Breast reconstruction in young women and their peculiarities

ABSTRACT

INTRODUCTION: To evaluate the main features and methods used in breast reconstruction in young women considering their unique characteristics. METHODS: A retrospective records review of patients who underwent mastectomy followed by breast reconstruction between January 2008 and December 2015 was conducted, selecting those reconstructions that were performed in women younger than 40 years. RESULTS: Forty-three patients were selected. The average age was 33.86 years. Many had some comorbidities, the most common being overweight and anxiety/depressive disorder. Fourteen patients had a family history of the disease. Most were diagnosed with invasive ductal carcinoma. Additional cancer treatments were administered in most cases. All patients underwent a full mastectomy in the breast with cancer, and in 16, there was the option of contralateral risk-reducing mastectomy. Of the 43 reconstructions, 36 were immediate and 7 were delayed, and 17 involved use of implants, 13 involved use of tissue expanders, 4 involved the TRAM, and 9 involved the GD. We observed 15 complications; the most severe were infection with reconstruction loss in one patient and areola necrosis in another. CONCLUSIONS: Young women undergoing breast reconstruction represent a population subgroup with its own characteristics. The tumors and personal and social patterns differ, and based on this long list of variables, treatment options are diverse. In our series, breast implants and tissue expanders were often most used.

RESUMO

INTRODUÇÃO: O objetivo do trabalho foi avaliar as principais características e métodos utilizados na reconstrução mamária de mulheres jovens, considerando suas peculiaridades. MÉTODOS: Foi realizada uma revisão retrospectiva dos prontuários das pacientes submetidas à mastectomia seguida de reconstrução mamária no período de janeiro de 2008 a dezembro de 2015, sendo selecionados aqueles cuja reconstrução foi realizada em mulheres de até 40 anos de idade. RESULTADOS: 43 pacientes foram selecionadas. A média de idade foi de 33,86 anos. Grande parte possuía alguma comorbidade, sendo as mais comuns o sobrepeso e o transtorno ansioso/depressivo. Quatorze pacientes tinham história familiar da doença. A maioria obteve o diagnóstico de carcinoma ductal invasor. Tratamentos oncológicos complementares foram realizados em grande parte dos casos. Todas as pacientes foram submetidas à mastectomia total na mama portadora da neoplasia, sendo que em 16 houve a opção pela mastectomia redutora de risco contralateral. Do total de 43 reconstruções, 36 foram imediatas e 7 tardias; sendo 17 reconstruídos com implantes mamários, 13 com expansores teciduais, 4 com TRAM e 9 com GD. Houve 15 complicações, sendo as mais graves um caso de infecção com perda da reconstrução e um caso de necrose de aréola. CONCLUSÕES: Mulheres jovens submetidas à reconstrução mamária representam um subgrupo populacional com características próprias. Os padrões tumorais, pessoais e sociais diferem e, com base nesta imensa lista de variáveis, as possibilidades de tratamento são diversas. Em nossa casuística, o emprego de implantes mamários e expansores teciduais foi realizado com maior frequência neste subgrupo de pacientes.

Breast reconstruction with a latissimus dorsi myocutaneous flap and implants: evaluation of strategies adopted to improve the results and evaluation of satisfaction by Breast-Q

ABSTRACT

Introduction: The paper describes a 14-year experience with breast reconstruction using a latissimus dorsi myocutaneous flap (LDMF) and breast implants. The objective was to delineate the experience with LDMF and breast implants, skin island schedules and dissection details, flap transposition and placement of the implant under a double layer, strategies to minimize damage in the donor area, and strategies of breast symmetry and reconstruction of the nipple-areola complex, in association with evaluation using the Breast-Q questionnaire.
Methods: A review of medical records was performed between April 2003 and June 2017.
Results: In the period, 76 patients with a mean age of 50.09 years underwent reconstruction with a LDMF, which was bilateral in 11, for the right breast in 34, for the left breast in 31, immediate in 41, late in 22, and for rescue in 13.
Conclusion: We conclude and verified with the Breast-Q questionnaire that with a precise indication, the proposed reconstruction technique with a double subpectoral plane and coverage with a LDMF is safer with a lower complication rate.

RESUMO

Introdução: O trabalho descreve 14 anos em reconstrução mamária com o retalho miocutâneo do grande dorsal (RGD) e implantes mamários. O objetivo é delinear experiência com o RGD e implantes mamários, programações das ilhas de pele e detalhes da dissecção, transposição de retalho e colocação do implante sob dupla camada muscular, estratégias para minimizar os danos na área doadora, estratégias de simetrização mamária e reconstrução do complexo areolopapilar, associado a avaliação do questionário Breast-Q.
Métodos: Foi realizada revisão de prontuários entre abril de 2003 a junho de 2017.
Resultados: No período 76 pacientes foram reconstruídas com o RGD, com idade média de 50,09 anos, sendo 11 bilaterais, 34 mama direita e 31 mama esquerda. 41 imediatas, 22 tardias e 13 de resgate. A média de satisfação foi 72,36% por meio do Breast-Q.
Conclusão: Concluímos e comprovamos com o questionário do Breast-Q que a indicação precisa aliada a técnica de reconstrução proposta com plano duplo subpeitoral associada a cobertura do RDG, propicia uma loja mais segura, diminuído o índice de rippling.

Breast reconstruction with creation of upper retropectoral and lower mixed subcutaneous cavities and use of flaps

ABSTRACT

Introduction: A total of 57,960 new cases of breast cancer were expected in Brazil in 2016 according to data from the National Cancer Institute, corresponding to 25% of cancers in the country (excluding non-melanoma skin tumors). This study aims at presenting a surgical approach for immediate breast reconstructions with upper retropectoral and lower mixed subcutaneous cavities.
Methods: The authors performed breast reconstruction using flaps of the pectoral muscle and inferior cutaneous pedicle associated with insertion of silicone breast implants. The medical records of patients operated between 2012 and 2016 at the Plastic Surgery Service of the senior author's private clinic at Daher Hospital and Armed Forces Hospital were retrospectively analyzed.
Results: The results were satisfactory, with low complication rates and high patient and author satisfaction. Thirty-six patients, with a mean age of 59 years, underwent breast reconstruction using the described technique. The complications were nipple-areola complex necrosis, dehiscence, seroma, hematoma, liponecrosis, implant displacement, and deep venous thrombosis. No patient needed salvage surgery or had recurrence of mammary neoplasia during the study period.
Conclusion: The present technique preserves the skin located at the lower portion of the breast, with a low risk of dehiscence or extrusion of the implant in this region, providing a double protection of the implant, with the pectoralis major muscle in the upper two thirds and the cutaneous-dermis-fat flap in the lower third, characterizing a "dual-plane" positioning.

RESUMO

Introdução: Segundo dados do Instituto Nacional de Câncer (INCA) de 2016, estima-se em 57.960 novos casos de câncer de mama no Brasil, o que corresponde a 25% dentre todos os tipos de cânceres no país (excluindo-se os tumores de pele não melanoma). O presente artigo visa apresentar uma forma de abordagem para as reconstruções mamárias imediatas com loja retropeitoral superior e subcutânea mista inferior.
Métodos: Os autores descrevem da técnica de reconstrução mamária com retalho do músculo peitoral e pedículo cutâneo inferior, associado à inclusão de implante mamário de silicone. Foi realizada análise retrospectiva de prontuários das pacientes operadas entre os anos 2012 e 2016 no Serviço de Cirurgia Plástica da clínica privada do autor sênior, no Hospital Daher e no Hospital das Forças Armadas.
Resultados: Os resultados são satisfatórios, com baixos índices de complicações e com satisfação elevada para os pacientes e os autores. Trinta e seis pacientes foram submetidas à reconstrução mamária com a técnica descrita, com média de idade de 59 anos. As complicações apresentadas foram necrose de complexo areolopapilar, deiscência, seroma, hematoma, liponecrose, deslocamento do implante e trombose venosa profunda. Nenhuma paciente teve necessidade de resgate da reconstrução ou apresentou recidiva da neoplasia mamária durante o período do estudo.
Conclusão: Trata-se de técnica que preserva a pele da mama em sua parte inferior, com baixa possibilidade de deiscência ou extrusão do implante nesta região, proporcionando uma dupla proteção deste implante com o músculo peitoral maior nos dois terços superiores e o retalho cutâneo-dermogorduroso no terço inferior, caracterizando um "dual-plane".

Prospective assessment of the repercussions on the lipid profile of surgeries involving liposuction and dermolipectomies

ABSTRACT

Introduction: Liposuction associated with dermolipectomies is the most commonly performed surgical procedure in plastic surgery. Although regarded as an extremely safe surgery, some considerations must be taken on the possible metabolic effects of these surgeries. The development of the tumescent technique in liposuction allowed the safer removal of large amounts of fat. The objective is to compare lipid profile variations in the early and late postoperative period in patients undergoing liposuction and dermolipectomies.
Methods: Between October 2006 and June 2012, 40 female patients who were candidates for surgeries involving liposuction and dermolipectomies were prospectively followed, and the lipid profile was analyzed through preoperative and postoperative examinations. The surgeries performed were mammoplasty + liposuction, abdominoplasty + liposuction, and lipoabdominoplasty + mammoplasty.
Results: Of the 40 female patients who were followed, 20 were selected (after applying the exclusion criteria). In agreement with our study, in 1996, Cazes showed that there were no changes in the lipid profile of patients 12 months after lipoabdominoplasty.
Conclusion: After a preoperative and postoperative analysis of 20 patients, it was observed that there were no statistically significant changes in the lipid profile and that the measurements after 1 year were close to those obtained in the preoperative period.

RESUMO

Introdução: Lipoaspiração associada a dermolipectomias é o procedimento cirúrgico mais comumente realizado em cirurgia plástica. Apesar de ser considerada uma cirurgia extremamente segura, algumas considerações devem ser levantadas a respeito dos possíveis efeitos metabólicos que essas cirurgias possam causar. O desenvolvimento da técnica tumescente de lipoaspiração permitiu a remoção de grande quantidade de gordura de modo mais seguro. O objetivo é comparar as variações do perfil lipídico em pós-operatório precoce e tardio de pacientes submetidos à lipoaspiração e dermolipectomias.
Métodos: Entre outubro de 2006 e junho de 2012, 40 pacientes do sexo feminino candidatas a cirurgias que envolviam lipoaspiração e dermolipectomias foram acompanhadas prospectivamente e o perfil lipídico foi analisado por meio de exames no pré-operatório e no pós-operatório. As cirurgias realizadas foram: mamoplastia + lipoaspiração, abdominoplastia + lipoaspiração e lipoabdominoplastia + mamoplastia.
Resultados: Das 40 pacientes que foram acompanhadas no estudo, 20 pacientes do sexo feminino foram selecionadas (após a aplicação dos critérios de exclusão). Em consonância com nosso estudo, Cazes, em 1996, demonstrou que após 12 meses de pós-operatório de lipoabdominoplastia não houve alteração do perfil lipídico das pacientes.
Conclusão: Após análise pré- e pós-operatória de 20 pacientes, observamos que não há alterações estatísticas significantes em relação ao perfil lipídico com tendência de equilíbrio das aferições em um ano em patamares próximos aos observados no pré-operatório.

Perception of breast cancer and breast reconstruction in medical school students

ABSTRACT

Introduction: Breast cancer is the most common type of cancer among women in the world and in Brazil, after non-melanoma skin cancer. Our objective was to evaluate the medical students' perception of breast cancer and breast reconstruction.
Methods: This is a cross-sectional pilot study composed of students from the fifth and sixth year of medical school, in May 2016.
Results: We questioned whether there is a possibility of breast reconstruction after mastectomy, and the response was unanimous (100%). For a better characterization, we asked if the reconstruction could be done at the same time as the mastectomy, and 69 (57.5%) students said yes and 51 (42.5%) denied. In the analysis of surgical knowledge, we asked whether they knew any breast reconstruction technique, and 49 (40.83%) answered yes and 71 (59.16%) denied knowing. With regard to referral to a medical specialist who was better prepared to follow and perform breast reconstruction, 93 (77.5%) students chose plastic surgery and 26 (21.66%) chose mastology. Regarding the possibility of breast reconstruction in patients who need to undergo adjuvant radiotherapy, 66 (55%) answered yes, 51 (42.5%) did not answer, and three (2.5%) did not know how to respond. Regarding this possibility in patients with silicone implants, 59 (49.16%) answered yes, three (2.5%) answered no, and 58 (48.33%) said they did not know about the subject.
Conclusion: We conclude that mastology has been gaining immense interest in the field of breast reconstruction, including in the academic world, based on the high percentage of respondents who stated that they are better prepared for this procedure than for plastic surgery for breast reconstruction.

RESUMO

Introdução: O câncer de mama é o tipo mais comum entre as mulheres no mundo e no Brasil, depois do câncer de pele não melanoma. O objetivo é avaliar a percepção dos estudantes de medicina sobre o câncer de mama e reconstrução mamária.
Métodos: Trata-se de estudo transversal piloto composto por alunos do 5º e 6º ano de curso de medicina, realizado de maio de 2016.
Resultados: Foi questionado se há possibilidade de reconstrução mamária após a mastectomia, sendo que de forma unânime 100% foi sim. Para melhor caracterização, foi perguntado se a reconstrução pode ser feita no mesmo momento da mastectomia, 69 (57,5%) estudantes marcaram sim e 51 (42,5%) negaram. Em análise aos conhecimentos cirúrgicos, foi perguntado se os mesmos conhecem alguma técnica de reconstrução mamária, sendo que 49 (40,83%) responderam que sim e 71 (59,16%) negaram conhecer. Em relação ao encaminhamento à especialidade médica mais preparada para acompanhar e realizar a reconstrução mamária, 93 (77,5%) discentes elegeram a cirurgia plástica e 26 (21,66%) a mastologia. Quanto à possibilidade de reconstrução de mama em pacientes que farão radioterapia adjuvante, 66 (55%) responderam sim, 51 (42,5%) não e 3 (2,5%) não souberam responder. Quanto a esta possibilidade mesmo em pacientes com implantes de silicone, 59 (49,16%) responderam sim, 3 (2,5%) responderam não e 58 (48,33%) afirmaram não saber sobre o assunto.
Conclusão: Observou-se que a Mastologia vem ganhando espaço na reconstrução de mama, inclusive no meio acadêmico, devido ao alto percentual de resposta de que a mesma seria mais preparada do que a Cirurgia Plástica para reconstrução mamária.

Post-Bariatric surgery: six surgeries in two steps

ABSTRACT

Introduction: Weight reduction achieved after bariatric surgery can result in deformities of various parts of the body. The need for reduction of leftover skin and body remodeling makes patients seek body contouring surgery to improve their quality of life. The aim of the study was to present a protocol for post-bariatric plastic surgery at Daher Hospital.
Methods: This was a retrospective longitudinal observational study. As routine protocol, we divided the procedures into two steps, performed in different surgical stages, which we termed the two-level principle. Herein we describe the routine of pre, intra, and postoperative care, as well as details of the surgical techniques. Photographic images were analyzed by an external evaluator.
Results: The study included 13 ex-obese patients, with a mean age of 44 years. Of them, 6 patients underwent lower- and upper-level surgeries and another 6 underwent lower-level surgeries. The mean surgical time for the lower level was 4 hours and 54 minutes and that for the upper level was 4 hours and 5 minutes. A total of 92.30% of the patients were satisfied with the results. An external evaluator classified 92.30% of the cases as good or great.
Conclusion: Circumferential abdominoplasty has immense benefits and complementary surgeries are vital for a satisfactory result. Reduction of the surgical time requires great synchronization of the surgical team, which allows procedures to be performed simultaneously by multiple surgical teams working in different body parts. Satisfactory results and low complication rates allow the applicability of our routine in post-bariatric patients.

RESUMO

Introdução: Redução ponderal conseguida após cirurgia bariátrica pode resultar em deformidades de diversos segmentos corporais. Necessidade da redução das sobras de pele e da remodelação do corpo faz com que a cirurgia do contorno corporal seja procurada por pacientes para melhorar a qualidade de vida. Objetivo do estudo é apresentar a rotina de cirurgia plástica pós-bariátrica do Hospital Daher.
Métodos: Estudo observacional longitudinal retrospectivo. Como rotina, dividimos os procedimentos em duas etapas realizadas em atos operatórios distintos: princípio dos dois andares. Descrevemos a rotina dos cuidados pré, intra e pós-operatório, bem como detalhes das técnicas cirúrgicas. Imagens fotográficas foram analisadas por um avaliador externo.
Resultados: Estudo composto 13 pacientes ex-obesos, média de idade de 44 anos. Seis pacientes foram submetidos a cirurgia do andar inferior e superior e outros 6 apenas do andar inferior. Tempo médio da cirurgia do andar inferior foi de 4 horas e 54 minutos e do andar superior foi de 4 horas e 5 minutos. 92,30% dos pacientes mostraram-se satisfeitos com o resultado. Avaliação externa revelou uma taxa de 92,30% dos casos classificados como bons ou ótimos.
Conclusão: Abdominoplastia circunferencial traz grandes benefícios e as cirurgias complementares são primordiais para um resultado satisfatório. Redução do tempo cirúrgico exige grande sincronização da equipe cirúrgica, permitindo realizar procedimentos combinados por times cirúrgicos trabalhando em compartimentos corporais diferentes, em tempo simultâneo. Resultados satisfatórios obtidos e baixos índices de complicação possibilitam aplicabilidade da nossa rotina em pacientes pós-bariátricos.

Quality of life and aesthetic results after mastectomy and mammary reconstruction

ABSTRACT

Introduction: Due to the increased incidence of breast cancer, the demand for breast reconstruction has been increasing, along with concerns regarding the satisfaction and quality of life of the patients. Mastectomy can be a traumatic experience, especially when it is perceived as a mutilation, which can impact self-esteem and emotional stability. The BREAST-Q® questionnaire was internationally validated and formulated for the pre- and postoperative assessment of quality of life related to breast reconstruction. This study aimed to evaluate quality of life and aesthetic result satisfaction in patients who underwent breast reconstruction with implants by comparing the period after breast reconstruction with the period before.
Method: A retrospective longitudinal observational study was carried out by reviewing the charts of patients who underwent breast reconstruction using silicone or tissue expander implants from January 2014 to December 2016, in association with a cross-sectional study of the Breast-Q® questionnaire and an evaluation of aesthetic results based on photographic analysis before and after surgery.
Results: We selected 74 patients who underwent breast reconstruction with implants (79.7% with silicone prostheses and 20.3% with expanders); 95.94% of the reconstructions were immediate, and no particular laterality predominated. We obtained statistical significance in the domains of both breast satisfaction and physical well-being. Most cases were considered satisfactory by the external evaluator.
Conclusion: The patients' quality of life in the period after breast reconstruction with breast implants was superior to that in the period prior to the procedure.

RESUMO

Introdução: Em decorrência do aumento na incidência de câncer de mama, a procura pela reconstrução mamária vem crescendo, juntamente com a preocupação em relação à satisfação e à qualidade de vida das pacientes. Mastectomia pode ser vivenciada de modo traumático, sendo considerada mutilação, afetando autoestima e estabilidade emocional. O questionário BREAST-Q® foi validado internacionalmente e formulado para avaliação pré e pós-operatória da qualidade de vida relacionada à reconstrução mamária. O objetivo do estudo é avaliar a qualidade de vida e satisfação com o resultado estético das pacientes submetidas à reconstrução mamária com implantes, comparando o período anterior com o período posterior à reconstrução mamária.
Métodos: Realizado estudo observacional longitudinal retrospectivo por meio da revisão de prontuários de pacientes submetidas à reconstrução mamária com uso de implantes de silicone ou de expansor de tecido no período de janeiro de 2014 a dezembro de 2016, associado a estudo transversal por meio da aplicação do questionário Breast-Q® e avaliação do resultado estético após análise fotográfica pré e pós-operatória.
Resultados: Foram selecionadas 74 pacientes que foram submetidas à reconstrução mamária com implantes (79,7% com prótese de silicone e 20,3% com expansor); 95,94% das reconstruções foram imediatas e não houve predomínio quanto à lateralidade. Obtivemos significância estatística tanto no domínio satisfação com a mama quanto no domínio bem-estar físico. A maioria dos casos foram considerados satisfatórios pelo avaliador externo.
Conclusão: A qualidade de vida das pacientes no período posterior à reconstrução mamária com implantes mamários é superior em relação ao período anterior ao procedimento cirúrgico.

Mammoplasty/mastopexy using implants: the Lockpocket technique

ABSTRACT

INTRODUCTION: Mammoplasty is the most commonly performed plastic surgery procedure in Brazil, which is second only to the United States in terms of the number of mammoplasties performed. Several techniques based on increasing volume using breast implants are used to correct breast ptosis. We aimed to describe a technical variation of implant mammoplasty: isolation of the implant in a closed pocket of fascioglandular tissue (Lockpocket). METHODS: This prospective study was carried out between June 2013 and June 2016. Forty-three patients underwent (1) augmentation mammoplasty using a silicone implant that was isolated from the external environment by a closed pocket of fascioglandular tissue, and (2) resection of excess dermoglandular tissue to correct mammary ptosis. Were subsequently analyzed statistically. RESULTS: Of the 43 patients, the majority (22 patients) presented grade II ptosis according to the Regnault classification. The volume of glandular tissue removed was similar to the volume of the prosthesis introduced. A total of seven complications were observed: partial dehiscence (n = 4), discrete asymmetry (n = 2), and residual ptosis (n = 1). CONCLUSION: Augmentation mammoplasty with correction of mammary ptosis (the Lockpocket technique) is a good option because it allows the surgeon to choose in advance the volume of the implant to be used. The method also isolates the implant in a totally closed plane of fascioglandular tissue, and involves the precise removal of breast tissue.

RESUMO

INTRODUÇÃO: A mamoplastia foi o procedimento em cirurgia plástica mais realizado no Brasil, ficando em segundo lugar no mundo, logo atrás dos Estados Unidos. Existem diversas técnicas para correção de ptose das mamas associado a aumento do volume com implantes mamários. O objetivo é descrever uma variação técnica de mamoplastia com prótese, isolando o implante em um bolsão fechado de tecido fascioglandular (Lockpocket). MÉTODOS: Realizado um estudo prospectivo entre junho de 2013 e junho de 2016 totalizando 43 pacientes que foram submetidos à mamoplastia de aumento com prótese de silicone, isolado do meio externo por um bolsão fechado de tecido fascioglandular, e ressecção de tecido dermoglandular excedente para correção de ptose mamária, sendo realizada análise estatística. RESULTADOS: Das 43 pacientes, a maioria (22 pacientes) apresentaram ptose grau II, segundo a classificação de Regnault. Os volumes de tecido glandular retirado e volume das prótese introduzidos foram semelhantes, sendo observado um total de sete complicações: deiscência parcial (n = 4), assimetria discreta (n = 2) e ptose residual em um caso. CONCLUSÃO: A mamoplastia de aumento com correção de ptose mamária - técnica Lockpocket - é uma boa opção, permitindo a escolha prévia do volume do implante utilizado, isolando-o em uma loja totalmente fechada de tecido fascioglandular, e exérese exata de tecido mamário.

Umbilicoplasty by vertical incision: description of the technique and assessment of satisfaction

ABSTRACT

INTRODUCTION: The abdomen is an important aesthetic functional segment in the characterization of the body contour. In abdominoplasty, the placement and shape of the umbilical scar are important for a satisfactory result. This study aimed to describe the technique of vertical omphaloplasty and to present the results obtained. METHODS: Retrospective longitudinal descriptive study composed of patients who underwent vertical omphaloplasty during classic abdominoplasty between 2014 and 2017. The technique consists of a vertical transcutaneous incision without removal of the skin spindle and with minimal degreasing of the wall around the umbilicus, followed by the transfixing a point in the lateral portion of the umbilical pedicle, allowing its invagination and fixation close to the aponeurosis of the rectus abdominis. RESULTS: In the 128 patients evaluated, the patients' age ranged from 25 to 62 years, with a mean of 32 years. The mean time of surgery was 2 h and 33 min. No surgical complications occurred, such as hematoma or abdominal flap necrosis. Two cases of dehiscence, four cases of hypertrophic scar, two cases of stenosis, and one case of umbilical necrosis were observed. In outpatient follow-up, 92.2% of the patients were very satisfied with the final result. The technical analysis presented an 88% degree of satisfaction. CONCLUSION: The technique described presents satisfactory results, reduces the possibility of the appearance of a cicatricial ring, and allows minimizing one of the stigmas of abdominoplasty, which is a very visible umbilical scar.

RESUMO

INTRODUÇÃO: O abdome é um segmento estético-funcional importante na caracterização do contorno corporal. Na abdominoplastia, o posicionamento e o formato da cicatriz umbilical são importantes para um resultado satisfatório. Este estudo teve como objetivo descrever a técnica de onfaloplastia vertical, bem como apresentar os resultados obtidos. MÉTODOS: Estudo longitudinal retrospectivo descritivo composto por pacientes submetidos à onfaloplastia vertical durante a abdominoplastia clássica no período de 2014 a 2017. A técnica consistiu em uma incisão linear vertical transcutânea sem retirada de fuso de pele e com mínimo desengorduramento da parede ao redor do umbigo, seguida da confecção de um ponto na porção lateral do pedículo umbilical, transfixante, permitindo sua invaginação e fixação rente à aponeurose do reto abdominal. RESULTADOS: Do total de 128 pacientes avaliados, a faixa etária variou de 25 a 62 anos, com média de 32 anos. O tempo médio do ato operatório foi de 2 horas e 33 minutos. Não foram observadas complicações cirúrgicas como hematoma ou necrose do retalho abdominal. Foram observados 2 casos de deiscências, 4 casos de cicatriz hipertrófica, com 2 evoluindo para estenose umbilical e 1 caso de necrose umbilical. No seguimento ambulatorial 92,2% dos pacientes mostraram-se muito satisfeitos com o resultado final obtido. A análise técnica apresentou um grau de satisfação de 88,8% dos casos. CONCLUSÃO: A técnica descrita apresenta resultados satisfatórios, diminui a possibilidade do surgimento do anel cicatricial e permite minimizar um dos estigmas da abdominoplastia, que é uma cicatriz umbilical muito visível.

Alar cartilage flap for projection of the nasal tip

ABSTRACT

Introduction: The nasal tip is a frequent object of study by surgeons, since it is a common reason for pre- and postoperative complaints. Thus, the tip should be thoroughly evaluated to determine the best technique to be used. The objective is to demonstrate an original alar cartilage flap technique for projection of the nasal tip, performed alone or in combination with other procedures for the same purpose.
Method: A retrospective cross-sectional study of 22 patients submitted to open rhinoplasty with use of the alar flap to project the nasal tip.
Results: A total of 22 open rhinoplasties were performed on primary bulbous, fatty, and under-projected tips. Among the patients in the study, 90% were black and 2% were white; 72.7% reported a high degree of satisfaction with the operation, 18.1% considered the result very good, and 9.2% considered the result acceptable. There were no complications/reoperations.
Conclusion: The alar cartilage flap for projection of the nasal tip is simple to perform and can be considered an excellent supplemental method for use in treatment of the black nose.
