Histological analysis of the pedicle of the muscular sub-units of latissimus dorsi muscle

ABSTRACT

Introduction: The latissimus dorsi muscle has been widely used in reconstructive microsurgery. The authors have described the use of parts of this muscle (subunities) as a substitute for mimic muscles. Objective: To demonstrate, by a histological analysis of the muscular subunities, the presence of an artery, a vein and a nerve into these small muscles. The subunities were selected accordingly its arc of rotation and hypothetical necessity of reconstruction of the facial muscles. Method: It were done nine dissections in fresh cadavers and anatomo-hystological analysis of its pedicle (three milimeters before hylus) and hylus and got diameters of external walls of those structures. Three cases of muscular subunits transplants were done and eletromyography confirmed activity and video registration of the functional aspects showed functional and coverture recovered. Results: It was encountered under histological analysis 98% of these piece of muscles artery, vein and nerve. Two percent represent the mistake of cutting on preparing the lamina, cause the formol retraction. There is not publishing about detailed anatomical study, as well the divisions of these vessels and about study of muscular sub-segments (sub-units), under histological evidence. Conclusion: The authors conclude that more study must be done and it is possible make muscular multiple functional transplants as a refinements of the surgical options to treat complex and functional lesions of the face.

RESUMO

Introdução: O músculo grande dorsal tem sido amplamente utilizado em microcirurgia reconstrutiva. Estudos anatômicos evoluíram na medida da necessidade e formas de uso deste músculo. Os autores introduziram uma nova maneira e um novo conceito de retalho muscular para reconstruções complexas e funcionais da face, a partir do estudo anatômico em cadáveres. Objetivo: Comprovar, anátomo-histologicamente, que há artéria (irrigação), veia (drenagem) e nervo (função motora) nos músculos, selecionados para a face do cadáver, de acordo com seus arcos de rotação e necessidade hipotética de reconstruí-los. Método: Foram realizadas nove dissecções de cadáveres frescos e analisados os pedículos e hilos musculares, em caráter anátomo-histológico, realizadas medições das paredes externas das artérias, veias e nervos. Resultados: Foram encontradas artéria, veia e nervo em 98% das peças, a ausência nos outros 2% deveu-se a erros de preparo nos cortes histológicos, devido à retração pelo formol. Não há nenhuma publicação na literatura referindo uso de subsegmentos (subunidades) musculares ou estudo anatômico com a riqueza de detalhes de divisões dos pedículos deste músculo. Conclusão: É possível realizar transplante funcional com múltiplas subunidades musculares (subsegmentos), baseado nas evidências anatômicas e histológicas e no fato de os autores já terem comprovado esta possibilidade técnica no refinamento da opção terapêutica de lesões complexas da face.

Surgical treatment of the nasal skin tumors in elderly patients

ABSTRACT

Introduction: Skin cancer is the most common neoplasm in Brazil and mainly affects the elderly. The most affected location of the body is the face, especially the nose. Surgical resection is also the most used treatment. Objective: To evaluate the effectiveness of surgical procedures performed in the treatment of nasal skin cancer in elderly patients. Method: They were reviewed 101 charts of patients who have been submitted to a surgical resection of nasal skin tumor from August 2005 to June 2008. Data regarding gender, age, phototype, location of tumor in the nose, surgical margins and type of reconstruction were analyzed. The chi-square statistical test of adhesion, Student t test and Fischer exact test were used for statistical analysis. Statistical significance was consider when p< 0.05. Results: Ages ranged between 61 to 94 years with an average of 77 years. The incidence was higher in women (67.3%) and in individuals with fair skin (65.3%). The tumors presented a heterogeneous distribution on nasal subunits (p = 0.001 chi-square). Basal cell carcinoma was the most frequent malignant neoplasm (92.9%) and 7.1% were squamous cell carcinoma. The distribution of tumors between men and women was similar (p = 0.672 Fischer exact test). Flaps were the most frequent type of reconstruction (54.9%). There was incomplete resection in 8.4% of the patients and the reoperation rate was 77.8%. Residual tumor was presented in 14.2% of the surgical specimens. There was no significant difference between the ages of the patients who were reoperated and the others who were observed (p = 0.486 Student t test). Recurrence was observed only in the group of patients who had a complete resection of the tumor (0.9%). Conclusion: Surgical procedures performed in this study have proven their effectiveness for the treatment of nasal skin cancer in elderly patients.

RESUMO

Introdução: O câncer de pele é a neoplasia mais frequente no Brasil e acomete principalmente indivíduos idosos. O local do corpo mais acometido é a face, especialmente o nariz. O tratamento mais utilizado é a ressecção cirúrgica. Objetivo: Analisar a eficácia dos procedimentos cirúrgicos realizados no tratamento do câncer de pele nasal em indivíduos idosos. Método: Foram revistos 101 prontuários de pacientes operados no período de agosto de 2005 a junho de 2008 e analisados dados referentes a gênero, idade, fototipo, localização do tumor no nariz, margens cirúrgicas, tipos de reconstrução. Utilizaram-se os testes estatísticos qui-quadrado de aderência, t de Student e teste exato de Fischer (p<0,05). Resultados: A faixa etária foi de 61 a 94 com média de 77 anos. A incidência foi maior em mulheres (67,3%) e em indivíduos de pele clara (65,3%). Os tumores distribuíram-se de modo heterogêneo entre as subunidades nasais (p=0,001 teste qui-quadrado). Dos tumores malignos, 92,9% eram do tipo carcinoma basocelular e 7,1% eram do tipo carcinoma espinocelular. A distribuição dos tumores entre homens e mulheres foi semelhante (p=0,672 teste exato de Fischer). Os tipos de reconstrução mais utilizados foram os retalhos (54,9%). Houve ressecção incompleta de tumor em 8,4% e taxa de reoperação de 77,8%. O achado de tumor residual nos espécimes foi de 14,2%. Não foi encontrada diferença estatisticamente significativa entre as idades dos pacientes reoperados e dos observados (p=0,486 teste t de Student). Houve recidiva somente no grupo de tumores com ressecção completa (0,9%). Conclusão: Os procedimentos cirúrgicos utilizados tiveram sua eficácia comprovada para o tratamento do câncer de pele nasal em idosos.

Augmentation mammoplasty and autologous fat transplantation: an alternative for the treatment of hypomastia and mild pectus excavatum - Case report

ABSTRACT

Pectus excavatum (PE) is a congenital chest deformity characterized by deep depression in the sternum. Autologous fat transplantation has been used for aesthetic purposes, mainly on the face, and has recently gained relevance in thoracic and breast surgeries. The present study aims to present a case of mild PE associated with hypomastia. A 24-year-old female patient attended the consultation for breast augmentation due to hypomastia, but the clinical examination also revealed an associated mild PE that the patient did not notice. Surgical planning included breast augmentation and autologous fat transfer. A 260ml silicone breast implant was used, and 250ml of fat was injected in the sternal region and the lower medial contour of the breasts. There were no complications during the 12-month follow-up period. The combination of augmentation mammoplasty and fat transplantation in treating PE deformity proved to be a minimally invasive, good, safe option with high patient satisfaction.

RESUMO

Pectus excavatum (PE) é uma deformidade torácica congênita, caracterizada como uma depressão profunda no esterno. O transplante autólogo de gordura tem sido utilizado para fins estéticos, principalmente na face, e recentemente ganhou relevância nas cirurgias torácica e das mamas. O objetivo do presente estudo é apresentar um caso de PE leve associado a hipomastia. Uma paciente de 24 anos compareceu à consulta para mamoplastia de aumento por hipomastia, mas o exame clínico também revelou um PE leve associado que não foi percebido pela paciente. O planejamento cirúrgico incluiu a mamoplastia de aumento e a transferência de gordura autóloga. Foi utilizado um implante mamário de silicone de 260ml, e uma quantidade total de 250ml de gordura foi injetada na região esternal e no contorno medial inferior das mamas. Não houve complicações durante o período de acompanhamento de 12 meses. A associação de mamoplastia de aumento e transplante de gordura no tratamento da deformidade de PE revelou-se uma opção minimamente invasiva, boa, segura e com alta satisfação da paciente.
