Tarsoconjunctival Flap in Total Plane Oncology Reconstructions of the Eyelids

ABSTRACT

The authors present the results obtained with the tarsoconjunctival flap technique in eyelid reconstructions of total plane defects and affecting more than 50% of the horizontal length of the lower eyelid. They describe how the method was used in 40 cases and discuss major aspects in the evolution of the procedure. They conclude by defending the utilization of the method as the technique of choice in complex lid lesions due to its satisfactory aesthetic and functional results.

RESUMO

Os autores apresentam os resultados obtidos com a utilização da técnica do retalho tarsoconjuntival nas reconstruções palpebrais em defeitos de plano total e de mais de 50% do comprimento horizontal das pálpebras inferiores. Descrevem a aplicação do método em 40 casos e discutem aspectos importantes na evolução deste procedimento. Concluem defendendo o emprego desse recurso em lesões complexas palpebrais como técnica de escolha devido aos resultados satisfatórios estéticos e funcionais.

Comparison of surgical techniques for prominent ear correction: Mustardé versus Converse

ABSTRACT

Introduction: prominent ears, popularly called "flappy ears," represent the most common congenital deformity of the external ear, affecting approximately 5% of the population.
Methods: Primary, prospective and intervention study comparing the results of patients undergoing the surgical procedure to correct prominent ears using the Converse and the Mustardé techniques, performed at the Plastic Surgery Service of the Hospital das Clínicas, Federal University of Pernambuco (HC) -UFPE).
Results: Twenty patients were evaluated, 10 with the Converse technique, and 10 with the Mustardé technique, from June 2016 to December 2017. Both groups showed a decrease in auricular mastoid distances at the end of the observation period, ranging from 6.67 to 14.6 mm, depending on the surgical technique and the evaluation point, but without statistical significance. Regarding the average auricular mastoid distances at the end of the observation period, a difference of a maximum of 6.3 mm was observed between the evaluated groups, but without statistical significance. Regarding the symmetry of the ears within the same group, the maximum mean level of asymmetry in the Mustardé and Converse groups was 0.9 mm and 0.5 mm, respectively. However, the percentage of loss of correction of the measures obtained surgically during the observation period in both groups ranged between 15 and 19%, without statistical significance. Regarding complications, there was 1 (10%) case of hematoma in the Mustardé group.
Conclusion: Converse and Mustardé techniques did not show statistical differences in the results.

RESUMO

Introdução: As orelhas proeminentes, popularmente chamadas de "orelhas em abano", representam a deformidade congênita mais comum da orelha externa, atingindo cerca de 5% da população.
Métodos: Estudo primário, prospectivo e de intervenção comparando os resultados de pacientes submetidos ao procedimento cirúrgico de correção de orelhas proeminentes por meio da técnica de Converse e de Mustardé, realizado no Serviço de Cirurgia Plástica do Hospital das Clínicas da Universidade Federal de Pernambuco (HC-UFPE).
Resultados: Foram avaliados 20 pacientes, 10 por meio da técnica de Converse e 10 por meio de Mustardé, no período de junho de 2016 a dezembro de 2017. Ambos os grupos mostraram diminuição das distâncias mastoideas auriculares ao final do período de observação, variando de 6.67 a 14.6 mm, a depender da técnica cirúrgica e do ponto de avaliação, mas sem significância estatística. Quanto às distâncias mastoideas auriculares médias ao final do período de observação, observou-se diferença de no máximo 6.3mm entre os grupos avaliados, mas sem significância estatística. Em relação a simetria das orelhas dentro do mesmo grupo, o nível máximo de assimetria média nos grupos Mustardé e Converse foi de respectivamente 0.9mm e 0.5mm. A porcentagem da perda de correção das medidas obtidas cirurgicamente ao longo do período de observação em ambos os grupos variaram de 15-19%, no entanto, sem significância estatística. No que se refere as complicações, houve 1 (10%) caso de hematoma no grupo Mustardé.
Conclusão: As técnicas de Converse e Mustardé demostraram não ter diferença estatística nos resultados.

Mandibular reconstruction with fibula free flap: case series

ABSTRACT

Introduction: Mandibular reconstruction is a complex procedure aimed at correcting defects of the lower third of the face and achieving functional rehabilitation, including chewing and oral competence. Fibula free flap is the first choice for the reconstruction of segment defects of the adjacent mandible and soft tissue.
Methods: A retrospective clinical study was conducted from January 2005 to July 2017, analyzing the medical records of patients undergoing microsurgical reconstructions after resection of head and neck neoplasms at the plastic surgery service of the Clinical Hospital of the Federal University of Pernambuco (HC-UFPE).
Results: This study included six patients, of which three were men (50%), aged between 12 and 48 years and with a mean age of 24 years. In 83% of the cases, reconstructions were performed with osteomyocutaneous fibula free flaps (in one case, there was no need for skin island flap). We observed an adequate coverage of the large defects analyzed, with good functional and aesthetic appearance in all cases. Immediate reconstruction was performed in 83% of cases. The fibula and receptive area were prototyped in two cases.
Conclusion: Fibula free flaps are a great alternative for head and neck reconstruction. Our initial experience and literature show satisfactory results, partially restoring the shape and function of the affected areas.

RESUMO

Introdução: A reconstrução da mandíbula é um procedimento complexo, que visa a correção das deformidades do terço inferior da face e reabilitação funcional, incluindo mastigação e competência oral. O retalho livre de fíbula é a primeira escolha para a reconstrução de falhas segmentares da mandíbula e tecido mole adjacentes.
Métodos: Foi realizado um estudo clínico retrospectivo, no período de janeiro de 2005 a julho de 2017, analisando os prontuários dos pacientes submetidos a reconstruções microcirúrgicas após a ressecção de neoplasias em cabeça e pescoço, operados pelo serviço de cirurgia plástica do Hospital das Clínicas da Universidade Federal de Pernambuco (HC-UFPE).
Resultados: Seis pacientes foram incluídos no estudo, sendo três do sexo masculino (50%), a idade variou de 12 a 48 anos, com média de 24 anos. Em 83% dos casos foram realizadas reconstruções com retalhos livres de fíbula osteomiocutâneos (em um dos casos não houve necessidade de confeccionar ilha de pele junto ao retalho). Observou-se uma cobertura eficaz dos extensos defeitos estudados, apresentando em todos os casos bons resultados quanto ao aspecto funcional e estético. Em 83% dos casos reconstrução imediata foi realizada. A prototipagem da fíbula e da área receptora foi realizada em dois casos.
Conclusão: Retalhos livres de fíbula constituem uma ótima alternativa para reconstrução em cabeça e pescoço. Nossa experiência inicial e a literatura mostram resultados satisfatórios, restaurando parcialmente forma e função das áreas acometidos.

Comparative analysis of mastectomies and breast reconstructions performed in the Brazilian Unified Health System in the last 5 years

ABSTRACT

Introduction: Breast cancer is the second most common malignant neoplasm in women in Brazil. A great challenge for health professionals and to reconcile the waiting line for both oncological and reconstructive surgeries. The objective is to evaluate the last 5 years of breast cancer surgeries compared to the number of reconstructive surgeries performed in the same period.
Methods: This is a descriptive study with a transversal and retrospective approach on mastectomies, segmentectomies and breast reconstructions performed at SUS, between the years 2015 and 2020. According to the procedures and codes chosen and tabulated in the Microsoft Excel 365 software, the data were collected in the SUS data transfer service.
Results: 204,569 breast cancer surgeries were performed, with 57% segmentectomies/quadrantectomies and 43% mastectomies. In the same period, 17,927 reconstructive plastic breast surgeries were performed with implants after mastectomy, with only 20.52% of mastectomized women undergoing immediate reconstruction with implants.
Conclusion: The number of reconstructive breast surgeries in Brazil is below the ideal level, leaving most women mastectomized with sequelae for a long time.

RESUMO

Introdução: O câncer de mama é a segunda neoplasia maligna mais comum em mulheres no Brasil. O grande desafio para os profissionais de saúde é conciliar a fila de espera tanto para as cirurgias oncológicas quanto para as reconstrutivas. O objetivo é avaliar o cenário dos últimos 5 anos das cirurgias de câncer de mama em comparação ao número de cirurgias reconstrutivas realizadas no mesmo período.
Métodos: Trata-se de um estudo descritivo com abordagem transversal e retrospectiva sobre mastectomias, segmentectomias e reconstruções mamárias realizadas no SUS, entre os anos de 2015 e 2020. Os dados foram coletados no serviço de transferência de dados do SUS, segundo os procedimentos e códigos escolhidos e tabulados no software Microsoft Excel 365.
Resultados: Foram realizadas 204.569 cirurgias de câncer de mama, sendo 57% segmentectomias/quadrantectomias e 43% mastectomias. No mesmo período, foram realizadas 17.927 cirurgias plásticas reconstrutivas de mama com implantes após mastectomia, sendo que apenas 20,52% das mulheres mastectomizadas foram submetidas à reconstrução imediata com implantes.
Conclusão: O número de cirurgias reconstrutivas de mama no Brasil está bem abaixo do ideal, deixando a maioria das mulheres mastectomizadas com sequelas por um longo período de tempo.

Innervation of the nipple-areolar complex after reduction mammaplasty: a histological study

ABSTRACT

INTRODUCTION: The periareolar dermal release maneuver in mammoplasty promotes better mobility of the nipple-areola complex. However, there are doubts on possible nerve damages in this kind of topography. This quantitative analysis compared the nerve branches density from the medial, lateral and caudal side-flow of the nipple-areola complex (NAC). METHODS: This was a prospective study. The study included 26 women who have undergone a mammaplasty reduction using the Pitanguy's classic technique. The dermis fragments collected from the medial, lateral and caudal sides were properly prepared and subjected to a histological study in order to determine the nerve branches density in each studied sides. RESULTS: Of 26 studied patients, 42.3% had a higher nerve branches density in the lateral side; 38.5%, on the medial side and 19.2% on the caudal side. The statistical analysis used to evaluate whether there was a predominance of one side where the dermis has been sectioned showed that the proportion comparison test was not significant (p = 0.304). CONCLUSION: The comparative analysis has shown that there is no preponderance of nerve density in any periareolar dermis side.

RESUMO

INTRODUÇÃO: A manobra de liberação dérmica periareolar na mamoplastia promove uma melhor mobilidade do complexo mamilo-aréola (NAC). No entanto, existem dúvidas sobre possíveis danos nos nervos neste tipo de topografia. Este estudo objetiva uma análise quantitativa comparando a densidade dos nervos do fluxo lateral medial, lateral e caudal do NAC. MÉTODOS: Trata-se de um estudo prospectivo. O estudo incluiu 26 pacientes do sexo feminino que foram submetidas à mamoplastia redutora pela técnica clássica de Pitanguy. Os fragmentos da derme coletados dos lados medial, lateral e caudal foram devidamente preparados e submetidos a um estudo histológico para determinar a densidade dos nervos em cada um dos lados estudados. RESULTADOS: Dos 26 pacientes estudados, 42,3% apresentaram maior densidade de nervos no lado lateral; 38,5%, do lado medial e 19,2% do lado caudal. A análise estatística utilizada para avaliar se houve predominância de um lado onde a derme foi seccionada demonstrou que o teste de comparação de proporções não foi significativo (p = 0,304). CONCLUSÃO: A análise comparativa mostrou que não há preponderância de densidade de nervos em qualquer lado da derme periareolar.

Reconstruction of the abdominal wall: a case series

ABSTRACT

INTRODUCTION: Patients with abdominal wall defects present challenging complications that require the use of advanced surgical approaches. METHODS: This primary, retrospective, and descriptive study evaluated patients who underwent abdominal wall reconstruction at the Plastic Surgery Service of the Clinics Hospital of the Federal University of Pernambuco. RESULTS: The medical records of 18 patients were reviewed, including 15 women (83.3%) and 3 men (16.7%), with a mean age of 41 years (range, 16-79 years). Seventeen patients (94.4%) had a history of previous surgery. The causes of abdominal injury were cesarean section in eight cases (44.4%), oncologic surgery in six (33.3%), trauma surgery in two (11.1%), and bariatric surgery in two (11.1%). The etiology of the defect was necrotizing fasciitis in eight cases (44.4%), incisional hernia in four (22.2%), trauma in two (11.1%), surgical wound dehiscence in two (11.1%), abdominal wall neoplasia in two (11.1%), and total thickness defect in one (5.5%). The surgical interventions included the component separation technique in seven cases (38.9%), simple VY advancement flap in six (33.3%), closure with abdominoplasty in three (16.7%), and tissue expander in two (11.1%). Four patients (22.2%) presented complications. CONCLUSIONS: Abdominal wall defects are challenging cases for plastic surgeons, as their treatment is difficult, but the results are satisfactory even in the most severe cases.

RESUMO

INTRODUÇÃO: Os pacientes com defeitos de parede abdominal chegam ao consultório do cirurgião plástico em situações muitas vezes complexas, necessitando de abordagem cirúrgica avançada. MÉTODOS: Estudo primário, retrospectivo e descritivo de pacientes submetidos a procedimentos cirúrgicos de reconstrução de parede abdominal pelo Serviço de Cirurgia Plástica do Hospital das Clínicas da Universidade Federal de Pernambuco (HC-UFPE). RESULTADOS: Foram revisados e incluídos os prontuários de 18 pacientes, 15 (83,3%) do sexo feminino e 3 (16,7%) do masculino, com idade variando de 16 a 79 anos (média de 41 anos). Dezessete pacientes possuíam histórico de cirurgia prévia (94,4%), sendo a cesárea presente em 8 dos casos (44,4%), seguida de cirurgia oncológica com 6 (33,3%), cirurgia do trauma com 2 (11,1%) e bariátrica com 2 (11,1%). Em relação à etiologia do defeito, 8 (44,4%) eram decorrentes de fasciite necrosante, 4 (22,2%) de hérnia incisional, 2 (11,1%) por trauma, 2 (11,1%) por infecção de ferida operatória e 2 (11,1%) por neoplasia de parede abdominal, sendo somente um (5,5%) paciente com defeito de espessura total. A técnica cirúrgica de separação dos componentes foi realizada em 7 dos casos (38,9%), seguida de retalho de avanço simples em 6 (33,3%), fechamento com tela associado à abdominoplastia em 3 (16,7%), e expansor tecidual em 2 (11,1%). Quanto às complicações, houve 4 casos (22,2%). CONCLUSÕES: Defeitos de parede abdominal são casos desafiadores para o cirurgião plástico, seu tratamento se mostra árduo, porém com resultados satisfatórios mesmo nos casos mais severos.

Lip reconstruction: functional and aesthetical approach after tumor resection

ABSTRACT

The authors present a functional and aesthetic evaluation of patients submitted to lip tumor surgery from June 1993 to June 1998 in the reconstructive and microsurgical plastic surgery service of the National Institute of Cancer - INCa-MS (RJ). Forty-eight patients submitted to resections of primary lip tumors who underwent immediate repair were retrospectively analysed. The characteristics of the tumors and the techniques used for resection and reconstruction of the resulting lip defects are detailed and indications according to the length of the lesion were detailed with the advantages of techniques that allow restoration of the mechanism of the oral sphincter being emphasized. Evaluations of the functional and aesthetic results of the reconstructions according to established criteria as assessed by the senior author were excellent/good in 81.2% and 85.4% respectively.

RESUMO

Os autores apresentam avaliação funcional e estética dos pacientes operados por tumores de lábio, no período de junho de 1993 a junho de 1998, no Serviço de Cirurgia Plástica Reconstrutora e Microcirurgia no Instituto Nacional de Câncer - INCa-MS (RJ). Foram analisados, retrospectivamente, 48 pacientes submetidos a ressecções de tumores labiais primários e reparação imediata. As características dos tumores e as técnicas empregadas para a ressecção e reconstrução dos defeitos labiais resultantes são detalhadas, ressaltando-se as indicações de acordo com a extensão da lesão e enfatizando-se a superioridade das técnicas que permitem a restauração do mecanismo esfincteriano oral. A avaliação dos resultados funcionais e estéticos das reconstruções, de acordo com os critérios estabelecidos, por meio da avaliação pelo autor senior, se mostrou ótima/boa em 81,2% e 85,4% dos pacientes, respectivamente.

Neoomphaloplasty with cutaneous graft

ABSTRACT

INTRODUCTION: The umbilical scar is due to the fall of the umbilical stump that occurs a few days after birth. Its presence, shape, and location on the abdominal wall provide the individual with an aesthetic and sensual connotation. METHODS: A primary prospective interventional study. The sample was of convenience, from February 2006 to June 2016, and included patients of both sexes attending the outpatient clinic of the Hospital das Clínicas of the Federal University of Pernambuco (HC-UFPE), a private clinic . The inclusion criteria were patients with abdominoplasty indications presenting with compromised circulation to the skin of the umbilical and periumbilical region caused by hernia defects in this area. The study followed the criteria of Helsinki and the patients signed an Informed Consent Form. RESULTS: Twenty-eight patients underwent surgery and good integration of the grafted skin was observed. This resulted in an umbilical scar with a natural appearance and without complications. CONCLUSIONS: Neoomphaloplasty with a cutaneous graft is easy to perform and, in the long term, has shown to provide good aesthetic results, especially in thick abdominal flaps, thus proving to be an additional technical option for neoomphaloplasty procedures.

RESUMO

INTRODUÇÃO: A cicatriz umbilical é decorrente da queda do coto umbilical, que ocorre alguns dias após o nascimento. Sua presença, formato e localização na parede abdominal fornecem ao indivíduo uma conotação estética e sensual. MÉTODOS: Estudo primário, prospectivo, de intervenção. A casuística foi de conveniência, no período de fevereiro de 2006 a junho de 2016, incluindo pacientes de ambos os gêneros, alocados do ambulatório do Hospital das Clínicas da Universidade Federal de Pernambuco (HC-UFPE) e clínica privada. Os critérios de inclusão foram pacientes com indicação de abdominoplastia e que apresentavam um comprometimento da irrigação da pele da região umbilical e periumbilical devido a defeitos herniários da região. O estudo seguiu os critérios de Helsinki e os pacientes assinaram o Termo de Consentimento Livre e Esclarecido. RESULTADOS: Foram operados 28 pacientes, sendo observada uma boa integração da pele enxertada, resultando em uma cicatriz umbilical de aparência natural e sem complicações. CONCLUSÕES: A neo-onfaloplastia com enxerto cutâneo é de fácil execução e que, a longo prazo, tem demonstrado um bom resultado estético, principalmente nos retalhos abdominais espessos, demonstrando ser mais uma opção técnica na realização de neo-onfaloplastias.

Craniofacial reconstructions: is there still room for locoregional flaps?

ABSTRACT

INTRODUCTION: Advanced tumors of the head and neck still have a high prevalence in Brazil. Reconstructing a defect resulting from a craniofacial resection is a challenge for the plastic surgeon. Free flaps are the first choice for these reconstructions, and locoregional flaps have been used in selected cases. The objective of this study was to evaluate a number of reconstructions with locoregional flaps in patients undergoing craniofacial oncologic resection, to demonstrate the applicability of these flaps and the end result of the reconstructions. METHODS: We retrospectively studied four craniofacial reconstructions with locoregional flaps on the forehead and scalp. The patients were operated at the Clinical Hospital of the Federal University of Pernambuco. They signed a consent form allowing the scientific publication of their photographs and clinical case records. RESULTS: All reconstructions were successful. There were no cases of local infection or meningitis, flap necrosis, liquor fistula, or dehiscence. Reconstructions with locoregional flaps were safe and simple to perform for extensive defects in the craniofacial region. CONCLUSIONS: Plastic surgeons can perform complex reconstructions with locoregional flaps, demonstrating that there is still room for this type of reconstruction.

INTRODUÇÃO: Os tumores avançados de cabeça e pescoço ainda têm elevada prevalência no Brasil. A reconstrução de um defeito resultante de ressecção craniofacial é um desafio para o cirurgião plástico. Os retalhos livres são a primeira escolha para essas reconstruções e os retalhos locorregionais têm sido utilizados em casos selecionados. O objetivo deste estudo é avaliar uma série de reconstruções com retalhos locorregionais em pacientes submetidos a ressecções oncológicas craniofaciais, demonstrando a aplicabilidade desses retalhos e o resultado final das reconstruções. MÉTODOS: Foram avaliadas, retrospectivamente, quatro reconstruções craniofaciais com retalhos locorregionais de fronte e couro cabeludo. Os pacientes foram operados no Hospital das Clínicas da Universidade Federal de Pernambuco (HC-UFPE) e assinaram termo de consentimento permitindo a publicação científica de suas fotos e casos clínicos. RESULTADOS: Todas as reconstruções foram bem sucedidas. Não ocorreram casos de infecção local ou meningite, necrose do retalho, fístulas liquóricas ou deiscências. As reconstruções com retalhos locorregionais apresentadas foram seguras e simples para reconstruir defeitos extensos em região craniofacial. CONCLUSÕES: Os cirurgiões plásticos podem realizar reconstruções complexas com estes retalhos, demonstrando que ainda há espaço para este tipo de reconstrução.

Festoons, edema, and malar bags: is there a consensus on aesthetic treatment?

ABSTRACT

Introduction: Periorbital changes are some of the first detectable signs of aging. The most outstanding currently, refers to the rejuvenation of this region, involving the treatment from the eyebrows to the transition orbital-malar, where are festoons, edemas, and malar bags. However, this management is complex, involving several approaches: invasive or noninvasive techniques. Thus, this review aims to describe the scientific evidence of the most current techniques used in the treatment of festoons, edema, and malar bags and to evaluate the complications related to each modality.
Methods: The research was carried out in three databases, PubMed, Cochrane, and LILACS - using the descriptors "bolsa malar," "malar mounds," "festoons" and "malar bags" in the period from 2014 to 2019, in English and Portuguese.
Results: We selected 13 articles; most of the studies were retrospective reviews (76.9%), seven dealt with noninvasive techniques, three about invasive, and three on the association of techniques. Regarding the procedures described, the noninvasive ones were represented by the use of Kinesio tape, tetracycline injection, doxycycline and hyaluronic acid, and the use of microneedling with radiofrequency. The invasive ones were represented by microaspiration, myocutaneous flap, subperiosteal lift of the middle face, and direct excision.
Conclusion: There are numerous techniques for treating festoon and malar bags, but it is up to the plastic surgeon to know its advantages and disadvantages to decide the most appropriate in each situation. Thus, there is no consensus, but it is vital to diagnose correctly to indicate the best treatment.

RESUMO

Introdução: As alterações periorbitais são alguns dos primeiros sinais detectáveis do envelhecimento. O destaque, atualmente, refere-se ao rejuvenescimento dessa região, envolvendo o tratamento desde as sobrancelhas até a transição órbito-malar, onde se encontram festoons, edemas e bolsas malares. Entretanto, este manejo é complexo, envolvendo várias abordagens: técnicas invasivas ou não invasivas. Assim, esta revisão objetiva descrever as evidências científicas relacionadas às técnicas mais atuais utilizadas no tratamento de festoons, edemas e bolsas malares e avaliar as complicações relacionadas à cada modalidade.
Métodos: A pesquisa foi realizada em três bases de dados - PubMed, Cochrane e LILACS - utilizando os descritores "bolsa malar", "malar mounds", "festoons" e "malar bags" no período de 2014 a 2019, na língua inglesa e portuguesa.
Resultados: Foram selecionados 13 artigos, a maioria dos estudos eram revisões retrospectivas (76,9%), sete versavam sobre técnicas não invasivas, 3 sobre invasivas e 3 sobre associação das técnicas. Em relação aos procedimentos descritos, os não invasivos foram representados pelo uso de Kinesio tape, injeção de tetraciclina, doxiciclina e de ácido hialurônico, e o uso de microagulhamento com radiofrequência. Já os invasivos foram representados por microaspiração, retalho miocutâneo, lift subperiosteal da face média e excisão direta.
Conclusão: Existem inúmeras técnicas para tratamento de festoon e bolsas malares, mas cabe ao cirurgião plástico conhecer suas vantagens e desvantagens para decidir a mais adequada em cada situação. Assim, não há consenso, mas é vital diagnosticar corretamente para indicar o melhor tratamento.

Reconstruction of complex lesions of the lower limbs with reverse fasciocutaneous flaps

ABSTRACT

INTRODUCTION: Complex lesions on the lower limbs have always been challenging because of their difficult resolution. Microsurgical free flaps have been an excellent surgical option; however, they have limited use because there are an insufficient number of clinicians trained in such structured services. Fasciocutaneous flaps, specifically reverse flow, have been shown to be an excellent option in treating lower limb injuries as an alternative to complex microsurgical free flaps. METHODS: From January 2012 to June 2015, nine patients underwent reconstruction of the lower limbs with reverse fasciocutaneous flaps. All had extensive lesions on these limbs. RESULTS: In eight patients, the results were satisfactory, and there were no major complications. In one patient, there was total necrosis of the flap. CONCLUSION: Because of the relative technical simplicity of performing such a technique, fasciocutaneous flaps are a viable option for the reconstruction of complex wounds of the lower limbs and are an additional therapeutic option for patients with these lesions.

RESUMO

INTRODUÇÃO: Lesões extensas em membros inferiores sempre foram desafiadoras devido a sua difícil resolução. Retalhos livres microcirúrgicos se mostraram como excelente opção cirúrgica, todavia, por carecer de equipe capacitada e serviços estruturados, têm seu uso restrito. Os retalhos fasciocutâneos, especificamente os de fluxo reverso, têm se mostrado excelente opção na abordagem de lesões de membros inferiores, numa alternativa aos complexos retalhos livres microcirúrgicos. MÉTODOS: De janeiro de 2012 a junho de 2015, nove pacientes foram submetidos à reconstrução de membros inferiores com retalhos reversos fasciocutâneos. Todos eram portadores de lesões extensas nos referidos membros. RESULTADOS: Em oito pacientes o resultado foi satisfatório, sem complicações maiores. Em um paciente houve necrose total do retalho. CONCLUSÃO: Pela sua relativa simplicidade técnica, os retalhos fasciocutâneos de fluxo reverso vêm se consolidando como opção bastante viável na reconstrução de feridas complexas dos membros inferiores, sendo mais uma opção terapêutica para os pacientes portadores destas lesões.

ABSTRACT

Introduction: The aesthetic procedure market is growing exponentially in Brazil. This growth has aroused the interest of several professional categories. The decision to practice in the sector must consider the market opportunities in the location in which you intend to operate. However, the area lacks comparative analyses documenting probable regional differences in the country. The objective of the study is to describe market differences in aesthetic procedures between Brazilian states and regions. An index of potential cosmetic consumption (IPCC) is calculated for such a comparative analysis.
Method: Cross-sectional study involving providers of nonsurgical aesthetic procedures in Brazil. Searches on Google Maps® were conducted using key terms, and telephone interviews were conducted to obtain information on professional categories, types of providers, and services offered. Positive predictive values were obtained for all search strategies and used to estimate the total number of providers. Population size and per capita income were considered to calculate the IPCCs for Brazilian states.
Results: São Paulo, Minas Gerais, and Rio de Janeiro presented the highest IPCCs, being 524, 210, and 180, respectively. Roraima had an IPCC of 14, the lowest in the country. The Southeast Region presented, on average, the highest IPCC (242) among all Brazilian regions.
Conclusion: Considering population size and income, the Southeast Region presents the greatest market opportunities for nonsurgical aesthetic procedures in Brazil. Our findings may be of interest to healthcare professionals and investors who work or intend to work in the sector.

RESUMO

Introdução: O mercado de procedimento estéticos cresce exponencialmente no Brasil. Tal crescimento tem despertado o interesse de várias categorias profissionais. A decisão de praticar no setor deve considerar as oportunidades de mercado da localidade na qual se pretende atuar. Entretanto, a área carece de análises comparativas documentando prováveis diferenças regionais no país. O objetivo do estudo é descrever as diferenças de mercado em procedimentos estéticos entre os estados e regiões brasileiras. Um índice de potencial consumo de cosmiatria (IPCC) é calculado para tal análise comparativa.
Método: Estudo transversal envolvendo prestadores de procedimentos estéticos não cirúrgicos no Brasil. Buscas no Google®-Google Maps® foram conduzidas usando termoschave e entrevistas telefônicas realizadas para obter informações sobre categorias profissionais, tipo de provedores e serviços oferecidos. Valores preditivos positivos foram obtidos para todas as estratégias de busca e usados para estimar o número total de provedores. O tamanho da população e a renda per capita foram considerados para o cálculo dos IPCCs para os estados brasileiros.
Resultados: São Paulo, Minas Gerais e Rio de Janeiro apresentaram os maiores IPCCs, sendo 524, 210 e 180, respectivamente. Roraima teve um IPCC de 14, o mais baixo do país. A Região Sudeste apresentou, em média, o maior IPCC (242) entre todas as regiões brasileiras.
Conclusão: Considerando o tamanho da população e a renda, a Região Sudeste apresenta as maiores oportunidades de mercado em procedimentos estéticos não cirúrgicos no Brasil. Nossos achados podem ser de interesse para profissionais de saúde e investidores que atuam ou pretendem atuar no setor.
