Brow ptosis: trans palpebral approach

ABSTRACT

Background: Transpalpebral browpexy could be performed as an adjuvant procedure to the upper blepharoplasty or as a separate procedure. Particular indications included patients with lateral brow ptosis and discrete forehead wrinkles and balding men. Method: During the years 2003 and 2004, a prospective study was conducted at the Antônio Pedro Hospital with this approach. During this period 68 surgeries were done. Every patient was photographed just before the surgery and after 6 months of the procedure; preoperative and postoperative pictures were analyzed to compare the results. Results: Almost 89% of patients were female. It was seen that 22.3% of patients had eyebrows asymmetry. Bilateral browpexy were done in 94.1% of patients. In most cases (88.9%), only one stitch was needed to accomplish the browpexy and this stitch was located at the temples in 94.7% of cases and at the nasal side in 5.3% of patients. There were no decreased sensation and paresthesis in the undermined area in this casuistic. Complications during the postoperative course were: local swelling (100%), ecchymosis (47.2%), slightly depression at the site of anchoring stitch (100%). Conclusions: Transpapebral browpexy proved to be useful addition to blepharoplasty.

RESUMO

Objetivo: Analisar a suspensão de supercílio via transpalpebral. Método: Foi realizado um estudo prospectivo, entre janeiro de 2003 e dezembro de 2004, incluindo todos os pacientes que procuraram o serviço com queixa de excesso de pele na pálpebra superior e/ou queixa de queda na posição do supercílio. No total, foram realizados 68 procedimentos cirúrgicos. Realizou-se registro fotográfico no pré-operatório e pós-operatório tardio, comparando-se o resultado obtido. Resultados: Quanto ao sexo, 88,9% dos pacientes eram do sexo feminino e 11,1% do sexo masculino. Observou-se que 22,3% dos pacientes tinham assimetria. A suspensão foi bilateral em 94,1% dos casos e unilateral em 5,9%. Na maioria dos casos (88,9%), a colocação de um ponto de sustentação foi suficiente. Em 94,7% dos casos, a suspensão envolveu a região temporal do supercílio e, em 5,3%, apenas a porção nasal. Não houve relato de alteração de sensibilidade local nesta casuística. As complicações encontradas foram: edema local (100%); equimose (47,2%); depressão discreta no local por onde se passou o fio (100%). Conclusão: As vantagens da técnica apresentada justificam, na opinião dos autores, o seu emprego, como mais uma opção para elevação do supercílio.
