Early active mobilization after flexor tendon repair in zone 2

ABSTRACT

Background: The first cause of bad results after flexor tendon repair is scar adhesions. An early mobilization protocol could help to reduce those adhesions under the increased risk of tendon rupture. This study aims to evaluate the functional results after flexor repair in zone 2. Method: A number of 136 hand flexor tendons were repaired in zone 2, in a total number of 82 patients, all of them with only one finger affected, with a total rupture of both flexor tendons in the case of long fingers or the flexor pollicis longus in the case of thumb, managed under an early active flexion -extension protocol. The results were based on both the International Federation of Societies for Surgery of the Hand (IFSSH) and the Strickland's systems of evaluation. The results of long fingers and thumbs were analysed separately. Results: The results of the long fingers group were excellent (72.2%), good (26.0%) and fair (1.9%), according to the Strickland's standards and were good (81.5%), fair (16.6%) and poor (1.9%), according to the IFSSH standards. The results of the thumbs group were excellent (96.4%) and fair (3.6%), according to the Strickland's standards and were excellent (82.1%), good (14.3%) and poor (3.6%), according to the IFSSH standards. A rupture rate was 6.09%, nevertheless functional results of reoperated patients was satisfactory. Conclusions: This study corroborates evidence that postoperative programs that incorporate early active motion (flexion/ extension) can produce good results after flexor tendon repair in zone 2 in a single digit injury.

RESUMO

Introdução: A primeira causa de maus resultados após uma reparação de tendão flexor em zona 2 é a formação de aderências. Um protocolo de mobilização precoce poderia ajudar a diminuir tais aderências, sob risco de aumentar a taxa de rupturas. Este estudo objetiva avaliar o resultado funcional do reparo de lesões de flexores na zona 2. Método: Foram tratados 136 tendões flexores da mão, na zona 2, em 82 pacientes, todos com lesões unidigitais, com secção completa de ambos os tendões, no caso dos dedos largos, o do flexor pollicis longus, no caso de polegar, submetidos a um protocolo de flexão e extensão ativa precoce. Os resultados foram classificados segundo a International Federation for Societies for Surgery of the Hands (IFSSH) e pela classificação de Strickland. Resultados: Os resultados funcionais dos dedos longos foram excelentes (72,2%), bons (26,0%) e regulares (1,9%), de acordo com a classificação de Strickland e bons (81,5%), regulares (16,6%) e pobres (1,9%), segundo a classificação da IFSSH. Os resultados do polegar foram excelentes (96,4%) e regulares (3,6%), de acordo com a classificação de Strickland e excelentes (82,1%), bons (14,3%) e pobres (3,6%), de acordo com a classificação da IFSSH. A taxa de ruptura foi de 6,09% ; não obstante, os resultados funcionais dos pacientes reoperados foram satisfatórios. Conclusões: O presente estudo sugere que o protocolo de mobilização ativa precoce no pós-operatório das tenorrafias de flexores da mão em zona 2 pode proporcionar bons resultados funcionais em lesões no dedo único.

Reconstruction of mandible bone defect: behavior of osseous integrated implants upon vascularized iliac bone graft and vascularized fíbula bone graft

ABSTRACT

Introduction: Reconstruction of mandible bone segment using vascularized bone grafts after segmental bone defects is the most used method nowadays, with donor sites including fibula, radius, scapula and iliac bone. Method: A descriptive and retrospective analysis was performed with authors experience in 45 cases of microsurgical mandible reconstruction, all with mandible bone defects. We used vascularized bone graft from iliac bone or fibula, evaluating the behavior of grafts related to the simplicity of use as well as a reliability to perform dental osseointegrated implants in them. Results: A fibular microsurgical graft was used in 10 patients and an iliac crest bone graft was used in the others 35 cases. . In all cases when fibula was used it was necessary to make an osteotomy of the graft and in just 4 cases successful osseointegrated dental implant was achieved. An iliac crest bone graft was performed without necessity of osteotomy and in all cases a successful osseointegration of dental implants was achieved, with a return of the masticatory function after 6 months. Conclusions: Reconstructions performed with vascularized iliac bone graft showed good functional results .In the cases where fibula bone graft was used we observe worse results related to osseointegration of dental implants.

RESUMO

Introdução: A reconstrução do segmento ósseo mandibular utilizando retalhos ósseos vascularizados após perdas segmentares é o método mais utilizado na atualidade, tendo zonas doadoras que incluem a fíbula, o rádio, a escápula e a crista ilíaca. Método: Apresenta-se uma análise descritiva e retrospectiva da experiência dos autores em 45 casos de reconstrução mandibular microcirúrgica em pacientes com defeitos ósseos da mandíbula. Foram utilizados enxertos ósseos vascularizados, ou da crista ilíaca ou da fíbula, avaliando o comportamento dos enxertos em relação a sua facilidade de uso e viabilidade de uma posterior ósseo-integração dos implantes. Resultados: O retalho microcirúrgico de fíbula foi utilizado em 10 pacientes, e o retalho de crista ilíaca foi em outros 35 casos. Em todos os casos nos quais se utilizou a fíbula, houve necessidade de osteotomia do segmento ósseo transferido e em somente quatro casos a implantação dentária foi satisfatória. No entanto, o retalho de crista ilíaca foi realizado sem a necessidade de osteotomia e foi possível a colocação dos implantes nos 35 retalhos, não sendo observada em nenhum caso falha na integração dos implantes, sendo possível o retorno à mastigação após 6 meses. Conclusões: As reconstruções realizadas com o retalho microcirúrgico de crista ilíaca evidenciaram bons resultados funcionais. Quanto aos casos em que se utilizou o retalho de fíbula, obtiveram-se piores resultados no que tange à integração dos implantes dentários.

Use of adipofascial reverse flap in skin loss of distal third of leg and hindfoot

ABSTRACT

Background: Reconstructive lower limb surgery may represent a great challenge to the surgeon, mainly when the injury is located in the distal third of the leg and hindfoot. Among options available for skin coverage, the reverse adipofascial flap with distal pedicle and constant vascular pattern may be performed in different sizes and reach even the more distal areas. Method: From April 1995 to May 2002, fifteen adipofascial flaps were used to coverage skin defects located in the distal third of the leg and hindfoot. Results: One case of partial flap necrosis was observed, while in all the others cases good skin coverage was achieved, which allowed return to weight bearing after 6 weeks. The reconstruction was immediate in eight patients, while in the others it was performed after a week. Conclusions: A reverse adipofascial flap is a reliably option with satisfactory results for skin coverage for the treatment of most distal dorsal leg and hindfood defects.

RESUMO

Introdução: A cirurgia reconstrutiva do membro inferior pode representar um grande desafio para o cirurgião, principalmente quando a lesão atinge as regiões aquiliana e do calcâneo. Dentre as opções existentes de reconstrução, o retalho adipofascial com pedículo distal apresenta vascularização constante, dimensão variável e pode atingir até mesmo as regiões mais distais. Método: No período de abril de 1995 a maio de 2002, realizaramse quinze retalhos adipofasciais para a cobertura cutânea do terço distal da perna e pé. Resultados: Em um caso observou-se necrose parcial, os demais pacientes obtiveram cobertura adequada que permitiu a deambulação em até seis semanas. A reconstrução imediata foi realizada em oito pacientes, sendo os demais submetidos à cirurgia cerca de uma semana após o trauma. Conclusão: O retalho adipofascial reverso é uma opção confiável e com resultados satisfatórios no tratamento dos defeitos mais distais da perna e região do calcâneo.
