ABSTRACT

In the treatment of breast cancer, the quadrantectomies, when repaired through the simple approximation of the ressected area borders, may lead to sequelae of difficult solution. The authors introduce a new option to immediate partial reconstruction of the superomedial breast quadrant by the use of bilobulated flap with lateral thoracic tissue. They evaluate the result of the reconstruction in thirty six patients in short, medium and long time follow-up, considering the age of these patients, the volume of ressected breast in each occasion, the side of incidence, histological type and stage of the tumor, as well as the aesthetical result achieved, operative time and the reconstruction tolerance to radiotherapy and chemotherapy. The bilobulated flap technique, in conclusion, appears as a good new alternative in the mammary reconstruction of the upper quadrants.

RESUMO

No tratamento cirúrgico do câncer de mama, as quadrantectomias, quando reparadas por meio da simples aproximação das bordas da área cruenta resultante, podem levar a seqüelas de difícil solução. Os autores apresentam uma nova opção para reconstrução parcial imediata do quadrante súpero-medial da mama por meio do uso de retalho bilobulado com tecido da região torácica lateral. Analisam o resultado da reconstrução em 36 pacientes, com seguimento a curto, médio e longo prazos, levando em consideração a idade dessas pacientes, o volume de mama ressecado por ocasião da quadrantectomia, a lateralidade, tipo histológico e estadio do tumor, avaliando o resultado estético alcançado, tempo operatório para realização da reconstrução e tolerância à radioterapia e à quimioterapia. Concluem que o retalho bilobulado surge como uma boa alternativa na reconstrução mamária dos quadrantes superiores.
