Forehead reduction in patients with precapillary incision: 10 years of experience

ABSTRACT

INTRODUCTION: The long forehead gives a less harmonious appearance, seems disproportionate and characterizes the aging face. Surgical procedures for frontal reduction with precapillary incision present insightful information and provide harmonious and pleasant restoration of the frontal region. This study evaluated results obtained in patients who underwent frontal reduction using precapillary incision at Daher Hospital plastic surgery service within the last ten years. METHODS: This was a retrospective longitudinal study. The surgical technique entailed a careful dissection of the bilateral supraorbital ridge. The scalp was dissected in the parietoccipital region in order to easily move forward the flap. The vigorous hemostasis and resection of excessive flap were performed. In the end, synthesis of plans were performed. RESULTS: The age of patients ranged from 48 to 76 years, the mean age was 58 years. Mean surgical time was 3 hours and 38 minutes. No complications were seen such as hematoma, flap necrosis, supratrochlear nerve injury, alopecia, infections, deep vein thrombosis, pulmonary embolism and/or dehiscence. Four patients (7%) had seroma that was drained. The symmetric of eyebrows and scars positioning was considered satisfactory in both assessment by patients and the technical evaluator. CONCLUSION: The aesthetic results in patients who underwent frontal reduction by precapillary incision were satisfactory. The choice of the ideal patient was crucial for success of the surgery.

RESUMO

INTRODUÇÃO: A testa longa confere uma aparência menos harmônica, desproporcional e caracteriza o envelhecimento. Procedimentos cirúrgicos para redução frontal com incisão pré-capilar apresentam indicações criteriosas e proporcionam a restauração harmônica da região frontal. O estudo avalia os resultados obtidos em pacientes submetidos à redução frontal por incisão pré-capilar no serviço de cirurgia plástica do Hospital Daher nos últimos dez anos. MÉTODOS: Trata-se de um estudo observacional longitudinal retrospectivo. A técnica cirúrgica consiste em uma dissecção cuidadosa até o rebordo supraorbital bilateral. O couro cabeludo é dissecado até a região parietoccipital de forma a avançar com facilidade o retalho. Procede-se à hemostasia vigorosa e ressecção do retalho excedente. Por fim, realiza-se a síntese por planos. RESULTADOS: A faixa etária das pacientes variou de 48 a 76 anos, com média de 58 anos. O tempo médio operatório foi de 3 horas e 38 minutos. Não foram observadas complicações como hematoma, necrose do retalho, lesão do nervo supratroclear, alopécia, infecções, trombose venosa profunda, embolia pulmonar e/ou deiscências. Quatro pacientes (7%) apresentaram seroma, os quais foram todos drenados. A simetrização das sobrancelhas e a posição das cicatrizes foram consideradas satisfatórias tanto pela avaliação feita pelos pacientes quanto pela avaliação técnica. CONCLUSÃO: Os resultados estéticos obtidos em pacientes submetidos à redução frontal pela incisão pré-capilar foram satisfatórios. A escolha do paciente ideal foi fundamental para o bom sucesso operatório.

Mastoplasty with inclusion of prosthesis during abdominoplasty in post-bariatric patients

ABSTRACT

INTRODUCTION: Augmentation mammaplasty during abdominoplasty reduces surgical time and has a better aesthetic effect as it only leaves a single scar. The authors of this study assessed the results obtained in post-bariatric patients undergoing this procedure in the plastic surgery service at Daher Hospital. METHODS: This was a retrospective longitudinal observational study. Of the 161 post-bariatric patients evaluated, 27 had indications for breast augmentation during abdominoplasty. The surgical technique consisted of classical abdominoplasty and creation of tunnels on the upper and middle abdomen for insertion of implants. RESULTS: The age of the patients ranged from 35 to 50 years, with a mean of 42.4 years. The volume of the prosthesis ranged from 285 to 300 ml. The average body mass index was 22.2 kg/m2.The average surgical time period was 2 hours and 35 minutes. None of the following complications were observed: deep vein thrombosis, pulmonary embolism, skin necrosis, hematoma, capsular contracture and/or event. Two patients (7.4%) presented with seroma in the mammary region and one patient developed infection. CONCLUSION: The aesthetic results were satisfactory, as there was only a single scar for these two procedures in post-bariatric patients. Selecting the ideal patient was crucial for good operative success.

RESUMO

INTRODUÇÃO: A mastoplastia com inclusão de prótese via abdominoplastia proporciona redução do tempo cirúrgico e um melhor efeito estético, em decorrência da presença de uma única cicatriz. Os autores desse estudo avaliaram os resultados obtidos em pacientes pós-bariátricos submetidos à inserção de prótese mamária pela via abdominal no serviço de cirurgia plástica do Hospital Daher. MÉTODOS: Trata-se de um estudo observacional longitudinal retrospectivo. Dos 161 pacientes pós-bariátricos operados, 27 apresentavam indicação de mamoplastia de aumento pela via abdominal. A técnica cirúrgica consistiu na realização da abdominoplastia clássica e confecção de túneis ao espaço retroglandular para inserção dos implantes. RESULTADOS: A faixa etária das pacientes variou de 35 a 50 anos, com média de 42,4 anos. O volume das próteses variou de 285 a 300 ml. A média do índice de massa corporal foi de 22,2 kg/m2. O tempo médio operatório foi de 2 horas e 35 minutos. Não foram observadas complicações como trombose venosa profunda, embolia pulmonar, necrose de pele, hematoma, contratura capsular e/ou deiscências. Duas pacientes (7,4%) apresentaram seroma na região mamária sendo que uma paciente teve infecção. CONCLUSÃO: Os resultados estéticos obtidos com a confecção de uma única cicatriz para esses dois procedimentos em pacientes pós-bariátricos foram satisfatórios. A escolha do paciente ideal foi fundamental para o bom sucesso operatório.

Forehead reduction using precapillary incision: experience report and indications

ABSTRACT

BACKGROUND: A long forehead can give a less attractive and disproportionate appearance. It may also indicate aging. The objective of this study is to show the experience of the authors in forehead reduction using precapillary incision and to support the indications for this procedure. METHODS: Patients who underwent precapillary incision for forehead reduction between 2005 and 2011 were retrospectively analyzed. The indications for an anterior hairline incision were eyebrow ptosis in patients with thin and sparse frontal hair, congenital long forehead, or extensive forehead wrinkles. RESULTS: Precapillary incision was performed in 31 patients, who were followed up for an average of 1.5 years. No problems related to flap vascularization were detected. All patients reported temporary paresthesia, from which they recovered within 1 year. Three patients presented seromas during the postoperative period and were treated using puncturing. All patients reported that the benefits of altering the hairline were greater than the disadvantages of a possibly more visible scar. CONCLUSIONS: Frontal rhytidectomy using precapillary incision is recommended in patients who wish to correct thin and sparse frontal hair, extensive forehead wrinkles, or congenital/ senile long forehead.

RESUMO

INTRODUÇÃO: A testa longa pode dar uma aparência menos atraente, desproporcional e caracterizar o envelhecimento. O objetivo deste trabalho é demonstrar a experiência dos autores na redução da região frontal com incisão pré-capilar e reforçar as indicações desse procedimento. MÉTODO: Foram estudados, retrospectivamente, pacientes submetidos a redução da região frontal por incisão pré-capilar, no período de 2005 a 2011. As indicações para incisão anterior na linha do cabelo foram ptose da sobrancelha, em pacientes que possuíam cabelos frontais finos e escassos, testa longa congênita ou enrugamento amplo de testa. RESULTADOS: A incisão pré-capilar foi realizada em 31 pacientes, com acompanhamento médio de 1,5 ano. Não houve problemas relacionados à vascularização do retalho. Todos os pacientes relataram parestesia temporária, com recuperação em até 1 ano. Três pacientes apresentaram seromas no pós-operatório, tratados com punção. Todos os pacientes relataram que os benefícios da redução da linha do cabelo ultrapassaram as desvantagens de uma cicatriz possivelmente mais visível. CONCLUSÕES: O procedimento de ritidectomia frontal com incisão pré-capilar é indicado para pacientes com cabelos frontais finos e escassos, com enrugamento amplo da testa ou com testa longa congênita/senil e que desejem reduzi-la.

Gluteal prosthesis: use of liposuction on the improvement of results

ABSTRACT

Introduction: The liposuction brought new possibilities for the improvement of the body contour. The use of prosthesis in order to increase the gluteal region brought interesting options for the treatment of moderate and accentuated hypertrophy, in which the lipograft did not fulfill the expected results. The purpose of the present study was to analyze the results of increase gluteoplasty, its association with liposuction, the surgical technique used, its results and its complications. Method: The study was developed at the Daher Hospital Study Center, retrospectively, where 41 patients had been submitted to increase gluteoplasty with the use of silicone prosthesis, between January 2005 and August 2010. The technique used was of placement of intramuscular prosthesis with incision in the intregluteal sulk. The liposuction was associated or not according to the judgment concerning its necessity. Results: From 41 patients, 40 were females and 1 male, averaging the age of 44.38 years old, ranging between 22 to 65 years of age. The prosthesis volume was ranging between 240 to 360 ml. The association among the surgeries happened in 70% of the patients. The complications were noticed in two patients. Discussion: The liposuction and the gluteal prosthesis appeared as an option for these big corrections, as the lipografting does not reach satisfying results in these cases, due to the lipoabsorption and fatty necrosis. Conclusion: Association of gluteal prosthesis with liposuction is a not so complex procedure, offering good results, with an improvement of the body contour and low rates of complication.

RESUMO

Introdução: A lipoaspiração trouxe novas possibilidades para a melhora do contorno corporal. O uso de próteses para aumentar a região glútea trouxe interessantes opções para tratamento de hipotrofias moderadas e acentuadas, nas quais a lipoenxertia não atingiu resultados satisfatórios. O objetivo deste estudo foi analisar os resultados da gluteoplastia de aumento, sua associação com lipoaspiração, a técnica cirúrgica usada, seus resultados e suas complicações. Método: O estudo foi realizado no Centro de Estudos do Hospital Daher, onde foram analisados 41 pacientes, retrospectivamente, submetidos à gluteoplastia de aumento com prótese de silicone, no período de janeiro de 2005 a janeiro de 2010. A técnica utilizada foi de colocação de prótese intramuscular com incisão no sulco interglúteo. A lipoaspiração foi associada ou não, de acordo com o julgamento quanto a sua necessidade. Resultados: Dos 41 pacientes, 40 eram do sexo feminino e 1 do sexo masculino, com média de idade de 44,38 anos, variando de 22 a 65 anos. O volume da prótese variou de 240 a 360 ml. A associação entre as cirurgias ocorreu em 70% dos pacientes. As complicações ocorreram em duas pacientes. Discussão: A associação entre lipoaspiração e prótese glútea na busca de melhores resultados possibilitou a correção de grandes lipodistrofias, para melhora do contorno corporal, quando a lipoenxertia não traz resultados satisfatórios. Conclusão: A associação de prótese glútea com lipoaspiração é uma conduta segura, de baixa complexidade, que apresenta bons resultados no contorno corporal, com baixas taxas de complicação.

Umbilicaplasty: experience with the skin mini-circle technique

ABSTRACT

Introduction: The umbilicaplasty is another important technical detail in the lower pubic transversal abdominoplasty. The described and published techniques have similar scope to preserve the navel pedicle but frequently leaving visible and even hypertrophic scars. From the stenotic umbilicus to the reverse open bally button bigger than the patterns, call the attention negatively base on the narrow safe limits to obtain the desired aesthetic aspects. Methods: Seventy two female patients submitted to abdominoplasty under the same umbilicaplasty procedures were followed in a period of 26 months. After the umbilical pedicle dissection from the skin flap a 5 mm mini skin circle is preserved on its free extremity. After the abdominoplasty steps be partially concluded the umbilicaplasty is performed. A small fat cone from the skin flap is resected below the umbilicus final position, a 7 mm transversal incision is done in the skin flap to extrude and suture with isolated non absorbable stitches the small skin circle of the umbilicus. Results: The obtained results were considered satisfactory by the surgical team and the patients, without post operative problems that had require special care. Conclusion: It has been an easy procedure without secondary aesthetic and technical problems and lack of visible scars.

RESUMO

Introdução: A umbilicoplastia compreende mais um detalhe técnico importante no decurso das abdominoplastias tipo incisão pubiana transversal baixa. As técnicas descritas e publicadas têm o mesmo princípio de preservar o umbigo original, deixando com frequência cicatrizes geralmente visíveis, retráteis e mesmo hipertróficas. Paralelamente, a estenose cicatricial ou o inverso, uma área umbilical maior do que a dos padrões estéticos, chama a atenção negativamente, decorrente de uma estreita margem de segurança para se obter efeitos estéticos desejados. Método: Foram revisados 72 pacientes do sexo feminino, operadas de abdominoplastia no período de 26 meses, com os mesmos detalhes técnicos de umbilicoplastia. Após da dissecação do pedículo umbilical, um minicírculo de pele com cerca de 5 mm de diâmetro é preservado na sua extremidade livre. Após as etapas da abdominoplastia, a reposição do umbigo é realizada mediante a exérese de um cone de gordura no retalho do abdome situado na projeção do mesmo e uma incisão cutânea transversal de 7 mm faz exteriorizar o círculo de pele que é suturado na pele mediantes pontos isolados de fio não absorvível. Resultados: Os resultados obtidos foram considerados satisfatórios pela equipe cirúrgica e pelas pacientes, sem presença de problemas pós-operatórios que exigissem cuidados especiais. Conclusão: A técnica de umbilicoplastia apresentada tem sido um procedimento de fácil execução, sem problemas secundários de caráter técnico ou estético e pela ausência de cicatrizes visíveis.
