Zygomatic-palpebral flap: an optional technique for lower eyelid reconstruction

ABSTRACT

Introduction: Lower eyelid reconstruction represents a unique challenge to plastic surgeons, since it involves a facial region of aesthetic and functional importance. The objective is to present an optional technique for the reconstruction of lower eyelid defects using zygomatic-palpebral skin flap. This technique represents an alternative in cases of scleroatrophic skin in older patients, and for young people who do not have an upper eyelid skin redundancy, which prevents, for example, the use of upper eyelid grafts or flaps such as those of Fricke or Tripier.
Methods: The authors describe the flap used in cases of cicatricial ectropion and reconstruction after resection of neoplasms and association with other flaps, such as those of Hughes, or for coverage of cartilage grafts. The flap consisted of a transposition flap made up of skin and underlying subcutaneous tissues, randomized . The technique was based on the use of a local flap with highly similar characteristics to the defect area, which allowed it to mimic functions, while being safe and feasible.
Results: The immediate and late results in terms of aesthetics and function were satisfactory and well accepted by patients and the surgical team, with adequate eyelid occlusion and preserved ocular lubrication.
Conclusion: The advantages of the zygomatic-palpebral flap are its ease of execution, minimal bleeding, low morbidity of the donor area, and the use of local anesthesia. The reconstruction of eyelid defects aims to restore anatomy and function. This can be a challenging task, especially in cases with larger defects that may be present after oncologic procedures both in young and old patients presenting with scleroatrophic skin and minimal tissue laxity.

RESUMO

Introdução: A reconstrução de pálpebra inferior representa um desafio peculiar ao cirurgião plástico, uma vez que traduz uma região facial de importância estética e funcional. O objetivo é apresentar uma técnica opcional para a reconstrução de defeitos da pálpebra inferior com o retalho cutâneo zigomático-palpebral. Esta técnica mostra uma alternativa nos casos de pele escleroatrófica de pessoas idosas, bem como para pessoas jovens, que não apresentam redundância da pele palpebral superior, impossibilitando por exemplo o uso de enxertos de pálpebra superior ou retalhos como o de Fricke ou Tripier.
Métodos: Os autores mostram a indicação do retalho em casos de ectrópio cicatricial, reconstrução após ressecção de neoplasias, associação com outros retalhos como de Hughes ou para cobertura de enxertos cartilaginosos. É um retalho de transposição composto por pele e subcutâneo, randomizado. A técnica baseia-se no uso de retalho local, de características muito semelhantes à área do defeito, o que permite mimetização, segurança e exequibilidade.
Resultados: Os resultados imediatos e tardios são satisfatórios e bem aceitos pelos pacientes e pela equipe cirúrgica em termos estéticos e funcionais, com oclusão palpebral adequada e lubrificação ocular preservada.
Conclusão: O retalho zigomático-palpebral apresenta como vantagens a simplicidade de execução, sangramento mínimo, baixa morbidade de área doadora e feito sob anestesia local. O objetivo de reconstruir defeitos da pálpebra é restaurar a anatomia e função. Esta pode ser uma tarefa desafiadora, especialmente em defeitos maiores que podem estar presentes nos procedimentos oncológicos em jovens e em idosos com pele escleroatrófica e mínima frouxidão.

Epidemiological study of burns at the Hospital of Clinics, Federal University of the Triângulo Mineiro

ABSTRACT

Introduction: Burns are a serious public health problem. Knowledge of the main epidemiological data of affected patients is paramount for preventing and establishing the best clinical treatment for these individuals. The objective was to survey the epidemiological profile of burns, the main mechanisms involved, treatments, and outcomes of these cases at the Hospital de Clínicas da UFTM.
Methods: This is a retrospective and cross-sectional study of hospitalizations for burns at HC-UFTM from January 2015 to December 2019. Gender, age, depth of burns, location, etiology, use of opioids, hospitalization duration, outcome (hospital discharge or death), and extent of burned areas (EBA%).
Results: This study was composed of 138 patients, divided into three groups: group 1 (G1), 39 patients; group 2 (G2), 89 participants, of which 5 died; and group 3 (G3), 10 participants, 4 of whom died. As for the trauma mechanism, the most common was scald (17.39%), followed by thermal burn (13.76%), alcohol (8.69%), and electric burns (5.79%).
Conclusion: There was a higher prevalence of second-degree burns in male patients. The most prevalent mechanism of trauma was scald, and the EBA had an average of 23.9%. The face and neck were the most affected segments, and 61.59% of the patients required opioids during hospitalization. Regarding the outcome, 91.30% of patients were discharged from the hospital, and 6.52% died, with most cases in G2.

RESUMO

Introdução: As queimaduras são um grave problema de saúde pública. O conhecimento dos principais dados epidemiológicos dos pacientes acometidos é de suma importância para a prevenção e instituição do melhor tratamento clínico a esses indivíduos. Objetivou-se realizar um levantamento sobre o perfil epidemiológico das queimaduras, os principais mecanismos envolvidos, tratamentos e desfechos destes casos no Hospital de Clínicas da UFTM.
Métodos: Trata-se de um estudo retrospectivo e transversal, das internações por queimadura no HC- UFTM, de janeiro de 2015 a dezembro de 2019. Foram avaliados: gênero, idade, profundidade das queimaduras, localização, etiologia, utilização de opioides, tempo de internação hospitalar, desfecho (alta hospitalar ou óbito) e extensão das áreas queimadas (SCQ%).
Resultados: Este estudo foi composto por 138 pacientes, divididos em 3 grupos: grupo 1 (G1), 39 pacientes; o grupo 2 (G2), 89 participantes, dos quais 5 foram a óbito; e o grupo 3 (G3), 10 participantes, sendo que 4 vieram a óbito. Quanto ao mecanismo do trauma, o mais comum foi por escaldadura (17,39%), seguida pela queimadura térmica (13,76%), pelo álcool (8,69%) e queimaduras elétricas (5,79%).
Conclusão: Houve uma maior prevalência de queimaduras de segundo grau em pacientes do gênero masculino. O mecanismo de trauma mais prevalente foi escaldadura e a SQC teve média de 23,9%. A face e o pescoço foram os segmentos mais acometidos e 61,59% dos pacientes necessitaram com uso de opioides na internação. Com relação ao desfecho, 91,30% dos pacientes receberam alta hospitalar e 6,52% vieram a óbito, com a maioria dos casos no G2.

ABSTRACT

The attention and enthusiasm of doctors in the pre- and postoperative period is of extreme importance in maintaining good doctor/patient relationships. An adequate organization and documentary record contributes to achieving a good relationship and ensures an important legal tool for physicians. With current technological advances, the electronic medical record is a secure and democratic way to deal with this information. The government has sought to implement this system in public institutions; however, results are still modest, perhaps mainly due to the lack of resources for investment in this area. In light of this, and given the relevance of practical, informative, and dynamic medical records, we aim to present our experience with the use of a complementary digital resource that is commonly associated with medical records and uses a free of cost digital platform for storing data in the "cloud". This system can provide additional information about each patient, including outpatient follow-up, as well as photographic records of the pre-, intra-, and post-operative periods, and also facilitates quick, synchronized, and remote access through the internet. The system generates optimization of resources, surgical planning, and improvement in patient/ doctor relationships. It also leads to greater integration of the medical team, particularly in the discussion of cases and distribution of surgeries by preceptors and residents. Thus, it is an alternative resource to improve medical charts with important data regarding the performance of medical teams, paying special attention to the peculiarities of plastic surgery.

RESUMO

A atenção e zelo do médico no período pré e pós-operatório é de extrema importância para a manutenção da boa relação médico/paciente. A organização e o adequado registro documental, contribui para a obtenção de um bom vínculo e assegura importante ferramenta jurídica ao médico. Hoje, com os avanços tecnológicos, o prontuário eletrônico é uma forma segura e democrática de lidar com estas informações. Nas instituições públicas o governo tem buscado implementar este sistema, mas os resultados são ainda discretos, talvez pela falta principalmente de recursos para investimento nesta área. Diante deste cenário e da grande relevância de um prontuário médico prático, informativo e dinâmico, objetivamos apresentar a nossa experiência com o uso complementar de um recurso digital sem custos. Associado ao prontuário médico de uso habitual dos serviços, descreve-se um sistema complementar, utilizando-se uma plataforma digital de armazenamento de dados na "nuvem". Por meio desse sistema é possível fornecer informações adicionais sobre cada paciente, incluindo o seguimento ambulatorial, assim como o registro fotográfico do pré, intra e pós-operatório, além de viabilizar um acesso rápido, sincronizado e remoto por meio da internet. O sistema gera economia de recursos, planejamento cirúrgico e melhora na relação médico/paciente. Favorece maior integração da equipe médica, discussão dos casos e distribuição das cirurgias por preceptor e residente. Assim, é um recurso alternativo para incrementar os prontuários médicos com dados importantes para a atuação das equipes médicas, com especial atenção às peculiaridades da cirurgia plástica.

Minilipoabdominoplasty with umbilicus detachment: indications and comparison of surgical tactics for umbilicus reintegration, an prospective study

ABSTRACT

INTRODUCTION: The minilipoabdominoplastia with umbilicus detachment represents a surgical procedure restricted to cases in which isolated liposuction would cause worsening of sagging skin, while the conventional abdominoplasty would imply excessive skin resection. It allows xyphopubic plication of the rectus abdominis muscle and good resection of the excessive lower abdominal skin. The objective is to provide patients with high umbilucus treated with liposuction, mini-abdominoplasty with umbilucus detachment and reinsertion below, and comparison of two techniques for umbilical reinsertion. METHODS: We included six women aged 32- to 50-year-old and who underwent surgery under spine and general anesthesia from January to June 2013 in the Hospital of the Federal University of Triângulo Mineiro, Uberaba, Minas Gerais, Brazil. RESULTS: After surgery, patients had natural silhouettes of body contours and umbilicus repositioned without external scars. CONCLUSION: The technique used allows treatment of moderate sagging skin of upper and lower abdomen with need of a small incision, the technique provides umbilicus scarring without external scarring and less chance of complications due to the small detachment.

RESUMO

INTRODUÇÃO: A minilipoabdominoplastia com desinserção do umbigo representa uma tática cirúrgica restrita àqueles casos nos quais a lipoaspiração isolada causaria piora da flacidez, enquanto a abdominoplastia convencional implicaria em ressecção exagerada de pele. Permite plicatura xifopúbica dos retos abdominais e boa ressecção do excedente cutâneo abdominal inferior. O objetivo é apresentar pacientes com umbigo alto tratados com lipoaspiração, miniabdominoplastia com desinserção de umbigo e reinserção abaixo, e comparar duas técnicas para reinserção umbilical. MÉTODOS: Foram operadas seis pacientes no período de janeiro a junho de 2013 no Hospital Escola da Universidade Federal do Triângulo Mineiro (UFTM), Uberaba, MG, todas do sexo feminino, com idades de 32 a 50 anos, submetidas à anestesia peridural e geral. RESULTADOS: Pós-operatórios com contornos corporais de silhuetas naturais e cicatrizes umbilicais reposicionadas sem cicatrizes externas. CONCLUSÃO: A técnica empregada permite o tratamento da flacidez moderada do abdome superior e inferior com uma incisão menor, proporciona cicatriz umbilical sem cicatriz externa e menores chances de complicações em virtude do menor descolamento.

ABSTRACT

Pyomyositis is a deep muscle infection of bacterial origin and subacute, which can evolve with multiple intramuscular abscesses. Despite being characterized as an affection of tropical zones, its incidence in temperate zones has been increasing due to immunosuppression factors, such as infection by the human immunodeficiency virus (HIV) and some types of immunosuppressive treatments. Even though it has been known for over a century, it is a rare and potentially serious condition that can lead to septic shock and death. This is a case report of a descriptive nature, which found the relevance of knowledge about this condition for an early diagnosis, enabling positive prognostic repercussions.

RESUMO

A piomiosite é uma infecção muscular profunda, de origem bacteriana e caráter subagudo, que pode evoluir com abcessos intramusculares múltiplos. Apesar de ser caracterizada como uma afecção de zonas tropicais, sua incidência em zonas temperadas vem aumentando por fatores de imunossupressão, como a infecção pelo vírus da imunodeficiência humana (HIV) e alguns tipos de tratamentos imunossupressivos. Mesmo sendo conhecida há mais de um século, é uma condição rara e potencialmente grave, podendo levar ao choque séptico e óbito. Trata-se de um relato de caso de caráter descritivo, que constatou a relevância do conhecimento desta afecção para um diagnóstico precoce, possibilitando repercussões prognósticas positivas.

ABSTRACT

Introduction: A major concern of patients who are candidates for a plastic surgery is failure to achieve expected results. Neurolinguistic programming (NLP) provides an understanding of the structure of language, clues about how people think, and how these patterns of thought affect behavior. The objective of this study was to validate the use of a formula based on NLP to assess patient satisfaction and expectations.
Methods: NLP techniques were employed to improve communication and assess patient expectations. Using NLP, the average number of surgeries performed in the past 5 years and the reappraisal index were compared with those in the previous 5 years, in which the methods described in this article were not employed.
Results: The average annual growth rate in the previous 5 years was 5% and the average rate verified after introduction of NLP methodology increased to 10%; the reappraisal and additional surgery index decreased from 20% to 10% in the same period.
Conclusions: The use of NLP helped us to understand patient expectations in a more consistent manner for the periods analyzed, significantly increasing the number of operated patients and reducing the rate of reappraisal and additional surgeries.

RESUMO

Introdução: Uma das maiores preocupações dos pacientes candidatos a uma cirurgia plástica é a de não se atingir os resultados esperados. A Programação Neurolinguística (PNL) oferece entendimentos da estrutura da linguagem para dar pistas sobre como as pessoas pensam e como esses padrões de pensamento, por sua vez, afetam o comportamento. O objetivo do presente trabalho é verificar a utilização de uma fórmula, com o uso da PNL, na satisfação e na determinação da expectativa do paciente e os resultados obtidos após a sua utilização.
Métodos: Técnicas de PNL foram empregadas para melhorar a comunicação e equalizar a expectativa dos pacientes. Foram analisados a média de cirurgias realizadas e o índice de refazimento de cirurgias nos últimos cinco anos em comparação com os cinco anos anteriores, em que não se empregavam os métodos descritos neste artigo.
Resultados: A média de crescimento nos cinco anos anteriores era de 5% ao ano e a média verificada após a introdução da metodologia apresentada aumentou para 10% ao ano e o índice de refazimento e complementações cirúrgicos reduziu de 20% para 10%, no mesmo período.
Conclusões: O uso da PNL nos ajudou a entender a expectativa, de maneira mais consistente, nos períodos analisados, aumentando significativamente o número de pacientes operados e reduziu o índice de refazimento e de complementações de cirurgias em nossa experiência.

Treatment of tuberous breast with combined incisions

ABSTRACT

BACKGROUND: Tuberous breast deformity is a rare entity, first described by Rees and Aston in 1976. In this condition, breast development is altered, with herniation of the parenchyma through the nipple-areolar complex, enlargement of this structure, and hypoplasia of the breast tissue, especially in the lower quadrants. The breast thus acquires a tubular shape rather than the natural conical look. METHODS: Four patients underwent a single surgical treatment, with combined inframammary and periareolar incisions. Technical details must be individualized for each case depending on the severity and classification type of the tuberous breasts. RESULTS: The surgical procedure used covers all aspects of tuberous breast deformity in a single-stage operation. Revision of periareolar surgery scar was not necessary in any case. In all cases, the final aesthetic result was satisfactory for the patient and the surgeon. The procedure adopted does not interfere with future lactation. CONCLUSIONS: Tuberous breast represents a real therapeutic challenge. The technique reported herein is very attractive and provides reliable and reproducible results.

RESUMO

INTRODUÇÃO: A deformidade tuberosa da mama é uma rara entidade, descrita por Rees e Aston em 1976. O desenvolvimento mamário encontra-se alterado, com herniação do parênquima pelo complexo areolopapilar, alargamento dessa estrutura e hipoplasia do tecido mamário, principalmente nos quadrantes inferiores. A mama, portanto, adquire um aspecto tubular ao invés do aspecto cônico natural. MÉTODO: No total, 4 pacientes foram submetidas a tratamento cirúrgico em um único tempo, com incisões combinadas: inframamária e periareolar. Detalhes técnicos devem ser individualizados para cada caso, conforme a gravidade e a classificação do tipo de mama tuberosa. RESULTADOS: O procedimento cirúrgico utilizado aborda todos os aspectos da deformidade da mama tuberosa em operação de um estágio. Cirurgia de revisão de cicatriz periareolar não foi necessária em nenhum caso. Em todos os casos, obteve-se resultado estético final aceitável e com satisfação da paciente e do cirurgião. O procedimento adotado não interfere em lactações futuras. CONCLUSÕES: A mama tuberosa representa um verdadeiro desafio terapêutico. A técnica utilizada é muito atraente e mostra resultados confiáveis e reprodutíveis.

Ear lobe rejuvenation: technical description and indications

ABSTRACT

The earlobe occupies a unique position among the structures of the face and is a very important region in aesthetic composition of the ear. The age alters the shape, width and length of the lobe due to sagging tissue and thus is at odds ratio with other aesthetic elements of the ear, requiring correction. Very little has been described to guide the surgeon in rejuvenating the ear. The correction aged lobe can be made alone or in combination with surgery of facial rejuvenation, as well as by secondary lobe rhytidoplasties inadequately secured to the face. The surgery is fairly simple, rapid recovery, with interesting results. The basic aim of the work is the description of the technique and its indications. The strategic positioning of the final scar in continuity with a depression anatomical ear that is escafa, and hidden by antitragus ensures a proper outcome, with little apparent scars. The technique described is a tactical option to reshape and rejuvenate the ear.

RESUMO

O lóbulo de orelha ocupa posição única entre as estruturas da face e é uma região importantíssima na composição estética da orelha. A idade altera o formato, a largura e o comprimento do lóbulo, em decorrência da flacidez dos tecidos e, assim, fica em desarmonia de proporção com outros elementos estéticos da orelha, exigindo correção. Muito pouco tem sido descrito para orientar o cirurgião no rejuvenescimento da orelha. A correção do lóbulo envelhecido pode ser feita isoladamente ou em combinação com uma cirurgia de rejuvenescimento facial, bem como em ritidoplastias secundárias por lóbulo inadequadamente preso à face. A cirurgia é bastante simples, de recuperação rápida, com resultados interessantes. O objetivo básico do trabalho consiste na descrição da técnica de rejuvenescimento do lóbulo da orelha e suas indicações. O posicionamento estratégico da cicatriz final em continuidade com uma depressão anatômica da orelha, que é a escafa, e ocultada pelo antitrágus garante resultado final adequado, com cicatrizes pouco aparentes. A técnica descrita representa uma opção tática para reformular e rejuvenescer a orelha.
