to diagnose, and is often confused with Klippel-Trenaunay-Weber syndrome, neurofibromatosis, or Sturge-Weber syndrome. In this study we describe two patients who were treated at the Plastic and Reconstructive Surgery Service of the Federal University of Paraná. METHOD: A 6-year-old male patient (case 1) presented to the Service with lipomatosis and asymmetry, as the primary findings. A 20-year-old (case 2) was admitted to the Service with a diagnosis of Klippel-Trenaunay-Weber syndrome, which later was shown to be Proteus syndrome. CONCLUSION: The etiological hypothesis that is most accepted for this disease is genetic. It is believed that somatic mosaicism may occur during pathogenesis, which can be lethal in the mosaic state. Premature death is common. However, the most common sequelae are the occurrence of unusual tumors. The care of patients with this syndrome is a challenge due to medical and psychosocial consequences.

RESUMO

INTRODUÇÃO: A síndrome de Proteus é uma doença complexa e rara, classificada nos grupo das hamartoses. Foi primeiramente descrita em dois pacientes, em 1979, por Cohen e Hayden. Existe dificuldade no diagnóstico, sendo comum a confusão com síndromes de Klippel-Trenaunay-Weber, neurofibromatose ou Stuge-Weber. Apresentamos dois casos tratados no Serviço de Cirurgia Plástica e Reparadora da Universidade Federal do Paraná. MÉTODO: Paciente masculino (caso 1), que chegou ao serviço aos 6 anos de idade, tendo como principais apresentações lipomatoses e assimetrias. A segunda paciente (caso 2) deu entrada no serviço com 20 anos de idade e diagnóstico de síndrome de Klippel-Trenaunay-Weber, que posteriormente mostrou se tratar de síndrome de Proteus. CONCLUSÃO: A hipótese etiológica mais aceita para a doença é genética. Acredita-se que exista mosaicismo somático e que a doença seja letal no estado não mosaico. Morte prematura é bastante frequente. Entretanto, a sequela mais comum é a ocorrência de tumores incomuns. O cuidado dos pacientes portadores da síndrome é um desafio devido às suas consequências médicas e psicossociais.

ABSTRACT

Pressure ulcers are alterations of the integrity of the skin and underlying tissues, caused by pressure, more commonly on bony prominences, especially in areas devoid of sensitivity, which lead to necrosis and ulceration. Data from the international literature estimate that 3-14% of hospitalized patients develop pressure ulcers. We herein describe the simultaneous correction of extensive sacral and ischial ulcers in a young paraplegic patient, using a gluteus maximus fasciomyocutaneous flap from the posterior aspect of the thigh.

RESUMO

Úlceras de pressão são alterações da integridade da pele e tecidos subjacentes, causadas por pressão, mais usualmente sobre proeminências ósseas, especialmente em áreas desprovidas de sensibilidade, levando à necrose e ulceração. Dados da literatura internacional estimam que 3 a 14% dos pacientes hospitalizados desenvolvem úlceras de pressão. Descrevemos a correção simultânea de úlceras sacral e isquiática extensas em paciente paraplégico jovem utilizando retalho fasciomiocutâneo de glúteo máximo e de face posterior da coxa.
