ABSTRACT

The aim of this study was to evaluate the characteristics of a group of patients suffering from congenital cleft palate, its treatment and the incidence of oronasal fistulas. A retrospective analysis of 98 patients operated on by six plastic surgeons from March 2002 to February 2003 is included. Fifty-one male and 47 female patients with a median age of 34 months were submitted to the surgery. The mean follow-up was 10 months. The length of the cleft was described using the classification by Spina and Veau. The variables observed were age, gender, surgical technique, surgeon and complications. For statistic analysis the Chi-square test of association was utilized. The most common approaches were the Bardach and Veau techniques. Complications occurred in 20.4% of the cases with oronasal fistulas occurring in 15.3% of the palatoplasties. This complication rate gave a significant difference when compared with the length of the cleft (p=0.006). No significant difference was seen in respect to the other variations considered. Our rate of fistulas is similar to that reported in the literature and shows that the different techniques utilized are acceptable in the treatment of cleft palate.

RESUMO

O objetivo deste estudo foi avaliar as características de um grupo de pacientes com fissura palatina congênita, seu tratamento e a incidência de fístulas oronasais. Inclui uma análise retrospectiva de 98 pacientes, operados por seis cirurgiões plásticos, no período de março/2002 a fevereiro/2003. Foram 51 pacientes do sexo masculino e 47 do feminino, idade mediana de 34 meses e acompanhamento médio de 10 meses. A extensão da fissura foi descrita pela classificação de Spina e Veau. As variáveis observadas foram: idade, sexo, técnica cirúrgica, cirurgião e complicações. Na análise estatística utilizou-se o teste Qui-quadrado de associação. As técnicas mais utilizadas foram a de Bardach e a de Veau. As complicações incidiram em 20,4% dos pacientes, sendo que a fístula oronasal ocorreu em 15,3% das palatoplastias. Esta taxa de complicação obteve diferença significativa quando relacionada à extensão da fissura (p=0,006); não havendo diferenças quando relacionada a outras variáveis. Nossa incidência de fístula encontra-se equiparada à literatura e mostra que as diversas técnicas utilizadas são válidas no tratamento da fenda palatina.
