ABSTRACT

INTRODUCTION: Augmentation mammoplasty is one of the most commonly performed surgical procedures by plastic surgeons. Back-to-front flipping of an implant after augmentation mammoplasty is a rare complication, occurring in 0-5% of cases. CASE REPORT: The authors report a patient who presented with back-to-front flipping of an anatomical implant 31 months after augmentation mammoplasty.

RESUMO

INTRODUÇÃO: A mamoplastia de aumento está entre os procedimentos mais realizados por cirurgiões plásticos. A rotação de trás para frente de implante após mamoplastia de aumento é uma complicação rara, que acontece entre 0 e 5% dos casos. RELATO DE CASO: Os autores relatam um caso de uma paciente que apresentou rotação de trás para frente de implante anatômico após 31 meses da mamoplastia de aumento.

Epidemiological study of patients treated at the outpatient clinic of the Burn Treatment Center of the Hospital Municipal do Tatuapé between January 2019 and January 2020

ABSTRACT

Introduction: Burns are in fourth place as the most common type of trauma globally, generating a challenge due to the severity of patients' injuries. This study investigates the epidemiological profile of patients treated at the Burn Treatment Center (BTC) outpatient clinic of the Hospital Municipal do Tatuapé (HMT/SP).
Methods: Data obtained from the outpatient records of the BTC outpatient clinic of the HMT/SP were analyzed from January 2019 to January 2020, using the following variables: gender, age, causal agent, burn extension, and affected region.
Results: Higher predominance of burns in males (55.3%). There was a higher percentage in the population between 21 and 30 years (15.9%), followed closely by children under 10 years of age (15.8%). The leading causative agent was scalded/heated liquids (56.8%); followed by contact burn (14.7%), the vast majority of patients were affected by medium-sized burn (90.1%), i.e., between 11 and 20% of the burned body surface and the thorax/abdomen the most affected region.
Conclusion: The epidemiological profile demonstrated in this study was compatible, in most data, with the profile found in other centers specialized in treating burns.

RESUMO

Introdução: As queimaduras encontram-se em quarto lugar como o tipo de trauma mais comum do mundo, gerando um desafio pela gravidade das lesões apresentadas pelos pacientes. Este estudo tem como objetivo rastrear o perfil epidemiológico de pacientes atendidos no ambulatório do Centro de Tratamento de Queimaduras (CTQ) do Hospital Municipal do Tatuapé (HMT/SP).
Métodos: Foram analisados os dados obtidos a partir das fichas de atendimento do ambulatório do CTQ do HMT/SP, no período de janeiro de 2019 a janeiro de 2020, utilizando as seguintes variáveis: sexo, idade, agente causal, extensão da queimadura e região acometida.
Resultados: Maior predomínio de queimadura no sexo masculino (55,3%). Houve porcentagem maior na população entre 21 a 30 anos (15,9%), seguidos de perto pelos menores de 10 anos (15,8%). O principal agente causador foi escaldo/líquidos aquecidos (56,8%); seguido de queimadura por contato (14,7%), a grande maioria dos pacientes foram acometidos por queimadura de médio porte (90,1%), ou seja, entre 11 e 20% de superfície corpórea queimada e o tórax/abdome a região mais acometida.
Conclusão: O perfil epidemiológico demonstrado nesse estudo, mostrou-se compatível, na maioria dos dados, com o perfil encontrado em outros centros especializados em tratamento de queimados.

Cost comparison between enoxaparin and new oral anticoagulants dabigatran and rivaroxaban in venous thromboembolism prophylaxis

ABSTRACT

BACKGROUND: Venous thromboembolism, an important complication that is easily prevented during surgery, is among the few clinical entities wherein the use of prophylaxis has been found to be effective, such as in the case of antibiotic prophylaxis for the prevention of surgical site infections. The objectives of prophylaxis are to reduce the incidence of deep vein thrombosis, minimize the risk of long-term complications such as chronic venous insufficiency and pulmonary hypertension, and prevent death caused by pulmonary embolism. In this study, we aimed to provide current information to plastic surgeons on the therapeutic options for prophylaxis of venous thromboembolism as well as to compare treatment costs of the drugs enoxaparin, dabigatran, and rivaroxaban. METHOD: For comparison, the prophylaxis regimen was started 6 hours after the surgery and continued for 10 days. The end user price was obtained from an internet search that included leading pharmacy chains. RESULTS: The patient costs of the new oral anticoagulants dabigatran and rivaroxaban were lower than that of enoxaparin. CONCLUSIONS:The cost of venous thromboembolism prophylaxis consisting of the new oral anticoagulants dabigatran and rivaroxaban is lower than that of low molecular weight heparin.

RESUMO

INTRODUÇÃO: O tromboembolismo venoso é uma complicação importante altamente evitável em cirurgia, sendo uma das poucas entidades clínicas em medicina passíveis de profilaxia comprovadamente eficiente, assim como ocorre na profilaxia antibiótica das infecções cirúrgicas. A profilaxia tem por objetivo diminuir a incidência de trombose venosa profunda, minimizar os riscos das complicações a longo prazo da insuficiência venosa crônica e hipertensão pulmonar, bem como prevenir a morte consequente de embolia pulmonar. Este estudo tem por objetivo atualizar os cirurgiões plásticos quanto às medicações que podem ser adotadas na profilaxia do tromboembolismo venoso, bem como comparar o custo dessas medicações (enoxaparina, dabigatran e rivaroxaban). MÉTODO: Para efeito de comparação foi adotado um esquema de profilaxia iniciado 6 horas após o fim da cirurgia e mantido por 10 dias. A pesquisa foi realizada na internet entre as principais redes farmacêuticas, levando em conta o custo para o consumidor. RESULTADOS: Os novos anticoagulantes orais dabigatran e rivaroxaban apresentaram custo menor para o paciente. CONCLUSÕES: Os novos anticoagulantes orais dabigatran e rivaroxaban possuem custo menor na profilaxia do tromboembolismo venoso em comparação à heparina de baixo peso molecular.
