ABSTRACT

"Turban tumor" corresponds to multiple cylindromas in almost the entire extension of the scalp. The disease is benign, autosomal dominant, and has a controversial histogenesis. A case of a 51 years old man is presented, in which some tumors were initially resected. Then, a large excision of the scalp was acomplished, followed by graft with skin from the thigh. Thirteen years later, the patient is in good conditions, with a satisfactory aesthetic result.

RESUMO

O "tumor em turbante" corresponde a múltiplos cilindromas em praticamente toda a extensão do couro cabeludo. A doença é benigna, autossômica dominante e tem histogênese controvertida. Descreve-se um caso em homem de 51 anos, do qual, de início, se retiraram alguns tumores; depois se fez exérese ampla de couro cabeludo, seguida de enxerto de pele de coxa. Treze anos depois, o paciente se acha em boas condições, com resultado estético satisfatório.

ABSTRACT

The authors report on a case of a patient who, after the injection of industrial silicon for breast enlargement by non-healthcare professionals, was consulted in the Plastic Surgery Outpatients' Clinic with algia and carcinophobia. The patient presented with multiple siliconomas and was submitted to a subcutaneous mastectomy and implantation of silicon prosthesis in the same operation. The anatomopathological examination demonstrated lobular carcinoma of the right breast as well as findings compatible with siliconomas. Four months after surgery, this patient presented with enlarged axillary nodules and the presence of metastatic lymph nodes of another tumor in the counter-lateral axillary region, thus synchronic tumors. Approximately one year after, the patient evolved with probable retro-ocular and hepatic metastasis and today is undergoing chemotherapy. There are works that show the relationship between the injection of silicon and the formation of siliconomas and the evolution to breast cancer, although this is still a controversial subject. However, the seriousness of such cases is not discussed due to the difficult and late diagnosis as in this case.

RESUMO

Relatamos o caso de uma paciente que, após injeção de silicone industrial por leigos, para aumentar o volume das mamas, procurou o ambulatório de Cirurgia Plástica com quadro de algia e carcinofobia. Apresentava múltiplos siliconomas. Foi submetida a mastectomia subcutânea e implante de próteses de silicone no mesmo tempo cirúrgico. O exame anatomopatológico revelou carcinoma lobular na mama direita, além de achado anatomopatológico compatível com siliconomas. Essa paciente, quatro meses após a cirurgia, apresentou nódulos axilares aumentados e presença de linfonodo metastático de outro tumor na axila contralateral, portanto tumores sincrônicos de mama. Aproximadamente um ano após, evoluiu com prováveis metástases retroculares e hepáticas e, atualmente, encontra-se em tratamento oncológico quimioterápico. Existem trabalhos que mostram a relação entre a injeção de silicone e formação de "siliconomas" e evolução com câncer de mama, sendo ainda assunto controverso; no entanto, não se discute a gravidade dos casos, pelo diagnóstico difícil e tardio, como nesse caso.
