Treatment of superficial second degree burn of face and neck with topical heparin: a comparative, prospective and randomized study

ABSTRACT

BACKGROUND: New treatment options for thermal injuries are very desirable, especially if they reduce healing time and pain without increase of infection rates. Recent studies suggest that heparin topical use can achieve those goals. This study has the objective to evaluate healing time, pain and infection rate comparing topical use of heparin and collagenase in the treatment of superficial second degree burns of face and neck. METHODS: Twenty patients were randomized into 2 groups: group treated with topical heparin and group treated with collagenase (control group). The exclusion criteria were: history of bleeding, blood discrasia, allergies to the product, active peptic ulcer and burns with more than 24 hours. Mann-Whitney test was applied to evaluate the results. The pain was measured by the use of opioid analgesics. RESULTS: The heparin was not effective in decrease of healing time nor the use of opioids, and the infection rate didn't present significant difference between the groups. CONCLUSIONS: The heparin can be used safely in treatment of superficial second degree burn of face and neck, but its beneficial effects need to be proven.

RESUMO

INTRODUÇÃO: Novas opções terapêuticas para o tratamento de lesões térmicas são constantemente buscadas, especialmente se reduzirem tempo de cicatrização e dor, sem aumentar as taxas de infecção das queimaduras. Estudos recentes sugerem que o uso tópico de heparina pode alcançar esses objetivos. Este estudo tem o objetivo de avaliar tempo de epitelização, dor e taxa de infecção, comparando o uso de heparina tópica ao uso de colagenase no tratamento de queimadura de segundo grau superficial de face e pescoço. MÉTODO: No total, 20 pacientes foram randomizados em dois grupos: grupo tratado com heparina sódica e grupo tratado com colagenase (controle). Os critérios de exclusão foram: história de sangramento, discrasia sanguínea, alergias ao produto, úlcera péptica ativa e queimadura há mais de 24 horas. O teste de Mann-Whitney foi utilizado para avaliar os resultados. A dor foi avaliada pela necessidade do uso de analgésicos opioides. RESULTADOS: A heparina não foi efetiva em diminuir o tempo de epitelização ou o uso de opioides, e a taxa de infecção não apresentou diferença estatística entre os grupos. CONCLUSÕES: A heparina pode ser usada com segurança no tratamento de queimadura de segundo grau superficial em face e pescoço, mas seus efeitos benéficos ainda precisam ser comprovados.

Alar cartilage deformity correction using conchal cartilage in patients with unilateral cleft lip-nose

ABSTRACT

BACKGROUND: Rhinoplasty in patients with cleft lip and palate is a challenging surgery to the plastic surgeon because it involves the correction of multiple factors associated with an altered anatomy. METHODS: A retrospective analysis of 15 patients that were submitted to surgery performed by the author, between February 2009 and January 2012, was performed. Investigated parameters included age, gender, and type of cleft. RESULTS: All patients were subjected to open rhinoplasty using a "folded" conchal cartilage for alar cartilage deformity correction on the cleft side, with satisfactory results. Necrosis of the columella, which healed by secondary intention, was reported in a single case (6.7%). CONCLUSIONS: The "folded" conchal cartilage graft is a good option for correcting alar cartilage deformities in patients with cleft lip or unilateral clef lip and palate.

RESUMO

INTRODUÇÃO: A rinoplastia do paciente com fissura labiopalatal é uma cirurgia desafiadora, por envolver a correção de múltiplos fatores de uma anatomia alterada. MÉTODO: Foi realizada análise retrospectiva de 15 pacientes operados pelo autor, no período de fevereiro de 2009 a janeiro de 2012. Foram analisados parâmetros como idade, gênero e tipo de fissura. RESULTADOS: Todos os pacientes foram operados com a técnica de rinoplastia aberta utilizando-se cartilagem conchal "dobrada" para correção da deformidade da cartilagem alar do lado da fissura, com resultados satisfatórios. Houve 1 (6,7%) caso de necrose do retalho columelar, que cicatrizou por segunda intenção. CONCLUSÕES: O enxerto de cartilagem conchal "dobrada" é uma boa opção de enxerto no tratamento da deformidade da cartilagem alar afetada do paciente com fissura labial ou labiopalatal unilateral.

The use of reverse abdominoplasty for treatment of chemical burn in the thoracoabdominal region

ABSTRACT

Chemical burn is a challenge owing to its severity compared to thermal burns. The depth and extent of lesions in different areas of the body lead us to look for different possibilities for the best treatment of the patient. A reverse abdominoplasty surgical technique was proposed; this involves a skin flap that could reduce the exposed area of a chemical burn. The result was satisfactory, reducing the patient's treatment, surgical and hospitalization time, highlighting the proposed technique as an important tool for thoracoabdominal reconstruction.

RESUMO

A queimadura química é um desafio devido à sua agressividade em comparação com as queimaduras térmicas. A profundidade e extensão das lesões em diferentes áreas do corpo nos levam a buscar diferentes possibilidades para auxiliar no melhor tratamento do paciente. Foi proposto, a partir da técnica cirúrgica abdominoplastia reversa, um retalho cutâneo que pudesse reduzir a área exposta de uma paciente vítima de queimadura química. O resultado foi satisfatório, reduziu tempo de tratamento, cirúrgico e de internação da paciente, evidenciando uma ferramenta importante de reconstrução toracoabdominal.

Surgical correction of unilateral and bilateral congenital macrostomia: a case report and literature review

ABSTRACT

INTRODUCTION: The incidence of craniofacial microsomia is 1 in 5600 live births. This is the second most common craniofacial anomaly after cleft lip and palate. Tessier cleft 7 is associated with 17% to 62% of cases of hemifacial microsomia. It begins on the labial commissure and may extend to the pre-auricular capillary line. Deformities of the external ear range from excessive pre-auricular skin to complete absence of the ear. Commissuroplasty is indicated in patients with macrostomia or true lateral facial cleft. The objective is to present two cases of macrostomia and perform a review of related literature. METHODS: In this study, we describe two cases of macrostomia treated with mucous flaps and zetaplasty. RESULTS: We obtained optimal repositioning of labial commissures in two patients, with excellent aesthetic results. CONCLUSION: The technique used is easily reproducible, and aesthetically and functionally corrects macrostomia.

RESUMO

INTRODUÇÃO: A incidência da microssomia craniofacial é de 1 em 5600 nascidos vivos. É a segunda anomalia craniofacial mais comum após as fissuras labiais e palatinas. A fissura número 7 está associada entre 17 a 62% dos casos de microssomia hemifacial e começa na comissura labial, podendo prolongar-se até a linha capilar pré-auricular. As deformidades da orelha externa vão desde excesso de pele pré-auricular até ausência completa da orelha. A comissuroplastia está indicada em pacientes com macrostomia ou fissura facial lateral verdadeira. O objetivo é demonstrar dois casos de macrostomia e fazer uma revisão da literatura sobre o tema. MÉTODOS: Em nosso estudo descrevemos dois casos de macrostomia tratados com retalhos de mucosa e plástica em Z. RESULTADOS: Obtivemos um ótimo reposicionamento das comissuras nos dois pacientes, com excelente resultado estético. CONCLUSÃO: A técnica utilizada é de fácil reprodutibilidade e corrige a macrostomia estética e funcionalmente.
