Photograph standardization and evaluation in periorbital surgery

ABSTRACT

Introduction: For comparative analysis of photographs the standardization becomes necessary. Unlike other regions of the body, the periorbital region has an extremely dynamic anatomy, that makes standardization a challenge. There are nine parameters if not standardized can influence the objective evaluation of periorbital region photographs. These parameters are: the facial coronal plane, camera's plane, the gaze position, camera height in relation to the patient, the use of flash, facial movements, distance of the patient to the camera, camera lens focal length (zoom) and lighting conditions. Objective: The objective of this study is to demonstrate that small variations in the parameters mentioned above can cause significant changes in the periorbital elements. Methods: To make the standardization an adapted ophthalmologic slit lamp was used. Twelve patients were photographed in front profile using the standardization and observed the change of the palpebral fissure measured at mid pupillary line by changing the coronal plane, gaze's position, use of flash and height variation of the camera. Results: With the parameters changes we obtained statistically changes of vertical palpebral fissure in all patients. Conclusion: The changes in the position used at work are subtle and hardly noticed when we took pictures free no setting plans and distances, falsifying results in objective analysis of the photographs when not adequately standardized.

RESUMO

Introdução: Para análise comparativa de fotografias é necessário haver padronização. Diferentemente de outras regiões do corpo, a região periorbital, por se constituir de uma anatomia extremamente dinâmica, torna a padronização um desafio. Existem nove parâmetros que, se não padronizados, podem influenciar a avaliação objetiva de fotografias da região periorbital. Entre estes parâmetros temos o plano facial coronal, plano da câmera fotográfica, posição do olhar, altura da câmera em relação ao paciente, uso do flash, mímica facial, distância do paciente à câmera fotográfica, distância focal da câmera fotográfica (zoom) e as condições de iluminação. Objetivo: O objetivo deste estudo é demonstrar que pequenas variações nos parâmetros citados anteriormente podem provocar alterações significativas nos elementos periorbitais. Método: Para manutenção dos nove parâmetros necessários à padronização foi utilizada uma lâmpada de fenda oftalmológica adaptada. Doze pacientes foram fotografados em perfil frontal, utilizando a padronização, sendo observada a variação da fenda palpebral medida na linha médio pupilar, alterando-se o plano coronal, posição do olhar, uso do flash e variação da altura da câmera. Resultados: Com a variação dos parâmetros foi obtida alteração significativa da fenda palpebral vertical em todos os pacientes. Conclusão: As mudanças de posição utilizadas no trabalho são sutis e dificilmente percebidas quando tiramos foto livre sem fixar os planos e distâncias, falseando resultados na análise objetiva das fotografias quando não adequadamente padronizadas.
