ABSTRACT

In 20 patients undergoing functional unilateral cleft lip repair, 10 complete palate and lip clefts and 10 incomplete cleft lips with intact nasal nostril, changes in the lengths of the lip were evaluated. The lengths were measured preoperatively, after the muscle layer were repaired, and after the skin was repaired with Z plasty. The authors described the historical evolution of the cleft lip repair, with special interest in the functional muscle repair and the techniques of lengthening of the skin, avoiding further scar retraction. The measurements were made in each patient and showed a good improvement of the lip length after the muscle layer repair and the skin Z plasty. Statistical analysis of the data and comparisons were made using the t-Student test valid at alpha < 0.01. Finally, the authors discuss his preference to the Z plasty, described by Perseu Lemos and modified by Spina, achieving good alar base rotation and a necessary lengthing of the lip.

RESUMO

Os autores apresentam o estudo da efetividade do alongamento conseguido pela plástica em Z e pela reconstrução muscular, em 20 pacientes portadores de fissuras labiopalatinas unilaterais, sendo 10 pré-foramen incisivo incompleta, com a presença do assoalho nasal e 10 com fissura transforamen incisivo completa. Foram realizadas medidas da altura do lábio em fase pré-operatória, após o tratamento muscular, após a simples adesão da pele e após completada a plástica em Z. Os resultados numéricos das medidas realizadas demonstraram que há um ganho significativo após a reparação muscular e também após a plástica em Z, comprovada pelo teste estatístico de t de Student, com nível de significância de 0,01. O alongamento necessário foi conseguido em todos os casos, atingindo a altura próxima das medidas ideais. Os autores discutem a evolução das técnicas cirúrgicas empregadas nas queiloplastias, com enfoque especial para a reconstrução da musculatura da região e nas táticas que possibilitam o alongamento do lábio, minimizando também sua retração cicatricial. Os autores comentam a sua preferência pela plástica em Z, descrita por Perseu Lemos e modificada por Spina, por fornecer uma maior rotação da asa e preservar em maior extensão a crista filtral, minimizando também a retração cicatricial do lábio.
