Using tamoxifen in keloid treatment

ABSTRACT

Background: We all know that the survival of the burn victims has increased due to the advances of the treatment in the critical phase. Because of that, the number of patients who shows sequels also increased. The objective of the present study is to measure the improvement in the quality of the lifetime of the patients with hypertrophic scars as result of burn or graft skin reducing, that way, the stigma caused by these scars. Many approaches have been done in order to prevent and treat pathological scars, frequently without success to some patients because the method or because of the difficulty of keeping the patient attached to the treatment. Method: Forty-six patients, 20 men and 26 women, have used 0.1% topic tamoxifen for a period of 4 to 12 months. Weekly evaluations were done during the first month and afterwards monthly, until a year. Photographs were taken after 6 months and a year. Results: Thirty-eight (82.6%) patients told that the improved in the first two weeks of treatment; 93.48% of the patients told that the scar height was reduced. In this study, we used tamoxifen citrate, an antineoplasic drug, to reduce the height of hypertrophic scars of burn marked patients. Conclusion: The results encouraged us to continue the treatment of scars with topic house use of tamoxifen.

RESUMO

Introdução: É sabido que a sobrevivência de pacientes vítimas de queimaduras tem aumentado significativamente devido aos avanços do tratamento na fase aguda. Como conseqüência, a quantidade de pacientes apresentando sequelas também aumentou. O propósito deste estudo foi avaliar a melhora na qualidade de vida de pacientes com cicatrizes hipertróficas resultantes de queimaduras ou após enxertos de pele, através da redução do estigma causado por estas cicatrizes. Várias abordagens têm sido descritas para prevenir e tratar cicatrizes patológicas, freqüentemente sem sucesso em alguns pacientes, por causa do método ou pela dificuldade em se conseguir a aderência do paciente ao tratamento. Método: Quarenta e seis pacientes, 20 homens e 26 mulheres, usaram tamoxifen tópico 0,1%, em um período de 4 a 12 meses. Avaliações semanais foram feitas no primeiro mês e, mensalmente, até o limite de um ano. Fotografias foram tiradas após seis meses e um ano. Resultados: Trinta e oito (82,6%) pacientes relataram melhora no prurido nas primeiras duas semanas de tratamento. Melhora na altura da cicatriz foi relatada por 93,48% dos pacientes. Neste estudo, usou-se citrato de tamoxifeno, uma droga antineoplásica, para redução da altura de cicatrizes hipertróficas de pacientes seqüelados de queimaduras. Conclusão: Os resultados nos encorajaram a continuar o tratamento de cicatrizes com o uso tópico domiciliar de tamoxifeno.
