Canthopexy and tarsal reinforcement using a periosteal flap

ABSTRACT

INTRODUCTION: The low resistance of the palpebral tissues to the retractable forces of postoperative scarring can predispose patients to undesirable surgical outcomes. Among the more effective techniques for solving this issue are the periosteal flaps. This report aims to present a technique for canthopexy and tarsal reconstitution using a periosteal flap to obtain a consistent and definitive reinforcement of the tarsal ligament and its continuity to the inner face of the sidewall of the orbit. METHODS: From July 2009 to December 2012, 49 patients underwent canthopexy with flap rotation of the arcus marginalis of the inferior tarsus. The functional and aesthetic results were evaluated together with the presence of complications. RESULTS: Eighty-four cantopexies and tarsal reinforcements using a periosteal flap were performed in 49 patients. No complications such as hematoma, infection, or necrosis were observed. Complete remission of the preoperative signals was observed in 45 (91.8%) patients, while the other 4 (8.2%) showed partial remission on at least one of the sides related to prior ocularis paralysis. CONCLUSIONS: Canthopexy reinforced by a tarsal periosteal flap provides good tarsal ligament reconstitution with appropriate positioning of the outer corner and good contact of the eyelid with the ocular globe.

RESUMO

INTRODUÇÃO: A pouca resistência dos tecidos palpebrais quando submetidos ao poder retrátil dos planos cicatriciais predispõe a resultados cirúrgicos indesejáveis. Dentre as técnicas mais resolutivas destacam-se as que utilizam retalhos de periósteo. O presente artigo tem por objetivo apresentar uma técnica de cantopexia e reconstituição tarsoligamentar com retalho de periósteo, a fim de obter um reforço consistente e definitivo do tarso e sua continuidade tecidual à face interna da parede lateral da órbita. MÉTODO: No período de julho de 2009 a dezembro de 2012, foram operados pacientes candidatos à realização de cantopexia com rotação de retalho do arcus marginalis sobre o tarso inferior. Foram avaliados os resultados estéticos e funcionais, bem como a presença de complicações. RESULTADOS: Foram realizadas 84 cantopexias e reforço tarsal com retalho de periósteo, em 49 pacientes. Não foram observadas complicações, como hematoma, infecção ou necrose. Remissão total dos sinais pré-operatórios foi verificada em 45 (91,8%) pacientes e 4 (8,2%) apresentaram remissão parcial em pelo menos um dos lados, relacionada à prévia paresia orbicular. CONCLUSÕES: A cantopexia com reforço tarsal com retalho de periósteo proporciona boa reconstituição tarsoligamentar, com posicionamento adequado do canto externo e bom contato da pálpebra com o bulbo ocular.

Versatility of the bilobed flap

ABSTRACT

Background: The bilobed flap is a double transposition flap. Its geometric drawing allows a better distribution of tensile forces along its axis of rotation, avoiding distortions and dog ears generated by other flaps and primary closure. It can be harvested in an axial or random pattern depending on where the defect is located on the body. Methods: Retrospectively, this study reviewed 45 consecutive bilobed flaps searching for pathology, anatomical area, clinical evolution and complications. The technique utilized in drawing and harvesting the flap is described in detail. Sizes of the defects were classified as small, moderate and large, taking into account the anatomical sites. A variety of clinical cases is presented exploring the versatility of the flap and etiology of the defects. Results: The bilobed flap was employed in oncological surgery in 93% of the cases while the skin of the head was involved in 71%.The post operative period was free of complications in 85%. Infection rate was 4.4%, trapdoor scaring rate was 4.4% and partial flap loss rate was 11.1%. The overall complication rate was 15%. In spite of all complications there were no compromise in the functional and aesthetic results and the bilobed flaps proved to be resolute and reliable in different clinical presentations and body regions. Conclusions: The bilobed flap is extremely versatile, easily reproducible with wide application in plastic surgery.

RESUMO

Introdução: O retalho bilobado é um retalho de dupla transposição. Sua estruturação geométrica permite melhor distribuição das forças de tensão ao longo de seu eixo de rotação, evitando distorções e redundâncias cutâneas geradas por outros retalhos ou sutura primária. Pode ser confeccionado com padrão vascular axial e aleatório, dependendo da região anatômica. Métodos: Este trabalho faz uma revisão de 45 casos operados, descrevendo em detalhes a técnica utilizada para confeccionar o retalho bilobado. Os defeitos são classificados em pequenos, médios e grandes e diversos casos clínicos são apresentados, explorando a versatilidade desse retalho em diferentes diagnósticos etiológicos e sítios anatômicos. Resultados: Em 93% dos casos, o retalho bilobado foi empregado em reconstruções oncológicas, sendo o segmento cefálico acometido em 71%. Não houve qualquer intercorrência pós-operatória em 85% dos casos. A taxa de infecção foi de 4,4%; de cicatriz em alçapão, de 4,4%; e de epiteliólise e necrose, de 11,1%. A taxa geral de complicações foi de 15%. Entretanto, tais complicações não comprometeram a evolução clínica dos casos e o retalho bilobado mostrou-se resolutivo em diferentes situações, propiciando bom resultado tanto funcional como estético. Conclusões: O retalho bilobado é versátil, de fácil execução e com ampla aplicabilidade em cirurgia plástica.

Breast reconstruction with latissimus dorsi expanded flap in a patient suffering from necrotizing fasciitis in the chest occurred in the neonatal period

ABSTRACT

Necrotizing fasciitis is an infection of soft tissues rapidly progressive, determining severe functional and aesthetic sequelae. In the chest, this entity is very uncommon, especially in newborns, a situation not found in the literature. This article reports the case of a newborn, female, suffering from necrotizing fasciitis in the breast and chest during the first week of life, being treated with antibiotics and surgical debridement with the wound closure by second intention. At fifteen years of age was observed a large scar area in the left hemithorax and complete absence of breast development on that side. Breast reconstruction was performed using an expanded latissimus dorsi flap and placement of tissue expander with subsequent replacement of the expander by silicone gel implant.

RESUMO

A fasciite necrotizante é uma infecção de tecidos moles, rapidamente progressiva, determinando graves sequelas funcionais e estéticas. No tórax, essa entidade é muito incomum, especialmente em recém-nascidos, situação não encontrada na literatura. Este artigo relata o caso de um recém-nascido, do sexo feminino, vítima de fasciite necrotizante em tórax e região mamária durante a primeira semana de vida, sendo tratada com antibioticoterapia e desbridamento cirúrgico, com fechamento da ferida por segunda intenção. Aos quinze anos de idade, procurou nosso serviço apresentando grande área cicatricial em todo hemitórax esquerdo e ausência completa de desenvolvimento mamário desse mesmo lado. Foi realizada reconstrução mamária com uso de retalho miocutâneo expandido de grande dorsal, com posterior substituição do expansor por implante definitivo de silicone gel.
