Prophylactic antibiotic therapy and/or therapeutic in patients undergoing plastic surgery: a necessity?

ABSTRACT

Introduction: Antibiotic therapy and therapy is often used in error by the surgeon to prevent infection, causing losses to the patient and contributing to the selection of resistant bacteria antibiotics. This study evaluates whether there is need for antibiotic therapy in aesthetic plastic surgery. Methods: This study included 488 patients who underwent plastic surgery between 2006 and 2009, divided into four groups: Group 1 - patient operated in 2006, 2007, outpatient, surgical time, up to 2 hours, receive prophylactic antibiotic therapy; Group 2 - patients operated on in 2008, 2009, outpatient, surgical time, up to 2 hours did not receive antibiotics; Group 3 - patients operated in 2006, 2007, regime hospital stay more than 3 hours, and received prophylactic antibiotic therapy; Group 4 - patients operated on in 2008, 2009, hospital system, duration more than 3 hours, received prophylactic antibiotics. Results: In Groups 1 e 2, there was no infection, in Group 3 was one (0.39%) case of infection and in Group 4, another (0.42%) case. Conclusion: Prophylactic antibiotic therapy did not influence the occurrence of infection in surgery that lasted up to two hours. The use of antibiotic therapy did not affect the incidence of infection in surgery that lasted over three hours.

RESUMO

Introdução: A antibioticoterapia profilática e terapêutica é muitas vezes usada de forma equivocada pelo cirurgião para prevenir a infecção, trazendo prejuízos ao paciente e contribuindo para a seleção de bactérias resistentes aos antibióticos. Este trabalho teve por objetivo avaliar se há necessidade de antibioticoterapia em cirurgia plástica estética. Método: Foram incluídos neste estudo 488 pacientes submetidos à cirurgia plástica estética, entre 2006 e 2009, divididos em quatro grupos: Grupo 1 - paciente operado em 2006, 2007, regime ambulatorial, tempo cirúrgico até 2 horas, receberam antibioticoterapia profilática; Grupo 2 - pacientes operados em 2008, 2009, regime ambulatorial, tempo cirúrgico até 2 horas, não receberam antibióticos; Grupo 3 - pacientes operados em 2006, 2007, regime hospitalar, duração mais de 3 horas, receberam antibioticoterapia profilática e terapêutica; Grupo 4 - pacientes operados em 2008, 2009, regime hospitalar, duração superior a 3 horas, receberam antibioticoterapia profilática. Resultados: Nos Grupos 1 e 2, não houve infecção, no grupo 3, houve um (0,39%) caso de infecção, no grupo quatro, outro (0,42%) caso. Conclusão: Antibioticoterapia profilática não influenciou na ocorrência de infecção em cirurgias que duraram até duas horas. O uso de antibioticoterapia terapêutica não interferiu na incidência de infecção em cirurgias que tiveram duração acima de três horas.
