Surgical treatment of axillary hidradenitis suppurativa using a parascapular flap

ABSTRACT

INTRODUCTION: Hidradenitis suppurativa (HS) is a disease with a prevalence of approximately 1% in the population and is difficult to clinically treat. Through chronification, it may lead to functional limitations, especially in the axillary region. METHODS: This study reports eight cases of axilla with chronic HS, Hurley stage III, treated with wide excision of the diseased part of the axillary region, followed by immediate reconstruction with a parascapular flap. The patients underwent surgery at the Plastic Surgery and Burns Service of the State Government Workers' Hospital of São Paulo. RESULTS: All the treated patients had no recurrence. All the parascapular flaps remained viable, and infection of one of the flaps (12.5%) was the immediate complication. Closure of the entire donor area was performed using primary synthesis. CONCLUSION: The treatment of chronic axillary HS, stage III, based on radical resection and reconstruction with a parascapular flap proved to be an effective alternative to locally control the disease and is a safe procedure to cover the axillary defect.

RESUMO

INTRODUÇÃO: A hidradenite supurativa (HS) é uma doença com prevalência em torno de 1% na população, de difícil tratamento clínico, que em sua cronificação pode levar a limitações funcionais, principalmente na região axilar. MÉTODOS: Este trabalho relata oito axilas com HS crônica estágio III de Hurley tratadas com exérese ampla da doença na região axilar e sua reconstrução imediata com o retalho paraescapular, operados no pelo Serviço de Cirurgia Plástica e Queimaduras do Hospital do Servidor Público Estadual de São Paulo. RESULTADOS: Todas as axilas tratadas não tiveram recorrência da doença (100%). Todos os retalhos paraescapulares mantiveram-se viáveis (100%), sendo a infecção de um dos retalhos (12,5%) a complicação imediata. Fechamento de 100% da área doadora em síntese primária. CONCLUSÃO: O tratamento da HS crônica axilar estágio III baseado na ressecção radical e reconstrução com retalho paraescapular mostrou-se uma opção efetiva no controle local da doença e um retalho seguro para cobertura do defeito axilar.

Nasolabial interpolation flap for nasal alar reconstruction after skin tumor resection

ABSTRACT

Introduction: The nose is a common site for skin neoplasms. Due to its functional and esthetic importance, nasal reconstruction, mainly that of the nose ala, is challenging. The objective is to describe the nasolabial interpolation flap for nasal alar reconstruction after skin tumor resection.
Methods: Patients with nonmelanoma skin tumors on the nasal ala without involvement of the alar and supra-alar sulcus underwent reconstruction with a nasolabial interpolation flap associated with conchal cartilage grafting. Details of the surgical planning and operative sequence and an analysis of the results are presented.
Resultados: In the treatment of skin tumors on the nasal ala, results from the oncological and esthetic point of view should be sought, i.e., maintenance of the three-dimensional structure and cutaneous features should be intended.
Conclusion: Use of the nasolabial interpolation flap was effective for nasal alar reconstruction despite the need for two surgeries.

RESUMO

Introdução: O nariz é sede frequente de neoplasias cutâneas. Pela importância estético-funcional, a reconstrução do nariz, em especial da asa nasal, é um desafio. O objetivo é descrever o retalho nasogeniano de interpolação na reconstrução da asa nasal após ressecção de tumores cutâneos.
Métodos: Pacientes com tumores de pele não melanoma de asa nasal, sem comprometimento dos sulcos alar ou supra-alar, foram submetidos à reconstrução com retalho nasogeniano de interpolação associado a enxerto de cartilagem conchal. Detalhes do planejamento cirúrgico e da sequência operatória, assim como a análise dos resultados, são demonstrados.
Resultados: No tratamento de tumores de pele localizados na asa nasal, deve-se buscar resultados sob o ponto de vista oncológico e estético. Assim, a preservação da estrutura tridimensional e das características cutâneas da asa nasal deve ser objetivada.
Conclusão: O retalho nasogeniano de interpolação mostrou-se eficaz na reconstrução da asa nasal, apesar da necessidade de dois tempos cirúrgicos.

Wound management of partial-thickness skin-graft donor areas with a collagen and alginate dressing (Fibracol®): findings in 35 patients

ABSTRACT

INTRODUCTION: The objective of this study was to report our findings with a collagen and alginate dressing (Fibracol®) used to cover donor areas of partial-thickness skin grafts. METHOD: We retrospectively evaluated the medical records of 35 patients in whom Fibracol® was used on donor areas. The routine used to manage the donor area is as follows: The area is covered with one or more units of Fibracol®, followed by application of a waterproof film. After three or four days, the dressing is removed and the area cleaned gently with saline and gauze; the area is dressed again if necessary. The mean patient age was 25.52 years (range, 1-65 years). RESULTS: The thigh was used as the donor area in 29 patients, the arm in 2, the leg in 4, and the trunk in 3. Two patients had more than one donor area. The mean time needed for epithelization was 4.51 days (range, 3-8 days). The Pearson correlation coefficient value correlating age and time of epithelization was -0.0755; p = 0.6685. None of the patients experienced an infection in the donor area. The ideal dressing for the donor area of split-thickness skin grafts would have multiple characteristics including low price, good patient comfort, low infection rate, and a short epithelization period. CONCLUSION: The authors report a positive experience with the use of Fibracol® in 35 patients over a period of 22 months. The mean epithelization period was 4.51 days, shorter than that in the majority of published studies, and had no correlation with the age of the patient.

RESUMO

INTRODUÇÃO: O objetivo deste estudo é relatar a nossa experiência com curativo de colágeno e alginato (Fibracol®) para cobrir áreas doadoras de enxerto de pele de espessura parcial. MÉTODO: Estudamos, retrospectivamente, 35 prontuários de pacientes que utilizaram o Fibracol® em áreas doadoras. Nossa rotina para cobrir a área doadora é a seguinte: cobertura da área com uma ou mais unidades de Fibracol® e, em seguida, com uma película à prova de água. Depois de três ou quatro dias, remover o curativo, limpar delicadamente com soro fisiológico e gaze e, quando julgava-se necessário, cobria-se novamente. A idade média foi de 25,52 anos (1-65). RESULTADOS: A coxa foi usada como área doadora em 29 pacientes, o braço em 2, a perna em 4 e tronco em 3 (2 pacientes tiveram mais de uma área doadora). O tempo médio necessário para epitelização foi de 4,51 dias (3-8). O valor de R do coeficiente de correlação de Pearson correlacionando a idade e tempo de epitelização foi -0,0755, com p = 0,6685. Nenhum dos pacientes teve infecção na área doadora. O curativo ideal para a área doadora do enxerto de pele de espessura parcial teria muitas características, incluindo: preço baixo, bom conforto do paciente, baixa taxa de infecção, período curto de tempo para epitelização, etc. CONCLUSÃO: Os autores relatam uma boa experiência usando Fibracol® em 35 pacientes, durante um período de 22 meses. O tempo para epitelização foi de 4,51 dias, mais curta do que a maioria dos trabalhos publicados, e não tinha correlação com a idade do paciente.

Comparative analysis of mammaplasty techniques based on the long-term effect on the nipple-areolar-complex to inframammary crease distance

ABSTRACT

BACKGROUND: Breast cosmetic surgery has focused on the correction of breast volume, suspension, and shape. In most of the breast reduction and mastopexy techniques currently in use, the vascular pedicles responsible for the blood supply to the nipple areolar complex (NAC) are of particular importance. The objective of the present study was to evaluate the results obtained with different techniques and pedicle choices with regard to the maintenance of lower breast pole shape during a postoperative 6-month follow-up period. METHODS: A total of 20 female patients (40 breasts) with a body mass index (BMI) ranging from 22 to 25 and ages between 21 and 54 years who underwent breast lift and reduction surgery at the Plastic Surgery and Burns Department of Hospital do Servidor Público Estadual de São Paulo were retrospectively analyzed. The superior medial pedicle and Liacyr Ribeiro type I pedicle techniques for mammary reduction were used. The distance between the vertical scar of the NAC and the inframammary crease was measured in the immediate postoperative period and after 6 months to compare the effect of each pedicle on vertical distance maintenance. Statistical analyses were performed by Student's t test, and P values < 0.005 were considered statistically significant. RESULTS: The pedicles used were superior (n = 16), medial (n = 20), and Liacyr Ribeiro type I (n = 4). The vertical scar (NAC-inframammary crease) measured between 5 cm and 6 cm (mean = 5.6 cm) during the immediate postoperative period, while later postoperative measurements ranged between 5.5 cm and 8 cm (mean = 6.6 cm). The differences in the NAC-inframammary crease distance between the Liacyr Ribeiro type I pedicle (P = 0.2048), superior pedicle (P = 0.0012), and medial pedicle (P = 0.0057) based surgeries only reached statistical significance in the superior pedicle surgery with respect to an increase in this measurement. CONCLUSIONS: Measurement of the vertical distance between the most caudal portion of the NAC and the inframammary crease scar is simple and might contribute to our understanding of the effect of different pedicle techniques on the long-term results of mammaplasty.

RESUMO

INTRODUÇÃO: A cirurgia plástica mamária tem como foco adequação do volume, suspensão e forma da mama. Diversas técnicas são utilizadas para redução e mastopexia, com grande atenção ao pedículo responsável pelo suprimento vascular do complexo areolopapilar (CAP). O objetivo deste trabalho é avaliar a técnica e a escolha do pedículo na manutenção da forma do polo inferior da mama no seguimento pós-operatório de 6 meses. MÉTODO: Foi realizada análise prospectiva de 20 pacientes do sexo feminino, totalizando 40 mamas, com índice de massa corporal (IMC) entre 22 e 25 e idade variando de 21 anos a 54 anos, submetidas a mamoplastia (redutora e mastopexia) no Serviço de Cirurgia Plástica e Queimaduras do Hospital do Servidor Público Estadual de São Paulo. Foram empregadas técnicas baseadas em pedículos superior e medial e pedículo tipo I de Liacyr Ribeiro. A distância entre a cicatriz vertical do CAP e o sulco inframamário foi mensurada no pós-operatório imediato e depois de 6 meses, a fim de comparar o efeito de cada pedículo na manutenção da distância vertical. A análise estatística foi feita pelo teste t de Student, considerando significativos valores de P < 0,005. RESULTADOS: Os pedículos utilizados foram superior (n = 16), medial (n = 20) e tipo I de Liacyr Ribeiro (n = 4). A distância da cicatriz vertical (CAP-sulco inframamário), no pós-operatório imediato, variou de 5 cm a 6 cm, com média de 5,6 cm. As medidas pós-operatórias tardias variaram entre 5,5 cm e 8 cm, com média de 6,6 cm. No que se refere às variações na medida da distância CAP-sulco inframamário quando relacionadas às técnicas de pedículo tipo I de Liacyr Ribeiro (P = 0,2048), de pedículo superior (P = 0,0012) e de pedículo medial (P = 0,0057), apenas o pedículo superior apresentou significância quanto ao alargamento dessa distância. CONCLUSÕES: A medida da distância vertical entre a porção mais caudal do CAP e a cicatriz horizontal posicionada no sulco inframamário (CAP-sulco inframamário) pode ser obtida de forma simples, a fim de entender a influência de cada pedículo no resultado tardio das mamoplastias.

Use of three rhomboid flaps for scalp defect coverage

ABSTRACT

Full-thickness scalp defects exposing the skull can be challenging for plastic surgeons. Scalp skin has low elasticity, so a large flap is necessary to cover even a small defect. This article presents 9 cases in which 3 rhomboid flaps were used for the closure of scalp defects. One patient experienced flap necrosis and required reoperation. Two other patients had minor complications treated with dressing. The method presented herein allows the harvest of 3 small flaps that collectively cover the defect as well as the primary closure of the donor area. This technique does not require the creation of a large flap or skin graft from the donor. Thus, the technique described herein is suitable for medium-thickness scalp defects and is a good alternative to large rotation flaps and skin grafts.

RESUMO

Defeitos no couro cabeludo podem ser um desafio para os cirurgiões plásticos quando afetam sua espessura total e deixam o crânio exposto. O couro cabeludo tem pouca elasticidade, assim um grande retalho é necessário para cobrir um defeito pequeno. O objetivo deste artigo é apresentar 9 casos em que 3 retalhos romboides foram utilizados para o fechamento de defeitos no couro cabeludo. Um paciente apresentou necrose do retalho e foi necessária reoperação. Dois outros pacientes tiveram complicações menores, que foram tratadas com curativos. O método apresentado permite a confecção de 3 pequenos retalhos que em conjunto cobrem o defeito, e as áreas doadoras são fechadas primariamente. Com a utilização da técnica descrita, a confecção de um retalho grande e a enxertia de pele da área doadora não são necessárias. Neste artigo é descrita uma técnica para fechamento de defeitos de tamanho moderado no couro cabeludo, que é uma boa alternativa a grandes retalhos de rotação ou enxerto de pele.
