Lateral canthopexy as primary procedure in lower lid blepharoplasty

ABSTRACT

Introduction: The lid scar retraction constitutes usual complication of the postoperative lower lid blepharoplasty. Objective: This article discusses the importance of the systematic association of the lateral canthopexy in lower lid blepharoplasty to reduce its incidence. Method: In this retrospective study, 200 patients were assigned to the transcutaneous or transconjunctival lower lid blepharoplasty linked or not to the Lessa's lateral canthopexy, with a follow-up of 12 months. Results: The patients group submitted to lower lid blepharoplasty with lateral canthopexy had 1% of scar retraction against 16% in the isolated lower lid blepharoplasty group. Discussion: The systematic indication of the lateral canthopexy in association with lower lid blepharoplasty brings different opinions of many authors. For its easy execution, low morbidity and confirmed effectiveness in the prevention of the scleral show, rounding of the lateral canthus and ectropion post lower lid blepharoplasty, the authors conclude that the lateral canthopexy must constitute primary procedure in lower lid blepharoplasty surgery.

RESUMO

Introdução: A retração cicatricial da pálpebra constitui complicação comum do pós-operatório de blefaroplastia inferior. Objetivo: Neste artigo, discute-se a importância de associar sistematicamente a cantopexia lateral à blefaroplastia inferior para reduzir sua incidência. Método: Estudo retrospectivo de 200 pacientes submetidos a blefaroplastia inferior transcutânea ou transconjuntival associada ou não à cantopexia lateral proposta por Lessa, com seguimento de 12 meses. Resultados: No grupo de pacientes submetidos a blefaroplastia inferior com cantopexia lateral observou-se índice de 1% de retração cicatricial, contrastando com 16% dessa complicação no grupo que realizou a blefaroplastia inferior isolada. Discussão: A indicação sistemática da cantopexia lateral associada à blefaroplastia inferior divide as opiniões de diversos autores. Por sua fácil execução, baixa morbidade e eficácia comprovada na prevenção da esclera aparente, do arredondamento do canto lateral do olho e do ectrópio pós-blefaroplastia inferior, os autores defendem a opinião de que a cantopexia lateral deve constituir procedimento primário em cirurgia de blefaroplastia inferior.

Use of septal flap and mucoperichondrial flap for alar and lateral nasal reconstruction: case report

ABSTRACT

The authors report a case of a female 64 years old patient with nasal alar and left wall defects secondary to two basal cell carcinoma resections and adjuvant radiotherapy. The patient was submitted to reconstruction with local flaps of septal cartilage and nasal mucosa, conchal cartilage graft and forehead flap. This report shows the importance of septal flaps in external valve reconstruction. The patient evolved with good graft development and viability of the flaps. Displaying restoration of nasal function and satisfactory cosmetic results.

RESUMO

Os autores relatam o caso de uma paciente do sexo feminino, de 64 anos, com defeito nasal de asa e parede lateral esquerda secundário a duas ressecções de carcinoma basocelular e radioterapia adjuvante. A paciente foi submetida à reconstrução com retalhos locais de cartilagem septal e pericondro mucoso nasal, enxerto de cartilagem auricular conchal e retalho médio-frontal. O relato ressalta a importância dos retalhos de mucosa e septo nasal como opção nas reconstruções de válvula nasal externa. A paciente evoluiu com boa integração do enxerto e viabilidade dos retalhos, apresentando restauração da função nasal e resultado estético satisfatório.
